Description
まず、負荷誘発試験により小麦依存性運動誘発アナフィラキシーと確定されている患者17名を被験者として、小麦のどのタンパク質が抗原であるのかを決定することとした。そこで、小麦粉から、α−グリアジン、β−グリアジン、γ−グリアジン、ω−5グリアジン、ω−1,2グリアジン、および、HMW−グルテニン、LMW(Low Molecular Weight)−グルテニンを、分子サイズ排除クロマトグラフィーおよびイオン交換クロマトグラフィーを用いて精製した。
次に、このHMW−グルテニンに反応するIgE抗体を保有する4名の患者の血清を再度準備し、IgE結合エピトープ配列を確定する作業を行った。初めに、既にデータベースに登録されているHMW−グルテニンのアミノ酸配列を基に、14長ペプチドを、アミノ酸の重複長を7長としてSPOTsメンブレン(シグマジェノシス社製)上に合成し、計16本のペプチドアレイを作製した。HMW−グルテニンの配列を配列表11に示した。また、図2に、14長×116本のペプチドアレイの対応図を示した。
次に、ω−5グリアジンに最も強く反応するIgE抗体を保有する患者4名(図1の患者1,2,3,および、5)の血清を用いてω−5グリアジンのIgE結合エピトープ配列を確定する作業を行うこととした。ω−5グリアジンの全長型のアミノ酸配列は報告されていなかったので、小麦EST(Expression Sequence Tag)データベースに報告されていたω−5グリアジン断片のアミノ酸配列から、全長型のアミノ酸配列を推測した。この推定ω−5グリアジンのアミノ酸配列を基に、13長ペプチドを、アミノ酸の重複長を6長としてSPOTsメンブレン(シグマジェノシス社製)上に合成し、計66本のペプチドアレイを作製した。推定ω−5グリアジンの配列を配列表12に示した。また、図6に、13長のペプチドアレイ66本を示した。
次に、負荷試験により確定診断された小麦依存性運動誘発アナフィラキシー患者28名の血清が、上記手法により決定された10種のエピトープ配列に対してどのように反応するかを調べた。図8に結果を示す。図示したように、すべての患者は、何れかのエピトープ配列に結合するIgE抗体を保有していた。

- 小麦依存性運動誘発アナフィラキシーのin vitro判定に際しての、配列番号1〜10に記載の10種のペプチドすべての使用。
- 小麦依存性運動誘発アナフィラキシーのin vitro判定に際しての、配列番号2,3,4および5に記載の4種のペプチドすべての使用。
- 配列番号1〜3に記載の3種のペプチドすべてを抗原として使用した小麦依存性運動誘発アナフィラキシーの診断キット。
- 配列番号1〜10に記載の10種のペプチドすべてを抗原として使用した小麦依存性運動誘発アナフィラキシーの診断キット。
- 配列番号2,3、4および5に記載の4種のペプチドすべてを抗原として使用した小麦依存性運動誘発アナフィラキシーの診断キット。
