Abstract
Description
本発明は一般に、T細胞受容体、および複数の別個の抗原決定基、例えば2つの別個の腫瘍抗原決定基に指向されたキメラ抗原受容体をコードする核酸を含む幹細胞(例えば、iPSCまたはHSC)の集団に関する。本発明は、T細胞受容体および複数の別個の抗原決定基に指向されたキメラ抗原受容体、例えば2つの別個の腫瘍抗原決定基を共発現するT細胞の集団も対象とする。本発明の細胞は、そのHLA型が集団の有意なセクターに適合する選択されたドナーに由来し得、様々な用途で、特に新生物状態の治療的処置との関連で有用である。

本発明は、例えば、目的の抗原決定基に指向されたTCR特異性を呈するT細胞に由来するiPSCにCARカセットをトランスフェクトすることにより2つの別個の抗原決定基に指向されたTCR/CAR二重発現T細胞を、一貫してかつ安定して生成できるという決定に一部基づく。後成的記憶の作用によって、このiPSCから分化したT細胞が、iPSCが由来した体細胞性T細胞のTCR特異性および別個の抗原決定基に指向されたCARの両方を安定して発現することが見出された。追加の抗原決定基に対する特異性は、細胞にそのような追加の抗原決定基に結合する分子をコードする追加の核酸を導入することによって達成することができる。それによりそのような複数特異性細胞は、現在利用可能な細胞より有効な治療転帰を提供する。したがってこれらの決定は、新生物状態、ウイルス感染、細菌感染または自己免疫性状態など所望されない細胞の集団によって特徴付けられる任意の疾患状態との関連で使用するための、安定して形質転換された二重抗原特異的T細胞、特に細胞傷害性CD8+αβTCR T細胞の継続する供給の開発を今や可能にした。この発見およびそれに基づく細胞の生成は、そのような状態、特に、転移性疾患を含めた固形腫瘍または血液がん(例えば、白血病)などの新生物状態を処置することを目的とする治療的処置レジメの改善を今や容易にした。
(1)生物のMHCレパートリーは、通常多遺伝子性である(複数の、相互作用する遺伝子による);
(2)MHC発現は、共優性である(遺伝性対立遺伝子の両方の組による);
(3)MHC遺伝子バリアントは、高度に多様性である(種内の生物と生物の間で多様に異なる)。

(i)Cre-Loxに媒介される導入遺伝子切除を含む単一カセットリプログラミングベクター;
(ii)アデノウイルスまたはセンダイウイルスなどの非組み込み型ウイルスによるリプログラミングがあるが、これに限定されない。別法として、タンパク質としてのリプログラミング因子の発現は、生殖細胞系に導入されるベクターDNAの組み込みを受けないiPSCを生成する手段を提供する。
(i)mRNAトランスフェクション-mRNAとしてリプログラミング因子を発現する能力は、ウイルスベクターの染色体組み込みが起こらないiPSCを作製する方法を提供する。Warrenらは、mRNAを転写して、効率的にリプログラミング因子を発現させる(Warrenら(2010年))。Yamanakaリプログラミング因子プロトコールにLin28を加え、5% O2で培養し、細胞培養培地にバルプロ酸を含めることにより、効率を増加させることができる。リプログラミング因子mRNAは市販されている。
(ii)miRNA感染/トランスフェクション-いくつかのmiRNAクラスターは、胚性幹細胞において強く発現される。成熟miR-302bおよび/またはmiR-372+4つのレンチウイルスYamanaka因子の合成模倣体が、MRC5およびBJ-1線維芽細胞に添加される場合、4つのレンチウイルス因子単独と比較してリプログラミング効率に10倍から15倍の増加がある(Subramanyamら(2011年))。ある特
定のmiRNAが、Yamanaka因子なしで、高効率で細胞をリプログラムできることも見出された。
(iii)PiggyBac-PiggyBacは、トランスポーゼースの存在下で染色体TTAA部位に組み込まれ、トランスポーゼースの再発現によりその後ゲノムから切除され得る可動性遺伝的エレメント(トランスポゾン)である。piggyBacベクターにクローニングし、MEFに共トランスフェクトした場合、Yamanaka因子は、トランスフェクション後14から25日で細胞をリプログラムすることができる(Kajiら(2009年);Woltjenら(2009年))。piggyBacベクターは、トランス
ポーゼースの再発現によりiPSCから切除され得る。
(iv)小環状ベクター-小環状ベクターは、真核生物プロモーターおよび発現されることになるcDNAだけを含有する最小のベクターである。ヒト脂肪間質細胞において発現されるLin28、GFP、Nanog、Sox2およびOct4小環状ベクターは、細胞をリプログラムすることができる(Narsinhら(2011年))。
(v)エピソーム性プラスミド-エピソーム性プラスミドとしてのリプログラミング因子の一過性の発現は、iPSCの生成を可能にする。例えばoriP/EBNAベクターは、1つのカセット中のYamanaka因子+Lin28で構築することができ、別のoriP/EBNAベクターは、SV40ラージT抗原を含有する(Chuoら(2011年))。これらのベクターは、酪酸ナトリウムで補充された培地中でCD34+臍帯血、末梢血および骨単核細胞において発現され、14日目にiPSCコロニーを得られることが示された。トランスフェクトされたプラスミドは、最終的に失われる

遮断(Linら(2009年);Ichidaら(2009年))、後成的修飾因子の機能の増強
(Estebanら(2010年))、ROCK経路の阻害(Noggleら(2011年))ならび
に解糖の誘導(Zhuら(2010年))を含めたリプログラミングを容易にすることが可
能になる。これら小分子の1つ、ヒストン脱アセチル化酵素阻害剤であるバルプロ酸および酪酸ナトリウムは、リプログラミングプロトコールにおいて最も一般的に使用される。また、リプログラミング過程における5%酸素中での細胞の培養が、iPSC誘起の効率を増加させ得ることは注目すべきである(Yoshidaら(2009年))。リプログラムが
特に困難な細胞の場合、小分子の添加および低酸素状態での培養が、改善を生むことがある。別の選択肢は、胚性幹細胞馴化培地(ESCM)を使用して、内在性リプログラミング因子の発現を誘導することである(Balasubramanianら(2009年))。効率は、バ
ルプロ酸の添加によりさらに改善することができる。そのような戦略を使用して、外因的に導入されたリプログラミング因子の能力を増強させ、それによりリプログラミング効率を増加させることもできる。

血液由来がんペプチド抗原特異的T細胞の濃縮
WT-1特異的TCR T細胞刺激および増大
WT-1特異的T細胞は、正常ヒト血液中では非常に稀であるが、検出するために増大および濃縮できる。これに関連して、末梢血単核細胞(PBMC)は、Ficoll-Hypaque密度勾配遠心分離を使用して単離した。新鮮分離したPBMCを、ヒトAB血清を補充した組織培養培地中に再懸濁し、WT-1が存在する場合にT細胞の共刺激因子として作用する、L-グルタミンおよびCD28モノクローナル抗体を添加した;抗CD28単独は、T細胞を活性化しない。次いでPBMCをウイルムス腫瘍1(WT-1)ペプチド、主なHLAクラスI結合モチーフを表す4つのWT-1ペプチド:WT-137(VLDFAPPGA、配列番号22)、WT-1126(RMFPNAPYL、配列番号23)、WT-1187(SLGEQQYSV、配列番号24)およびWT-1235(CMTWNQMNL、配列番号25)のそれぞれについて0.6nmol/mlを用いて一晩刺激した。本出願の実施例に示すデータは、WT-1ペプチド1-37をWT-1ペプチドのこのファミリーの代表として使用した。WT-1特異的T細胞は、HLA WT-1特異的四量体を用いて、または刺激されたが静止していないT細胞上での表面分子CD137の早期誘導によって同定できる。CD137は、腫瘍壊死因子(TNF)受容体ファミリーのメンバーである。それは4-1BBとしても公知である。24~36時間後、CD137陽性細胞(これは、WT-1で刺激されたT細胞である)を磁気細胞分離機を使用して磁気的に分離した。CD137陽性(WT-1特異的TCR)細胞を、ヒトAB-血清、組換えインターロイキン7、インターロイキン15およびインターロイキン21を補充したX-Vivo-15ベース培地からなるT細胞増大培地中で培養した。対応するCD137陰性細胞をCD3磁気分離にさらに供した。CD3陰性細胞(主にB細胞)をマイトマイシンC処置に供し、CD137陽性集団を誘導するためにWT-1ペプチドロード抗原提示フィーダー細胞として使用し、一方残りのCD3陽性細胞(非WT-1特異的)を下流の機能アッセイのための対照T細胞型として作用させるために培養で成長させた。組換えサイトカインを含む培地を1日おきに継ぎ足した。

ヒト血液T細胞由来iPSCの生成
ヒト血液T細胞からのiPSCの誘導のために、元来のT細胞の特性を様々なレベルで正確に維持する多数のアプローチがある。iPSCは、健常なヒト由来の末梢血Tリンパ球プール(T-iPSC)の幅広いレパートリーから産生された。T細胞は、例えば、マイトジェンPHAまたは抗CD3および抗CD28抗体を用いて予め活性化した。それぞれ2個のヤマナカリプログラミング因子(Oct4、Sox2、KLF、cMyc)を含有する二重レトロウイルスベクターカセットを使用して、複数のT-iPSCクローンを生成し、Nanog、Oct3/4、SSEA3,4、TRA-1-60およびTRA-1-81を含む広範なマーカーについてのフローサイトメトリーおよびqRT-PCRを含めて、細胞および分子レベルで検証した。それらの多能性をNOD-SCID-IL共通ガンマ鎖-/-(NSGマウス)への注射後のテラトーマ形成によって確認した。T細胞源の確認は、再構成されたTCR遺伝子を示すことによって確認した。
iPSC由来ヒトT細胞の誘導
本実施例は、iPSC由来の真正のT細胞の生成を示している。これらのT細胞は、胸腺によって通常産生されるような典型的T細胞の重要な特性を発現することが示された。それらは、メインストリームT細胞αβTCRならびにβおよびα鎖の両方を有するCD8を発現することが示された。
-8日目:マイトマイシン処置マウス胚性線維芽細胞フィーダー層を0.1%ゼラチンコートTCプレートに3mLのMEF培地(DMEM+15%FCS+1%pen/strep L-グルタミン)中、0.3×106(細胞14,250個/cm2)で蒔き、一晩インキュベートした。OP9細胞を0.1%ゼラチンコート10cm TCプレートにOP9培地(αMEM+20%FCS+1%pen/strep)11mL中細胞0.25×106個で蒔くことによって予備調製した。
0日目:造血特殊分化の開始。iPSコロニーを分離させ、HSC分化のためにOP9に蒔いた。コロニー懸濁物をOP9プレートへの均一な分配のために滴下で添加した。新鮮分化培地を1、5および9日目に添加した。
13日目:T細胞分化の誘導:13日目のOP9馴化(造血誘導された)細胞のOP9DL-L1細胞への移行
発現した。さらにこれらの細胞は、他の報告された系での30日目と比較してOP9 DL-L1細胞上での培養16日目には早くも存在していた。
CAR構築物の開発
キメラ抗原受容体(CAR)-T細胞の構成要素は、CD8またはCD28ヒンジによってアンカリングされ膜貫通(TM)領域を含む1本鎖Fv(scFv)によって表されるscFv外部ドメインによって媒介されるCARの抗原認識構成要素、およびCD28、4-1BBおよびCD3ゼータ(CD3ζ)鎖によって表される細胞質性エンドドメインを介するCARのシグナル伝達である。2つの好適なウイルス送達系-レトロウイルスおよびレンチウイルスもある。例示的CARおよびCD47結合受容体構築物を図11に示す。
キメラ抗原受容体ベクタークローニング戦略
例示的キメラ抗原受容体ベクタークローニング戦略を図12~13に例示する。図14は、本発明者らの第2世代CARおよび非シグナル伝達抗CD47構築物のための戦略を示している。キメラ抗原受容体、非シグナル伝達抗原結合受容体およびそれらの種々のドメインの例示的配列を配列番号1~20に提供する。
T細胞のキメラ抗原受容体形質導入
レンチウイルス産生
293T細胞をポリ-L-リシン(Sigma)コート175cm2フラスコに蒔いた。トランスフェクションの2時間前に培地を10%FCSを補充したDMEMで置き換えた。レンチウイルス移入ベクターDNAを、パッケージングおよびエンベローププラスミドDNAと合わせ、リポフェクタミン2000と混合した。溶液を短時間ボルテックスし、室温、30分間インキュベートした。これに続いて溶液を再度混合し、次いで細胞に滴下で添加した。フラスコをインキュベーターに戻した。6時間後、新鮮成長培地を添加した。48時間後にウイルス上清を回収し、1500rpm、5分間、4℃での遠心分離によって透明化し、次いで0.45μmポアPVDF Millex-HV filter(Millipore)に通した。超遠心分離法を使用するレンチウイルスの濃縮をAH-629ローターを使用してSorval Discovery 100 SE遠心分離機を用いて実施した。ろ過したウイルス上清30mLを36mLポリアロマーコニカルチューブ(Beckman)に添加した。遠心分離を90分間、20,000gで実施した。上清を完全に除去し、ウイルス沈殿を300μL PBSに再懸濁し、使用まで-80℃で保存した。
図11および配列番号1~6は、開発したキメラ抗原受容体(CAR)およびCD47結合受容体構築物のパネルを、TAG72またはCD19(陽性対照として)のいずれかに特異的なscFvと共に示している。これらの構築物は、ヒンジ領域としてヒトCD8またはCD28のいずれかおよびCD28、CD3ζ鎖または4-1BB細胞質性活性化シグナル伝達ドメインを使用する。CARおよびCD47結合受容体構築物を上の章に記載のレンチウイルスベクターにクローニングする。
TAG72 CAR-T細胞(正常PBMCから作った)を、TAG72発現標的がん細胞をin vitroで死滅させるそれらの能力について検討した。リアルタイム細胞モニタリングシステム(xCELLigence)をin vitroでのCAR-T細胞の死滅効率を決定するために用いた。標的細胞(例えばTAG72+卵巣がん細胞株CaOV4)10,000~2×106個/100μLをRTCAプレートに入れた。いくつかの場合では、抗hCD40による、もしくはプレートのヒトフィブロネクチンプレコーティングによる標的細胞のテザリングは、必要である場合がある。細胞付着を可能にするように標的細胞を37℃、5%CO2で3~12時間維持する。標的細胞の付着に続いて、CAR-Tエフェクター細胞を種々のエフェクター:標的比で添加した(1:1から10:1の範囲)。一部の実験では、CAR-Tのエフェクター細胞は、CAR-T細胞のGFP発現に基づいて使用前にFACSを介して単離した。共培養は、最適な成長条件で少なくとも12時間維持された。細胞インピーダンスを全体を通じてモニターし;インピーダンスでの減少は、細胞脱離および最終的には細胞死の指標である。
iPSCのキメラ抗原受容体形質導入
多重特異性CAR-T細胞の産生は、既存の血液T細胞のCAR形質導入(図15)または次にT細胞に誘導されるiPSCの形質導入(がん特異的TCRおよびCARを発現している)(例えば、配列番号1~6)によるものを含む複数のアプローチによって達成できる。複数のiPSC株は、CAR-T形質導入を進行するために使用されている。これらのiPSCは、非T細胞由来、または、TCR遺伝子再構成を保持しているがん抗原特異的T細胞由来(例えば、WT-1)であってよい。これらのiPSCは、成体線維芽細胞由来または特異的がん抗原(WT-1ペプチド)に特異的な内在性TCRを有するT細胞由来のいずれかであった。
