軽度、及び中等度~重度のADは、異なる症状及び経過を有する別のもののようである。疫学的研究によると、軽度のADはADの最も一般的なタイプであり、米国の1200万人のAD患者のうち約60%が罹患している。「軽度」の分類にもかかわらず、このより軽度のADの形態は、顕著で場合により重症の掻痒に特徴付けられる。重度のADと比較すると、軽度のADは病変が少なく、軽度の紅斑及び最小限の硬結/丘疹又は滲出/痂皮を呈し、これらは典型的には寛解、再発を繰り返す。IGAスコアが1又は2の患者は軽度のADを有するとみなされることがある。軽度のADは現在、皮膚衛生、局所皮膚軟化剤、抗ヒスタミン剤、局所ステロイド、カルシニューリン阻害剤、及び、クリサボロールなどのcAMP特異的3’,5’-環状ホスホジエステラーゼ4(PDE4)阻害剤により治療されている。しかしながら、軽度のAD患者における重症となりうる掻痒の治療には、満たされていない重要なニーズが未だ存在する。
重度のADは、顕著な硬結/丘疹又は滲出/痂皮を伴い慢性的な経過をたどるより多数の紅斑性病変を呈する。IGAスコアが3又は4の患者は、中等度~重度のADを有するとみなされる場合がある。重度のADは、局所レジメン、光線療法、及び免疫調節薬で治療されることがある。
軽度のADに伴う顕著な掻痒、関連する引っ掻きによる病変の悪化、及び睡眠障害は、未だ満たされていない重要な医療ニーズの典型である。強力かつ迅速な作用を有する忍容性のある全身性の鎮痒剤は患者に大きな価値を提供し、治療アルゴリズムにおいて、主要な位置を占める可能性が高いが、AAD治療ガイドラインでは現状中等度~重度のADであって局所レジメンや光線療法では疾患を適切にコントロールできないことがわかった場合にしか免疫調節薬が示唆及び推奨されていない。
掻痒感は、少なくとも部分的には、複数の皮膚細胞上に発現するニューロキニン-1(NK-1)受容体での結合を介して、内因性神経ペプチドであるサブスタンスP(SP)の作用により誘発される可能性がある。NK-1受容体(NK-1R)は、身体の様々な組織で発現し、主な活性は神経組織中で見られる。神経組織中でのSPとNK-1Rの相互作用は、局所的に神経原性炎症を制御し、また中枢神経系を介して痛覚経路を制御する。内皮細胞や免疫細胞等のその他の組織も、SP及びNK-1Rの活性を発現する。天然リガンドSPによるNK-1Rの活性化は、痛覚、行動ストレス要因、欲求、並びに嘔気及び嘔吐のプロセス等の数多くの生理的プロセスに関与している。神経組織又は末梢部でSPが不適切に過剰発現すると、物質依存、不安、嘔気/嘔吐、及び掻痒等の病態をひきおこす可能性がある。NK-1Rアンタゴニストは、このようなNK-1Rの過剰刺激を低減する能力を有する可能性があり、その結果、これらの病態における症状の根底にある病態生理に対処できる可能性がある。NK-1アンタゴニストとしては、とりわけ、アプレピタント、カソピタント、エズロピタント、ホスアプレピタント、ネツピタント、ロラピタント、セルロピタント、トラジピタント、ベスピタント(Vestipitant)、及びヴォフォピタント(Vofopitant)が含まれる。

トラジピタントは生物学的に利用可能な様々な投与経路で治療的に投与されることが知られている。米国特許第7,320,994号では、例えば経口、吸入、皮下、筋肉内、静脈内、経皮、経鼻、経直腸、経眼、局所、舌下、口腔等の、経口及び非経口のトラジピタントの投与が開示されており、治療には一般的に経口投与が好まれる。加えて、国際公開第2016/141341号には、掻痒及びアトピー性皮膚炎の治療のためのトラジピタントの使用が開示されている。トラジピタントは安全であり忍容性に優れることがわかっている。
患者を医薬で治療する場合、通常、患者が治療に応答する可能性と処方薬が提供しうる治療的恩恵の程度を医師が判断する必要がある。処方の判断には、患者の病歴や患者の症状の重症度などいくつかの要因が関係している可能性がある。一部の医薬では、特定の診断検査によって、特定の医薬から恩恵を受けることが期待できる患者を特定するための基礎が提供されうる。このようなコンパニオン診断検査は、当該検査が有意な治療応答の見込みをほとんど又はまったく示さない場合、患者を医薬による治療から除外することに役立つ場合がある。この点について、米国食品医薬品庁のガイダンスでは、コンパニオン診断が特定の治療薬から恩恵を受けやすい患者を特定しうることが述べられている(https://www.fda.gov/medicaldevices/productsand medicalprocedures/invitrodiagnostics/ucm407297.htm)。
本発明の一態様では、アトピー性皮膚炎(AD)患者の治療において最適な治療応答を達成する可能性を高めるための方法が提供される。様々な実施形態において、前記患者は、アトピー性皮膚炎を治療するのに有効な量のNK-1アンタゴニストを投与することが治療の選択肢となり得る患者でありうる。特に、前記NK-1アンタゴニストはトラジピタント又はその医薬的に許容可能な塩でありえ、特に、米国特許第7,381,826号明細書に記載されるように、IV型又はV型トラジピタントでありうる。
前記方法は、患者のAD疾患の重症度を評価するのに十分な方法で前記患者を検査することを含み、ここで、軽度のADは、とりわけ、IGAスコアで1又は2、及び/又は、BSAスコアで10%以下であることにより示される。IGAスコアで3又は4、及び/又はBSAスコアで10%超であることは、非軽度のAD、すなわち中等度~重度のADを示す。患者が、とりわけIGAスコア及び/又はBSAスコアに基づいて軽度のADを有すると判断される場合、前記方法は、前記患者が使用するための、トラジピタント等のNK-1アンタゴニストを、前記患者のADを治療するのに有効な量で処方することを含む。その後、当該治療有効量のトラジピタントは前記患者に投与される。前記患者のADを治療するのに有効な量は、100~400mg/日、150~400mg/日、100~300mg/日、150~300mg/日、100~200mg/日、170~340mg/日、又は170~255mg/日のトラジピタントでありうる。いくつかの実施形態において、前記量は170mg/日のトラジピタントであってもよく、85mgを1日2回(bid)として投与されてもよい。前記患者が、とりわけ、前記患者のIGAスコア又はBSAスコアに基づいて、NK-1アンタゴニスト、例えばトラジピタントを処方されない場合、前記方法は、(1)NK-1アンタゴニスト、例えばトラジピタント以外の代替の薬物療法、又は(2)NK-1アンタゴニスト、例えばトラジピタントの使用、のいずれが、それでもなおその患者を治療するためのより適切な医学的介入であるかを評価することを含む。

本発明の様々な実施形態において、本開示に記載される方法は、アトピー性皮膚炎(AD)に罹患する患者の治療において最適な治療応答を達成する可能性を高めるための方法を含み、特に、前記患者が、治療有効量のNK-1アンタゴニストの投与がADを治療するための治療の選択肢となりうる患者である場合の前記方法を含む。NK-1アンタゴニストには、例えば、アプレピタント、カソピタント、エズロピタント、ホスアプレピタント、ネツピタント、ロラピタント、セルロピタント、トラジピタント、ベスピタント(Vestipitant)、及びヴォフォピタント(Vofopitant)が含まれることが当技術分野で知られている。本開示に記載されるいくつかの実施形態及び実施例では、前記NK-1アンタゴニストは特にトラジピタントでありうる。しかしながら、当業者は、トラジピタント以外のNK-1アンタゴニストで説明される方法で患者を処置することにより患者の掻痒及び/又はADを処置してもよいことは理解される。
そのようなAD患者の治療において最適な治療応答を達成する可能性を高めるための方法のひとつにおいて、提供者(provider)はまず1又は複数の病変を伴う掻痒が報告された患者を特定する。本開示で使用される「提供者」とは、医療提供者(例えば医師、他の医療従事者、医薬処方資格者等)を意味しうる。提供者は、患者の臨床症状を検査又は評価してADの診断及びこれに伴う掻痒を確認し、掻痒及び根源となるADの重症度を判断しうる。
特に、提供者は、皮膚病変の外観及び範囲に基づいてADのタイプを識別し、多数の診断ツール及び基準のいずれかを使用して病変の物理的外観を特徴付けうる。かかる診断ツール及び基準としては、当技術分野で知られるように、例えば、医師による全般的評価(IGA;Investigator’s Global Assessment)、かゆみのための100mm単位の視覚アナログ尺度(VAS;Visual Analog Scale)、口頭式評価スケール(VRS;Verbal Rating Scale)、皮膚科関連QOL評価指標(DLQI;Dermatology Life Quality Index)、臨床全般印象度-変化(CGI-C;Clinical Global Impression of Change)、患者利益指数(PBI;Patient Benefit Index)、主観的又は客観的SCORAD(SCORing Atopic Dermatitis Index)、SKINDEX-16、湿疹面積・重症度指数(EASI;Eczema Area and Severity Index)、かゆみ及びADの両方に対する、変化に対する患者の全体的印象(PGIC;Patient Global Impression of Change)スケール、及び症状の重症度やアトピー性皮膚炎の重症度の他の公知の基準が挙げられる。提供者はさらに、ADに影響を受ける百分率で表される患者の体表面積(BSA;body surface area)を評価及び決定しうる。これらの因子は患者が軽度、中等度、又は重度のADを有するかどうかを判断するために用いうる。
特に、IGA、BSA、又はIGA及びBSAは、AD疾患の重症度を決定するために用いうる。IGAスコアが1又は2である患者、BSAが10%以下である患者、又はIGAが1又は2であってBSAが10%以下である患者は、軽度のADを有すると特定されうる。IGAスコアが3又は4である患者、BSAが10%超である患者、又はIGAスコアが3又は4であってBSAが10%超である患者は、中等度~重度のADを有すると判断されうる。
他の実施形態において、その他の評価も、単独又は互いに組み合わせて、又はIGA、BSA、若しくは前述のもののいずれかの組み合わせとともに用いてADの重症度を判断及び/又は確認しうる。特に、提供者は生化学的マーカー及び遺伝的マーカーを評価して患者が軽度のADに罹患している又は罹患の傾向があるかどうかを判断しうる。例えば、ひとつの実施形態では、提供者は患者の好酸球血球数を評価するために診断検査を実施するか、実施を要請しうる。これは例えば全血球計算(CBC)、白血球分画、又は絶対好酸球数の実行により行いうる。上記はいずれも、治療される患者から採取された生化学的サンプル、例えば血液サンプルに対して実行されうる。正常範囲内の好酸球数は軽度ADと関連しうる(図2C)。
他の実施形態において、提供者は、治療される患者から採取した生化学的サンプルに対して遺伝子型アッセイを行って、遺伝的変異型、例えばフィラグリン(FLG)遺伝子の遺伝的変異型を特定しうる。FLGはS100融合型タンパク質(SFTPファミリー;S100 fused type protein)のメンバーであり、1番染色体のq21に位置する。FLGは、皮膚バリア構造タンパク質であるフィラグリンタンパク質をコードする。前記生物学的サンプルは、当技術分野で公知のように、例えば、血液、血清、唾液、尿などを含みうる。患者のFLG遺伝子における変異の蓄積は、患者における非軽度の、例えば中等度~重度のAD疾患の重症度に関連し、患者のFLG遺伝子における変異の非蓄積は、患者における軽度のAD疾患に関連する(図2D)。
前述の重症度の徴候、例えば、IGAスコアが1若しくは2、又はBSAスコアが10%以下といった兆候、及び、比較的低い好酸球数、FLG遺伝子型、及び他の基準などの関連するバイオマーカーを使用して、患者を、軽度のAD、又は非軽度(すなわち中等度~重度)のADに罹患していると特定できる。本開示の実施例に記載されるように、軽度のADに罹患している患者は非軽度のADに罹患している患者よりもトラジピタントの治療に対する応答を経験する可能性が高い。
患者が、IGAスコア、BSAスコア、及び/又は本開示に記載される他の徴候に基づいて、軽度のADに関連する掻痒を患っていると判断された場合、NK-1アンタゴニスト、例えばトラジピタントが、患者による使用のために、前記患者のアトピー性皮膚炎を治療するのに有効な量で処方されうる。様々な実施形態において、患者の掻痒又はアトピー性皮膚炎を治療するのに有効な量は、例えば、100~400mg/日、150~400mg/日、100~300mg/日、150~300mg/日、100~200mg/日、170~340mg/日、又は170~255mg/日でありうる。いくつかの実施形態では、前記量は170mg/日でありえ、85mgを2回(bid)として投与されうる。前記NK-1アンタゴニスト、例えばトラジピタントは、続いて、患者の掻痒及び/又は基礎となるADを治療するために患者にそのような量で投与されうる。鎮痒効果はトラジピタント投与の最初の丸一日の直後に現れ得、夜間の睡眠において顕著な改善が現れうる。
前記兆候、例えばIGAスコアやBSAスコアに基づいて、患者がNK-1アンタゴニスト、例えばトラジピタントを処方されない場合、すなわち患者が軽度ADに罹患していると診断されない場合、提供者はその後(1)NK-1アンタゴニスト、例えばトラジピタント以外の代替の薬物療法、又は(2)NK-1アンタゴニスト、例えばトラジピタントの使用のいずれがその特定の患者を治療するためのより適切な医学的介入であるかを評価しうる。(1)の場合、より適切な治療選択肢が当技術分野で知られるものから選択される。そのような代替法は、https://www.mayoclinic.org/diseases-conditions/atopic-dermatitis-eczema/diagnosis-treatment/drc-20353279に見出すことができ、局所薬(例えば、コルチコステロイドクリーム若しくは軟膏、カルシニューリン阻害剤、又は抗菌クリーム若しくは軟膏)、及び全身薬(例えば、経口コルチコステロイド、例えばプレドニゾン、又は注射可能な製剤、例えばデュピルマブ)を含みうる。
上記判断に応じて、代替の薬物療法が施行されるか、NK-1アンタゴニスト、例えばトラジピタントが投与されうる。患者にトラジピタントが投与されるが、IGAスコア及び/又はBSAスコアに基づくものではない場合、トラジピタントによる治療に応答を示すAD患者はトラジピタント投与の最初の丸一日の直後にでも応答しうるため、患者は治療に対する応答についてより綿密にモニタリングされうる。提供者は少なくとも投与後この時間枠で患者をモニタリングし、トラジピタントにより満足のいく治療応答が得られるかどうかを評価しうる。
当業者は、上記の実施形態を組み合わせることによって、又は本開示に記載される例を参照することによって、さらなる実施形態を選択しうることを理解するであろう。
〔実施例:EPIONE試験〕
医師による全般的評価尺度(IGA)の決定による軽度(23%)から中等度(64%)及び重度(13%)までの疾患重症度の範囲の、重症の掻痒を伴うアトピー性皮膚炎(AD)患者を対象として、無作為化プラセボ対照第III相試験が行われる。この試験では、患者(n=341)は1:1で無作為化され、8週間の治療期間中、トラジピタント又はプラセボのいずれかの投与を受ける。試験のトラジピタント群の患者は、トラジピタント85mgを1日2回投与する。患者は、ベースライン時及び無作為化後に、一定の間隔で複数の症状及び疾患重症度スケールで評価される。

8週目に、試験のトラジピタント群とプラセボ群の患者はいずれも、最悪のかゆみの数値評価尺度(WI-NRS)で測定される掻痒の有意かつ意義のある改善を示している。治療群間の差は統計的に有意ではないが、トラジピタントの改善の大きさはプラセボに優っている。
医師による全般的評価尺度(IGA)によりスケール1~4で疾患の重症度を測定すると、ベースラインの疾患重症度と治療との間に有意な相互作用が観察される(p=0.0004)。このことは、ベースラインの疾患重症度が異なる試験参加者は、異なる治療成績を経験することを示唆している。
ベースラインの疾患重症度と治療の相互作用を考慮すると、トラジピタントではプラセボと比較して、全試験集団において、事前に指定された8週目の最終時点において、WI-NRSの有意に大きな改善がみられた(p=0.0217)。2週目、4週目、6週目、及び8週目を含む、無作為化後の全ての来院において、治療期間を通じて同様の効果が見られる(表1)。
サブグループ分析は、軽度の疾患重症度の患者(試験患者の23%、IGA1、2)はプラセボに対して最大の改善を経験することを示している。具体的には、軽度のAD群において、トラジピタントは、すべての来院において、プラセボに対してWI-NRSを有意に改善する(表1、図1)。カテゴリ別のWI-NRS応答者分析(4ポイント超の改善)は、プラセボ患者の33.3%と比較して、トラジピタント患者の72.5%が臨床的に意義のある応答を有したことを示した。
これらの結果は、軽度のADにおけるトラジピタントの大きくかつ有意な鎮痒効果を示唆しており、これは患者の毎日の日記(daily diary)の記入と一致している。軽度のAD患者の場合、応答の時間経過は、トラジピタント投与の最初の丸一日の直後に鎮痒効果が見られることも示しており、大きくかつ迅速な治療効果を示唆している。重度の掻痒の患者ではしばしば中断される夜間の睡眠にも同様の改善が見られる。
さらに、結果は、軽度及び非軽度(例えば、重度)のADは、異なる原因因子及び経過のセットを伴う異なるエンドタイプでありそうであることを示唆している。図2Cに示すように、非軽度、すなわち中等度~重度のAD(IGA3、4)は、軽度のAD(IGA1、2)と比較して、血液中の好酸球数の顕著な増加を伴う。好酸球は炎症反応のメディエーターであり、病変に他の免疫細胞を動員することに寄与している。好酸球は、ヒスタミン、セロトニン、サブスタンスP、NGF、及び多数のインターロイキンサイトカインを含む起掻性及び炎症性メディエーターの産生及び分泌に直接的又は間接的に関与している。そのため、軽度及び重度のADに関連する重症の掻痒は、異なるメディエーターを有する可能性がある(図3A~3D)。
さらに、全ゲノム配列分析(WGAS;whole genome sequence analysis)は、軽度のADと非軽度(すなわち、中等度~重度)のADには特定の遺伝子マーカーが関連していることを示している。例えば、患者のフィラグリン(FLG)遺伝子におけるまれな機能喪失(LOF;loss of function)変異の蓄積は、患者の非軽度(例えば、中等度~重度)のAD疾患重症度と関連している一方、患者のFLG遺伝子の変異の非蓄積は、患者の軽度のAD疾患に関連している(図2D)。
トラジピタントは、軽度のADにおいて大きくかつ迅速な鎮痒効果をもたらすようであり、比較的安全なプロファイルを有しながら、治療の最初の丸一日までに掻痒の顕著かつ迅速な軽減をもたらす。これは、軽度のAD病変の重症度を経験しているが、依然として重大な掻痒を患っている大多数のAD患者に切望されている治療法を提供する可能性がある.
本発明の様々な態様に関する上記説明は、例示及び説明の目的で提示されたものである。これは、網羅的であること又は開示された正確な形態に本発明を限定することを意図するものではなく、修正及び変形が可能である。当業者に明らかでありうるそのような修正及び変形は、添付の特許請求の範囲によって定義される本発明の範囲内に含まれることが意図されている。
Claims (14)
- アトピー性皮膚炎を治療するのに有効な量のトラジピタントを投与することが治療の選択肢となり得るアトピー性皮膚炎患者の治療において最適な治療応答を達成する可能性を高めるための方法であって、
前記患者の医師による全般的評価(IGA;Investigator’s Global Assessment)スコア及び体表面積(BSA;body surface area)スコアの一方又は両方を評価するのに十分な方法で前記患者を検査すること;及び
前記患者のIGAスコアが2以下である場合、又は前記患者のBSAスコアが10%以下である場合、又は前記患者のIGAスコア及びBSAスコアがそれぞれ2以下及び10%以下である場合、前記患者が使用するためのトラジピタントを、前記患者のアトピー性皮膚炎を治療するのに有効な量で処方することであって、その後、前記量のトラジピタントが前記患者に投与される、前記処方すること;及び
前記患者が前記IGAスコア又はBSAスコアに基づいてトラジピタントを処方されない場合、(1)トラジピタント以外の代替の薬物療法又は(2)トラジピタントの使用のいずれが、その患者を治療するためのより適切な医学的介入であるかを評価すること、

- 前記患者の掻痒又はアトピー性皮膚炎を治療するのに有効なトラジピタントの量が170mg/日である、請求項4に記載の方法。
- 前記患者の掻痒又はアトピー性皮膚炎を治療するのに有効なトラジピタントの量が、85mgを1日2回である、請求項5に記載の方法。
- アトピー性皮膚炎を治療するのに有効な量のNK-1アンタゴニストを投与することが治療の選択肢となり得るアトピー性皮膚炎患者の治療において最適な治療応答を達成する可能性を高めるための方法であって、
前記患者の医師による全般的評価(IGA)スコア及び体表面積(BSA)スコアの一方又は両方を評価するのに十分な方法で前記患者を検査すること;及び
