Description
本発明は、ヒト患者のHBV感染を処置する方法であって、治療有効量のコアタンパク質アロステリックモジュレータを投与することを含む方法に関する。
B型肝炎ウイルス(HBV)感染は、世界的に大きな公衆衛生上の問題であり、世界人口の約30%が、現在又は過去の感染の血清学的証拠を示している。1980年初期にウイルスに対する安全で効果的な予防ワクチンが導入されたにもかかわらず、世界中で依然として2億5700万人を超える慢性HBVキャリアが存在し、そのうち高い割合が最終的に肝硬変又は肝細胞癌腫(HCC)を発症すると推定されている(WHO Hepatitis B.Fact Sheet N°204)。2010年のGlobal Burden of Disease研究(R Lozano,et al.Lancet,380(2012)、2095-2128)では、HBV感染は世界で最も健康優先順位が高く、死因の第10位(年間78万人が死亡)であった。HBsAgの持続的クリアランスは、肝硬変、肝不全、及びHCCを含むHBV合併症の疾患の進行及び発症に対して保護的であることが示されている。したがって、HBsAg喪失(機能的治癒)は、慢性B型肝炎(CHB)患者の最適なエンドポイントと考えられている。機能的治癒は、抗HBsへのセロコンバージョンの有無にかかわらず、有限の処置経過の完了後の血清中の持続的で検出不能なHBsAg及びHBV DNAとして定義される(Lok AS,Zoulim F,Dusheiko G,et al.Hepatitis B cure:from discovery to regulatory approval.J Hepatol 2017;67:847-861)。
現在、CHBの処置に利用可能な2つの治療クラスが存在する:皮下投与されるインターフェロン-アルファ(IFN-α)調製物(従来のもの、又はPEG-IFN-α)及び経口投与されるヌクレオシ(チ)ドアナログ(NUC)、例えばテノホビル、エンテカビル、アデホビル、テルビブジン及びラミブジン。どちらの処置も、高い割合の機能的治癒(セロコンバージョンを伴う又は伴わないHBsAgの持続的喪失)を達成しない。さらに、IFNに基づく治療は多くの副作用に関連するが、NUCはしばしば長期間又は場合によっては生涯にわたる治療を必要とし、いくつかはウイルス耐性の高いリスクに関連する。
最近、HBV感染患者の処置のためにCpAM(コアタンパク質アロステリックモジュレータ)が開発された。化合物(I)は、国際公開第2015132276号に開示されている、HBVカプシドを標的とすることによるヒトにおける処置及び/又は予防のためのCpAMであり、その構造を以下に示す:
化合物(I)の安全性、耐容性、薬物動態(PK)及び薬力学(PD)を特徴づけるために、健康なボランティア及びCHB患者の両方における3つの臨床試験(YP39364(NCT02952924)、YP39406(NCT03570658)及びYP40218(NCT03717064)のパート1及び2)が完了した。短期PDマーカー(HBV DNA及びHBV RNA)は、化合物(I)を用いた4週間の処置中に堅固な減少を示したが、HBsAgの有意な減少はそのような期間では観察されなかった。したがって、有限の処置期間及び優れた耐容性でより高い機能的治癒率を達成することができる化合物(I)による新規な処置方法に対する満たされていない医学的必要性がある。

本発明では、12~144週間の、化合物(I)単独療法及び他の抗HBV剤と組み合わせた化合物(I)の方法を開発した。化合物(I)は、慢性B型肝炎患者において安全で忍容性が高い。現在のNUC処置と比較して、本発明の現在の有限処置方法は、薬物耐性のリスクを最小限に抑え、NUC処置の有害作用を減少させる(例えば、腎損傷、骨密度損失及びクレアチンキナーゼ上昇のリスクなど)。
有効性に関して、本発明の処置方法は、患者における堅固なHBV DNA及びHBV RNAの減少を実証した。さらに、他の抗HBV剤と組み合わせた化合物(I)の12~144週間の処置方法は、B型肝炎表面抗原(HBsAg)の著しい減少又は喪失をもたらす。さらに、現在のところ、免疫耐性患者(高度に複製性(HBV DNAレベルが上昇している)及び低炎症性(ALTレベルが正常であるか、又は肝生検による有意な炎症若しくは線維症の徴候がないと定義される)の患者として定義される)に対する有効な抗ウイルス処置はない。しかしながら、化合物(I)を用いた本発明の処置方法はまた、そのような患者において堅固なHBV DNA及びHBV RNAの減少を誘発することができる。
定義
「NUC」という用語は、HBV治療として使用されるヌクレオシ(チ)ド類似体を表し、それには、ラミブジン、アデホビル・ジピボキシル、エンテカビル(ETV)、テルビブジン、テノホビル・ジソプロキシル・フマル酸塩(TDF)及びテノホビル・アラフェナミド(TAF)が含まれるが、これらに限定されない。

「絶食」という用語は、次の食事の前又は最後の食事の後の12時間以内の任意の時間であり得る期間にわたる食事なしの投与を表す。
「食物」という用語は、限定はしないが、高脂肪食、軽食又は標準食を含む任意の種類の食物を表す。
「IFN-α」という用語は、従来のインターフェロン-α又はペグ化インターフェロン-α(PEG-IFN-α)を表す。IFN-αの例には、Pegasys(登録商標)(Roche)及びPEG-Intron(登録商標)(Schering Corporation)が含まれるが、これらに限定されない。
「皮下注射」又は「SC」という用語は、皮膚と筋肉との間の患者の組織層に短い針を介して薬物が送達され、皮下用量が任意の適切な部位、例えば腹部又は大腿上部に投与されることを意味する。複数回投与の場合、投与部位を交代させるべきである。
「静脈内注射」又は「IV」という用語は、針又はチューブを使用して薬物が患者の静脈内に直接送達されることを意味する。
用語「TLRアゴニスト」は、ナチュラルキラー細胞、B細胞及びT細胞の抗ウイルスサイトカイン産生及び活性化を増強することができるToll様受容体アゴニストのクラスを意味し、これらの免疫応答は、CHBの機能的治癒の達成において役割を果たし得る(すなわち、HBsAg損失)。TLRアゴニストの例には、TLR7アゴニストGS-9620(Gilead Sciences Incs)、TLR8アゴニストGS-9688(Gilead Sciences Incs)、TLR7アゴニストJNJ-64794964(Janssen Research&Development LLC)及びTLR9モジュレータAIC649(AiCuris Anti-infective Cures GmbH)が含まれるが、これらに限定されない。

「低分子干渉RNA」又は「siRNA」という用語は、二本鎖RNA分子のクラスである低分子干渉リボ核酸を表す。siRNAは、任意のウイルス転写物を標的とし、RISC/Ago 2複合体によるその分解を誘導し、遺伝子サイレンシングをもたらす。siRNAの例としては、ARB-1467(Arbutus Biopharma)、ARO-HBV(Arrowhead Pharma)、AB-729(Arbutus Biopharma)、DCR-HBVS(Dicerna)、Vir-2218(Alnylam and Vir Biotech)、BB-103(Benitec)及びLunar-HBV(Arcturus,USA with Janssen)が挙げられるが、これらに限定されない。
「HBV LNA」という用語は、B型肝炎ウイルス(HBV)ゲノム由来mRNA種に相補的な、CHB感染の処置を目的とした、N-アセチルガラクトサミン(GalNAc)標的化ロックド核酸(LNA)含有一本鎖オリゴデオキシリボヌクレオチドである分子を表す。
「HBsAg阻害剤」という用語は、HBsAgの集合/放出、産生又は侵入を妨げることができる分子を表す。HBsAg阻害剤の例としては、REP 2139(Replicor)及びREP 2165(Replicor)、並びにMyrcludex B(MYR Pharma)が挙げられるが、これらに限定されない。
「HBV治療ワクチン」又は「HBVワクチン」という用語は、宿主免疫応答を刺激又は増強して免疫制御を回復させ、HBV複製の持続的抑制及び最終的にはHBsAgの喪失をもたらすことができる分子を表す。HBVワクチンの例には、ABX203(Center for Genetic Engineering and Biotechnology)、INO-1800(Inovio)、HB-110(Ichor Medical Systems with Janssen)、TG1050(Transgene)及びHepTcell(Altimmune)が含まれるが、これらに限定されない。

処置の組成物

本発明の化合物は、任意の便利な投与形態、例えば錠剤、粉末、カプセル、溶液、分散液、懸濁液、シロップ剤、スプレー、座薬、ゲル、エマルション、貼付剤などで、投与されることができる。このような組成物は、薬学的調製物において従来的である構成成分、例えば、希釈剤、担体、pH調整剤、甘味料、充填剤、及びさらなる活性剤を含有することができる。

処置方法
本発明は、(i)ヒト患者におけるHBV感染を処置する方法であって、該患者に、化合物(I)の活性成分を200mg QD又はBID~1000mg QD又はBIDの量で含む医薬組成物を、他の抗HBV剤とともに又は伴わずに、12~144週間投与することを含む方法に関し、化合物(I)は、3-[(8aS)-7-[[(4S)-5-エトキシカルボニル-4-(3-フルオロ-2-メチル-フェニル)-2-チアゾール-2-イル-1,4-ジヒドロピリミジン-6-イル]メチル]-3-オキソ-5,6,8,8a-テトラヒドロ-1H-イミダゾ[1,5-a]ピラジン-2-イル]-15 2,2-ジメチル-プロパン酸である。

本発明のさらなる実施形態は、(vii)化合物(I)が、1又は2種の他の抗HBV剤とともに投与され、他の抗HBV剤が、NUC、IFN-α、TLRアゴニスト、RIG-Iモジュレータ、siRNA、HBVLNAオリゴヌクレオチド、HBsAg阻害剤及びHBVワクチンから独立して選択される、(i)から(vi)のいずれか1つに記載の方法。他の抗HBV剤の処置方法は、既存の治療基準に従って適用することができる。
