Description
本発明は痛みを軽減する医薬組成物に関する。特に、本発明は治療有効成分として(a)トラマドール及び(b)デキストロメトルファン又はその製薬上許容し得る塩からなる咳の症状に伴う痛み以外の痛み、例えば関節炎の痛み、仙腰痛、筋骨格の痛み、咽喉痛に伴う痛みの治療用医薬組成物に関する。
背景技術
一般にトラマドールとして知られている化合物であるシス−2−[(ジメチルアミノ)メチル]−1−(3−メトキシフェニル)シクロヘキサノールは、塩酸塩として市販されている鎮痛薬である。トラマドールが製造される方法は、米国特許第3652589号に記載されており、その内容は、本明細書に参考として引用される。トラマドールは、オピエート、例えばモルヒネ、コデイン、ヒドロコドン及びオキシコドンに化学的に関係していない。しかし、トラマドールの投与に伴う多数の副作用、例えばめまい感、傾眠、嘔吐、便秘、発汗及びかゆみは、オピオイドのそれに似ている。トラマドールは、オピオイドより呼吸の抑制が顕著に低いため、トラマドールの鎮痛活性の増大又は増強は、鎮痛作用の低下なしに、標準の投与量レベルのそれより少なく医薬の投与量レベルを低下させるだろう。別に、トラマドールの増強は、標準の投与量レベルを維持させるが、鎮痛作用を増大させるだろう。何れにしても、トラマドール誘発鎮痛の安全性は、その鎮痛の有効性の増強によりかなり改善されるだろう。
デキシトロメトルファンは、レボルファノールのコデイン同族体のd−異性体である。1−異性体とは異なり、デキシトロメトルファンは、鎮痛性又は依存性を有しないといわれる(Goodman and Gilman´s「The Pharmacological Basis of Therapeutics」8版、McGraw−Hill、Inc.(1990)518ページ)。
米国特許第5336691号は、咳止め剤例えばデキシトロメトルファン、カラミノフェン及びそれらの許容できる塩と任意に組み合わされた、トラマドール及びアセトアミノフェン単独を含む鎮痛組成物を記述している。
従来、トラマドールの鎮痛の有効性が、鎮痛増強量のデキシトロメトルファン又はそれに関する限り任意の他のNMDA受容体拮抗剤の投与の前、その間又はその後にトラマドールを投与することによりかなり増加できるという認識又は理解は存在しなかった。
発明の開示
本発明によれば、治療有効成分として(a)トラマドール及び(b)デキストロメトルファン又はその製薬上許容し得る塩からなる咳の症状に伴う痛み以外の痛みの治療用医薬組成物が提供される。
本発明の方法は、咳に伴う痛みという唯一の例外ですべての種類の痛みの治療に適用できる。従って、痛みの緩和を要するが咳の症状を示さない哺乳動物に、トラマドールの同等量に対する増強したレベルの鎮痛、又はトラマドールの低下した量に対する同等なレベルの鎮痛は、トラマドールが鎮痛増強剤の投与の前、その間又はその後に投与されるとき、達成できる。
用語「トラマドール」は、トラマドールの遊離塩基、並びにその製薬上許容できる酸付加塩を含むと理解すべきである。
用語「N−メチル−D−アスパルテート受容体」は、NMDA受容体に伴う結合部位の下部カテゴリー、例えばグリシン結合部位、フェニルシクリジン(PCP)結合部位など、並びにNMDAチャンネルのすべてを含むと理解すべきである。従って、本発明は、NMDA受容体結合部位例えばデキストロファン、又はNMDAチャンネル例えばマグネシウムの源例えば硫酸マグネシウムをブロックする非毒性物質の使用を含む。
本明細書で使用されるとき、用語「非毒性」は、相対的な意味で理解すべきであり、ヒトへの投与のために米国のFood and Drug Administration(「FDA」)により承認されている、又は確立された規制基準及びプラクティスと一致して、ヒトへの投与のためにFDAにより承認されうる任意の物質を指名することを目的とする。用語「非毒性」は、また、それらの毒性がそれらの治療上の使用を実際上排除するNMDA受容体拮抗剤、例えばMK801(化合物、5−メチル−10、11−ジヒドロ−SH−ジベンゼ[a、d]シクロヘプテン−5、10−イミン)、CPP(化合物、3−[2−カルボキシピペラジン−4−イル]プロピル−1−ホスホン酸)及びPCP(化合物、1−(1−フェニルシクロヘキシル)ピペリジン)から、本発明の実施に有用であるNMDA受容体拮抗剤又はブロッカーを区別するのに使用される。
用語「痛みの軽減」は、本発明が、現存する痛みの軽減、並びに切迫した痛みを生じさせる事象からさもなければ結果として起こるであろう痛みの抑制又は阻止に適用可能であることから、用語「痛みの抑制」及び「痛みの阻止」を含むことを本明細書では理解すべきである。
用語「鎮痛増強」は、関節炎にかかったラットにおける機械的な痛覚過敏の測定(Randall−Sellitoテスト、下記)により決定される、トラマドール単独の鎮痛有効性そして鎮痛増強剤単独の鎮痛有効性に基づいて予想されるであろうものを越えてトラマドールの鎮痛有効性を増大、増強又は相乗するための鎮痛増強剤の能力を意味することと本明細書では理解すべきである。トラマドールの鎮痛有効性のこの予想されない増大は、それゆえ、本発明による鎮痛増強剤の投与により生ずる相乗作用の一つであると考えることができる。

発明を実施するための最良の形態
本発明によりNMDA受容体をブロックしそしてそれ自体トラマドールの鎮痛活性を増強するのに有用である非毒性の物質の中には、デキストロメトロファン((+)−3−ヒドロキシ−N−メチルモルフィナン)及びその代謝生成物であるデキストロファン((+)−3−ヒドロキシ−N−メチルモルヒナン)、それらの混合物及びそれらの製薬上許容できる塩である。NMDA受容体をブロックする他の有用な非毒性物質は、アマンタジン(1−アミノアダマンチン)、メマンチン(3、5−ジメチルアミノアダマントン)、ピロロキノリンキノン及びシス−4−(ホスホノメチル)−2−ピペリジンカルボン酸を含む。NMDA受容体拮抗剤の中で、その臭化水素酸塩の形のデキストロメトロファンが、その高度の証明された安全性及びその容易な入手性のために本発明で使用するのに好ましい。デキストロロファン及びその製薬上許容できる塩は、また優れた結果をもたらすだろうが、現時点で工業的に製造されていることは知られていない。
NMDA受容体に関するブロッカーに加えて又はその代わりに、NMDA受容体活性化の主な細胞内の結果をブロックする少なくとも1種の非毒性物質も、本発明で鎮痛増強剤として使用できる。興奮性アミノ酸受容体のサブタイプであるNMDA受容体の活性化は、神経細胞の官能性活性における多数の変化、特に細胞内Ca++濃度の増加による依存性物質の存在下の興奮性又は阻害に関するそれらの能力を誘発する。NMDA受容体活性化の主な結果は、神経細胞内に生ずる事象の以下の連鎖又はカスケードを含む。
a)プロティンキナーゼ例えばプロティンキナーゼCの転移及び活性化→基体蛋白例えば細胞質ゾル酵素、チャンネル蛋白質、受容体蛋白質の燐酸化→官能性活性の変化;
b)増加した細胞内Ca++又はCa++活性化プロティンキナーゼの何れかによる初期遺伝子(c−fos、c−jun、zif−268など)の発現の開始→細胞酵素(例えばプロティンキナーゼ)、受容体蛋白質(例えばNMDA受容体)、イオンチャンネル蛋白質(例えばK+、Na+、Ca++チャンネル)、神経ペプチド(例えばダイノルフィン)などの生成に関係のある官能性遺伝子の発現→官能性活性の変化;
c)酵素及び他の細胞成分のCa++/カルモジュリン(又は他のCa++結合蛋白質)誘発活性化→Ca++/カルモジュリン−プロティンキナーゼ系例えばCa++/カルモジュリンキナーゼIIの活性化→酵素(例えばCa++/カルモジュリンキナーゼII)又は他の官能性蛋白質の自動燐酸化→官能性活性の変化;
d)構成性酸化窒素シンターゼのCa++/カルモジュリン誘発活性化並びに誘発可能な酸化窒素シンターゼの誘発→酸化窒素の生成→i)プロティンキナーゼの活性及び初期遺伝子発現を生ずるグアノシンシクラーゼの活性化による環状グアノシンモノホスフェートの生成;ii)直接蛋白修飾例えば酵素、受容体及び/又はチャンネル蛋白質;iii)遊離基のスキャベンジによる脂質膜修飾及び/又は核酸修飾;iv)高い酸化窒素レベルでの神経毒の誘発;v)隣接ニューロン又は膠細胞の戻り作用例えばグルタメート放出の促進/NMDA受容体活性化及び/又は後シナプスNMDA受容体の阻害→官能性活性の変化;
e)環状アデノシンモノホスフェート/プロティンキナーゼA系、ホスホリパーゼC−イノシトールトリホスフェート−Ca++/ジアシルグリセロール−プロティンキナーゼ系、ホスホリパーゼA2−アラキドン酸/プロスタノイド/ロイコトリエン系との相互作用→NMDA受容体/Ca++/Ca++−カルモジュリン/プロティンキナーゼ系以外の第二のメッセンジャー系により誘発される官能性活性の変化;並びに
f)非NMDA受容体及び代謝向性(metabotropic)受容体を含む他の興奮性アミノ酸受容体サブタイプとの相互作用、並びにこららのアミノ酸受容体サブタイプの活性化に伴う細胞内事象→NMDA及び代謝向性受容体の活性化。
NMDA受容体をブロックする物質は、事象の前記の主な細胞内連鎖のすべてが起こることを有効似防ぐだろう。しかし、NMDA受容体の活性化でも、トラマドール、並びに事象の前記の主な細胞内連鎖の少なくとも一つをブロックする物質を投与することにより、本発明に従って痛みを治療することも可能である。そのため、例えば、プロティンキナーゼCの転移及び活性化、又は構成性酸化窒素シンターゼのカルモジュリン誘発活性化並びに誘発可能な酸化窒素シンターゼの誘発と干渉する物質も、本発明の実施に有用である。
NMDA受容体活性化の主な細胞内連鎖をブロックしそれゆえ本発明の実施に有用である非毒性物質は、プロティンキナーゼCの阻害剤、例えばガングリオシド例えばガングリオシドGM1(モノシアロガングリオシド)及びガングリオシドGT1b(トリシアロガングリオシド);両親媒性長鎖塩基例えばスフィンゴシン、N、N、N−トリメチルスフィンゴシン、スフィンガニン及びサイコシン;キノリルオキサゾール−2−オン例えば4−メチル−5−(3−キノリニル)−2−(3H)−オキサゾロン及びフェニル−5−(2−キノリニル)−2、3(3H)−オキサゾロン;1、4−ビス−(アミノヒドロキシアルキルアミノ)−アンスラキノン例えば1、4−ビス−(3−プロピルアミノ−2−ヒドロキシプロピルアミノ)−9、10アンスラセンジオン及び1、4−ビス−(3−ベンジルアミノ−2−ヒドロキシプロピルアミノ)−9、10アンスラセンジオン;及び前記の任意のものの混合物及び製薬上許容できる塩を含む。
NMDA受容体活性化の主な細胞内連鎖をブロックしそしてそれ自体本発明の実施に有用な追加の非毒性物質は、カルモジュリンの阻害剤例えばフェノチアジン、特にクロロプロマジン、クロロプロマジンスルホキシド、プロクロロペラジンジマレエート、ペルフェナジン、トリフルオロペラジン、フルフェナジン、フルフェナジンエナンテート、フルフェナジンデカノエート、チオリダジン、メソリダジン、ベシレート、ピペラセタジン、アセトフェンジンジマレエート及びフェノチアジンスルホキシド;ナフタレンスルホンアミド例えばN−(6−アミノヘキシル)−5−クロロ−1−ナフタレンスルホンアミド、N−(6−アミノヘキシル)−5−クロロ−2−ナフタレンスルホンアミド及びN−(6−アミノヘキシル)−5−ブロモ−2−ナフタレンスルホンアミド;4−置換−4H、6H−ピロロ[1、2−a][4、1]ベンゾオキサゼピン例えば1、3−ジヒドロ−1−{1−[(4−メチル−4H、6H−ピロロ[1、2−a][4、1]ベンゾオキサゼピン−4−イル)メチル]−4−ピペリジニル}−2H−ベンジイミダゾール−2−オン;ベンズヒドリル例えばN−[2](ジフェニルメチルチオエチル)−2−(トリフルオロメチル)−ベンゼンエタンアミン、N−[2−(ビス(4−フルオロフェニル)メチルチオ)−エチル]−2−(トリフルオロメチル)ベンゼンエタンアミン及びN−[2−(ビス(4−フルオロフェニル)メチルチオ)エチル]−3−(トリフルオロメチル)ベンゼンエタンアミン;三環抗うつ薬例えばイミプラミン、2−クロロイミプラミン及びアミトリプチリン;ペンフルリドール;ハロペリドール;ピモジド;クロザピン;カルミダゾリン;並びに前記の任意のものの混合物及び製薬上許容される塩を含む。
鎮痛増強剤の二つの群のなかで、NMDA受容体拮抗剤が好ましく、これらのなかで、デキストロメトロファンが前記の理由から好ましい。
トラマドールの投与量のレベルは、もちろん、有効な程度の鎮痛を誘発するのに十分なものであろう。従って、例えば、成人では、トラマドールの代表的な単位投与量レベルは、約50mgから約100mgに変化できる。これらより多い又は少ないトラマドールの投与量レベルは、もちろん、もし所望ならば使用できる。
その鎮痛活性増強効果を示すために、NMDA受容体活性化の主な細胞内連鎖をブロックする非毒性NMDA受容体拮抗剤及び/又は物質は、鎮痛増強量で存在しなければならない。この量は、本発明の実験データを考慮して日常の投与量決定の手順を用いる当業者によりこのタイプの所定の物質について実験的に決定できる。NMDA受容体拮抗剤であるデキストロメトロファンの場合には、成人に関する通常の推薦された投与量は、少なくとも約30mgであり、好ましくは少なくとも約60−120mgであろう。もしもちろん十分な複数の単位投与物が一度に投与されて前記の最低のデキストロメトロファンの投与をもたらすならば、単位投与物は、これらの量より少ないデキストロメトロファンを含むことができる。それゆえ、例えばそれぞれ50mgのトラマドール及び約30mgのデキストロメトロファンを含む1又は2個の錠剤が、一度に投与されて(24時間に8回以内)本発明により増強された鎮痛をもたらす。
トラマドール及び鎮痛増強剤はともに投与される必要はないが、それらは、同時に有効なレベルで患者に両者が存在すべきである。トラマドール及び鎮痛増強剤を別々に投与することも本発明の範囲内にあるが、都合の良さの点で、それらが単一の治療組成物としてともに投与されることが好ましい。投与のすべての態様が包含され、例えば投与は、経口、直腸内又は静脈内、筋肉内、皮下、くも膜下、硬膜外又は脳室内の注入により行うことができる。
トラマドール及び鎮痛増強剤を含む治療組成物は、周知且つ確立されたプラクティスに基づいて1種以上の治療上許容できる成分により元来処方されるだろう。従って、組成物は、液体、粉末、エリキシル、注入可能な溶液などとして処方できる。経口使用のための処方は、薬理学的に活性な成分が不活性固体希釈剤例えば炭酸カルシウム、燐酸カルシウム又はカオリンと混合される錠剤又はハードカプセルとして提供でき、又は活性成分が油性の媒体例えば液体パラフィン又はオリーブオイルと混合されるソフトカプセルとして提供できる。
水性懸濁物は、薬理学的に許容できる助剤、例えば沈澱防止剤例えばナトリウムカルボキシメチルセルローズ、メチルセルローズ、ヒドロキシプロピルメチルセルローズ、アルギン酸ナトリウム、ポリビニルピロリドン、トラガントガム及びアラビアガム;分散又は湿潤剤例えば天然ホスファチド例えばレシチン又はアルキレンオキシドと脂肪酸との縮合生成物例えばポリオキシエチレンステアレート、又はエチレンオキシドと長鎖脂肪族アルコールとの縮合生成物例えばヘプタデカエチレンオキシセタノール、又はエチレンオキシドとヘキトール及び脂肪酸由来の部分エステルとの縮合生成物例えばポリオキシエチレンソルビトールモノオレエート、又はエチレンオキシドと脂肪酸及びヘキシトール無水物の部分エステルとの縮合生成物例えばポリオキシエチレンソルビタンモノオレエートを含むことができる。水性懸濁物は、また1種以上の保存剤例えばエチル又はn−プロピル−p−ヒドロキシベンゾエート、1種以上の着色剤、1種以上の香料及び1種以上の甘味料例えば蔗糖、サッカリン又はナトリウム或いはカルシウムシクラメートを含むことができる。
トラマドール及び鎮痛増強剤に加えて、本発明の治療組成物は、少なくとも1種の他の薬理学的に活性な物質例えばアセトアミノフェン、非ステロイド抗炎症薬剤例えばアスピリン、ジクロフェナック、ジフルシナル、エトドラック、フェンブフェン、フェノプロフェン、フルフェニサル、フルルビプロフェン、イブプロフェン、インドメタシン、ケトプロフェン、ケトロラック、メクロフェナム酸、メフェナム酸、ナブメトン、ナプロセン、オキサプロジン、フェニルブタゾン、ピロキシカム、スリンダック、トルメチン、ゾメピラックなど、麻酔性鎮痛薬例えばコデイン、オキシコデインなど、又は局所麻酔薬例えばリドカイン、ジブカイン、ブピビケインなど(組成物が注入物として提供されるとき)を含むことができる。
以下の実施例は、Sprague−Dawleyラットに誘発された関節炎の痛みを軽減する本発明の方法の例である。
実施例
A.相乗作用及びMethod of Isoboles
2種の医薬の相乗的な組合せは、個々の薬剤単独の有効性に基づいて予想されるであろうものより大きい有効性を有する組合せとして定義される。Berenbaum「what is Synergy?」Pharmacological Reviews、v.41、p.93(1989)により説明且つ立証されるように、薬剤の組合せの予想される効果を決定する唯一の一般に確かな方法は、有効性の等しい投与の実験的な決定、即ちそれがより普通によばれる「Method of Isoboles」である。Method of Isobolesの完全な導出そしてその原理については、Berenbaumを参照する。2種の異なる薬剤(薬剤A及び薬剤B)の組合せが研究されつつあるとして、Method of Isobolesは、一般に、単独で作用する各薬剤の用量・反応曲線の知識を要する。さらに、薬剤Aの投与量da及び薬剤Bの投与量dbの或る固定した組合せの有効性が決定される。DAと呼ばれる薬剤A単独の用量・反応曲線から、組合せと有効性が等しいAの投与量が決定される。同様に、Dbと呼ばれる薬剤B単独の用量・反応曲線から、組合せと有効性が等しいBの投与量が決定される。このやり方で、DA、Db、及びAのdA及びBのdbの投与量の組合せは、すべて同じレベルの有効性が生じる。
縦軸が薬剤Aの投与を示しそして横軸が薬剤Bの投与を示す図1に示されるサンプルIsobolesでは、点(Da、0)及び(0、Db)は、直線で結ばれる。もし点(da、db)としてグラフに示されるとき組合せで使用される投与量(da、db)が、Da及びDbとを結ぶ線に入るならば、組合せは、相乗的でも拮抗的でもないと結論される。しかし、もし点(da、db)がDa及びDbを結ぶ線の左そして下に入るならば(点線で示されるように)、組合せのA及びBのより少ない投与量が、単独で作用するAそして単独で作用するBの有効性のレベルに基づいて有効性の同じレベルを達成するのに必要とされる。これは、A及びBの組合せが相乗的であることを示す。
理論的には、たとえ少量に過ぎなくてもDa及びDbを結ぶisoboleの下に入る任意の点が、相乗的であることを示す。実際には、単独又は組合せで作用する薬剤の有効性の実際のレベルは、正確に知ることができない。平均的な有効性のこれらのレベルは、所定の実験的なアッセイの正確さまで評価できるに過ぎない。アッセイの結果が相乗性の肯定的な証拠をもたらすかどうかを決めることを試みるには、データに示される変化性のレベルに対する処理間の差のサイズを判断する方法を使用しなければならない。有意性の統計的なテストは、実験データの分析に基づいて、2種の薬剤及びそれらの組合せの評価に含まれる処理のなかの実際の差に関する結論ができるかどうかを決定する規則を提供する。有意のテストは、一般に、二つの量の計算を含む。(a)これらの差が概算される正確さに対する処理のなかの差のサイズを測定するテストの統計値、及び(b)テストの統計値の有意のレベル。
有意のレベルが小さければ小さいほど、有効性にまさに等しい処理が観察されるのと同じ大きさの差を生じたとは考えられない。事実、有意のレベルは、実際には確率である。特に、それは、比較される処理が等しいとして、それらを比較するテストの統計値がその観察された値と少なくとも同じであろうという確率である。統計的な有意のテストは、もしこの確率又は有意のレベルが小さければ、比較される処理の同等性の帰無仮説を斥ける。Cochranら「Experimental Designs」(John Wiley and Sons、New York、1957)に述べられているように、0.05が、統計的に有意であることを決定する普通に使用されるカットオフ値である。p<0.05カットオフ値を使用して、その有意のテストが0.05より低い有意のレベルを生ずる処理は、一般に、有意に差があると考えられる。
前記のMethod of Isobolesにより要求されるように、単独で投与されるトラマドール(TRA)及びデキストロメトロファン臭化水素酸塩(DM)の両者の投与量の範囲の有効性のレベルを評価するアッセイが行われた。投与量の一つの組合せも研究された。図2−4は、これらのアッセイの結果を要約している。
図2−4のグラフでは、バーの高さは、その処理に関するアッセイにより評価される有効性の平均のレベルに相当する。バーの頂部からの線の長さは、有効性の評価された平均レベルの正確さの目安を提供する。短い線は、データにおけるより少ない変化性を示し、それゆえ、その処理の真の平均有効性が評価された平均に近いというより大きな信頼性を示す。長い線は、大きな変化性を意味する。さらに、星印「*」を付されたこれらのバーは、0.05の前記の有意のレベルを使用して統計的に意味のある薬剤反応を示す。図2では、DMの投与の何れも、使用されるアッセイ法(下記のRandall−Sellitoテスト)により塩水よりも統計的に有意な鎮痛を示さなかった。図3では、評価されるTRAの種々の投与量のなかで、TRA40投与のみが、塩水よりも統計的に有意な鎮痛を有することが観察された。図4では、TRA20/DM50投与のみが、塩水単独、TRA20単独及びDM50単独より統計的に有意な鎮痛を有することが観察された。これらの結果は、以下に詳細に開示される。
B.テストの手順
関節炎の痛み(単関節炎)は、完全アジュバントの関節包内注入によりテストラットに誘発された(Butlerら「Pain」48(1992)pp.73−81)。完全アジュバントは、パラフィン油(6mL)、0.9%NaCl(4mL)及びTween80(1mL)の混合物中の60mgの死滅したMycobacterium butyricumから作られた。ラットを、混合N2O/O2ガス中で投与されたイソフルランにより短時間麻酔にかけた。1滴の完全アジュバント(約0.05mL)を26ゲージ針で踝(足根脛骨)関節中に注入した。単関節炎の痛みが注入約7日後に生じ、約2週間後にピークになった(Butlerら、同上)。
関節炎の痛みの以下の行動テストは、完全アジュバントの注入14日後でなされて文献(Butlerら、同上;Perrotら「Pain」52、(1993)pp.41−47)に記述されているようにテストについてベースラインのスコアを確立した。
機械的痛覚過敏
Randall−Sellitoテストを使用して、機械的痛覚過敏を調べた(Butlerら、同上;Perrotら、同上)。Randall−Sellito装置により伝達される等級のついた機械的な力(g)を、病気にかかった踝の領域に適用した。ラットは、適用される力がその痛みの閾値に達したとき、その後ろ足を引っ込めるか、又は声をあげた。この力は記録されて、機械的痛覚過敏の程度を示した。
このテストのベースラインのスコアの決定後、各ラットは以下のテスト物質の経口投与をうけた。(1)種々のレベルのTRA単独、(2)種々のレベルのDM単独、(3)20mg/kgのTRA及び50mg/kgのDMの組合せ、(4)塩水(コントロール)単独。各投与1.5時間後、上記の行動テストを繰り返して、関節炎の痛みに対する各テスト物質の効果を調べた。
C.テスト結果
図2、3及び4は、前記のアッセイから得られたデータをグラフで示す。データは、分散の分離2×2階乗積分析を使用して分析した。処理の個々の対間の比較は、Waller−Duncan K比tテスト(「SAS Procedures Guide」Version6,34d ed,SAS Institute Inc.Cary,NC(1990)pp.705以下)を使用して行われ、分散の階乗積分析からのプールした平均平方誤差を分母にした。

種々のレベルでのDM単独及び塩水単独の投与前及び後の後足の引っ込みの閾値の変化%を使用して機械的痛覚過敏の程度を表示した。テスト物質の投与1.5時間後でテストしたとき、後足引っ込みの閾値は、塩水コントロールに比較して、テストされた12.5、25、50及び100mg/kgの投与量のすべてにおいてDMの単一の経口投与をされたラットにおいて統計的に有意の程度に低下しなかった。これらの結果は、それ自体で投与されるとき、DMは、Randall−Sellitoテストにより測定して、有意な鎮痛有効性を有しないことを示す。

種々のレベルでのTRA単独及び塩水単独の投与前及び後の後足の引っ込みの閾値の変化%を使用して機械的痛覚過敏の程度を表示した。テスト物質の投与1.5時間後でテストしたとき、後足引っ込みの閾値は、塩水コントロールに比較して、テストされた10及び20mg/kgのTRAの単一の経口投与をされたラットにおいて統計的に有意の程度に低下しなかった。しかし、40mg/kgでのTRA経口投与は、後足引っ込みの閾値において統計的に有意な(塩水コントロールに比較して*=p<0.05)増加を生じ、種々のレベルでのDM単独及び塩水単独の投与前及び後の後足の引っ込みの閾値の変化%を使用して機械的痛覚過敏の程度を表示した。テスト物質の投与1.5時間後でテストしたとき、後足引っ込みの閾値は、塩水コントロールに比較して、Randall−Sellitoテストにより測定して、有意に増強した鎮痛効果を立証した。

各テスト物質の投与前及び後の後足の引っ込みの閾値の変化%を使用して機械的痛覚過敏の程度を表示した。テスト物質の投与1.5時間後でテストしたとき、後足引っ込みの閾値は、10mg/kg及び20mg/kgのTRAの単一の経口投与をされたラットでは統計的に有意な程度に低下しなかったが、40mg/kgの投与量をうけたラットでは、統計的に有意(塩水コントロール単独にくらべて*=p<0.05)に低下した。DM(50mg/kg)と組合わさったTRA(20mg/kg)の経口投与も、後足引っ込みの閾値において統計的な有意な(塩水コントロールに比較して*=p<0.05)増加を生じ、Randall−Sellitoテストにより測定して、TRA及びDMの組合せについて有意に増強した鎮痛効果を立証した。120mg/kg及び20mg/kgの投与量のTRA単独が統計的に有意な鎮痛をもたらさなかったという事実、さらに投与された投与量のレベルのすべてでのDMが統計的に有意な鎮痛をもたらさなかったという事実があるため、20mg/kgのTRA及び50mg/kgのDMの組合せが統計的に有意な鎮痛を有し、その上40mg/kgのTRAの投与量のそれと、もし優れないとしても、匹敵したレベルの鎮痛を有することは全く予想されずそして驚くべきことである。
Claims (6)
- 治療有効成分として(a)トラマドール及び(b)デキストロメトルファン又はその製薬上許容し得る塩からなることを特徴とする咳の症状に伴う痛み以外の痛みの治療用医薬組成物。
- (a)及び(b)が単一投与形態として同時投与される請求項1記載の組成物。
- (a)及び(b)が徐放投与形態として同時投与される請求項2記載の組成物。
- (b)の投与量が少なくとも15mg/kgである請求項1記載の組成物。
- (b)の投与量が少なくとも20mg/kgである請求項4記載の組成物。
- 痛みが関節炎症状による請求項1記載の組成物。
