【発明の属する技術分野】
本発明は医療費補助システムに関し、特に医療機関が自治体および健康保険組合に対して医療費補助を申請する医療費補助システムに関する。
【０００２】
【従来の技術】
現在、乳幼児、老人が支払う医療費は、健康保険による補助とともに、医療費に上限を設けて、不足分を各自治体が補助する制度が採られている。例えば、ある自治体では、受給者は、受診してかかった医療費のうち、健康保険補助分が控除され、さらに、その金額の自治体補助分を控除した額を医療機関に支払う制度を採っている。

各自治体は、対象となる受給者に対して、医療費補助を受けることができる受給者証を発行する。対象年齢や補助の負担割合などは各自治体によって異なる。乳幼児、老人の医療費補助を受けるには、受給者が、各自治体に行って医療費補助申請用紙をもらう必要がある。受給者は医療機関で受診した後、受給者が、受給者証を見ながら、各自治体が発行する医療費補助申請用紙に、市町村番号、受信者氏名、性別・生年月日、受給者番号、保険者番号などの必要事項を記入して提出する。

一方、医療機関は、受給者から提出された医療費補助申請用紙の記載内容について、受給者証を見ながら確認し、この医療費補助申請用紙に、医療機関名、診療年月日、点数および一時負担金などの請求内容を記入し、自治体に提出する。

【発明が解決しようとする課題】
しかしながら、受給者証を有する者が自治体の医療費補助を受けようとするためには、受給者は、各自治体に置いてある医療費補助申請用紙を取りに行かなければならず、時間的、地理的な制約があった。

また、受給者は、この医療費補助申請用紙に受給者証を見ながら必要事項を記入しなければならず、医療費補助申請用紙を受け取った医療機関も、受診者の受給者証を見ながら申請内容を確認し、診察後、受診者の診察にかかった医療費、診療した日時、受診者に出した薬に関する情報など、必要事項を記入しなければならない。そのため、記入する煩わしさがあり、さらに、受給者が記入する必要事項や、医療機関が記入する請求金額などの申請内容が誤って記入された場合には、医療費が正常に支払われないなどの問題が発生する可能性もある。

さらに、医療費補助を受けようとする受給者は、受診した医療機関の所在地と同じ自治体に所属するとは限らず、医療機関は、自治体ごとに医療費補助申請用紙を提出しなければならないので、手間がかかるといった問題点があった。

自治体が発行する医療費補助申請用紙がないまま医療機関を受診してしまった場合には、医療費補助を受けられなくなる場合もある。
また、医療費補助申請用紙を受け取る自治体では、各受給者の受給者証情報や医療機関、請求金額などの確認作業を行い、手続きしなければならない。そのため、医療費補助の手続きには多くの手間がかかるといった問題点があった。

本発明はこのような点に鑑みてなされたものであり、医療費補助を受ける際の手続きを自動化し、補助金の申請、支払いを効率的に行う医療費補助システムを提供することを目的とする。

【課題を解決するための手段】
本発明では上記問題を解決するために、医療費補助の申請手続きを行う医療費補助システムにおいて、受給者証が有する受給者証情報を読み取る受給者証情報読み取り手段と、医療機関名を識別する情報である医療機関情報と受診者の診察内容である診察内容情報とが入力される申請内容入力手段と、前記受給者証情報読み取り手段によって読み取られた前記受給者証情報と前記申請内容入力手段によって入力された前記医療機関情報と前記診察内容情報とを有する申請手続き情報を送信先に応じた形態で生成する申請手続き情報生成手段と、前記申請手続き情報生成手段によって生成された前記申請手続き情報を送信する申請手続き情報送信手段と、を有する受給者証読み取り装置と、前記申請手続き情報送信手段によって送信された前記申請手続き情報を受信する申請手続き情報受信手段と、登録された受給者の情報である登録受給者情報が格納された登録受給者情報格納手段と、前記申請手続き情報受信手段によって受信された前記申請手続き情報を用い、前記医療機関情報から補助金の支払いを行う医療機関の情報である補助金支払い先情報を生成し、前記診察内容情報から支払いを行う前記補助金の額の情報である補助金額情報を生成し、登録受給者情報を申請内容の重複有無の確認に用いることができるよう、前記受給者証情報と前記医療機関情報と前記診察内容情報とを新たな登録受給者情報として前記登録受給者情報格納手段に格納する申請手続き処理手段と、前記申請手続き処理手段によって生成された前記補助金支払い先情報と前記補助金額情報とを金融機関が有する端末装置に送信する補助金支払い処理情報送信手段と、を有する医療費補助処理装置と、を有し、前記医療費補助処理装置は、前記申請手続き情報受信手段によって受信された前記申請手続き情報が有する前記受給者証情報と、前記登録受給者情報格納手段に格納された前記登録受給者情報と、を照合する受給者照合手段をさらに有し、前記申請手続き処理手段は、前記受給者照合手段による前記照合が成立した場合に、前記補助金支払い先情報と前記補助金額情報との生成を行い、前記補助金支払い処理情報送信手段は、前記受給者照合手段による前記照合が成立した場合に、前記補助金支払い先情報と前記補助金額情報とを金融機関が有する端末装置に送信することを特徴とする医療費補助システムが提供される。

このような医療費補助システムによれば、医療機関において、受給者は、受給者証読み取り装置の受給者証情報読み取り手段により自治体の発行する受給者証情報、例えば市町村番号、受信者氏名、性別・生年月日、受給者番号、保険者番号などを読み取らせるので、医療費補助申請用紙に受給者証に記載されている内容を転記しなくてもよい。

また、申請内容入力手段にて入力された医療機関を識別する医療機関情報と、受診者の診察にかかった医療費、診療した日時、受診者に出した薬に関する情報などの診察内容情報と、受給者証情報読み取り手段により読み取られた受給者証情報とから、申請手続き情報生成手段により申請手続き情報を生成して、例えば自治体や健康保険組合が有する医療費補助処理装置に送信するので、医療機関が受給者証を確認しながら、医療費補助申請用紙を作成するという手間がなくなる。

送信された申請手続き情報は、例えば、自治体や健康保険組合が有する医療費補助処理装置の申請手続き情報受信手段によって受信されるので、医療機関が、受給者証情報、受診者の診察にかかった診察内容情報、例えば医療費、診療した日時、受診者に出した薬に関する情報などが記入された医療補助申請用紙を提出する必要がない。

受信された申請手続き情報から、申請手続き処理手段により、補助金の支払いを行う医療機関の情報である補助金支払い先情報と、支払いを行う補助金の額の情報である補助金額情報とが生成され、補助金支払い処理情報送信手段が、補助金支払い先情報と補助金額情報とを金融機関が有する端末装置に送信する。

これにより、受給者は、自治体の発行する受給者証情報を、医療機関の受給者証読み取り装置で読み取らせるだけで、医療費の補助を受けられるとともに、医療機関の補助金を受けるための手続きが自動化される。
また、申請手続き情報に含まれる受給者証情報、医療機関情報および診察内容情報は、新たな登録受給者情報として登録受給者情報格納手段に格納され、医療費補助処理装置において、医療機関のはしごや多受診を防ぐ目的で、申請内容の重複有無の確認に用いることができる。それにより、保険行政の適正化が図れる。

【発明の実施の形態】
以下、本発明の実施の形態を図面を参照して詳細に説明する。
本発明の実施の形態に係る医療費補助システムは、医療機関に受給者証読み取り装置を設置するとともに、自治体、健康保険組合に医療費補助処理装置を設置し、受給者証情報および医療費補助に関する申請内容を含む診察内容情報を、インターネットを介して伝達し、医療機関が、自治体、健康保険組合に対して、受診にかかった医療費のうちの自治体補助分である公費負担金額、健康保険補助金額を請求するシステムである。

図１は医療費補助システムの原理構成を示す図である。
医療費補助システム１は、医療機関１０に設置した受給者証読み取り装置５０と、自治体２０に設置した医療費補助処理装置７０と、健康保険組合３０に設置した医療費補助処理装置８０とがインターネットを介して接続されて構成される。

医療機関１０に設置された受給者証読み取り装置５０は、受給者証情報を読み取る受給者証情報読み取り手段１１と、医療機関名を識別する情報である医療機関情報と受診者の診察内容である診察内容情報とが入力される申請内容入力手段１２と、受給者証情報読み取り手段１１によって読み取られた受給者証情報と、申請内容入力手段１２によって入力された医療機関情報と診察内容情報とを有する申請手続き情報を生成する申請手続き情報生成手段１３と、申請手続き情報生成手段１３によって生成された申請手続き情報を送信する申請手続き情報送信手段１４と、受給者証読み取り手段１１によって読み取られた受給者証情報の内容を医療費補助申請用紙に印刷する受給者証情報印刷手段１５とから構成されている。

自治体２０に設置された医療費補助処理装置７０は、医療機関が有する申請手続き情報送信手段１４によって送信された申請手続き情報を受信する申請手続き情報受信手段２１と、登録された受給者の情報である登録受給者情報が格納された登録受給者情報格納手段２２と、申請手続き情報受信手段２１によって受信した申請手続き情報が有する受給者証情報と、登録受給者情報格納手段２２に格納された登録受給者情報とを照合する受給者照合手段２３と、申請手続き情報受信手段２１によって受信した申請手続き情報を用い、医療機関情報から補助金の支払いを行う医療機関の情報である補助金支払い先情報を生成し、診察内容情報から支払いを行う補助金の額の情報である補助金額情報を生成し、受給者証情報と医療機関情報と診察内容情報とを新たな登録受給者情報として登録受給者情報格納手段２２に格納する申請手続き処理手段２４と、申請手続き処理手段２４によって生成された補助金支払い先情報と補助金額情報とを金融機関４０が有する端末装置に送信する補助金支払い処理情報送信手段２５とから構成される。

健康保険組合３０に設置された医療費補助処理装置８０は、医療機関が有する申請手続き情報送信手段１４によって送信された申請手続き情報を受信する申請手続き情報受信手段３１と、登録された受給者の情報である登録受給者情報が格納された登録受給者情報格納手段３２と、申請手続き情報受信手段３１によって受信した申請手続き情報が有する受給者証情報と、登録受給者情報格納手段３２に格納された登録受給者情報とを照合する受給者照合手段３３と、申請手続き情報受信手段３１によって受信した申請手続き情報を用い、医療機関情報から補助金の支払いを行う医療機関の情報である補助金支払い先情報を生成し、診察内容情報から支払いを行う補助金の額の情報である補助金額情報を生成し、受給者証情報と医療機関情報と診察内容情報とを新たな登録受給者情報として登録受給者情報格納手段３２に格納する申請手続き処理手段３４と、申請手続き処理手段３４によって生成された補助金額情報と補助金支払い先情報とを金融機関４０が有する端末装置に送信する補助金支払い処理情報送信手段３５とから構成される。

医療機関に設置された受給者証読み取り装置５０が有する申請手続き情報送信手段１４は、例えばインターネットを介して自治体に設置された医療費補助処理装置７０が有する申請手続き情報受信手段２１、および健康保険組合に設置された医療費補助処理装置８０が有する申請手続き情報受信手段３１と接続されている。

上記の構成の医療費補助システム１において、受給者が医療費補助を受けようとする場合、医療機関１０にて、自治体２０から受給者に発行された受給者証に記載されている、受給者に関する情報である受給者証情報が読み取られる。受給者証情報読み取り手段１１はＯＣＲ（Optical Character Reader）認識機能を有しており、受給者証に記載されている文字情報を判読し、データ変換して読み取ることができる。さらに、医療機関１０では受診者の診察後、申請内容入力手段１２により医療機関１０を識別する情報である医療機関情報と、受診者の診察にかかった医療費、診療した日時、受診者に出した薬に関する情報などの診察内容である診察内容情報を入力する。申請手続き情報生成手段１３は、医療機関１０が入力した医療機関情報と診察内容情報と、受給者証情報読み取り手段１１にて読み取った受給者証情報とを、各送信先に必要な情報を選択し、インターネットを介して送受信可能なデータ形式に変換して申請手続き情報を生成する機能を有する。変換された受給者証情報と医療機関情報と診察内容情報とを含む申請手続き情報は、申請手続き情報送信手段１４により、インターネットを介して自治体２０に設置した医療費補助処理装置７０および健康保険組合３０に設置した医療費補助処理装置８０に送信される。

自治体２０は、医療機関１０に設置された受給者証読み取り装置５０から送信された、自治体の公費負担を受けるのに必要な受給者証情報と医療機関情報と診察内容情報とを含む申請手続き情報を、医療費補助処理装置７０が有する申請手続き情報受信手段２１により受信すると、まず、受信した申請手続き情報が有する受給者証情報について、受給者照合手段２３により、登録受給者情報格納手段２２に格納されている受給者証情報との照合を行う。受給者証情報との照合が成立すると、申請手続き処理手段２４により、受信した申請手続き情報を用いて、医療機関情報から補助金の支払いを行う医療機関の情報である補助金支払い先情報とを生成し、診察内容情報から支払いを行う補助金の額の情報である補助金額情報を生成し、さらに、受給者証情報と医療機関情報と診察内容情報とを新たな登録受給者情報として登録受給者情報格納手段２２に格納する。補助金支払い処理情報送信手段２５は、補助金額情報と補助金支払い先情報とから、公費負担の補助金額を補助金支払い先である医療機関へ支払うよう金融機関４０が有する端末装置へ支払い指令を送信する。補助金の支払いに関する処理は、申請手続き情報を受信した自治体２０が、通常行われている振込み方法によって行ってもよい。

健康保険組合３０は、医療機関１０に設置された受給者証読み取り装置５０から送信された、健康保険組合３０の医療補助を受けるのに必要な受給者証情報と医療機関情報と診察内容情報とを含む申請手続き情報を、医療費補助処理装置８０が有する申請手続き情報受信手段３１により受信すると、まず、受信した申請手続き情報が有する受給者証情報について、受給者照合手段３３により、登録受給者情報格納手段３２に格納されている受給者証情報との照合を行う。受給者証情報との照合が成立すると、申請手続き処理手段３４により、受信した申請手続き情報を用いて、医療機関情報から補助金の支払いを行う医療機関の情報である補助金支払い先情報とを生成し、診察内容情報から支払いを行う補助金の額の情報である補助金額情報を生成し、さらに、受給者証情報と医療機関情報と診察内容情報とを新たな登録受給者情報として登録受給者情報格納手段３２に格納する。補助金支払い処理情報送信手段３５は、補助金額情報と補助金支払い先情報とから、公費負担の補助金額を補助金支払い先である医療機関１０へ支払うよう金融機関４０が有する端末装置へ支払い指令を送信する。この補助金の支払いに関する処理は、申請手続き情報を受信した健康保険組合３０が、通常行われている振込み方法によって行ってもよい。

図２は医療費補助システムの処理の流れを示すフローチャートである。
受給者が医療費補助を受けようとする場合、まず、医療機関に設置された受給者証読み取り装置５０により、受給者証に記載されている受給者証情報を読み取らせる（ステップＳ１）。次いで、受診者の診察後、医療機関を識別する情報である医療機関情報と、受診者の診察にかかった医療費、診療した日時、受診者に出した薬に関する情報などの診察内容である診察内容情報を医療機関が入力する（ステップＳ２）。医療機関情報と診察内容情報との入力が終了しているか否かが判断され（ステップＳ３）、必要事項の入力が完了していれば、受給者証情報と医療機関情報と診察内容情報とを、送受信可能なデータ形式に変換して申請手続き情報を作成する（ステップＳ４）。ステップＳ３において、医療機関情報と診察内容情報との入力が完了していない場合には、そのまま医療機関情報および診察内容情報の入力完了待機状態となる。ステップＳ４において作成された申請手続き情報は自治体に設置された医療費補助処理装置７０、および健康保険組合に設置された医療費補助処理装置８０に送信される（ステップＳ５）。自治体および健康保険組合が有する医療費補助処理装置７０、８０が申請手続き情報を受信する（ステップＳ６）と、受信した受給者証情報と、医療費補助処理装置７０、８０が有する、登録された受給者の情報である登録受給者情報との照合を行う（ステップＳ７）。ステップＳ７において登録受給者情報との照合が成立すると、補助金額情報と補助金支払い先情報とを作成する（ステップＳ８）。最後に、作成された補助金額情報と補助金支払い先情報とを金融機関に送信する（ステップＳ９）。なお、ステップＳ７において、格納されている登録受給者情報との照合が成立しない場合には、送信された申請手続き情報を送信元に返信し（ステップＳ１０）、医療機関が送信した申請手続き情報を受理しなかったことを通知し、その後の手続きは行わない。

以上説明した医療費補助システムについて、医療機関、自治体、健康保険組合の各々の申請手続き処理について以下に説明する。
まず、医療機関での医療費補助の申請手続きについて説明する。

図３は医療機関に設置される受給者証読み取り装置の概略を説明する模式図である。
医療機関に設置される受給者証読み取り装置５０は、主に、受給者証に記載されている市町村番号、受給者番号、保険者番号、受診者、住所の受給者証情報を読み取る受給者証読み取り機構部５１と、受給者証読み取り機構部５１で読み取られた受給者証情報について処理を行う端末装置５２とから構成されている。

受給者証読み取り機構部５１は、主に、受給者証を挿入し、かつ読み取り後に受給者証を返却する受給者証挿入口５１ａと、挿入された受給者証に記載されている受給者証情報を読み取るイメージセンサ５１ｂと、医療費補助申請用紙が収納されている医療費補助申請用紙スタッカ５１ｃと、医療費補助申請用紙に受給者証情報と診察内容情報とを有する申請手続き情報を印字する印字リボン５１ｄおよび印字ヘッド５１ｅと、印字した医療費補助申請用紙が受給者に排出されて渡される医療費補助申請用紙排出口５１ｆと、イメージセンサ５１ｂと印字リボン５１ｄと印字ヘッド５１ｅとを制御するコントローラ５１ｇとから構成されている。

図４は受給者証読み取り装置が有する端末装置の機能を説明するブロック図である。
端末装置５２は、医療機関が補助金を自治体や健康保険組合に申請するために、医療機関を識別する情報である医療機関情報と、受診者の診察にかかった医療費、診療した日時、受診者に出した薬に関する情報などの診察内容である診察内容情報とを入力する申請内容入力部５２ａと、受給者証読み取り機構部５１で読み取られた受給者証情報と、申請内容入力部５２ａにて入力された医療機関情報と診察内容情報とを格納する申請手続き情報格納部５２ｂと、申請手続き情報格納部５２ｂに格納されている受給者証情報と医療機関情報と診察内容情報とから申請手続き情報を生成する申請手続き情報生成部５２ｃと、申請手続き情報生成部５２ｃにて生成された申請手続き情報を送信する申請手続き情報送信部５２ｄとから構成されている。

さらに、端末装置５２には、申請手続き情報生成部５２ｃにて生成された申請手続き情報を表示するモニターなどの外部表示機能を有する装置からなる申請手続き情報表示部５３が接続されている。

上記構成の受給者証読み取り装置５０により医療費補助の手続きを行う場合、まず、受給者は受給者証挿入口５１ａから受給者証を挿入する。受給者証読み取り装置５０は、受給者証の挿入を検知し、イメージセンサ５１ｂで受給者証に記載されている受給者証情報を読み取る。この受給者証には市町村番号、受給者番号、通院または入院の対象、保険給付割合、保険者番号、受給者の住所・氏名、受診する幼児の住所・氏名、受給期間、発行機関名、および交付年月日などの受給者証情報が記載されている。これらの記載内容をイメージセンサ５１ｂにて、ＯＣＲ認識して受給者証に記載されている受給者証情報を判読し、受給者証情報の読み取り後、受給者証を受給者証挿入口５１ｂに排出し、受給者に返却する。

受給者証から読み取った受給者証情報は、一旦、端末装置５２の申請手続き情報格納部５２ｂに格納される。医療機関は、診察後、診察内容が確定した段階で、医療機関を識別するための医療機関情報、診療年月日、受診者の診察にかかった医療費、薬などの診察内容である診察内容情報を端末装置５２の申請内容入力部５２ａから入力する。これら受給者証情報と医療機関情報と診察内容情報とを用いて、申請手続き情報生成部５２ｃが送信先に必要な情報を選択して所定のフォーマット、データ形式の申請手続き情報を生成し、端末装置５２の申請手続き情報送信部５２ｄからインターネットを介して自治体および健康保険組合が有する医療費補助処理装置７０，８０へと送信する。また、この申請手続き情報は、端末装置５２に接続されたモニターなどの外部表示機能を有する装置からなる申請手続き情報表示部５３に表示される。

図５は申請手続き情報の表示例である。
受給者証読み取り装置５０で読み取られた、受給者証に記載されている市町村番号、受給者番号、保険者番号、受診者、住所の受給者証情報と、医療機関が診察後に入力した診察年月日および点数と、医療費の総計、健康保険補助後の受給者負担、受給者の医療機関への支払い、自治体の公費負担の各金額とがモニターなどの表示装置の画面上に表示される。例えば、図５に示す表示例では、医療費の総計が８８００円の場合、健康保険による補助で、受診者の医療費負担が医療費の総計の３割負担であれば、健康保険補助後の受給者負担は２６４０円になる。さらに、自治体による補助が、この受給者負担の２割負担であれば、受給者の医療機関への支払い額は５３０円であり、その差額２１１０円が自治体の公費負担となる。

上記のように、受給者は、医療費の補助を受ける際、自治体が発行した受給者証を持って、受診する医療機関で、受給者証読み取り装置５０に受給者証を通すだけで医療費の補助を受けることができる。

また、医療機関も、従来のように、受給者が受給者証に記載されている受給者証情報を記入した医療費補助申請用紙の内容を、受給者証を見ながら確認しなくてもよい。

さらに、受給者証情報と医療機関情報と診察内容情報とを有する申請手続き情報がインターネットを介して自治体が有する医療費補助処理装置に伝達されるので、医療機関は、必要事項が記入された医療費補助申請用紙を自治体や健康保険組合に提出するという手間を省くことができる。

上記の説明では、申請手続き情報を、自治体および健康保険組合が有する医療費補助処理装置にインターネットを介して送信することとしたが、受給者証読み取り装置で受給者証情報を医療費補助申請用紙に印字し、この医療費補助申請用紙を受給者が医療機関に提出するように選択できるようにしてもよい。

この場合、受給者証読み取り装置５０において受給者証情報がイメージセンサ５１ｂによりＯＣＲ認識されると、医療費補助申請用紙スタッカ５１ｃにあらかじめ収納されている医療費補助申請用紙に、印字リボン５１ｄと印字ヘッド５１ｅとにより印字され、受給者が記入すべき受給者証情報が印字された医療費補助申請用紙が医療費補助申請用紙排出口５１ｆから排出される。挿入された受給者証は、イメージセンサ５１ｂでの受給者情報の読み取り後、受給者証挿入口５１ａから排出される。

受給者は、従来と同様の手順で、受給者証情報の印字された医療費補助申請用紙を医療機関に提出し、医療機関が医療費補助申請用紙の記載内容を確認して、受診後、申請内容に関する診察内容情報を記入し、受給者証情報と診察内容情報とを有する申請手続き情報の記載された医療費補助申請用紙を自治体に提出する。これにより、受給者が、医療機関での受診前に、医療費補助申請用紙を自治体へ取りに行く手間を省くことができる。また、必要事項を医療費補助申請用紙に記入する手間を省くことができる。

次に、自治体における医療費補助の申請手続きについて説明する。
