Description
BCMAとBAFFおよびAPRILの結合はNF−kBを活性化させ、抗アポトーシスBcl−2のメンバー、たとえばBcl−xLまたはBcl−2およびMcl−1の上方調節を誘導する。BCMAとそのリガンドの相互作用は様々な面から体液免疫、B細胞の生長、分化を調節することによって人体内環境の安定した平衡を維持する。
本発明の目的は、BCMAを標的とするキメラ抗原受容体およびその製造方法と使用を提供することにある。
具体的に、本発明の目的は、BCMA抗原を標的とするキメラ抗原受容体の配列、およびその修飾T細胞(CART−BCMA)の製造方法と活性同定を提供することにある。
本発明の第一の側面では、キメラ抗原受容体(CAR)(配列)であって、前記キメラ抗原受容体の抗原結合ドメインはBCMAを標的とする細胞外領域の抗体一本鎖可変領域の配列であるものを提供する。
もう一つの好適な例において、前記の抗原結合ドメインはBCMA配列の24〜41番目のアミノ酸残基を標的とする抗体一本鎖可変領域の配列である。
もう一つの好適な例において、前記のBCMA配列のNCBI登録番号はAY684975.1である。

もう一つの好適な例において、前記の腫瘍は、BCMA陽性腫瘍、好ましくはBCMA陽性のB細胞性リンパ腫、多発性骨髄腫、または形質細胞性白血病である。
本発明の第九の側面では、本発明の第四の側面に記載の細胞、または本発明の第六の側面に記載の製剤の使用であって、癌または腫瘍の予防および/または治療における使用を提供する。
もう一つの好適な例において、前記疾患は癌または腫瘍である。
もちろん、本発明の範囲内において、本発明の上記の各技術特徴および下記(たとえば実施例)の具体的に記述された各技術特徴は互いに組み合わせ、新しい、または好適な技術方案を構成できることが理解される。紙数に限りがあるため、ここで逐一説明しない。
本発明者らは、幅広く深く研究し、大量のスクリーニングを行ったところ、初めてBCMA抗原を標的とするキメラ抗原受容体を得た。具体的に、本発明では、BCMA−1、BCMA−20、BCMA−CA8、BCMA−MO6の4つのモノクローナル抗体配列に基づいて構築されたBCMAを標的とするキメラ抗原受容体の構造を獲得し、そしてこれらのキメラ抗原受容体の初代T細胞における発現レベル、体外活性化能力および腫瘍細胞殺傷力などの面における分析と同定を完成させた。研究では、本発明のキメラ抗原受容体は、BCMA陽性細胞を標的とし、BCMA陽性のB細胞性リンパ腫、多発性骨髄腫、形質細胞性白血病またはほかの疾患の治療に使用することができることが明らかになった。
本発明において、BCMA抗原を標的とするCAR構造で修飾されたT細胞の製造プロセスを改良し、主に1%ヒト血清アルブミンを添加したGT−551無血清培地で体外でリンパ球を培養した。
本開示が理解しやすくなるように、まず、一部の用語を定義する。本願に用いられるように、本明細書で別途に明確に規定しない限り、以下の用語はいずれも下記の意味を有する。出願全体において、ほかの定義が記述されている。
用語「約」とは当業者によって決定される特定の値または組成の許容される誤差範囲内にある値または組成で、それによって部分的にどのように値または組成を計量または測定するかというのが決まる。
用語「投与」とは当業者に既知の様々な方法および送達システムの任意の一つによって本発明の製品を物理的に被験者に導入することで、静脈内、筋肉内、皮下、腹膜内、脊髄またはほかの胃腸外の投与経路、たとえば注射または輸注によるものを含む。
本発明のキメラ抗原受容体(CAR)は、細胞外ドメイン、膜貫通ドメイン、および細胞内ドメインを含む。細胞外ドメインは、標的特異的結合エレメント(抗原結合ドメインとも呼ばれる)を含む。細胞内ドメインは、共刺激シグナル伝達領域およびζ鎖部分を含む。共刺激シグナル伝達領域は、共刺激分子の細胞内ドメインの一部を含む。共刺激分子は、リンパ球の抗原に対する有効な応答に必要な細胞表面分子で、抗原受容体またはそのリガンドではない。
本発明の一つの好適な実施形態において、本発明によって提供されるCARの細胞外ドメインはBCMAを標的とする抗原結合ドメインを含む。本発明のCARがT細胞において発現される場合、抗原結合の特異性に基づいて抗原を認識することができる。それが関連抗原に結合すると、腫瘍細胞に影響し、腫瘍細胞が成長せず、死亡するように、またはほかの形で影響され、そして患者の腫瘍負荷が軽減または解消する。抗原結合ドメインは、共刺激分子およびζ鎖から由来する一つまたは複数の細胞内ドメインと融合することが好ましい。好ましくは、抗原結合ドメインは4−1BBシグナル伝達ドメイン、およびCD3ζシグナルドメインと組み合わせた細胞内ドメインと融合している。
本発明のCARにおける細胞内ドメインは、4−1BBのシグナル伝達ドメインおよびCD3ζのシグナル伝達ドメインを含む。

本明細書で用いられるように、用語「CAR−T細胞」、「CAR−T」、「CART」、「本発明のCAR−T細胞」とはいずれも本発明の第二の側面に記載のCAR−T細胞である。
本発明は、BCMAを標的とするキメラ抗原受容体の構造の構築、BCMAを標的とするキメラ抗原受容体の遺伝子改変T細胞の製造方法およびその活性同定に関する。
所期の分子をコードする核酸配列は本分野で既知の組み換え方法、たとえば遺伝子を発現する細胞においてライブラリーをスクリーニングすること、既知の当該遺伝子を含むベクターから当該ベクターを得ること、あるいは標準の技術で当該遺伝子を含む細胞および組織から直接単離することによって得ることができる。必要により、興味のある遺伝子は合成によって生産することもできる。
また、本発明は本発明の発現カセットが挿入されたベクターを提供する。レトロウイルス、たとえばレンチウイルス由来のベクターは、導入された遺伝子の長期間で、安定した組み込みおよびその子細胞における増殖を可能にするため、長期間の遺伝子の移動を実現させる適切な道具である。レンチウイルスは、増殖しない細胞、たとえば肝細胞に導入することができるため、発癌性レトロウイルス、たとえばマウス白血病ウイルスのベクターを超える利点を持つ。また、低免疫原性という利点もある。
本発明の発現構築体は標準の遺伝子送達プロトコールによって、核酸免疫および遺伝子療法に使用することもできる。遺伝子送達の方法は、本分野では既知である。たとえば、米国特許番号5,399,346、5,580,859、5,589,466を参照し、ここで引用によって全文を取り込む。もう一つの実施形態において、本発明は、遺伝子療法ベクターを提供する。
さらに、発現ベクターはウイルスベクターの形態によって細胞に提供することができる。ウイルスベクターの技術は本分野公知のもので、そしてたとえばSambrookら(2001, Molecular Cloning: A Laboratory Manual, Cold Spring Harbor Laboratory, New York)およびほかのウイルス学と分子生物学のマニュアルに記載されている。ベクターとして使用できるウイルスは、レトロウイルス、アデノウイルス、アデノ随伴ウイルス、ヘルペスウイルスやレンチウイルスを含むが、これらに限定されない。通常、適切なベクターは少なくとも1種類の有機体において作用する複製開始点、プロモーター配列、便利な制限酵素切断部位および1つまたは複数の選択できるマーカーを含む(たとえば、WO01/96584、WO01/29058および米国特許番号6,326,193)。
ポリヌクレオチドを宿主細胞に導入する物理的方法は、リン酸カルシウム沈殿、リポフェクション、粒子衝突、マイクロインジェクション、エレクトロポレーションなどを含む。ベクターおよび/または外来核酸を含む細胞を生産する方法は本分野では公知である。たとえば、Sambrookら(2001, Molecular Cloning: A Laboratory Manual, Cold Spring Harbor Laboratory, New York)を参照する。ポリヌクレオチドを宿主細胞に導入する好適な方法は、リン酸カルシウム形質導入である。
ポリヌクレオチドを宿主細胞に導入する化学的手段は、コロイド分散系、たとえば大分子複合体、ナノカプセル、マイクロスフェア、ビーズ、および水中油型乳剤、ミセル、混合ミセル、やリポソームを含む脂質に基づいた系を含む。体外および体内の送達担体(delivery vehicle)として使用される例示的なコロイド系はリポソーム(たとえば、人工膜小胞)である。
本発明の一つの好適な実施形態において、前記ベクターはレンチウイルスベクターである。
本発明は、本発明の第一の側面に記載のCAR−T細胞と、薬学的に許容される担体、希釈剤または賦形剤とを含有する製剤を提供する。一つの実施形態において、前記製剤は液体製剤である。好ましくは、前記製剤は注射剤である。好ましくは、前記製剤で、前記CAR−T細胞の濃度が1×103〜1×108細胞/mL、より好ましくは1×104〜1×107細胞/mLである。
一つの実施形態において、前記製剤は、緩衝液、たとえば中性緩衝食塩水、硫酸塩緩衝食塩水など、炭水化物、たとえばブドウ糖、マンノース、ショ糖やデキストラン、マンニトール、タンパク質、ポリペプチドまたはアミノ酸、たとえばグリシン、酸化防止剤、キレート剤、たとえばEDTAやグルタチオン、アジュバント(たとえば、水酸化アルミニウム)、および防腐剤を含んでもよい。本発明の製剤は、静脈内で施用するように調製することが好ましい。
本発明は、本発明の発現カセットをコードするレンチウイルスベクター(LV)で形質導入された細胞(たとえば、T細胞)で行われる治療的使用を含む。形質導入された細胞は、腫瘍細胞のマーカーであるBCMAを標的とし、協同してT細胞を活性化させ、T細胞の免疫応答を引き起こすことによって、その腫瘍細胞に対する殺傷効率を顕著に向上させることができる。
そのため、本発明は、哺乳動物の標的細胞群または組織に対するT細胞による免疫応答を刺激する方法であって、哺乳動物に本発明のCAR−T細胞を施用する工程を含む方法も提供する。
一つの実施形態において、本発明のCAR−T細胞は安定した体内におけるT細胞の増殖を経て延長した時間で持続することができる。また、CARによる免疫応答は養子免疫療法の工程の一部でもよく、ここで、CAR修飾T細胞はCARにおける高原結合ドメインに特異的な免疫応答を誘導する。たとえば、抗BCMAのCAR−T細胞はBCMAを発現する細胞に抵抗する特異的な免疫応答を引き起こす。
治療可能な癌は、血管新生がしていない腫瘍または血管新生が基本的にしていない腫瘍、および血管新生がした腫瘍を含む。癌は、非固形腫瘍(たとえば血液腫瘍、たとえば白血病やリンパ腫)または固形腫瘍を含んでもよい。本発明のCARで治療する癌の種類は、癌、胚細胞腫瘍や肉腫、および一部の白血病やリンパ性悪性腫瘍、良性腫瘍および悪性腫瘍、および悪性腫瘍、たとえば肉腫、癌やメラノーマを含むが、これらに限定されない。成人腫瘍/癌および児童腫瘍/癌も含む。
固形腫瘍は、通常、嚢腫や液体領域の組織の腫瘤を含まない。固形腫瘍は良性でも悪性でもよい。異なる種類の固形腫瘍はこれらを形成する細胞の種類によって命名される(たとえば肉腫、癌やリンパ腫)。固形腫瘍は、たとえば肉腫および癌の例は、線維肉腫、粘液肉腫、脂肪肉腫、中皮腫、リンパ性悪性腫瘍、膵臓癌、卵巣癌を含む。
体外免疫について、i)細胞の増殖、ii)CARをコードする核酸の細胞への導入、および/またはiii)細胞の凍結保存のうちの少なくとも一つが細胞を哺乳動物に施用する前に体外で行われる。
体外プロトコールは本分野では公知で、そして後記でより完全に検討される。簡単に言えば、細胞を哺乳動物(好ましくはヒト)から単離して本明細書で公開されるCARを発現するベクターで遺伝子修飾を行う(すなわち、体外形質転換または形質導入)。CAR修飾細胞は哺乳動物のレシピエントに施用し、治療の有益な効果を提供することができる。哺乳動物のレシピエントはヒトでもよく、そしてCAR修飾細胞はレシピエント自身の細胞でもよい。必要により、細胞はレシピエントに対して同種異系、同系(syngeneic)または異種でもよい。
体外免疫について細胞に基づいたワクチン以外、本発明は、体内免疫によって患者における抗原に対する免疫応答を引き起こす組成物および方法も提供する。
本発明のCAR修飾T細胞は単独で、または薬物組成物として希釈剤および/またはほかの成分、たとえばIL−2、IL−17やほかのサイトカインまたは細胞群と合わせて施用することができる。簡単に言えば、本発明の薬物組成物は、本明細書に記載の標的細胞群を含み、一つまたは複数の薬学または生理学的に許容される担体、希釈剤または賦形剤と合わせてもよい。このような組成物は、緩衝液、たとえば中性緩衝食塩水、硫酸塩緩衝食塩水など、炭水化物、たとえばブドウ糖、マンノース、ショ糖やデキストラン、マンニトール、タンパク質、ポリペプチドまたはアミノ酸、たとえばグリシン、酸化防止剤、キレート剤、たとえばEDTAやグルタチオン、アジュバント(たとえば、水酸化アルミニウム)、および防腐剤を含んでもよい。本発明の組成物は、静脈内で施用するように調製することが好ましい。
「免疫学的な有効量」、「抗腫瘍有効量」、「腫瘍抑制有効量」または「治療量」と記載する場合、施用される本発明の組成物の精確な量は、患者(対象)の年齢、体重、腫瘍の大きさ、感染または転移の程度および病症の個体差を考慮し、医師によって決められる。通常、本明細書に記載のT細胞を含む薬物組成物は、104〜109個細胞/kg体重の投与量、好ましくは105〜106個細胞/kg体重の投与量(これらの範囲内におけるすべての整数値を含む)で施用することができる。T細胞の組成物はこれらの投与量で数回施用してもよい。細胞は、免疫療法で公知の注入技術(たとえばRosenbergら,NewEng.J. of Med. 319:1676,1988)によって施用することができる。具体的な患者対する最適な投与量および治療プランは、患者の疾患の所見をモニタリングしてそれに基づいて調整して治療することができ、医学分野の技術者によって容易に決めることができる。
(a)本発明のキメラ抗原受容体は、細胞外抗原結合ドメインが特定の抗BCMA scFvで、当該特定の抗BCMA scFvは特定のヒンジ領域および細胞内ドメインと結合してなるCARは非常に強力な腫瘍細胞に対する殺傷能力を示し、しかも細胞毒性が小さく、副作用が低い。
(b)本発明によって提供されるキメラ抗原受容体はCAR遺伝子を担持するレンチウイルスでT細胞を感染させた後、CARタンパク質の安定した発現および膜局在化を実現することができる。
(c)本発明のCAR修飾T細胞は体内で生存時間が長く、かつ抗湯用効果が強く、本発明で使用されるscFvはヒト化抗体またはヒト由来の抗体で、特異的な免疫拒絶反応が生じる可能性が小さい。
レンチウイルス発現ベクターの構築
コードプラスミドの構築は、上海博益生物科技有限公司に全長DNAの合成およびクローンを依頼し、実現させた。pWPTレンチウイルスベクターをクローニングベクターとし、クローニング部位はBamH IおよびSal I部位である。具体的な配列は前記の通りである。
CAR−T細胞の製造
(1)健常者の静脉血を採取し、密度勾配遠心法によって単核球を単離した(PBMCs)。
(2)0日目に、PBMCsを事前に最終濃度が5μg/mLのCD3モノクローナル抗体(OKT3)および最終濃度が10μg/mLのRetronectin(TAKARA社から購入)でコーティングした細胞培養瓶に接種し、培地は1%ヒト血清アルブミン含有GT−T551細胞培地で、培地に最終濃度が1000U/mLになるように組み換えヒトインターロイキン−2(IL−2)を添加し、37℃、飽和湿度、5% CO2のインキュベーターで培養した。
(4)3日目に、新鮮な培養液、濃縮・精製されたCAR−BCMAsレンチウイルス液、プロタミン硫酸塩(12μg/ml)、および最終濃度が1000U/mLのIL−2を入れた。37℃、5% CO2のインキュベーターで12時間感染させた後、培養液を捨て、新鮮な培地を入れ、37℃、5% CO2のインキュベーターで培養を続けた。
(5)6日目から、CART−BCMAs細胞を取って相応する活性検出試験を行うことができた。
CAR遺伝子のT細胞ゲノムにおける組み込み率およびそれによってコードされるタンパク質の膜表面における発現レベルの検出
それぞれ0.5×106の実施例2で7日目(図3A)、21日目(図3B)および29日目(図3C)まで培養したCART−BCMAs細胞のサンプルを取り、組み換えヒトBCMAタンパク質のFc断片で染色した後、フローサイトメーターによってCAR−BCMAタンパク質のT細胞の膜表面における発現レベルを分析した。
結果は図3に示すように、本発明で設計された4つのCAR構造はいずれもその相応する修飾T細胞で発現され、そして細胞膜表面における局在化が完成した。
CART−BCMAsの体外活性化能力の検出
実施例2における7日目まで培養されたCART−BCMAs細胞で細胞の活性化レベル指標であるタンパク質CD137およびIFNγの検出を行った。順に7日目まで培養したCART−BCMA細胞を1×105個取り、それぞれBCMA陽性のK562−BCMA+E7腫瘍細胞系、ならびにBCMA陰性のK562腫瘍細胞系と、あるいは腫瘍細胞を添加せずに、200μlのGT−551培地において1:1の比率で18h培養した後、フローサイトメトリーによってT細胞の膜表面おけるCD137の発現レベルを、ELISA方法によって培養上清におけるIFNγの分泌レベルを検出した。
結果は図4Aおよび図4Bに示すように、4つのCART細胞の表面はいずれもCD137の発現が検出され、かついずれも培養上清にIFNγの発現が検出された。中では、CAR−BCMA−20は最も優れたCD137活性化レベルおよびIFNγ放出レベルを有し、ヒト化MO6抗体配列に基づいて構築されたCART−BCMA−MO6はネズミ由来抗体配列に基づいて構築されたCART−BCMA−CA8と比べて弱いCD137活性化レベルを有するが、高いIFNγ放出レベルを有する。
CART−BCMAs細胞による腫瘍細胞の末期アポトーシスの誘導活性の検出
(1)1:1、2.5:1、5:1、10:1、20:1の比率(図5に示される比率)で、それぞれ実施例2における17日目まで培養されたCART−BCMAs細胞を1×104個のCFSEで標識されたBCMA陰性細胞(NH929)またはBCMA陽性の構築された細胞NH929−BCMA過剰発現腫瘍細胞株と混合し、100μlのGT−551培地において4h共培養した後、100μlの25%PI色素で15min染色し、フローサイトメーターによってCFSE陽性細胞におけるPI陽性細胞の比率を分析した。
(2)1:1、5:1、10:1、20:1、40:1の比率(図6Bに示される比率)で、それぞれ実施例2における22日目まで培養されたCART−BCMAs細胞を1×104個のCFSEで標識されたBCMA陰性細胞(NH929)、BCMA陽性の構築された細胞NH929−BCMA過剰発現腫瘍細胞株、または天然のBCMAを発現するMM.1S細胞系と混合し、100μlのGT−551培地において4h共培養した後、100μlの25%PI色素で15min染色し、フローサイトメーターによってCFSE陽性細胞におけるPI陽性細胞の比率を分析した。
結果は図5および図6に示すように、4つのCART細胞はいずれも好適にBCMA陽性腫瘍細胞のアポトーシスを誘導することができた。中では、CART−BCMA−20はCART−BCMA−1よりも好適にBCMA陽性腫瘍細胞の末期アポトーシスを誘導し、CART−BCMA−MO6およびCART−BCMA−CA8はBCMA陽性腫瘍細胞の末期アポトーシスを誘導する能力が類似する。
CART−BCMAsのRPMI−8226骨髄腫異種移植モデルに対する抑制作用
対数増殖期にあるRPMI−8226細胞を収集し、6−8週のB−NDGマウスの右側背部に4.0×106個の腫瘍細胞を皮下接種し、腫瘍体積が120mm3程度に達した時点で、動物を腫瘍体積によってランダムに4群に各群の腫瘍体積の違いが平均値の10%よりも小さいように分けた後、それぞれ尾静脈から溶媒対照、7.5×106個のNTおよび7.5×106個のCART−BCMAs細胞を注射した。
結果は図7に示すように、対照群と比べ、CART−BCMA−1およびCART−BCMA−20の単回の尾静脈注射は、有効にヒト骨髄腫RPMI−8226細胞の生長を抑制し(相対腫瘍増殖率%T/CRTV≦40%、P<0.05)、かつ顕著にヒト骨髄腫担持マウスの生存時間を延ばし(対照群で生存期間が23日で、CART−BCMAs治療群で生存期間の中央値>33日)、中では、CART−BCMA−1およびCART−BCMA−20による治療を受けたマウスは、相対腫瘍増殖率および生存期間の中央値に有意差がなかった。
本願のキメラ抗原受容体のスクリーニング過程中において、発明者らは大量の候補配列をテストしたが、以下、例を挙げて説明する。
スクリーニングされた抗体は、BCMA−1、BCMA−2、BCMA−69、BCMA−72、BCMA−2A1、BCMA−1E1、BCMA−J22.9、BCMA−20、BCMA−CA8、BCMA−MO6である。上記抗体に基づいて構築された、BCMAを標的とするキメラ抗原受容体の構造である。ここで、BCMA−1およびBCMA−2はすでに発表されたCar−T配列で、スクリーニングの陽性対照として、CAR−T細胞の製造方法は実施例2と同様で、検出方法は実施例3、4と同様である。
結果は図1に示すように、それぞれ2ロットのCAR−T細胞の実施結果である。BCMA−Fc融合タンパク質でCar−Tの発現を検出したところ、初代T細胞に高発現が見られ、図1Aを参照する。図1Bにおいて、BCMA−1、BCMA−20、BCMA−1E1、BCMA−CA8、BCMA−MO6、およびBCMA−J22.9はBCMA抗原によって活性化されたことがわかる。図1Cでは、活性化されたBCMA−1、BCMA−20、BCMA−1E1、BCMA−CA8、BCMA−MO6、およびBCMA−J22.9のCar−Tは比較的に高いIFN−γを生成したことが示された。これらの結果をまとめると、BCMA−1、BCMA−20、CA8およびMO6で得られたCAR−Tは機能が近いことが示されたため、BCMA−20、CA8およびMO6CAR−Tをさらに分析し、研究した。
Claims (12)
- キメラ抗原受容体であって、前記キメラ抗原受容体の抗原結合ドメインはBCMAを標的とする細胞外領域の抗体一本鎖可変領域の配列であることを特徴とする、前記キメラ抗原受容体。
- 前記抗原結合ドメインがBCMA配列の24〜41番目のアミノ酸残基を標的とする抗体一本鎖可変領域の配列であることを特徴とする、請求項1に記載のキメラ抗原受容体。
- 前記抗原結合ドメインの構造が下記式Iで表されることを特徴とする、請求項1に記載のキメラ抗原受容体。

(ただし、VHは抗体の重鎖可変領域で、VLは抗体の軽鎖可変領域で、「−」は連結ペプチドまたはペプチド結合である。

- 請求項6に記載のベクターを含むか、あるいは染色体に外来の請求項5に記載の核酸分子が組み込まれているか、あるいは請求項1に記載のキメラ抗原受容体を発現することを特徴とする宿主細胞。
- CAR−T細胞を製造する方法であって、前記CAR−T細胞は、請求項1に記載のCARを発現する、
請求項5に記載の核酸分子または請求項6に記載のベクターをT細胞内に形質導入することによって、前記CAR−T細胞を得る工程、
を含むことを特徴とする、前記方法。 - 請求項1に記載のキメラ抗原受容体、請求項5に記載の核酸分子、請求項6に記載のベクター、または請求項7に記載の宿主細胞と、薬学的に許容される担体、希釈剤または賦形剤とを含有することを特徴とする製剤。
- 請求項1に記載のキメラ抗原受容体、請求項5に記載の核酸分子、請求項6に記載のベクター、または請求項7に記載の宿主細胞の使用であって、癌または腫瘍を予防および/または治療する薬物または製剤の製造に使用されることを特徴とする、前記使用。
- 前記腫瘍が、BCMA陽性腫瘍であることを特徴とする、請求項10に記載の使用。
- 前記腫瘍が、BCMA陽性のB細胞性リンパ腫、多発性骨髄腫、または形質細胞性白血病であることを特徴とする、請求項10に記載の使用。
