Abstract
Description
本出願は、2016年9月15日出願の米国特許仮出願第62/395,319号、2016年8月24日出願の同第62/379,175号の利益を主張する。これら出願のそれぞれの内容は参照によりその全体が本明細書に組み込まれる。
ジストニアは、異常な頻発することが多い反復運動、異常姿勢、または両方を生ずる主動筋および拮抗筋の持続的または間欠的筋肉収縮を特徴とする。ジストニー運動は、通常パターン化し、捻転性であり、また、振戦性の場合がある。ジストニアは多くの場合、意志行為により開始または悪化し、また、過剰筋活動に関連する。ジストニアは次の2つの軸:発症年齢、体内分布、時間的パターンおよび関連特徴(追加の運動障害または神経学的特徴)を含む臨床的特徴、ならびに神経系病理学および遺伝を含む病因学に沿って分類される(Albanese 2013a)。
プリドピジン(4−[3−(メチルスルホニル)フェニル]−1−プロピルピペリジン)(以前にはACR16として知られていた)は、ハンチントン病の治療用として開発中の薬物である。プリドピジンは、運動亢進を抑制するか、または機能低下を改善することにより運動活性を調節することが示されている。プリドピジンの神経保護特性は、そのσ1受容体(S1R、結合IC50=約100nM)に対する高親和性が原因であることが示唆され、一方、プリドピジンの運動活性は、主に、ドーパミンD2受容体(D2R)(結合IC50=約10μM)に対する低親和性、アンタゴニスト活性により媒介され得る(Ponten 2010)。プリドピジンは、別の受容体にマイクロモル範囲の低親和性結合を示す。

本発明による使用のための化合物は、原料化合物の形態で投与することができるが、必要に応じて、活性成分を生理学的に許容される塩の形態で、1種または複数のアジュバント、賦形剤、担体、緩衝剤、希釈剤、および/または他の慣用的な医薬助剤と共に医薬組成物中に導入することが好ましい。
本明細書で使用される場合、別段の記載がない限り、以下の各用語は、下記の定義を有するものとする。

健康な被験者の8つの試験(そのうちの1つの試験には統合失調症の患者も含まれていた)、パーキンソン病の患者の1つの試験、統合失調症の患者の2つの試験(上記で述べた試験を含む)、およびHDの患者の6つの試験(1つのオープンラベル継続試験を含む)を含む、プリドピジンを用いた16の臨床試験が完了している。加えて、HDの患者のプリドピジンのための人道的使用プログラムがヨーロッパで進行中であり、またオープンラベル、長期安全性試験が米国(US)およびカナダで進行中である。これらの試験の概要は、国際公開第2014/205229号で発表されている。この特許の内容は、参照により本明細書に組み込まれる。
実施例1:ハンチントン病患者の対症療法のためのプリドピジン45mg、67.5mg、90mg、および112.5mgの1日2回投与とプラセボ投与を比較して安全性および有効性を評価する、第II相の用量設定、ランダム化、並行群、二重盲検プラセボ対照試験(「PRIDE−HD」)
PRIDE−HD試験は、ハンチントン病統一評価尺度(UHDRS)総運動スコア(TMS)を用いて、少なくとも52週にわたる1日2回(bid)のプリドピジン45mg〜112.51mgのHD患者の運動障害に対する有効性を評価した。この試験はまた、改訂版身体機能検査(mPPT)に対する、プリドピジン45mg〜112.5mgのbidによる少なくとも52週の治療の効果を評価する。この試験はまた、UHDRS尺度の全機能的能力(TFC)および認知機能評価バッテリー(CAB)に対するプリドピジン45mg〜112.5mgのbidによる少なくとも52週の治療の効果を評価する。この試験はまた、全ての患者からのデータをHD亜集団で得られたデータと比較する。この試験はまた、(i)少なくとも52週の治療の間、HD患者への一連のプリドピジン投与量の安全性および耐容性を評価し、(ii)試験集団でのプリドピジンの薬物動態(PK)を調査し、(iii)プリドピジンへの暴露と結果の測定値との間の関連性(例えば臨床的有効性と毒性パラメータ)を調査した。
一般的設計と試験スキーム
これは、HDの運動障害の治療において、プリドピジン45mg、67.5mg、90mgおよび112.5mgのbidを、プラセボと対比して、有効性および安全性を比較するランダム化、並行群、二重盲検、プラセボ対照試験であった。
患者に対するプリドピジンの投与は、表2にまとめている。試験の手順および評価は、表3にまとめている。選別手順およびその他の手順を含む詳細な臨床手順は、米国特許出願公開第2014/0378508号および国際公開特許第2014/205229号の実施例3に記載されている。これらの特許の内容は、参照により本明細書に組み込まれる。
この試験のための主要な有効性変数およびエンドポイントは、26週または52週目でのUHDRS TMS(全てのUHDRS運動ドメイン評点の合計として定義される)のベースラインからの変化とした。運動障害の主要尺度は、UHDRS運動評価セクションであり、これは、訓練した検査者により管理された。運動評価の第1の部分は、下記に提供される5つのTMSサブスコアからなる。全31項目の総合計は、総運動スコア(TMS)と呼ばれる。副次的な有効性変数およびエンドポイントは、26週および52週目のmPPTのベースラインからの変化であった。TMSスケールは、ジストニアの測定を含む。

耐容性変数およびエンドポイントには次のものが含まれる:

主要なPK測定はプリドピジン血漿濃度の決定であった。濃度はプリドピジン母集団PKモデルにも組み込まれ、また試験患者に対する個々の曝露(CmaxおよびAUC)が計算された。
プリドピジン(プリドピジン塩酸塩として)は、45mgプリドピジンを含有する白色ゼラチンカプセルサイズ2、および22.5mgのプリドピジンを含む白色硬質ゼラチンカプセルサイズ4として提供された。プラセボは、22.5mgまたは45mgプリドピジンカプセルに一致するが、活性成分は含有せず、賦形剤のみ(ケイ酸化微結晶セルロースおよびステアリン酸マグネシウム)を含有する白色の硬ゼラチンカプセルとして提供された。
a.これらの来診(V0およびV4〜12)の手順および評価は、所定の来診時間枠内に完了する限り、数日間にわたり実施してよい。
b.選択規準/除外基準は、患者がランダム化される前に、0日目の選別および再調査時に適合しなければならない。
c.電解質のみ。
d.スクリーニング時の血清妊娠検査(確認のため、必要に応じて尿検査を含む);その後の時点での尿妊娠検査。血清妊娠検査の不確かな読みは2回チェックすべきであり(尿検査)、必要であれば患者を婦人科医に照会する。
e.スクリーニングでは、1回のECGが行われた。スクリーニング時に延長されたQTcF間隔(>450ミリ秒のQTcF間隔として定義される)の証拠が検出された場合には、ECGを2回繰り返し、3回のスクリーニング測定値の平均値を、患者が当該試験に含めるのに適しているかどうかを決定するために使用した。
f.ベースライン来診時において、投与前QTcFは、各々の3回繰り返したECG(互いに10〜20分以内)(合計9回の記録)の平均値によって決定された。投与後ECGは、最初の投与の1〜2時間後に3回繰り返して実施された。PKサンプルは、現場で最初の用量の投与前およびその投与の1〜2時間後に収集された。ECGと同時になるときには、PK試料はECG記録の後に収集される。
g.1回のECGは午後の投与の前、およびその投与の1〜2時間後に3回繰り返して行われる。
h.ECGは、地方条例により要求されない限り、8週目はオプションである。ECGは、追加のECGを正当化する臨床的環境がある場合、例えば、以前にQT延長を伴わない低カリウム血漿の発症を有する患者の場合に、試験責任医師の裁量によって行われるべきである。
i.52週目に、3回繰り返したECGおよびPK試料を、最後の試験(朝)の投与の前に採取された。
j.ECGは経過観察来診時のオプションであるが、以前に観察された心臓の懸念および/またはベースラインからのQTc変化がある全ての患者については行われるべきである。
k.CAG、チトクロムP450 2D6の状況、遺伝的長QT症候群(試験中止に導くような、研究薬物の投与に続いてQT延長を経験する患者においてのみ評価する)、またはプリドピジン応答またはハンチントン病に関連したいずれか他の遺伝的解析を含む。

q.4、6、16週目に、午後の投与の前および投与の1〜2時間後にPK試料が採取された。ECGと同時になるときには、PK試料はECG記録の後に収集された。
r.最後の試験(52週目)には、試験薬投与は、投与前のPK試料の取得の後で、試験実施現場で行われる。
s.経過観察来診では、1PK試料を採取した。SAEの場合、追加のPKサンプリングはSAEに最も近い時点で収集することを目的とすべきである。ECGと同時になるときには、PK試料はECG記録の後に収集された。
t.この情報は、併用薬の調査の一部として収集された。
u.採取のみ。
v.試験遵守は、TC中に再調査される。
w.全ての患者は、全試験期間中、3カプセルを1日2回(bid)、すなわち、朝に3カプセルおよび午後(朝の投与後7〜10時間)に3カプセルの投与を受けた。試験薬は、早期終了の来診時には投与されなかった。訪問調査時には、午後の投与は施設の現地で実施される。
x.安全性または耐容性の理由のために、試験薬治療を停止しなければならない患者は、試験を継続し、試験薬を摂取しない場合に概説された来診スケジュールに従うことを要求された。
UHDRSは、HDに関連した特徴の広範な評価を含んでいる(Huntington Study Group 1996)。UHDRSは、HDの臨床的特徴および経過の均一な評価を提供するために開発された研究ツールである。UHDRSのTMS成分は、UHDRSの15項目からの31の評価を含み、各評価は0(ノーマル)〜4(最大異常)の5点尺度で評価される。
副次的な有効性変数およびエンドポイントの、改訂版身体機能検査(mPPT)は身体的作業における患者の動作を定量化する(Brown 2000)。それは、機能的作業に対する患者の動作を測定する標準化された9項目の試験である。支援機器は、立位を必要とする作業(項目6〜9)のために許可される。患者が項目を完遂する速度と精度の両方がスコアリング時に考慮された。この試験の最大スコアは36であり、より高いスコアはより良い動作を示す。
臨床医の問診に基づく変化の印象+介護者入力
CIBIC−Plus(バージョンADCS−CGIC)は、アルツハイマー病における抗認知症薬の試験において開発、検証されたものであり、この種の試験によく使用される(Joffres 2000)。独立の評価者が、プリドピジンまたはプラセボの開始に先立って、患者の全体的な疾患の重症度を評価した。CIBISとして知られているこの評価は、極めて厳しいHDから、無症状HDまでの7点リッカート尺度で患者を評価する。
PDSは、試験中に身体障害の尺度として使用された。患者は、10(「全面介護を必要とする固定姿勢−胃切開、カテーテル挿入」)から100(「正常;明らかな疾患なし」)までのスケールで採点された(Myers 1991)。
UHDRSのFAスケールは、日常生活の25の作業において機能を評価した(例えば、「彼/彼女は慣れた仕事での有給雇用に従事することができますか?」)。各質問には、「はい」または「いいえ」で答えた。
CGI−Sは、ベースラインで評価され、CGI−Cは、ベースラインからの変化を評価するために、以降の全ての時点で使用された。CGI−Sスケールは、最初は精神障害患者の治療応答を評価するために設計された(Guy 1976)が、今では広範囲の疾患において広く使用されている。
UHDRSのTFCスケールは、身体障害に関連した次の5つの機能ドメイン(仕事、収入の管理、家事(例えば、洗濯、皿洗い)、日常生活動作、ケアレベル)を評価するために使用される標準化スケールである。全機能的能力スコアは、0〜13の範囲であり、疾患進行を照準とした試験のための十分に確立されたエンドポイントである。全機能的能力スコアは、Huntington Study Group(HSG、1996)により開発され、複数試験で20年にわたり実施され、監督官庁により認められている。

家事:0=不可、1=障害、2=正常。
ADL:0=全面介護、1=大雑把な作業のみ、2=最小限の障害、3=正常。
ケアレベル:0=全時間熟練看護、1=自宅または長期ケア、2=自宅。
UHDRSの自立性尺度は、患者の自立性の程度が10%(チューブ栄養、全ベッド介護)〜100%(特別なケアは必要ない)までのパーセンテージで与えられる評価尺度である。
ハンチントン病の生活の質
HD−QoLは、健康関連生活の質を測定するための標準化された測定器具である(Hocaoglu 2012)。これは、HD用に設計された検証済みの疾患特異的尺度であり、全体的な健康関連生活の質の要約尺度だけでなく、いくつかの個別の尺度に対するスコアを提供することができる。

多発性硬化症歩行スケール
多発性硬化症歩行スケール(MSWS−12)は、歩行および移動性の一般的な尺度になるように構成され、ウォーク12と改名された。
EQ5D3レベルバージョン(EQ5D−3L)が、1990年に導入された(EuroQol Group 1990)。これは基本的にEQ5D記述システムと、EQ視覚的アナログスケール(EQ VAS)からなる。EQ5D−3L記述システムは、次の5次元を含む:運動、セルフケア、通常の動作、疼痛/不快感、および不安/鬱。
運動障害は、異なるQ運動評価を用いて客観的に評価することができる。全てのQ運動評価は、多施設臨床試験において、予め検量され、温度制御された力変換器ならびにセッションおよび現場間にわたる非常に高感度で、試験・再試験による信頼性のある三次元位置センサの適用に基づいている。したがって、Q運動測定は、カテゴリ的な臨床評価尺度の限定された感度、評価者内および評価者間での変動、およびUHDRS−TMSなどの尺度において観察されるプラセボ効果を減少させることを目的としている。加えて、Q運動評価は、臨床試験における意図しない運動の副作用の客観的なモニターを可能にする。したがって、Q運動は、客観的で信頼性が高く、高感度の、評価者による偏りやプラセボ効果の影響が限定されている運動機能の尺度である。図10は、正常な患者、軽度の障害のある患者および重度の障害のある患者のQ運動タップ測定値を示す。Track−HD試験では、症状発現前および初期段階HDのQ運動タッピング傷害の最大の自然史研究は、臨床スコアならびに限局性脳萎縮と相関した(図11a、11b、12およびBechtel 2010)。

この作業は、手の掌と背側面との間で交互に繰り返し回内/回外運動を行った場合に実施するハンドタップの規則性を評価した。ハンドタップの力と持続時間が、高速化タッピング作業と同様に記録される。評価の開始と終了を音声により合図する。持続時間10秒の5回の試験が、両手に対し行われる。
身体の両側で、手のひらが下を向くように前腕部と手を回転する(回内)および手のひらが上を向くように前腕部と手を回転する(回外)能力の評価。
この作業では、精密グリップにおける等角把持力に関する親指と人差し指の間の協調を評価する。把持力は、把持の開始段階、物体を運ぶ段階、および静的保持段階の間で評価される。患者は、精密グリップ中に力変換器と3次元位置センサを搭載した装置を親指と人差し指との間に掴んで持ち上げ、10センチの高さのマーカーに隣接させて安定に保持するように指示される。把持力と、対象物の3次元位置および向きを記録する。静止段階における平均等角把持力および把持力変動(変動係数=標準偏差[SD]/平均値x100として表される)(GFV−C)が、最初の合図音の8秒後に開始して15秒間計算される。20秒の持続時間の5回の試験が両手に対し行われる。舞踏病は、「位置指数」および「配向指数」を算出して評価される。評価の開始および終了は合図音によって通知される。

TUGは、人の移動を評価するために使用する簡単なテストで、静的および動的の両方のバランスを必要とする。それは、人が椅子から立ち上がって3メートルを歩き、向きを変えて、椅子へ歩いて戻り、座るのに要する時間を用いる。試験中、人は彼らの通常の履物を着用し、彼らは通常、必要となる任意の移動補助手段を使用することが予測される。TUGは管理が容易であり、一般的に殆どの高齢者が完結できるので、高齢者集団において頻繁に使用される。この試験は迅速で、特別な機器やトレーニングを必要とせず、定期的な健康診断の一部として容易に組み込める(Podsiadlo 1991)。UHDRSと併せてTUG試験を使用することが、HDの臨床試験に対し推奨されている(Rao 2009)。
次の6セクションでは、簡易CABの一部である試験を説明する。
1.符号数字モダリティー検査
SDMTは、精神運動速度および作業記憶の筆記試験である。
2.感情認識
6つの基本的な感情または中性の表現を描いた写真のコンピュータ化された提示を使用して、感情の顔表現の感情認識が検査される。
3.トレイルメイキングテストAおよびB
視覚的注意および作業切り替えが、紙の標準シート上の25の円からなるトレイルメイキングテストを用いて評価される。トレイルAの場合、参加者は、数字を含む円を可能な限り迅速に数値の昇順で接続することを要求される。トレイルBでは、参加者は数字および文字を含む円を、数字と文字の間で交互に、昇順で、可能な限り迅速に接続することを要求される(例えば、1、A、2、B、3、C等)。
4.改訂版ホプキンス言語学習テスト
HVLT−Rは、言語学習と記憶(認識と想起)の簡易評価を提供する。
5.ペースタッピング試験
精神運動機能を、一定ぺースでのタッピング試験で評価する。参加者は、親指で交互に3.0Hzでマウスの左ボタンと右ボタンをタップする。彼らは最初に所望のタップ速度の提示音に耳を傾け、その後、その音に合わせてタッピングを始める。音と共に11タップした後に、音の繰り返しは中断され、参加者は試験終了まで(31タップ以降)同じ速度でタップの継続を試みる。
6.ケンブリッジのワンタッチストッキング(One Touch Stockings of Cambridge)(OTS)
OTSは、前頭葉機能の尺度を与える空間計画作業である。OTSは、ケンブリッジのストッキング作業の変形であり、参加者が解を視覚化しなければならないため、作業記憶に大きな需要をおく。
HDでは精神病的症候が顕著なので、行動評価を必要とする全てのHD研究においては、最も一般的な精神症状の包括的スクリーニングとしてPBA−s形態を使用することが推奨される(Craufurd 2001、Kingma 2008)。
この実施例では、安全性は次の事項を評価することにより、資格を持った試験スタッフにより評価された:報告された有害事象、臨床検査結果、バイタルサイン測定、ECG所見、(体重を含む)身体的および神経学的検査結果(体重を含む)、および併用薬の使用状況。
臨床検査(電解質、血液学および尿検査を含む血液生化学検査)を下記に記載のように実施した。
脈拍、血圧、および体温を含むバイタルサインが測定された。
主要なPK測定はプリドピジン血漿濃度の決定である。濃度はプリドピジン母集団PKモデルにも組み込まれ、また試験患者に対する個々の曝露(CmaxおよびAUC)が計算された。
用量設定期間:0日目(ベースライン)−最初の投与の前およびその1〜2時間後および14日目の午後の投与後の1〜2時間後。総量治療期間:28日目−午後の投与前および午後の投与の1〜2時間後、42日目−午後の投与前および午後の投与の1〜2時間後、84日目−午後の投与の1〜2時間後、112日目−午後の投与前および午後の投与の1〜2時間後、140日目−午後の投与の1〜2時間後、182日目−朝の投与前および追跡調査来診時。
試料の、プリドピジンおよびその主な代謝物TV−45065(以前はACR30と呼ばれた)が、適切な実証された方法を用いて分析された。血漿中のプリドピジンおよびTV−45065の定量下限値は、それぞれ約1.6〜1.8ng/mLおよび1.5〜1.9ng/mLである。
血液試料(10mL)を、遺伝子分析のための選別来診時に、2つのエチレンジアミン四酢酸二カリウム(K2EDTA)プラスチック試験管内に回収した。分析には、CAG反復、CYP2D6の状態、および遺伝性QT延長症候群、またはプリドピジン応答もしくはHDに関連する任意の他の遺伝子分析を含む。
UHDRS−TMSのベースラインからの変化は、反復測定モデル(REPEATEDサブコマンドのSAS(登録商標)MIXEDプロシージャ)を用いて分析した。このモデルには次の固定効果が含まれる:治療相互作用による臨床試験での分類上の週、施設、神経遮断薬の使用または不使用、およびベースラインUHDRS−TMSスコア。患者内での繰り返し観測のための非構造化共分散行列が使用された。モデルが収束しない場合には、デフォルトの拘束ML(REML)の代わりに最尤(ML)推定法が使用される。しれでもモデルが収束しない場合は、次の順序に従って、より少ないパラメータを有するより単純な共分散構造が使用される:異種自己回帰(1)[ARH(1)]、異種複合対称(CSH)、自己回帰(1)[AR(1)]、および複合対称(CS)。26週目の来診でのUHDRS−TMSのベースラインからの変化の推定平均が、実薬治療群とプラセボ群の間で比較される。
UHDRS−TMSで観察された効果が、舞踏病UHDRS−TMSのサブスコア、ジストニアUHDRS−TMSサブスコア、または不随意運動(舞踏病+ジストニア)UHDRS−TMSのサブスコアによって推進されるかどうかを評価するための感度解析は、次記のように実施された:
次の3つの変数が計算された:(1)26週目と52週目での、舞踏病項目を除くUHDRS−TMS項目の合計におけるベースラインからの変化、(2)26週目と52週目での、舞踏病およびジストニア項目を除くUHDRS−TMS項目の合計におけるベースラインからの変化、および(3)26週目と52週目での、舞踏病およびジストニア項目を除くUHDRS−TMS項目の合計におけるベースラインからの変化。これらの変数は、UHDRS−TMSベースラインの代わりに、ベースラインでの変数の評価がこのモデルに含められることを除き、主要有効性エンドポイントと同じ方法で解析される。
濃度はまたプリドピジン集団のPKモデルに組み込まれ、試験患者についての個々の曝露(CmaxおよびAUC)が計算される。
多くの臨床医および診断医が、HDの進行を追跡するために、TFCスコアに基づくShoulsonとFahnの評価尺度を採用している。この評価尺度は、全TFCスコアを5つの疾患段階に分類し、より低い段階がより損なわれない機能動作を示す。下表4は、診断由来の年平均TFCスコアおよび各疾患段階に対する典型的なケアレベルのための広範なガイドラインを提供する。(Johnson 2014)。
この実施例の結果を図1〜18に示す。
評価者偏りに依存しないエンドポイントは、Q運動評価などのプラセボ効果を受ける傾向が少なかった。検出されたシグナルは、プリドピジンの生物学的作用を示唆する。全機能的能力(TFC)は、26週の治療後、プリドピジンを支持する傾向を示した。高用量にもかかわらず、大きな安全性調査結果に関する報告はなかった。
6ヶ月のプラセボ群で、予測した約0.5ポイントの低下が認められた。前に採取したデータは、ハンチントン病患者では、TFCの約1ポイント/年の低下を示している。TFCは、12〜20週目でプラセボからの乖離を示し始め、26週目では、この乖離の傾向が強くなる。TFCデータは、プリドピジンが機能低下の進行を遅らせるという知見を裏付ける。
次の項目は、この実施例におけるプラセボ効果の説明となり得る:評価者偏り、ハンチントン病における希望の欠如、ならびに、効果的治療に対する高い期待および患者からよくなるという願望、プリドピジン治療に伴う全体的に良好なデータが高い期待をもたらす、患者は積極療法を受ける80%の機会を有する、多数の丸薬は期待を持たせ得る、試験中のプロトコルの変更、および来診当たりの評価数。
図、特に図6〜7、8(i、j、k、l)、9(i、j、k、m)および15〜18は、プリドピジン治療を受けた患者は、プラセボを受けた患者に比べて、改善されたジストニアスコアを得た。例えば、図15〜18は、特に、ベースラインである程度のジストニアの患者(GE4)では、45および67.5mgプリドピジンのbidの投与量で数値的な改善を示し、抗ジストニア効果を示す。
TMSおよび運動エンドポイント:
運動効果は、ハンチントン病段階1亜集団で統計的に有意であった。例えば、全TMS、不随意運動(ジストニア、舞踏病)、歩行運動(TMS 歩行およびバランス、タイムアップアンドゴー、ウォーク12)における統計的に有意な変化がHD段階1患者サブグループで認められた。

論理的根拠
ほとんどの形態の病態生理学の原発性および二次性ジストニアにおける線条体関与および異常なシナプス結合性に関する証拠が存在する。DYT−1ジストニアの患者では、陽電子放出断層撮影(PET)および拡散テンソル画像(DTI)MRI調査により、大脳基底核、視床、皮質、および/または小脳領域(例えば、代謝)または回路の異常性が示唆されている。
プリドピジンの定期的な経口投与(例えば、1日1回または2回)は、ジストニアに罹患したヒト患者の治療に効果的である。プリドピジンの定期的経口投与(例えば1日1回または1日2回)は、ジストニアを罹患している対象を治療するのに効果的である。プリドピジンの投与は、罹患患者のジストニアを低減するのに効果的である。
目的
DYT1およびその他の原発性遺伝性型のジストニアを有する患者集団において、プリドピジン対プラセボを受けた患者のベースラインと比較して、26週の治療後の(Burke−Fahn−Marsden(BFMD)評価尺度またはジストニア統一評価尺度(UDRS)を用いて)ジストニアの重症度の変化を評価するランダム化二重盲検プラセボ対照試験を実施すること。

- DYT1ジストニアは、姿勢型ジストニアまたは動作型ジストニアである、請求項6に記載の方法。

- 前記ジストニアは、音楽家のジストニアである、請求項1〜9のいずれか1項に記載の方法。
- 前記ジストニアは、病理学に起因するものではない、請求項1〜13のいずれか1項に記載の方法。
- 前記病理学は、脳卒中、外傷性脳損傷、外傷、脳腫瘍、神経学的組織損傷、または神経学的組織変性である、請求項14に記載の方法。
- 前記対象は、遺伝子検査により、DYT1またはその他の原発性遺伝性型のジストニアに罹患したことが確証されている、請求項1〜15のいずれか1項に記載の方法。
- 前記対象は、6より大きいBurke−Fahn−Marsdenジストニア評価尺度(BFMDRS)スコアを有する、請求項1〜16のいずれか1項に記載の方法。
- 前記対象は、染色体9q34上に位置するTOR1A(トルシンA)遺伝子のコード領域内に3塩基対インフレーム欠失を有する、請求項1〜7のいずれか1項に記載の方法。
- 前記対象は、認知機能障害を罹患していない、請求項1〜18のいずれか1項に記載の方法。
- 前記対象は、認知機能障害を罹患している、請求項1〜18のいずれか1項に記載の方法。
- 前記対象は、ハンチントン病に罹患していない、請求項1〜20のいずれか1項に記載の方法。
- プリドピジンの量は、ジストニア症状の臨床的に有意な改善をもたらすのに効果的である、請求項1〜21のいずれか1項に記載の方法。
- ジストニア症状の前記臨床的に有意な改善は、診療または臨床研究で使用される評価尺度、好ましくは、前記UHDRSスケールのジストニア項目または前記Burke−Fahn−Marsdenジストニア評価尺度で測定して、プリドピジンで治療されていないヒト患者に比べて、プリドピジンを投与された前記対象のベースラインからの少なくとも20%の変化である、請求項22に記載の方法。
- 前記プリドピジンの量は、前記対象のジストニアの1つまたは複数の症状のレベルを下げるまたは維持するのに効果的である、請求項1〜21のいずれか1項に記載の方法。
- 前記症状は、Burke−Fahn−Marsdenジストニア評価尺度またはジストニア統一評価尺度により測定される、請求項24に記載の方法。
- 前記症状は、臨床全般印象(CGI)スケール、患者総合評価スコア、疼痛の視覚的アナログスコア、全体的反応の患者評価、Burke−Fahn−Marsdenスケール(BFMDS)、または健康関連生活の質スコア(EQ−5D、SF−36)により評価される、請求項24に記載の方法。
- 前記1つまたは複数の症状は、不随意四肢運動または筋肉収縮;四肢または体幹の捻転姿勢;四肢または体幹の異常固定姿勢;内反尖足;脚の内旋;腕の内旋;手、頭部、体幹または腕の振戦;足の引きずり;斜頸;書痙;および体幹および/または四肢のジストニアからなる群より選択される、請求項24〜26のいずれか1項に記載の方法。
- 1日当たり22.5mg、45mg、67.5mg、90mg、100mg、112.5mg、125mg、135mg、150mg、180mg、200mg、250mg、または315mgのプリドピジンが前記患者に投与される、請求項1〜27のいずれか1項に記載の方法。
- 前記プリドピジンの量は、22.5mg、45mg、67.5mg、90mg、100mg、112.5mg、125mg、135mg、150mg、180mg、200mg、250mg、または315mgのプリドピジンの単位用量により投与される、請求項28に記載の方法。
- 前記単位用量が1日1回投与される、請求項1〜29のいずれか1項に記載の方法。
- 前記単位用量が、1日2回以上投与される、請求項1〜29のいずれか1項に記載の方法。
- 前記単位用量が1日2回投与される、請求項1〜29のいずれか1項に記載の方法。
- 前記定期的投与が経口投与である、請求項1〜32のいずれか1項に記載の方法。
- 前記対象がヒト対象である、請求項1〜33のいずれか1項に記載の方法。
- 前記プリドピジンが、プリドピジン塩酸塩の形態である、請求項1〜34のいずれか1項に記載の方法。
- パッケージであって、
a)一定量のプリドピジンを含む医薬組成物;および
b)ジストニアに罹患した対象を治療するための、前記医薬組成物の使用説明書、を含むパッケージ。 - ジストニアに罹患した対象に投与するための、または対象への投与に使用するための治療用パッケージであって、
a)1つまたは複数の単位用量であって、一定量のプリドピジンを含み、前記単位用量中の前記プリドピジンの量が、前記対象への投与時に、前記対象を治療するのに効果的である1つまたは複数の単位用量、および
b)完成品の医薬容器であって、前記単位用量または複数の単位用量を含み、さらに、前記対象の治療に際し前記パッケージの使用を指示するラベルを含むまたは具備する容器、を含む、治療用パッケージ。 - ジストニアに罹患した対象の治療に使用するためのプリドピジン。
