発明分野
本発明は、遺伝性乳白色ゾウゲ質における突然変異遺伝子dsppの検出方法であり、この方法は、直接的配列決定、或いは制限酵素消化、或いはオリゴ核酸プローブとのハイブリダイゼーションを含む。更に詳しく言えば、本発明は、遺伝性乳白色ゾウゲ質を引き起こす突然変異遺伝子座をdsppの第三エクソン(exon)中に位置づける方法である。加えて、本発明は、本発明の方法により製造されたキット及び遺伝性乳白色ゾウゲ質の検出と患者の治療方法におけるその使用に関する。

技術背景
遺伝性乳白色ゾウゲ質は、常染色体優性遺伝病であり、発病率は1/8000で、1973年Shieldsにより、II型象牙質形成不全症(Dentinogenesis Imperfect TypeII)と分類された。患者としては、乳歯も永久歯も影響を受け、影響を受けた歯は、灰青色、或いは濃い褐色が現れ、それに乳白色光沢も伴うなっている。X線フィルムにおいて、歯冠は球形に現れ、歯根は小さくなったのが見られるし、髓室と歯根管は狭くなり、或いは完全閉鎖されている。歯エナメルの形成は、遺伝性乳白色ゾウゲ質によって直接影響は受けないが、歯質と歯エナメルの接触面は、貝状から平状に変わるので、歯エナメルは歯質から非常に脱落し易くなり、それが発育不全の歯質の摩耗を促進し、歯は著しく短くなって、最も短いのは、歯槽突起まで低くなる。過度の摩耗で、年齢が高い患者の場合は、歯槽骨の増殖と歯肉の増厚、及び繊維性変化が現れる。結果として、遺伝性乳白色ゾウゲ質患者は、止むを得ず多くの歯の看護と高額の治療を受けなければならない。

遺伝性乳白色ゾウゲ質の発病遺伝子は、既に、4q21における2MBに広がる領域中に位置付けられ、この範囲において、4個の遺伝子は組職の鉱物化過程に関係することが証明されている。これらは、それぞれ、分泌されたリン酸タンパク質(Secreted Phosphoprotein 1、SSP1)と、歯質マトリックスタンパク質1(dentin matrix protein 1、DMP1)と、歯質シアロリン酸タンパク質(dentin sialophospho protein、DSPP)と、骨シアロタンパク質(bone sialoprotein、IBSP)である。最近、dspp遺伝子の配列決定が(AF163151)されている。Dsppは、DSPとDPPの2個のタンパク質をコードしており、その中でも、DPPは、高度にリン酸化され、ヒドロキシアパタイト結晶体の形と大きさを調節し、ヒドロキシアパタイト結晶体の核形成過程においては、重要な役割を果たしていると考えられる。

本発明の別の目的は、遺伝性乳白色ゾウゲ質の治療におけるdspp突然変異遺伝子の適用を提供することである。上述のように、本発明のさらなる目的は、外側順方向プライマー、外側逆方向プライマー、内側順方向プライマー、内側逆方向プライマー、10×PCRバッファー、dNTP、TaqDNAポリメラーゼ、MgCl2、制限酵素RsaI、10×RsaIバッファー、及びBSAを含むキットを提供することである。診断は、本発明において発見されたナンセンス突然変異に隣接する、デザインされたプライマーでのPCRを実施し、PCR産物を制限酵素RsaIで消化することにより、迅速かつ簡便に行われる。このキットは、遺伝性乳白色ゾウゲ質の臨床での診断をより正確にするだけでなく、出生前診断にも用いられる。

実施例1
1.遺伝性乳白色ゾウゲ質の発病遺伝子の位置付け
1.1 対象
研究した遺伝性乳白色ゾウゲ質家系は、天津医科大学口腔病院において発見された(図1)。この家系には36個体あり、15個体が病に冒されていた。発端者は、V2、男性、5歳であり、乳歯はまだ完全には脱落していないが、髄室は閉鎖傾向にあり、前歯はさらに明らかであった。患者の臨床的症状は遺伝性乳白色ゾウゲ質の主な特徴と合致した。この家族の他の患者もこの疾病の主な特徴を有していた。発病遺伝子の輸送原則は、常染色体優性遺伝の特徴に合致した。我々は、この家族の13人(10人の患者を含む)のDNAを解析した。

1.2 連鎖解析
1.2.1 末梢血細胞からのDNAの単離
被験者の同意を得た後、家族構成員の13人の血管から5mlの血液を採取し、抗血液凝固剤を添加し、混合した。混合物を15分間遠心分離し(4℃、3000rpm)、ピペットで上清の血漿を除去した。5〜10倍量のddH2Oを、その結果物の溶液に添加した後、その混合物を室温で10分間温置し、15分間遠心分離(4℃、3500rpm)した。上清を真空ポンプで除去し、TES(Tris-HCl pH 8.0 15mmol/l, EDTA pH 8.0 15 mmol/l, 15mmol/l NaCl 4ml)を15mmol/lを添加し、沈殿物を再懸濁した。10%SDSを270μl添加し、混合した後、プロテインキナーゼK(10mg/ml)40μlを添加し、混合した。50℃で2時間温置した後、混合物を37℃で一晩温置した。氷上で0℃に冷却した後、1倍量のフェノールを添加し、混合して、15分間遠心分離(4℃、3500rpm)した。上清をフェノールで抽出した。上清をとり、1倍量のクロロホルム:イソアミルアルコール(24:1)を添加し、混合し、室温で5分間温置した後、5分間遠心分離(4℃、3500rpm)した。上清をクロロホルム:イソアミルアルコール(24:1)で再度抽出した。上清をとり、あらかじめ氷で冷却した2.5倍量の95%エタノールを添加した後、DNAが出現し、球状になるまで軽く振った。DNAを70%エタノールで洗浄し、1.5mlのエッペンドルフチューブに入れ乾燥した後、1.0mlのTEに溶解した。TEに完全に溶解した後、DNAの量をUV分光光度計で測定した。

1.2.2 STRPs蛍光標識PCRおよびPCR産物のスキャニング分析(1)STRPsプライマーの選択
8個の選択されたSTRPsを用いた連鎖解析を4q21について行った。それらは、D4S2915、D4S2932、GATA62A11、D4S2409、DSP STRP、SPP1、D4S1563およびD4S1544である。8個のSTRPsのプライマー配列は、表1に列挙した。PCR増幅は、上記8個のプライマーで、Perkin Elmer 9600 Thermal Cyclerで実施した。反応系は、12.5μlの;10×PCRバッファー 1.25μl、2mmol/l dNTP 0.5μl、1% FdNTP 0.5μl、2U/μl Taqポリメラーゼ(元原平晧生物技術有限会社から入手)0.25μl、5pmol/μl プライマー 0.25μl、40ng/μl ゲノムDNA 1.0μl、である。サイクルプログラムは以下のようにした:94℃での変性を50秒、アニーリングを1分(アニーリング温度は、表1に示した)、72℃での伸長を50秒、で35回、および最後の伸長は72℃で5分間行った。

STRPsの対立断片の分析
PAGEゲルでPCR産物を単離した後、同じ個体の異なった部位からのものを混合し、その後、混合物2μlをABI ROX−400分子マーカー0.4μl、ホルムアミド2.1μl、色素0.4μlと混合し、95℃2分で変性し、即座に氷上で冷却し、その後、電気泳動を、ABI377シーケンサー(Perkin Elmer社)上で、4%PAGEおよび36%尿素で、2時間実施した。データの収集、レーンの補正、内部分子量マーカーの補正、および断片サイズの測定は、GENESCAN2.1ソフトウエアで実施した。

1.2.3 連鎖解析
Lodスコアは、LINKAGEソフトウエアパッケージ中のMLINK連鎖解析ソフトウエアで、以下の条件で計算した:常染色体優性遺伝の2対立遺伝子系のモードにおいて、ヘテロ接合体における発現遺伝子頻度および発現遺伝子の表現率は、それぞれ0.00001および0.99に設定した;対立断片の頻度を同様に設定した、その後、組換え頻度をそれぞれ0、0.01、0.05、0.1、0.2、0.3および0.4としてLodスコアを計算した。結果を表2に示す。1を超すLodスコアに従って、遺伝性乳白色ゾウゲ質の発病遺伝子は、染色体4q上に位置づけられた。

実施例2 候補遺伝子の突然変異スクリーニング
その突然変異をスクリーニングするため、dsppを候補遺伝子として選択した。nested−PCR法の実施のため、Primer3ソフトウエアで、最初の4個のエクソンの8組のPCRプライマーを設計した。E1out、E2out、E34foutおよびE34loutは、PCR増幅の最初のサイクルのためのプライマーであり、E1in、E2in、E34finおよびE34linは、PCR増幅の第二のサイクルのためのプライマーである。最初の3組のプライマーでの、PCRの最初のサイクルの反応系は50μlである。

続いて68℃で7分間伸長する。PCR産物5μlを90Vで15分間電気泳動し、写真撮影した後、映像を保存した。最初のサイクルのPCR産物を250〜1250倍に希釈したものを第二のサイクルのPCRに用いた。反応条件は、94℃10秒、アニーリング30秒および72℃での伸長を25サイクルである(アニーリング温度および伸長時間は、表3に示した)。PCR産物を電気泳動し、写真撮影した後、映像を保存した。第二のサイクルのPCR産物を、シーケンシングプライマーとしてPCRプライマーを用いた直接配列決定に付した。得られた配列地図を解析した。ヘテロ接合性の2つのピークが現れた場合は、その部位がヘテロ接合状態であることを示唆し、更なる解析が必要となる。ヘテロ接合部位を含む配列をGenBankの配列と比較し、通常のリーディングフレームにしたがって翻訳した。その結果、ナンセンス突然変異が、患者の第三のエクソンに存在した、ここで、dsppの第三のエクソンのヌクレオチド3658位でのC−T転移が起こり、この突然変異が、通常の、Gln45をコードする、コードCAGから終止コードTAGへの変化を作り出しており、そのため、dsppの翻訳が事前に止まり、発現産物の一つ、dspp遺伝子のDPPが産性され得なくなる(図2)。

実施例3 制限酵素消化による同定
3.1 PCR増幅
最初のサイクルのPCRを、E34foutプライマーで実施し、電気泳動の後、写真撮影し、画像を保存した。その後、最初のサイクルのPCR産物を250倍に希釈したものをE34loutプライマーでの第二のサイクルのPCRに用いた。反応系は100μlであり、反応条件は、突然変異スクリーニングのものと同じである。第二のサイクルのPCR産物を電気泳動した後、写真撮影し、画像を保存した。

3.2 PCR産物の回収(PCR回収キットは、Life Technology社のConcert Papid PCR Purification Systemである)
洗浄バッファー4mlに95%エタノール10mlを添加し、完全に混合した。
