Claims (20)
- 流体源に接続可能なポンプを含む流体管理システムと、
ポンプから手術空間へ流体を流すカニューレに取り外し可能に接続できるよう構成された流入ラインと、
前記流入ラインのフロー圧力を測定し、測定圧力値を生成するために配置されたフロー圧力センサーと、
前記ポンプ及び前記フロー圧力センサーに接続されたコントローラと、
を具備し、
前記コントローラは、(1)前記測定圧力値から、初期背圧テーブルから選択された背圧値を減算することによって計算された初期背圧調整圧力値(BAPV)に基づいてポンプ速度を制御することにより圧力設定値を維持し、(2)前記初期BAPVを監視して、前記初期BAPV が所定の初期BAPV範囲内にあるかどうかを判断するよう構成されていることを特徴とする、

- 前記コントローラはさらに、前記初期BAPVが前記所定の初期BAPV範囲内にある期間中は、前記初期BAPVに基づいて前記ポンプ速度を制御することにより前記圧力設定値を維持し続けるよう構成されていることを特徴とする請求項1に記載のシステム。
- 前記コントローラはさらに、前記初期BAPVが前記所定の初期BAPV範囲外になったときに、前記フロー圧力センサーと自由空間中の前記流入ラインの端部との間の、前記ポンプによって生じた種々の流量における背圧値を表す代替的背圧テーブルにアクセスするよう構成されていることを特徴とする請求項2に記載のシステム。
- 前記流入ラインの前記端部は、前記流入ラインから体内の前記手術空間に流体を流すのに適合したカニューレに取り外し可能に接続できるように構成されていることを特徴とする請求項4に記載のシステム。
- 前記コントローラはさらに、前記測定圧力値から、ポンプ速度に基づいて前記代替的背圧テーブルから選択された値を減算することによって計算された代替的BAPVに基づき、監視されている前記初期BAPVが前記所定の初期BAPV範囲外になったときに、前記ポンプ速度を制御するように構成されていることを特徴とする請求項4に記載のシステム。
- 前記コントローラはさらに、前記初期BAPVが前記初期BAPV範囲内の値に戻ったときに、前記初期背圧テーブルに再アクセスするように構成されていることを特徴とする請求項6に記載のシステム。
- 前記コントローラはさらに、前記監視されている前記初期BAPVが前記所定の初期BAPV範囲外になった後に、前記流入ライン及び置換カニューレに接続されたときに前記ポンプによって生じた複数の流量における背圧値を計測することによって生成した置換背圧テーブルにアクセスするように構成されていることを特徴とする請求項1に記載のシステム。
- 前記ポンプは、前記流入ラインに接続された流入ユニットと、流出ラインに接続された流出ユニットとを具備することを特徴とする請求項1に記載のシステム。
- 前記手術空間への低侵襲で挿入するよう構成された内視鏡をさらに具備し、前記内視鏡はさらに、取り外し可能に流入ラインへ取り付けるよう構成され、前記流体源から前記流体を流すための内腔を有することを特徴とする請求項9に記載のシステム。
- 前記内視鏡の前記内腔を通して挿入するように構成され、前記流出ラインに接続するための抽出チャンネルを有するインターベンショナルツールさらに具備することを特徴とする請求項10に記載のシステム。
- (a)ポンプを使用して手術部位に遠位端を有するカニューレに接続された入口ラインを通して流体を送り込むステップと、
(b)前記入口ラインの圧力センサーを使用して圧力値を測定するステップと、
(c)測定圧力値から、初期背圧テーブルから選択された背圧値を引くことにより、初期背圧調整圧力値(BAPV)を計算するステップと、
(d)前記初期BAPVを圧力設定値に維持するために、前記ポンプのポンプ速度を制御するステップと、
(e)前記初期BAPVを監視して、前記初期BAPVが所定の初期BAPV範囲内にあるかどうかを判断するステップと、
(f)前記初期BAPVが前記所定の初期BAPV範囲内にある期間中は、前記初期BAPVを前記圧力設定値に維持するよう前記ポンプ速度を制御するステップと、
を具備する組織を治療する方法。 - 前記初期背圧テーブルは、前記ポンプが流入ライン及び前記カニューレに接続されている間であって、前記カニューレの遠位端が自由空間にある間に、種々のポンプ速度で流体を送り込んでいる間に測定された背圧値を提示するものであることを特徴とする請求項12に記載の方法。
- 前記初期BAPVの監視値が前記所定の初期BAPV範囲外になったとき、前記ポンプによって生じた種々の流量における、フロー圧力センサーと自由空間中の流入ラインの端部との間の背圧値を含む代替的背圧テーブルにアクセスするステップをさらに具備することを特徴とする請求項12に記載の方法。
- (g)前記代替的背圧テーブルから選択された背圧値を前記測定圧力値に加算することにより、代替的背圧調整圧力値(BAPV)を計算するステップと、
(h)前記代替的BAPVを前記圧力設定値に維持するためにポンプ速度を制御するステップと、
をさらに具備することを特徴とする請求項14に記載の方法。 - (k)前記初期BAPVを監視して、前記初期BAPVが前記所定の初期BAPV範囲内の値に戻ったかどうかを判定するステップと
(l)前記初期BAPVが前記初期BAPV範囲内の値に戻ったときに、前記初期BAPVを前記圧力設定値に維持するようポンプ速度を制御するステップと、
をさらに具備することを特徴とする請求項15に記載の方法。 - 前記初期BAPVが前記初期BAPV範囲外になった後、流入ライン及び置換カニューレに接続されたときに前記ポンプによって生じる複数の流量で背圧を測定することによって生成される置換背圧テーブルにアクセスするステップをさらに具備することを特徴とする請求項12に記載の方法。
- (g)前記置換背圧テーブルから選択された背圧値を前記測定圧力値に加算することにより置換BAPVを計算するステップと、
(h)前記置換BAPVを前記圧力設定値に維持するためにポンプ速度を制御するステップと、
をさらに具備することを特徴とする請求項17に記載の方法。 - 前記手術部位は関節腔であることを特徴とする請求項12に記載の方法。
- コントローラと、手術空間に流体を流すための流体の流入ラインに動作可能に接続されたポンプシステムとを含む流体管理システムと、
前記手術空間を設定圧力に維持するために、前記流入ライン内の圧力センサーにリンクされた圧力制御ループを具備する第1のコントローラアルゴリズムと、
閾値圧力に対する前記流入ライン内の背圧の変化を検出するための第2のコントローラアルゴリズムと、
を具備し、
背圧の変化の検出に対応する前記コントローラは、前記ポンプシステムを調整して、前記手術空間内への流体の流入の速度を低下させることを特徴とする、
組織を治療するシステム。
