50年以上、特定の疾患部位に対する電離放射線の送達ビヒクルとして放射標識薬剤が使用されてきた(Larson SM. Cancer 67:1253-1260 (1991);Britton KE. Nucl Med Commun. 18:992-1007 (1997))。放射標識抗体、抗体フラグメント、代替的な足場、および小分子を含む、多くの分子が、放射性同位元素の標的送達のために考えられてきた(Tolmachev V, et al. Cancer Res. 67:2773-2782 (2007);Birchler MT, et al., Otolaryngol Head Neck Surg. 136:543-548 (2007);Reubi JC, Maecke HR. プレターゲット放射免疫療法(PRIT)では、腫瘍抗原および小分子ハプテンの両方に対して特異性を有する非放射性二価抗体が投与され、腫瘍に局在化させることができる。抗体の十分な血液クリアランスの後、放射標識小分子が投与され、プレターゲット抗体によって取り込まれる。しかしながら、PRIT系に使用される多くの小ペプチドおよび金属キレートハプテンは、大幅な全身保持を示し、これは、画像化に対するシグナル・バックグラウンド比を制限する望ましくないバックグラウンド活性をもたらし、療法適用の最大耐用線量を制限する非特異的照射をもたらす(Orcutt et al., Mol Imaging Biol 13:215-221 (2011))。

除去剤(CA)は、腫瘍抗原に対する放射性同位元素のプレターゲットの間に血液循環から対象とする生体分子を急速に除去するよう設計された化合物のクラスである。デキストランベース除去剤は、一般に高い治療指数のDOTAベースのプレターゲット放射免疫療法(DOTA PRIT)に使用される。除去剤足場として極めて有効ではあるが、固有の多分散性およびインビボにおける酵素分解の反応速度のためにデキストランには投与の問題がある。
本技術の組成物は、PRIT(例えば、アルファ粒子放射免疫療法)に有用な新規のN−アセチルガラクトサミノデンドロン除去剤を含む。本明細書中で開示されている除去剤組成物は、DOTA−PRIT放射標識二重特異性抗体(BsAb)の増大した血液クリアランスを示し、DOTA−PRITの間の治療指数(TI)を向上させる。例えば、過剰な本技術のデンドロン除去剤の単回用量の投与の5分以内に(注射された131I−BsAb対CAのモル比が1:2.3)、血液中の131I放射能が6.7%ID/gのベースラインから2.4%ID/gまで64%低下した。ヒト結腸直腸がんのマウス異種移植モデルにおけるDOTA−PRIT試験は、177Lu放射能の注射後の24時間の時点で177Lu−DOTA−BnのCA用量依存的な腫瘍対血液取り込み比を示した(例えば、平均腫瘍対血液比は、0μg(ビヒクル)、15μg、または20μgのデンドロン−CAに関して、それぞれ2.9、26、および59であった)。25μg用量のデンドロン−CAは、76の平均腫瘍対血液比をもたらし、デキストラン−CAを用いた予め最適化した投与(77の平均腫瘍対血液比)とほぼ一致した。まとめて、これらの結果は、本技術のデンドロン除去剤が高いTIのDOTA−PRITに適していることを示唆する。

「結合親和性」とは、分子(例えば、抗体)の単一の結合部位とその結合パートナー(例えば、抗原)との間の総合的な非共有結合性相互作用の強さを意味する。分子XのそのパートナーYに対する親和性は、一般に解離定数(Kd)によって表すことができる。親和性は、本明細書に記載されているものを含む当該技術分野において知られている標準的な方法によって測定することができる。低親和性の複合体は、一般に容易に抗原から解離する傾向がある抗体を含む一方で、高親和性の複合体は、一般により長い期間、抗原と結合したままである傾向がある抗体を含む。
本明細書中で使用される場合、「対照」は、実験において比較のために使用される代替的なサンプルである。対照は、「陽性」または「陰性」であってもよい。例えば、実験の目的が、特定のタイプの疾患または状態に対する処置のための治療用薬剤の効果の相関性を判断することである場合は、陽性対照(所望の治療効果を示すことがわかっている化合物または組成物)および陰性対照(治療を受けていないか、またはプラセボを与えられている対象またはサンプル)が一般に利用される。
本明細書中で使用される場合、「有効量」の組成物という用語は、所望の予防効果または治療効果を達成するのに十分な量、例えば、処置される疾患、例えば、標的ポリペプチドと関連する疾患または医学的状態(例えば、乳がん、結腸直腸がん、脳がんなど)と関連する症状の低減をもたらす量である。対象に投与される本技術の組成物の量は、疾患の程度、タイプおよび重症度ならびに全身の健康状態、年齢、性別、体重および薬物に対する耐性などの個体の特徴によって決まることになる。当業者は、これらの要素および他の要素に応じて適切な投与量を決定することができるであろう。本技術の組成物はまた、1つまたは複数の付加的な治療用化合物と組み合わせて投与することもできる。

本明細書中で使用される場合、「連続的な」治療的使用という用語は、異なる時間における少なくとも2つの活性成分の投与を指し、投与経路は、同一または異なる。特に、連続的な使用は、他の投与を開始する前の1つの活性成分の完全な投与を指す。したがって、他の活性成分を投与する前に、数分間、数時間、または数日間かけて1つの活性成分を投与することが可能である。この場合、同時の処置ではない。

放射免疫療法(RIT)の治療指数(TI)は、固形腫瘍の有効な処置のために最大にされるべきである(Larson SM et al., Nature Reviews Cancer 15: 347-60 (2015))。放射標識IgG抗体を用いたRITは、IgG担体のゆっくりとした薬物動態のためTIが低いという難点があり、そのため、許容できる用量では効果がないことが多い。
代替案は、RITにプレターゲットアプローチ(PRIT)を使用することである。PRITの過程では、抗体が関係するゆっくりとした腫瘍標的ステップが放射能の投与とは分離される。プレターゲットは、骨髄などの正常組織に望ましくない毒性をもたらす腫瘍標的抗体のゆっくりとした血液クリアランスを解決する多段階プロセスである。プレターゲットでは、放射性核種またはその他の診断用薬剤もしくは治療用薬剤は、より好ましい薬物動態を有する担体に付着させられ(例えば、急速な腎クリアランスならびに低い正常組織取り込みおよび全身クリアランスを有する低分子量化合物(Orcutt KD et al., Molecular imaging and biology 13: 215-21 (2011)、例えば、DOTA−ハプテン)、腫瘍を標的とすると同時に、正常組織に対する線量が最小限にされる。放射能を腫瘍に向けるために、循環する治療用薬剤(例えば、DOTA−ハプテン)は、腫瘍内に局在するBsAbによって取り込まれるか、またはそうでなければ、腎臓経路により効率的に除去される。
極めて有効ではあるが、デキストラン−CAの使用は、欠点を有する。天然に存在するグルコース重合体として、デキストラン足場は、固有に多分散系であるため、再現可能なバッチ間製造およびインビボにおける使用に関する難題を生じる。また、RESデキストラン−1,6−グルコシダーゼによる酵素分解は、ハプテン・そのフラグメントの血液循環への導入につながる可能性があり、それが、腫瘍・BsAbが結合する放射性ハプテンと競合する可能性がある。臨床的ストレプトアビジン・ビオチンPRITの過程においてアルブミンベースのCAを用いると、これもまた類似の問題が見られた(Knox SJ et al., Clinical Cancer Research 6: 406-14 (2000);Breitz HB et al., Journal Nuclear Medicine 41: 131-40 (2000))。

本技術の化合物の例となる合成
全般。DOTA−Bn−イソチオシアナート(p−SCN−Bn−DOTA)をMacrocyclics, Inc.(Plano、TX)から購入し、アミン−PEG4−DOTAをCheMatech(Dijon、フランス)から購入した。Optima(商標)グレードの塩酸を、Thermo Fisher Scientific(Waltham、MA)から購入した。Chelex−100樹脂、200〜400メッシュをBio−Rad Laboratories(Hercules、CA)から購入した。PD−10ゲルろ過サイズ排除カラム(8.3mLのSephadex(商標)G−25樹脂/カラムを含む)を、GE Healthcare Life Sciences(Pittsburgh、PA)から購入した。その他のすべての試薬および合成グレードの化学薬品を、Sigma−Aldrich(St. Louis、MO)から購入し、さらなる精製を行わずに使用した。HPLC分析(HPLCグレード)および化合物精製のために使用したすべての溶媒も、Thermo Fisher Scientific(Waltham、MA)から購入した。すべてのバッファーおよび溶液は、超純水(18MΩ・cmの抵抗率)を使用して調製した。

CCA−16−酸:
生物学的試験に関する例となる実験材料および方法
材料。抗GPA33 DOTA−PRIT BsAb(huA33−C825)を、WO2016/130539に以前に記載されたとおりに作製した。GPA33(+)ヒト結腸直腸がん細胞株SW1222を、Ludwig Institute for Cancer Immunotherapy(New York、NY)から入手し、10%熱失活ウシ胎仔血清、2.0mMのグルタミン、100単位/mLのペニシリン、および100μg/mLのストレプトマイシンを加えた最小必須培地中で維持した。
131I−BsAbの血液クリアランス。131I−BsAbを付加的なキャリア抗体と混合することによって注射用の抗体トレーサーを調製して、1用量あたり250μg(1.19nmol)を実現した。すべての血液サンプル採取および注射は、尾静脈により行った。正常マウス(n=15)に、t=0に250μg(1.19nmol)の131I−BsAbおよびt=24hにビヒクル(生理的食塩水;n=3)または25μgの除去剤(250μLの生理的食塩水中で製剤化された;n=12)のいずれかを静脈内投与した。ビヒクルに関して、t=1、2、3、4、24、25、26、27、および28hに血液を採取した(30〜40μL;各時点n=3)。除去剤に関して、t=1、2、3、4、24、24.1(CA注射の5分後)、24.3(CA注射の15分後)、24.5、25、26、27、および28hに血液を採取した(30〜40μL;各時点n=3)。ガンマカウンター(PerkinElmer Life Sciences Wallac Wizard 3)において計数することによって各サンプルの放射能濃度を測定した。計数率をバックグラウンド補正および減衰補正し、同位元素固有の系校正係数を使用して放射能に変換し、投与された放射能に正規化し、投与線量パーセント/g(ID/g)として表した。除去剤の注射後の時間間隔(y2)におけるベースラインからの血液放射能の変化率(すなわち、除去剤注射の直前;y1)を、式((y2−y1)/y1)×100を使用して算出した。
統計分析。平均間の差は、対になっていないスチューデントt検定を使用して判定した。
SPECT/CT画像化。抗GPA33−DOTA−PRIT + [111In]Proteus−DOTAを用いたインビボにおける標的化後のSPECT/CT画像化および体内分布試験のために、huA33−C825 BsAbおよびCCA−16−DOTA−Y3+;25μg;2.76nmol)の注射を受けたSW1222腫瘍をもつマウスに172pmol/1.67MBq[45μCi]の[111In]1(n=4)または790pmol/7.66MBq[207μCi]の[111In]1(n=1)を注射した。翌日、[111In]Proteus−DOTAのより大きな投与放射能を与えられた1匹のマウスを、SPECT/CTによって注射の20h後に画像化し、すべての動物を体内分布のために注射の24h後に屠殺した。
本技術の除去剤はインビボにおける放射標識二重特異性抗体(BsAb)の血液クリアランスを増大させる
131I−BsAb(131I−huA33−C825)の血液クリアランスの評価。正常な(腫瘍のない)ヌードマウスを使用してインビボ実験を行って、131I−BsAbの血液クリアランスに対する過剰な本技術のデンドロン−CAの単回投与の効果を評価した。
図1の下に示されるとおり、まずCA群とビヒクル群の間のベースライン(注射の24h後)の血液131I放射能に著しい差はなかった。注射の5分後に6.7%ID/gから2.4%ID/gへと平均血液放射能濃度が64%著しく低下したため、CAは、循環する131I−BsAbを減少させるのに有効であった。CAまたはビヒクルの注射の1時間後に、平均血液放射能は、それぞれ、5.8(−13%)または1.3%ID/g(−81%)であった。
これらの結果は、本技術の除去剤が、放射線療法に対する腫瘍の感受性を高めるための方法、および/またはがんを処置することを必要とする対象のがんを処置するための方法に有用であることを示す。
本技術の除去剤は、DOTA PRIT法に有用である
さまざまな用量のCCA−16−DOTA−Y3+を用いて達成される腫瘍のプレターゲットの結果を図2に示す。5〜25μg用量のデンドロン−CAは、177Lu放射能の注射の24h後に27〜41%ID/gの平均腫瘍177Lu−DOTA−Bn取り込みをもたらし、これは、デキストランCA群およびビヒクル群(約35%ID/g)で観察された腫瘍177Lu−DOTA−Bn取り込みと同等であった。25μg用量のデンドロン−CA(CCA−16−DOTA−Y3+)は、62.5μg用量のデキストランCAで観察された腫瘍取り込み:血液比と同等の腫瘍取り込み:血液比をもたらした。腫瘍取り込みに対するN−アセチルガラクトサミノデンドロン除去剤CCA−16−DOTA−Y3+の効果は用量依存的であった。図3を参照。さらに、図4は、除去剤なし(ビヒクル)ならびに500kDのデキストラン−DOTAハプテン複合物除去剤(すなわち、デキストラン−CA)と比較した、本技術の除去剤を用いて達成される腫瘍のプレターゲットの結果の比較を示す。CCA−16−DOTA−Y3+を用いて達成される腫瘍取り込み:正常組織比は、デキストランCAで観察されたものと同等であった。
図6(B)に示されるとおり、[225Ac]Proteus−DOTA(デキストラン−CAと組み合わせて使用された)および[111In]Proteus−DOTA(CCA−16−DOTA−Y3+と組み合わせて使用された)を用いて達成された抗GPA33−DOTA−PRITの結果は同等であった。
これらの結果は、本技術の除去剤が、放射線療法に対する腫瘍の感受性を高めるための方法、および/またはがんを処置することを必要とする対象のがんを処置するための方法に有用であることを示す。
DOTAプレターゲット放射免疫療法のためのN−アセチルガラクトサミノデンドロン除去剤の使用
デンドロン−CA CCA−16−DOTA−Y3+を含む、抗GPA33 PRIT + 177Lu−アミノベンジルDOTA ([177Lu]LuDOTA−Bn)は、高い治療指数(TI)を達成するためにデキストラン−CAの代わりに使用することができるという重要な仮説を試験した。治療指数は、本明細書中では推定腫瘍/正常組織吸収線量比と定義される。
デンドロン−CA CCA−16−DOTA−Y3+を含む、PRIT + [177Lu]LuDOTA−Bnに関する吸収線量推定値:デンドロン−CA(本技術の除去剤)を含む、PRIT + [177Lu]LuDOTA−Bnに関する吸収線量を算出するために、GPA33発現SW1222異種移植片をもつヌードマウスの群(n=5/群)に、[177Lu]LuDOTA−Bn(20pmol、3.7MBq[100μCi])の投与の28h前にBsAb huA33−C825(250μg;1.19nmol)を静脈内注射し、4h前にデンドロン除去剤CCA−16−DOTA−Y3+(25μg;2.76nmol)を静脈内注射した。これらのマウスを、体内分布アッセイのために[177Lu]LuDOTA−Bnの投与の1、4、24、または48h後に屠殺した。各組織に関して、減衰補正されていない時間放射能濃度データを、Excelを使用して1成分、2成分またはそれ以上の複素指数関数に適切にフィッティングし、解析的に積分して、単位投与放射能あたりの集積放射能濃度(1MBqあたりのMBq−h/g)を得た。腫瘍間および選択臓器間自己吸収線量を推定するために非透過性放射線に関する177Lu平衡吸収線量定数(8.49g−cGy/MBq−h)を使用し、177Luベータ線のみの完全な局所的吸収を想定し、ガンマ線および非自己線量寄与を無視した。これらの試験から得られた結果を図9に示す。
デンドロン−CA CCA−16−DOTA−Y3+を含む、PRIT + [177Lu]LuDOTA−Bnの上記例に関して、最大耐用プレターゲット[177Lu]LuDOTA−Bn放射能は、線量制限臓器として骨髄を用いて、21MBqであった。この放射能において、腫瘍に送達される推定吸収線量は、9836cGy(98Gy)になり、血液(骨髄)に対して246cGyおよび腎臓に対して280cGyになるであろう。したがって、約73Gyの腫瘍吸収線量のため、マウスの異種移植片における有効で安全なCRC療法が予想され、この吸収線量は、15.6MBqの投与放射能で達成されるであろう(血液(骨髄)に対して183cGyおよび腎臓に対して207cGy)。
これらの結果は、本技術の除去剤が、放射線療法に対する腫瘍の感受性を高めるための方法、および/またはがんを処置することを必要とする対象のがんを処置するための方法に有用であることを示す。

- 前記脳がんは、下垂体腺腫、髄膜腫、神経芽腫、または頭蓋咽頭腫である、請求項15に記載の方法。
- 前記複合体は、組織1グラムあたりの投与線量パーセンテージ(%ID/g)として表される放射能レベルを放射する、請求項8〜16のいずれか1項に記載の方法。
- 前記対象は、約2:1、3:1、4:1、5:1、6:1、7:1、8:1、9:1、10:1、15:1、20:1、25:1、30:1、35:1、40:1、45:1、50:1、55:1、60:1、65:1、70:1、75:1、80:1、85:1、90:1、95:1または100:1の腫瘍対正常組織取り込み比を示す、請求項8〜17のいずれか1項に記載の方法。
- 前記放射標識DOTAハプテンは、213Bi、211At、225Ac、152Dy、212Bi、223Ra、219Rn、215Po、211Bi、221Fr、217At、255Fm、86Y、90Y、89Sr、165Dy、186Re、188Re、177Lu、67Cu、111In、67Ga、51Cr、58Co、99mTc、103mRh、195mPt、119Sb、161Ho、189mOs、192Ir、201Tl、203Pb、68Ga、227Th、および64Cuからなる群から選択される放射性核種で標識される、請求項8〜18のいずれか1項に記載の方法。
- 前記放射標識DOTAハプテンは、Proteus−DOTA、S−2−(R−アミノベンジル)−1,4,7,10−テトラアザシクロドデカン四酢酸(DOTA−Bn)、DOTA−Bn−ビオチン、BAD(((S)−2−(4−(2−ブロモ)−アセトアミド)−ベンジル)−DOTA)、NBD((S)−2−(4−ニトロベンジル)−DOTA)、DOTA−RGD、DOTA−PEG−E(c(RGDyK))2、DOTA−8−AOC−BBN、p−NO2−Bn−DOTA、DOTA−PESIN、DOTA−ビオチン−サルコシン(DOTA−ビオチン)、1,4,7,10−テトラアザシクロドデカン−1,4,7,10−四酢酸酸モノ(N−ヒドロキシスクシンイミドエステル)(DOTA−NHS)、またはDOTATyrLysDOTAのうちの1つまたは複数を含む、請求項19に記載の方法。
- 請求項1〜7のいずれか1項に記載の化合物、および使用のための説明書を含むキット。
- 少なくとも1つの抗DOTA BsAbをさらに含む、請求項21に記載のキット。

- 1つまたは複数の放射性核種で標識されてもよいDOTAハプテンをさらに含む、請求項21〜23のいずれか1項に記載のキット。
- 前記1つまたは複数の放射性核種は、213Bi、211At、225Ac、152Dy、212Bi、223Ra、219Rn、215Po、211Bi、221Fr、217At、255Fm、86Y、90Y、89Sr、165Dy、186Re、188Re、177Lu、67Cu、111In、67Ga、51Cr、58Co、99mTc、103mRh、195mPt、119Sb、161Ho、189mOs、192Ir、201Tl、203Pb、68Ga、227Th、および64Cuからなる群から選択される、請求項24に記載のキット。
