血小板及びフィブリンの血餅は、止血及び血栓症の血管を閉塞する。本明細書において、活性化好中球によって放出される抗菌DNA繊維である好中球細胞外トラップ(NET)に基づく血管閉塞の非標準的なメカニズムを報告する。2つのDNase、DNase1及びDNase1様3が、マウスの炎症のエピソード中に、循環中でNETを分解することを示す。両方のDNaseが存在しない場合、血管内NETは血餅を形成し、これは血管を閉塞させ、臓器損傷を引き起こす。重度の細菌感染症の患者における血管閉塞は、ex vivoでのNET分解の欠損及び血管内NET血餅の形成に関連している。DNase1及びDNase1様3は独立して発現し、したがって炎症におけるNETの有害な影響に対する二重の宿主保護を提供する。
患者の血漿
ドイツでの2011年のSTEC−HUSアウトブレイク中に、患者からクエン酸血漿サンプルを採取した(16)。本研究に含める基準は、STEC/腸管出血性E.coliもしくは血性下痢の陽性同定、血小板数≦150×109/Lもしくは1週間で≧25%の減少、溶血のエビデンス(正常限界を上回るLDH、正常限界を下回るハプトグロビン、または破砕赤血球の存在)及び急性腎損傷ステージ≧1である。
ヒト組織を室温で一晩、5%リン酸緩衝ホルマリンで固定化した。すべての組織は、剖検で採取し、もともと診断目的で当社に提出されものであり、国家倫理原則に従って研究した。患者の死因を図10に示す。
すべてのマウスは、C57BL/6の遺伝的バックグラウンドを有していた。以前に記載されたDNase1−/−及びDNase1様3−/−マウス(26,28)を交雑させてDNase1/DNase1l3−/−マウスを生成した。DNase1−/−マウスのこの系統は、ミトコンドリアシャペロンをコードするDNase1重複遺伝子Trap1/Hsp75にオフターゲット変異を含むことが最近報告された(29)。したがって、対照実験のためにDNase1/DNase1l3−/−マウスを生成するための代替DNase1−/−系統(30)を含めた(図15A〜図15C)。
以前に記載したように、修正を加えてDPZを実行した(19)。簡潔に述べると、DNAを含まない4%(v/v)スタッキングゲルと、200μg/mlのサケ精巣DNAを含む10%(v/v)分離ゲル(Sigma−Aldrich、ドイツ)を用いてドデシル硫酸ナトリウム(SDS)ポリアクリルアミドゲルを調製した。DNase1を検出するために、0.5μlのマウス血清を14.5μlの水と5μlのSDSゲル充填緩衝液(BioRad、ドイツ)と混合した。混合物を5分間煮沸し、ゲルにロードした。電気泳動は、トリス/グリシン電気泳動緩衝液(25mMトリス、192mMグリシン、0.1%(w/v)SDS、pH8.7)を使用して120Vで実施した。電気泳動後、5%(w/v)粉乳、10mM Tris/HCl pH7.8、3mM CaCl2、3mM MgCl2、100U/mLペニシリン、及び100μg/mLストレプトマイシンを含むリフォールディング緩衝液中でゲルを37℃で一晩インキュベートして、タンパク質をリフォールディングした。ゲルを、粉乳を含まないリフォールディング緩衝液に移し、37℃でさらに24時間インキュベートした。DNAを1×SYBR Safe(Thermo Scientific、ドイツ)で蛍光標識し、蛍光スキャナー(Molecular Imager FX、BioRad、ドイツ)を使用してゲルの蛍光画像を記録した。
DNase1l3の検出のために、2μlの血清を12μlの水、5μlのSDSゲルローディングバッファー、及び1μlのβメルカプトエタノール(BME、Sigma−Aldrich)と混合した。BMEはDNase1のジスルフィド架橋を減少させ、その不活性化を引き起こす(11)。混合物を5分間煮沸し、ゲルにローディングした。DNase1について記載したように電気泳動を実施した。ゲルを10mM Tris/HCl pH7.8で50℃、30分間、2回洗浄することにより、SDSを除去した。10mM Tris/HCl pH7.8、1mM BME、100U/mLペニシリン及び100μg/mLストレプトマイシンを含むリフォールディングバッファー中で、タンパク質を37℃、48時間インキュベートすることによりリフォールディングさせた。次いで、ゲルを3mM CaCl2及び3mM MnCl2を添加したリフォールディングバッファー中、37℃でさらに48時間インキュベートした。DNase1l3によるタンパク質フリーDNAの効率的な分解を可能にするには、Mn2+の添加が必要である(11)。DNase1について記載したように、DNAを標識し、画像化した。
総DNase活性を測定するために、Mn2+を含む緩衝液(20mM Tris−HCl pH7.8、10mM MnCl2、2mM CaCl2、及び2×SYBR Safe)にサケ精巣由来の55μg/ml DNAを溶解させた。DNA溶液を50℃で10分間加熱し、等量の2%ultra−pureアガロース(Thermo Scientific)と混合した。混合物をプラスチックトレイに注ぎ、固化するまで室温で保存した。2μlのマウス血清を直径1.0mmのウェルにロードした。ゲルを湿潤チャンバー内で37℃、4時間インキュベートした。ゲルのDNA蛍光を蛍光スキャナーで記録した。
NET分解を、以前に記載されたように分析した(19)。無血清DMEM中の精製ヒト好中球を、0.001%ポリリジン(Sigma)でコーティングした滅菌96ウェルプレート(Falcon,BD Technologies,ドイツ)に、ウェルあたり5×104細胞の濃度で播種した。NET形成を誘発するために、好中球を、100nMホルボール12−ミリスチン酸13−アセテート(PMA;Sigma−Aldrich)で、5%CO2及び加湿下、37℃、4時間活性化した。リン酸緩衝生理食塩水(PBS)を加え、96ウェルプレートを4℃で一晩保存した。NETをPBSで洗浄し、0.5%TritonX−100を含むPBSで5分間処置し、再度PBSで洗浄した。2価陽イオンを含むHBSS(HBSS+;Thermo Scientific)中の10μM PPACK(Santa Cruz,Heidelberg,ドイツ)を添加した10%マウス血清または10%クエン酸ヒト血漿の存在下でNETをインキュベートした。5%CO2及び加湿下、37℃で3時間にわたってNETを分解可能とした。上清を廃棄し、PBS中に2%PFAを加えて室温で1時間、NET分解を停止させた。PFAを廃棄し、PBS中に2μMの蛍光DNA色素SytoxGreen(Thermo Scientific)を加えて、非分解NETを蛍光標識した。蛍光染色したNETの画像は、倒立蛍光顕微鏡(Axiovert200M,Zeiss,ドイツ)で取得した。プレートリーダー(励起:485nm;放射:535nm)で蛍光強度を記録するか、ImageJソフトウェア(NIH,USA)を使用して顕微鏡写真でNETの占有面積を測定することにより、非分解NETを定量化した。
pLIVEプラスミド(Mirus Bio,USA)を使用して、マウスにおいてタンパク質を発現させた。このベクターは、長期にわたって肝細胞特異的なタンパク質を発現させることができる。マウスDNase1、DNase1l3、及びCSF3を用いてpLIVEプラスミドを生成した。マウスDNase1については、cDNA(Genbank登録番号NM010061)のPCRを、SalI及びXhoI制限酵素部位を含むプライマーペアDNase1−F 5’−GTCGACATGCGGTACACAGG(配列番号27)及びDNase1−R 5’−CTCGAGTCAGATTTTTCTGAGTGTCA(配列番号28)を用いて実施した。DNase1l3については、cDNA(Genbank登録番号AF047355)のPCRを、BamHI及びSacI制限酵素部位を含むプライマーペアDNase1l3−F 5’−GAAGTCCCAGGAATTCAAAGATGT(配列番号29)及びDNase1l3−R 5’−GCGTGATACCCGGGAGCGATTG(配列番号30)を用いて行った。両方のcDNAを、適切な酵素で事前に消化し、T4リガーゼ(New England Biolabs,ドイツ)を使用して、pLIVEベクターのマルチクローニングサイト(MCS)にクローニングした。DNase1l3を含むpLIVEベクターを、コンセンサスコザック配列と一致させ、タンパク質発現を最適化するために、mutDNase1l3−F 5’−AGTCGACTCCCGGCCACCATGTCCCTGCA(配列番号31)とそれに相補的なmutDNase11l3−R 5’−TGCAGGGACATGGTGGCCGGGAGTCGACT(配列番号32)のプライマーペアを用いて部位特異的変異誘発に供した。CSF3のcDNAを含むpLIVEベクターの生成は外部委託した(Eurofins Genomics,ドイツ)。CSF3(Genbank登録番号BC120761)のcDNAを、制限酵素部位SalIとXhoIの間のMCSに挿入した。生成されたすべてのベクターの配列は、二本鎖DNA配列決定によって確認した。対照として、追加的なインサートを有さない親のpLIVEプラスミドを使用した。PureLink HiPure Plasmid Maxiprep Kitを使用してすべてのプラスミドを精製し、High Capacity Endotoxin Removal Spin Column(いずれもThermo Scientific)を使用して、エンドトキシンの潜在的な混入物を除去した。
他の場所に記載するDNase1またはDNase1l3のcDNAを含むプラスミドを、10%KnockOut血清代替品(Thermo Scientific)を添加したDMEM培地(Thermo Scientific)の存在下、無血清条件でLipofectamine3000(Thermo Scientific)を用いて、ヒト胎児腎臓(HEK)細胞にトランスフェクトした。72時間後、上清を回収し、500×gで10分間遠心分離し、滅菌濾過し、3Kカラム(Amicon Ultra,Millipore,Darmstadt,ドイツ)を用いた超遠心分離により濃縮した。
DNase1、DNase1l3、CSF3を含むpLIVEプラスミド、または空の対照プラスミドを、ハイドロダイナミックな尾静脈注射によりマウスに投与した。簡潔に述べると、50μgのプラスミドを、マウスの体重の10%に相当する量の0.9%生理食塩水で希釈した。マウスをイソフルランで麻酔した後、尾静脈を介してプラスミド溶液を5〜8秒かけて静脈内注射した。稀な場合では、最初の24時間以内にマウスが注射から完全に回復せず、これらの動物は研究から除外した。共発現研究のために、50μgのCSF3プラスミドを50μgの空の対照プラスミド、またはDNase1及びDNase1l3を含むプラスミドと混合した。両方のプラスミドを含む溶液は、ハイドロダイナミックな尾静脈注射により投与した。
4週齢または8〜12週齢の雌マウスに、CSF3を含むpLIVEプラスミド50μgを注射して、G−CSFの過剰発現及び好中球増加症を誘発した。マウスを、注射後最初の1週間は8時間ごとにモニタリングし、その後は毎日モニタリングした。肛門周囲の温度を非接触赤外線温度計(Etekcity,ドイツ)で測定した。生存研究では、動物が苦痛の徴候を示した場合(自発運動がない、目を閉じる、時々喘ぐ)、マウスを安楽死させ、「非生存」と記録した。すべての場合において、これらの苦痛の徴候は、プラスミド注射前の体温に比べて≧4℃の体温の低下と定義される急速に進行する重度の低体温、及び血尿を伴っていた。非低体温マウスは苦痛の徴候を示さず、実験終了時に安楽死させた。臓器、血液、及び尿の採取物について、3つのD1/D1L3−/−マウスの4群を分析した。各群のマウスに、CSF3と空の血漿、DNase1、またはDNase1l3の混合物を注射した。マウスの各群は、群の最初の動物が苦痛、重度の低体温、及び血尿の徴候を示した場合に、生体サンプル採取のために安楽死させた。この時点は「転帰」と定義され、注射後3〜6日以内に発生した。
マウスに2μg/gの血小板除去抗体またはアイソタイプ対照抗体(いずれもEmfret,ドイツ)を腹腔内注射した。この処置は、血小板の>95%を循環から枯渇させる。処置はCSF3の注射の24時間後に開始し、実験が完了するまで48時間ごとに繰り返した。
粉末ダビガトランエテキシレート(Pradaxa,Boehringer Ingelheim,ドイツ)を40mg/gの用量で通常の粉末(Altromin,ドイツ)と混合した。粉末を蒸留水と混合し、室温で乾燥させることによりペレットを調製した。ダビガトランの給餌は、CSF3の注射の24時間後に開始した。ダビガトラン食餌を1日間与えたWTマウスは、ダビガトランを含まない通常の食餌を与えたマウスに比べて、活性化部分トロンボプラスチン時間の6.48±1.19倍(平均±SD、N=4)の増加を示した(スチューデントt検定;P<0.001)。
Escherichia coli(XEN14,Perkin Elmer)を、50μg/mlカナマイシンを含むLB培地で一晩培養した。細菌を、4000×gで10分間遠心分離することによりペレット化し、PBSで洗浄し、再懸濁した。1.5×109細菌/mlのアリコートを70℃で15分間インキュベートして細菌を熱殺菌した。アリコートは、さらなる使用までの間、−20℃で保存した。
8〜15週齢の雄マウスに、0.9%生理食塩水中のSalmonella enterica血清型thyphimurium(Sigma−Aldrich)由来の1μg/gのLPSを毎日3回腹腔内注射した。3回目のLPS注射に加えて、マウスには1.5×107熱殺菌E.coli/gの静脈注射を行った。表示されている生存時間は、E.coliの注射後の時間を示している。安楽死の時点で血液と臓器を採取した。図4A〜図4Iに示す完全な分析を行うには不十分な生体サンプルが2匹の動物から得られた(1×D1/D1L3−/−+Ctrl、1×D1/D1L3−/−+D1L3)。動物が重度の苦痛の徴候(接触に対する無反応性)を示した場合、マウスを安楽死させ、「非生存」と記録した。すべての非生存マウスは血尿及び四肢蒼白を示した。熱殺菌E.coliの静脈内注射の24時間後、すべての生存マウスを安楽死させ、「生存」と記録した。
顎下穿刺または眼窩後洞によって血液を採取した。血液を200μlの血清チューブ(Monovette;Sarstedt,ドイツ)及び200μlのEDTAチューブ(GK150、KABE Labortechnik,ドイツ)に回収した。3000×gで15分間遠心分離した後、血液の上清を採取して血漿を得た。血清は、血液を室温で1時間凝固させ、続いて3000×gで15分間遠心分離することにより得た。血清及び血漿サンプルは、さらなる使用まで−20℃で一定量に分けて保存した。臓器分析のために、PBSの心臓内注入によりマウスを灌流した。臓器を採取し、4℃、4%PFAで24時間固定化した。固定化臓器をパラフィンに包埋した。
血漿中のLDH、AST、ALT、クレアチニン、及びBUNを、標準化キット(Biotron Diagnostics,CA,USA)を使用し、製造元の説明書に従って定量した。マウスG−CSFはQuantikine ELISA Kit(R&D,英国)で定量した。EDTA血液中のヘモグロビンは、自動血球計数器(Idexx ProCyte Dx Hematology Analyzer,Netherlands)によって定量した。好中球をFACSで定量するために、0.2μgのフィコエリトリン標識抗マウスCD11b(M1/70,Biolegend,ドイツ)及び0.5μgのAlexa Fluor488抗マウスLy6G(1AB,Biolegend,ドイツ)の存在下で、全血を氷上で15分間インキュベートした。次いで、血液を0.5mlのPBSで希釈し、FACSCalibur(BD Bioscience,USA)で分析した。EDTA抗凝固処置した血液の血液塗抹を、ポリリジンでコーティングしたスライド(Hecht Assistant,ドイツ)上で調製した。風乾後、ドライアイス上で、1μM SytoxGreenを添加したメタノール中、1分間インキュベートするか、または市販のキット(In Vitro Diagnostikum,ドイツ)を使用してギムザで染色した。
パラフィン包埋切片を脱蝋し、再水和し、クエン酸緩衝液(10mMクエン酸ナトリウム、0.1%Tween、pH6)中で100℃、25分間抗原賦活化に供した。その後、切片を2.5%正常ヤギ血清(Vector,英国)で30分間ブロッキングした後、マウスオンマウスブロッキングキット(Vector)を用いて1時間インキュベートした。次いで、切片を、MPO(A0398,Agilent,ドイツ)、CRAMP(PA−CRLP−100,Innovagen,スウェーデン)、シトルリン化ヒストン3(ab5103,Abcam,英国)フィブリン[クローン59D8]、またはヒストンH2A、H2B、及びDNAの複合体に対する2μg/mlの一次抗体の存在下で4℃、一晩インキュベートし、クロマチンを検出した(2)。切片を、AlexaFluor488またはAlexaFluor555(すべてThermo Scientific)とコンジュゲートした抗ウサギ及び抗マウスIgG抗体の存在下で1時間インキュベートした。洗浄後、DNAを1μg/ml DAPIで2分間標識した。Sudan Black(70%エタノール中0.1%)存在下での25分間のインキュベーションにより自己蛍光をクエンチし、Fluoromount G(Southern Biotech,USA)で切片をマウントした。蛍光標識切片の画像は、倒立蛍光顕微鏡(Axiovert200M,Zeiss,ドイツ)または共焦点顕微鏡(TCS SP5,Leica,ドイツ)で取得した。NET血餅の画像は、ソフトウェア(ImageJ)を使用して定量した。NET血餅のvWFまたはフィブリン染色領域が3%未満である場合、陰性とみなした。
抗原賦活化後、脱パラフィン切片を2.5%正常ウマ血清(Vector)で30分間ブロッキングした。切片を2μg/mlの好中球エラスターゼに対する一次抗体(ab68672,Abcam)の存在下で4℃、一晩インキュベートした。洗浄後、抗ウサギIgG−APキット(ImmPRESS,Vector)を使用して、製造元の説明書に従って切片を染色した。切片をヘマラム(Merck,ドイツ)で対比染色し、Neo−Mount media(Merck)を用いてマウントした。倒立顕微鏡(Axiovert200M,Zeiss)で染色切片の画像を取得した。
精製したヒト好中球を、2%BSAを添加したDMEM中、3×107細胞/mlで組織培養プレートに播種した。加湿及び回転条件(300rpm)下、37℃、5%CO2で4時間、細胞を0.1μM PMAで活性化した。未刺激好中球、及び10U/ml組換えヒトDNase1(ドルナーゼα,Roche,Germany)の存在下、PMAで活性化した好中球を対照として用いた。NET血餅を2%PFAで4℃、一晩固定化した。固定化したNET血餅をパラフィンに包埋し、上記のようにNETについて切片を染色した。
データは平均±標準偏差として示す。統計解析は、Prism Software(GraphPad,USA)を使用して実施し、スチューデントt検定、一元配置分散分析及び二元配置分散分析、それに続くボンフェローニの多重比較検定、及び対数順位検定を含んでいた。結果はP<0.05で有意とみなした。
慢性好中球増加症は、野生型マウスにおいて忍容される。生成した映像は、CSF3発現プラスミド(CSF3)またはCSF3を欠く対照プラスミド(Ctrl)の注射から1週間後の野生型マウスを示す。いずれの動物も正常な行動を示し、苦痛の徴候はない。

序論
制御されない炎症は、過剰な病態を引き起こす(1)。好中球細胞外トラップ(NET)は、毒性タンパク質で装飾されたDNAフィラメントである(2)。NETは微生物感染から保護するが、過剰なNET形成は炎症性疾患及び自己免疫疾患を引き起こし、悪化させる(3)。血液中に見出されるDNA分解酵素であるDNase1(D1)は、NETをin vitro及びin vivoで無害化するために広く使用されている。実際、嚢胞性線維症(CF)のFDA承認薬(ドルナーゼα)である組換えヒトD1の注入により、様々な疾患モデルにおいてNETの病理学的影響が予防されるが(3)、これは予防療法のためのD1の使用を支持するものである。
実施例1では、D1に加えて、DNase1様3(D1L3)もNETを分解することが明らかとなった。両方の酵素は、炎症反応中に血液と組織の恒常性を維持する。簡潔に述べると、本発明者らは、炎症状態の特徴である好中球増加症時に、無菌状態及び感染状態下で血管内にNETが生成されることを示した。D1及びD1L3はNETを切断するため、NETのDNAフィラメントが血管の血餅に凝集するのを予防する。D1及びD1L3が存在しない場合、NET血餅は血流を妨げ、臓器の損傷及び死を引き起こす。NET血餅は、血小板またはフィブリンとは独立して形成されるため、従来の抗血栓療法に耐性がある。したがって、本発明者らは、血管閉塞の新しいメカニズムとしてNET血餅を発見し、また、NETに対する新規予防療法であるD1L3を発見した(実施例1参照)。
本実施例では、細胞外クロマチン(NETを含む)に対する治療用に改変したD1及びD1L3のバリアントを生成した。バリアントを設計するために、クロマチンを分解するヒトD1L3の能力は、ヒトD1には存在しない個々のアミノ酸及び/またはアミノ酸配列によってコードされると仮定した。さらに、これらの個々のアミノ酸及び/またはアミノ酸配列をヒトD1L3からD1に転移させると、クロマチン分解活性が増加した機能亢進性D1バリアントが生成すると推測した。同様に、酵素特性(例えば、グリコシル化)をD1からD1L3に転移させることができる。仮説を試験するために、従来のアミノ酸変異を採用し、新規ビルディングブロック(BB)技術を開発し、DNase1タンパク質ファミリーのDNaseのバリアントを改変した。
従来のアミノ酸置換によるDNase1バリアントの改変
本発明者らの中心的な仮説は、クロマチンを分解するヒトD1L3の能力は、D1に存在しないアミノ酸によって媒介されるというものである。配列アライメントにより、ヒトD1及びヒトD1L3のアミノ酸の44%が同一であることが示された(図17)。可変アミノ酸(56%の非共有アミノ酸)のみを変異させて、D1及びDL3バリアントを生成した。
D1L3は、追加のアミノ酸を含む3つの部位を特徴とする:Q282(NH2−SSRAFTBSKKSVTLRKKTKSKRS−COOH)の後に始まるC末端テール(配列番号33)、ならびにS79/R80及びK226の酵素内の2つの部位(図17)。D1L3のC末端の23アミノ酸は、D1のC末端に結合している(10、28)。DNase1の226位にアルギニン残基を挿入すると(A226_T227insK)、dsDNAを分解する酵素活性が低下したD1バリアントが生成されたが、置換T227Kではそのような効果は観察されなかった(図27)。したがって、K/R残基を挿入すると、D1機能が低下するリスクが伴う。荷電アミノ酸の挿入は、局所タンパク質構造に影響を与え得る。D1が1つのアミノ酸鎖を含む球状酵素であると考えると[図28、(27)]、そのような局所的な変化が酵素全体を不活性にし得ると考えられる。実際、D1アミノ酸配列全体にわたる多くの非保存的変異が酵素を不活性化する(12、25、26)。単一のアルギニン及びリジン残基だけでなく、隣接するアミノ酸配列を移植することによる局所タンパク質構造の転移により、不活性化のリスクが低下すると仮定した。挿入のアンカーとして使用することができるA226_T227insK付近の保存アミノ酸をD1及びD1L3内で検索した。本発明者らは、N末端及びC末端アンカーとして、D1のD223/T224/T225モチーフ及び保存されたT229をそれぞれ同定した(図27)。D1(ATP)内の3つのアミノ酸を、D1L3(V[K]KS)から、K226を含む4つのアミノ酸にin silicoで置き換えた。HEK239細胞において新規D1バリアント(A226_P228delinsVKKS)のcDNAを発現させることにより、野生型D1と比較した場合に同様のdsDNA分解活性を有する機能的に活性な酵素が明らかとなった(図27)。データは、保存されたアミノ酸間の可変アミノ酸がD1とD1L3の間で交換可能であることを示唆している。
(1)ドナー及びレシピエントDNaseのタンパク質間アライメントを提供し、
(2)転移用の可変アミノ酸またはアミノ酸配列(ビルディングブロック)を同定し、
(3)それぞれビルディングブロック(アンカー)の上流と下流に位置するドナー及びレシピエントDNaseの保存アミノ酸を同定し、
(4)レシピエントDNaseのCアンカーとNアンカーの間のビルディングブロックをコードするcDNAを、ドナーDNaseのアンカー間のcDNAと置き換え、
(5)キメラDNaseのcDNAを合成し、その後、好ましくはドナー及びレシピエントDNaseの両方を欠くレシピエント生物(例えば、CHO細胞またはDnase1−/−Dnase1l3−/−マウス)内でin vitro/in vivo発現させる。
ヒト、マウス、ラット、及びチンパンジー由来のD1及びD1L3の複数種のアライメント(図30)は、これらの種の間でN及びC末端アンカーが保存されていることを示した。これらのアンカーアミノ酸またはアミノ酸配列は、可変アミノ酸及びアミノ酸配列の62個のビルディングブロックに隣接しており、これらには、上記の実験のD1(ATP)及びD1L3(VKKS)のアミノ酸配列がビルディングブロック#49として含まれる(図31)。
D1L3のBBクラスター60〜62は、そのC末端テール由来のアミノ酸を含む(図33)。テールはL281の後に始まり(図34)、二分のNLSを含むアミノ酸配列QSSRAFTBSKKSVTLRKKTKSKRS(配列番号33)を有する。D1L3は、トランスフェクション試薬及びアポトーシス小体を含む脂質小胞内のDNAを分解するためにC末端テールを必要とする(7、10、28)。さらに、D1L3による脂質フリー及び細胞フリーのクロマチンの分解にはC末端が必要である(7)。両方の調査結果は、Q282の後のアミノ酸が欠失したD1L3バリアントに基づいているため、これらの基質の分解では不活性である。Wilbertらは、D1L3のC末端延長部をD1のC末端に結合させ、脂質小胞内でのDNA分解能力をD1に転移させた(10、28)。本明細書において、BB技術を使用して、C末端をD1L3からD1へ、またはその逆に転移させた。Nアンカーとして、D1の末端βシートの前に位置する保存されたS272/D273/H274モチーフ(図28)を使用し、D1とD1L3の間で末端の3つのビルディングブロックを交換した(図34)。このアプローチにより、変異Y275F/V279_M280delinsFK/K282delinsQSSRAFTNSKKSVTLRKKTKSKRS(配列番号16)によるD1L3のC末端延長部を特徴とするD1バリアント、及び変異F275Y/F279_K280delinsVM/Q282_S305delinsK(配列番号17)による短縮型DIL3バリアントを生成した。HEK293細胞内でバリアント及び野生型酵素の両方を発現させ、上清中のDNase活性を特徴決定した。予想に反して、D1L3のC末端を有するD1バリアントはクロマチンを分解せず、一方、野生型酵素のC末端を欠くD1L3バリアントは依然としてクロマチンを切断することができることが観察された(図34)。実際、このD1L3バリアントではクロマチン分解活性の増加が観察された。この発見を裏付けるために、D1L3の短縮型バリアントがin vivoで存在するかどうかを試験し、その発現によって好中球Dnase1−/−Dnase1l3−/−マウスのNETを介した致死性から保護されることを観察した(図34)。結論として、C末端の欠失は、D1L3の機能を阻害せず、むしろ増加させ、脂質フリー及び細胞フリーのクロマチンを分解する。
最後に、D1L3の治療効果をin vivoで試験した。NETの血管血餅の組織学的分析は、クロマチンの密な凝集体を示し(図36A)、このことはD1のヒストンフリーの切断部位がin vivoでは稀であることを示す。したがって、野生型D1L3を用いた急性療法は、野生型D1に比べて血管内NETの分解に効果的であると仮定した。仮説を検証するために、Dnase1−/−Dnase1l3−/−欠損マウスでCsf3を発現させて、NETの血管血餅を誘発した。重要な点として、転帰に先行して、NETによる血管閉塞を有するマウスは血尿及び低体温を発症するため、これにより非侵襲的に検出することができる。したがって、動物モデルにより、NETを形成したマウスを同定し、したがって選択して、NETに対する急性療法の薬物候補を試験することができる(図36B)。本明細書において、NETに対する急性療法用の、精製した野生型ヒトD1(ドルナーゼα;配列番号1)、野生型ヒトD1L3(配列番号2)、及び機能亢進性D1L3バリアント[D1L3V;(配列番号17)]を試験した。Csf3をDnase1−/−Dnase1l3−/−欠損マウスに注射した。72時間後、最初の個々の動物が低体温症及び血尿を発症し始めた。4℃以上の体温の低下と赤色尿の存在を選択基準として使用した。これらの表現型を発症したマウスを、異なる治療群に無作為化した。動物に、1mg/kgのD1、D1L3、またはビヒクルを尾静脈から単回用量で静脈内注射した。30分後、動物を安楽死させて、生体サンプル(血液及び臓器)を採取した。肺の脈管構造におけるNET血餅の定量化により、ビヒクルまたはD1療法を受けているマウスにおいてNETが血管を閉塞したことが示された(図36D)。重要な点として、D1L3及びD1L3Vを注射したマウスは、NET血餅を全くまたはほとんど示さなかった(図36D)。患者においては、循環DNAはモノヌクレオソームにおけるDNAのサイズ[180塩基対、(33)]を有しており、このことはそれがクロマチンの高分子量DNAの分解産物であることを示している。循環DNAは、血管内NET形成の代替マーカーとして一般的に使用されるが、損傷組織及び壊死組織にも由来し得る(34)。蛍光DNAプローブを使用して血清中のDNAを定量した。D1L3及びD1L3Vの注射により、循環DNAレベルの強い増加が引き起こされたが、D1またはビヒクルでは引き起こされなかったことが観察された(図36E)。循環DNAの単離及び可視化により、多量体の180塩基対断片が示され、D1L3療法を受けたマウスの血清中のモノ及びオリゴヌクレオソームの存在が示された(図36F)。D1L3Vを投与したマウスは、モノヌクレオソーム及びジヌクレオソームのみを示した。データからは、in vivoでのD1L3Vの機能亢進性が確認される。D1療法を受けたマウスの血清由来のDNA単離物は、様々なサイズのかすかなDNAスメアを示したが、ビヒクルを注射したマウスの血清からはDNAを単離することができなかった(図36F)。したがって、D1L3は、予防のためだけでなく、NETに関連する疾患の急性治療にも使用し得る。
すべてのタンパク質治療薬のほぼ70%がチャイニーズハムスター卵巣(CHO)を使用して製造されている(36)。実際、野生型DNase1(ドルナーゼα)はCHO細胞において産生される。大きな利点にもかかわらず、CHOでの細胞株開発と大規模生産には、かなりの程度の変動性と、細胞増殖特性を予測またはモデル化するための信頼できる方法がないことから、依然として大きな課題が残されている(36)。重要な点として、CHO細胞はD1の機能亢進性バリアントを安定して産生することができなかったため、それらの臨床製造は妨げられていた(12)。D1L3の製造特性は不明である。したがって、野生型D1(配列番号1)、機能亢進性D1バリアント(配列番号7)、野生型D1L3(配列番号2)、及び機能亢進性D1L3バリアント(配列番号17)を産生する安定したCHO細胞株を開発した。細胞株をバイオリアクター内で培養したところ、野生型D1及びD1バリアントで安定したタンパク質発現が観察され、後者は、発現レベルが大幅に増加していたが(図38)、このことはCHOにおいて機能亢進性D1バリアントを産生する実行可能性を示している。予想に反して、野生型D1L3及びD1L3バリアントは、わずかなタンパク質レベルが観察されたに止まり(図38)、D1L3及びそのバリアントの臨床製造の主要な課題が指し示された。
2つのDNA分解酵素は、実験モデル及び患者において治療的に広く使用されている。Streptococcus種から分泌されるDNaseであるストレプトドルナーゼは、皮膚の同種移植片、膀胱血餅、創傷壊死組織切除、髄膜炎の疾患モデルにおいて試験されている。DNase1(膵臓DNase1を含む)は、がん、嚢胞性線維症、ループス、血栓症、脳卒中、敗血症、肺損傷、アテローム性動脈硬化症、ウイルス感染症、鎌状赤血球症、心筋梗塞、耳感染症、創傷治癒、肝損傷、心内膜炎、肝臓感染症、膵炎、原発性移植片機能不全、四肢虚血再灌流障害、腎臓損傷、血液凝固、喘息、ミョウバン誘発炎症、肝腎損傷の実験モデルにおいて治療的に使用されている。ストレプトドルナーゼはまた、胸水滲出、血胸、胆管内血餅、肺炎後貧血、潰瘍、耳鼻咽喉病態、口腔感染症、軽傷、副鼻腔炎、術後鼻形成術、不妊症、膀胱カテーテル、創傷洗浄、皮膚反応検査を有する患者の治療にも使用されている。重要な点として、DNase1は、肺炎球菌髄膜炎、痛風、下肢潰瘍、嚢胞性線維症、カルテゲナー症候群、喘息、肺葉無気肺、慢性気管支炎、気管支拡張症、ループス、原発性線毛機能不全、細気管支炎、膿胸、胸膜感染症、がん、ドライアイ疾患、下気道感染症、慢性血腫、アルツハイマー病、及び閉塞性肺疾患を有する患者において治療的に試験されている。本実施例では、D1L3、そのバリアント、及びD1バリアントをDNase療法の新規候補として同定した(図39)。
クロマチン分解活性の検出
クロマチン分解活性を測定するために、サンプルを、HEK293(反応あたり1,500,000〜3,000,000個)から単離した核、20mM Tris−HCl pH7.4、2mM MgCl2、2mM CaCl2、50mM NaCl及びプロテアーゼ阻害剤と混合した。サンプルを37℃で1時間インキュベートした後、サンプルを65℃まで10分間加熱した。QIAamp DNA Blood Mini Kit(Qiagen)を使用して、製造元のプロトコールに従ってDNAの単離を行った。次いで、溶出液をアガロースゲルにロードし、ゲル電気泳動によりDNA断片を分離した。ゲルのDNA蛍光を蛍光スキャナーで記録した。
