いくつかの実施形態では、リンパ球枯渇(LD)レジメンは、BCMA CAR-T細胞の初回のおよび/または後続の投与の前に対象に実施される。いくつかの実施形態では、リンパ球枯渇レジメンは、CAR-T細胞の初回のおよび/または後続の投与と同時に対象に実施される。いくつかの実施形態では、リンパ球枯渇レジメンは、BCMA CAR-T細胞の初回のおよび/または後続の投与の前、最中、および/または後に実施される。
いくつかの実施形態では、本開示の同種異系BCMA CAR-T細胞は、均一用量を使用して投与される。他の実施形態では、同種異系BCMA CAR-T細胞は、用量バンディングを使用して投与される。例えば、広範囲のCAR-T細胞曝露のリスクを回避するために用量バンディングを使用してもよい。いくつかの実施形態では、重量バンドを使用することができる。例えば、限定されないが、66kg未満の対象はX用量で投与されてもよく、66kg以上の対象は約1.33Xの用量で投与されてもよい。いくつかの実施形態では、50kg以上である対象に1回の用量を投与することができ、50kg以下の対象は異なる用量で投与されてもよい。
BCMA CAR-T細胞による再治療(再投与)のための方法も本明細書に提供される。特に、この方法は、初回用量を受けた対象に細胞の1回以上の後続の用量を投与すること、および/または初回および1回以上の後続の用量を投与することを含む。用量は、一般に、特定の量で、特定のタイミングパラメータに従って投与される。いくつかの実施形態では、方法は、一般に、細胞の初回用量を投与することを含み、それにより、疾患負荷を低減し、その後、細胞の後続の用量が、初回用量に関して特定の時間枠の間に投与されるか、またはそのような初回用量を受けた対象に後続の用量の投与を続ける。いくつかの実施形態では、その後、追加の後続の用量が、例えば、後続の用量に関して同じまたは同様の時間枠内で投与される。いくつかの実施形態では、投与される細胞の数および複数回投与のタイミングは、対象に対する毒性の可能性または程度を低減したり、投与された細胞に対する対象の曝露および/もしくは投与された細胞の持続性を改善したり、かつ/または治療効果を改善するなど、1つ以上の結果を改善するように設計される。細胞を含み、そのような投与レジメンに従った投与用に設計された製品も提供される。
本開示はまた、開示された方法で使用するためのキットおよび製品を提供する。本開示のキットは、BCMA特異的CARをコードするポリヌクレオチド、または本明細書に記載のBCMA特異的CARをコードするポリヌクレオチドを含む操作された免疫細胞(例えば、BCMA-1 CAR-T細胞、例えば、BCMA-CAR+_TCRαβ-_CD52+/-T細胞)を含む1つ以上の容器(例えば、ガラスバイアル)、および本明細書に記載の開示の方法のいずれかに従って使用するための指示書を含む。いくつかの実施形態では、操作された免疫細胞は、約5%のDMSOを含む溶液中に処方される。さらに、操作された免疫細胞は、凍結状態で提供することができる。
図1~16は、BCMA-1の生成および試験を示す。BCMA-1は、BCMA CARを発現する、統合された自己不活化第3世代の組換えレンチウイルスベクターを含む同種異系T細胞である。BCMA CARは、scFvを含み、CARのscFVは、表1のP5A2である。scFVは、VHおよびVLを含み、VHは、配列番号33に示されるアミノ酸配列を含み、VLは、配列番号34に示されるアミノ酸配列を含む。CARの細胞外領域はまた、リツキシマブによる認識を付与する2つのミモトープを含む。
図19は、難治性/再発性MMの治療のための第一相試験(デザインA)の概要を示す。デザインAの設計には、フルダラビン(flu)、30mg/m2/日のIV;シクロホスファミド(cy)、300mg/m2/日のIV;およびCD52抗体、治療の3~5日前に13mg/日のIV、であるリンパ球枯渇段階と、20~80×10^6細胞IV(50kg以上の対象の場合)または7~360×10^6細胞IV(50kg未満の対象の場合)の漸増用量を含む治療段階(0日目)と、が含まれる。

第二相は、第一相デザインAまたはデザインBからの許容可能な毒性を伴う最も高い用量(RP2D-第一相からの用量制限毒性の約20%を生じる用量レベル、またはRP2D用量を超える用量レベルのいずれか)を用いて、6~12人の対象からなるさらなるコホートを試験することを含むことができる。対象は、flu/cyなしでCD52抗体を受けることができる。CD52抗体は、CAR-T細胞治療の前に、約40mg(13mg/日×3日)の用量で投与することができ、CAR-T細胞治療後7、14、および21日目に13mg/日で繰り返すことができる。
