p38MAPK阻害剤とHIF−1α安定化剤との任意の組み合わせ、例えば、本明細書に記載のものは、エクスビボまたはインビトロ培養で幹細胞(例えば、造血幹細胞)の幹細胞性を保存するために使用することができる。幹細胞性とは、未分化細胞が未分化細胞として再生するが、依然として特定のタイプの細胞を産生するように分化するこの能力を保持する能力を指す。幹細胞能、または幹細胞(例えば、造血幹細胞)の「幹細胞性」は、特性:静止、再増殖能、自己再生能、および多系統分化能の組み合わせに依存する。幹細胞(例えば、HSC)における細胞周期静止は、分化または老化から細胞を保護することによって幹細胞性を維持する。
本明細書に記載のエクスビボ培養方法によって調製された幹細胞は、例えば、神経変性疾患および状態、糖尿病、心臓疾患、他の状態を含む、疾患または状態を治療または予防するための幹細胞の使用である、幹細胞療法において使用することができる。幹細胞療法によって治療される適切な状態の例には、急性骨髄性白血病(AML)、慢性骨髄性白血病(CML)、急性リンパ芽球性白血病(ALL)、ホジキンリンパ腫、非ホジキンリンパ腫、神経芽細胞腫、ユーイング肉腫、骨髄異形成症候群、神経膠腫、および他の固形腫瘍が含まれるが、これらに限定されない。幹細胞療法は、サラセミア、再生不良性貧血、ファンコニ貧血、免疫不全症候群、または先天性代謝異常などの非悪性状態にも適用することができる。いくつかの実施形態では、本明細書に記載のエクスビボ培養方法によって調製されたHSCは、急性骨髄性白血病(AML)、慢性骨髄性白血病(CML)、急性リンパ芽球性白血病(ALL)、慢性リンパ性白血病(CLL)、若年性骨髄単球性白血病、ホジキンリンパ腫、および非ホジキンリンパ腫を含むがこれらに限定されない、造血障害の治療における移植に使用することができる。
本明細書に記載のエクスビボ培養方法のいずれかによって調製されたヒトHSCなどのヒト造血幹細胞(HSC)の生着を評価するために、HSCを、HSC移植を必要とするヒト対象からを入手または誘導し、免疫不全マウス(例えば、NSGマウス)などの適切な免疫不全動物に移植することができる。他の適切な免疫不全動物、例えば、Charles River Laboratoriesによって提供されるものもまた当該技術分野で知られている(以下の表2を参照されたい)。

概要
遺伝子療法(GT)は、適切な移植ドナーがいない単一遺伝子障害を有する患者を治癒するための同種造血幹細胞移植(HSCT)の魅力的な代替である1、2、3−15。その成功は、遺伝子修飾造血幹細胞(HSC)が、同時に自己再生して生涯にわたる血球産生を維持しながら、造血前駆細胞(HPC)に分化して造血系を再生する能力に依存する16−18。GTのために、稀なHSC(CD34+38−90+45RA−49f+)19を含有するCD34+造血幹細胞および前駆細胞(HSPC)を、HSC分裂を強制するサイトカイン富化培地において2〜4日間培養し、遺伝子操作20による影響を受けやすくし、次いで移植前化学療法前処置後に移植する。
エクスビボHSC分裂および遺伝子導入は、LTRPの喪失および骨髄性歪み
遺伝子修飾子孫をもたらす。
NOD.Cg−Prkdcscid Il2rgtm1Wjl/SzJ(NSG)マウス異種移植片モデルをまず成体(BM/MPB由来)HSCモデルに適合させた。100万個のMPB CD34+細胞を致死照射NSGマウスに移植し、骨髄(BM)における連続生着を6、12、および24週で評価した(循環ヒト細胞はBMにおける生着と相関しない、図1、パネルAおよびB)。B−、T−、および骨髄性細胞からなる多能性移植片は、1:1二次移植片(2T)が成功したヒト二次生着(>0.01%ヒトCD45+細胞)をもたらした場合のみ、24週までに明らかであった(図1、パネルCおよびD)。多能性生着を一次(1T)マウスにおいて24週で、LTRPを2Tマウスにおいて1.5〜3ヶ月で評価した(図2)。
次に、p38阻害が培養および形質導入HSCのLTRPを保持することができるメカニズムを調査した。p38iは、CD34+細胞の総数、またはインビトロでの生存率に影響を及ぼさず、インビトロまたはインビボでの遺伝子導入効率にも影響を及ぼさなかった(図11、パネルA〜C)。HSC区画はまた、p38阻害でアポトーシス、形質導入効率、またはROSレベルにおいて差を示さず、表現型HSC集団の増加は、p38iありまたはなしで、長期培養を除いて、表現型HSCがp38iでより高かった場合に同様であった(図12)。
形質導入および未形質導入非周期および周期HSCのS−G2M動態は、周期HSCにおいてDSBが起こった場合(培養2日後のRV形質導入)、SおよびG2M後期にHSCの蓄積があったことを示した(図14、パネルC)。しかしながら、最初の24時間以内に(LVで)ベクター組み込みが起こった場合、この現象は見られなかった(図14、パネルD)。さらに、この効果はRVに特異的ではなかったが、形質導入された場合HSCの細胞周期に特異的であった。同じドナーからのCD34+細胞を約2日間(44時間)培養し、44時間および68時間にLV(LV後期またはLVL)、またはRVのいずれかで形質導入するか、またはHSCをLVで24時間以内に形質導入したが72時間(LVE)培養し続けた(図17、パネルA)。SおよびG2M後期蓄積は、LVLまたはRV群における形質導入(GFP+)周期HSCにおいてのみ観察された(図14、パネルEおよびF)。LVは周期および非周期の両方のHSCを形質導入するので、G2M蓄積はRVよりもLVLにおいて少なく、したがってGFP+集団は混合集団で構成された。特に、このG2M蓄積はアポトーシスをもたらさず(むしろ、HSC数は増加し、図7、パネルCおよび図12、パネルC)、連続的に続く場合、HSCはG2M期から移行した(図14、パネルE)。このG2M蓄積は、非周期HSCを形質導入した場合(24時間LVまたはLVE、図14、パネルE)、細胞周期分析をLVE対RVでの形質導入後の同じ時点で行った場合(図14、パネルCおよびD)にも見られなかった。
次に、NSGマウスにおけるp38iおよび/またはPGE2の併用処理での限界希釈移植を行った。CD34+細胞を一晩培養し、次いで12時間あけて(18時間および30時間で)、42時間の総培養期間にわたってLVで形質導入し、以前の実験で使用された半分の用量(500KのCD34+/マウス)から開始して、CD34+細胞の制限用量でNSGマウスに移植した。RV形質導入細胞と同様に、500Kおよび250Kの細胞群において骨髄性歪みおよびB−リンパ性能の喪失が6ヶ月で観察された(図20、パネルA〜D)。注目すべきことに、生着HSCに重度の再生ストレスを課す、さらなる制限HSC用量で、未形質導入CD34+細胞においても骨髄性歪みおよびBリンパ性能の喪失が観察され、過剰なHSC増殖が骨髄性バイアス子孫を誘導することを示した。p38i単独での処理は、より高いHSPC投与量で未形質導入子孫における系統バランスを回復するのに十分であったが、(初期の実験における高HSC用量で観察されたように[図9])形質導入HSC子孫に対する効果は少なかった。PGE2単独は系統歪みを回復させるのに十分であったが、過剰な周期および形質導入ストレスの組み合わせが存在した場合、最低細胞用量で、PGE2およびp38iの組み合わせは、PGE2単独よりも骨髄性歪みを逆転させ、Bリンパ性能を回復させるのに最も効果的であった(図20、パネルA〜D)。また、1T後24週での骨髄における全ヒト生着は、より高いCD34+用量群においてp38iおよびPGE2の組み合わせでより高い生着レベルを明らかにし、そこでは生着を有意に分析することができた(図20、パネルE)。さらに、6ヶ月のBMにおける競合的再増殖単位(CRU)は、p38iまたはPGE2での単回処理と比較して、併用処理でほぼ5倍向上した(図20、パネルF)。最後に、3ヶ月の2TマウスにおけるLTRPの評価は、LTRPおよび骨髄性歪み表現型の逆転の両方の増加について、p38iおよびPGE2の併用処理の効果を示した(図20、パネルG、I〜L)。p38iもしくはPGE2または組み合わせの使用は、長期二次ヒト移植片においても、GFPマーキングにおけるいかなる効果も有さなかったことに留意されたい(図20、パネルH)。
本明細書に提示されているのは、HSCのインビトロ遺伝子操作がHSCの喪失および運命の変化をもたらす、重要かつ異なるメカニズムである。p38ストレスシグナル伝達は、ROSによって誘導されることが示されている37。この研究では、p38活性化がDDRを増加させ、これがHSCのLTRPを低減することが示されている。本研究はまた、休止から出たHSCおけるDDR増加のメカニズムおよびインビボマウスHSCにおいて結果として見られるHSC消耗を説明し69、DDR応答が遺伝子導入媒介DSBによってさらに誇張され、これはp38阻害でも低減することを示す。
ヒトCD34+細胞単離、培養、および形質導入
新鮮なG−CSF動員末梢血細胞を、IRB承認プロトコルを用いて(全対象からインフォームドコンセントを得て)健常なボランティアからアフェレーシスによって収集し、以前21記載されたようにCliniMac(Miltenyi Biotech、ドイツ)またはIndirect CD34Micro Bead Kit、ヒト(Miltenyi Biotech,Inc.Bisley、ドイツ)を用いて95%より高い純度まで正の選択に供した。新鮮なまたは凍結保存されたCD34+をエクスビボ培養および形質導入のために新たに使用した。対照として、新たに単離されたCD34+細胞を直ちにNSGマウスに移植した。いくつかの実験では、正常骨髄ドナー由来のCD34+細胞を購入した。

レンチウイルスベクター(LV)pRRL.SIN.cPPT.MNDU3.eGFP.WPRE86は、MNDU3プロモーターの制御下で向上された緑色蛍光タンパク質(GFP)をコードする。ベクターは、VSV−Gエンベロープを用いてパッケージングされた。レトロウイルスベクター(RV)pSRS11.EFS.GFP.PRE(GALV)87は、短いEF1α(EFS)プロモーターの制御下でGFPをコードし、GALVエンベロープを用いてパッケージングされた。
ヒトCD34(BD Biosciences560710または555824)、CD38(eBiosciences、San Diego、CA47−0389−42)、CD90(BioLegend328120)、CD45RA(eBiosciences17−0458−42)、CD49f(BD Biosciences555736)、CD45(BD55485またはBiolegend304026)、CD33(eBiosciences25−0338−42)、CD19(BD Biosciences557791)、またはCD3(BD Biosciences555340)に対するPE−Cyanine7またはAPC−eFluor780、APCまたはPE複合抗体。細胞(約5×105〜1×106)を、適切なアイソタイプコントロールと製造業者の指示通りに抗体で標識するために使用した。ホスホフローについて、一次抗体ホスホ−p38(Thr180/Tyr182、4092S)、ホスホ−p44/42MAPK(ERK1/2)(Thr202/Tyr204、4094S)、およびホスホ−JNK(Thr183−Tyr185PE複合体、575 5S)(全てCell Signaling製)を一晩染色に使用し、続いて洗浄およびヤギ抗ウサギIgG(H+L)またはヤギ抗ウサギIgG(H+L)またはPacific Blue(P−10994)抗体を含む二次抗体染色を行った。製造業者の説明書に従って、PEアネキシンVアポトーシス検出キット(BD Biosciences559763)を使用してアポトーシスアッセイを実施した。総ROSおよびミトコンドリア特異的ROSを、製造業者の指示に従って、それぞれ10μMのCM−H2DCFDA(ThermoFisher Scientific C6827)および5μMのMitoSOX Red(ThermoFisher Scientific M36008)を用いて検出した。
FACS選別CD34+38−90+細胞を2%パラホルムアルデヒドで固定し、0.1%Triton X−100(Sigma−Aldrich)を用いて透過処理した。細胞を次いで、一次抗体抗ホスホH2AX(Ser139)(Millipore、05−636)、抗53BP1抗体(Novus Biologicals、NB100−904)、または抗HIF−1α(ab103063)で染色した。二次抗体について、Alexa Fluor594複合ヤギ抗ウサギIgG(H+L)(A−11037)またはAlexa Fluor647複合ヤギ抗ウサギIgG(H+L)(A−21245)(共にThermoFisher Scientific製)を37℃で1時間使用した。細胞を対比染色し、DAPIを含むVectaShield褪色防止封入剤(H−1200Vector Laboratories)で封入した。画像を、Nikon90i直立顕微鏡を用いて得た。
CD45sgRNAを、GeneArt(商標)Precision gRNA Synthesis Kitを用いてアセンブリPCRおよびインビトロ転写により合成した。sgRNA試料の品質を、それを10%Novex(商標)TBE−尿素ゲル上で泳動することにより決定し、100塩基での離散バンドが無傷のsgRNAを示した。Cas9緩衝液を、20mMのHEPES pH7.5、150mMのKCl、1mMのMgCl2、10%グリセロール、および1mMのTCEPで調製した。解凍され、44時間予備刺激されたヒトMPB由来CD34+細胞をCas9RNPでのヌクレオフェクションに供した。Cas9RNPを、以前に記載されたように、CD34+細胞のヌクレオフェクション直前に組み立てた29。20μLの細胞懸濁液(150,000〜200,000細胞)をCas9RNPでヌクレオフェクトするために、Cas9緩衝液中に120pmolのsgRNAを含有する5μLの溶液を調製した。Cas9緩衝液中に100pmolのCas9タンパク質を含有する5μLの溶液を調製し、約30秒間かけてゆっくりとsgRNA溶液に添加し、室温で20分間インキュベートした。各ヌクレオフェクションについて、150,000〜200,000のCD34+細胞を20μLのP3溶液(Lonza)に再懸濁し、10μLのCas9RNPと混合した。この混合物を次いで、E0100プログラムを用いてLonza4Dヌクレオフェクターを用いてヌクレオフェクトした。ヌクレオフェクトされた細胞を(上記のようにヒトサイトカインを補充した)新鮮な培地に回収し、さらなる時間37℃で培養した。細胞を、細胞周期染色のためにヌクレオフェクション後6および24時間で採取した。残りの細胞を、ジェノタイピングおよびフローサイトメトリーのためのCD45染色の前にさらに5〜8日間培養した。
1つの実験において、p38iでの24時間および72時間の処理ありならびに処理なしでのFACS選別培養および形質導入CD34+38−90+45RA−49f+HSCからのRNAについてRNAseq分析を行った。qRT PCRのために、培養および形質導入された細胞を採取し、FACS Ariaセルソーター(BD)を用いてCD34+CD38−CD90+マーカーについて直接TRI試薬(Molecular Research Center,Inc.)に選別した。クロロホルムによる相分離およびイソプロパノールによる沈殿で全RNAを精製した。SuperScript IV VILO Master Mix(ThermoFisher Scientific11756050)を用いてcDNAを生成した。以下:FZD3Hs00184043_m1、WNT5B Hs01086864_m1、p34CDC2Hs00938777_m1(ABI/ThermoFisher、4331182)のようなプライマー/プローブセットを使用してqPCRによりcDNAを増幅し、ABI7900高速リアルタイムPCRシステム上で分析した。qPCR設定に使用したマスターミックスは、iTaq Universal Probes Supermix(BioRad1725134)であった。遺伝子発現レベルは、ヒトグリセルアルデヒド−3−リン酸デヒドロゲナーゼ(GAPDH)(ABI/ThermoFisher)に対して正規化したΔΔCt法を用いて計算した。
データは、平均±標準誤差(SEM)として表す。群に応じて、GraphPad Prism(V.7)ソフトウェアを使用して、図の凡例に示されるように、両側マン・ホイットニーU検定、対応のあるスチューデントt検定、またはウィルコクソン検定、ログランク検定を用いることによってデータを分析した。二群間の対比較を行ったので、多重比較検定は用いなかった。P値≦0.05を有意とみなした。

Claims (18)
- 向上した生着活性を有する造血幹細胞(HSC)を調製するための方法であって、
(i)HSCを提供することと、
(ii)有効量のp38マイトジェン活性化タンパク質キナーゼ(MAPK)阻害剤および有効量の低酸素誘導因子−1α(HIF−1α)安定化剤の存在下で前記HSCを培養することと、
を含む、方法。 - 前記HSCが、DNA二本鎖切断を誘導する遺伝子操作を受けている、請求項1に記載の方法。
- 前記遺伝子操作が、組み込みベクターの形質導入を含む、請求項1または2に記載の方法。
- 前記組み込みベクターが、ウイルスベクターである、請求項3に記載の方法。
- 前記ウイルスベクターが、レトロウイルスベクターまたはレンチウイルスベクターである、請求項4に記載の方法。
- 前記遺伝子操作が、ゲノム編集を含む、請求項1または2に記載の方法。
- 前記HSCが、分裂HSCである、請求項1〜6のいずれか一項に記載の方法。
- 前記HSCが、対象から得られる、請求項1〜7のいずれか一項に記載の方法。
- 前記対象が、ヒト対象である、請求項8に記載の方法。
- 前記HSCが、前記ヒト対象の骨髄または末梢血細胞から得られる成体HSCである、請求項9に記載の方法。
- 前記HSCが、前記ヒト対象の臍帯血細胞から得られる、請求項9に記載の方法。
- 前記MAPK阻害剤が、ドラマピモド、ラリメチニブ、p38MAPKのアミノピリジンベースのATP競合阻害剤、またはピリジニルイミダゾール阻害剤である、請求項1〜11のいずれか一項に記載の方法。
- 前記HIF−1α安定化剤が、プロスタグランジンE2(PGE2)または16−16ジメチルプロスタグランジンE2(dmPGE2)である、請求項1〜12のいずれか一項に記載の方法。
- (iii)それを必要とする対象に、ステップ(ii)から得られたHSCを投与することをさらに含む、請求項1〜13のいずれか一項に記載の方法。
- 約50,000〜約500,000個のHSCの用量が、前記対象に投与される、請求項14に記載の方法。
- 約50,000〜約100,000個のHSCの用量が、前記対象に投与される、請求項15に記載の方法。
- 前記対象が、前記HSCが得られた同じ対象である、請求項14〜16のいずれか一項に記載の方法。
- ステップ(ii)が、1〜7日間行われる、請求項1〜17のいずれか一項に記載の方法。
