Abstract
Description

本発明は、手術または他の介入処置中の心臓弁の接合高さの評価に関する。
心臓弁機能の評価は現在、さまざまな方法で行われている。心臓弁膜症の患者の評価では、心エコー図が心臓弁の機能を評価するための主要な評価ツールであることがよくある。4つの主要な弁(大動脈弁、肺動脈弁、僧帽弁、および三尖弁)の各々には、特定の関連する解剖学的および機能的特性がある。これらの弁は心エコー図によって評価されるため、逆流つまり逆方向への流れの程度、および狭窄つまり順流に対する障害は、弁機能の重要な側面である。弁が適切に機能していない場合、機能が低下する原因となるいくつかの異なるメカニズムが1つ以上ある可能性がある。逆流性または漏出性の弁の場合、隣接する心臓弁尖間の適切かつ安定した接合の欠如が、弁を介した漏出の根本的な理由であることが多い。適切な接合の欠如の正確なメカニズムはさまざまで、弁の下の支持構造(弁下組織)の問題、弁尖の問題、または弁の周りの支持構造(弁輪)の問題を含む。
本開示は、既知の方法を用いる心臓弁の評価における接合高さの測定に関連付けられる課題の多くを克服する。本明細書に記載される接合測定装置は、カテーテル法の手順中または弁の開放外科的評価中に心臓弁を横切るように配置される。接合測定装置は、弁の接合表面に沿った弁の接合高さを定量化することができる。この接合高さ測定は、弁の接合表面に沿って評価され、弁のさまざまな局面を判断して、心臓弁を評価および/または修復しているカテーテル検査室の心臓専門医または外科医に貴重な情報を提供することができる。接合高さの情報は、弁の介入の妥当性を導き、ならびに調整および評価するために用いることができる。

Claims (21)
- 接合測定装置であって、
近位接続部材の端部にある細長いセンサ本体と、
前記センサ本体の面を横切るアレイ状の複数のセンサとを備え、
前記複数のセンサの各センサは、心臓弁の一部が前記センサと接触しているかどうかを検出するように構成される、接合測定装置。 - 複数のセンサが、前記センサ本体の第2の面を横切るように配置される、請求項1に記載の接合測定装置。
- 前記複数のセンサの各センサは、前記細長いセンサ本体の幅の少なくとも半分の幅を有する、請求項1または2に記載の接合測定装置。
- 前記細長いセンサ本体は可撓性である、請求項1〜3のいずれか1項に記載の接合測定装置。
- 前記細長いセンサ本体は、数ミリメートルから数センチメートルの幅である、請求項1〜4のいずれか1項に記載の接合測定装置。
- 前記複数のセンサは、抵抗器要素または温度感知要素を含む、請求項1〜5のいずれか1項に記載の接合測定装置。
- 前記複数のセンサは、光ファイバ要素または超音波要素を含む、請求項1〜6のいずれか1項に記載の接合測定装置。
- 前記アレイはコンデンサアレイである、請求項1〜7のいずれか1項に記載の接合測定装置。

前記複数のセンサのどのセンサが、対応のセンサが前記心臓弁と接触していることを検出するかを検出することと、
