Abstract
Description
真性糖尿病(すなわち、糖尿病)は、ホルモンインスリンを産生するまたはホルモンインスリンに応答する身体の能力が損なわれ、その結果炭水化物の代謝異常と血中および尿中のブドウ糖レベルの上昇が起こる疾患である。疾患は、いくつかのサブタイプに細分され、1型糖尿病、インスリン依存性糖尿病(IDDM)、若年者の成人発症型糖尿病(MODY)、成人潜在性糖尿病(LADA)、不安定型糖尿病、痩せ型糖尿病、1.5型、2型、3型、肥満関連糖尿病、妊娠糖尿病、および当分野によって受け入れられる他の命名法として、代替的に記載される。
インスリン依存性糖尿病患者は、新しいインスリン産生細胞の移植により、治癒する可能性があるが、これらの細胞が十分な量と質で得ることが困難であるため、このアプローチはこれまでに限定的であった。例えば、Pagliuca FW,et al.Cell,154(2):428−439(2014)を参照されたい。したがって、より効率的かつ予測可能な方法でヒト幹細胞からインスリン産生β細胞を生成することは、生体医学研究の長い間の目標であった。同上。この目標を達成するためには、ブドウ糖に曝されるとインスリンを生成する均一なβ細胞集団を生成するプロトコルを確立しなければならない。しかしながら、このようなプロトコルはまだ難しいままである。多くの研究グループが異なる細胞株を使用して様々な結果をもたらした様々なプロトコルを提案してきた。このような機能性β細胞の一貫性のない産生は、治療上の利益を達成するために必要な全細胞用量を増加させるため、潜在的な治療のコストを増加させ、変わりやすい結果のために臨床適用性を制限する。
本明細書で使用される場合、「約」という用語は、当業者によって理解され、それが使用される文脈に応じてある程度変化する。用語が使用される文脈から考えると、当業者には明確でない用語の使用がある場合、「約」は、特定の用語のプラスまたはマイナス10%までを意味する。
従来の細胞ベース療法の1つの限界は、異なる細胞が成熟した細胞型に分化する異なる傾向を有することである。例えば、開始細胞集団のエピジェネティックなサインは、「エピジェネティックメモリ」と呼ばれる現象である、リプログラム化された細胞において持続することができることが報告されている。その結果、iPS細胞および他のリプログラム化された細胞は、それらが由来した同じ胚葉に属する細胞に優先的に分化し得る。したがって、いくつかの実施形態では、インスリン産生細胞を生成するための開示された方法で使用される幹細胞は、多能性または多分化能幹細胞にリプログラム化された成熟内胚葉細胞に由来し得る。いくつかの実施形態では、開示される方法において使用される幹細胞は、再プログラムされたヒト膵臓細胞に由来し得る。かかるドナー膵臓細胞は、糖尿病で治療されている対象(すなわち、自己ドナー)または糖尿病で治療されていない人(すなわち、同種異系ドナー)から得られることができる。いくつかの実施形態では、開示される方法において使用される幹細胞は、同意した健常な成人ドナー膵臓のランゲルハンス島からのリプログラム化された初代細胞であってもよい(例えば、図1Aを参照されたい)。
哺乳動物幹細胞(例えば、iPS細胞、胚性幹細胞、およびリプログラム化された細胞)が、胚膵臓発生を模倣することによってインスリン産生β細胞に分化することができることが示されている。例えば、Borowiak M.et al.Curr Opin Cell Biol.,21:727−32(2009)を参照されたい。多能性細胞は段階的に膵臓細胞に分化する。第1段階は、内胚葉系譜に向けた分化である。内胚葉細胞はさらに多分化能膵臓始原細胞に分化させ、次いで、それは島細胞に分化させ、次いで、ブドウ糖曝露時にインスリンを産生するβ細胞に分化させることができる。現在の分化プロトコルは、これらの段階をインビトロで模倣しようとしているが、これらの分化プロトコルの臨床適応の効率と成功率はかなり異なる。例えば、Pagliuca FW,et al.Cell,154(2):428−439(2014)を参照されたい。確かに、現在の分化プロトコルは、それらが適用される個々の細胞に応じて適応されなければならないことが多い。これにより、今日まで、普遍的で標準化された分化プロトコルの確立が妨げられている。実際、現在の分化プロトコルからのいくつかの一貫したガイダンスは、異なる遺伝子背景からの幹細胞株の分化を阻害し得る。本明細書に開示される改善された方法は、当該技術分野におけるこの欠陥に対処する。例えば、いくつかの実施形態では、開示されるプロトコルは、細胞特異的であり得る他の分化プロトコルとは対照的に、様々な細胞源からインスリン産生β細胞を堅牢に生成することができる。
本明細書に開示されるインスリン産生細胞は、治療を必要とする対象における糖尿病を治療するために使用され得る。いくつかの実施形態では、治療を必要とする対象は、インスリン依存性糖尿病を有する哺乳動物、例えばヒト対象である。
島の収穫:登録臓器提供から適切に同意され、匿名化されたヒト全膵臓が得られた。葉に、島分離液(0.35gのNaHCO3/Lおよび1%ヒト血清アルブミン(Roche A9731)を含有するHanks平衡塩類溶液(Invitrogen#14065−056))中に1.4mg/mlで再懸濁されたコラーゲナーゼP(Roche#1129 002 001)を注入した。膨張した葉を37Cで15〜25分間、軽度の撹拌でインキュベートした。消化物を冷島分離液で希釈し、1500RPMで5分間遠心分離した。上清を廃棄し、ペレットを激しく粉砕しながら冷島分離液中で洗浄した。この溶液を420μmのふるい(Bellco Glass,Inc,Cat#1985−00040)を通して濾過し、遠心分離した。ペレットを1.100g/mlのHistopaque(Sigma #10771、Sigma #11191)中に再懸濁し、1200RPMで30分間遠心分離した。上清を収集し、島分離液中で2倍希釈し、1500RPMで5分間遠心分離した。ペレットを島単離液中ですすぎ、遠心分離し、加湿インキュベータ中、37Cおよび5%CO2でE8培地(Gibco #A1517001)で培養した。翌日、島を1500RPMで5分間遠心分離し、未希釈TryplE Select 10X(Life Technologies #A12177)に再懸濁し、37Cで10分間インキュベートした。解離した島をE8培地中で希釈し、遠心分離し、100ng/mLのヒドロコルチゾン(Sigma#H0135)、1U/mLのトロンビン(Sigma#T9326)、および100ng/mLのEGF(Sigma E5036)を補充したE8中に再懸濁した。細胞を、メーカーの指示に従って、ビトロネクチン(Life Technologies #A14700)でコーティングした皿上で培養した。
細胞株誘導体。iPSC株は、成熟細胞の核にリプログラミング遺伝子を導入することによって生成された。これらの遺伝子、典型的には、OCT4、Sox2、KLF4、およびc−Mycは、胚性幹細胞の遺伝子発現パターン、形態、および挙動を採用するように誘導した。この効果の持続時間は不明であるが、「エピジェネティックメモリ」と呼ばれる現象であるリプログラム細胞において、開始細胞集団のエピジェネティックサインが持続することが報告されている。膵臓系譜に効率的に分化するiPSC株を生成する可能性を最大限に引き出すために、同意された健康成人ドナー膵臓のランゲルハンス島からの初代細胞をリプログラミングのため選択した(図1A)。
)、正常な核型を有する(図3B)。そのDNA STRプロファイルは、ドナー組織と一致し(図3C)、NIH、ATCC、およびDSMZデータベース内のすべての指紋と比べてユニークである単一の細胞株を確認する。加えて、SR1423は、多能性細胞株に典型的な速度で増殖する(図3D)。同じドナーおよびリプログラミング実験からの他のiPSC細胞株も優先分化を示したことが観察された。IPSC株「B」はまた、内胚葉に対して良好に分化したが、iPSC株「C」および「D」は内胚葉系譜への分化の優先性を示さなかった(データは示さない)。発現遺伝子の全ゲノムマイクロアレイプロファイリングをSR1423ならびに株B、C、およびDにおいて行った。この比較によって、ドナーまたはリプログラミング方法による遺伝子発現の違いが排除された。少なくともLog2の倍率変化発現に基づく教師なし階層的クラスタリング分析により、SR1423クラスタが細胞株Bとともにクラスタリングされたが、CおよびDとは異なっていたことが明らかになった。これにより、内胚葉系譜に対する堅牢かつ優先的な分化と相関する遺伝子発現パターンを同定する(図4A)。10個の最も差次的に発現された遺伝子のうち、BHMT2、Cox7A1、HSPB2、およびNAP1L1は、qRT−PCR測定を使用する内胚葉を形成する能力と有意に相関した(図4B)。これらの結果は、定義された遺伝子サブセットの遺伝子発現を使用して、治療的有用性を有する特定のiPSC株を同定することができることを示唆する。
アルギン酸塩カプセル化:SR1423の分化平面培養物をセルリフターを使用して手動で遊離させ、6ウェル浮遊培養皿中で95rpmで一晩揺り動かした。形成されたクラスタを130mMのNaCl、10mMのMOPS、pH7.4中ですすぎ、2%Pronva UP MVGアルギン酸塩(Novamatrix)中に2E6細胞/mlの密度で再懸濁した。アルギン酸塩/細胞混合物をシリンジに充填し、20mMのBaCl2、130mMのNaCl、10mMのMOPSの重合浴に、0.24μmノズルで4ml/分および7kVでNisco静電液滴発生器を介して供給したか、または手動で滴下した。ビーズを4回すすぎ、移植まで分化培地に戻した。
多能性幹細胞を単離し、幹細胞をインスリン産生膵臓系譜に効率的に分化するための開示された方法は、非ヒト組織から始めて、非膵臓組織から始めるときにも有効である。アカゲザル種の3つの非ヒト霊長類(NHP)の皮膚生検から線維芽細胞を採取し、ヒト細胞について記載したものと同様に培養した(すなわち、実施例1の培養および分化ステップ)。リプログラミング遺伝子Oct4、Sox2、Klf4、およびL−Mycを、2つのエピソーム発現プラスミドのエレクトロポレーションを介して初代線維芽細胞に導入した。NHPドナーの一次組織から生成された幹細胞株は、多能性マーカーOct4、SSEA4、Tra−1−80、およびTra−1−60を発現した。これらの株を、内胚葉誘導剤アクチビンAおよびワートマニンに4日間曝露した後に、内胚葉マーカーを発現する能力についてスクリーニングした。NHPドナーの各々から内胚葉マーカーを発現する細胞の割合が最も高い培養物を選択し、その後、12日間の膵臓分化プロトコルに曝露した後に膵臓マーカーを発現する能力についてスクリーニングした。3つすべてのNHPドナーからの組織は、記載の12日間の分化プロトコルに曝露すると、膵臓内胚葉細胞を効率的に生成する少なくとも1つの幹細胞株を得た。図9は、NHPドナーに由来するこれらの株のうちの1つの代表的な例を示す。
この実施例は、ヒト成人におけるI型糖尿病を治療するための治療細胞を生成するために開示されたプロトコルを使用する方法を例示する。

b.内分泌誘導因子を含む第2の培地で(a)からの前記内胚葉細胞を培養することにより、前記内胚葉細胞を内分泌細胞に分化させることと、を含み、
前記哺乳動物幹細胞が、内胚葉細胞への分化前にケラチノサイト増殖因子(KGF)に曝露されなかった、方法。 - 前記哺乳動物幹細胞が、ヒト幹細胞である、請求項9に記載の方法。
- 前記哺乳動物幹細胞が、非ヒト霊長類幹細胞である、請求項9に記載の方法。
- 前記内胚葉誘導因子が、アクチビンAを含む、請求項9〜11のいずれか一項に記載の方法。

- 前記第2の培地が、ノギンを含む、請求項9〜16のいずれか一項に記載の方法。
- 前記第2の培地が、KGFを含む、請求項9〜17のいずれか一項に記載の方法。
- 前記細胞が、前記第2の培地で1〜4日間培養される、請求項9〜18のいずれか一項に記載の方法。
- KGFを含む第3の培地で前記内分泌細胞を培養することにより、前記内分泌細胞を膵臓始原細胞に分化させることをさらに含む、請求項9〜19のいずれか一項に記載の方法。
- 前記第3の培地が、ノギンおよび上皮増殖因子(EGF)を含む、請求項20に記載の方法。
- 前記第3の培地が、レチノイン酸およびシクロパミンを含む、請求項20または請求項21に記載の方法。
- 前記細胞が、前記第3の培地で1〜4日間培養される、請求項20〜22のいずれか一項に記載の方法。

b.レチノイン酸およびシクロパミンを含む第2の培地で(a)からの前記内胚葉細胞を培養することにより、前記内胚葉細胞を内分泌細胞に分化させることと、
c.KGF、ノギン、およびEGFを含む第3の培地で(b)からの前記内分泌細胞を培養することにより、前記内分泌細胞を膵臓始原細胞に分化させることと、
d.ノギン、EGF、γ−セクレターゼ阻害剤XXI、およびAlk5i IIを含む第4の培地で(c)からの前記膵臓始原細胞を培養することにより、前記膵臓始原細胞をインスリン産生細胞に分化させることと、を含む方法。 - 前記第2の培養培地が、KGFをさらに含む、請求項42に記載の方法。
- 前記第3および第4の培地が、レチノイン酸およびシクロパミンをさらに含む、請求項42または請求項43に記載の方法。
- Alk5I IIおよびレチノイン酸、ならびに任意選択的にシクロパミンを含む第5の培地で(d)からの前記インスリン産生細胞をさらに培養する、請求項42〜44のいずれか一項に記載の方法。
- Alk5i II、ニコチンアミン、IGF−I、ならびに任意選択的にレチノイン酸およびシクロパミンを含む第6の培地で前記細胞をさらに培養することをさらに含む、請求項45に記載の方法。

a.内胚葉誘導因子を含む第1の培地内の接着性基質上でヒト幹細胞の集団を培養し、前記哺乳動物幹細胞はWnt3aに曝露されないことと、
b.レチノイン酸およびシクロパミンを含む少なくとも1つの追加の培地で前記細胞をさらに培養することと、
c.前記細胞が三次元細胞構造を形成するときに、前記細胞を浮遊培養に移すことと、を含み、
前記細胞は、少なくとも20日間、レチノイン酸およびシクロパミンに曝露され、前記細胞は、少なくとも20日間、レチノイン酸およびシクロパミンに曝露される方法に従って誘導される、細胞ベースの組成物。 - 前記内胚葉誘導因子が、アクチビンAおよびワートマニンを含む、請求項52に記載の細胞ベースの組成物。
- 前記ヒト幹細胞が、膵臓一次組織に由来する、請求項49〜53のいずれか一項に記載の細胞ベースの組成物。
- 前記ヒト幹細胞が、ヒト胚性幹細胞である、請求項49〜53のいずれか一項に記載の細胞ベースの組成物。
- 前記ヒト幹細胞が、人工多能性幹細胞である、請求項49〜53のいずれか一項に記載の細胞ベースの組成物。
- 前記ヒト幹細胞が、多能性ではないリプログラム化された細胞である、請求項49〜53のいずれか一項に記載の細胞ベースの組成物。
- 前記リプログラム化された細胞が、リプログラミング遺伝子を、前記細胞のゲノムに前記リプログラミング遺伝子を組み込むことなく発現することによって、リプログラム化されている、請求項57に記載の細胞ベースの組成物。
- 前記リプログラミング遺伝子が、少なくとも1つのエピソーム発現プラスミド上にコードされる、請求項58に記載の細胞ベースの組成物。
- 前記リプログラミング遺伝子が、Oct4、Sox2、Klf4、およびL−Mycを含む、請求項59に記載の細胞ベースの組成物。

- 内胚葉系譜に優先的に分化する未分化細胞を同定する方法であって、未分化細胞におけるBHMT2およびNAP1L1の発現を評価することと、BHMT2の発現が対照細胞と比較して下方制御され、かつNAP1L1の発現が対照細胞と比較して上方制御される場合に、前記細胞を内胚葉系譜に分化する優先度を有するものとして同定することと、を含む方法。
- 前記未分化細胞におけるCox7A1およびHSPB2の発現を評価することと、Cox7A1およびHSPB2の両方の発現が対照細胞と比較して下方制御される場合、前記細胞を内胚葉系譜に分化する優先度を有するものとして同定することと、をさらに含む、請求項63に記載の方法。
- 前記対照細胞が、前記内胚葉系譜への優先的分化を示さないまたは中胚葉系譜に分化することが実質的に不可能である、多能性細胞である、請求項63または64に記載の方法。
- BHMT2の発現が、前記対照細胞に対して少なくとも2log下方制御され、かつNAP1L1の発現が、前記対照細胞に対して少なくとも2log上方制御される、請求項63に記載の方法。
- Cox7A1およびHSPB2の両方の発現が、前記対照細胞に対して少なくとも2log下方制御される、請求項66に記載の方法。
- GLIS2、CCDC58、MTX3、およびC7orf29の発現を評価することをさらに含む、請求項63または64に記載の方法。
