M(2013)Curr.Gene Ther.13(5):370-81)。しばしば、組換えAAVベクターは、「ヘルパー」成分をコードする第1のプラスミドと、cap遺伝子及びrep遺伝子を含む第2のプラスミドと、ウイルスにパッケージングされる介在DNA配列を含有するウイルスITRを含む第3のプラスミドとで細胞株がトランスフェクトされる、三重トランスフェクション(triple transfection)を用いて産生される。キャプシドに包まれたゲノム(目的のITR及び介在遺伝子)を含むウイルス粒子は、その後、凍結融解サイクル、超音波処理、界面活性剤、又は当該技術分野で公知の他の手段によって細胞から単離される。次いで、塩化セシウム密度勾配遠心分離又は親和性クロマトグラフィーを用いて粒子を精製し、続いて細胞、組織、又はヒト患者等の生物の目的の遺伝子に送達する。
認識配列を含むため、パッケージング細胞株で発現する任意のメガヌクレアーゼは、ウイルスゲノムがウイルス粒子にパッケージングされる前にウイルスゲノムを切断することができる。このことは、パッケージング効率の低下及び/又は断片化したゲノムのパッケージングをもたらす。以下を含むいくつかのアプローチを使用して、パッケージング細胞におけるメガヌクレアーゼ発現を防止することができる。
1.メガヌクレアーゼを、パッケージング細胞において活性ではない組織特異的プロモーターの制御下に置くことができる。例えば、筋組織にメガヌクレアーゼ遺伝子を送達するためにウイルスベクターを開発する場合、筋特異的プロモーターを使用することができる。筋特異的プロモーターの例としては、C5-12(Liu et al.(2004),Hum Gene Ther.15:783-92)、筋特異的クレアチンキナーゼ(MCK)プロモーター(Yuasa、et al.(2002),Gene Ther.9:1576-88)、又は平滑筋22(SM22)プロモーター(Haase et
al.(2013),BMC Biotechnol.13:49-54)が挙げられる。CNS(ニューロン)特異的プロモーターの例としては、NSE、シナプシン、及びMeCP2プロモーターが挙げられる(Lentz et al.(2012),Neurobiol Dis.48:179-88)。肝臓特異的プロモーターの例としては、アルブミンプロモーター(Palb等)、ヒトα1-抗トリプシン(Pa1AT等)、及びヘモペキシン(Phpx等)が挙げられる(Kramer et al.(2003),Mol.Therapy 7:375-85)。眼特異的プロモーターの例としては、オプシン、及び角膜上皮特異的K12プロモーターが挙げられる(Martin et al.(2002),Methods(28):267-75)(Tong et al.(2007),J Gene Med、9:956-66)。これらのプロモーター又は当該技術分野で公知の他の組織特異的プロモーターは、HEK-293細胞において高度に活性ではなく、従って、本発明のウイルスベクターに組み込まれた場合に、パッケージング細胞において有意なレベルのメガヌクレアーゼ遺伝子発現を生じることは期待されない。同様に、本発明のウイルスベクターは、不適合組織特異的プロモーター(即ち、周知のHeLa細胞株(ヒト上皮細胞)及び肝臓特異的ヘモペキシンプロモーターを用いる)の使用と共に他の細胞株を使用することを企図している。組織特異的プロモーターの他の例としては、滑膜肉腫PDZD4(小脳)、C6(肝臓)、ASB5(筋肉)、PPP1R12B(心臓)、SLC5A12(腎臓)、コレステロール調節APOM(肝臓)、ADPRHL1(心臓)、及び一遺伝子性奇形症候群TP73L(筋肉)が挙げられる(Jacox E et al.(2010),PLoS One 5(8):e12274)。
2.別の方法として、ベクターは、メガヌクレアーゼが発現されにくい異なる種に由来する細胞にパッケージングすることができる。例えば、ウイルス粒子は、非哺乳動物パッケージング細胞において活性ではない、周知のサイトメガロウイルス初期プロモーター又はSV40ウイルス初期プロモーター等の哺乳動物プロモーターを用いて、微生物、昆虫、又は植物細胞中に産生させることができる。好ましい実施形態では、Gaoらによって記載されているようなバキュロウイルス系を用いてウイルス粒子を昆虫細胞中に産生させる(Gao et al.(2007),J.Biotechnol.131(2):138-43)。哺乳動物プロモーターの制御下にあるメガヌクレアーゼは、これらの細胞中で発現する可能性は低い(Airenne et al.(2013),Mol.Ther.21(4):739-49)。更に、昆虫細胞は、哺乳動物細胞とは異なるmRNAスプライシングモチーフを利用する。したがって、ヒト成長ホルモン(HGH)イントロン又はSV40ラージT抗原イントロン等の哺乳動物イントロンをメガヌクレアーゼのコード配列に組み込むことが可能である。これらのイントロンは昆虫細胞のプレmRNA転写物から効率的にスプライシングされないため、昆虫細胞は機能的メガヌクレアーゼを発現せず、全長ゲノムをパッケージングする。対照的に、得られた組換えAAV粒子が送達される哺乳動物細胞は、プレmRNAを適切にスプライシングし、機能的メガヌクレアーゼタンパク質を発現する。Haifeng Chenは、HGH及びSV40ラージT
抗原イントロンを使用して、昆虫パッケージング細胞中の毒性タンパク質バルナーゼ及びジフテリア毒素断片Aの発現を減弱させ、これらの毒素遺伝子を担持する組換えAAVベクターの産生を可能にすることを報告した(Chen(2012),Mol Ther Nucleic Acids.1(11):e57)。
3.メガヌクレアーゼ遺伝子は、メガヌクレアーゼ発現のために小分子誘導物質が必要とされるように、誘導性プロモーターに作動可能に連結され得る。誘導性プロモーターの例としては、Tet-Onシステム(Clontech;Chen et al.(2015),BMC Biotechnol.15(1):4))及びRheoSwitchシステム(Intrexon;Sowa et al.(2011),Spine 36(10):E623-8)が挙げられる。両方のシステム及び当該技術分野で公知の類似のシステムは、小分子アクチベーター(それぞれドキシサイクリン又はエクジソン)に応答して転写を活性化するリガンド誘導性転写因子(それぞれTetリプレッサー及びエクジソン受容体の変異体)に依存する。このようなリガンド誘導性転写アクチベーターを用いて本発明を実施することは、1)対応する転写因子に応答するプロモーターの制御下にメガヌクレアーゼ遺伝子を配置することであって、メガヌクレアーゼ遺伝子が(a)転写因子の結合部位を有することと、2)パッケージングされたウイルスゲノム中の転写因子をコードする遺伝子を含むこととを含む。転写アクチベーターが同じ細胞にも供給されない場合、組換えAAV送達後に標的細胞又は組織中にメガヌクレアーゼが発現しないため、後者のステップが必要である。次いで、転写アクチベーターは、コグネイト小分子アクチベーターで処理された細胞又は組織中にのみメガヌクレアーゼ遺伝子発現を誘導する。このアプローチは、小分子誘導因子がいつ、どの組織に送達されるかを選択することによって、メガヌクレアーゼ遺伝子発現を空間時間的に調節することができるため有利である。しかしながら、担持能力が著しく制限されたウイルスゲノムに誘導因子を含める必要性は、このアプローチの欠点を作り出す。
4.別の好ましい実施形態では、組換えAAV粒子は、メガヌクレアーゼの発現を妨げる転写リプレッサーを発現する哺乳動物細胞株に産生される。転写リプレッサーは当該技術分野で公知であり、Tetリプレッサー、Lacリプレッサー、Croリプレッサー、及びLambdaリプレッサーが挙げられる。エクジソン受容体等の多くの核内ホルモン受容体は、それらのコグネイトのホルモンリガンドの非存在下で転写リプレッサーとしても作用する。本発明を実施するために、パッケージング細胞は、転写リプレッサーをコードするベクターでトランスフェクション/形質導入され、ウイルスゲノム中のメガヌクレアーゼ遺伝子(パッケージングベクター)は、プロモーターに作動可能に結合され、このプロモーターは、リプレッサーがプロモーターをサイレンシングするように、リプレッサーのための結合部位を含むように改変されている。転写リプレッサーをコードする遺伝子は、種々の位置に配置することができる。この遺伝子は、別のベクターにコードすることができ、ITR配列の外側のパッケージングベクターに組み込むことができ、cap/repベクター又はアデノウイルスヘルパーベクターに組み込むことができ、又は最も好ましくは、構成的に発現されるようにパッケージング細胞のゲノムに安定的に組み込むことができる。転写リプレッサー部位を組み込むために一般的な哺乳動物プロモーターを改変する方法は、当該技術分野で公知である。例えば、Chang and Roninsonは、強力な構成的CMV及びRSVプロモーターをLacリプレッサーのオペレーターを含むように改変し、改変プロモーターからの遺伝子発現がリプレッサーを発現する細胞で大きく減弱したことを示した(Chang及びRoninson(1996),Gene 183:137-42)。非ヒト転写リプレッサーの使用は、メガヌクレアーゼ遺伝子の転写が、リプレッサーを発現するパッケージング細胞においてのみ抑制され、得られる組換えAAVベクターで形質導入される標的細胞又は組織においては抑制されないことを確実にする。
本発明の実施形態は、本明細書に記載の操作されたメガヌクレアーゼ及びその変異体を
包含する。本発明の更なる実施形態は、本明細書に記載のメガヌクレアーゼをコードする核酸配列を含む単離されたポリヌクレオチド及びそのようなポリヌクレオチドの変異体を包含する。
形態では、変異HVR1領域又は変異HVR2領域は、親HVR内の特定の位置で見出されるアミノ酸残に対応する残基を含むことができる。本文脈中、「対応する」とは、変異HVRのアミノ酸残基が、同じ相対位置で(即ち、親配列の残りのアミノ酸に対して)親HVR配列中に存在する同じアミノ酸残基(即ち、別個の同一残基)であることを意味する。例として、親HVR配列が位置26にセリン残基を含む場合、残基26「に対応する残基を含む」変異HVRは、親位置26に相対的な位置にもセリンを含む。
PCSK9認識配列を認識し切断するメガヌクレアーゼの特性評価
1.PCS7-8認識配列を認識し切断するメガヌクレアーゼ
本明細書で「PCS7-8メガヌクレアーゼ」と集合的に呼ばれる操作されたメガヌクレアーゼ(配列番号6~14)は、PCS1-2認識配列(配列番号4)を認識し切断するように操作され、PCS1-2認識配列(配列番号4)は、PCSK9遺伝子内に位置する。操作されたPCS7-8メガヌクレアーゼの各々は、SV40に由来するN末端ヌクレアーゼ局在化シグナルと、第1のメガヌクレアーゼサブユニットと、リンカー配列と、第2のメガヌクレアーゼサブユニットとを含む。各PCS7-8メガヌクレアーゼの第1のサブユニットは配列番号4のPCS7認識ハーフサイトに結合し、第2のサブユニットはPCS8認識ハーフサイトに結合する(図2参照)。
PCS7-8L.197(配列番号6)のPCS7結合領域である配列番号15と少なくとも90%の配列同一性を共有する。配列番号6~14のPCS8結合領域は、それぞれ配列番号24~32として提供される。配列番号24~32の各々は、メガヌクレアーゼPCS7-8L.197(配列番号6)のPCS8結合領域である配列番号24と少なくとも90%の配列同一性を共有する。
PCS7-8メガヌクレアーゼがその対応する認識配列(配列番号4)を認識し切断できるかどうかを決定するために、先に記載されたCHO細胞レポーターアッセイを用いて各操作されたメガヌクレアーゼを評価した(国際公開第2012/167192号及び図3を参照)。アッセイを行うために、細胞のゲノムに組み込まれた非機能的緑色蛍光タンパク質(GFP)遺伝子発現カセットを担持するCHO細胞レポーター株を産生させた。各細胞株のGFP遺伝子は、メガヌクレアーゼによるいずれかの認識配列の細胞内切断が機能的GFP遺伝子をもたらす相同組換え事象を刺激するように、一対の認識配列によって割り込まれた。
Gene Pulser Xcellを使用して、PCS7-8細胞(1.0×106個)に、細胞ごとに1×106コピーのメガヌクレアーゼmRNAをエレクトロポレーションした。トランスフェクション後の指定された時点で、細胞をフローサイトメトリーによって評価し、GFP陽性細胞の割合を決定した。CHO-23/24メガヌクレアーゼも、陽性対照として各時点で含めた。
これらの研究は、本発明に包含されるPCS7-8メガヌクレアーゼが、細胞中の対応する認識配列を効率的に標的とし切断できることと、この効果は時間経過にわたって一貫
していることと、ヌクレアーゼは細胞に対して非毒性であることとを実証した。
HEK293細胞におけるPCS7-8認識配列の切断
1.実験プロトコル及びT7Eアッセイ
この研究は、本発明に包含されるPCS7-8メガヌクレアーゼがHEK293細胞におけるPCS7-8認識配列を切断できることを実証した。
Xcellを使用して、2.7ugの所与のPCSメガヌクレアーゼmRNAをエレクトロポレーションした。トランスフェクションの2日後及び5日後に、ゲノムDNA(gDNA)を細胞から回収し、T7エンドヌクレアーゼI(T7E)アッセイを行って内因性PCS7-8認識配列における遺伝子改変を評価した(図6)。T7Eアッセイでは、PCS7-8座は、PCS7-8認識配列に隣接するプライマーを用いたPCRによって増幅された。PCS7-8座内にindel(無秩序な挿入又は欠失)があった場合、得られるPCR産物は野生型対立遺伝子及び突然変異対立遺伝子の混合物からなる。PCR産物は変性され、ゆっくりとリアニールされ得る。ゆっくりとしたリアニールにより、野生型及び突然変異対立遺伝子からなるヘテロ二本鎖が形成され、ミスマッチ塩基及び/又はバルジが生じる。T7E1酵素はミスマッチ部位で切断され、ゲル電気泳動により可視化され得る切断産物が生じる。
トランスフェクションの2日後及び5日後に、低分子量DNA断片がPCS7-8x.66及びPCS7-8x.88を受け取った細胞で観察されたが、GFP対照のみをコードするmRNAで模擬トランスフェクト又はトランスフェクトされた細胞は、全長PCR産物を示した(図6)。これらの低分子量DNA断片は、PCS認識部位でのメガヌクレアーゼの活性により起こるミスマッチDNAにおけるT7エンドヌクレアーゼI切断によって産生される。
T7エンドヌクレアーゼIアッセイは、PCS7-8メガヌクレアーゼで処理されたHEK293細胞内のPCS7-8メガヌクレアーゼ認識部位周辺におけるindelの存在を検出し、標的部位の切断及び非相同末端結合(NHEJ)により当該部位の誤りがちな修復を示す。
PCS7-8認識配列におけるindelを観察するためのディープシーケンシング
1.ディープシーケンシングプロトコル
目的のPCS7-8メガヌクレアーゼ標的部位における挿入又は欠失を直接観察するために、ディープシーケンシングプロトコルを用いた。2e6 HEK293細胞に、製造者の指示に従ってBioRad Gene Pulser Xcellを使用して、5ugのPCS7-8メガヌクレアーゼmRNAをエレクトロポレーションした。模擬エレクトロポレーションはmRNAなしでも行った。トランスフェクションの48時間後、ゲノムDNA(gDNA)を細胞から回収した。PCS7-8座は、PCS7-8認識配列に隣接するプライマーを用いたPCRによって増幅された。このアンプリコンは、目視による確認のためにアガロースゲルで行い、Macherey-Nagel社製Nucleospin Gel and PCR Clean-upキットを用いて抽出し、New England Biolabs社製のIllumina用NEBNext Ultra
II DNA Library Kitを使用してシーケンシングライブラリーを調製した。ペアエンドシーケンシングライブラリーは、Illumina社製Miseq D
NAシーケンサで読み取った。シーケンスシングデータはカスタムスクリプトを用いて分析した。全長アンプリコンの25bp内にない始点又は終点を含むリードは除いた。indelを含むリードの割合は、認識配列の中間8bpのうちの少なくとも1つを組み込んだindelを持つ全長リードの数を全長リードの総数で除して計算した。
表3に示すように、ディープシーケンシングにより評価されたPCS7-8メガヌクレアーゼの各々は、模擬処理と比較して、その目的の認識部位におけるindelの割合の少なくとも100倍の増加を示した。
これらの実験は、本発明のPCS7-8メガヌクレアーゼがHEK293細胞内のその目的とする標的部位(即ち、PCS7-8認識配列)を切断し、非相同末端結合による誤りがちな修復を介したindelの出現を誘導する能力をはっきりと実証している。
PCS7-8メガヌクレアーゼを用いた霊長類肝臓のインビボ遺伝子編集
1.方法及び材料
実験を行って、PCS7-8メガヌクレアーゼが非ヒト霊長類モデルの肝臓細胞をインビボでPCS7-8認識部位編集する能力を評価し、被験体のPCSK9の血清レベルに対するそのような編集の効果を決定した。
よる血清PCSK9タンパク質レベルの分析、総コレステロール、HDL、LDL、及びトリグリセリドの分析、並びにアラニンアミノトランスフェラーゼ(ALT)レベルの分析のために、-3日目及び0日目、並びに投与後168日間(低用量動物)又は280日間(高用量及び中用量動物)の間に複数の時点で採取した。加えて、PCS7-8認識配列における挿入及び欠失(indel)のPCR分析、並びにインサイチューハイブリッド形成(ISH)による肝細胞におけるPCS7-8x.88メガヌクレアーゼ発現の分析のために、肝生検サンプルを投与後17日目に得た。
血清PCSK9タンパク質レベルは、-3日目、0日目、及びAAVベクターの投与後の複数の時点にてELISAにより測定した。高用量及び中用量動物は、投与後280日間追跡調査したが、低用量動物は168日間追跡調査された。
血清コレステロールレベル、LDLレベル、HDLレベル、及びトリグリセリドレベルに対するPCS7-8メガヌクレアーゼ治療の効果は、-3日目、0日目、及びAAVベクターの投与後の複数の時点にも測定した。
17日目に肝生検を行い、PCS7-8認識配列における挿入又は欠失(indel)の存在について検査した。indelは、認識配列に隣接するPCRプライマー、ゲノムの介在領域の増幅、及び得られたPCR産物のシーケンシングにより検出した。種々のindel、即ち、異なった長さの挿入及び欠失の両方がPCS7-8認識配列にて種々の頻度で検出された。図10は、被験体RA1857及びRA1866において最も頻繁に観察された8つのindelとその対応する頻度を示す。これら8つのindelは、被験体RA1857で観察されるindelの73%、被験体RA1866で観察されるindelの66%を占めていた。
更なる研究を行って、最適化された第2世代PCS7-8L.197メガヌクレアーゼの有効性を決定した。研究プロトコルは前述したものと同様であった。PCS7-8L.197メガヌクレアーゼは組換えAAVベクターを介して導入された。AAVベクターはAAV8キャプシドを有し、5’から3’へ、5’逆方向末端反復、肝臓特異的ヒトチロキシン結合グロブリン(TBG)プロモーター、イントロン、PCS7-8L.197メガヌクレアーゼのコード配列、ウッドチャック肝炎ウイルス(WHP)転写後調節エレメント、及び3’逆方向末端反復を含んでいた。AAVベクターは、医薬組成物で調製され、0日目に各々の体重が約6.5kgの1匹のオスと1匹のメスのアカゲザルに6×1012ゲノムコピー(GC)/kgの単回用量で単回注入として投与した。初期の血液サンプルは、ELISAによる血清PCSK9タンパク質レベルの分析及び総LDLレベルの分析のために、0日目及び投与後7日目に採取した。
肝生検サンプルから単離したDNAを、アンカードマルチプレックスPCRシーケンシング(AMP-seq)により生じたアンプリコンのディープシーケンシングによる指定したrhPCSK9エキソン7標的部位でのオンターゲット編集のために特性決定した。AMPアンプリコンは、入れ子状の遺伝子座特異的プライマーを用いて産生されるが、フォワードプライマーは、標的とされるメガヌクレアーゼ切断部位の50bp上流の固定位置にハイブリダイズし、アンプリコンの3’末端は、増幅前のゲノムDNAのランダムシェアリング後にDNA断片の長さによって画定される。
ルについて、挿入は経時的に一貫して減少した。PCS7-8L.197 AAVで治療された被験体から得たサンプルの同様のオンターゲット分子分析は、中用量のAAVを受けた被験体RA1857で観察されたようなindelのスペクトル及び頻度を示した。
これらの研究から、ISHで観察されるように、PCS7-8メガヌクレアーゼは霊長類肝細胞においてインビボで首尾良く発現され、その後、PCS7-8認識配列に切断部位を生ずることが明らかになった。切断部位における誤りがちな非相同末端結合は、その後、当該部位のPCR分析及びAMP-seq分析で観察されるように多数のindelを生じさせ、これにより、ELISAで観察されるようにPCSK9タンパク質発現を阻害する。これらの研究は、本発明者らに公知の霊長類肝臓における遺伝子編集の最初の報告された観察である。更に、これは霊長類におけるPCSK9遺伝子の遺伝子編集の最初の観察であり、これは血清PCSK9タンパク質レベル、血清LDLレベル、及び総コレステロールレベルのかなりの持続的な減少を伴った。本発明者らは、これらの実験で投与されたAAVの用量が比較的多いことを認め、有効性及び安全性プロファイルの両方を決定するために、追加のPCS7-8メガヌクレアーゼを用いた更なる実験を大幅に少ない用量で実施する。総コレステロールレベル、血清LDLレベル、及び肝細胞LDLレベルの更なる分析も、コレステロールを減少させるこの方法の前臨床検証のために実施する。

- 前記mRNAは、請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼ及び少なくとも1つの更なるポリペプチドをコードする多シストロン性mRNAである、請求項17に記載のポリヌクレオチド。
- 請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼをコードする核酸配列を含む、組換えDNA構築物。
- 前記組換えDNA構築物は、プロモーターと、請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼ及び少なくとも1つの更なるポリペプチドをコードする多シストロン性核酸配列とを含むカセットを含み、前記プロモーターは、前記多シストロン性核酸配列の発現を駆動し、標的細胞に多シストロン性mRNAを産生する、請求項19に記載の組換えDNA構築物。
- 前記組換えDNA構築物は、請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼをコードする前記核酸配列を含むウイルスベクターをコードする、請求項19に記載の組換えDNA構築物。
- 前記ウイルスベクターは、レトロウイルスベクター、アデノウイルスベクター、レンチウイルスベクター、又は組換えアデノ随伴ウイルス(AAV)ベクターである、請求項21に記載の組換えDNA構築物。
- 前記ウイルスベクターは組換えAAVベクターである、請求項21又は22に記載の組換えDNA構築物。
- 請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼをコードする核酸配列を含むウイルスベクター。
- 前記ウイルスベクターは、レトロウイルスベクター、アデノウイルスベクター、レンチウイルスベクター、又はAAVベクターである、請求項24に記載のウイルスベクター。
- 前記ウイルスベクターは組換えAAVベクターである、請求項24又は25に記載のウ
イルスベクター。 - 前記ウイルスベクターは、プロモーターと、請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼ及び少なくとも1つの更なるポリペプチドをコードする多シストロン性核酸配列とを含むカセットを含み、前記プロモーターは、前記多シストロン性核酸配列の発現を駆動し、標的細胞に多シストロン性mRNAを産生する、請求項24~26のいずれか1項に記載のウイルスベクター。
- 薬学的に許容される担体と、
(a)請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼをコードする核酸、または
(b)請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼと
を含む医薬組成物。 - 前記操作されたメガヌクレアーゼをコードする前記核酸は、請求項17又は18に記載の前記mRNAである、請求項28に記載の医薬組成物。
- 前記医薬組成物は、請求項19~23のいずれか1項に記載の前記組換えDNA構築物を含む、請求項28に記載の医薬組成物。
- 前記医薬組成物は、請求項24~27のいずれか1項に記載の前記ウイルスベクターを含む、請求項28に記載の医薬組成物。
- 前記医薬組成物は、請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼを含む、請求項28に記載の医薬組成物。
- 前記医薬組成物は、脂質ナノ粒子内にカプセル化された請求項17又は18に記載の前記mRNAを含む、請求項28又は29に記載の医薬組成物。
- 被験体のPCSK9の発現を減少させる方法であって、前記方法は、前記被験体の標的細胞に、
(a)有効量の請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼをコードする核酸であって、前記操作されたメガヌクレアーゼは前記標的細胞で発現される、核酸、又は
(b)有効量の請求項1~15のいずれか1項に記載の前記操作されたメガヌクレアーゼ
を送達することを含み、前記操作されたメガヌクレアーゼは、前記標的細胞の配列番号4を含む認識配列を認識し切断し、対照細胞と比較してPCSK9の発現を減少させる、方法。 - 前記被験体は、高コレステロール血症を有する、請求項34に記載の方法。
- 前記被験体は、家族性高コレステロール血症を有する、請求項34又は35に記載の方法。
- 前記被験体は、常染色体優性家族性高コレステロール血症を有する、請求項34~36のいずれか1項に記載の方法。
- 前記方法は、請求項28~33のいずれか1項に記載の前記医薬組成物を前記被験体に投与することを含む、請求項34~37のいずれか1項に記載の方法。
- 前記医薬組成物は、請求項26に記載の前記組換えAAVベクターを含む、請求項38に記載の方法。
- 前記医薬組成物は、脂質ナノ粒子内にカプセル化された請求項17又は18に記載の前記mRNAを含む、請求項38に記載の方法。
- 前記標的細胞は肝細胞である、請求項34~40のいずれか1項に記載の方法。
- 前記肝細胞は肝実質細胞である、請求項41に記載の方法。
- 前記被験体は、ヒト又は非ヒト霊長類である、請求項34~42のいずれか1項に記載の方法。
- 前記細胞表面上におけるLDL受容体のディスプレイは、ベースラインLDL受容体レベルと比較した場合、肝細胞において増加する、請求項34~43のいずれか1項に記載の方法。

- 血清総コレステロールレベルは、PCSK9の発現減少後、前記被験体において減少する、請求項34~45のいずれか1項に記載の方法。
- 血清総コレステロールレベルは、前記ベースライン血清総コレステロールレベルと比較した場合、
(a)少なくとも約5%、10%、20%、30%、40%、50%、60%、70%、80%、90%、95%、若しくは最高100%、又は
(b)5~15mg/dL、10~20mg/dL、10~30mg/dL、15~30mg/dL、20~30mg/dL、25~35mg/dL、25~40mg/dL、25~50mg/dL、40~60mg/dL、50~70mg/dL、60~80mg/dL、若しくは70~100mg/dL減少する、請求項46に記載の方法。 - 血清LDLコレステロールレベルは、治療後、前記被験体において減少する、請求項34~47のいずれか1項に記載の方法。
- 血清LDLコレステロールレベルは、前記ベースライン血清LDLコレステロールレベルと比較した場合、
(a)少なくとも約5%、10%、20%、30%、40%、50%、60%、70%、80%、90%、95%、若しくは最高100%、又は
(b)5~15mg/dL、10~20mg/dL、10~30mg/dL、15~30mg/dL、20~30mg/dL、25~35mg/dL、25~40mg/dL、25~50mg/dL、40~60mg/dL、50~70mg/dL、若しくは60~80mg/dL減少する、請求項48に記載の方法。
