Abstract
Description
HCCは、現在世界の癌による死因の第3位であり、500,000人以上が冒されている。肝細胞癌、とりわけ進行したか、または再発肝細胞癌の場合の治療選択肢は限られている。手術および放射線治療は初期のステージの肝癌の選択肢であるが、進行したか、または再発肝細胞癌にはあまり効果的ではない。体系的な化学療法は特に効果的であるわけではなく、使用可能な薬物の数は非常に限られている。
複数の実施形態において、本開示は肝細胞癌を含む肝癌を患う対象を治療する方法を提供し、本方法は1つ以上のバイオ拡散チャンバを対象に移植することを含み、1つ以上のバイオ拡散チャンバは照射された腫瘍細胞、照射されたインスリン様成長因子1受容体のアンチセンスオリゴデオキシヌクレオチド(IGF-1R AS ODN)を含み、その腫瘍細胞は切除部位において熱を発生しない組織モルセレータを使用して対象の切除部位から除去される。
本明細書に定義されない全ての用語は、その共通の技術認識されている意味を有する。

2つの主な肝癌のタイプがある;悪性肝癌としても知られる肝細胞癌(HCC)、および胆管細胞癌。
アンチセンス分子は、ワトソンおよびクリック塩基対合則によってmRNAの標的相補的配列に結合することによって機能する核酸である。標的mRNAの翻訳は、相補的ならせん間でハイブリダイゼーションが起こる場合、能動的および/または受動的な機序によって抑制される。受動的な機序において、mRNAおよび外因性のヌクレオチド配列との間のハイブリダイゼーションは、リボソーム複合体がそのメッセージを読むことを防ぐ2本鎖形成につながる。能動的な機序において、ハイブリダイゼーションは、RNaseHの結合を促進し、RNaseHはRNAを破壊するがアンチセンスをそのままにし、別の相補的なmRNA標的とハイブリダイズする。いずれかの、または両方の機序により、悪性の表現型に寄与する、または持続するタンパク質の翻訳が抑制される。治療剤としては、アンチセンス分子は、はるかに選択的であり、結果として、従来の薬物に比べて、より効果的でより有毒でない。
ある実施形態において、アンチセンスは、IGF-1R(IGF-1R AS ODN)に対するデオキシヌクレオチドである。IGF-1Rの全長コーディング配列は、配列番号15として提供される(例えば、参照によってその全体が援用されるPCT/US2016/26970を参照)。

導入
免疫療法は、現在、1つの共通の細胞抗原を有する血液学的悪性腫瘍を標的にするために使用される。残念なことに、固形腫瘍ははるかに複雑であり、同定不能な数の腫瘍特異的な標的を有する悪性状態への遺伝的変化のエピジェネティックな進行を表す。さらに困難なことに、WHO診断の癌群内には、腫瘍表現型における顕著な変動が存在する。自家細胞ワクチンは、全てのそのような変動および全てのそのような標的を包含し得、固形腫瘍癌のための理想的な対象特異的な免疫療法になるであろう。しかしながら、連続的な継代は腫瘍表現型を変え、したがって一連の腫瘍特異的な抗原を減少させるので、自家癌細胞ワクチンは初代細胞培養から得ることはできない。これには、各継代において実現しがたいロットリリース認定も必要になる。本開示は、新たに切除され、細片化された腫瘍細胞をプレーティングし、デポ(depot)抗原として24時間以内にそれらを再移植することによってこれらの懸念を除去する。ある態様において、本明細書において達成された優れた結果は、本明細書に記載されている他の詳細の中で適切な数の細胞がチャンバ内に存在することを保証することによって得られる。
代表的な拡散チャンバは、第1端および第2端の2つの端を有するチャンババレルを含む。複数の実施形態において、バイオ拡散チャンバは、Millipore Corporationによって製造されるDuropore membraneなどの多孔性、細胞不透過性メンブレンによって両側がキャップされた小さなリングである。任意選択的に、両端のうちの一方は、チャンバ本体の部分として閉鎖され、片方の端だけを開いたままにして、多孔性メンブレンを使用して密閉され得る。メンブレンは、強くて可撓性があり、化学処理に耐えることができるプラスチック、テフロン(登録商標)、ポリエステル、または任意の不活性材料から作られ得る。チャンバは、限定するものではないが、プラスチック、テフロン(登録商標)、ルーサイト、チタン、プレキシガラスまたは、ヒトに対して無毒であり、十分に耐えられる不活性材料などの任意の物質から作られ得る。それに加えて、チャンバは、滅菌を耐えられるようにすべきである。いくつかの態様において、拡散チャンバは、使用前にエチレンオキシドで滅菌される。他の好適なチャンバは、2018年1月24日に出願された米国仮出願第62/621,295号明細書、米国特許第6,541,036号明細書、PCT/US16/26970、および、米国特許第5,714,170号明細書において記載され、それらは参照によってそれらの全体が本明細書に援用される。
自家ワクチン接種に使用される腫瘍細胞は、対象から外科的に除去される。複数の実施形態において、腫瘍細胞は、組織モルセレータを使用して患者から除去される。この摘出装置は、好ましくは静止外部カニューレ内に高速往復内部カニューレと電子制御可変吸引とを組み合わせる。外部カニューレは1.1mm、1.9mm、2.5mm、または3.0mmの直径を有し、10cm、13cm、または25cmの長さを有する。器具はまた、デシケーターの平滑末端から0.6mmに位置する、側面に開口する切除吸引用開口部に依存する。除去される組織への開口部の穏やかな前方圧力と吸引との組み合わせにより、所望の組織が側面開口部に引き込まれ、内部カニューレの相互切断作用によって制御された正確な組織の切除が可能になる。重要な特徴は、回転ブレードがないことである。これにより、意図しない組織が開口部に引き込まれることを防ぐ。好適な装置の一例は、Myriad(登録商標)組織アスピレータ(インディアナ州インディアナポリスのNICO Corporation(登録商標))であり、直接、顕微鏡、または内視鏡による視覚化での軟部組織の除去に使用され得る低侵襲手術システムである。削り取った組織を吸引し、回収チャンバ内に集め、滅菌組織トラップ内に回収する。滅菌組織トラップ内への組織の回収中、調製物から血液を除去する。好ましくは、滅菌トラップは、そのトラップの底部に回収ディッシュと、トラップにアクセスを提供するステムとを含む。トラップ構造はまた、トラップからの組織除去を容易にするための、トラップから取り外し可能な柄杓型内部構造を含み得る。

全身投与
チャンバの移植の代替として、または補足として、IGF-1R AS ODNが全身投与され得る。したがって、複数の実施形態において、IGF-1R AS ODNは全身投与のために医薬組成物中に提供される。IGF-1R AS ODNに加えて、医薬組成物は、例えば、生理食塩水(0.9%塩化ナトリウム)を含み得る。組成物はリン脂質を含み得る。いくつかの態様において、リン脂質は生理的pHで帯電していないか、中性の電荷を有する。いくつかの態様において、リン脂質は中性リン脂質である。ある態様において、中性リン脂質はホスファチジルコリンである。ある態様において、中性リン脂質は、ジオレオイルホスファチジルコリン(DOPC)である。いくつかの態様において、リン脂質は本質的にコレステロールを含まない。
ヒト対象におけるアンチセンスの全身投与のための投与量は、約0.025g/kg、約0.05g/kg、約0.1g/kg、約0.15g/kg、または約0.2g/kgであり得る。ある実施形態において、全身投与のための投与量は、0.025g/kg~0.2g/kgであり得る。いくつかの実施形態において、投与量は約0.2g/kgである。他の実施形態において、投与量は0.004g/kg~0.01g/kgである。他の実施形態において、投与量は0.01g/kg未満である。さらなる実施形態において、投与量は0.01g/kg~0.2g/kgではない。ある態様において、アンチセンスは凍結乾燥された粉末として供給され、投与前に再懸濁される。再懸濁されたとき、アンチセンスの濃度は約50mg/ml、約100mg/ml、約200mg/ml、約500mg/ml、約1000mg/ml、またはそれらの量の間の範囲であり得る。
歴史的には、癌療法は、放射線、化学療法、またはその両方を用いて対象を治療することを必要としていた。そのようなアプローチは文書による十分な裏付けのある課題を有する。しかしながら、有利には、本明細書に開示されるチャンバ移植方法は、単剤療法として癌を有する対象を治療するために使用され得る。したがって、本明細書に開示される方法が化学療法または放射線治療を含まないことが好ましい。しかしながら、本明細書の単剤療法アプローチによって達成される優れた効果にもかかわらず、ある状況下では当該チャンバ方法を例えば、放射線治療などの他の療法と組み合わせることは有益であり得る。ある実施形態において、放射線治療は、限定するものではないが、内部線源放射線治療、体外照射療法、および全身性ラジオアイソトープ放射線治療を含む。ある態様において、放射線治療は体外照射療法である。いくつかの実施形態において、体外照射療法は、限定するものではないが、ガンマ放射線治療、X線療法、強度変調放射線治療(IMRT)、および画像誘導放射線治療(IGRT)を含む。ある実施形態において、体外照射療法は、ガンマ放射線治療である。放射線は、チャンバ移植前、または移植後に、例えばサルベージ療法として投与され得る。通常、そのようなサルベージ療法アプローチは癌が再発したと確定されるまで実施されない。
好適な対象は癌を有する動物であり、通常、対象はヒトである。肝癌は、特に本明細書に記載されるこの方法から恩恵を受けるが、本方法は一般的に癌に適用する。したがって、本開示は肝細胞癌および胆管細胞癌からなる群から選択されるものを含む癌治療の方法を提供する。特定の実施形態において、肝癌は肝細胞癌である。いくつかの実施形態において、肝癌は転移性肝細胞癌である。いくつかの実施形態において、肝癌は再発肝細胞癌である。ある実施形態において、治療の候補である対象はWHOグレードII、WHOグレードIII、またはWHOグレードIVの腫瘍を患っている。例えば、ある実施形態において、腫瘍は、グレードIの肝細胞癌、グレードIIの肝細胞癌、グレードIIIの肝細胞癌、およびグレードIVの肝細胞癌から選択される。ある実施形態において、腫瘍は、ステージIの肝細胞癌、ステージIIの肝細胞癌、ステージIIIの肝細胞癌およびステージIVの肝細胞癌から選択される。
理論に縛られることなしに、AS ODNはIGF-1R発現を下方制御することにより、対象のM2細胞を減少し、および/または細胞のM2細胞への極性化を阻害すると考えられる。いくつかの実施形態において、M2細胞におけるIGF-1R発現は、アンチセンスで処理していない細胞と比較して、少なくとも約1%、少なくとも約2%、少なくとも約5%、少なくとも約10%、少なくとも約20%、少なくとも約30%、少なくとも約40%、少なくとも約50%、少なくとも約60%、少なくとも約70%、少なくとも約80%、少なくとも約90%、または少なくとも約95%下方制御される。M2細胞におけるIGF-1R発現は定量的RT-PCRによって測定され得る。
完成チャンバの調製は、多数の構成要素および多数のステップを必要とする。本開示の別の態様において、本明細書に開示される方法を実施するための構成要素を含むキットが提供される。ある態様において、キットは1つの部分、または2つの半分ずつであり得るチャンバ本体を含む。1つ以上のメンブレン、接着剤、および溶媒(例えば、アルコール、または2ジクロロエタン)を含むチャンバを密閉するためのアイテムも含まれ得る。任意選択的に、メンブレンは密閉を作るようにチャンバに超音波溶接され得る。キットはアンチセンスODNを含む。任意選択的に、ODNは2つの部分に分割され得る。対象から外科的に除去した後に細胞を処理するための第1の部分、および、対象に導入されたときに細胞と組み合わせるための第2の部分。他の任意選択的なキットアイテムは、細胞培養のための培地、および培地中での細菌の増殖を防ぐための抗生物質を含む。
実施例1
バイオ拡散チャンバの調製
腫瘍組織は、組織アスピレータ(NICO Myriad(登録商標))を使用して肝細胞癌患者から外科的に除去され、滅菌組織トラップ内に配置される。滅菌組織トラップは、その後、指定のBSL-2施設に移され、そこで腫瘍組織はプロセスされバイオ拡散チャンバ内に配置される。
ワクチン接種はマウス肝細胞癌モデルで抗腫瘍応答を誘導する
抗腫瘍応答がインビボで誘導されるか見るために、本明細書に記載されるバイオ拡散チャンバをマウスの肝細胞癌モデルで試験した。これらの実験において、Hepa1-6細胞を単独で、または100万個の細胞あたり4mgのIGF-1R AS ODNで一晩培養した。細胞を翌日に採取し、NOBEL(2μg)を含む各チャンバに100万個の細胞を追加した。PBS単独を含むチャンバ、Hepa1-6細胞を含むチャンバ、または、一晩NOBELでインキュベートされたHepa1-6細胞とチャンバ内の追加のNOBELとを含むチャンバをマウスの脇腹に48時間外科的に移植し、その後除去した。35日目に、反対の脇腹において、皮下に106個のHepa1-6細胞を移植することでマウスを接種した。
実施例3
NOBELは培養で増殖したHepa1-6細胞の細胞死を引き起こさない
また、NOBELが培養液中で成長したHepa1-6細胞において細胞死を引き起こすかどうかを試験するために実験を行った。これらの実験において、Hepa1-6細胞は1ウェルあたり105個の細胞で、24ウェルプレートで培養された。各ウェルの培地中にNOBELを添加し、3重反復実験で様々な用量で滴定した。NOBEL(0.5μg、1μg、2μg、4μg、40μg、400μgおよび4mg)と24時間インキュベートした後、プレートを遠心し、任意の付着していない、おそらく死んでいる細胞をペレット化した。培地を除去し、細胞をトリプシン処理し、その後培地に再懸濁した。その後細胞を96ウェル-V底プレートのウェルに移し、PBSで洗浄した。生/死染色は、PBS中のZombie Green(商標)生存率染色を使用して行った。その後細胞を洗浄し、固定し、フローサイトメトリーを行って生存率を決定した。
実施例4
肝細胞癌のある患者における自家腫瘍細胞およびIGF-1R AS ODNでのワクチン接種
肝細胞癌患者は、標準的な療法を使用して治療前、後、または治療と同時に特定される。各患者は以下の基準を満たし得る:年齢>18、60以上のカルノフスキー・パフォーマンススコア、および治療を妨げるような合併症がない。

(a)前記対象から得られた腫瘍細胞であって、組織モルセレータを使用して前記対象から得られたものである前記腫瘍細胞をIGF-1R AS ODNの存在下、前記バイオ拡散チャンバ内にカプセル化することと、
(b)前記バイオ拡散チャンバを照射すること

- 前記拡散チャンバが48時間、前記対象に移植される請求項18~20のいずれか1項に記載の方法。
- 前記肝は肝細胞癌である請求項18~21のいずれか1項に記載の方法。
- 前記肝細胞癌は、ステージI、ステージII、ステージIII、およびステージIVの肝細胞癌から選択される請求項22に記載の方法。
- 前記肝細胞癌は、ステージIVの肝細胞癌である請求項28に記載の方法。
- 前記方法は、化学療法なしに、放射線治療なしに、または両方なしに実施される請求項18~24のいずれか1項に記載の方法。
- 第1移植に続いてチャンバの第2移植を含む請求項18に記載の方法。
- 前記第2移植は第1移植で使用した前記チャンバの前記細胞と同時に前記対象から得られた腫瘍細胞を含むチャンバを使用する請求項26に記載の方法。
- 前記第2移植は、第1移植が完了した後前記腫瘍が再発したか、または第1移植に反応しなかった前記対象から得られた腫瘍細胞を使用する請求項26に記載の方法。
- 肝癌を有する対象をワクチン接種する方法であって、
(i)前記対象から細片化された腫瘍組織を得ることと、
(ii)その細片化した組織を滅菌トラップ内に回収することと、
(iii)その細片化した組織から接着細胞を採取することと、
(iv)バイオ拡散チャンバ内で配列番号1の配列を有するインスリン様成長因子1受容体のアンチセンスオリゴデオキシヌクレオチド(IGF-1R AS ODN)と共に採取した細胞をカプセル化することと、
(v)前記チャンバを照射することと、
(vi)前記対象に前記チャンバを移植することと
を含み前記肝癌に対する免疫応答が得られる、方法。 - 前記肝癌は、肝細胞癌(HCC)および胆管細胞癌から選択される請求項29に記載の方法。
- 前記肝癌はHCCである請求項30に記載の方法。
- 前記肝癌は胆管細胞癌である請求項30に記載の方法。
- カプセル化する前に18時間まで前記接着細胞をIGF-1R AS ODNで処理するステップを含む請求項29~32のいずれか1項に記載の方法。
- 約48時間、20個のチャンバで前記対象をワクチン接種する請求項29~33のいずれか1項に記載の方法。
- 腫瘍細胞は体温を超えた温度に曝露されない請求項29~34のいずれか1項に記載の方法。
- 前記HCCはステージI、ステージII、ステージIII、およびステージIVのHCCから選択される請求項31に記載の方法。
- 前記HCはステージIVのHCCである請求項36に記載の方法。
- 肝癌を有する対象への移植用のバイオ拡散チャンバであって、
(a)照射された腫瘍細胞であって、
前記腫瘍細胞は、前記対象の腫瘍組織から得られた接着細胞を含み、
前記腫瘍細胞は、前記チャンバ内にカプセル化する前にインスリン様成長因子1受容体のアンチセンスオリゴデオキシヌクレオチド(IGF-1R AS ODN)で事前にインキュベートされたものである、前記照射された腫瘍細胞と、
(b)照射されたIGF-1R AS ODNであって、前記IGF-1R AS ODNは配列番号1の配列を有する、前記照射されたIGF-1R AS ODNと
を含む、バイオ拡散チャンバ。 - 前記肝癌は肝細胞癌(HCC)および胆管細胞癌から選択される請求項38に記載のバイオ拡散チャンバ。
- 前記肝癌はHCCである請求項39に記載のバイオ拡散チャンバ。
- 前記肝癌は胆管細胞癌である請求項39に記載のバイオ拡散チャンバ。
- 前記チャンバ内の前記腫瘍細胞は、前記対象から得られた前記腫瘍組織と比べてネスチンポジティブ細胞が富化されたものである請求項38~41のいずれか1項に記載のバイオ拡散チャンバ。
- 前記腫瘍細胞は組織モルセレータを使用して前記対象から得られたものである請求項38~42のいずれか1項に記載のバイオ拡散チャンバ。
- 前記組織モルセレータは、静止外部カニューレ内に高速往復内部カニューレを含む請求項43に記載のバイオ拡散チャンバ。
- 前記組織モルセレータは、前記腫瘍組織を前記対象から得るとき、熱を発生しない請求項43に記載のバイオ拡散チャンバ。
