Abstract
Description

研究デザインおよび患者:
本研究は、2009年11月から2013年2月までドイツ全土で33か所の学際的な集中治療室(ICU)で実施された、重症敗血症(SISPCT)における亜セレン酸ナトリウムおよびプロカルシトニン誘導抗微生物療法のプラセボ−対照トライアルの二次分析である(26)。適格基準は、ACCP/SCCMコンセンサス会議委員の敗血症−1の定義に従い、さらに2016年の定義(SEPSIS−3および敗血症性ショック−3)(4)に従って分類された、≧18才の、新しく重症敗血症または敗血性ショックの発症を(≦24時間に)提示している成人患者を含んだ。研究デザイン、データ収集、および管理の詳細は、以前説明した(26)。Jena大学病院および他のすべてのセンターの倫理委員会から研究を承認され、必要に応じて書面によるインフォームドコンセントを得た。
患者は、重症敗血症または敗血症性ショックの診断後最大24時間までに登録し、その直後にPCT、CRP、および乳酸を測定した。PCTは、0.02〜5000ng/mlの測定範囲、ならびにそれぞれ少なくとも0.06ng/mlおよび0.02ng/mlの機能アッセイ感度および検出下限を有するデバイスで測定した。すべての患者から追加の血液試料を収集し、−80°CでJenaの中央研究室に保管した。MR−proADM血漿濃度は、0.05nmol/Lの検出限界で遡及的に測定した(Kryptor(登録商標)、Thermo Fisher Scientific、ドイツ)。研究登録の際に、連続的臓器不全評価(SOFA)、急性生理学的および長期的健康評価(APACHE)II、および簡易急性生理学(SAPS)IIスコアを含む臨床重症度スコアを取得した。
28日死亡率に関する人口統計学的特性および臨床的特徴の差は、分布の正規性に応じて、カテゴリ変数についてはχ2検定を、連続変数についてはスチューデントt検定またはMann−Whitney U検定を使用して評価した。正規および非正規分布変数は、それぞれ平均(標準偏差)および中央値[第1四分位−第3四分位]と表した。すべての時点での死亡率と各バイオマーカーおよび臨床スコアとの関連性は、年齢、ならびに併存疾患および敗血症性ショックの存在に基づいて修正した多変量解析を用いて、受信者動作特性曲線(AUROC)の下の面積およびCox回帰分析を使用して評価した。患者はさらに、各時点での各バイオマーカーおよび臨床スコアの合計集団にわたり、所定の感度および90%に近い特異性を用いて、2つのAUROCカットオフの計算に基づいて、3つの重症度サブグループ(低、中程度、高)に分類した。その後、ICU関連の手順または合併症(病巣洗浄手順、緊急手術、新たな感染の出現、血液製剤の注輸、コロイドの注入、侵襲的機械的換気、腎/肝代替、または昇圧剤療法、ならびに患者の一般的な臨床徴候および症状を含む)の存在しない、臨床的に安定したサブグループの患者を識別し、以前の測定以降増加が見られなかった、対応する低MR−proADM濃度でさらなるグループを識別した。死亡率および平均滞在期間は、両方のグループで計算し、特定の各時点で退室させた患者グループと比較した。
研究登録時の患者の特徴を表1に要約する。
単変量および多変量のCox回帰分析により、MR−proADMは、合計患者集団、ならびに敗血症−3および敗血症性ショック−3サブグループ内の28日死亡率と、最も強い関連性を有することが見出された(表2)。対応するAUROC分析により、APACHE II(敗血症−3患者サブグループ)を除き、MR−proADMとすべてのバイオマーカーおよび臨床スコアとの比較において、有意な差が見られた。
既存のSOFA重症度レベル、ならびに各サブグループで評価した28日死亡率を予測するバイオマーカーおよび臨床スコアの性能に従って、合計患者集団をさらに層別化した。MR−proADMは、低(SOFA≦7)および中程度(8≦SOFA≦13)重症度のSOFAサブグループにおいて、すべてのパラメータで最も高い正確性を示した(表5、表6)。
研究コホートは、病巣洗浄手順、緊急手術、新たな感染、血液製剤の注輸、コロイドの注入、侵襲的機械的換気、腎/肝代替、患者の一般的な臨床徴候および症状の悪化などのICUに関係する手順または合併症に直面しなかった、臨床的に安定した患者のサブセットで構成される。
臨床的に安定した患者のこのグループは、低リスク患者として分類した。
時間依存的なコックス回帰分析により、1日目にMR−proADMベースライン値についての予後診断情報で、最も早い有意な追加の増加が観察され、その後の単回または累積測定により、28日死亡率との有意に強い関連性が生じることが示された(表14)。したがって、2つのPCT誘導アルゴリズムモデルを構築して、ベースラインから1日目または4日目のいずれかのPCT変化を調査し、MR−proADMの重症度分類に基づいて、対応するサブグループ分析を行った。
MR−proADMは、感染源に関係なく、肺炎および腹腔内感染症の患者、ならびにグラム陽性菌感染の患者で、最も強い関連性を示した(表19〜20)。緊急手術、非緊急手術、および待機的手術の履歴に従って患者をグループ分けしてICUに入室させると、MR−proADMは、すべてのグループで、28日死亡率と最も強く最もバランスの取れた関連性を提供した(表21)。
MR−proADMは、ベースラインですべてのバイオマーカーおよびSOFAスコアと最大の相関性を有し、ベースライン値が1日目のSOFAスコアと相関すると有意に増加した。最大の相関性は、10日目のMR−proADMとSOFAとの間に見ることができ、個々のSOFAサブスコアとの差が全体に見られた(表22〜24)。
同様の結果は、高MR−proADM濃度と、低または中程度SAPS II値(高MR−proADMサブグループ[死亡率54.8%および65.6%]、残りのSAPS II集団[死亡率19.7%および30.0%])のいずれかを有する124人(12.0%)の患者のサブグループ、ならびに低または中程度APACHE II値(高MR−proADMサブグループ[死亡率56.9%および66.7%]、残りのAPACHE II集団[死亡率19.5%および30.3%])のいずれかを有する109人(10.6%)の患者に見ることができた。

PCT誘導抗生物質アルゴリズム内で組み合わせると、MR−proADMは、抗生物質療法の将来の変更または修正を必要とするであろう患者とそうでない患者とを層別化することができる。
低から中程度重症度レベルの、増加しているMR−proADM濃度を有する患者は、継続的に低レベルであった患者よりも4日目に抗生物質療法の修正を必要とする可能性がより高かった(オッズ比[95%CI]:1.5[0.6〜4.1])。
中程度から高重症度レベルに増加しているMR−proADM濃度、または継続的に高濃度を有する患者はまた、継続的に低MR−proADM濃度を有する患者よりも4日目に抗生物質療法の変更を必要とする可能性がより高かった(オッズ比[95%CI]:それぞれ5.9[1.9〜18.1]および2.9[0.8〜10.4])。
ベースラインから1日目、またはベースラインから4日目のいずれかでのPCT濃度の増加にもかかわらず、継続的に低MR−proADM濃度を有する患者は、継続的に中程度または高濃度の患者よりも、処方された抗生物質治療に対する修正が有意に低かった。
疾患重症度の正確かつ迅速な評価は、最も早い機会に最も適切な治療を開始するために重要である。実際、治療が遅れる、あるいは不十分であると、患者の臨床状態が全般的な悪化に至り得、結果的に将来の治療の効果が低下し、全体的な帰結が悪化する可能性が高くなる(8、27)。その結果、この満たされていない臨床的ニーズを満たすために多数のバイオマーカーおよび臨床的重症度スコアが提案されており、現在、連続的臓器不全評価(SOFA)スコアが最も適切なツールとして強調されており、2016年の敗血症−3の定義における中心的な役割をもたらしている(4)。SISPCTトライアルのこの二次分析(26)は、重症敗血症および敗血症性ショックを患う大規模な患者集団において、初めて、乳酸、プロカルシトニン(PCT)、およびSOFAなどの、従来のバイオマーカーおよび臨床スコアの連続的測定を、微小循環機能障害マーカー、MR−proADMの測定と比較した。

Claims (16)
- 感染症を患っており、1つ以上の抗生剤で治療を受けている患者における抗生物質療法のガイダンス、層別化、および/または制御のための方法であって、
−第1の試料を前記患者から単離することと、
−前記第1の試料の単離および抗生物質治療の開始後の時点で、前記患者から第2の試料を単離することと、
−前記第1および前記第2の試料中のプロカルシトニン(PCT)またはその断片(複数可)のレベルを決定することと、
−少なくとも前記第2の試料中のproADMまたはその断片(複数可)のレベルを決定することと、を含み、
−前記第1および第2の試料中の前記PCTまたはその断片(複数可)のレベル、ならびに前記第2の試料中の前記proADMまたはその断片(複数可)のレベルが、1つ以上の抗生剤での治療に変更が必要であるかどうかを示す、方法。 - 前記患者の感染症の症状を決定する際に前記第1の試料が単離され、前記症状の最初の決定および抗生物質治療の開始後に前記第2の試料が単離される、請求項1に記載の方法。
- 前記第2の試料が、感染症の症状の決定および抗生物質治療の開始後30分以内、または感染症の症状の決定および抗生物質治療の開始から少なくとも30分、1時間、2時間、6時間、および/もしくは12時間後に前記患者から単離される、請求項2に記載の方法。
- 前記第2の試料が、感染症の症状の決定および抗生物質治療の開始から12〜36時間および/または3〜5日後に前記患者から単離される、請求項2に記載の方法。
- 前記患者が、敗血症および/または敗血症性ショックを患っていると診断される、請求項1〜4のいずれかに記載の方法。
- MR−proADMのレベルを決定することを含む、請求項1〜5のいずれか一項に記載の方法。
- −前記第1の試料と比較して上昇したレベルの、前記第2の試料中のPCTまたはその断片(複数可)、および
−前記第2の試料中の中程度または高重症度レベルのproADMまたはその断片(複数可)が、前記1つ以上の抗生剤の変更が必要であることを示し、
−中程度重症度レベルのproADMまたはその断片(複数可)が、4nmol/l超、好ましくは3nmol/l超、より好ましくは2.7nmol/l超かつ6.5nmol/l未満、好ましくは6.95nmol/l未満、より好ましくは10.9nmol/l未満であり、
−高重症度レベルのproADMまたはその断片(複数可)が、6.5nmol/l超、好ましくは6.95nmol/l超、より好ましくは10.9nmol/l超である、請求項1〜6のいずれか一項に記載の方法。 - 加えて、前記第1の試料中のproADMまたはその断片(複数可)のレベルを決定することを含む、請求項1〜7のいずれか一項に記載の方法。
- 前記第1の試料と比較して上昇したレベルの、前記第2の試料中のproADMまたはその断片(複数可)が、前記1つ以上の抗生剤の変更が必要であることを示す、請求項1〜8に記載の方法。
- −前記第1の試料と比較して低レベルの、前記第2の試料中のPCTまたはその断片(複数可)、および
−第2の試料中の高重症度レベルのproADMもしくはその断片(複数可)、または低重症度レベルから中程度もしくは高重症度レベルへの、もしくは中程度重症度レベルから高重症度レベルへの上昇などの、第1の試料と比較して上昇した重症度レベルの第2の試料中のproADMまたはその断片(複数可)が、
−前記1つ以上の抗生剤の変更が必要であることを示し、
−低重症度レベルのproADMまたはその断片(複数可)が、4nmol/l未満、好ましくは3nmol/l未満、より好ましくは2.7nmol/l未満であり、
−中程度重症度レベルのproADMまたはその断片(複数可)が、4nmol/l超、好ましくは3nmol/l超、より好ましくは2.7nmol/l超かつ6.5nmol/l未満、好ましくは6.95nmol/l未満、より好ましくは10.9nmol/l未満であり、
−高重症度レベルのproADMまたはその断片(複数可)が、6.5nmol/l超、好ましくは6.95nmol/l超、より好ましくは10.9nmol/l超である、請求項1〜9のいずれか一項に記載の方法。 - −前記第1の試料と比較して50%超低レベルの、前記第2の試料中のPCTまたはその断片(複数可)、および
−前記第1の試料中の低重症度レベルのproADMまたはその断片(複数可)と比較して中程度重症度レベルの、前記第2の試料中のproADMまたはその断片(複数可)が、前記1つ以上の抗生剤の変更が必要であることを示す、請求項1〜10のいずれか一項に記載の方法。 - −前記第1の試料と比較して50%未満低レベルの、前記第2の試料中のPCTまたはその断片(複数可)、および
−第1の試料中の中程度重症度レベルのproADMまたはその断片(複数可)と比較して高重症度レベルの、第2の試料中のproADMまたはその断片(複数可)は、1つ以上の抗生剤の変更が必要であることを示す。 - 前記第1および前記第2の試料が、血液試料、血清試料、血漿試料、および/または尿試料からなる群から選択される、請求項1〜12のいずれか一項に記載の方法。
- 感染症を患っており、1つ以上の抗生剤で治療を受けている患者における、請求項1〜13のいずれか一項による、抗生物質療法のガイダンス、療法の層別化および/または制御のための方法であって、加えて、
−前記第1および前記第2の試料中の少なくとも1つの追加のバイオマーカーまたはその断片(複数可)のレベルを決定することを含み、前記少なくとも1つの追加のバイオマーカーが、好ましくは乳酸および/またはC反応性タンパク質であり、
−前記第1および第2の試料中の前記少なくとも1つの追加のバイオマーカーのレベル、ならびに前記第2の試料中の前記proADMまたはその断片(複数可)のレベルが、継続中の抗生物質治療に変更が必要かどうかを示す、方法。 - 感染症を患っており、1つ以上の抗生剤で治療を受けている患者における、請求項1〜14のいずれか一項による、抗生物質療法のガイダンス、療法の層別化および/または制御のための方法であって、加えて、

−前記第1および第2の試料の単離の時点での前記少なくとも1つの臨床スコア、ならびに前記第2の試料中の前記proADMまたはその断片(複数可)のレベルが、継続中の抗生物質治療の変更が必要かどうかを示す、方法。 - 請求項1〜15のいずれか一項に記載の方法を実行するためのキットであって、
−対象からの試料中の前記proADMまたはその断片(複数可)のレベルを決定するための、および任意選択的に加えて前記PCTまたはその断片(複数可)のレベルを決定するための検出試薬と、
−請求項6および/または9に記載のproADMの重症度レベル、ならびに任意選択的にPCTレベルに対応する参照レベルなどの参照データであって、前記参照データが、コンピュータ可読媒体に記憶され、かつ/あるいはproADMまたはその断片(複数可)の前記決定されたレベル、および任意選択的に加えてPCTまたはその断片(複数可)の前記決定されたレベルを前記参照データと比較するために構成されたコンピュータで実行可能なコードの形式で用いられる、参照データと、を含む、キット。
