GABAA受容体陽性アロステリック調節因子(PAM)を収納した容器と

用語「GABA受容体活性化リガンド」とは、1種または複数種のGABA受容体に対するアゴニストである薬剤をいう。特定の実施形態において、上記GABA受容体活性化リガンドは少なくともGABAA受容体に対するアゴニストである。
GABA受容体活性化リガンドは当業者に周知である。かかるリガンドとしては、例えば、GABA受容体の天然のリガンドであるガンマ-アミノ酪酸(GABA)が挙げられる。他のGABA受容体活性化リガンドとしては、ホモタウリン、バマルゾール、ガバミド、GABOB、ガボキサドール、イボテン酸、イソグアバシン、イソニペコチン酸、ムシモール、フェニバット、ピカミロン、プロガビド、キスカラミン、プロガビド酸(SL 75102)、チオムシモール、プレガバリン、ビガバトリン、6-アミノニコチン酸、XP13512((±)-1-([(α-イソブタノイルオキシエトキシ)カルボニル]アミノメチル)-1-シクロヘキサン酢酸)などが挙げられるが、これらに限定はされない。
GABAA受容体の陽性アロステリック調節因子(PAM)は当業者には周知である。例示的なPAMとしては、アルコール(例えば、エタノール、イソプロパノール)、アベルメクチン(例えばイベルメクチン)、バルビツレート(例えばフェノバルビタール)、ベンゾジアゼピン、臭化物(例えば臭化カリウム)、カルバメート(例えば、メプロバメート、カリソプロドール)、クロラロース、クロルメザノン、クロメチアゾール、ジヒドロエルゴリン(例えば、エルゴロイド(ジヒドロエルゴトキシン))、エタゼピン、エチフォキシン、イミダゾール(例えばエトミデート)、カバラクトン(カバに含まれる)、ロレクレゾール、神経活性ステロイド(例えば、アロプレグナノロン、ガナキソロン)、非ベンゾジアゼピン系(例えば、ザレプロン、ゾルピデム、ゾピクロン、エスゾピクロン)、ペトリクロラール、フェノール類(例えばプロポフォール)、ピペリジンジオン(例えば、グルテチミド、メチプリロン)、プロパニジド、ピラゾロピリジン(例えばエタゾレート)、キナゾリノン(例えば、メタクアロン)、スカルキャップの構成成分(例えば、バイカレインなどのフラボノイドを始めとする、但しこれらに限定されないScutellaria sp.の成分)、スチリペントール、スルホニルアルカン(例えば、スルホンメタン、テトロナール、トリオナール)、バレリアンの構成成分(例えば、吉草酸、バレレニン酸)、及びある種の揮発性物質/ガス(例えば、クロラール水和物、クロロホルム、ジエチルエーテル、セボフルラン)が挙げられるが、これらに限定はされない。種々の実施形態において、上記GABA受容体活性化リガンドと併用されるPAMからは、アルコール、及び/またはカバラクトン、及び/またはスカルキャップもしくはスカルキャップの構成成分、及び/またはバレリアンもしくはバレリアンの構成成分、及び/または揮発性ガスが除外される。
種々の実施形態において、1種または複数種のGABA受容体活性化リガンドと1種または複数種のGABAA受容体の陽性アロステリック調節因子とを含む医薬製剤が企図される。特定の実施形態において、上記GABA受容体活性化リガンドは、GABA受容体活性化リガンドを単独で含む治療用製剤よりも低い単位投与量で存在する、及び/または上記PAMは、上記PAMを単独で含む一般的な、及び/または認可された、及び/または推奨される治療用製剤中よりも低い単位用量で存在する。
特定の実施形態において、本明細書に記載の方法を実施するためのキットが提供される。種々の実施形態において、上記キットは、GABA活性化リガンドを収納した容器と、本明細書に記載のGABAA受容体の陽性アロステリック調節因子を収納した容器とを備える。特定の実施形態において、上記キットはGABA及びアルプラゾラムを備える。
この実施例において、本発明者らは、ベンゾジアゼピン アルプラゾラム(XANAX(登録商標))のGABAとの併用を、ベータ細胞の増殖及び/または生存に及ぼすその効果に関して検討した。アルプラゾラムは、不安症及びパニック障害の治療に広く処方されている。アルプラゾラムは1981年の導入後、急速に処方箋が5千万件を超える大ヒット薬物となった。アルプラゾラムは、指示通りに用いられれば長期間使用しても安全である(www.accessdata.fda.gov/drugsatfda_docs/label/2011/018276s045lbl.pdfにおけるFDA認可のラベル及びJonas and Cohon (1993) J. Clin. Psychiatry. 54 Suppl:25-45;discussion 6-8を参照のこと)。本発明者らは、アルプラゾラムを、1)その長期的安全性プロファイル、及び2)その適合するGABAA-Rサブユニットとの相互作用に基づいて、検討することを選択した。
GABAはヒトによる摂取に対して安全である。GABAは栄養補助食品として処方箋なしで販売されている。本発明者らは、26週間のGABA処理によって、脾臓単核細胞の総数またはCD4+、CD8+、T及びBリンパ球の割合が有意に変化しない(Tian et al. (2004) J. Immunol. 173(8): 5298-5304)だけでなく、自己反応性T細胞がGABA媒介性阻害に対して脱感作されることもない(Tian et al. (2004) J. Immunol. 173(8): 5298-5304;Tian (2011) Autoimmunity, 44: 465-470)ことを認めた。1950年代~1980年代に、ヒトによる治験において、長期間の経口投与でのGABAによる治療が、数百人の個人において、てんかん発作を低減する能力及び脳血管障害を改善する能力について試験された(Otomo et al. (1981) Arzneimittelforschung. 31(9): 1511-1523;Loeb et al. (1987) Epilepsy Res. 1(3): 209-212;Tower and Roberts eds. Inhibition in the Nervous System and GABA. New York: Pergamon Press, 1960;Kuriyama and Sze (1971) Neuropharmacol. 10(1): 103-108)。結果は、悪影響は示さなかったが臨床的利点も示さず、これはGABAが血液脳関門を通過しないためである可能性がある。
GABA-Rには2種の異なる型、すなわちGABAA-R及びGABAB-Rがある。免疫細胞は主としてGABAA-Rを発現する(Tian et al. (1999) J. Neuroimmunol. 96(1): 21-28;Tian et al. (2004) J. Immunol. 173(8): 5298-5304;Bhat (2010) Proc. Natl. Acad. Sci. USA, 107(6): 2580-2585;Bjurstom et al. (2008) J. Neuroimmunol. 205: 44-50)一方、β細胞はGABAA-R及びGABAB-Rの両方を発現する(Ligon et al. (1997) Diabetologia, 50(4): 764-773;Tian et al. (2013) Diabetes, 62(11): 3760-3765;Braun et al. (2010) Diabetes, 59(7): 1694-2701;Gu et al. (1993) Life Sci. 52(8): 687-694;Brice et al. (2002) Diabetologia, 45(2): 242-252;Martin et al. (1988) Neuropsychobiology, 19(3): 146-148)。GABAA-Rは、迅速作動性(fast-acting)塩素イオンチャネルを形成する種々のサブユニットの五量体である。GABAB-Rは2種のサブユニットのみのヘテロ二量体であるが、それらの特性は他の細胞タンパク質によって変化し得る(Schwenk et al. (2010) Nature, 465(7295): 231-235)。GABAB-Rは遅効性(slow-acting)Gタンパク質共役型受容体を形成する(Bettler et al. (2004) Physiol. Rev. 84(3): 835-867)。したがって、GABAA-R及びGABAB-Rは大きく異なる(塩素イオンチャネル対Gタンパク質共役型受容体)。この実施例において本発明者らはGABAA-Rに焦点を当てている。というのもGABAA-Rは自己免疫応答を阻害するのと同時にβ細胞の複製及び生存を促進する能力を有するからである。
ベンゾジアゼピンはGABA結合部位ではなく、GABAA-R上の他の部位(図中の「BZD」部位)に結合する。ベンゾジアゼピンは塩素イオンチャンネルを開口することはできないが、GABAが上記受容体に結合している場合にはCl-コンダクタンスを増加させる陽性アロステリック調節因子として作用する。血液脳関門を通過する能力がほとんどまたは全くないGABAとは異なり、ベンゾジアゼピンは血液脳関門を通過し、CNS神経細胞によって産生されるGABAの作用を亢進させることができる。異なるベンゾジアゼピンは、異なるサブユニット組成を有する異なるGABAA-R(サブタイプ)に優先的に結合する。
本発明者らは、ヒトβ細胞により発現される特定のGABAA-Rサブタイプ(複数可)に結合するベンゾジアゼピンが、膵島内で局所的に放出されるGABAの作用を亢進させることができるとの仮説を立てた。臨床治験において、T1DまたはT2Dによってグルコース制御が低下した(HbA1cの平均値が約12)不安症の及び不安症ではない個人にアルプラゾラムを投与した。8週間の治療後、アルプラゾラムの投与を受けた患者においてはHbA1cレベルが有意に低下したが、プラセボにおいては低下が見られなかった(Lustman et al. (1995) Diabetes Care, 18(8): 1133-1139)。この効果は不安症の及び不安症ではない患者の両方で同様であり、このことは、HbA1cの低下は不安症の改善に依存しないことを示した。
本発明者らはパイロットでの検討において、アルプラゾラム単独及び低レベルのGABAとの併用によって、イン・ビトロでヒト膵島細胞の複製が亢進し得るかについて試験した。本発明者らは、ベンゾジアゼピン自体はGABAA-R塩素イオンチャンネルを開口させることはできないが、β細胞はGADを発現しGABAを分泌することから、アルプラゾラム単独を試験した。したがって、本発明者らは、アルプラゾラムが、内因的に産生された膵島GABAの作用を亢進させることができるとの仮説を立てた。
次に本発明者らは、アルプラゾラム(30ng/ml)と共に、またはアルプラゾラムなしで、ある用量範囲のGABAを含有するヒト膵島培養物におけるヒト膵島細胞の増殖を比較した。0.03mMのGABAによる処理ではヒト膵島細胞の増殖が有意に亢進しなかった一方で、同一用量のGABAとアルプラゾラムとで一緒に処理するとヒト膵島細胞の増殖が138%亢進した(図3)。GABAを単独で用いて同様のレベルの増殖を達成するには、10倍高いレベルのGABA(0.3mM)を必要とした。したがって、アルプラゾラムによって、十分確実なβ細胞の増殖を誘導するのに必要なGABAの量が大幅に低下する。これらのデータは、GABAとアルプラゾラムとの併用が、イン・ビトロでのヒト膵島細胞の増殖に対して相乗的なプラスの効果を有することを実証している。
アルプラゾラムと共にGABAを投与することの更なる利点は、相乗的に自己反応性T細胞を阻害し、Tregを促進する可能性である。血液中のGABAのレベルは非常に低い(GABAを産生するCNS外のGADは非常に僅かしかなく、GABAの循環系中での半減期は非常に短い)。GABAを伴わずにアルプラゾラム単独では、免疫細胞に対する大きな効果はないはずであるが、併用においては、アルプラゾラムとGABAは、おそらくは少ない用量のそれぞれの薬物を用いても、相乗的に作用して炎症を抑制しTregを促進し得る。アルプラゾラム+GABAの免疫応答に対する効果の研究は大きな関心事ではあるが、これらの研究は本研究の範囲外であり、本研究はヒトβ細胞の複製に特化した。ヒトにおけるβ細胞の複製及び生存を亢進させるためのアルプラゾラムの変換能。アルプラゾラムはヒトにおいて、免疫抑制を起こさずまたは感染症に対する感受性を高めることはなく(http://labeling.pfizer.com/ShowLabeling.aspx?id=547にてアルプラゾラムの概況報告書を参照のこと)、このことは、GABAを伴わない場合、アルプラゾラムはヒト免疫細胞上のGABA-Rを活性化できないという考えと整合する。アルプラゾラムの主な副作用は眠気である。
この実験の目的は、イン・ビトロにおいて、INS-1細胞の増殖に対する以下の化合物の効果を試験することであった。
1.3H-チミジン取り込みアッセイ:1×105/ウェルのINS-1細胞を表示した濃度の個々の化合物で処理し(同一試料を3回繰り返し)、0.3μCi/ウェルの3H-チミジンの存在下、10%のFCS RPMI164培地中で48時間(最適な期間)培養した。上記細胞を回収し、個々のウェルにおける3H-チミジンの取り込み量のレベルをベータカウンターにより測定した。
1.ミナゾラムによる処理には、イン・ビトロでINS-1細胞の増殖を亢進させる多少の能力があるが、クロナゼパムによる処理にはその能力はない。本発明者らは、10-4~10-9Mのクロナゼパムによる処理では、INS-1細胞の増殖は有意に変化しないことを見出した。10-5Mのミダゾラムによる処理では、INS-1細胞に対して僅かに毒性が見られ、10-7~10-8Mによる処理では、イン・ビトロでINS-1細胞の増殖が有意に亢進した(図4A、p<0.05)。次に本発明者らは、これらの結果に基づいて、これらの薬物をより少ない用量範囲にわたって、更に中間的な用量も追加して試験し、10-7~3×10-8Mのミダゾラムによって、イン・ビトロにおいてINS細胞の増殖が有意に亢進することを認めた(図4B)。同様のパターンの細胞増殖がMTTアッセイによって認められた(データ非表示)。

AP325またはAP3(単独)による処理では、イン・ビトロにおいて、INS-1細胞の増殖は刺激されない。
本発明者らは、10-4~10-5MのAP325またはAP3による処理では、イン・ビトロにおいて、INS-1細胞の増殖が有意に阻害されることを見出した(図6)。10-6~10-10ではどちらの化合物による処理でもINS-1細胞の増殖は阻害されなかったが、これらの処理ではINS-1細胞の増殖は有意に増加することはなかった。注目すべきことに、10-7~10-8のAP325による処理ではINS-1細胞の増殖が僅かに増加した(但し、これは有意な増加ではなかった)が、AP3による処理では増加は見られなかった。同様のパターンの細胞の生存がMTTアッセイによって検出された(データ非表示)。したがって、高用量の両方の化合物はINS-1細胞に対して毒性を有し、10-6~10-10のこれらの化合物はイン・ビトロにおいてINS細胞の増殖の如何なる刺激も示さなかった。
次に本発明者らは、イン・ビトロにおいて、GABA誘導性のINS-1細胞の増殖に対するAP325またはAP3の効果を試験した。本発明者らは、1~0.1mMのGABAによる処理では、イン・ビトロにおいて、INS-1細胞の増殖が刺激されるが、GABA 0.03または0.01mMによる処理では、イン・ビトロにおいて、INS-1細胞の増殖は有意に刺激されないことを見出した(図7)。10-6MのAP325による処理ではINS-1細胞の増殖は有意に変化しなかった(データ非表示)。しかしながら、10-7~10-8MのAP325による処理、特に3×10-7~3×10-8MのAP325による処理では、GABA誘導性のINS-1細胞の増殖が有意に亢進した。注目すべきことに、0.03または0.01mMのGABA単独での処理では、本発明者らの実験条件下でINS-1細胞の増殖を刺激することができなかった一方で、0.03mMのGABAを伴う3×10-7~10-8MのAP325による処理では、INS-1細胞の増殖が有意に促進された。更に、0.01mMのGABAを伴う10-7MのAP325による処理でも、イン・ビトロにおいて、INS-1細胞の増殖が有意に刺激された。同様のパターンのデータがMTTアッセイから得られた(データ非表示)。したがって、これらのデータは、AP325によって、イン・ビトロにおいて、GABA刺激性のINS-1細胞の増殖が亢進することを示した。
AP3の分析によって、10-6MのAP3による処理では、INS-1細胞の増殖が有意に変化しないことが明らかになった(データ非表示)。しかしながら、3×10-7~3×10-8MのAP3による処理では、0.1mM GABA誘導性のINS-1細胞の増殖が有意に亢進した(図8)。注目すべきことに、0.03mMのGABAを伴う3×10-7~10-8MのAP3による処理ではINS-1細胞の増殖が有意に促進された。更に、0.01mMのGABAを伴う10-7~10-8MのAP3による処理でも、イン・ビトロにおいて、INS-1細胞の増殖が有意に刺激された。同様のパターンのデータがMTTアッセイから得られた(データ非表示)。したがって、これらのデータは、AP3によって、イン・ビトロにおいて、GABA刺激性のINS-1細胞の増殖が亢進することを示した。
AP325及びAP3はイン・ビトロにおいてGABA誘導性のINS-1細胞の増殖を亢進させることができる。
1型糖尿病(T1D)研究の重要な目標は、ヒトβ細胞の複製を安全に促進するための治療法を開発することである。β細胞上のγ-アミノ酪酸受容体(GABA-R)の活性化がβ細胞の生存及び複製を促進することができることが最近認識されるようになっている。神経細胞のGABAA-Rに対するGABAの作用を亢進させる多くの陽性アロステリック調節因子(PAM)が臨床で使用されている。これらのGABAA-R PAMを糖尿病の治療に役立てるための別の目的に用いることは、それらの安全性ならびに、β細胞から分泌される、または外因的に投与されるGABAのβ細胞の複製及び生存を促進する能力を亢進する潜在的能力という理由により、理論的に魅力がある。ここでは、本発明者らは、臨床的に適用可能なGABAA-R PAMがINS-1 β細胞の複製を亢進させることができること、及びこれが外因性GABAの適用によって増幅されることを示す。更に、GABAA-R PAMは、イン・ビトロにおいて、ヒト膵島細胞の複製を促進した。この効果はGABAA-Rアンタゴニストによって消失し、このことはβ細胞から放出されたGABAが、PAMによって亢進し得る自己分泌有糸分裂促進効果を有し得ることを示唆した。PAMと低レベルの外因性GABAの併用によって、ヒトβ細胞の複製を促進する能力が向上した。これらの検討によって、T1D治療に対する可能性をもつ新規な種別の薬物が特定され、GABAA-R PAMが糖尿病患者においてHbA1cを低減した過去の所見を説明することができる。
T1D研究の目標は、β細胞の複製を安全に促進できる手法を見出すことである。しかしながら、ヒトβ細胞の複製を亢進させることができる薬剤はほとんど見つかっていない。げっ歯動物及びヒトのβ細胞は、GABA-R(GABAA-R及びGABAB-Rの両方)を発現することが長い間知られている(Braun et al. (GABA) (2010) Diabetes, 59: 1694-1701)が、ごく最近になって、GABA-Rを活性化することによってβ細胞の生存及び複製を促進し、量を増加させることができることが明らかになっている(Ligon et al. (2007) Diabetologia, 50: 764-773;Soltani et al. (2011) Proc. Natl. Acad. Sci. USA, 108: 11692-11697;Tian et al. (2013) Diabetes, 62: 3760-3765;Prud’homme et al. (2103) Transplantation, 96(7): 616-623;Purwana et al. (2014) Diabetes, 63(12): 4197-4205)。GABAA-R PAMはGABAの作用を亢進させる。GABAA-R PAMはGABA結合部位に結合するのではなく、GABAA-R上の他の場所に結合し、GABAA-R塩素イオンチャネルを開口させることができない一方で、GABAが受容体に結合していると、Cl-コンダクタンスを増加させる。GABAは血液脳関門(BBB)を通過する能力がほとんどまたは全くないことから、ベンゾジアゼピンなどのBBB透過性GABAA-R PAMが、発作、不眠症、及び不安症などのCNS障害を治療するために、CNS神経細胞によって分泌されるGABAの作用を亢進させるために開発された。理論的には、これらのPAMを、β細胞の有糸分裂誘導を促進するために膵島内で放出されるGABAの作用を亢進させるための異なる目的に用いることができる。
アルプラゾラム、ミダゾラム、クロナゼパム、PK11195、及びビククリンはSigma-Aldrichから購入し、AP3はAlgiax Pharmaceuticals GmbHから供給を受けた。アルプラゾラム(10mMのDMSO溶液)、ミダゾラム(6.25mMの水溶液)、クロナゼパム(50mMのEtOH溶液)、またはAP3(50mMのDMSO溶液)の原液を表示した濃度へと培地中で希釈した。
1×105/ウェルのINS-1細胞を表示した濃度の個々の化合物で処理し(同一試料を3回繰り返し)、3H-チミジン(0.3μCi/ウェル)の存在下、10%のFCS RPMI164培地中で48時間(最適な期間)培養した。個々のウェルにおける3H-チミジンの取り込み量のレベルをシンチレーションカウンターにより測定した。
INS-1細胞はベンゾジアゼピン結合GABAA-Rサブユニットを発現する。
GABAはINS-1細胞の増殖を亢進させることができる(Soltani et al. (2011) Proc. Natl. Acad. Sci. USA, 108: 11692-11697)が、INS-1細胞がベンゾジアゼピンに感受性のGABAAサブユニットを発現するかは不明である。本発明者らは、qRT-PCRを用いて、ベンゾジアゼピンに対する感受性を付与するGABAA-Rサブユニット(α1、α2、α3、及びα5)の発現に関して、INS-1細胞RNAを試験した。本発明者らは、α1、α3、及びα5転写物ならびにβ1、γ1、γ2、γ3サブユニットを検出し(データ非表示)、このことはINS-1細胞がベンゾジアゼピンに感受性であるGABAA-Rを発現し得ることを示す。
β細胞はGADを発現し、自己分泌の形態で作用し得るGABAを分泌する。INS-1細胞もまたGABAを合成するかどうかは不明である。したがって、本発明者らはINS-1細胞におけるGAD酵素活性を試験した。本発明者らは、INS-1細胞におけるGAD活性はヒト膵島におけるGAD活性よりもかなり小さく、同様にマウスの脳におけるGAD活性よりも小さいことを見出した(図9A)。それにもかかわらず、INS-1細胞におけるGAD活性は、陰性対照293T細胞のGAD活性よりも一貫して約2倍大きく、このことは、INS-1細胞が自己分泌の形態で作用し得る低レベルのGABAを産生することができることを示唆している。
本発明者らは、イン・ビトロにおいてINS-1細胞の増殖に対する種々の濃度の各PAMの影響を試験した。10nMまたは/及び100nMのアルプラゾラムまたはミダゾラムによる処理ではINS-1細胞の増殖が有意に刺激されたが、クロルナゼパムまたはAP3による処理では刺激されなかった(図9B)。更に、0.03~0.3mMのGABA(単独)による処理では、以前の所見と同様にINS-1細胞の増殖が刺激された(図9C~G)(Soltani et al. (2011) Proc. Natl. Acad. Sci. USA, 108: 11692-11697)。0.01mMのGABAによる処理ではINS-1の増殖が刺激されなかった(図9C~G)一方で、30nMまたは100nMのアルプラゾラムの存在下での同一用量のGABAでは、INS-1細胞の増殖が有意に増加し、該増殖はアルプラゾラム単独による処理または10倍高い濃度のGABA単独(0.1mM)による処理と比較した場合でさえも有意に高かった(図9C)。しかし、高濃度のGABA(0.1mMまたは0.3mM)では、アルプラゾラムのGABAの有糸分裂促進効果を亢進させる能力がより小さく、これはおそらく高濃度のGABAが既に最大のGABAA-R応答を誘導したためであった。同様に、低用量のミダゾラム、クロナゼパム、またはAP3では、GABA刺激性のINS-1細胞の増殖が亢進した(図9D~G)。したがって、GABAA-R PAMは、GABA誘導性のβ細胞の増殖を亢進させることができる。
次に本発明者らは、PAMがヒト膵島細胞の複製を亢進させることができるかを試験した。本発明者らはアルプラゾラムの試験に絞り込んだ。その理由はアルプラゾラムの安全性の記録及びアルプラゾラムによる治療が糖尿病患者のHbA1cを低減したとの知見にあった(Lustman et al. (1995) Diabetes Care18: 1133-1139)。本発明者らはある用量範囲のアルプラゾラムと共にヒト膵島を培養し、低濃度のアルプラゾラムがヒト膵島細胞の増殖を促進することを認めた(図10)。免疫組織学的研究により、GABAはヒトβ細胞の複製を増加させるが、α細胞の複製には影響を及ぼさないことが明らかになっており(Purwana et al. (2014) Diabetes, 63(12): 4197-4205及び本発明者らの未発表の所見)、他の膵島細胞型はGABA-Rを発現しないことから、複製する膵島細胞の大部分はβ細胞である可能性が高い。これらの培養物にCa2+チャネル遮断因子であるニフェジピン(1uM)を添加したところ、アルプラゾラムの有糸分裂促進効果がブロックされ(データ非表示)、このことは、このPAMがGABAA-R媒介性のPI3K/Akt経路の活性化を亢進させるとの考えと整合する(Soltani et al. 次に本発明者らは、イン・ビトロにおいて、アルプラゾラムが、外因性GABAのヒト膵島細胞の複製を促進する能力を亢進させることができるかを試験した。ヒト膵島を、100nMのアルプラゾラムの存在下または非存在下で、種々の濃度のGABAを用いてまたは用いずに処理した。本発明者らは、0.3~3mMのGABA(単独)では、用量依存の形態でヒト膵島細胞の増殖が有意に亢進するが、より低い用量では亢進しないことを見出した(図12)。アルプラゾラムとGABAとの併用療法では、対応するGABA(単独)の用量と比較してヒト膵島細胞の増殖が有意に増加した(図12)。注目すべきことに、0.03mMのGABAによる処理ではヒト膵島細胞の増殖は有意に亢進しなかった一方で、同一用量のGABAとアルプラゾラムとで一緒に処理すると、ヒト膵島細胞の増殖が有意に亢進した(図12)。GABAを単独で用いて同様のレベルの増殖を達成するには、10倍高いレベルのGABA(0.3mM)を必要とした。したがって、アルプラゾラムは、β細胞の増殖を誘導するのに必要なGABAの量を大幅に減少させる。
GABA-Rの活性化によって、β細胞をアポトーシスから保護し、β細胞の複製を促進することができる(Ligon et al. (2007) Diabetologia, 50: 764-773;Soltani et al. (2011) Proc. Natl. Acad. Sci. USA, 108: 11692-11697;Tian et al. (2013) Diabetes, 62: 3760-3765;Prud’homme et al. (2103) Transplantation, 96(7): 616-623;Purwana et al. (2014) Diabetes, 63(12): 4197-4205)。経口投与でのGABAによる処置によって、膵島異種移植物において、ヒトβ細胞の複製が数倍、一般的には成人のヒトβ細胞の1%未満から約2~3%へと増加し(Tian et al. (2013) Diabetes, 62: 3760-3765;Purwana et al. (2014) Diabetes, 63(12): 4197-4205)、これは出生直後に起こる最大レベルのβ細胞の複製に類似する。この亢進はGABAによる処置の5週間後において減弱せず、ヒト膵島異種移植物においてβ細胞の量及び機能が増加する結果となった(Purwana et al. Claims (104)
- 哺乳動物におけるベータ細胞の複製、増殖、及び/または生存、及び/または機能の促進方法であって、前記哺乳動物に、前記哺乳動物におけるベータ細胞の複製、及び/または生存、及び/または機能を促進する、及び/または前記ベータ細胞の量を増加させるのに十分な量でGABAA受容体陽性アロステリック調節因子(PAM)を投与することを含む方法。
- 前記GABAA受容体陽性アロステリック調節因子(PAM)がGABA受容体活性化リガンドとの併用で投与される、請求項1に記載の方法。

- 前記GABA受容体活性化リガンドが、バマルゾール、ガバミド、GABOB、ガボキサドール、イボテン酸、イソグアバシン、イソニペコチン酸、ムシモール、フェニバット、ピカミロン、プロガビド、キスカラミン、プロガビド酸(SL 75102)、チオムシモール、プレガバリン、ビガバトリン、6-アミノニコチン酸、ホモタウリン、及びXP13512[(±)-1-([(α-イソブタノイルオキシエトキシ)カルボニル]アミノメチル)-1-シクロヘキサン酢酸]からなる群より選択される薬剤を含む、請求項78~98のいずれか1項に記載のキット。
