I.概要

A.結合特異性および機能特性
本発明は、タウに結合する抗体を提供する。いくつかの抗体は、ヒトのタウの微小管結合領域(MTBR)領域内のエピトープに特異的に結合する。いくつかの抗体はリン酸化状態に関係なくタウに結合する。いくつかの抗体は、リン酸化を受ける残基を含まないエピトープに結合する。これらの抗体は、天然源から精製するかまたは組換え発現させたタウポリペプチドで免疫することにより取得できる。抗体は、非リン酸化型のタウ、およびリン酸化を受けやすい1つもしくは複数の残基がリン酸化された形態のタウへの結合に関してスクリーニングできる。このような抗体は、好ましくは、非リン酸化タウと比較して、区別できない親和性で結合するか、またはリン酸化タウに少なくとも1.5倍、2倍または3倍の係数の範囲内で結合する(すなわち、「汎特異的である」)。3D6は、汎特異的モノクローナル抗体の一例である。本発明はまた、例えば、3D6のエピトープなどの上記の抗体のいずれかと同じエピトープに結合する抗体を提供する。また、例えば、3D6と競合するものなどの上記の抗体のいずれかとタウへの結合に関して競合する抗体も含まれる。

(3)そうでなければ、CDR領域と相互作用する(例えば、CDR領域の約6Å内である)、(例えば、相同な既知免疫グロブリン鎖の解析された構造上で軽鎖または重鎖をモデリングすることにより識別される)、または
(4)VL-VH界面に関与する残基である、
ことを想定することは理にかなっている。
キメラ抗体、ベニヤ抗体、またはヒト化抗体の重鎖および軽鎖の可変領域は、ヒト定常領域の少なくとも一部に連結できる。定常領域の選択は、ある程度、抗体依存性細胞傷害、抗体依存性細胞食作用および/または補体依存細胞毒性が所望されるか否かに依存する。例えば、ヒトアイソタイプIgG1およびIgG3は、補体依存性細胞傷害を有し、ヒトアイソタイプIgG2およびIgG4は有しない。ヒトIgG1およびIgG3はまた、ヒトIgG2およびIgG4より強力な細胞媒介エフェクター機能を誘導する。軽鎖定常領域は、ラムダまたはカッパであり得る。定常領域のナンバリング規則には、EUナンバリング(Edelman,G.M.et al.,Proc.Natl.Acad.USA,63,78-85(1969))、カバットナンバリング(Kabat,Sequences of Proteins of Immunological Interest(National Institutes of Health,Bethesda,MD,1991)、IMGT独自ナンバリング(Lefranc M.-P.et al.,IMGT unique numbering for immunoglobulin and T cell receptor constant domains and Ig superfamily C-like domains,Dev.Comp.Immunol.,29,185-203(2005))、およびIMGTエキソンナンバリング(Lefranc、上掲書)が挙げられる。
抗体発現細胞株(例えば、ハイブリドーマ)を用いた、キメラおよびヒト化抗体の産生に関するいくつかの方法が知られている。例えば、抗体の免疫グロブリン可変領域は、よく知られた方法を用いて、クローン化および配列決定できる。一方法では、重鎖可変VH領域は、ハイブリドーマ細胞から調製したmRNAを用いてRT-PCRによりクローン化される。コンセンサスプライマーを、翻訳開始を5’プライマーとして、g2b定常領域を特異的3’プライマーとして包含するVH領域リーダーペプチドに用いる。代表的プライマーは、Schenkらによる米国特許出願公開第2005/0009150号(以降では「Schenk」)に記載されている。複数の独立に誘導されたクローン由来の配列を比較して、増幅中に変化が導入されていないことを保証できる。VH領域の配列はまた、5’RACE RT-PCR法および3’ g2b特異的プライマーにより得たVHフラグメントをシーケンシングすることにより決定または確認できる。
上記のように、抗体は、最初に目的とする結合特異性に関しスクリーニングされ得る。能動免疫原は、同様に、このような結合特異性を有する抗体を誘導する能力に関しスクリーニングされ得る。この場合、能動免疫原を用いて実験動物を免疫し、得られた血清を適切な結合特異性に関し試験する。
神経原線維濃縮体の存在は、アルツハイマー病、ダウン症候群、軽度認知障害、原発性年齢関連タウオパチー、脳炎後パーキンソニズム、外傷後認知症またはボクサー認知症、ピック病、C型ニーマン・ピック病、核上性麻痺、前頭側頭型認知症、前頭側頭葉変性症、嗜銀性顆粒病、球状グリア性タウオパチー、グァム筋萎縮性側索硬化症-パーキンソン病/認知症複合;大脳皮質基底核変性症(CBD)、レビー小体型痴呆、レビー小体型アルツハイマー病(LBVAD)、慢性外傷性脳症(CTE)、球状グリア性タウオパチー(GGT)、および進行性核上麻痺(PSP)を含むいくつかの疾患で認められている。本投与計画はまた、これらの疾患のいずれかの処置または予防にも使用できる。神経疾患および病状とタウの間の広範囲に及ぶ関係のために、本投与計画は、神経疾患のない個体の平均値と比較して、(例えば、CSF内の)タウもしくはリン酸化タウのレベルの上昇を示す任意の対象の処置または予防に使用できる。本投与計画は、神経疾患と関連するタウの変異を有する個体における神経疾患の処置または予防にも使用できる。本方法は、アルツハイマー病、および特に患者における処置または予防に特に適している。
本発明はさらに、上記重鎖および軽鎖のいずれか(例えば、配列番号7、配列番号11、配列番号76~80、配列番号90~91および配列番号83~85)をコードする核酸を提供する。代表的な核酸としては、配列番号30~31、93~99、100~102、および105~106が挙げられる。場合によりこのような核酸はさらに、シグナルペプチドをコードし、重鎖可変領域または軽鎖可変領域に連結されたシグナルペプチドを発現できる。核酸のコード配列は、調節配列に作動可能に連結され、プロモーター、エンハンサー、リボソーム結合部位、転写終結シグナル、などのコード配列の発現を確実にできる。調節配列は、プロモーター、例えば原核生物プロモーターまたは真核生物プロモーターを含んでよい。重鎖または軽鎖をコードする核酸は、宿主細胞中での発現のためにコドン最適化できる。重鎖および軽鎖をコードする核酸は、選択可能な遺伝子をコードできる。重鎖および軽鎖をコードする核酸は、単離型で生じ得、または1つまたは複数のベクター中に挿入できる。核酸は、例えば、固相合成またはオーバーラッピングオリゴヌクレオチドのPCRにより合成できる。重鎖および軽鎖をコードする核酸は、例えば、発現ベクター内の1つの連続した核酸として連結でき、または別々の、例えば、それぞれ自身の発現ベクター中に挿入できる。

予防的適用では、抗体またはその医薬組成物は、疾患(例えば、アルツハイマー病)に罹患しやすい患者、あるいは、疾患のリスクのある患者に、疾患のリスクを低減する、疾患の重症度を軽減する、または疾患の少なくとも1つの徴候もしくは症状の発症を遅延させるのに有効な投与計画(投与の用量、頻度および経路)で投与される。特に、投与計画は、好ましくは脳内でタウもしくはリン酸化タウおよびそれから形成されるフィラメント対を抑制もしくは遅延させるか、および/またはその毒性作用を抑制もしくは遅延させるか、および/または行動欠陥の発症を抑制もしくは遅延させるのに有効である。治療用途では、抗体は、疾患の少なくとも1つの徴候または症状の改善または少なくともさらなる悪化を抑制するのに効果的な投与計画(投与の用量、頻度および経路)で疾患(例えば、アルツハイマー病)の疑いのある患者、または既に疾患に罹患している患者に投与される。特に、この投与計画は、タウ、リン酸化タウ、もしくはそれから形成されるフィラメント対のレベルのさらなる上昇、関連する毒性および/または行動障害を低減または少なくとも抑制するのに効果的であるのが好ましい。
インビボイメージング、診断方法、および最適化免疫療法
本発明は、患者のタウタンパク質沈着物(例えば、神経原線維濃縮体およびタウ封入体)をインビボイメージングする方法を提供する。本方法は、本発明のヒト化抗体を患者に投与し、その後、結合した後の抗体を検出することにより作用する。投与した抗体の除去反応は、Fabなどの完全長の定常領域を欠く抗体フラグメントを使用することによって回避または低減することができる。いくつかの方法では、同一の抗体は、処置および診断試薬の両方として機能し得る。
本発明波佐羅にキット(例えば、容器)を提供し、キットは、本明細書で開示の抗体および関連物、例えば、使用説明書(例えば、添付文書)を含む。使用説明書は、例えば、抗体および必要に応じ、1種または複数の追加の薬剤の投与のための説明書を含んでよい。抗体の容器は、単位用量、大量包装(例えば、複数回投与用包装)または分割単位用量であってよい。
臨床診断または処置または研究の場合、抗体は、タウまたはそのフラグメントの検出に使用できる。例えば、抗体を使用して、生物試料がタウ沈着物を含むという指標として生物試料中のタウの存在を検出することができる。抗体の生物試料への結合は、抗体のコントロール試料への結合と比較できる。コントロール試料および生物試料は、同じ組織起源の細胞を含み得る。コントロール試料および生物試料は、同じ個体または異なる個体から、同じ時期または異なる時期に得ることができる。必要に応じ、複数の生物試料および複数のコントロール試料が複数の時期に評価され、試料間の差異とは関係のないランダムな変動に対し保護される。その後、生物試料(単一または複数)とコントロール試料(単一または複数)との間の直接比較が行われ、生物試料(単一または複数)に対する抗体結合(すなわち、タウの存在)がコントロール試料(単一または複数)に対する抗体結合と比べて、増加、減少、または同じであるかどうかが判定される。抗体の、コントロール試料(単一または複数)に対する結合と比べて、生物試料(単一または複数)に対する結合の増加は、生物試料(単一または複数)中のタウの存在を示す。いくつかの事例では、抗体結合の増加は統計的に有意である。場合により、生物試料に対する抗体結合は、コントロール試料に対する抗体結合よりも、少なくとも1.5倍、2倍、3倍、4倍、5倍、10倍、20倍または100倍である。
実施例1.タウモノクローナル抗体の特定
タウに対するモノクローナル抗体を以下のように生成した。P301S変異を含む組換えN末端Hisタグ付き383a.a.ヒトタウ(4R0N)[免疫原A]またはP301S変異を含み、N末端Hisタグを欠く組換え383a.a.ヒトタウ(4R0N)[免疫原B]を用いて、免疫化を実施した。免疫原は、RIBIアジュバント中に乳化した。
383aa 4R0Nヒトタウタンパク質全体にわたる一連の重複ビオチン化ペプチドをマウス3D6抗体のマッピングに使用した。別のペプチドを使用して、タンパク質のC末端およびN末端の潜在的翻訳後修飾のモデル化を行った。

方法:B103神経芽腫細胞へのタウの結合の抗タウモノクローナル抗体による阻害
1.B103細胞を5x105細胞/mLの濃度で再懸濁する。MSD高結合プレートにウェル当たり50μLの細胞懸濁液を蒔く。これにより、25k細胞/ウェルが得られる。プレートを覆い、37℃、5%CO2下で2時間細胞を付着させる。
2.細胞付着後、プレートを反転することにより、ウェルからPBSを除去し、静かにタッピングして過剰緩衝液を除去する。50μlのPBS中の3%MSDブロッカーA緩衝液または他の好適するブロッキング緩衝液を各ウェルに加え、震盪せずに室温でプレートを1時間インキュベートした。
3.プレートブロッキングステップ中に、タウおよび抗タウ抗体を以下のように共インキュベートする:
a.2mg/mLの抗タウ抗体から出発し、PBSで1:2の系列希釈により追加の7種の希釈を行う。
b.タウをPBS中20nMに希釈する。タウ濃度はそれぞれのウェルで一定になる。
c.10nMの最終タウ濃度となるように、また1mg/mLの開始抗タウ濃度として、タウおよび抗タウ抗体を1:1で混合する。
d.混合物を室温で約1時間震盪(600rpm)しながらインキュベートする。
4.ステップ2のプレートブロッキング後、プレートを反転してウェルからブロッキング緩衝液を除去し、静かにタッピング後、マルチチャンネルピペットを用いてプレートをPBSで2回洗浄した。過剰緩衝液を確実に完全に除去する。タウ:抗タウ複合体を加える前に、蒔いた細胞を4℃に冷却する。
5.ステップ3の50μLの冷却した複合体を播種した細胞に加え、氷上で30分間インキュベートする。
6.以前記載したように、プレートを冷却PBSで2回洗浄する。
7.細胞表面結合タウの検出のために、ウェル当たり50μLの16B5.SULFO-TAGを加える。氷上で30分間インキュベートする。
8.以前記載したように、プレートを再度、冷却PBSで2回洗浄する。
9.ウェル当たり150μLの1X界面活性剤不含リードバッファT(H2Oで希釈)を加え、MSD SECTOR(商標)600測定器で直ちに読み取る。リードバッファを加えるときに、気泡の混入を避ける。
10.MSDシグナル対タウの濃度を報告する。
方法:組換えタウ-N末端6xHisタグを有する精製組換えタウを、1xPBS(pH7.4)中の等モル量の低分子量ヘパリンと混合し、旋回装置を用いて37℃で96時間インキュベートした。チオフラビンTに対する結合により、試料の凝集を確認した。
抗体とインキュベーション-凝集した組換えタウと共に抗体を、回転または旋回装置を用いないで37℃で96時間インキュベートした。実験の終わりに、試料を25mMのチオフラビンTと共にインキュベートし、放出蛍光(450/482 ex/em)を測定することにより、凝集を測定した。シグナルから、緩衝液試料までバックグラウンドを差し引いた。

- タウ凝集または沈着に関連する疾患を有するまたはその疾患のリスクのある対象におけるタウタンパク質沈着物を検出する方法であって、請求項1~84のいずれかで定められる抗体を対象に投与すること、および前記対象においてタウに結合した抗体を検出することを含む、方法。
- 前記タウの凝集または沈着に関連する疾患が、アルツハイマー病、ダウン症候群、軽度認知障害、原発性年齢関連タウオパチー、脳炎後パーキンソニズム、外傷後認知症またはボクサー認知症、ピック病、C型ニーマン・ピック病、核上性麻痺、前頭側頭型認知症、前頭側頭葉変性症、嗜銀性顆粒病、球状グリア性タウオパチー、グァム筋萎縮性側索硬化症-パーキンソン病/認知症複合;大脳皮質基底核変性症(CBD)、レビー小体型痴呆、レビー小体型アルツハイマー病(LBVAD)、慢性外傷性脳症(CTE)、球状グリア性タウオパチー(GGT)、または進行性核上麻痺(PSP)である、請求項124に記載の方法。
- 前記抗体が、前記対象の身体中に静脈内注射により投与される、請求項124に記載の方法。
- 前記抗体が、頭蓋内注射によりまたは前記対象の頭蓋骨を通る孔をあけることにより前記対象の脳に直接投与される、請求項124に記載の方法。
- 前記抗体が、標識される、請求項124に記載の方法。
- 前記抗体が、蛍光標識、常磁性標識、または放射性標識により標識される、請求項128に記載の方法。
- 前記放射性標識が、陽電子放出断層撮影(PET)または単一光子放射型コンピュータ断層撮影(SPECT)を用いて検出される、請求項129に記載の方法。
- タウ凝集または沈着に関連する疾患について処置される対象における処置の効力を測定する方法であって、
(a)請求項1~84のいずれか1項で定められる抗体を対象に投与することによる処置の前に前記対象中のタウタンパク質沈着物の第1のレベルを測定すること、および前記対象中のタウに結合した抗体の第1の量を検出すること、
(b)前記対象に前記処置を投与すること、
(c)前記抗体を対象に投与することによる処置の後で前記対象中のタウタンパク質沈着物の第2のレベルを測定すること、および前記対象中のタウに結合した抗体を検出すること
を含み、
前記タウタンパク質沈着物のレベルの低下が、処置に対する陽性応答を示す、方法。 - タウ凝集または沈着に関連する疾患について処置される対象における処置の効力を測定する方法であって、
(a)請求項1~84のいずれか1項で定められる抗体を対象に投与することによる処置の前に前記対象中のタウタンパク質沈着物の第1のレベルを測定すること、および前記対象中のタウに結合した抗体の第1の量を検出すること、
(b)前記対象に前記処置を投与すること、
(c)前記抗体を対象に投与することによる処置の後で前記対象中のタウタンパク質沈着物の第2のレベルを測定すること、および前記対象中のタウに結合した抗体の第2の量を検出すること
を含み、
タウタンパク質沈着物のレベルに変化のないことまたはタウタンパク質沈着物におけるわずかな増加が、処置に対する陽性応答を示す、方法。
