本発明は、HER2/Neu (ERBB2)発現がん細胞を処置するための組成物および方法に関する。いくつかの態様において、本発明は、標的細胞上のERBB2369~377エピトープに高い親和性を有し、かつそれに特異的に結合する単離されたT細胞受容体(TCR)を含む。他の態様は、ERBB2特異的TCRを発現するように改変されたT細胞またはT細胞集団を含む。さらなる態様は、ERBB2発現がん細胞を処置するためにERBB2特異的TCR遺伝子移入を使用する方法を含む。
本発明は、標的細胞上の表面抗原に高い親和性を有し、かつそれに特異的に結合する精製されたT細胞受容体(TCR)に関する。1つの態様において、TCRはチロシン-タンパク質キナーゼHER2/Neu (ERBB2)特異的TCRである。別の態様において、ERBB2特異的TCRは、SEQ ID NO: 2または6を含むTCRα鎖およびSEQ ID NO: 4または7を含むTCRβ鎖より選択される少なくとも1つのTCR鎖を含む。
1つの態様において、本発明の改変T細胞は、改変T細胞が共刺激分子を発現するような、共刺激分子をコードする核酸をさらに含む。ある特定の態様において、共刺激ドメインは、CD3、CD27、CD28、CD83、CD86、CD127、4-1BB、4-1BBL、PD1およびPD1Lより選択される。
細胞に核酸を導入する方法には、物理的方法、生物学的方法および化学的方法が含まれる。DNAまたはRNAのような、ポリヌクレオチドを宿主細胞に導入するための物理的方法には、リン酸カルシウム沈殿、リポフェクション、粒子衝撃、マイクロインジェクション、エレクトロポレーションなどが含まれる。エレクトロポレーション(Amaxa Nucleofector-II (Amaxa Biosystems, Cologne, Germany))、(ECM 830 (BTX) (Harvard Instruments, Boston, Mass.)またはGene Pulser II (BioRad, Denver, Colo.)、Multiporator (Eppendort, Hamburg Germany)を含む市販の方法を用いて、RNAを標的細胞に導入することができる。リポフェクションを用いたカチオン性リポソーム媒介トランスフェクションを用いて、ポリマーカプセル化を用いて、ペプチド媒介トランスフェクションを用いて、または「遺伝子銃」のような微粒子銃粒子送達系を用いて、RNAを細胞に導入することもできる(例えば、Nishikawa, et al. Hum Gene Ther., 12(8):861-70 (2001)を参照のこと)。
増大の前に、T細胞の供給源が対象から得られる。対象の非限定的な例としては、ヒト、イヌ、ネコ、マウス、ラット、およびそれらのトランスジェニック種が挙げられる。好ましくは、対象はヒトである。T細胞は、末梢血単核細胞、骨髄、リンパ節組織、脾臓組織、臍帯および腫瘍を含む、いくつかの供給源から得ることができる。ある特定の態様において、当技術分野において利用可能な任意の数のT細胞株が用いられうる。ある特定の態様において、T細胞は、フィコール(Ficoll)分離のような、当業者に公知の任意の数の技法を用いて、対象から収集された血液の単位から得ることができる。1つの態様において、個体の循環血液からの細胞は、アフェレーシスまたは白血球除去輸血によって得られる。アフェレーシス生成物は、典型的には、T細胞、単球、顆粒球、B細胞、他の有核白血球、赤血球および血小板を含めて、リンパ球を含む。アフェレーシスによって収集された細胞を洗浄して、血漿画分を除去し、リン酸緩衝生理食塩水(PBS)のような、適切な緩衝液もしくは培地またはその後の加工処理段階のため、カルシウムを欠くかつマグネシウムを欠きうるか、もしくは全部ではないが多くの二価陽イオンを欠きうる洗浄溶液の中に細胞を配してもよい。洗浄後、細胞は、例えば、Ca不含、Mg不含PBSのような、種々の生体適合性緩衝液に再懸濁されうる。あるいは、アフェレーシスサンプルの望ましくない成分が除去され、細胞は培地に直接再懸濁されうる。
1つの態様において、T細胞を増大させることは、T細胞を培養することをさらに含む。別の態様において、増大させるT細胞の供給源は、末梢血単核細胞である。
本明細書において記述される改変T細胞は、対象の処置のための組成物中に含まれうる。組成物は、薬学的組成物を含み、薬学的に許容される担体をさらに含みうる。改変T細胞を含む薬学的組成物の治療的有効量が投与されうる。
本発明の薬学的組成物は、本明細書において記述される改変T細胞集団を、1つまたは複数の薬学的にまたは生理学的に許容される担体、希釈剤または賦形剤と組み合わせて含みうる。そのような組成物は、中性緩衝生理食塩水、リン酸緩衝生理食塩水などのような緩衝液; グルコース、マンノース、スクロースまたはデキストランのような炭水化物、マンニトール; タンパク質; ポリペプチドまたはグリシンのようなアミノ酸; 抗酸化剤; EDTAまたはグルタチオンのようなキレート剤; アジュバント(例えば、水酸化アルミニウム); および保存料を含みうる。本発明の組成物は、好ましくは、静脈内投与のために製剤化される。

レトロウイルスパッケージングは、不死化正常胎児腎293GP細胞(Center of Cancer Research, National Cancer Institute, Bethesda, MD)において実施された。ヒト細胞株: 卵巣がん細胞株SKOV3、OVCAR3、OVCAR-2およびOV55-2、ヒト乳がん細胞株MDA231、黒色腫細胞株624および938、ヒトT細胞リンパ芽球性リンパ腫細胞株SupT1、ならびにT2リンパ芽球様細胞株。細胞株は、10% (体積比)熱不活性化ウシ胎児血清(FBS)、2 mmol/lのL-グルタミン、100 μg/mlのペニシリンおよび100 U/mlのストレプトマイシンを補充したRPMI-1640 (Invitrogen)中で維持された。全ての細胞株は、マイコプラズマ汚染がないか日常的に試験された。
全ての患者は、ペンシルバニア大学病院のアフェレーシスユニット(Apheresis Unit)において単球濃縮のセッティングを用いBaxter CS3000で初期白血球除去輸血を受けた。白血球除去輸血および洗浄を組み合わせてワクチン接種後の患者から末梢血単球を得た。単球を洗浄し、計数し、滅菌24ウェルプレート中にて10%ウシ胎仔血清(FBS)、500 IU/mlの組み換え研究等級のヒト顆粒球マクロファージコロニー刺激因子(GM-CSF)および250 IU/mlのインターロイキン-4 (IL-4)を補充したRPMI培地中3×106個/mlで4日間培養した。5日目に、1,000単位/ mlのIFN-γを培養物に添加し、続けて37℃で終夜インキュベーションを行った。6日目に、LPSを6時間10 ng/mlで添加して、樹状細胞の成熟を完了させた。樹状細胞のDC1表現型を、CD80、CD86、CD83およびCD40に対するモノクローナル抗体を用いたフローサイトメトリーによって分析した。その後、DC1の半分にHLAクラスI結合ERBB2369~377特異的ペプチドを、残りの半分にERBB2689~697ペプチドを2時間パルスした。2時間後に細胞を収集し、洗浄し、計数し、CD8+ T細胞との共培養の前に生存性について評価した。
自己ERBB2ペプチド負荷樹状細胞を、カラム精製されたワクチン接種後CD8+ T細胞とともに、48ウェルプレート中にて10:1の比率で共培養した。2日目に培養物にIL-2 (50 IU/ml)を添加した。感作の10日後、CD8+ T細胞を収集し、関連のあるもしくは関連のないペプチドを用いてパルスしたT2細胞または腫瘍細胞株で再刺激した。24時間後に上清を収集し、ELISAにより分析した。
サイトカイン放出アッセイ法は、1ウェルあたり1×105個のT細胞を1×105個の腫瘍細胞またはペプチド負荷T2細胞と96ウェル丸底プレート中3つ組で完全培地200 μl中で共培養することにより行った。ペプチド負荷T2 APCの調製のため、後者を1×107個/mlで再懸濁し、37℃で2時間、さまざまなペプチド濃度(1 ng/ml~10 μg/ml)でERBB2またはMART1ペプチドを負荷した。次いで、T2細胞をPBSで2回洗浄し、10%熱不活性化FBSを補充したRPMI-1640により1×106個/mlで再懸濁した。20~24時間後、BioLegend ELISA MAX(商標) Deluxeキットを用いて、無細胞上清をIFN-γの存在についてアッセイした。
TCR遺伝子の配列を同定するために、T細胞クローンから単離されたRNAを用いて、CDR3を含むTCRα鎖およびTCRβ鎖の可変領域を増幅する5'-RACE-PCR (Kit)を実施した。RACE-PCR産物を配列決定した。TCRα鎖およびTCRβ鎖を2Aペプチドリンカー(TCRb-P2A-TCRa)によって連結し、MSCV長末端反復配列(LTR)を利用するベクターpMSGV [ネズミ幹細胞ウイルス(MSCV)に基づくスプライス-gagベクター]の誘導体であるレトロウイルスベクタープラスミドpMSGV1ベクターの骨格に完全な構築体をクローニングした(Cohen et al., J Immunol 175, 5799-5808, 2005)。
複製欠損レトロウイルスベクターは、既述のように産生した(Wargo et al., cancer immunology, immunotherapy : CII 58, 383-394, 2009)。簡潔に言えば、Lipofectamine 2000試薬(Invitrogen)およびOptimem培地(BD Biosciences)を用いて6ウェルプレート中1×106個の293-GP細胞(一過性ウイルス産生細胞)に、各構築体由来のレトロウイルスベクターDNA 1.5 μgおよびエンベロープDNA (RD114) 0.5 μgを同時にトランスフェクションした。18時間後に10% FBSを有するDMEMに培地を交換し、48時間の時点でウイルス上清を収集した。
初代ヒトCD8+ T細胞は、ペンシルベニア大学のHuman Immunology Coreから購入し、陰性選択によって白血球除去輸血後に健常ボランティアドナーから単離した。全ての試料は、大学施設審査委員会が承認したプロトコルのもとで収集し、各ドナーから書面による同意を得た。T細胞を24ウェルプレート(Costar)中1×106個/mlで、完全培地(10%熱不活性化FBS、100 U/mlのペニシリン、100 μg/mlの硫酸ストレプトマイシン、10 mM HEPESを補充したRPMI 1640)中にプレーティングし、形質導入の前に18~24時間、製造業者(Invitrogen)によって記述されているように抗CD3および抗CD28-mAbコーティングビーズで刺激した(Levine et al., J Immunol 159, 5921-5930, 1997)。レトロウイルス形質導入のため、非組織培養処理した12ウェルプレート(Becton Dickinson Labware, Franklin Lakes, NJ)を、製造業者(RetroNectin, Takara, Otsu, Japan)によって記述されているように4℃で25 μg/mlの組み換えレトロネクチンで処理した。終夜インキュベーションの後、レトロネクチンを除去し、ウェルを室温で30分間PBS中2%のBSAによりブロッキングした。次いで、レトロウイルスベクター上清(2~3 ml)を遠心分離(2時間2000×g)によって適用し、吸引によって除去した。刺激されたT細胞5×105個を、RMPI増殖培地1 mlの最終容量中で各ウェルに添加した。プレートを1000×gで10分間遠心分離し、終夜インキュベートした。翌日、形質導入プロセスを繰り返した。形質導入後、10% FBSを有するRPMI中で細胞を増殖させ、ヒト組み換えインターロイキン-2 (IL-2) (Novartis)を1日おきに100 IU/mlの終濃度まで添加した。0.5~1×106個の細胞/mlの細胞密度を維持した。
T細胞抗原特異性を判定するため、抗CD8-FITCおよびアロフィコシアニン(APC)標識ERBB2369~377またはMART127~35四量体(Becton Dickinson, San Jose, CA)でCD8+ T細胞を染色した。T細胞活性化表現型を評価するため、T細胞を上記の試薬にPerCPCy5.5標識抗ヒトCD69 mAbを加えたもので染色した。CD14-PerCPCy5.5、CD11c-APC、HLA-DR-PE、CD80-FITC、CD86-FITC、CD83-FITCおよびCD40-FITCを用いて、樹状細胞の表現型を評価した。全ての抗体はBD Biosciencesから購入した。
RT-PCRを用いて、腫瘍細胞株におけるヒトTAP1、TAP2、タパシン、LMP2 (APM成分)の発現を分析した。RNA easyキット(Qiagen)を用いて、RNAをまず腫瘍細胞から単離した。次いで、First Strand Ready-To-Goビーズ(GE Healthcare)を用いてRNA 1 μgからcDNAを生成した。次いで、TAP1、TAP2、タパシン、LMP2およびβ-アクチンに特異的なApplied Biosystemのタックマン(taqman)プライマーを用いて、リアルタイムPCRを3つ組で行った。mRNAレベルをβ-アクチンに対して正規化し、APM欠損T2細胞のmRNAレベルと比較した。データはmRNAレベルの倍数として提示される。
全ての動物は、ペンシルベニア大学のAbramson Cancer Centerの幹細胞および異種移植コア(Stem Cell and Xenograft Core)から得た。マウスは、ペンシルベニア大学IACUC承認プロトコルのもと病原体のない条件下にて組織内で飼育され、処置され、維持された。インビボT細胞機能評価のため、6~12週齢の雌性NSGマウスの脇腹に、PBS 0.2 ml中1×106個のERBB2特異的T細胞と予め混合された1×106個のMDA231細胞を皮下注射した。対照マウスに、1×106個のMART1特異的T細胞と混合されたMDA231腫瘍細胞を注射した。各群は5匹のマウスからなっていた。キャリパー測定によって経時的に腫瘍成長を判定し、長さが最大縦径であり、幅が最大横径である式V=1/2(長さ×幅2)を用いて腫瘍体積を計算した。マウスは40日後に、または苦しみ瀕死になった場合はそれよりも早く絶命(terminate)させた。絶命(termination)後、腫瘍を切除し、撮影し、秤量した。
GraphPad Prism 4.0 (GraphPadソフトウェア)を統計分析に用いた。

HLAクラスI拘束ERBB2369~377ペプチドを含む、HLAクラスIおよびクラスIIペプチドのカクテルを用いてパルスした自己樹状細胞(DC)で予めワクチン接種されていたHLA-A2+患者(M10)から末梢血単球および末梢血T細胞を得た(Czerniecki et al., Cancer Res 67, 1842-1852, 2007)。この患者のワクチン接種後のCD8 + T細胞は、ERBB2369~377ペプチドを用いてパルスした自己DCに対するおよびHLA-A2+/ERBB2+乳がん細胞株MDA231に対する強いIFN-γ応答を示した。注目すべきことに、患者のワクチン接種前のCD8+ T細胞はいずれかの標的に対して低レベルのIFN-γを示し、強力なワクチン誘導抗ERBB2応答の証拠を確立するものであった。患者の末梢血単球は、インビトロ・プロトコルを利用してDCに成熟され、CD80、CD86、CD83およびCD40の比較的高い発現レベルを示した(図6)。次いで、成熟したDCをERBB2369~377ペプチドを用いてパルスし、患者のワクチン接種後の末梢血から精製されたCD8+ T細胞のインビトロ刺激のために用いた。インビトロ刺激の7日後、生存CD8+ T細胞集団のほぼ3%が、HLA-A2/ERBB2369~377四量体の結合によって評価した場合に刺激性ERBB2369~377ペプチドを認識した(図1)。これは、ワクチン接種後の患者の血液中で観察されたERBB2特異的T細胞の開始時の割合と比べて、1週間の間に17倍の増加を示した。ERBB2特異的T細胞はMART-126~35四量体複合体に結合せず、ERBB2369-377ペプチドに対するそれらの特異性を実証した。対照的に、MART-1 TCR形質導入T細胞は、ERBB2369~377四量体複合体に結合しなかったが、MART-126~35四量体複合体への強い結合を示した(図1)。まとめると、ERBB2ペプチド負荷DCは、ERBB2369~377ペプチド特異的T細胞の頻度を高めることができた。
ERBB2特異的T細胞を、そのエフェクタ機能を評価するために、最初にERBB2369~377ペプチドを予め負荷したHLA-A2+ T2細胞に曝露した。ERBB2特異的T細胞は、関連するERBB2ペプチドを負荷した抗原提示細胞(T2細胞)との共培養の際に、高いペプチド特異的IFN-γ産生を呈した。予想通り、無関係のMART-126~35ペプチドを用いてパルスしたT2細胞への曝露でIFN-γは産生されなかった。機能性の陽性対照として、MART-1特異的T細胞はMART-126~35ペプチド負荷T2細胞を認識し、かつ該細胞に対して反応した(図2A)。
T細胞による腫瘍の認識は、早期活性化マーカーCD69のようなT細胞活性化表面抗原の特異的な上方制御を伴うことが多い。腫瘍提示されたERBB2369~377ペプチドに対する高い結合活性を有するERBB2369~377特異的T細胞を捕捉するために、ERBB2特異的T細胞をHLA-A2+ ERBB2+ MDA231細胞とともに24時間共培養した。CD69 (図7)および結合HLA-A2/ERBB2369~377四量体を上方制御させたERBB2特異的T細胞を次に、蛍光活性化流動選別(FACS)によって単離した。捕捉されたERBB2特異的T細胞のTCR可変(TCRV)α鎖およびTCRVβ鎖レパートリーを判定するために、全RNAを選別細胞から単離し、5' RACEに供した。23個の個別α鎖cDNAクローンおよび14個の個別β鎖cDNAクローンを、2つの独立したPCR反応から完全に配列決定した。配列データは、β鎖のBV3-1 (9S1)ファミリーに属するTCRVβレパートリーにおける2つの比較的優性な配列を示した。TCRVαレパートリーではAV3およびAV12-1α鎖についてそれぞれ2つの反復を伴って、より多くの異種性が観察された(表1)。
次に、TCR7 (SEQ ID NO: 5、6および7)が初代ヒトT細胞における発現で付与する機能的特性を調べた。CD8+ T細胞へのレトロウイルスTCR遺伝子移入は、特異的HLA-A2/ERBB2369~377四量体結合を生じた(図3B)。しかしながら、SupT1細胞と比較した場合、四量体+細胞の割合は低く(およそ10%)、形質導入されたTCRは、それらを検出できるようにアセンブルされえないこと、または内因性α鎖との誤対合が起こりえたことを示唆するものであった。重要なことに、低い四量体結合頻度でさえ、ERBB2 TCR7形質導入T細胞は、ペプチドをパルスしたAPC標的に対して特異的で強固な反応性を示した(図4A)。ERBB2 TCR T細胞は、1 ng/mlという低いペプチド濃度で高いIFN-γレベルを分泌したことから、高いペプチド結合活性を示した(図4C)。ERBB2 TCR CD8+ T細胞の腫瘍反応性を分析することにより、HLA-A2+ ERBB2+ OVCAR-2およびMDA231腫瘍細胞に応答して、最初のERBB2ポリクローナルT細胞集団によって産生されたものと同様のレベルでIFN-γ分泌が観察された(図2Cおよび図4B)。HLA-A2もしくはERBB2発現を欠く腫瘍または非常に低レベルのERBB2を発現するHLA-A2+ 624黒色腫細胞に対して反応性は観察されなかった(図4B)。
インビボでERBB2369~377特異的TCR7を発現するT細胞の抗腫瘍効果を判定するため、等数のTCR7-または対照MART-126~35 TCR-形質導入CD8+ T細胞およびMDA231腫瘍細胞を、NOD/SCID/IL2-γc null (NSG)マウスの皮下に同時注射し、腫瘍成長をモニターした。MDA231腫瘍は注射から14日後に侵襲的に成長し、触診できる腫瘍が明白であった。MART-1 TCR特異的T細胞と比較して、ERBB2 TCR7形質導入T細胞は、経時的に腫瘍負荷を有意に遅延させることができた(図5A)。研究の終了時に、マウスを安楽死させ、腫瘍を摘出した。ERBB2 TCR7群からの切除腫瘍は、測定された腫瘍体積(図5A)と一致して、目に見えるほど小さく(図5B)、MART-1 TCRで処置したマウスにおけるものと比較して重量が有意に少なかった(図5C)。
腫瘍特異的TCR遺伝子の導入は、腫瘍反応性T細胞を単離する必要なく、がん免疫療法用の新規抗腫瘍リンパ球を産生する方法として提案されている(Cordaro et al., J Immunol 168, 651-660, 2002; Sadelain et al., Nat Rev Cancer 3, 35-45, 2003; Schumacher, Nat Rev Immunol 2, 512-519, 2002; Willemsen et al., Hum Immunol 64, 56-68, 2003)。この提案では、多様なヒトがんに共通する腫瘍抗原の存在、およびこれらの天然腫瘍抗原を認識する適切なT細胞集団からの腫瘍反応性TCRの単離を必要とする。その発見以来、合成ERBB2369~377ペプチドは、インビトロでの刺激(Anderson et al., Clin Cancer Res 6, 4192-4200, 2000; Brossart et al., Cancer Res 58, 732-736, 1998; Keogh et al., J Immunol 167, 787-796, 2001; Liu et al., Cancer Res 64, 4980-4986, 2004; Rongcun et al., J Immunol 163, 1037-1044, 1999; Seliger et al., Int J Cancer 87, 349-359, 2000; zum Buschenfelde et al., Cancer Res 62, 2244-2247, 2002)またはワクチン接種(Brossart et al., 2000; Knutson et al., 2002; Murray et al., 2002; Peoples et al., J Clin Oncol 23, 7536-7545, 2005; Zaks and Rosenberg, Cancer Res 58, 4902-4908, 1998)後のERBB2特異的CTLのエクスビボおよびインビボ生成について広く調べられている。いくつかのERBB2特異的T細胞は、ERBB2ペプチドに対して高い反応性を発揮するとはいえ、ERBB2+腫瘍によって提示される内因性にプロセッシングされたペプチドを認識することはできないが(Conrad et al., J Immunol 180, 8135-8145, 2008; Zaks and Rosenberg, , Cancer Res 58, 4902-4908, 1998)、最近の研究では、ERBB2369~377特異的T細胞が重複性HLAクラスI拘束ERBB2373~382ペプチドと交差反応することが実証されている(Henle et al., J Immunol 190, 479-488, 2013)。重要なことに、ERBB2373~382は天然にプロセッシングされ、ERBB2373~382特異的T細胞もERBB2369~377ペプチドと交差反応する(Henle et al., J Immunol 190, 479-488, 2013)ことから、その天然のプロセッシングに関する論争は存在するものの、ERBB2369~377ペプチドに対する継続的な臨床重要性が示唆された。
Claims (18)
- SEQ ID NO: 2または6を含むT細胞受容体(TCR)α鎖およびSEQ ID NO: 4または7を含むTCRβ鎖からなる群より選択される少なくとも1つのTCR鎖を含むチロシン-タンパク質キナーゼHER2/Neu (ERBB2)特異的TCRである、標的細胞上の表面抗原に対する親和性を有する精製されたTCR。
- SEQ ID NO: 9である369~377番目のアミノ酸を含むERBB2受容体タンパク質のエピトープ(ERBB2369~377)に結合する、請求項1記載の精製されたTCR。
- 標的細胞がHLA-A2+である、請求項1記載の精製されたTCR。
- 少なくとも1つのジスルフィド結合を含む、請求項1記載の精製されたTCR。
- TCRα鎖およびβ鎖がペプチドリンカーによってつながれている、請求項1記載の精製されたTCR。
- T細胞受容体(TCR)が、SEQ ID NO: 1を含むTCRα鎖をコードする核酸、SEQ ID NO: 3を含むTCRβ鎖をコードする核酸ならびにSEQ ID NO: 5を含む連結されたTCRα鎖およびβ鎖をコードする核酸からなる群より選択される少なくとも1つの核酸配列によってコードされるチロシン-タンパク質キナーゼHER2/Neu (ERBB2)特異的TCRである、標的細胞上の表面抗原に対する親和性を有するTCRをコードする精製された核酸配列。
- TCR鎖のうちの少なくとも1つのヌクレオチド配列がコドン最適化されている、請求項6記載の精製された核酸。

- 請求項10記載の組み換え発現ベクターを含む、改変された哺乳動物細胞。
- 末梢血単核細胞、臍帯血細胞、初代T細胞、およびT細胞株細胞からなる群より選択される、請求項11記載の改変された哺乳動物細胞。
