(RBC)、WBCおよび血小板などの造血細胞の負枯渇を用いて血液試料中のCTCなどの無標識の非造血細胞を露顕させるいわゆる「CTCチップ」)である。
CTCであることができる。このように、本発明の方法は500個のWBC中で約1個のCTCの純度のレベルを達成することができる一方で、本発明の方法は、いくつかの既知のCTC分析方法で必要とされるような高度に精製されたCTCを必要としない。

単離技法は、例えば国際PCT出願第2015/058206号パンフレットおよびOzkumurら、「Inertial Focusing for Tumor Antigen−Dependent and −Independent Sorting of Rare Circulating Tumor Cells」、Sci.Transl.Med.、5:179ra47(2013)の中で記載されているいわゆる「CTC−iチップ」などの超高処理能マイクロ流体学を用いて血液試料からCTCを濃縮するために用いられる。CTC−iチップは、血液試料中のWBCを磁気ビーズで標識してその後に試料を2つの濃縮段階を通じて処理するというCTC抗原非依存的な手法を用いる。第1段階では、決定論的横置換法を用いて小さく柔軟な細胞(粒子)(RBC、血小板、未結合磁気ビーズおよび血漿)を除去し、その一方でより大きい細胞(CTCおよびWBC)が保持される。第2段階では、慣性集束を用いて全ての細胞を狭い流体流の中で移動させ、その後、磁場を用いてビーズ標識されたWBCを集束流から引き出し、高度に濃縮されたCTCが残る。10mlの全血からのCTC−iチップ産物は、通常は<500,000個のRBC、<5,000個のWBCおよび変動する数のCTCを含有する。
1.DNA突然変異は腫瘍特異的ではなく、また、健常個体がいくつかの未確認の癌細胞を血中に有するという発見は、臨床上非常に困難な状況である。これとは対照的に、腫瘍特異的なRNA(例えば、肺に対して前立腺)を選択することによって、新規方法は血中の癌細胞の根源を特定することができる。
新規検査方法は、液滴デジタルPCR機器からのデータの解析に至るまで、またはそれを含めて、単離システム、例えばマイクロ流体負枯渇システムを用いて部分的に純粋なCTCを単離することから始まる。主に8つの検査ステップがあり、そのいくつかは、任意で選択されるが概してより良好な結果を与えるものである。
2.希少細胞含有産物の体積を減らすこと(任意);
3.例えば細胞溶解によって、産物からリボ核酸(RNA)分子を単離し、単離RNAから;例えば産物中に含まれている細胞から放出したRNAのRT−PCRによって、cDNAを生成させること;
4.RT−PCRステップ中に合成されたcDNAの浄化(任意);
5.遺伝子特異的な標的化前増幅プローブを使用して、例えばFluidigm BioMark(商標)入れ子式PCR手法、または非特異的完全トランスクリプトーム増幅を用いて、例えばClontech SMARTer(商標)を使用して、cDNAを前増幅すること(任意);
6.cDNA分子を個々の液滴中に、例えばPCR試薬と共に、カプセル化すること;
7.CTC由来cDNAには特異的に結合するが他の細胞由来cDNAには結合しない構成のレポーター基の存在下、各液滴内で、例えばPCRを用いて、cDNA分子を増幅させること;
8.レポーター基を含有する液滴(例えば「陽性」液滴)を液滴中のCTC由来cDNA分子の存在の指標として検出すること;ならびに
9.検出された液滴中のCTCを分析して、例えば、患者または被験体における特定疾患の存在を判定すること。
1.CTCの単離
患者の血液をCTC−iチップ、例えば、バージョン1.3Mまたは1.4.5Tの中に流し、試料を氷上の15mL円錐形チューブ内に回収した。CTC−iチップは、以前に記載されているとおりに設計および作製された(Ozkumurら、「Inertial Focusing for Tumor Antigen−Dependent and −Independent Sorting of Rare Circulating Tumor Cells」、Science Translational Medicine、5(179):179ra47(DOI:10.1126/scitranslmed.3005616)(2013))。
および白血球および血小板)および未結合ビーズをCTCから分離する。CTCを溶液中に回収する一方で、赤血球、血小板、未結合ビーズおよびタグ付き白血球を廃液チャンバ内に回収する。プロセスは自動化されており、10mlの血液が1時間で処理される。
産物中に単離された全ての細胞を完全に溶解させるためには、典型的な出発点である数ミリリットから最終体積の約100μlに産物体積を低減することが好ましい。これは、例えば、産物を遠心分離し、細胞溶解およびcDNAの生成に備えてプルロニック緩衝液中に再懸濁させることによって、成し遂げることができる。この時点で、RNAlater(商標)(ThermoFisher)の添加およびそれに続く液体窒素中での瞬間凍結および−80℃での保存により、試料を長期保存のために処理することができる。
RNA単離ステップは、RT−PCRに備えて細胞から全てのRNA分子を完全に放出させるためにプロセスにとって重要である。ワンステップでのチューブ内反応を用いて、標準的な移送ステップの間に招く可能性のある細胞およびRNAの損失の危険性を最小限に抑えることができる。例えば、ペレット状産物にRT−PCRマスターミックスを直接添加し、ピペット操作で完全に溶解させ、1:1のRNA:cDNA比を目標とするキットプロトコルに従って反応を実施することにより、qRT−PCRキット用のインビトロジェンSuperScript III(登録商標)第一鎖合成Supermix(登録商標)を使用することができる。一旦cDNAが合成されたならばRNアーゼHを反応に適用して、残存しているあらゆるRNAを除去する。あるいは、後のステップにおいて試料の完全トランスクリプトーム前増幅を実施したいのであれば、専売オリゴヌクレオチドおよび逆転写酵素を使用するものであるSMARTer(商標)Ultra Low Input RNAキットプロトコルを使用して、cDNAを合成することができる。
プロセスの別の有用なステップは、任意ステップではあるが、溶解試薬をcDNA含有溶液から除去することを含む。強すぎる洗剤の存在は、cDNA含有溶液がddPCR機器へ移されるとすぐに、ddPCR法で使用する液滴の不安定化を招く可能性がある。洗剤の除去は、例えば、固相可逆固定(SPRI)を用いることによって成し遂げることができる。この技法では、被覆磁気ビーズを使用して最初に特定の大きさ範囲のcDNAを結合させ、その後に洗剤含有上清を除去し、最後に純粋なcDNAをddPCR機器への投入のために溶離させる。RT−PCRの浄化に加えて、SPRIプロセスもまた、cDNAの大きさの選択を達成し、それは、プロセスのddPCR段階に入る非標的cDNA分子の数を低減し、次いでそれはバックグラウンドおよびノイズを低減する。
cDNAの前増幅は、液滴PCRステップで検出できる鋳型分子の数を増加させる、したがってシグナル対ノイズ比を向上させ陽性読み値の信頼性を高める、任意ステップである。それは、CTCにおいて低レベルで発現されるマーカーの検出のためおよび、例えば転移前の疾患の早期検出との関連において、アポトーシス性である可能性のある非常に少ない数のCTCを含有する試料を分析するための、非常に強力な手法である。これらの2つの手法は、d−CTC検査の作業流れにおいて適用されるために改変されている。特異的標的増幅(STA)は、Fluidigm BioMark(商標)入れ子式PCRプロトコルに基づいて、液滴PCRステップで使用されるプローブによって標的化される領域を増幅するように特異的に設計されたプライマーを使用することに依拠している(表2参照)。これらのプライマーは、健常対照におけるノイズの増加なしに効率的で特異的な増幅を確保するために、それらの各々の蛍光プローブと組み合わせて慎重に設計および試験された。その代わりに、完全トランスクリプトーム増幅は、SMARTer(商標)U
ltra Low Input RNAキットプロトコルに基づき、WBCにおいて見受けられる転写物とCTCにおいて見受けられる転写物との両方を含めた産物中の全ての転写物の増幅に依拠している。
一旦cDNAが合成され、混入している洗剤を除去して精製されたならば、溶液中のcDNA分子の全ての凝集物とqPCR試薬とを、例えば液滴製造機器、例えば液滴生成器、例えば1試料あたり20,000個の液滴を精製するBiorad自動液滴生成器によって、多くの小区画化された反応に分割する。各反応は、非水性液体、例えば油(PCR適合性、例えば販売者からの専売製剤)の極めて小さい液滴から構成され、それは、Taqman型PCR試薬を遺伝子特異的プライマーおよびオリゴヌクレオチドプローブ、ならびに少量の試料を含有している。液滴生成が完了した時点で、試料は、多大な数の個々のPCR対応型反応を含有しているエマルジョンから構成される。
qPCRサイクル条件を用いて、エマルジョン試料全体に対して標準的なPCRサイクルを実施する。反応は、個々の液滴PCR体積の大部分が終点へともたらされることを確かにするために、45サイクルまで行う。このことは重要であるが、反応はTaqman型qPCR試薬を使用して実施され、qPCR条件下で繰り返されるが、試料の蛍光強度はPCRサイクル中には測定されずに次のステップで測定されることになるからである。
個々に区画されたPCRの各々は、いかなる蛍光測定も実施する前に完全に終点へともたらされるため、個々の液滴はそれぞれ、十分に蛍光を発する液滴となるか、または蛍光を実質的に全く含まないことになる。これにより、陽性(蛍光を発する)および陰性(蛍光を発しない)の液滴を簡単に数え上げることが可能になる。
上流でのRT−PCRは1:1のRNA:cDNA比を目標としたため、各陽性液滴は、生じる単一のRNA転写物を表すはずである。この解釈は、標的cDNA分子の数をはるかに上回る個々の液滴の数に依拠している。新規プロセスでは、一極において本発明者らは、単一のCTCを単離および溶解させて、数個のRNA転写物を遊離させ、次いでそれを1:1でcDNAへと逆転写し、分画し、PCR増幅させ、数え上げる、ということの可能性を検討する。
生成し、それは、少数の単離CTCおよび適度に発現された標的遺伝子について液滴1個当たり2つ以上の標的cDNA分子が決して存在しないことを確かなものにする。他方、CTCの数が数ダースまたは数百に達する場合、適度に発現する遺伝子について液滴1個当たり多数の標的cDNA複製物が存在する可能性がある。そのような場合、生じる転写物のおおよその数はポアソン統計学を用いて推定することができる。
上に述べたように、周囲を取り巻く正常な血球との関連において癌細胞に対して高度に特異的である遺伝子転写物の同定は、新規方法の中心部分である。癌細胞においてより高度に発現される遺伝子は多く知られているが、これらの遺伝子の大多数もまた、血液を含めた正常な組織において通常は少なくとも限られた発現を有する。この検査に必要とされる並外れた感度を考慮すれば、正常な血球における信号の欠如は、血流中の腫瘍細胞の信頼性の高い同定にとって必須である。

Technologies)によって合成し、プローブをFAMまたはHEX、ZENおよびIABkFQで標識して、アンプリコンの中間部を標的とするプローブを作出する。ヒトCTCにおいてデジタル式のPCRに基づくmRNA転写検出を好結果に適用するために必要とされる本発明者らのPCRプライマー設計方式の独特の特徴としては、例えば下記が挙げられる:1)細胞mRNA転写物は、固定されていない脆弱な細胞、例えばCTCにおいて特に、5’末端から分解する傾向にある、ということを考慮した、各mRNA転写物の3’末端の特異的標的化;2)例えば富化CTC混合物中に過剰に混入している白血球に由来する、混入しているゲノムDNAの意図しない増幅を排除するための、イントロンにまたがるアンプリコンを生成させるプライマーの設計;および3)特異的スプライス変異形の臨床的妥当性に関するいくつかの事例における不確定性を考慮した、所与の遺伝子の多数のスプライス変異形を包括的に増幅させるプライマーの設計。
血小板および白血球の負枯渇を実施するものであり、(健常個体に存在するはずのない)CTCを含めた、白血球マーカーを発現しない血中の細胞が富化された試料産物を生成する。各血液試料について、CTC−iチップからの産物にRNA安定化溶液(RNAlater(登録商標)、Life Technologies)を補給し、標準的方法を用いるRNA抽出およびcDNA合成のための処理を行った。その後、液滴デジタルPCR(Biorad、カリフォルニア州)を用いて、試験されている特異的プライマー対に基づいて各試料中に存在する転写物の数を定量した。この段階中に液滴デジタルPCRによって評価した試料は、癌細胞株からのcDNA、CTC−iチップを通じて処理された健常提供者からの白血球cDNA(試験されているプライマー対1つ当たり少なくとも4人の健常個体)、および陰性対照としての水を含んでいた。
以下の表1は、((Genbank ID)および配列識別番号(SEQ ID NO)による)遺伝子の名前の一覧を、それらの選択対象である癌タイプ(Br:乳房、Lu:肺、Li:肝、Pr:前立腺、Panc:膵臓、Mel:メラノーマ)と共に提供する。加えて、各遺伝子について最適化されたプライマーの組(プライマー1および2)を、デジタルPCR産物を最適に可視化するための蛍光プライマープローブの構成(例えば、タグ付きプローブ用の6−FAM(商標)(青色蛍光標識)またはHEX(商標)(緑色蛍光標識)、およびZEN−31ABkFQ消光剤)と共に列挙する。
CTCに由来する最小量のRNAからの腫瘍特異的mRNAの検出を向上させるために、本発明者らは、微少量のCTCチップ産物からの多様な遺伝子転写産物を試験することのできる多重検査を確立した。これは、独立した多数の遺伝子を使用することのより高い感度/特異性を、投入される鋳型の量が限られている(したがって多数の反応へと希釈されるべきでない)という事実と組み合わせるものである。本発明者らの検査は、1反応当たり4つの遺伝子を含み、各遺伝子は、様々な比率で蛍光結合プライマーを選択すること
によって独特に二次元空間に分解される。それゆえ、鋳型を希釈する必要なく、1回の反応で4つの遺伝子転写物を独立に測定することができる。種々の癌について本発明者らは最大で4つもの異なる反応(つまり、最大20個の異なる遺伝子転写物)を行っており、入れ子式RT−PCRデジタル検査を適用すれば、実施できる反応の数に制限はない。
d−CTC検査法の応用
外科的に切除または根絶することのできる時点での上皮癌の早期検出は、治癒の絶好の機会を提供し、最小限の癌播種の開始時における補助化学療法の投与は、確立された転移性疾患の処置に比べて治癒を得るのにはるかにより効果的である。しかしながら、早期癌検出の現在の取り組みは、特異性の欠如に悩まされている。例えば、血清PSA測定を使用した男性の前立腺癌についての広範なスクリーニングは初期癌を発見するのに有効ではあるが、それは、はるかに数多くの非悪性前立腺症状(例えば、腺の良性の肥大)、さらには浸潤性になることが決してない緩慢性の癌さえも識別する。こういった事情から、広範なPSAスクリーニングは公的医療機関によって推奨されていない、というのも、過剰診断による(死を含めた)合併症は早期癌検出の計算上の利点と競合的であるかまたはそれを上回るからである。

CTCチップ−液滴検査の実行可能性を試験するために、本発明者らはまず、前立腺腫瘍細胞では特異的に発現されるが混入している白血球には存在しないいくつかの転写物を選択した。これらは、前立腺系統特異的マーカーであるKLK3(カリクレイン関連ペプチダーゼ;または前立腺特異的抗原すなわちPSMA)、FOLH1(葉酸加水分解酵素;または前立腺特異的膜抗原すなわちPSMA)およびAMACR(アルファメチルアシルCoAラセミ化酵素)ならびにEpCAM(上皮細胞接着分子)であった。PCR条件は、イントロンにまたがるプライマーとIntegrated DNA Technologies(コーラルヴィル、アイオワ州)のZEN二重消光FAM標識プローブとを使用して標準的なqPCRプロトコルに従って最適化した。系の動的範囲を調査するために、これらの条件をまず、癌細胞と白血球との混合物からカプセル化されたcDNAを使用して試験した。次に、個々の細胞を選択する手作業での単離技法を用いて、0個、3個、
6個、12個、25個および125個の前立腺癌LNCaP細胞を、HD血液の個々の5ml分取の中にスパイクし、続いてCTC−iチップ処理、RT−PCRおよび、RainDropシステムを使用した液滴カプセル化を行った。本発明者らは、KLK3をこの実験の標的転写物として選択した、というのもそれは程よく豊富に存在することが予測されるからである。5,000の強度閾値を用いて、本発明者らは、おおよそ250個の液滴でたった3個の細胞に値するKLK3転写物が容易に検出されることを見出した。
この実施例は、本明細書に記載の方法に使用できる一般的デジタルCTC検査プロトコルを提供する。本明細書中に記載されるいくつかの実施例では、この一般的プロトコルの様々な態様を用いた。例えば、以下に記載する(RNA精製からcDNA合成までに関する)プロトコルのステップ3の手法1を用いて、図15A〜15Cのデータが生成された。ステップ3の手法2を用いて図19A〜24Bのデータが生成された。

7.液滴試料をPCR機の中に入れた。PCR条件はBioradの推奨とは異なっていた。本発明者らは、全ての液滴を確実に同じ温度にするために、緩やかな傾斜ではなく段階的降下を用いた。これは、RainDanceが用いるものともBioradが用いるものとも異なっている。勾配ではなく段階的降下を用いることによって、よりよい結果(すなわち、より大きな信号および、陽性液滴と陰性液滴との間でのより大きな分離)を得ることができる。
9.データを収集し、TIBCO(登録商標)Spotfire(登録商標)解析ソフトウェアを使用して解析した。

遺伝子特異的プライマーに対して多数の患者を試験する場合、または多数の遺伝子に対して多重化するプライマーを試験する場合、ddPCR supermixとプライマーとを含むものであるマスターミックスをウェル内へ分取し、続いて各ウェルに患者cDNAを加え、十分に混合した。
表1に列挙する特異的マーカー遺伝子を選択するために下記のプロトコルを用いた。

本発明者らは、微少量のCTCチップ産物から多くの異なる遺伝子転写物を試験することのできる多重検査を確立した。

この多重方略の有効性を検証および実証するために、本発明者らは、(スパイク細胞実験を用いた)概念と患者由来の試料とを示した。図4は、健常提供者(HD)からの正常な対照血液試料をCTCチップによって処理して4つの異なる遺伝子転写物(その全てが陰性(すなわちブランク液滴)である)についてdーCTC検査に供した結果を示す。
腫瘍特異的RNAの検出を改善するために、遺伝子特異的増幅の各々について入れ子式PCR方略を最適化した。これを成し遂げるために、CTCに由来するcDNAをまず、d−CTC検査に使用する遺伝子特異的プライマーの数塩基対外側に配置される遺伝子特異的プライマーで増幅した。各遺伝子について、2〜3個のプライマー組を試験し、遺伝子特異的d−CTC検査プライマーに適合しておりかつHD血液において試験結果が陰性であるプライマー組を患者試料の分析のために選択した。

10倍濃縮の特異的標的増幅(STA)プライマー混合物の調製
1.)DNA非存在フード内で、0.5μLの200μMのプライマーの対の各々(0.5μLの順方向プライマーおよび0.5μLの逆方向プライマー)を混合した。
3.)混合物の渦流撹拌を20秒間行い、遠心沈殿を30秒間行った。
1.)96ウェルPCRプレートの各ウェルに下記の混合物を調製する。
3.)20サイクルではなく18サイクルの変性およびアニーリング/伸長ステップを用いて下記に一覧で示す熱サイクル条件を用いた(注:18サイクルは、TSA前増幅プロトコルを全トランスクリプトーム増幅と比較するために用いた)。
図14は、1ngの非増幅細胞株cDNAならびに、10、14および18サイクルの前増幅後の1μlの前増幅産物から得られた、7つのマーカー(PIP、PRAME、RND3、PKP3、FAT1、S100A2およびAGR2)の液滴PCR信号を示す。さらなるサイクルの前増幅は信号の増加をもたらす。注目すべきことに、この細胞株において非常に低レベルで発現されるマーカーであるPRAMEは、18サイクルの前増幅の後にのみ検出され、当該技法の有用性を実証している。
臨床研究からの実際の患者試料を使用して本明細書に記載の検査を検証した。これらは、転移癌(肺、乳房、前立腺およびメラノーマ)を有する患者および限局癌(前立腺)を有する患者を含む。検査は実施例2〜5に記載されているとおりに行った。
OXB13)、7(AGR2)は、転移癌患者において最も強い信号を有しており、プローブ2(FAT1)および4(STEAP2)は、限局癌患者において最も陽性であった。この結果は、血中の癌細胞における信号の異種性、および検査の中で差異のある信号を詳細に分析することの重要性を明確に例証している。(癌患者試料と等しく処理された)HD対照からの血液は、信号が全く存在していなかった。
血液試料中に存在するCTCのデジタル式(定量的)手段を提供することに加えて、本発明者らのd−CTC検査は、血中の腫瘍細胞に独特である特異的シグナル伝達経路の分析も可能にする。例えば、前立腺系統特異的遺伝子のサブセットはアンドロゲンシグナル伝達(例えばPSA)が動因となったが、別のサブセットはアンドロゲンシグナル伝達(例えばPSMA)によって抑制された。これらの遺伝子を一緒に分析することにより、CTC内でのアンドロゲンシグナル伝達の状態を確認することができる。必須経路の標的化および遮断における治療剤の有効性についての情報を同時に用いて血中のCTC信号の総数を定義することは治療モニタリングのために重要である。
実施例5と同様に、非特異的全トランスクリプトーム増幅(WTA)を用いてCTC特異的転写物の検出率を向上させることができる。この方法は、対象標的だけでなく産物中に見つかるあらゆる伝達を増幅させるランダムなプライマーの使用に依拠する。この実施例では、下記のとおりSMARTer(商標)Ultra Low RNAキットプロトコル(Clontech)を用いた。
1)1:50,000に希釈された1uLのERCC Spike−In Mix 1を各試料に添加する
2)各試料の体積を10uLに増やす

サイクルの回数はRNA投入量に応じて調節することができる(例えば、単一の細胞には18サイクル、または10ngのRNA投入量では9サイクル)。加えて、4℃の停止点は一晩である。
PCR産物をlo−bind1.5mLエッペンドルフに移し、第2組のチューブに試料IDのラベルを付け;最終的に溶離させるときまで室温でSPRIプロトコルを行う
9)AMPure(商標)XPビーズ[4度]を室温で少なくとも30分間保温する

14)試料を磁石の上に置き、5分間静置する
15)ビーズを乱さずに上清(約95uL)をピペットで吸い出す(ピペット先端が褐色になっているか確認し、相当な量のビーズの損失があればチューブに戻す)
16)200uLの80%エタノールで2回洗浄する−ビーズペレットを混合または撹乱しないこと。
エタノール洗浄の合間にビーズペレットが乾燥しないようにすること。
21)約15uLの溶離した増幅cDNAをピペットで吸い取り、ピペット先端にビーズがあるか確認する。ビーズが存在しているならば、ピペットで溶液をビーズペレットの上に戻し、溶離をもう1回試みる前に約1分間静置する。そうでなければ、新しいlo−bind1.5mLエッペンドルフ、PCRチューブまたは96ウェルPCRプレートの中に保存する。注:溶離産物中に繰り返しビーズが入るようであれば、吸入体積を14uL以下にすることが唯一の解決策となり得る。
各試料のために各患者から10〜20mLの血液を採取した。血液を到着から3時間以内に、負枯渇モードで流しているCTC−iチップで処理した。Qiagen RNeasy(商標)plusマイクロキットを使用して産物からRNAを抽出し、取得可能な17uLのうちの5uLを、ClonTechのv3 SMARTer(商標)全トランスクリプトーム増幅(WTA)方略を用いて増幅させた。その後、WTA産物の1%をデジタルPCRプレートの各ウェルの中に投入し、500nMのTaqman(商標)プライマー/プローブ合剤を使用して対象の各遺伝子について転写物濃度を決定した。転写物計数を血液体積に規格化してHCC、HDおよびCLDの患者同士で比較した。HCC患者は、生検確認済の未切除肝細胞癌腫として定義され、CLD患者は、超音波/MRIが陰性である、様々な病因(アルコール媒介性、HBV、HCV)の肝臓疾患を有する患者である。HDは、10〜20mLの血液を提供する、研究所外の健常提供者である。
おける液滴総数を示す。HCC患者はCLDおよびHDのどちらと比べてもより多い数の液滴を示し、パネルから高リスクCLD群がかなり取り除かれていることおよび、パネルを用いてそれらの患者を肝臓癌の発症に関してスクリーニングできることを示唆している。これは、現在診療所で利用可能な肝臓癌のためのスクリーニング方法cの特異性が低いことを考慮すれば重要な結果である。CLD患者についてアメリカ肝臓病協会は、6ヶ月毎の超音波(US)(詳細なアルゴリズムは、検出される肝臓病巣の大きさに依存する)を推奨している。中国において、AFP遺伝子マーカーと超音波スクリーニングとの有望な組み合わせは、スクリーニングされた母集団がたった60%の遵守率を維持した場合でさえ、スクリーニングされた人が、スクリーニングされなかった人に比べて死亡率の37%の得をする、ということを実証した。
8人の転移性肺患者および8人の健常提供者の血液試料を、先に記載したようにCTCチップに通して処理した。試料に遠心沈殿を行い、RNAlater(商標)で処理し、−80Cで保存した。RNAを精製し、記載したようにcDNAを合成した。6μlのcDNAおよび図中に列挙されるプローブに対応する入れ子型プライマーを使用して各試料に対してSTAを実施した。各液滴PCR反応につき1μlのSTA産物を投入した。
9人の転移性乳癌患者、5人の限局性乳癌患者および15人の健常提供者からの血液試料をCTCチップに通して処理した。産物をペレット化し、RNAlater(商標)で処理し、−80Cで保存した。先に記載したように各試料からRNAおよびcDNAを調
製した。図22中に列挙されるプローブに対応する入れ子型プライマー(FAT2、SCGB2A1、PGR、PRAME、TFAP2C、S100A2、FAT1、AGR2、PKP3、RND3およびPIP)を使用して各試料から6μlのcDNAをSTA増幅した。液滴数を血液体積に正規化し、各マーカーについて、最も高い健常提供者の値を患者試料の値から差し引いた。
10人の転移性乳癌患者、7人の健常提供者からの血液試料をCTCチップに通して処理した。産物をペレット化し、RNAlater(商標)で処理し、−80Cで保存した。先に記載したように各試料からRNAおよびcDNAを調製した。6μlの未増幅cDNAを各液滴PCR反応に投入した。試料は、アンドロゲン受容体のv7アイソフォーム(ARv7、配列は表1中)に対するプローブを使用して分析した。液滴数を血液体積に正規化した。
(各々多数の描点(合計描点:182)を有する)34人の転移性または切除不可能なメラノーマ患者および15人の健常提供者からの血液試料をCTCチップに通して処理した。産物をペレット化し、RNAlater(商標)で処理し、−80Cで瞬間凍結した。先に記載したように各試料からRNAおよびcDNAを調製した。図24A中のグラフの下部に沿って列挙されているプローブに対応する入れ子型プライマー(個々のマーカーはPMEL、MLANA、MAGEA6、PRAME、TFAP2CおよびSOX10))を使用して、各試料から12μlのcDNAを特異的標的増幅(10サイクル)によって増幅させた。液滴数を血液体積に正規化した。図24Bは、健常提供者と比較した場合のメラノーマ患者において検出された液滴信号のドットプロット分布図を示す。全患者の描点について検出感度は81%であった(患者の描点は、6つのマーカーのうち任意の1つが、その特定マーカーについてのHDでの最も高いバックグラウンド信号を上回る液滴信号を示す場合に、陽性に採点される)。個々のマーカーのうち、PMELおよびMLANAは最も高い検出率を示した。
本発明をその詳細な説明と併せて記載してきたが、上記の記載が例示を意図したものであり、別記の特許請求項の範囲によって定義される本発明の範囲を制限するものではない、ということは理解されるべきである。その他の態様、利点および変化形態は添付の特許請求の範囲に含まれる。
Claims (28)
- 被験体において極めて高い感度および特異性で癌を検出するための方法であって、
前記被験体からの血液試料から循環腫瘍細胞(CTC)を単離することと、
CTC由来RNAをcDNAに変換することと、
前記cDNAを個々の液滴中にカプセル化することと、
CTC由来cDNAには特異的に結合するが血中の他の細胞由来cDNAには結合しない構成のレポーター基の存在下、各液滴中において前記cDNA分子を増幅させることと、
レポーター基について陽性である液滴の総数を決定して、被験体における癌の存在を示すCTCの存在を決定することと
を含む、方法。 - 血液試料中の循環腫瘍細胞(CTC)を分析する方法であって、
前記血液試料から、血中に存在するCTCおよび他の細胞を含む産物を単離することと、
前記産物からリボ核酸(RNA)分子を単離することと、
溶液中で単離RNAからcDNA分子を生成させることと、
cDNA分子を個々の液滴中にカプセル化することと、
CTC由来cDNAには特異的に結合するが他の細胞由来cDNAには結合しない構成の1以上のレポーター基の存在下、各液滴内でcDNA分子を増幅させることと、
液滴中のCTC由来cDNA分子の存在の指標として前記レポーター基を含有する液滴を検出することと、
