Abstract
(式中、N及びMは、独立して5-位または2’-位にハロゲンまたはヒドロキシが結合されたデオキシウリジン(deoxyuridine、dU)、デオキシシチジン(deoxycytidine、dC)、ウリジン(uridine、U)またはシチジン(cytidine、C)であり、x及びyは、独立して0~10の整数であり(但し、xとyが同時に0である場合は除く。)、nは1~10の整数であり、mは1~10の整数である。)

Description
実施例1-1.ゲムシタビン含有オリゴヌクレオチドの合成
28種のオリゴヌクレオチドを設計して作製した。作製された各々のオリゴヌクレオチドにゲムシタビンを結合し、オリゴヌクレオチド変形体を作製した。設計された28種のオリゴヌクレオチド配列、及びオリゴヌクレオチドに変形核酸が結合した28種のオリゴヌクレオチド変形体を下記表3に示す。以下、ゲムシタビンは、Gemと表記することもある。
前記「1-1.」に記載された方法と同様の方法で((N)x-[TGG]4[TTG][TGG]4/または5、([TGG]4[TTG][TGG]4/または5-(N)x)および((N)x-[TGG]4[TTG][TGG]4/または5-(N)y)を合成した(下記表4参照)。
ゲムシタビン(2,2’-difluorodeoxycytidine、dFdC)は、血漿中でシチジンデアミナーゼ(cytidine deaminase)によって非活性代謝物である2,2’-ジフルオロデオキシウリジン(difluorodeoxyuridine、dFdU)に変形され、この場合には体内の抗癌効能が低下する。ゲムシタビンをオリゴヌクレオチドに結合させた場合のシチジンデアミナーゼによるゲムシタビンの体内分解抑制効果を確認するために、血漿中のdFdC及びdFdUの濃度を確認した。
マウスの血漿中のdFdC、dFdU及びゲムシタビンの活性代謝物であるゲムシタビン三リン酸塩(triphosphate)(dFdCTP)の濃度を確認するために、liquid chromatography-tande mass spectrometry(LC-MS/MS)分析法を変形してマウスの血漿および組織の分析法を確立した。確立されたLC-MS/MS分析法により得られたクロマトグラムから内部標準物質のピーク面積に対するdFdC、dFdU及びdFdCTPのピーク面積比を求め、予め作成した検量線からマウスの血漿中濃度及び組織内濃度を計算した。
マウスの血液1mLを採血してテトラヒドロウリジン(THU)10mg/mL(in DW)10uLが入っている1.5mLのエッペンドルフチューブ(eppendorf tube)に入れた。遠心分離器を用いて14,000rpm、4℃の条件下で2分間遠心分離した後、血漿のみを取って他のバイアルに移し、-80℃で保存した。血漿50uLを1.5mLのアンバーエッペンドルフチューブ(amber eppendorf tube)に移した後、内部標準物質(meltform 1.29ug/mL in ACN)150uLを入れて10分間ボルテックス(vortex)した。遠心分離器を用いて14,000rpm、4℃の条件下で20分間遠心分離した。遠心分離したチューブで試料150uLを他の1.5mLのアンバーエッペンドルフチューブに移した。GeneVac EZ-2 automated evaporation system(GeneVac Ltd、Ipswich、UK)を用いて有機溶媒を乾燥した(GeneVac HPLC mode、max temperature 35℃、1hr)。有機溶媒を乾燥した1.5mLのアンバーエッペンドルフチューブに125uLのDWを入れて再構成(reconstitution)した。5分間ボルテックスした後、遠心分離器を用いて14,000rpm、4℃の条件下で5分間遠心分離した。遠心分離された溶液の上清液をPTFE(hydrophilic)0.2umのシリンジフィルター(syringe filter、Toyo Toshi、Japan)を用いて、LC-MS/MS用バイアル(vial)に100uLずつ入れた。5uLをLC-MS/MSに注入した。
質量分析計(Mass spectrometry)としては、AB SCIEX QTRAP 5500、Electrospray ion mode(Sciex、Framingham、MA、USA)を用いた。定量化(Quantitation)の条件は、下記表5に示す。
2-1-1及び2-1-2に従って各試料を処理し、(Gem)x-[TGG]m[TTG][TGG]nの化合物の種類によるラット血漿中のdFdC/dFdUの比率を測定し、下記表7に示す(Gem=ゲムシタビン)。
3-1.IO101の膵臓癌細胞株に対する抗癌効果の確認
膵臓癌細胞株としてBxPC3、PANC-1、Miapaca-2、Capan-1をそれぞれ培養した。数回の継代培養後に細胞が安定したとき、薬物の効果を検証するために96ウェルプレートに1×103個の細胞を播種(seeding)した。翌日、IO101、IO100、IO100-Con、IO101-Con及びGemを最高濃度100uMで1/10ずつ希釈して100nMまで4つのポイントの濃度で準備し、細胞が播種(seeding)された96ウェルプレートに処理した。48時間が経過した後、WST-1溶液を10ulずつウェルに添加し、37℃の培養器で1時間培養した。その後、サンプルをマイクロプレートリーダー(micro plate reader、VERSA Max)で440nmの値を測定した。測定が完了した数値を計算して、処理濃度別の細胞生存率のグラフを作成した。
前記「3-1.」の効能評価方法と同様の方法で(Gem)2-[TGG]4[TTG][TGG]5(IO101L)の膵臓癌細胞株で細胞生存力測定(cell viability assay)を用いて効能を検証した。4種の膵臓癌細胞株のすべてにおいて、対照群と比較してIO101Lが膵臓癌細胞の成長を効果的に阻害した(図7参照)。
ラットにゲムシタビンを静脈注射(8mg/kg)またはオリゴヌクレオチドにゲムシタビンが結合された新しい製剤である(Gem)x-[TGG]m[TTG][TGG]nを静脈注射(160mg/kg、ゲムシタビン基準で8mg/kg)して、ゲムシタビン及びその代謝物である2’,2-ジフルオロデオキシウリジン(dFdU)シアニジン-3-グルコシドの薬物動態の変化を比較した。
Sprague-Dawley雄性ラットを吸入麻酔薬であるイソフルラン(isoflurane)で誘導麻酔し、PE(polyethylene)チューブ(Clay Adams、Becton Dickinson、NJ、USA)を用いて頸動脈(血液採取用)に挿入して縫合糸で縫合し、その先端を首の後に固定した。手術中にエーテルを用いて麻酔を維持し、ヘパリンが(20units/mL)含有された生理食塩水を約0.5mL注入して、カニューレ(cannula)に血液が凝固することを防止した。手術が終わった後、ラットをそれぞれ代謝ケージ(metabolic cage)に入れて麻酔から完全に回復させた後(4~5時間)、(Gem)2-[TGG]4[TTG][TGG]5(N=4)及びゲムシタビン投与群(N=4)に分けて投与した。ゲムシタビン及び(Gem)2-[TGG]4[TTG][TGG]5を投与容量(8mg/kg及び160mg/kg)に合わせて電子スケール(CP224S、Sartorius、GER)で秤量した後、0.9%滅菌生理食塩水に溶解して調製した。1及び2ml/kgの薬物を尾静脈に投与した。(Gem)2-[TGG]4[TTG][TGG]5及びゲムシタビンの投与直前(0分)、投与後1、5、15、30、45、60、120、360、720分に頸静脈からそれぞれ血液を0.3mL採取した。血液中のゲムシタビンがシチジンデアミナーゼ(cytidine deaminase)によって代謝物に変化することを防止するために、血液1mL当たりシチジンデアミナーゼ阻害剤であるテトラヒドロウリジン(teterahydrouridine、THU)を10mg/mLの濃度で溶かした蒸留水10μLを予め入れたエッペンドルフチューブに入れてすぐに遠心分離した後、血漿を保存した。ゲムシタビンは、光によって分解され得ることが知られているので、すべての血漿試料は、茶色のエッペンドルフチューブにそれぞれ50μLずつ入れて、LC-MS/MS分析時まで-80℃で保存した。
試料処理とゲムシタビン及びdFdUの血漿中濃度の分析は、既に確立したアセトニトリル(acetonitrile)を用いた血漿タンパク質沈殿法で前処理した後、分析機器アジレント1200シリーズ(Agilent Technologies)、AB SCIEX QTRAP 5500、Electrospray ion mode(Sciex、Framingham、MA、USA)を用いて、LC-MS/MSを行った。すべての分析は、チューブを遮蔽(shielding)して行った。定量化(Quantitation)の条件はSRM(selected reaction monitoring)モード(前記表5参照)、固定相の条件はHypersil gold C18、1.9μm、100×2.1mm2(Thermo Scientific、Matriks、Oslo、Norway)、移動相の条件はそれぞれ0.1%ギ酸(formic acid)含有した蒸留水(A)とアセトニトリル(B)で行った(前記表6参照)。
ゲムシタビン及びdFdUをそれぞれ蒸留水に溶かし、1mg/mLの貯蔵溶液を製造して冷凍保管した。3次蒸留水で希釈してゲムシタビンの濃度が200、400、800、2000、10000、20000、100000ng/mL及びdFdUの濃度が200、400、800、2000、10000、20000、50000ng/mLになるように作業溶液を作成して冷蔵庫に保管した。内部標準物質(IS)であるメトホルミン(metformin)は、アセトニトリルに溶かして645ng/mLの作業溶液を製造した。ゲムシタビンとdFdUの作業溶液をラットの空血漿に添加し、ゲムシタビンとdFdUの血漿中濃度がそれぞれ10、20、40、100、500、1000、5000ng/mL及び10、20、40、100、500、1000、2500ng/mLになるように標準血漿試料をそれぞれ作成した。
50μLの血漿に内部標準物質メトホルミン(645ng/mL in acetonitrile)150μLを入れ、10分間ボルテックス(vortex)した後、15,000rpmで-4℃で20分間遠心分離した。その後、上清液150μLを取って他のエッペンドルフチューブに移した後、窒素気流下で有機溶媒を乾燥した。乾燥したエッペンドルフチューブに3次蒸留水125μLを入れて再溶解した。その後、5分間ボルテックス(vortex)し、15,000rpmで-4℃で5分間遠心分離した後、上清液100μLを取ってLC-MS/MSバイアル(vial)に移した後、5μLを注入してLC-MS/MSで分析した。
分析過程の適合性を判断するために、試料処理中に分析試料バッチごとに検量線作成用試料を分析した後、分析適合性試料(ゲムシタビン及びdFdUのそれぞれの最低定量限界:LLoQ、10ng/mL、低濃度:LoQC、30ng/mL、中濃度:MiQC、900ng/mL、高濃度:HiQC、4000ng/mL及び最低定量限界:LLoQ、10ng/mL、低濃度:LoQC、30ng/mL、中濃度:MiQC、900ng/mL、高濃度:HiQC、2000ng/mL)を2回ずつ分析した。6個の適合性試料のうち、少なくとも67%(例えば、6個中4個)の試料が理論値の±15%以内でなければならず、同じ濃度で50%以上の試料が理論値の±15%以内であるかを確認した。
ゲムシタビンの薬物動態パラメータは、Winnonlin Professional(Pharsight、Mountain View、CA、USA)プログラムを用いて求めた。血漿濃度-時間曲線下面積(AUCt)の値は、薬物投与してから最終定量時点までの血漿濃度-時間曲線からlog-linear台形公式(血漿濃度が増加する区間では、linear-台形公式を用いる一方、血漿濃度が減少する区間では、濃度値をログ変換して台形公式を用いる。)を用いた。経口投与後、血漿濃度-時間曲線から最高血漿濃度(Cmax)及び最高血漿濃度に到達する時間(Tmax)を求めた。無限時間までの血漿濃度-時間曲線下面積(AUCinf)の値は、下記式を用いて求めた。末端消失速度定数(γZ)及び半減期(t1/2)は、血漿濃度推移の消失相の傾きから求めた。
ラットにゲムシタビンを静脈注射(8mg/kg)または(Gem)2-[TGG]4[TTG][TGG]5を静脈注射したとき(160mg/kg、ゲムシタビンとして8mg/kg)のゲムシタビンの血漿中濃度を図8に、dFdUの血漿中濃度を図9に示す。また、ゲムシタビンを静脈注射または(Gem)2-[TGG]4[TTG][TGG]5を静脈注射したときのゲムシタビン及びdFdUの薬物動態パラメータの値を下記表8に平均値±標準偏差で示す。下記表8において、AUCtは0分から最後の採血時間までの曲線下面積、t1/2はterminal half-life(血漿消失半減期)、CLはtotal body clearance、Vdssは体内分布容積、MRTは薬物が体内に滞留する平均時間(mean residence time)、tmaxはmedian(ranges)であり、Metabolic conversion ratioの値は、dFdU AUCtの値をゲムシタビンAUCtの値で割って計算した値である。
5-1.豚皮の準備ステップ
豚皮を水道水で3回洗浄し、1次精製水で3回洗浄した後、3kg(20cm×20cm)に分けて-20℃の冷凍庫に保管した。冷凍豚皮を4℃で2時間静置して解凍した後、1.5cm×8cmサイズに細かく切り、0.5M酢酸7.5Lを加えて一晩静置し、豚皮が膨潤することを確認した。
膨潤された豚皮を取り出して1.5cm×1.5cmサイズに切った後、新しい0.5M酢酸7.5Lをさらに加えて数時間静置した後、ふるいを用いて真皮のみをろ過回収した。10Lの精製水を用いて真皮を洗浄した。洗浄過程を計5回繰り返した。洗浄が終わった真皮にエタノール20Lを加えた後、4℃で一晩撹拌した。一晩撹拌が終わったサンプルから真皮のみを回収してエタノール20Lを加えた後、さらに4℃で1時間撹拌した。ふるいを用いて真皮をふるい分け、1時間程度静置してエタノールを除去した。脂肪が除去された真皮を適度な重量(500g)に小分けして、-80℃の超低温冷凍庫に保管した。
4℃で静置して解凍した3kgの冷凍真皮に0.5M酢酸7.5Lを加えた後、30分間静置した。ふるいを用いてろ過して酢酸を除去した後、真皮を250gずつ小分けした。250gの真皮と精製水2Lをミキサーに入れて2分間粉砕した後、さらに精製水2Lを加えて2分間粉砕した。粉砕した組織に0.73M酢酸4Lを加えた。さらにホモジナイザー(Homogenizer)を用いて3分間組織を再粉砕した。粉砕過程を合計4回繰り返して1kgの冷凍真皮を粉砕、混合した。これに0.73M酢酸18Lを加えて、最終的に酢酸の濃度が0.5Mになるように調整し、pHが2.5~4程度であることを確認した。攪拌機を用いて3時間低速攪拌した。
撹拌が終わった真皮サンプルに真皮1kg当たりに15×107unitのペプシンを加え、撹拌機を用いて24時間低速撹拌した。ペプシン処理が終わったサンプルに10M NaOHを加え、攪拌し、pH8~9とし、さらに10分間攪拌してペプシンを非活性化した。塩基処理によるペプシンの非活性化後、4M HClを加え、攪拌し、pH3.4とし、さらに10分間攪拌した。遠心分離器を用いてサンプルを7800rpm、4℃で10分間遠心分離した後、上澄液の表面の脂肪を除去して残りの上澄液を収集保管した。
真皮から製造された上澄液1Lに対して5M NaClを163mlの割合でゆっくりと加え、15分間攪拌した後、4℃の条件下で一晩静置して塩析した。塩析後、上澄液を吸引除去した後、沈殿物を遠心分離(7800rpm、10分、4℃)して上澄液を完全に除去した。沈殿物に30Lのエタノールを加えて4℃の条件下で一晩撹拌して洗浄した。遠心分離(7800rpm、10分、4℃)後、さらに沈殿物に30Lのエタノールを加え、4℃の条件下で6時間攪拌して2次洗浄した。遠心分離(7800rpm、10分、4℃)後、沈殿物であるアテロコラーゲン中間体の重量を測定し、小分けして-80℃の超低温冷凍庫に保管した。
アテロコラーゲン中間体200gに0.02M尿素(Urea)2.8Lを加えた後、一晩攪拌した。Centramate(Tangential Flow Filtration System)を用いて、アテロコラーゲンのダイアフィルトレーション(diafiltarion)を行い、回収された溶液を攪拌機で攪拌しながら0.5M NaOHを加えてpH7とした。製造された中性のアテロコラーゲンをジップロック(登録商標)に薄くて平らに小分けして、-80℃の超低温冷凍庫に保管した。凍結乾燥機の予備凍結(-40℃)を少なくとも1時間以上行った後、超低温冷凍庫に保管されているアテロコラーゲンを凍結乾燥機に入れて凍結乾燥を行った。凍結乾燥が完了した医療用アテロコラーゲンは適当なサイズに切って真空包装した後、冷蔵保管した。
5-7-1.凍結乾燥されたアテロコラーゲンを酢酸ナトリウムバッファー溶液(0.3M Sodium Acetate(NaOAC)、45%acetic acid)に溶かすステップ
0.3M NaOAC、45%酢酸(acetic acid)バッファ溶液の製造のために、3次滅菌水55mlと99%以上の純度の酢酸(acetic acid)溶液45mlにCH3CO2Naを2.4g溶かす。その後、pHメーター(pH meter)でpHが3.0になるように酢酸(Acetic acid)で滴定した。凍結乾燥アテロコラーゲン3gを0.3M NaOAC、45%酢酸(acetic acid)溶液に溶かすために、滅菌鉗子とハサミで細かく切って準備した。スターラーバー(Stirring bar)で溶液を混ぜながら、細かく切ったアテロコラーゲンを少しずつ入れて完全に溶かした。
準備されたTFFシステムに3%アテロコラーゲン溶液を、図のようにポンプを用いて流しながら(TFF 100Kで)コラーゲン溶液を透過し、ろ過(filtration)された酢酸ナトリウムバッファー溶液は廃棄物(waste)として移動廃棄し、残りの溶液は再びreservoirでretentate管を介して移動するようにして準備した。1X PBSバッファ溶液をreservoirに添加して3.0%アテロコラーゲン溶液の水位を一定に維持しながら透析ろ過を続けた。最初に準備したアテルロコラーゲン-酢酸ナトリウムバッファー溶液の10倍に相当する体積のPBS溶液を用いて透析ろ過を続けた。これにより、バッファ交換(buffer exchange)を行った。バッファ交換が完了すると、透過されて出てくる溶液のpHをモニタリングして中性pHが検出されることを確認する方法により、バッファ交換の完成をチェックした。透析とろ過過程が終了した3%アテロコラーゲンを滅菌処理されたチューブに10mlずつ分取(aliquot)して移し、4℃の冷蔵庫に保管した。
6-1.アテロコラーゲン(Atecollagen、AC)分散液の調製
NaOAc/HAc溶液(0.3M酢酸ナトリウム(Sodium Acetate)、45%酢酸(acetic acid))に高純度の医療用アテロコラーゲンを0.5%、1.0%、1.5%、2.0%及び3.0%(バッファ100ml当たりアテロコラーゲンをそれぞれ0.5g、1.0g、1.5g、2.0g及び3.0g)入れて、pH3.0に維持し、攪拌して完全に溶かした。この溶液をTFF(Tangential Flow Filtration)を用いて透析ろ過し、アテロコラーゲン溶液を10倍体積(volume)の1XPBS溶液でダイアフィルトレーション(dia-filtration)して、PBS溶液の医療用アテロコラーゲン分散液を調製した。
(Gem)2-[TGG]4[TTG][TGG]4(IO101)または(Gem)2-[TGG]4[TTG][TGG]5(IO101L)をPBSに入れ、室温でミキサー(mixer)を用いて完全に溶かした後、アテロコラーゲン分散液(0.5%、1.0%、1.5%、2.0%、3.0%それぞれ)400μl当たり、IO101またはIO101LがPBSに混合された溶液を0.5mg、1.0mg、1.5mg、2.0mg、4.0mg、8.0mg混合した。
先に製造した高濃度のコラーゲン分散液と(Gem)2-[TGG]4[TTG][TGG]4(IO101)または(Gem)2-[TGG]4[TTG][TGG]5(IO101L)をゾル-ゲル型の製造方法と同様の方法で混合液を調製した。
ラット血漿中のゲムシタビン、(Gem)x-[TGG]m[TTG][TGG]n及び医療用の高純度アテロコラーゲン剤形から放出されるゲムシタビン及びゲムシタビンの非活性代謝物であるdFdUの濃度を分析した。試料の処理、検量線の作成、LC-MS/MSの分析条件は、前述の通りである。
ゲムシタビン/アテロコラーゲン・ディスクの血漿中安定性を評価するために、下記表12に示すように5つの群に分けて実験を行った。
IO101L/アテロコラーゲン・ディスク剤形のラット血漿中での安定性を評価するために、5つの群(グループ1~5)に分けて実験を行った(下記表12参照)。アテロコラーゲンとの比較のために、通常のコラーゲン(sk社製)を用いた。
8-1.皮下膵臓癌の動物モデルを用いたIO101/アテロコラーゲン(ゾル-ゲル型)の抗癌効能
膵臓癌細胞株を移植した皮下膵臓癌の動物モデルを用いて、IO101/アテロコラーゲン(ゾル-ゲル型)の膵臓癌治療効果を検証した。
ゲムシタビン/アテロコラーゲン(ゾル-ゲル型)と比較して、IO101/アテロコラーゲン(ゾル-ゲル型)の方が膵臓癌の抑制効果が優れることを確認した。様々な臨床状況に対応するために、局所注射治療が可能なゾル(Sol)型の薬物を使用した。注射後の腫瘍サイズの変化と組織学的変化を確認して治療効果を検証した。
IO101/アテロコラーゲン(ゾル-ゲル型)を皮下注射した後、動物の体重減少、毒性を評価して、アテロコラーゲン分散液400uL当たりIO101の容量を2mgに固定した。アテロコラーゲン(AC)濃度によって、薬物の持続時間、治療効果などに差異が出ることがあるので、0.5%、1.0%、1.5%の濃度(バッファ100ml当たりのg数)のアテロコラーゲンに対して比較実験を行った。1.5%の濃度(バッファ100ml当たりのg数)のアテロコラーゲンにおいて、皮下膵臓癌のマウスの治療効果が最も高いことを確認した(図12参照)。
膵臓癌細胞株(Pancreatic cancer cell line)であるBXPC3をBALB/Cヌードマウスsubcutaneous 2×106ずつ移植した。3日後、腫瘍の上にグループごとに各3匹ずつディスクを移植した。移植後30日間、週に2回、マウスの腫瘍体積をキャリパー(calliper)を用いて(長軸×短軸)2×0.5で測定し、重量(weight)とともに週に2回測定した。インビボにおいて、他のグループに比べてIO101/ACディスクの方が膵臓癌がより抑制されることを確認することができた。ディスクの移植後、POD30 Mouse imageから腫瘍サイズを観察した。IO101/ACディスクが他のグループに比べて腫瘍体積が小さいことを肉眼でも確認することができた。IO101/ACディスクは、全般的に腫瘍体積が小さいことが確認され、IO101-2mg/1.5~3.0%ACグループの腫瘍体積が最も小さかった(図13及び14を参照)。
Balb/c-ヌード(雄性、6週齢、30匹)を特定のベクター(vector)の置換によってルシフェラーゼ(luciferase)発現する5×105 BxPC3癌細胞株とsalineを用いて膵臓に注入した。膵臓癌のマウスモデルが構築される2週間の時点でルシフェラーゼ・イメージング(luciferase imaging)を行って腫瘍を確認した。自己蛍光を減らすために、画像撮影の1週間前に無蛍光飼料を提供した。IO101/ACディスク及び比較薬物を腹腔内に挿入した。(Intra-Abdominal cavity insertion using surgery)モデルの構築直後、インビボIVISスペクトル(spectrum)機器を用いて、ルシフェラーゼ・イメージングを行って腫瘍及び腫瘍サイズを測定した。IVISスペクトル機器を用いて、ルシフェラーゼ・イメージングを時間帯別(16th、18th、21th、23th、25th、28th、31th、35th day)に測定した。最後のインビボIVISスペクトル・イメージングを行った後、動物を犠牲にして各臓器別(spleen、liver、heart、lung、kidney、pancreas include tumor)にエクスビボ(ex-vivo)を行った。摘出した腫瘍のサイズをキャリパー(calliper)で測定して比較した。実験動物にディスク薬物を挿入した直後からルシフェラーゼ・イメージングにより腫瘍サイズの変化を時間帯別に確認した。IVISスペクトルを用いて、ルシフェラーゼ・イメージングの変化を以下のようにして測定した(図15参照)。実験動物を犠牲にした後、エクスビボで腫瘍の実質的なサイズを測定した。下記表14に摘出した腫瘍の実質的なサイズの変化を示し、それを図16にグラフで示す。
Balb/c-ヌード(雄性、6週齢、30匹)を特定のベクター(vector)の置換によってルシフェラーゼ(luciferase)発現する5×105 BxPC3癌細胞株とsalineを用いて膵臓に注入した。膵臓癌のマウスモデルが構築される2週間の時点でルシフェラーゼ・イメージング(luciferase imaging)を行って腫瘍を確認した。自己蛍光を減らすために、画像撮影の1週間前に無蛍光飼料を提供した。IO101/ACディスクまたはIO101L/ACディスク及び比較薬物を腹腔内に挿入した。(Intra-Abdominal cavity insertion using surgery)モデルの構築直後、インビボIVISスペクトル(spectrum)機器を用いて、ルシフェラーゼ・イメージングを行って腫瘍及び腫瘍サイズを測定した。IVISスペクトル機器を用いて、ルシフェラーゼ・イメージングを時間帯別(16th、18th、21th、23th、25th、28th、31th、35th day)に測定した。最後のインビボIVISスペクトル・イメージングを行った後、動物を犠牲にして各臓器別(spleen、liver、heart、lung、kidney、pancreas include tumor)にエクスビボ(ex-vivo)を行った。各臓器に対する腹腔転移有無を観察した。ACディスク移植材は、ほとんど腹腔内肝、腎臓、横隔膜などに転移されたことを観察したが、IO101/ACディスク及びIO101L/ACディスクでは、腹腔内癌転移が観察されなかった(図18参照)。
膵臓癌同所移植マウスモデル(Orthotopic mouse model)において、IO101L/ACディスク移植後の濃度別生存率を比較した。ACディスクのみを移植した後は、32日後に5匹中2匹が生存し、IO101L-2mg(400μLアテロコラーゲン分散液当たりIO101Lを2mg混合)/ACディスクを移植した後は、5匹中4匹が32日まで生存した。IO101L-4mg(400μLアテロコラーゲン分散液当たりIO101Lを4mg混合)/ACディスク及びIO101L-8mg(400μLアテロコラーゲン分散液当たりIO101Lを8mg混合)/ACディスクでは、6日目に5匹がすべて死亡した(図19参照)。
8-6-1.BxPC3膵臓癌移植マウスの実験
同所移植(Orthotopic)膵臓癌のマウスで外科的手術で腫瘍を除去した後、IO101L/ACゾル-ゲル型またはIO101L/ACディスク薬物を挿入して、残りの腫瘍の抑制および腹腔内の他の臓器への転移が抑制されたかどうかを観察した。つまり、Balb/c-ヌード(雄性、6週齢、21匹)を特定のベクター(vector)の置換によってルシフェラーゼ(luciferase)発現する5×105 BxPC3癌細胞株(25uL)を準備し、腹腔麻酔したマウスの腹部を切開して膵臓(pancreas)を取り出し、準備しておいたBXPC-3-Luc cellを膵臓(pancreas)に注入した。膵臓癌のマウスモデルが構築される2週間の時点でルシフェラーゼ・イメージング(luciferase imaging)を行って腫瘍を確認した。使用される腫瘍細胞株(tumor cell line)は、BxPC3細胞で特定のベクターの置換によってルシフェラーゼを発現するBxPC3-Luc cellであり、ルシフェリン(luciferin)を用いて腫瘍を測定する。自己蛍光を減らすために、画像撮影の1週間前に無蛍光飼料を提供した。腹腔麻酔したマウスの腹部を切開して膵臓癌を除去した後、IO101L/ACゾル-ゲル型、IO101L/ACディスク、Gem/アテロコラーゲン・ゾル-ゲルまたはGem/アテロコラーゲン・ディスクを残りの腫瘍部位に移植した。(Intra-Abdominal cavity insertion using surgery)ルシフェラーゼ・イメージングを用いて腫瘍サイズおよび転移を確認し、それを図20に示す。
膵臓癌同所移植(orthotopic)マウスモデルの作製のために、注文したマウスが届いたら、1週間の馴化期間を有する。馴化期間中capan-1細胞を1×106細胞/100ul準備した。マウスの皮膚を2mm開腹し、脾臟(spleen)を見つけて反らし、膵臓に直接注射器で細胞を注入した。4週間後、腫瘍の直径が5~6mmになっているかをMRIで確認した。癌細胞の直径が5~6mmになったことが確認されると、開腹して膵臓癌をできるだけ多く除去した。除去後、残存癌の上にIO101L/ACディスク、アテロコラーゲン・ディスク(対照群)を挿入して縫合した(図21参照)。
8-7-1.患者由来の膵臓癌細胞を用いたPDX膵臓癌のマウスモデルの構築
PDXモデルは、腹腔鏡下膵臓切除術を受けた65歳の女性患者から切除された膵臓癌を用いて正常に確立した。病理学的検査の結果、サイズは3.2cmであり、リンパ管浸潤が頻繁な膵管腺癌(pancreatic ductal adenocarcinoma)が発見された。検索された7つのリンパ節のうち、1つの転移性リンパ節が発見された(AJCC 8th T2N1M0、IIB)。H&Eとnucleoline immunostatingによるPDXと元の原発腫瘍の組織学的特徴を比較したところ、全組織学的類似性は、原発腫瘍とPDX腫瘍との間で発見された(図24参照)。
PDXマウスの皮膚を切開した後、対照群(No treatment)、Gem-IP(;ゾル-ゲル型)、IO101-IP(;ゾル-ゲル型)、IO101/ACディスク(2.0mg/3.0%)、IO101-Con/AC(2.0mg/3.0%)及びGem/アテロコラーゲン・ディスク(0.12mg/3.0%)を、皮膚と腫瘍との間の皮下無感覚解剖を実施して局部的に移植した。腫瘍サイズは、キャリパー(Mitutoyo、Absolute AOS Digmatic、Kawasaki、Japan)により毎週3回測定した。体積は前述の通りにして計算した。薬物処理された動物の腫瘍成長をビヒクル処理されたマウスと比較して腫瘍成長速度(腫瘍体積/初期の腫瘍体積)で示した。データの統計的有意性は、IBM(登録商標)SPPS(登録商標)Statistics version 23で計算した。すべての結果は、平均±標準偏差で示し、マン・ホイットニー(Mann-Whitney)のUは、異なるグループによって連続変数を比較するために適用した。0.05未満のP値は、有意なものとみなされた(図25参照)。
顕微鏡検査によっては、肝、肺、腎臓および脾臓組織での潜在的な毒性(炎症または壊死性の変化など)を示唆する証拠はないことが確認された(図27参照)。血液検査によってもIO101/ACディスク群では、白血球減少症、貧血症及び好中球減少症が観察されなかった。これに対して、ゲムシタビンの全身効果と関連するIP-GEM群では、白血球減少症(WBC)(3.2±2.9 vs 5.4±2.9、P=0.028)、低ヘモグロビン値(HB)(10.3±4.6 vs 18.5±11.9、p=0.010)、好中球減少症(Neutrophil)(0.76±0.71 vs 2.69±2.66、p=0.010)が示された。下記表15は、血液検査の結果を示す。
(N)x-[TGG]4[TTG][TGG]4/または5、[TGG]4[TTG][TGG]4/または5-(N)xおよび(N)x-[TGG]4[TTG][TGG]4/または5-(N)y)のBxPC3(膵臓癌)、MD-MBA 231(乳癌)、Uuh-7(肝癌)、HT29(大腸癌)及びMv4-11(AML)細胞株に対する細胞増殖抑制効果を検証した。

- 前記アテロコラーゲン分散液は、PBS溶液100mlに対してアテロコラーゲンが0.5~5.5g含まれたものである、請求項8に記載の癌の予防または治療用薬学組成物。
- 前記組成物は、ゾル-ゲル型またはパッチ型である、請求項6に記載の癌の予防または治療用薬学組成物。
- 前記アテロコラーゲンは、a)コラーゲン含有動物組織をアルカラーゼとカタラーゼ、ペプシン(Pepsin)及びパパイン(papain)のうちの少なくとも一つを処理して抽出するステップと、b)抽出された物質を1次ろ過し、得られたろ液に中性塩を添加して塩析し、2次ろ過するステップと、c)前記2次ろ過により得られたコラーゲン塩析物を溶解して脂肪を吸着し、3次ろ過するステップと、d)前記3次ろ過工程で得られたろ液を凍結乾燥して凍結乾燥粉末を回収するステップと、e)前記凍結乾燥粉末を希塩酸(dil-HCl)、希酢酸(acetic acid)またはpH4~pH8のリン酸塩緩衝液に溶解し、濃縮してアテロコラーゲン溶液を製造し、製造されたアテロコラーゲン溶液をポリマービーズが充填されたカラムにカラムベッドボリュームの5~20%の体積で注入し、希塩酸、希酢酸またはpH4~pH8のリン酸塩緩衝液で展開してアテロコラーゲンを回収するステップとを経て製造されたものである、請求項8に記載の癌の予防または治療用薬学組成物。
