本発明がより容易に理解されることができるように、特定の技術用語および科学用語を以下に具体的に定義する。本文書中の他の個所で具体的に定義されてない限り、本明細書で用いるすべての他の技術用語および科学用語は、本発明が属する技術分野での当業者によって一般に理解される意味を有する。
一部の態様において、本発明は単離されたAAVを提供する。AAAVに関して本明細書で用いる場合、「単離された」という用語は、その自然環境(例えば、宿主細胞、組織または対象)から単離されているAAVを指す。単離されるAAVは組換え方法を用いて生成されてもよい。そのようなAAVは、本明細書では「組換え型AAV」と称する。組換え型AAV(rAAV)は、rAAVの導入遺伝子が1つまたは複数の所定の組織に特異的に送達されるように、組織特異的ターゲティング能力を有することが好ましい。AAVカプシドは、これらの組織特異的ターゲティング能力を判定するうえで重要なエレメントである。このように、標的にされる組織に適しているカプシドを有するrAAVが選択されることが可能である。一部の実施形態において、rAAVは、配列番号97に記載のアミノ酸配列を有するAAV9カプシドをコードする配列(例えば配列番号96)またはそれとの実質的な相同性を有するタンパク質を含む。
本明細書に記載の「組換え型AAV(rAAV)ベクター」とは、一般的に、最低でも、導入遺伝子(例えばGlut1をコードする)とその調節配列、および5’と3’AAV末端逆位配列(ITR)から構成される。カプシドタンパク質にパッケージングされて、選択された標的細胞に送達されるのは、この組換え型AAVベクターである。一部の実施形態において、導入遺伝子は、ポリペプチド、タンパク質、機能性RNA分子(例えば、miRNA、miRNAインヒビター)または他の目的の遺伝子産物(例えばGlut1)をコードするベクター配列と非相同的な核酸配列である。核酸コード配列は、標的組織の細胞において導入遺伝子転写、翻訳および/または発現を可能にする様式で調節成分に作用可能に結合される。

rAAVベクターの導入遺伝子配列の組成は、結果として得られるベクターの用途に依存する。例えば、1種類の導入遺伝子配列は、発現すると検出可能なシグナルを生成するレポーター配列を含む。別の例において、導入遺伝子は治療用Glut1タンパク質または治療用機能性RNAをコードする。別の例において、導入遺伝子は、研究目的、例えば導入遺伝子を内部に持つ体細胞遺伝子導入動物モデルを作製する、例えば導入遺伝子産物の機能を調査する目的での使用が意図されるタンパク質または機能性RNAをコードする。別の例において、導入遺伝子は、疾患の動物モデルを作製するための使用が意図されるタンパク質または機能性RNAをコードする。適切な導入遺伝子コード配列は、当業者にとって明らかである。
rAAVは、所望の組織の細胞をトランスフェクトし、過度の有害作用を起こすことなく十分なレベルの遺伝子トランスファーと発現をもたらすように十分な量で投与される。従来の投与経路および薬学的に許容される投与経路としては、選択された組織への直接送達(例えば脳内投与、くも膜下腔内投与)、静脈内、経口、吸入(鼻腔内送達および気管内送達を含む)、眼内、静脈内、筋肉内、皮下、皮内、腫瘍内および他の非経口投与経路が挙げられるがこれらに限定されない。投与経路は、必要に応じて組み合わせてもよい。
rAAVは、当技術分野で既知の任意の適切な方法によって、組成物中で対象に送達されることができる。rAVVは、好ましくは、生理学的に適合する担体(例えば、組成物中で)に懸濁され、対象、例えば、ヒト、マウス、ラット、ネコ、イヌ、ヒツジ、ウサギ、ウマ、ウシ、ヤギ、ブタ、モルモット、ハムスター、ニワトリ、シチメンチョウまたは非ヒト霊長類(例えばマカク)に投与されてもよい。特定の実施形態において、組成物はrAAVを単独で、または、1種または複数種のウイルス(例えば、1種または複数種の異なる導入遺伝子をコードする第2のrAAV)との組み合わせで含むことができる。
「薬学的に許容される」という語句は、宿主に投与されると、アレルギー有害反応または類似の有害反応をもたらさない分子実体と組成物を指す。
本発明はrAAVビリオンを含む安定な医薬組成物を提供する。本組成物は、凍結/解凍サイクルにかけても、ガラスを含む種々の材料製の容器に保管されても、安定性および活性を維持し続ける。
また、本発明は、特定のコンストラクトと、本明細書に記載のGlut1タンパク質をコードする核酸とを含む。配列番号28〜75と80〜97を含む表1に収載の選択された配列を含み、ならびに特定の態様において配列番号2〜5、7〜9、11〜14、16〜18、20〜23、25〜27および80〜95のうちの1または複数を含む特定のコンストラクトおよび配列は、本発明の実施形態において有用でありうる。予想外に、本明細書に記載するように、例えば2Aペプチドをコードする核酸配列が所望のレベルのGlut1を発現しないことが判明した。このように、好ましいrAAVコンストラクトは、核酸配列番号6、15および/または24が欠如し、これらすべては2Aコード配列に対応する。
本発明の組成物の医薬組成物または無菌組成物を調製するために、AAV9ベクターまたは関連する組成物は薬学的に許容される担体または賦形剤と混合されることができる。例えば、Remington’s Pharmaceutical Sciences and U.S.Pharmacopeia:National Formulary, Mack Publishing Company,Easton,PA(1984)を参照されたい。
非侵襲性経路による投与(例えば経口で;例えば丸薬、カプセル剤もしくは錠剤で)も、本発明の範囲内である。
本発明は、キット形態で本発明の成分の組み合わせを含むキットも提供する。本発明のキットは、本明細書に記述のrAAV9−Glut1ベースの化合物を含むがこれに限定されない1種または複数種の成分と、関連して、本明細書に記述の薬学的に許容される担体および/または化学療法薬を含むがこれに限定されない、1種または複数種の追加の成分とを含む。rAAV9−Glut1ベースの化合物もしくは組成物および/または治療薬は純粋な組成物として、または医薬組成物中で薬学的に許容される担体との組み合わせで製剤化されことができる。

Glut1:促進性グルコーストランスポーターメンバー1(SLC2A1)の溶質キャリアファミリー2としても知られているグルコーストランスポーター1は、ヒトにおいてSLC2A1遺伝子によってコードされるユニポータータンパク質である。Glut1は哺乳動物細胞の細胞膜を横切ってグルコースの輸送を促進する。Glut1は特性を明らかにされた最初のグルコーストランスポーターであった。Glut1は、ヒトGlut1タンパク質(hGlut1)(受入番号:NP_006507.2;配列番号79)およびマウスGlut1タンパク質(mGlut1)(受入番号:NP_035530.2;配列番号78)で高度に保護されており、98%の相同性を共有する。Glut1は他のGlutと40%の相同性を示す。

図1A〜Bに示すように、緑色蛍光タンパク質(GFP)レポーターをコードするヌクレオチドカセットに結合したマウスまたはヒトSLC2A1遺伝子のいずれかを担持する4つのDNAコンストラクトを生成した。これらの4つのコンストラクトは、Glut1とGFPのオープンリーディングフレームの間に組み込まれた口蹄疫ウイルス(FMDV)由来の16アミノ酸長の2Aペプチドをコードする核酸配列も含む。2Aペプチドは、Glut1−GFP融合タンパク質がGlut1タンパク質の構造または活性を変えうる可能性を回避するためにコンストラクト内に含まれる。2Aペプチドは、続いて起こるプロセシング現象においてFMDVポリタンパク質の一次シス「切断」を媒介し、最終的に成熟FMDVタンパク質を生成する(Donnelly,M.L.ら,(2001))。この戦略ではGlut1タンパク質がその天然状態で生成されるコンストラクトを作製することが予想された。しかし、以下に記述するように、これらのコンストラクトのどれも満足のゆくレベルでGlut1を発現しなかった。
2つの細胞株を用いて、細胞培養中で種々の組換え型プラスミドを評価した。1つの細胞株は チャイニーズハムスター卵巣(CHO)細胞株であり、他の細胞系はGlut1患者由来の線維芽細胞株である(Yangら,2011)。細胞培養実験およびそれに続くウエスタンブロットは、2Aペプチドを含有するプラスミドコンストラクトが満足のいく濃度のGlut1またはGFPを発現しなかったことを示した。予想外ではあるが、SLC2A1/slc2A1遺伝子と関連して、2Aペプチドはタンパク質の発現に悪影響を及ぼす可能性がある。対照的に、マウスまたはヒトSLC2A1遺伝子だけを含有する対照コンストラクトは確固たる濃度のタンパク質を発現することが判明した。Glut1コンストラクトを含有する2A(図1A〜B)のいずれも変異体モデル系においてGlut1発現を回復させることについてさらに追跡しなかった。
hGlut1−eGFP融合タンパク質のコンストラクト(phGlut1::eGFP)をAAV9プラスミドに再クローン化し、続いてウイルスベクターへのパッケージングを行った。再クローン化コンストラクトが継続したタンパク質を発現するかを確認するために、それをチャイニーズハムスター卵巣(CHO)細胞に一時的にトランスフェクトし、タンパク質濃度をウエスタンブロット解析によって調べた(図2A)。CHO細胞でのタンパク質発現の分析は、cDNAをコードするhGlut1だけまたは異なるプロモーター配列によって駆動されるhGlut−eGFO融合をコードするhGlut1だけを発現するコンストラクトと比較して、AAV9−hGlut1−eGFPプラスミドが低濃度のGlut1を発現した(図2A)ことを示した。しかし、改変(AAV9−hGlut1−eGFP)融合コンストラクトは効果的かつ満足のいく量でGlut1タンパク質を発現し続けた。さらに融合タンパク質は、Glut1 cDNAの3’末端でのGFPタグのため予想通りに、天然gGlut1タンパク質よりもかなり大きいように見えた(図2A)。これらの結果は、hGlut−eGFPタンパク質がCHO細胞へのグルコースの取り込みを増加させることが判明したグルコース取り込みアッセイと一致する(図2B)。図2Cは、CHO細胞へのトランスフェクション後のこのコンストラクトのGFP蛍光を示す。並行して、ヒトSLC2A1遺伝子とマウスslc2a1遺伝子もAAV9プラスミドにクローニングし、患者の線維芽細胞へのトランスフェクション時に、Glut1発現を引き起こし、グルコース取り込みを増加させることが判明した。したがって、各々をAAV9カプシドにパッケージングし、約1013ゲノムコピーをGlut1 DSモデルマウスへの投与のために調製した(米国特許第8,734,809号に記載の方法、およびGriegerとSamulski 2005およびGriegerとSamulski 2012に従った)。
AAV9ベクターにパッケージングされたコンストラクトを発現するGlut1の投与の条件を最適化するために、野生型マウスにおけるAV9−GFP(Foust,K.D.ら,2009)ベクターの分布を分析した(Gao,G.P.,とSena−Esteves,M.(2012),In Molecular Cloning,Vol 2:A Laboratory Manual(M.R.GreenとJ.Sambrook編)に記載の方法に従った)。

複数の細胞型を感染させる組換え型アデノ随伴ウイルス9(rAAV9)の能力を機能として用いて、ヒト疾患のマウスモデルにマウスSlc2a1遺伝子を再導入した。一野生型Slc2a1対立遺伝子が存在しない場合、Glut1 DSマウスはロータロッドアッセイにおいてパフォーマンスが不十分であり、評価項目はヒト患者で観察される運動行動障害、すなわち運動表現型をモデル化すると思われた。生後3日目の変異マウスに、コンストラクトpAAV9 CB6 PI mGlut1(配列番号35)によってもたらされた約4×1011ゲノムコピー、またはビヒクル(PBS)のいずれかを注射した。一元配置分散分析を用いてP値を算出した。生後3日目に変異マウスにsLC2A1遺伝子を回復させることによって、早くも6週齢で、ロータロッド(標準条件下で行った。Wangら,2006を参照されたい)でのパフォーマンスに著しい改善が見られた(図3A)。パフォーマンスの改善は、実験を終了した20週齢まで持続した。
AAV9−Glut1処置動物のパフォーマンス改善の分子基盤を調査するために、動物の脳組織と肝組織内でのマウスSlc2a1遺伝子の発現を評価した。pAAV9 CB6 PI mGlut1(配列番号35)で処置した変異動物では脳組織と肝組織内で高レベルのSlc2a1遺伝子がひしめいていた(図4A〜B)。処置マウスおよび対照マウスから脳組織と肝組織を抽出した。RNAを調製し、次いで逆転写し、Q−PCRアッセイでSlc2al転写物の増幅を行った。β−アクチンを用いてSlc2a1遺伝子発現を正規化した。図4Aは、処置変異マウスおよび関連する対照マウスにおける相対的なSle2a1遺伝子発現を示す。図4Bは野生型Glut1+/−マウスにおける発現のパーセントとしてSlc2a1発現を示す。イントロン1にわたるプライマーを用いて、処置変異マウス(Glut1+/−)と関連対照マウス(Glut1+/+およびPBS処置Glut1+/−変異マウス)において定量的PCRによって、転写物をコードするGlut1を増幅させた。マウスを安楽死させて、PBSを用いる経心腔的潅流に続いて組織を抽出した。メーカーの説明書に従って、Qiagen RNAeasyキットを用いてRNAを調製した(Qiagen、Valencia,CA)。標準手順に従ってRNAを逆転写し、以下のプライマーを用いて逆転写物をコードするGlut1を増幅した:Glut1QPCR F1:5’CTT GCT TGT AGA GTG ACG ATC 3’(配列番号76)およびGlut1QPCR R1:5’CAG TGA TCC GAG CAC TGC TC 3’(配列番号77)。予想された212bpバンドをEppendorf Realplex Cycler(Eppendorf,Germany)内で定量化した。
Glut1 DSを決定付ける特徴は髄液糖減少症(脳脊髄液グルコースの低下)である。Glut1 DSモデルマウスはこの表現型を示す。モデルマウスへのGlut1の回復により髄液糖減少症が改善したかまたは緩和したかを判定するために、動物から血液および脳脊髄液(CSF)を抽出してグルコース濃度を測定した。すべてのマウスを一晩断食させて、測定を行った。基本的には前述(Wangら,2006;Flemingら,1983)のようにCSFを大槽から分離した。手短に言うと、頭蓋上部から背側胸郭まで切開を施し、頭蓋底部から第1頸椎までの筋組織を除去して大槽を覆う髄膜を露出させた。半透明の髄膜に穴をあけないように注意して大槽の上の組織を切除した。周辺部位の残留血液/間質液を洗浄してから、30G針に取り付けたマイクロピペットを用いて、大槽を覆うクモ膜を穿刺して、CSFを5〜15μL抽出した。この全手順は5分内に完了し、CSFグルコースをAscensia Elite XLグルコースメーター(Bayer Corp.)で測定した。血糖値を同じように判定してから、尾を切開して約10μLの血液を抽出することによってCSFの抽出を行った。各マウスの血糖濃度と脳脊髄液糖濃度のそれぞれの測定値を評価した。平均値を報告する。
Glut1 DSマウスにおいて疾患経過の初期にGlut1発現をモデルマウスに回復させる(生後3日でのAAV9−mGlut1コンストラクトの注射を例とした)ことは、明白に治療価値がある。症状があるマウスにおいてGlut1回復の時間枠を判定するために、8週齢の変異マウスにpAAV CB6 PI mGlut1コンストラクト(配列番号35)を注射することを含む実験を開始した。Glut1 DSマウスはこの時点で明らかに症状があり、ロータロッドで同腹仔の野生型対照よりは動きがよくなかった。したがって、Glut1 DSのコホートにビヒクルまたはAAV9−mGlut1の1×1012ゲノムコピーのいずれかを全身的に注射した。すべてのマウスはこの手法に耐え、成体齧歯動物においてウイルス注射が安全であることを示した。上述のものに類似する分子、細胞および行動のアッセイを評価して、Glut1欠損症の症状を処置/軽減することを可能にする時間枠ならびに疾患表現型の経過を逆転させるための何らかの期限を決定する。
新生(生後3日目)Glut1 DSマウスにおいてAAV9媒介のGlut1(マウス)タンパク質の充足によりGlut1発現が上昇し、この疾患で特徴的に認められる髄液糖減少症を緩和し、脳の大きさを回復させ、および運動機能を著しく改善することを本データは示している。対照的に、8週齢でのGlut1の充足は疾患表現型をレスキューできなかった。Glut1 DSモデルマウスにおいて治療好機は限定されており、ヒトの病態にも反映する可能性があることが判明したことをこれらの結果は示唆している。早くも2週齢で変異マウスにおいてCSFグルコース濃度が有意に低下していることを予備段階データも示している(変異群:23.25±3.77mg/dL、対照群:53.33±5.20mg/dL、P<0.01、t検定)。依然として、明白な表現型が識別できる前に、この時点でGlut1を回復させることで治療効果がもたらされるかどうかは不明瞭である。
成体マウス(8週齢)へのGlut1回復はGlut1 DS表現型を緩和しなかったことは明白であるが、これらのマウスを高脂肪食で前処置することで、良好な結果がもたらされうる可能性がある。そのような食事療法でのマウスは年長のGlut1 DS患者のコホートをより正確に表す。該患者は、理想的な治療好機を逃したかもしれないが、それでもケトン誘発食は保護効果が早いため、Glut1タンパク質の遅い回復からの恩恵を受けうる。そのような食事療法は、ケトン体、すなわちモノカルボン酸トランスポーターを介して血液脳関門を横切るエネルギー源の不完全ではあるが代替物を脳に供給する。したがって、2週齢のマウスでの実験に加えて、本発明者らは、7Cトリグリセリドトリヘプタノインを投与された(生後7日目に開始)成体(6〜8週齢)変異マウスへのGlut1回復の効果を試験する。現在、Glut1 DSに関する臨床試験中であるトリヘプタノインは、TCA回路のためにアセチル補酵素Aに代謝されるだけでなく、栄養素は最終的に細胞のエネルギー必要量を供給するために分解されるので、同回路に不可欠なアナプレロティック基質を提供するとも考えられている。手短に言えば、この実験は、変異マウスにAAV9−Glut1(pAAV9 CB6 PI mGlut1)コンストラクトが投与されるまで、トリヘプタノイン(82mg/g)を用いて、またはトリヘプタノインを用いずに同マウスを処置することを含む。次いで、現在、利用可能な(ケント誘発食)処置を受けている年長患者においてGlut1発現の回復の治療結果を予測する方法として異なるコホートのマウスを上述のように、評価する。
Glut1 DSマウスにおいてslc2a1発現の有害作用は現在まで観察されてないが、コンストラクトpAAV9 CB6 PI mGlut1ベクター(配列番号35)を投与された野生型の小サンプルについての予備検査はCSF濃度および血糖値の低下を示した。この予想外の結果は、AAV9の好ましい標的器官の肝臓でのslc2a1発現の上昇、したがって、この組織へのグルコーストランスポートの改善に起因する可能性がある。最終結果は、低血糖状態に反映される循環グルコースの低下である。そのような結果を予想して、AAV9が特に高い親和性を有する器官の肝臓において高濃度でそれが発現することを妨げるようにhGlut1コンストラクト(pAAV CB6 PI hGlut1)を改変する(Zincarelliら,2008;Pacakら,2006)。そのためには、miRNA−miR−122(特に肝細胞で発現する)の結合部位(BS)をコンストラクトに導入する。この戦略は以前には順調に実行されており(Xieら,2011を参照されたい)、標的mRNAのmi−RNA媒介エンドヌクレアーゼ的切断を活用し、このように転写物の発現を対象組織に制限する。図1Dおよび表1に示すそのようなコンストラクト(pAAV CB6 PI hGlut1−in3xmiR−122 BS、pAAV CB6 PI hGlut1−out3xmiR−122 BS、pAAV CB6 PI mGlut1−in3xmiR−122,pAAV CB6 PI mGlut1−out3xmiR−122)を最初(無改変)のGlut1発現ベクターと平行してマウスのサブセットで試験し、Glut1発現レベルと治療効果を各々調べる。低血糖を引き起こす原型コンストラクトの高い傾向は、その後の実験および試験において新規のGlut1−miRNA−BSを用いる利点を示す。
大型哺乳動物モデルにおいて本発明者らが選択したコンストラクトの生体内分布、発現および毒性を判定するために、ウイルスベクターをカニクイザルのコホートに投与する。手短に言うと、6匹の動物に各生後1日目、生後90日目および2歳で、AAV9ベクターを5×1013ゲノムコピー/kgの用量で全身的に投与する。コンストラクト(複数可)の急性毒性を判定するために、ベクター投与から1日目、3日目および7日目に動物から採血する。さらに、ベクター注射から2週後、3週後および4週後に採血する。すべての場合において、重要な臨床化学パラメータおよび血液学パラメータを評価する。血清試料中のAAV9への中和抗体の力価を形質導入ベースの定量的中和抗体アッセイ(Raptiら,2012)によってモニターし、PBMC中の導入遺伝子またはカプシドに特異的なT細胞の存在を、ELISPOT、細胞内サイトカイン染色法およびフローサイトメトリーを用いて、判定する(Walkerら,2001)。上記の免疫試験を補足するために、4週目に各コホートから動物3匹を安楽死させ、すべての主要な器官系において病理組織学、ベクター生体内分布調査および導入遺伝子発現分析を行う。これらの実験により、ウイルス投与後早い時期を遅い時期と対比して、血清、白血球およびPBMCにおけるカプシドまたは導入遺伝子に対するB細胞とT細胞の免疫応答の試験と比較もおこなうことが可能になる。長期安定性試験を行うために、各群の残り3匹の動物については、3ヵ月間にわたり経過観察を行い、上記のように臨床化学試験および血液学試験のために毎月採血する。急性毒性調査において、最終的にこれらの動物は注射から3ヵ月後に安楽死させ、上述のように分析を行う。ヒトGlut1タンパク質はカニクイザルと約99%の相同性を共有するにもかかわらず、積極的な免疫応答を誘発することが可能である。これを防止するには、miR−142−3pとmiR−155の2つのmiRNA結合部位を試験コンストラクトに導入する。独立した調査からの予備結果は、これらのmiRNAが抗原提示細胞中で発現し、その結果その転写物がmiRNAのための結合部位を含むタンパク質の発現を抑制する。まとめると、これらの調査により、ヒト臨床試験において本明細書に記載の試験Glut1コンストラクトの最終的使用が容易になる。
