Description

【発明の詳細な説明】 本発明は、外科用のステープラ装置に関する。
特に本発明は、端部と端部とを吻合させるステープラ
装置に関し、更に詳しくは、外科用ステープラ装置のア
ンビル組立体に関する。
従来、体の組織をステープルで止めるための種々の形
式の外科用ステープラ装置が知られている。例えば、胃
および食道の手術に種々の形式のステープラを用いるこ
とが知られており、例えば端部と端部、端部と側部又は
側部と側部を吻合する古典的又は進歩した胃の再形成手
術にも使用されている。或る場合には、U.S.Surgical C
orporation社製のEEA型の吻合器具、又は器具の環状ハ
ウジング内に引っ込めることができる中心ロッドのねじ
端部にアンビルを取付けることができるように構成され
た吻合器具等を用いて吻合部を形成することが知られて
いる。また或る場合には、かような器具は、アンビルを
除いて、胃の内腔の中に導入される。次いで、吻合した
箇所に形成された開口部に中心ロッドのねじ先端部を通
せば、アンビルをねじロッドの端部に取付けることがで
きるようになる。従ってアンビルを十二指腸内に挿入し
て、十二指腸の端部を例えば巾着（きんちゃく）縫合に
より結紮することができる。次いで、胃を十二指腸に結
合するため、器具を閉じてステープルを打ち出す。よく
知られているように、次に器具は、胃の体腔に最初に形
成された開口部から取出される。
吻合器具が、その中心ロッドにアンビル組立体を螺着
するように構成されている場合には、中心ロッドを胃壁
と整合させたり胃壁に通したりするときに、ねじによっ
て胃の組織が切り裂かれてしまう危険性があることが分
っている。特に、中心ロッドを通すために切除を受けて
いる開口部の径が中心ロッドの直径よりも小さい場合に
は、切り裂かれてしまう危険性が大きい。また、胃壁に
開口部を形成する場合に、例えば巾着縫合糸アプリケー
タと共に種々の形式の切除装置が使用される。また、ス
テープル止め作業を行なう間に吻合器具から打ち出され
るステープルに対してアンビルのバケットの環状配置を
整合させるには、アンビルを吻合器具のステープルカー
トリッジと整合させなければならないことも知られてい
る。このために、アンビルは２つの部分から構成されて
いる。アンビルの一方の部分にはバケットが設けてあ
り、また、アンビルの一方の部分は、例えば、アンビル
の平らな部分を中心ロッドの同様な平らな部分で案内す
ることによって、中心ロッドを整合されるようになって
いる。アンビルの第２の部分には内ねじが設けてあっ
て、該第２の部分を回転することによって中心ロッドに
螺着できるようになっている。しかしながら、外科医は
それぞれの主観に基いてアンビルを中心ロッドに対して
ねじ込むため、アンビルを中心ロッドに螺着する度合い
にはバラツキが生じ、このため、アンビルをステープル
カートリッジとの間の距離は最適距離から外れてしま
う。
従って本発明の目的は、端部と端部とを吻合する形式
のステープラを使用するのに要する時間を短縮すること
にある。
本発明の他の目的は、アンビル組立体のシャフトを通
す開口部を体組織に穿けることのできる外科用ステープ
ラ装置を提供することにある。
本発明の他の目的は、吻合を行なうのに要する時間を
短縮することにある。
本発明の他の目的は、吻合を行なう場合の体組織の損
傷を小さくすることにある。
本発明の他の目的は、取外し自在のアンビルを吻合器
具上で正確に位置決めすることにある。
簡単にいえば、本発明により、管状のハウジングと、
ステープルプッシャ組立体と、該ステープルプッシャ組
立体内に取付けられていてトロカールおよびアンビル組
立体のいずれか一方を選択的に取付ける手段とを有する
外科用ステープラ装置が提供される。
管状のハウジングは任意の適当な構造にすることがで
き、例えば、ハウジングの先端部を固定シャフト器具で
構成してもよいし、その先端部を可撓シャフト器具で構
成してもよい。
ステープルプッシャ組立体も任意の適当な構造にする
ことができ、ステープルプッシャ組立体がハウジング内
に引っ込んだ位置と環状に配置されたステープルを押出
すべく突出した位置との間で移動できるようにハウジン
グの先端部の中に移動自在に取付けられている。ステー
プルの環状配置は、１列以上の環状列を含んでいる。
トロカール又はアンビル組立体を取付けるためにステ
ープルプッシャ組立体内に取付けられた手段は少くとも
１つの戻り止めを備えており、この戻り止めは、トロカ
ール又はアンビル組立体のシャフトを保持する保持位置
と、トロカール又はアンビル組立体のシャフトを解放す
る解放位置との間で半径方向に移動できるように取付け
られていう。この戻り止めは、解放位置から保持位置に
向って動くようにばねで押圧されている。
１つの実施例においては、トロカール又はアンビル組
立体を取付ける手段が、ステープルプッシャ組立体内で
摺動自在に取付けられたシリンダとして構成されてお
り、該シリンダはその先端部に中央ボアを備えていて選
択されたトロカール又はアンビル組立体のシャフトを受
入れることができ、前記シリンダの側壁には前記ボアと
連通する少くとも１つも長手方向のスロットが設けてあ
る。更に、トロカール又はアンビル組立体を取付ける前
記手段が、前記ボア内に設けられた少くとも１つの戻り
止めを備えており、該戻り止めは前記スロットと整合し
ていて、前記シャフトを前記シリンダ内に保持しておく
拘束位置と前記シャフトを解放すべく前記スロット内に
ある解放位置との間で半径方向に移動できるようになっ
ている。
本発明の外科用ステープラ装置は更にアクチュエータ
手段を有しており、該アクチュエータ手段はステープル
プッシャ組立体に連結されていて、該ステープルプッシ
ャ組立体を少くとも引っ込み位置からステープルを押出
すための突出位置まで移動できるようになっている。更
に本発明のステープラ装置は第２のアクチュエータ手段
を有しており、該第２のアクチュエータ手段はトロカー
ル又はアンビル組立体を選択的に取付ける取付け手段に
連結されていて、ステープルプッシャ組立体内の引っ込
み位置と受入れられたシャフトを解放できるようにする
ためステープルプッシャ組立体に対して突出した位置と
の間で前記取付け手段を移動できるようになっている。
これらの両アクチュエータ手段は、ステープラ装置の手
元側に近い方の端部から外科医によって操作される。
使用するトロカールは任意の適当な構造のものとする
ことができる。基本的にはトロカールは、その手元側に
近い方の端部に環状凹所が設けられたシャフトからなっ
ており、前記環状凹所は、トロカールを管状のハウジン
グ内に取付ける手段の１つ以上の戻り止めと係合するよ
うになっている。
アンビル組立体は、ハウジング内に着脱自在に取付け
られるシャフトと、押出されたステープルをクリンチす
なわち折り曲げて締付けるべくステープルプッシャ組立
体に向くようにシャフトに取付けられたアンビルとから
構成されている。シャフトにはまた、上記のようにして
取付け手段の戻り止めと係合する凹所が設けられてい
る。
また、前記アンビル組立体は、前記アンビルの環状に
配置されたバケットを前記ステープルプッシャ組立体内
のステープルと整合させるべく、前記取付け手段まで前
記シャフトを円周方向に整合させる整合手段を備えてい
る。例えばこの整合手段は、ステープラ装置の環状ハウ
ジング内に配置された環状のシェルライナを備えてお
り、該シェルライナは該シェルライナの円周方向に配置
された長手方向に延びる複数の内スプラインを有してお
り、該内スプラインはシャフトの円周方向に配置された
長手方向に延びる複数の外スプラインと協働して、シャ
フトを挿入する際にシャフトを整合させるようになって
いる。例えばスェルおよびシャフトの各スプラインには
面取りされた端部が設けてあり、シャフトの挿入の間に
シャフトを案内すなわち回転させることができる。
アンビル組立体にはまたロック手段が設けてあり、シ
ャフトのアンビルを着脱自在に固定できるようになって
いる。アンビルはハウジングを備えており、ロック手段
はシャフトに設けた凹所と、シャフトのハウジングをロ
ックするときに少くとも一部が前記凹所内に配置され
る、ハウジングに設けたばねと、ハウジング内に移動で
きるようにい取付けられていて、ラッチの解放位置への
運動に応答して前記凹所からばねを折出すべくばねに当
接するラッチとを備えている。ラッチを解放位置にする
と、ばねがシャフトの凹所から出るように動かされて、
ハウジングをシャフトから引き抜くことができるように
なる。
アンビル組立体はまた、アンビルのハウジングをシャ
フト上で整合させるための整合手段を備えており、アン
ビル上で環状に配置されたバケットをステープルプッシ
ャ組立体内のステープルと整合させるようになってい
る。
例えば胃と十二指腸との間に吻合を形成するのに本発
明の外科用ステープラ装置を使用するとき、最初にステ
ープラ装置にトロカールを装着して引っ込み位置に移動
させる。胃の適当な開口部を通してステープラ装置を胃
の内腔まで入れた後、外科医がアクチュエータ手段を操
作してトロカールを突出位置に移動させる。このとき、
トロカールの鋭い先端部が露出して、胃壁（例えば以前
に縫合した巾着縫合糸の領域）を突き刺すのに用いられ
る。
その後アクチュエータ手段を更に操作すると、トロカ
ールの取付け手段が更に突出して、該取付け手段はトロ
カールを解放できる位置まで移動される。次いで取付け
手段からトロカールを軸線方向に取外して、アンビル組
立体を挿入する。この場合、アンビル組立体の全体を挿
入してもよいし、あるいはアンビル組立体のシャフトの
みを挿入してもよい。いずれにせよ、一旦シャフトが取
付け手段に挿入されたならば、アクチュエータ手段を操
作してシャフトを管状ハウジング内に引き入れ、シャフ
トをハウジングに対して固定した位置に保持する。
アンビル組立体のハウジングが予めシャフトに取付け
られていない場合には、ハウジングをシャフトの先端部
から差し込めば、ハウジングに設けたばねがシャフトの
凹所と係合することにより、所定位置にパチンとロック
される。これにより吻合が行なえる状態になるが、本発
明のステープラ装置は、この形式の従来の吻合器具と同
様に使用することができる。
本発明の目的および利点は、添付図面に基く本発明の
実施例についての以下の詳細な説明により明らかになる
であろう。
第１図には、例えば胃12の吻合を行なうための外科用
のステープラ装置10が示してあり、該ステープラ装置10
は例えば剛性又は可撓性材料で作られた長いシャフト11
を備えている。図示のように、ステープラ装置10の手元
側の端部は、胃12に外科的に準備して穿けられた開口に
通されている。また、シャフト13が胃12の胃壁15に形成
した開口部14から突出しており、アンビル16が受入れる
ことができるようになっている。
第２図には、ステープラ装置10の手元側の端部のみを
示してある。なぜならば、ステープラ装置10の残部は従
来からよく知られた構造で構成されているからである。
図示のように、ステープラ装置10は、多数の部分から形
成された管状のハウジング17を備えている。例えばハウ
ジング17は、シリンダ18と、該シリンダ18の端部と衝合
しているシェルライナ19と、該シェルライナ19をシリン
ダ18の端部に固定するための環状のシェル支持体20と、
該シェル支持体20上で摺動できる大径の取付けリング21
とから構成されている。取付けリング21の円錐状部分に
は適当な通気孔22が設けられており、この通気孔22は公
知のようにハウジング17の内部との通気をするためのも
のである。図示のように、取付けリング21は、先端部の
ステープルカートリッジ23を受入れることができる大き
さを有している。このカートリッジ23は適当な形状のも
のでよく、例えば環状の２列に配置されたステープルを
収容できるものとすることができる。
ステープラ装置10は更にステープルプッシャ組立体25
を有しており、該プッシャ組立体25は、第２図に示すよ
うにハウジング10内に引っ込められた位置と、ステープ
ル24の環状列を押し出すための突出位置との間で移動で
きるようにハウジング10の先端部の中で移動自在に取付
けられている。プッシャ組立体25は円錐形のプッシャ26
を備えていて、該プッシャ26は、取付けリング21に設け
た通気孔22と整合する複数の通気孔27′を有している。
更にプッシャ組立体25は環状のハウジング27を備えてお
り、該リング27からは複数のプッシャフィンガ28が軸線
方向に延在していて、これらのプッシャフィンガ28はカ
ートリッジ23内のステープル24と整合している。更に、
プッシャフィンガ28内には、環状のブレード29が同心状
に取付けられている。
ステープルプッシャ組立体25は更に、円筒状部材すな
わちクイル30を備えており、該クイル30はハウジング17
のシリンダ18内で摺動自在に装着されている。このクイ
ル30の先端部には弧状のフィンガが設けてあり、該フィ
ンガはシェルライナ19に形成して適当な開口部31を貫通
してプッシャ26と衝合している。クイル30には適当なア
クチュエータ手段32が連結されており、プッシャ組立体
25を、第２図に示す引っ込み位置から突出位置（図示せ
ず）まで駆動してステープル24を押出すようになってい
る。
ステープラ装置10には更に、第２図に示すようなトロ
カール33又は第１図および第６図に示すようなアンビル
組立体13、16のいずれか一方を選択的に取付けるべくス
テープルプッシャ組立体25内に取付けられた手段を有し
ている。この手段は、プッシャ組立体25のクイル30に摺
動自在に装着されたシリンダ34を備えている。該シリン
ダ34には中央のボア35が形成されており、このボア35は
シリンダ34の先端部から中間点まで内方に延在してい
る。シリンダ34の側壁には、互に直径方向に対向して配
置されてありかつ長手方向に延在している少くとも１対
のスロット36が設けられており、これらのスロット36は
ボア35と連通している。ボア35内には、スロット36と整
合するようにＵ字形ばねの形状をなすばね手段37が配置
されている。このばね手段37は基部38を備えており、該
基部38はボア35の領域内に保持されるようにするため、
ボア35の底部内に取付けられている。ばね手段37は更
に、基部38から延びている１対のアーム39を備えてお
り、該アーム39はそれぞれのスロット36と整合してい
る。スロット36を整合している各アーム39の先端部には
戻り止め40が設けられており、該戻り止め40はスロット
36によって拘束位置の間で半径方向に移動することがで
きる（第４図参照）。
第２図に示すように、シリンダ34にはステンレス鋼等
で作られたスリーブ41がきつく嵌入されており、該スリ
ーブ41はシェルライナ19と衝合してばねアーム39を図示
の拘束位置に保持している。このスリーブ41はばねアー
ム39の材料よりも硬質の材料で作られていて、ステープ
ル装置10の使用中にアーム39内に滑り込んだり、アーム
39から摺動して外れたりする運動を確実に行なえるよう
になっている。シリンダ34のまわりにはコイルばね42が
配置されておりかつクイル30内の凹所に入れられてい
て、スリーブ41をシェルライナ19に対して押圧してい
る。このコイルばね42は、クイル30が以下に延べるよう
にして移動するとき、スリーブ４とクイル30とが相対運
動することを可能にしている。
第２図に示すとき、トロカール33はシャフト41′を有
しており、該シャフト41′には環状凹所42′が形成され
ていて、この環状凹所内にはばね37の戻り止め40が係合
するようになっている。またトロカール33は、体の組織
を突き刺すための鋭い先端部を備えている。例えばトロ
カール33の先端部には２段の面取り部、すなわち、先端
の面取り部33aと該部分33aの直ぐ後に続く面取り部33b
とが形成されており、先端の面取り部33aはトロカール3
3の軸線に対して後方の面取り部33bよりも大きな角度で
傾斜している。これにより、組織を最初に突き刺すため
の鋭い尖端部が形成される。
また、もしも必要な場合には、Ｘ線装置等を用いて組
織の一部内にトロカール33を配置できるようにするた
め、トロカール33を硫酸バリウムのようなＸ線透過性の
材料でコーティングしておくこともできる。
第３図に示すように、シリンダ34の手元側の端部には
アクチュエータ手段43が連結されていて、シリンダ34を
第２図に示す引っ込み位置と第４図に示す完全に突出し
た位置との間で移動することができるようになってい
る。図示のように、アクチュエータ手段43は相互に係合
している１対のバンドとして構成されており、バンドの
一端は、シリンダ34を貫通している１対のピン44により
シリンダ34の手元側の端部のスロット内に固定されてい
る。
第５図に示すように、アンビル組立体のシャフト13の
手元側の端部には、例えばばね37の戻り止め40を受入れ
るための環状凹所45の形状をなす取付け手段が設けられ
ている。また、シャフト13の手元側の端部は円錐形の先
端部として形成されており、シリンダ34内にシャフト13
を整合することあるいはもしも組織の管内にアンビル組
立体を入れる必要があるときにはトロカールとして働ら
かせることができるようになっている。更に、ハウジン
グ17内でシャフト13を円周方向に整合させるための整合
手段が設けられている。この点に関しては第10図および
第11図に示すように、この整合手段は、シャフト13の外
周面に配置された複数の長手方向のスプライン47として
構成されている。シェルライナ19に設けられたスプライ
ン46の各々の先端部は面取りされておりまたシャフト13
に設けたスプライン47も面取りされている。このため、
シャフト13のスプライン47をシェルライナ19内に最初に
挿入しようとするとき、両スプライン46、47の面取りさ
れた端部によって、シャフト13が第11図に示すように僅
かに回転され、これにより両スプライン46、47が以下に
述べる理由により適正に係合するようになる。
第６図に示すように、シャフト13の先端部にはアンビ
ル16が摺動自在に装着される。アンビル16はアンビルプ
レート48を備えていて、該アンビルプレート48のステー
プルカートリッジ23と対面する面にはバケット49が環状
に配置されている。またアンビル16は２つの部分からな
るハウジングを備えている。ハウジングの２つの部分と
は、環状のアンビル支持ボディ50およびアンビル先端部
51であり、アンビル支持ボディ50にはアンビルプレート
48が固定されておりまたアンビル先端部51はセメントす
なわち接着剤のようなものでアンビル支持ボディ50に固
定されている。
アンビル支持ボディ50は、バケット49を複数の位置の
中の１つに位置決めする（すなわち、カートリッジ23の
ステープル24と整合させる）ため、シャフト13の本体を
整合させる整合手段を備えている。この整合手段は、ア
ンビル支持ボディ50に設けられていてスプライン47と係
合する複数の内スプライン53からなる。前述のように、
シャフト13の手元側の端部に設けたスプライン47は、該
シャフト13をシェルライナ19従ってプッシャフィンガ28
と整合させるべく作用する。カートリッジ23は今度はプ
ッシャフィンガ28と整合する。スプライン52、53は、バ
ケット49がステープル24と整合することができるように
配置されている。
アンビル16は更に、該アンビル16ろシャフト13に着脱
自在に固定するためのロック手段を備えている。この自
然のため、弧状の端部を備えた全体として長方形をなす
受け板54が、アンビル支持ボディ50の先端部に設けられ
た同様な形状の凹所内に取付けられている。受け板54に
は中央の開口部55が設けてあり、該開口部55の直径はシ
ャフト13の小径先端部の直径よりも大きい。アンビル16
がロックされた位置にあるとき、受け板54はシャフト13
の小径端に設けた環状凹所56と整合した状態にある。
ロック手段は更にばね57を備えている。該ばね57は受
け板57上で摺動しかつその少くとも一部が環状凹所56内
に配置されてアンビル支持ボディ50をシャフト13にロッ
クするようになっている。第８図および第９図に示すよ
うに、ばね57はＵ字形をなしていて、ロック位置におい
て環状凹所56内に配置される１対の脚58を備えている。
第８図に示すように、アンビル先端部51内には、ばねで
押圧されたラッチ59がばね57と衝合関係をなして移動可
能に取付けられている。ラッチ59の手元側の端部には凹
所が形成されていてばね57の脚58を受入れておりかつば
ね57の脚58に対するカム面として作用する中央の衝合壁
60を有している。
第９図に示すように、ラッチ59がアンビル先端部51内
に移動すなわち解放位置に移動されると、ラッチ59の衝
合壁60によってばね57の脚58が拡げられ、シャフト13の
環状凹所から脚58が外される。これにより、２つの部分
からなるアンビル16をシャフト13から引き抜くことがで
きる。
第６図に示すように、シャフト13の先端部には円錐形
のヘッド61が設けてあり、アンビル16をシャフト13に取
付けるときにばね57の脚58がヘッド61を乗り越え易いよ
うにしてある。また、シャフト13aは、アンビル16に隣
接した箇所に小径部すなわち環状溝62を設けておき、該
環状溝62に、巾着縫合糸または巾着縫合糸を引抜くとき
に生じる体組織のひだ（盛上り）を収容できるようにし
てもよい。
図示のように、アンビル先端部51の先端には球状をな
す部分が設けてあり、十二脂腸のような体組織内に挿入
したり、腸その他の組織に対する手技を行なうことがで
きるように滑らかな表面を与えている。
本発明による外科用のステープラ装置10を使用するひ
際に、該ステープラ装置10には、第２図に示すようにス
テープルプッシャ組立体25内の凹んだ箇所に配置される
トロカール33を最初に取付けておくことができる。この
位置において、トロカール33のシャフト41′はばね37の
両端の戻り止め40によって保持され、シャフト41′をシ
リンダ34から軸線方向に引抜くことができないようにす
る。すなわち、第２図に示すように、ばね37の脚すなわ
ちアーム39はシリンダ34のスロット36内に押し拡げられ
るため、ばね37をシリンダ34のボア35から軸線方向に移
動することができなくなるのである。同時に、アーム39
がスリーブ41に当接するため、アーム39が半径方向外方
に動くことが防止される。
ステープラ装置10を胃12の開口部から内腔に挿通させ
た後、外科医がアクチュエータ手段43を操作して、シリ
ンダ34を第３図に示すような部分的に伸長した位置に移
動させる。この位置においてトロカール33の尖端部が管
状のハウジング17から軸線方向に突出し、層として図示
してある胃壁15に突き刺さる。同時に、シリンダ34がコ
イル30およびスリーブ41に対して移動し、ばね37の脚す
なわちアーム39は未だシェルライナ19およびブッシャ28
によって半径方向に拡がらないように保持された状態に
ある。すなわち、トロカール33のシャフト41′は未だ拘
束位置に保持された状態にある。
次に第５図に示すように胃壁15に開口部が形成された
ならば、アクチュエータ手段43を更に操作してシリンダ
34を更に突出させれば、ばね37の脚39が自由に半径方向
に動くことができるようになり、トロカール33のシャフ
ト41を解放できるようになる。次にトロカール33を適当
に掴み、胃壁15に形成された開口部を通してステープラ
装置10から取外す。トロカールのシャフト41を取外した
後、ばね37の戻り止め40は、図示のように拘束位置に押
し戻される。
次いで第５図に示すようにアンビル組立体のシャフト
13をシリンダ34内に挿入すれば、ばね37の戻り止め40が
シャフト13の環状凹所45内にパチンと嵌まり込む。その
後、アクチュエータ手段43を操作して、シリンダ34をク
イル30内に引っ込める。このとき、シャフト13のスプラ
イン47がシェルライナ19の内スプライン46に近付くと、
両スプライン46、47が互に嵌合し合う関係にない場合に
は、両スプライン46、47の面取りされた端部によりシャ
フト13が僅かに回転される。
シリンダ34が第５図に示すように引っ込み位置に引っ
込められたとき、シャフト13は第１図に示すようにアン
ビル16を受入れることができる状態にあり、アンビル16
がシャフト13にパチンと嵌め込まれる。
第６図に示すように、アンビル16がシャフト13に差し
込まれるとき、スプライン52、53によってアンビル支持
ボディ50（従ってバケット49）とカートリッジ23内のス
テープル24とが整合される。次に、例えばアンビル先端
部51を十二脂腸（図示せず）に挿入することにより、公
知の方法で吻合を完了することができる。
アンビル組立体の全体すなわちアンビル16およびシャ
フト13を、必要に応じて単一体としてシリンダ34内に挿
入することもできる。
また、シリンダ34内に内挿入する前に、アンビル組立
体を十二脂腸又は他の適当な内腔の中に挿入することが
できる。この場合、シャフト13のみを露出させておき、
十二脂腸又は内腔をアンビル16のまわりで閉じておき、
次にシャフト13をシリンダ34内に挿入して、前述のよう
にばね戻り止め40と係合させることもできる。次にアク
チュエータ手段43を操作して、第７図に示す吻合を行な
う位置にアンビル組立体を十二脂腸とを引っ込める。
一旦アンビル組立体13、16が吻合を行なう位置に位置
決めされたならば、ステープルプッシャ組立体25のアク
チュエータ手段32を作動させてステープル24を押し出
し、この間環状ブレード29により組織が「ドーナツ」状
に切断される。次いで、吻合箇所からステープラ装置10
が通常の方法で取外される。
かくして本発明による外科用のステープラ装置10は、
取外し自在のトロカールによりステープラ装置自体のた
めの孔を形成し、次にステープル作業を行なうためにア
ンビル組立体を受入れることができる。
更に本発明によれば、ねじ止め作業をすることなくア
ンビルを迅速かつ容易に所定位置に取付けることができ
るアンビル組立体が提供される。
また本発明のステープル装置は、小数の器具と少数の
手術手順とによって吻合を行なうことができ、従って、
最短時間内で吻合を行なうことが可能になる。
また本発明によれば、アンビルをパチンと嵌合するこ
とによって迅速に固定することができかつステープルカ
ートリッジ内のステープルに対してアンビルの位置を正
確に位置決めすることができる。
更に、吻合を行なった後にステープル装置を容易に取
外すことができるようにするため、外科医はアンビルの
シャフトからアンビルを取外し、次いで組織を通してア
ンビルを除去することが可能になる。

Claims (20)

    (57)【特許請求の範囲】
  1. 【請求項１】管状のハウジング17と、該ハウジング17の
    先端部内で移動できるように取付けられていて、前記ハ
    ウジング17内に引っ込められた位置と環状に配置された
    ステープルをハウジング17から押出すための突出位置と
    の間で移動することができるステープルプッシャ組立体
    25とを有している外科用ステープラ装置において、前記
    ステープルプッシャ組立体内に取付けられていて、トロ
    カール33およびアンビル組立体13、16を選択的に取付け
    ることができる取付け手段を有することを特徴とする外
    科用ステープラ装置。
  2. 【請求項２】前記取付け手段が少くとも１つの戻り止め
    40を備えており、該戻り止め40が、前記トロカール33お
    よびアンビル組立体13、16のいずれか一方を保持する保
    持位置と、解放する解放位置との間で半径方向に動き得
    るように取付けられている請求項（１）に記載の外科用
    ステープラ装置。
  3. 【請求項３】前記戻り止め40は、前記解放位置から前記
    保持位置に向ってばねで押圧されていることを特徴とす
    る請求項（２）に記載の外科用ステープラ装置。
  4. 【請求項４】前記取付け手段が前記ステープルプッシャ
    組立体25内で摺動自在に取付けられたシリンダ34を備え
    ており、該シリンダ34がその先端部に中央ボア35を備え
    ていてトロカール33およびアンビル組立体13、16のいず
    れか一方のシャフト41′、13を受入れることができ、前
    記シリンダ34の側壁には前記ボア35と連通する少くとも
    １つの長手方向のスロット36が設けてあり、前記取付け
    手段が更に前記ボア35内に設けられた少くとも１つの戻
    り止め40を備えており、該戻り止め40は前記スロット36
    と整合していて、前記シャフト41′、13を前記シリンダ
    34内に保持しておく拘束位置と前記シャフト41′、13を
    解放すべく前記スロット36内にある解放位置との間で半
    径方向に移動することができることを特徴とする請求項
    （１）に記載の外科用ステープラ装置。
  5. 【請求項５】前記ステープラプッシャ組立体25が、前記
    シリンダ34を摺動自在に受入れている円筒状のクイル30
    を備えていることを特徴とする請求項（４）に記載の外
    科用ステープラ装置。
  6. 【請求項６】Ｕ字形のばね37を更に有しており、該ばね
    37は前記ボア35内に取付けられた基部38と前記スロット
    36と整合している１対のアーム39とを備えており、前記
    各アーム39には前記トロカール33およびアンビル組立体
    13、16のいずれか一方のシャフト41′、13と係合する戻
    り止め40が設けられていることを特徴とする請求項
    （４）に記載のステープラ装置。
  7. 【請求項７】前記ステープルプッシャ組立体25を少くと
    も前記引っ込み位置から前記突出位置まで移動させるべ
    く前記ステープルプッシャ組立体25に連結された第１の
    アクチュエータ手段32と、該第１アクチュエータ手段32
    を前記ステープルプッシャ組立体25内の引っ込み位置と
    ステープルプッシャ組立体25に対して突出して、トロカ
    ール33およびアンビル組立体13、16のいずれか一方の受
    入れられている方を解放する突出位置との間で移動させ
    る第２のアクチュエータ手段43とを有していることを特
    徴とする請求項（１）に記載の外科用ステープラ装置。
  8. 【請求項８】前記ステープルプッシャ組立体25が、前記
    取付け手段を受入れる円筒状のクイル30と、該クイル30
    に固定されたプッシャ26と、前記ステープルプッシャ組
    立体25からステープルを押出すべく前記プッシャ26に固
    定された環状に配置されたフィンガ28と、該フィンガ28
    内で前記プッシャ26の同心状に固定された環状のブレー
    ド29とを備えていることを特徴とする請求項（１）に記
    載の外科用ステープラ装置。
  9. 【請求項９】管状のハウジング17と、該ハウジング17の
    先端部内で移動できるように取付けられていて、前記ハ
    ウジング17内に引っ込められた位置と環状に配置された
    ステープルをハウジング17から押出すための突出位置と
    の間で移動することができるステープルプッシャ組立体
    25とを有している外科用ステープラ装置において、前記
    ハウジング17内で着脱自在に嵌着されるシャフト13と、
    該シャフト13に取付けられていて前記ステープルプッシ
    ャ組立体25上に押出されたステープルを締付けるべく前
    記ステープルプッシャ組立体25と対面しているアンビル
    16とからなるアンビル組立体13、16を有することを特徴
    とする外科用ステープラ装置。
  10. 【請求項１０】前記ステープルプッシャ組立体25内で移
    動できるように取付けられていて前記シャフト13を解放
    し得るように該シャフト13と係合する取付け手段34、37
    と、該取付け手段34、37に連結されていて、該取付け手
    段34、37を前記ステープルプッシャ組立体25内の引っ込
    み位置と前記シャフト13を解放可能にする突出位置との
    間で移動させるアクチュエータ手段43とを有しているこ
    とを特徴とする請求項（９）に記載の外科用ステープラ
    装置。
  11. 【請求項１１】前記シャフト13が凹所45を備えており、
    前記取付け手段が少くとも１つの戻り止め40を備えてい
    て、該戻り止め40は前記凹所45内の保持位置と凹所45の
    外にある解放位置との間で半径方向に移動できるように
    取付けられていることを特徴とする請求項（10）に記載
    の外科用ステープラ装置。
  12. 【請求項１２】前記アンビルの環状に配置されたバケッ
    ト49を前記ステープルプッシャ組立体25内のステープル
    24と整合させるべく、前記取付け手段内で前記シャフト
    13を円周方向に整合させる整合手段を有していることを
    特徴とする請求項（10）に記載の外科用ステープラ装
    置。
  13. 【請求項１３】前記整合手段が、環状のシェル19の円周
    方向に配置された長手方向に延在する複数の内スプライ
    ン46と、前記シャフト13の円周方向に配置された長手方
    向に延在する複数の外スプライン47とからなることを特
    徴とする請求項（10）に記載の外科用ステープラ装置。
  14. 【請求項１４】前記シェル19の前記スプライン46の各々
    が面取りされた端部を備えていて、該端部が前記シャフ
    ト13の前記スプライン47と係合しかつ所定の整合位置を
    占めるように円周方向に前記スプライン47を案内するこ
    とを特徴とする請求項（10）に記載の外科用ステープラ
    装置。
  15. 【請求項１５】前記アンビル16を前記シャフト13に解放
    自在に固定するためのロック手段57を有していることを
    特徴とする請求項（９）に記載の外科用ステープラ装
    置。
  16. 【請求項１６】前記アンビル組立体13、16が、ハウジン
    グ50、51と、前記シャフト13に設けた凹所56と、前記ハ
    ウジング50、51を前記シャフト13にロックすべく少くと
    も一部が前記凹所56内に配置された前記ハウジング50、
    51のばね57と、前記ハウジング50、51内で移動できるよ
    うに取付けられておりかつ前記ばね57と衝合しているラ
    ッチ59であって、該ラッチ59の運動に応答して前記ばね
    57を前記凹所56から外して解放位置まで押圧し、前記ア
    ンビル組立体のハウジング50、51を前記シャフト13から
    引抜くことを可能にするラッチ59とを備えていることを
    特徴とする請求項（９）に記載の外科用ステープラ装
    置。
  17. 【請求項１７】シャフト13と、該シャフトの一端に摺動
    自在に取付けられておりかつ一方の端面に環状に配置さ
    れたバケット49が設けられているアンビル16と、該アン
    ビル16を前記シャフト13に解放自在に固定するロック手
    段57とを有していることを特徴とする外科用ステープラ
    のアンビル組立体。
  18. 【請求項１８】前記アンビル16がハウジング50、51を備
    えており、前記ロック手段が、前記シャフト13に設けた
    凹所56と、前記ハウジング50、51を前記シャフト13にロ
    ックすべく少くとも一部が前記凹所56内に配置された前
    記ハウジング50、51のばね57と、前記ハウジング50、51
    内で移動できるように取付けられておりかつ前記ばね57
    と衝合しているラッチ59であって、該ラッチ59の運動に
    応答して前記ばね57を前記凹所56から外して解放位置ま
    で押圧し、前記アンビル組立体のハウジング50、51を前
    記シャフト13から引抜くことを可能にするラッチ59とを
    備えていることを特徴とする請求項（17）に記載の外科
    用ステープラのアンビル組立体。
  19. 【請求項１９】前記シャフト13には複数の外スプライン
    47が設けてありかつ前記ハウジング50、51には複数の内
    スプライン53が設けてあり、これらのスプライン47、53
    は互に係合して、前記バケット49の配置を複数の位置の
    うちの１つに位置決めすべく前記シャフト13上で前記ハ
    ウジング50、51を整合させることを特徴とする請求項
    （18）に記載の外科用ステープラのアンビル組立体。
  20. 【請求項２０】前記凹所56が環状の凹所であり、前記ば
    ね57が、前記凹所56の両側で該凹所56内に配置される１
    対の脚58を備えたＵ字形をなしてあり、前記ラッチ59は
    前記脚58の中央で脚58に当接して該脚58を前記凹所56か
    ら外れるように押し拡げて、前記アンビル組立体のハウ
    ジング50、51を前記シャフト13から解放できるように構
    成したことを特徴とする請求項（19）に記載の外科用ス
    テープラのアンビル組立体。
