发明背景
造血系统是不同成熟细胞谱系的细胞的复杂层次结构。这些包括针对病原体提供保护的免疫系统细胞、携带氧气贯穿身体的细胞和参与伤口愈合的细胞。所有这些成熟细胞源自能够自我更新并分化为任何血细胞谱系的造血干细胞(HSC)库。造血干细胞具有补充整个造血系统的能力。

发明概述
令人惊讶地,发明人发现了环孢菌素H(CsH)在改善包括造血干细胞和祖细胞在内的细胞的转导中比环孢菌素A表现得好得多。CsH在中等感染复数(multiplicity ofinfection)10次时单次慢病毒载体应用(“命中”)后提供了几乎100%的转导效率和每个基因组多达3个载体拷贝。这些发现是在临床相关的动员(mobilised)的外周血衍生的造血干细胞中产生的,并且观察到CsH不对细胞的生物学特性产生有害影响。

一方面,本发明提供了环孢菌素H(CsH)或其衍生物用于增加通过病毒载体对分离的细胞群体的转导效率和/或增加在通过病毒载体转导时分离的细胞群体的基因编辑效率的用途。
增加转导效率是指与在不存在试剂的情况下但在其它方面基本上相同的条件下所实现的转导相比,在存在试剂(例如CsH或其衍生物)的情况下细胞(例如造血干细胞和/或祖细胞;或T细胞)的转导增加。因此,增加的转导效率可以减少感染的复数(MOI)和/或减少实现有效转导所需的转导时间。
在一个实施方案中,增加由载体转导的细胞的百分比。在另一个实施方案中,增加每个细胞的载体拷贝数。优选地,两者同时实现。
确定由载体转导的细胞的百分比的方法是本领域已知的。合适的方法包括流式细胞仪、荧光激活细胞分选(FACS)和荧光显微术。所采用的技术优选地是一种适于自动化和/或高通量筛选的技术。
例如,可用携带报告基因的载体转导细胞群体。可以构建载体以使得当载体转导细胞时表达报告基因。合适的报告基因包括编码荧光蛋白的基因,例如绿色、黄色、樱桃色、青色或橙色荧光蛋白。一旦载体转导了细胞群体,就可以使用合适的技术例如FACS来定量表达和不表达报告基因的细胞数量。然后可以计算载体转导的细胞的百分比。

环孢菌素H
环孢菌素H(CsH,CAS号:83602-39-5)是具有以下结构的环状十一肽:

星状孢子素是最初自Streptomyces staurosporeus分离的天然产物。它通过防止ATP与激酶的结合,显示出作为蛋白激酶抑制剂的活性。
造血干细胞和祖细胞
干细胞能够分化为多种细胞类型。能够分化为所有细胞类型的细胞称为全能。在哺乳动物中,只有合子和早期胚胎细胞是全能的。在大多数(如果不是全部)多细胞生物中发现了干细胞。它们的特征是通过有丝分裂细胞分裂来更新自身并分化为极其多种专门细胞类型的能力。哺乳动物干细胞的两种主要类型是从胚泡的内部细胞团中分离出来的胚胎干细胞,以及在成人组织中发现的成人干细胞。在发育中的胚胎中,干细胞可以分化为所有专门的胚胎组织。在成年生物体中,干细胞和祖细胞可作为人体的修复系统起作用,补充专门的细胞,而且还可以维持诸如血液、皮肤或肠道组织的再生器官的正常运转。
造血干细胞(HSC)是多能干细胞,其可在例如外周血、骨髓和脐带血中发现。HSC能够自我更新并分化为任何血细胞谱系。它们能够重新定殖整个免疫系统,以及所有造血组织(如骨髓、脾脏和胸腺)中的红系和髓样谱系。它们提供了造血细胞所有谱系的终生产生。
造血祖细胞具有分化为特定类型细胞的能力。然而,与干细胞相比,它们已经更加特异:它们被推动分化为其“靶”细胞。干细胞和祖细胞之间的区别在于,干细胞可以无限复制,而祖细胞只能分裂有限的次数。造血祖细胞只能通过功能性体内测定(即移植并证明它们是否能长时间产生所有血液谱系)与HSC严格区分开。
本发明的造血干细胞和祖细胞包含CD34细胞表面标志物(表示为CD34+)。
造血干细胞和祖细胞(HSPC)来源
造血干细胞和/或祖细胞群体可以从组织样品中获得。
例如,可以从外周血(例如成人和胎儿外周血)、脐带血、骨髓、肝或脾获得造血干细胞和/或祖细胞群体。优选地,这些细胞获自外周血或骨髓。它们可以通过生长因子处理在体内动员细胞后获得。
可以使用例如G-CSF、plerixaphor或其组合来进行动员。其他试剂,例如NSAID和二肽基肽酶抑制剂也可用作动员剂。
随着干细胞生长因子GM-CSF和G-CSF的可用性,使用从外周血而不是从骨髓中收集的干细胞进行现在大多数造血干细胞移植程序。收集外周血干细胞提供更大的植入物,无需对供体进行全身麻醉以收集植入物,导致更短的植入前时间,并且可以提供更低的长期复发率。
骨髓可通过标准抽吸方法(稳态或动员后)或提供使用下一代收获工具(例如Marrow Miner)进行收集。
另外,造血干细胞和祖细胞也可以源自诱导的多能干细胞。
HSC特征
通过流式细胞术程序,HSC通常具有低前向散射和侧向散射概况。如通过允许测定线粒体活性的若丹明标记证明,一些是代谢静止的。HSC可包含某些细胞表面标志物,例如CD34、CD45、CD133、CD90和CD49f。它们也可以定义为缺乏CD38和CD45RA细胞表面标志物表达的细胞。然而,这些中的一些标志物的表达取决于HSC的发育阶段和组织特异性情况。如通过流式细胞仪检测,某些称为“侧群细胞”的HSC排除Hoechst 33342染料。因此,HSC具有允许其鉴定和分离的描述性特征。
阴性标志物
CD38是人HSC的最成熟且有用的单阴性标志物。
人HSC对于谱系标志物如CD2、CD3、CD14、CD16、CD19、CD20、CD24、CD36、CD56、CD66b、CD271和CD45RA也可以呈阴性。然而,这些标志物可以需要组合使用以富集HSC。
通过“阴性标志物”应理解为人HSC缺乏这些标志物的表达。
阳性标志物
CD34和CD133是HSC的最有用的阳性标志物。
一些HSC对于谱系标志物如CD90、CD49f和CD93也呈阳性。然而,这些标志物可以需要结合使用以富集HSC。
通过“阳性标志物”,应理解人HSC表达这些标志物。
在一个实施方案中,造血干细胞和祖细胞是CD34+CD38-细胞。
分化细胞
分化细胞是与干细胞或祖细胞相比更加特化的细胞。分化发生在多细胞生物体的发育过程中,因为该生物体从单一合子转变为组织和细胞类型的复杂系统。分化也是成年人的常见过程:成年干细胞在组织修复和正常细胞周转过程中分裂并创建完全分化的后代细胞。分化显著改变细胞的大小、形状、膜电位、代谢活性和对信号的响应性。这些变化很大程度上是由于基因表达中的高度控制的修饰。换言之,分化细胞是具有特定结构并由于涉及特定基因的激活和失活的发育过程而执行某些功能的细胞。在此,分化细胞包括造血谱系的分化细胞,例如单核细胞,巨噬细胞,嗜中性粒细胞,嗜碱性粒细胞,嗜酸性粒细胞,红细胞,巨核细胞/血小板,树突细胞,T细胞,B细胞和NK细胞。例如,通过检测在未分化细胞上不表达或表达程度较低的细胞表面分子,可以将造血谱系的分化细胞与干细胞和祖细胞区分开。合适的人谱系标志物的例子包括CD33,CD13,CD14,CD15(髓样),CD19,CD20,CD22,CD79a(B),CD36,CD71,CD235a(红系),CD2,CD3,CD4,CD8(T)and CD56(NK)。
T细胞
T细胞(或T淋巴细胞)是在细胞介导的免疫中起关键作用的一类淋巴细胞。通过在细胞表面上T细胞受体(TCR)的存在,可以将它们与其他淋巴细胞(例如B细胞和自然杀伤细胞(NK细胞))区分开。
在一个实施方案中,T细胞是CD4+T细胞。在另一个实施方案中,T细胞是CD3+T细胞。
细胞群体的分离和富集
如本文所用,术语细胞的“分离的群体”可以指先前已经从体内取出的细胞群体。分离的细胞群体可以使用本领域已知的标准技术离体或体外培养和操作。分离的细胞群体可随后重新引入受试者。所述受试者可以是最初从其分离细胞的相同受试者或不同的受试者。

结合细胞标志物的试剂可以是抗体,例如抗CD34或抗CD38抗体。
术语“抗体”是指能够结合所选靶标的完整抗体或抗体片段,包括Fv、ScFv、F(ab')和F(ab')2、单克隆和多克隆抗体、工程化抗体,包括嵌合抗体、CDR植入的和人源化的抗体,以及使用噬菌体展示或备选技术产生的人工选择的抗体。
另外,经典抗体的备选也可以用于本发明中,例如“avibodies”、“avimers”、“anticalins”、“纳米抗体(nanobodies)”和“DARPins”。

标记物可以允许例如从其环境中纯化和/或检测标记的试剂和与其结合的任何细胞(例如,可以用磁珠或亲和标签(例如抗生物素蛋白)标记试剂)。适用作标记物的可检测标志物包括荧光团(例如绿色、樱桃色、青色和橙色荧光蛋白)和肽标签(例如His标签、Myc标签、FLAG标签和HA标签)。
分离表达特定标志物的细胞群体的许多技术是本领域已知的。这些方法包括基于磁珠的分离技术(例如,基于磁珠的闭合回路分离)、流式细胞仪、荧光激活细胞分选(FACS)、亲和标签纯化(例如,使用亲和柱或珠,例如生物素柱来分离抗生物素蛋白标记的试剂)和基于显微术的技术。
也可以使用不同技术的组合来进行分离,例如基于磁珠的分离步骤,然后通过流式细胞术针对一种或多种其他(阳性或阴性)标志物对所得细胞群体进行分类。
例如,可以使用系统(Miltenyi)进行临床等级分离。这是基于磁珠的闭合回路分离技术的一个例子。
还包括染料排斥特性(例如,侧群或若丹明标记)或酶活性(例如,ALDH活性)可用于富集造血干细胞。
基因编辑
术语“基因编辑”是指其中在细胞中插入、失或替换核酸的遗传工程类型。可以使用工程化核酸酶实现基因编辑,所述工程化核酸酶可以靶向多核苷酸(例如基因组)中的期望位点。此类核酸酶可在期望位置处产生位点特异性双链断裂,然后可通过非同源末端连接(NHEJ)或同源重组(HR)进行修复,从而导致靶向突变。

本领域已知的合适核酸酶的实例包括锌指核酸酶(ZFN)、转录活化物样效应器核酸酶(TALEN)以及成簇规则间隔短回文重复序列(CRISPR)/Cas系统(Gaj,T.et al.(2013)Trends Biotechnol.31:397-405;Sander,J.D.et al.(2014)Nat.Biotechnol.32:347-55)。
大范围核酸酶(Silve,G.et al.(2011)Cur.Gene Ther.11:11-27)也可以用作合适的核酸酶用于基因编辑。

在一个实施方案中,病毒载体是逆转录病毒载体。在一个优选的实施方案中,病毒载体是慢病毒载体。
在一个实施方案中,慢病毒载体源自HIV-1、HIV-2、SIV、FIV、BIV、EIAV、CAEV或维斯那慢病毒。
在一个实施方案中,病毒载体是γ-逆转录病毒载体。
通过“载体源自”某种类型的病毒,应理解,载体包含至少一种可源自该类型的病毒的组成部分。
逆转录病毒和慢病毒载体
逆转录病毒载体可以源自或可源自任何合适的逆转录病毒。已经鉴定出许多不同的逆转录病毒。例子包括鼠白血病病毒(MLV)、人T细胞白血病病毒(HTLV)、小鼠乳腺肿瘤病毒(MMTV)、劳斯肉瘤病毒(RSV)、藤浪(Fujinami)肉瘤病毒(FuSV)、莫洛尼鼠白血病病毒(Mo-MLV)、FBR鼠骨肉瘤病毒(FBR MSV)、莫洛尼鼠肉瘤病毒(Mo-MSV)、Abelson鼠白血病病毒(A-MLV)、禽髓细胞瘤病(myelocytomatosis)病毒29(MC29)和禽成红细胞增多病病毒(AEV)。逆转录病毒的详细列表可以参见Coffin,J.M.et al.(1997)Retroviruses,ColdSpring Harbour Laboratory Press,758-63。

在缺陷型逆转录病毒载体基因组中,gag、pol和env可以是不存在或非功能性的。
在典型的逆转录病毒载体中,可以从病毒中除去复制必需的一个或多个蛋白质编码区的至少一部分。这使得病毒载体变为复制缺陷。病毒基因组的部分也可以用可操作连接到载体基因组中的调节控制区和报告部分的编码候选调节部分的文库替换,以产生包含候选调节部分的载体,该载体能够转导靶宿主细胞和/或将其基因组整合到宿主基因组中。
慢病毒载体是大量逆转录病毒载体的一部分。慢病毒的详细列表可以参见Coffin,J.M.et al.(1997)Retroviruses,Cold Spring Harbour Laboratory Press,758-63。简而言之,慢病毒可以分为灵长类和非灵长类组。灵长类慢病毒的实例包括但不限于人免疫缺陷病毒(HIV)、人获得性免疫缺陷综合症(AIDS)的致病原;和猿免疫缺陷病毒(SIV)。非灵长类慢病毒的例子包括原型“慢病毒”visna/maedi病毒(VMV),以及相关的山羊关节炎-脑炎病毒(CAEV)、马传染性贫血病毒(EIAV)和最近描述的猫免疫缺陷病毒(FIV)和牛免疫缺陷病毒(BIV)。
慢病毒家族与逆转录病毒家族的不同之处在于慢病毒具有感染分裂细胞和非分裂细胞两者的能力(Lewis,P et al.(1992)EMBO J.11:3053-8;Lewis,P.F.et al.(1994)J.Virol.68:510-6)。相反,其他逆转录病毒(例如MLV)不能感染非分裂或缓慢分裂的细胞,例如组成肌肉、脑、肺和肝组织的细胞。
如本文所用,慢病毒载体是包含至少一个可源自慢病毒的组成部分的载体。优选地,该组成部分参与载体感染细胞,表达基因或被复制的生物学机制。
慢病毒载体可以是“灵长类”载体。慢病毒载体可以是“非灵长类”载体(即源自主要不感染灵长类,特别是人的病毒)。非灵长类慢病毒的实例可以是慢病毒科的任何不天然感染灵长类的成员。
作为基于慢病毒的载体的实例,下面描述基于HIV-1和HIV-2的载体。
HIV-1载体含有在简单逆转录病毒中也发现的顺式作用元件。已经显示了延伸到gag可读框中的序列对于包装HIV-1是重要的。因此,HIV-1载体通常包含gag的相关部分,在该部分中翻译起始密码子已发生突变。此外,大多数HIV-1载体还含有env基因的一部分,其包括RRE。Rev结合RRE,所述RRE允许全长或单剪接的mRNA从细胞核转运至细胞质。在没有Rev和/或RRE的情况下,全长HIV-1RNA积累在细胞核中。或者,可以使用来自某些简单的逆转录病毒(例如Mason-Pfizer猴病毒)的组成性转运元件来缓解对Rev和RRE的需求。从HIV-1LTR启动子进行有效转录需要病毒蛋白Tat。
大多数基于HIV-2的载体在结构上与HIV-1载体非常相似。与基于HIV-1的载体相似,HIV-2载体也需要RRE以有效转运全长或单剪接的病毒RNA。
在一个系统中,载体和辅助构建体来自两种不同的病毒,并且降低的核苷酸同源性可以降低重组的可能性。除了基于灵长类慢病毒的载体外,还开发了基于FIV的载体作为源自病原性HIV-1基因组的载体的备选。这些载体的结构也类似于基于HIV-1的载体。
优选地,用于本发明的病毒载体具有最小的病毒基因组。
通过“最小病毒基因组”,应当理解,病毒载体已被操纵以便除去非必需元件并保留必要元件,以提供对靶宿主细胞进行感染、转导和递送感兴趣的核苷酸序列所需要的功能性。此策略的更多详情可以参见WO 1998/017815。
优选地,用于在宿主细胞/包装细胞内产生病毒基因组的质粒载体会具有足够的慢病毒遗传信息以允许将RNA基因组在存在包装组分的情况下包装成病毒颗粒,所述病毒颗粒能够感染靶细胞但不能独立复制以在最终的靶细胞内产生感染性病毒颗粒。优选地,载体缺乏功能性gag-pol和/或env基因和/或复制必需的其它基因。
然而,用于在宿主细胞/包装细胞内产生病毒基因组的质粒载体还将包括与慢病毒基因组可操作连接以指导宿主细胞/包装细胞中基因组转录的转录调节序列。这些调节序列可以是与转录的病毒序列相关的天然序列(即5'U3区),或者它们可以是异源启动子,例如另一种病毒启动子(例如CMV启动子)。
载体可以是自灭活(SIN)载体,其中病毒增强子和启动子序列已被删除。可以产生SIN载体,并且在体内以野生型载体的功效相似的功效转导非分裂细胞。SIN原病毒中的长末端重复序列(LTR)的转录失活应当阻止具有复制能力的病毒的动员。这还应当通过消除LTR的任何顺式作用来实现自内部启动子调节的基因表达。
载体可能是整合缺陷的。整合缺陷型慢病毒载体(IDLV)可以通过如下产生:例如,通过将载体与无催化活性的整合酶包装在一起(例如在催化位点中带有D64V突变的HIV整合酶;Naldini,L.et al.(1996)Science 272:263-7;Naldini,L.et al.(1996)Proc.Natl.Acad.Sci.USA 93:11382-8;Leavitt,A.D.et al.(1996)J.Virol.70:721-8)或通过修饰或缺失载体来自LTR的必需att序列(Nightingale,S.J.et al.(2006)Mol.Ther.13:1121-32),或通过上述项的组合。
HIV衍生载体
就HIV毒株而言,用于本发明的HIV衍生载体没有特别限制。可以在HIV序列数据库(http://www.hiv.lanl.gov/content/index)上找到许多HIV毒株序列的例子。
例如,HIV-1衍生载体可以源自HIV-1毒株NL4-3、IIIB_LAI或HXB2_LAI(亲X4)或BAL(亲R5)或其嵌合物中的任一种。优选地,HIV-1衍生载体源自pMDLg/pRRE Gag-Pol表达性包装构建体(US 7629153;US 8652837;Naldini,L.et al.(1996)Science 272:263-7;Follenzi,A.et al.(2002)Methods Enzymol.346:454-65)。

本发明中使用的载体优选包含感兴趣的核苷酸。
优选地,感兴趣的核苷酸产生治疗作用。
合适的NOI包括但不限于编码酶、细胞因子、趋化因子、激素、抗体、抗氧化剂分子、工程化免疫球蛋白样分子、单链抗体、融合蛋白、免疫共刺激分子、免疫调节分子、反义RNA、微小RNA、shRNA、siRNA、引导RNA(gRNA,例如与CRISPR/Cas系统结合使用)、核酶、miRNA靶序列、靶蛋白的跨域(transdomain)负突变体、毒素、条件毒素、抗原、肿瘤抑制因子蛋白、生长因子、转录因子、膜蛋白、表面受体、抗癌分子、血管活性蛋白和肽、抗病毒蛋白和核酶及其衍生物(例如具有相关报告基团的衍生物)。NOI也可以编码前药活化酶。
NOI的一个例子是β-珠蛋白链,其可用于地中海贫血/镰刀形细胞病的基因疗法。
NOI还包括那些可用于治疗其他需要在髓样谱系中进行非紧急/选择性基因矫正的疾病的NOI,例如:慢性肉芽肿病(CGD,例如gp91phox转基因)、白细胞粘附缺陷、无持续严重感染的患者的其他吞噬细胞病症和遗传性骨髓衰竭综合征(例如,范科尼(Fanconi)贫血)以及原发性免疫缺陷(SCID)。
NOI还包括那些可用于治疗溶酶体贮积病和免疫缺陷的NOI。
本发明对T细胞的适用性还促进其在基于将经修饰的T细胞输注入患者中的细胞疗法中的应用,包括抗癌策略(例如使用工程化的CAR-T细胞)和基于通用供体T细胞的输注的方法。因此,NOI也可以包括例如嵌合抗原受体(CAR)。
药物组合物
可以配制本发明的细胞以与药学上可接受的载体、稀释剂或赋形剂一起施用于受试者。合适的载体和稀释剂包括等张盐水溶液,例如磷酸盐缓冲盐水,并潜在含有人血清白蛋白。
优选地,细胞疗法产品的处理按照FACT-JACIE国际细胞疗法标准进行。
造血干细胞和/或祖细胞移植
本发明提供了根据本发明的方法制备的细胞群体,例如造血干细胞和/或祖细胞群体或T细胞群体,用于疗法,例如用于基因疗法。
该用途可以作为细胞移植程序的一部分,例如造血干细胞和/或祖细胞移植程序。
造血干细胞移植(HSCT)是源自骨髓(在此种情况下称为骨髓移植)或血液的血液干细胞的移植。干细胞移植是血液学和肿瘤学领域的医疗程序,通常是针对患有血液或骨髓疾病或某些类型癌症的人进行的。

在另一个实施方案中,根据本发明的方法制备的造血干细胞和/或祖细胞群体作为同种异体干细胞移植物程序的一部分施用。
如本文所用,术语“自体干细胞移植物程序”是指下述的程序,其中起始细胞群体(它们然后根据本发明的方法转导)是从与接受经转导的细胞群体施用的受试者相同的受试者获得的。自体移植物程序是有利的,因为它们避免了与免疫学不相容性相关的问题,并且无论遗传匹配供体的可用性如何,可用于受试者。

此类造血祖细胞基因疗法可以适合于治疗获得性病症,例如癌症,其中感兴趣的(潜在毒性)抗癌核苷酸的时间限制性表达可以足以根除该疾病。

材料和方法
载体

细胞系
人胚胎肾脏293T细胞(HEK293T)以及人K562髓性白血病细胞系均维持在Iscove改良的Dulbecco培养基(IMDM;Sigma)中。人THP-1细胞维持在Roswell Park MemorialInstitute培养基(RPMI;Lonza)中。两种培养基均补充有10%的胎牛血清(FBS;Gibco)、青霉素(100IU/ml)、链霉素(100μg/ml)和2%的谷氨酰胺。
原代细胞
根据制造商的说明(Miltenyi),通过正磁珠选择从脐带血(CB)或根据机构伦理委员会批准的协议(TIGET01)从健康志愿者在知情同意后收集的单个核细胞中分离人CD34+HSPC、CD4+或CD3+T细胞和CD14+单核细胞。在其他情况下,直接从Lonza或Hemacare购买CB、骨髓(BM)或G-CSF动员的外周血(mPB)CD34+细胞。在补充有10%FBS、青霉素(100IU/ml)、链霉素(100μg/ml)、2%谷氨酰胺、植物血凝素(PHA)(2μg/ml,Roche)和IL-2(300IU/ml,Novartis)的RPMI中活化CD4+T细胞达三天,并维持在补充10%FBS、青霉素(100IU/ml)、链霉素(100μg/ml)、2%谷氨酰胺和IL-2(300UI/ml)的RPMI中。在其他情况下,按照制造商的指示,使用缀合有抗人CD3和CD28抗体(Dynabeads人T活化剂CD3/CD28;Invitrogen)的磁珠刺激CD3+T细胞,并在补充有青霉素、链霉素、10%FBS和各5ng/ml IL-7和IL-15(PeproTech)的Iscove的改良Dulbecco培养基(IMDM)(GIBCO-BRL)中培养。在补充有10%FBS、青霉素(100IU/ml)、链霉素(100μg/ml)、2%谷氨酰胺和5%人血清AB(Lonza)的DMEM中,自分离的CD14+单核细胞将单核细胞衍生的巨噬细胞(MDM)分化7天。

转导
转导前16至24小时将人CB衍生的造血干/祖细胞(HSPC)培养在补充有青霉素(100IU/ml)、链霉素(100μg/ml)、100ng/ml重组人干细胞因子(rhSCF)、20ng/ml重组人血小板生成素(rhTPO)、100ng/ml重组人Flt3配体(rhFlt3)和20ng/ml重组人IL6(rhIL6)(全部来自Peprotech)的无血清StemSpan培养基(StemCell Technologies)中。然后,在相同培养基中以指示的感染复数(MOI)用水疱性口炎病毒糖蛋白(VSV-G)-假型化的SINLV以每毫升1x106个细胞的浓度转导HSPC达16小时。将骨髓和G-CSF动员的外周血CD34+细胞置于含有细胞因子混合物:60ng/ml IL-3、100ng/ml TPO、300ng/ml SCF和300ng/ml FLT-3L(均来自Cell Peprotech)的CellGro培养基(Cell Genix)中用纤维连接蛋白(retronectin)包被的非组织培养物处理的孔(T100A Takara)上的培养物中。然后在相同的含细胞因子的培养基中用指定剂量的载体转导细胞14-15小时。用单命中报告物LV转导后,将细胞洗涤并维持在如上所述的补充有细胞因子的无血清培养基中,直到通过FACS读取阳性细胞的百分比,然后再将它们维持在补充有10%FBS、25ng/ml rhSCF、5ng/ml rhIL6-3、25ng/ml rhFlt3和5ng/ml rhTPO的IMDM中达7天,之后进行载体拷贝数分析。在选择用于临床应用的预见到两轮转导的方案中,将细胞在补充有细胞因子的CellGro SCGM培养基中洗涤10小时,并在与第一命中相同的条件下进行第二转导命中,如先前报告(Biffi,A.et al.(2013)Science341:1233158)。转导结束时,对细胞进行计数并收集以进行克隆形成测定法、流式细胞术和体内研究。将剩余的细胞铺板在具有细胞因子(IL-3,60ng/ml;IL-6,60ng/ml;SCF,300ng/ml)的IMDM 10%胎牛血清(FBS)中,并且培养总共14天。此后,收集细胞用于分子和生化研究。将在补充有青霉素(100IU/ml)、链霉素(100μg/ml)的StemSpan培养基中新鲜分离的未刺激的HSPC转导16-24小时,然后在人细胞因子和10μM逆转录酶抑制剂3TC(SIGMA)存在下维持以避免由于细胞因子刺激而引起的后续转导。
分化后7天转导MDM。在刺激2-3天后,以106个细胞/ml的浓度转导T淋巴细胞。将细胞暴露于载体16-20小时。
化合物
将环孢菌素A(CsA)、环孢菌素H(CsH)和雷帕霉素(均来自Sigma-Aldrich)重新悬浮并按照制造商的说明进行贮存。将它们以指定的浓度添加到转导培养基中,并在16-20小时后与载体一起洗出。在描述的地方,在LV转导前2小时以10μM的终浓度添加前列腺素E2(来自Yonsung的地诺前列酮)。
线性扩增介导的PCR和测序
对从培养细胞中提取的约300ng DNA进行线性扩增介导(LAM)-PCR。简而言之,进行了载体-基因组连接的100个循环线性PCR预扩增,然后通过链霉亲合素偶联的磁珠对生物素化的靶DNA进行磁性捕捉,六核苷酸引发,使用MluCI、HpyCHIV4和AciI酶进行限制性酶切消化,并且与限制性位点-互补接头盒连接。然后,通过两个巢式PCR用对载体长末端重复序列(LTR)和接头盒序列特异性的引物扩增连接产物。在Shimadzu MultiNA微芯片电泳系统上分离LAM-PCR扩增子,以评估每个样品的PCR效率和条带模式。先前已经描述了使用的引物和PCR热方案(Schmidt,M.et al.(2007)Nat Methods4:1051-1057)。然后,通过AmpureXP珠纯化LAM-PCR产物,并用QubitTM荧光计(Thermo Fisher Scientific,Pittsburgh,PA)进行定量。40ng PCR产物用以下各项再扩增:Fusion-LTR(AATGATACGGCGACCACCGAGATCTACACTCTTTCCCTACACGACGCTCTTCCGATCTNNNNNNNNNNNNXXXXXXXXACCCTTTTAGTCAGTGTGGA)和Fusion-LC(CAAGCAGAAGACGGCATACGAGATGTGACTGGAGTTCAGACGTGTGCTCTTCCGATCTNNNNNNNNNNNNXXXXXXXXgatctgaattcagtggcacag)引物(其含有8核苷酸(X)标签,该标签允许在扩增子的LTR和接头盒侧两者上对样品加条形码)、允许用Illumina MiSeq系统进行配对末端测序的特异性序列和增加簇分离的随机12核苷酸(N)序列。使用Qiagen TAQDNA聚合酶在以下条件下进行PCR:95℃2min,和95℃45s,58℃45s,72℃1min,12个循环,然后进行在72℃再温育5分钟。通过荧光定量对条形码化的LAM-PCR产物进行定量,并以等摩尔比组装到文库中,并且避免相同对的重复。
常见插入位点分析
通过使用滑动窗方法(Abel,U.et al.(2007)PLoS One 2:e570)和插入位点(CIS)分析异常值的Grubbs检验(Biffi,A.et al.(2011)Blood 117:5332-5339)来鉴定重大的常见插入位点(IS)。对于滑动窗方法,使用“Cluster”函数利用R包(2012年8月的最新更新),该函数返回IS的原始输入列表,带有附加注释字段,例如代表每个CIS中含有的最大整合数目的“CIS max order”和代表将一个或多个CIS间隔聚簇的基因组窗的“Cluster ID”。对于基因中心方法,使用RefSeq基因的注释(从UCSC Genome Browser下载的General FeatureFormat文件,版本hg19,NCBI Build 37),并计算每个基因符号的平均转录物长度。然后,对异常值进行Grubbs检验,以将每个靶定基因的整合频率相对于数据集中所有靶定基因的平均频率进行比较。为了比较从在DMSO存在下转导的细胞相对于在CsA存在下转导的细胞中检索到的IS分布,我们提取了在整个基因组上滑动1Mb的5Mb窗内的IS的覆盖。然后,我们计算每个窗的log2倍数变化,比较CsA与DMSO。
人和小鼠细胞的基因编辑

NOD-SCID-IL2Rg-/-(NSG)小鼠购自Jackson laboratory。所有动物程序均根据Ospedale San Raffaele动物护理和使用委员会(IACUC 784)批准的方案进行,并根据意大利法律告知卫生部和地方当局。预刺激人骨髓衍生的CD34+细胞,并且如先前所述在存在或不存在指定的DMSO、CsA和/或雷帕霉素的情况下以100的MOI用IDUA-LV转导。预刺激人mPBCD34+细胞,并且如先前所述用IDUA-LV在存在或不存在指定的DMSO、CsH和/或16-16二甲基前列腺素E2或其组合的情况下以MOI为50转导。转导后,将2-5x105个细胞输注入亚致死照射的8-10周龄NSG小鼠的尾静脉中(辐射剂量:体重18-25g的小鼠为200cGy,体重超过25g的小鼠为220cGy)。还将转导的和未转导的细胞体外培养14天以进行进一步分析。然后将处死时从移植小鼠中分离的体外培养细胞和BM细胞用于通过qPCR定量VCN。
集落形成细胞测定法
通过在基于甲基纤维素的培养基(Methocult GF4434;Stem Cell Technologies)中铺板8x102个在不同化合物存在下转导的人HSPC进行集落形成细胞测定法。15天后,通过光学显微术在集落数目和形态学上对集落进行评分。CFU-E和BFU-E评为红系集落,而CFU-G、CFU-M和CFU-GM和CFU-GEMM评为髓样集落。此外,采集单集落用于分子分析。
流式细胞仪
所有细胞计数分析均使用FACSCanto III仪和LSRFortessa仪(BD Biosciences)进行,并使用FACS Express软件(De Novo软件)进行分析。
经转导的细胞
在转导后5-7天测量转导细胞中的GFP或BFP表达。通过在5mM PBS-EDTA中刮擦分离粘附的MDM,洗涤并重悬于含有2%胎牛血清(FBS)的PBS中。洗涤在悬浮液中培养的细胞,并重悬于含有2%FBS的PBS中。为了测量HSPC亚群组成,在转导后16或72小时收获细胞,在4℃与抗人受体封闭抗体一起温育15分钟,然后在4℃用抗人CD34、CD38、CD45RA、CD90或用抗人CD34、CD133、CD90抗体(对于抗体,参见表3)染色20分钟。为了从分析中排除死细胞,添加10ng/ml 7-氨基放线菌素D(7-AAD)。
人集落
从单个板(集落池)中收获来自CFC的人分化细胞,并混合成单细胞悬液中。然后洗涤细胞,并重悬于含有2%FBS的PBS中。为了进行免疫染色,将细胞与抗人受体封闭抗体在4℃温育15分钟,然后在4℃用抗人CD235a和CD33抗体染色20分钟(关于抗体,参见表3)。为了从分析中排除死细胞,将细胞洗涤并重悬于含有10ng/ml 7-氨基放线菌素D(7-AAD)的PBS中。
来自小鼠的外周血
对于每只小鼠,将250μl外周血添加到15μL含45mg/mL EDTA的PBS中。为了进行免疫染色,首先将已知体积的全血(100μl)与抗人FcγIII/II受体(Cd16/Cd32)封闭抗体在4℃温育15分钟,然后在存在单克隆抗体(对于抗体,参见表3)的情况下在4℃温育20分钟。在存在等体积的FBS(100μl)的情况下,用TQ-Prep工作站(Beckman-Coulter)裂解除去红细胞,以保护白细胞。
骨髓
通过在PBS 2%FBS溶液中冲洗股骨获得BM细胞。洗涤细胞(1x106个细胞),重悬于100μl含2%FBS的PBS中,并与抗人受体(Cd16/Cd32)封闭抗体在4℃温育15分钟。然后在4℃下用单克隆抗体(对于抗体,参见表3)染色20分钟。洗涤细胞,最后重悬于含有2%FBS的PBS中。
脾
首先将脾捣碎,并且使所得的细胞悬液通过40μm尼龙滤膜,然后在含有2mM EDTA和0.5%牛血清白蛋白(BSA)的冷磷酸盐缓冲盐水(PBS)中洗涤。将细胞与抗人受体(Cd16/Cd32)封闭抗体在4℃温育15分钟,然后在4℃用抗人单克隆抗体(对于抗体,参见表3)染色20分钟。最后洗涤细胞,并重悬于含有2%FBS的PBS中。
Ki67和Hoechst流式细胞术
洗涤细胞并用BD Cytofix缓冲液(Cat.#554655)固定,洗涤并且用BD Perm 2(Cat.#347692)透化,洗涤并用偶联有PE的Ki67抗体(BD)染色,最后重悬于具有1μg/mLHoechst的BD Cytofix缓冲液中。然后在带有UV激光的BDLSRII仪上分析细胞。
细胞增殖测定法
在细胞因子预刺激24小时后且细胞转导前用细胞增殖染料670(Affimetrix,eBioscience)染色细胞。此种荧光染料与任何含有伯胺的细胞蛋白结合,并且随着细胞分裂,染料在后代细胞之间均等分布,所述后代细胞可以以染料的荧光强度的连续减半进行测量。转导后的不同时间点,收集细胞并通过流式细胞仪分析。细胞增殖染料670具有647nm的峰值激发,并且可以通过红色(633nm)激光线激发。它具有670nm的峰值发射,并且可以用660/20带通滤器(相当于APC,/>647或/>660)检测。
ROS定量
用CM-H2DCFDA(Thermo Fisher Scientific)染色细胞,所述CM-H2DCFDA被动扩散到细胞中,在那里其乙酸酯基团被胞内酯酶切割,并且其硫醇反应性氯甲基与胞内谷胱甘肽和其他硫醇反应。随后的氧化产生荧光加合物,其被捕获在细胞内并使用流式细胞仪监测。将对N-乙酰基-L-半胱氨酸(NAC,来自SIGMA)和过氧化氢(H2O2,SIGMA)分别以1mM和10mM的浓度添加到荧光探针中。
RNA提取、qPCR和基因表达分析
使用RNeasy Plus mini Kit(Qiagen)从细胞中提取RNA。简言之,将细胞在补充有β-巯基乙醇的缓冲液RLT plus中裂解。然后根据制造商的说明提取RNA。使用SuperScriptVilo试剂盒(11754250;Invitrogen)对提取的mRNA进行逆转录。使用来自Applied Biosystems的TaqMan探针(见下文)进行Q-PCR分析。使用Viia 7仪运行Q-PCR达40个循环,然后使用Viia 7软件提取原始数据(Ct)。为了确定基因表达,通过应用相等的阈值来计算每个基因的阈值循环(Ct)与参考基因的阈值循环之间的差异(ΔCt)。相对定量值通过公式2-ΔΔCt计算为感兴趣基因相对于其在参考样品中表达的倍数变化表达。使用管家基因HPRT1将表达标准化。使用了来自Applied Biosystems的以下Taqman探针:PPIA(Hs99999904_m1),PPIB(Hs00168719_m1),OAS1(Hs00973637_m1),IRF7(Hs01014809_g1),MX2(Hs01550808_m1)和HPRT1(Hs01003267_m1)。
复制中间体
在存在或不存在CsA或CsH的情况下,以MOI为100转导CB衍生的CD34+细胞。为了分析病毒复制中间体,将经转导的细胞洗涤并且重悬于Monini裂解缓冲液(0.1%聚氧乙烯10月桂基醚(Sigma)、0.1mg/mL蛋白酶K(Promega))(25μl/1x 105个细胞)中,于65℃温育2小时,并且在94℃加热灭活15分钟(Monini,P.et al.(1999)Blood 93:4044-4058)。分别在转导后6或24小时进行细胞裂解以回收晚期-RT和2-LTR中间体。通过定量液滴数字PCR(dd-PCR)测定法使用以下所述的引物测量晚期RT和2LTR循环,并使用人TERT基因进行标准化。

基因组DNA提取和qPCR
使用Maxwell 16仪(Promega)或Blood&Cell Culture DNA微试剂盒(Qiagen)提取来自细胞培养液和血液的DNA。使用针对LV的引物结合位点区的以下引物(HIV有义:5′-TACTGACGCTCTCGCACC-3′;HIV反义:5′-TCTCGACGCAGGACTCG-3′)和探针(FAM 5′-ATCTCTCTCCTTCTAGCCTC-3′)通过定量液滴数字PCR(dd-PCR)对整合的慢病毒载体的每个二倍体基因组的载体拷贝(载体拷贝数,VCN)进行定量。如前所述(Matrai,J.et al.(2011)Hepatology 53:1696-1707),在转导后三天进行总慢病毒DNA(整合和非整合)的VCN定量。使用以下引物通过定量液滴数字PCR(dd-PCR)进行逆转录逆转录病毒载体基因组(整合的和非整合的两者)的拷贝数,区分它与从瞬时转染残留的质粒:RT-RV;ΔU3有义:5'-CGAGCTCAATAAAAGAGCCCAC-3',PBS反义:5'-GAGTCCTGCGTCGGAGAGAG-3'。载体拷贝数和复制中间体相对于基因组DNA含量标准化,所述基因组DNA含量使用人TERT基因评估。通过dd-PCR进行的VCN分析涉及通过使用双重靶标和参考测定法对靶标和参考基因座的定量。在QuantaSoftTM软件中,通过计算靶分子浓度与参考分子浓度之比乘以基因组中参考种类的拷贝数(通常为2)来确定拷贝数。

Western印迹
如先前所述(Kajaste-Rudnitski,A.et al.(2011)J Virol 85:5183-5196;Kajaste-Rudnitski,A.et al.(2006)J BiolChem 281:4624-4637),从HSPC制备全细胞提取物。将样品进行SDS-PAGE,通过电印迹法转移到PVDF膜上,并用针对CypA的小鼠多克隆抗体(Ab)(Santa-Cruz Biotechnology)进行印迹。抗肌动蛋白抗体(Sigma-Aldrich)用作标准化剂。
统计分析
在所有研究中,值表示为平均值±平均值的标准误差(SEM)。如所示,通过不成对的Student t检验或多重比较的ANOVA进行统计分析。将百分比转换为Log ODD以进行统计分析。认为差异在p<0.05时是统计学显著的。

结果
缩短的基于CsA和雷帕霉素的方案安全实现与当前临床标准相似的基因转移效率,并改善HSPC植入
在有或没有化合物的情况下用临床级表达IDUA的或表达ARSA的LV的单命中转导人BM衍生的CD34+细胞。作为对照,用当前的标准临床转导方案转导细胞,该方案由以载体的MOI为100的两个转导命中组成(图1A)。与没有药物情况下的1命中对照相比,CsA和雷帕霉素能够增加临床级IDUA和ARSA-LV两者的转导效率,在体外达到与2命中金标准相当的VCN(图1B-C)。此外,没有观察到集落形成能力的变化(图1D)。
对用单命中转导的HSPC移植的NSG小鼠的外周血(PB)的FACS分析显示与临床方案相比更好的人CD45+细胞的植入,特别是在CsA的情况下,在移植物后长达16周时(图2A)。此外,未观察到不同谱系输出的变化(图2B)。这两种方案均实现改善的体内长期再增殖的HSPC的转导,因为在移植后22周在小鼠的BM和脾中实现与2命中金标准相当的VCN/人基因组(图2C)。对于所有较短的方案,较短的离体培养期本身也改善小鼠的BM和脾中的HSPC植入(图2D)。
先前已显示雷帕霉素不影响人HSPC中的LV整合位点概况分析(Wang,C.X.et al.(2014).Rapamycin relieves lentiviral vector transduction resistance in humanand mouse hematopoietic stem cells.Blood)。我们在这里就LV整合位点概况分析而言评估基于CsA的基因转移方案的安全性概况。尽管在存在CsA的情况下系统地检索到更多整合位点,但与其增加每个细胞的载体拷贝数的能力相一致,我们未检测到从不存在或存在CsA的情况下转导的HSPC检索到的整合位点的分布和与从最近的基因疗法试验(Aiuti,A.et al.(2013)Science 341:1233151;Biffi,A.et al.(2013)Science 341:1233158)中获得的大量IS研究相比之间的任何显著差异(图9)。
CsA不依赖于转导离体保存原始HSC
为了调查对CsA条件观察到的人细胞的更高植入后的原因,我们评估了CsA在体外对干细胞组成的影响。在存在或不存在CsA的情况下,以MOI为10用SINLV-GFP转导人BM衍生的CD34+细胞。在TD后16小时通过FACS评估原始干细胞和祖细胞的百分比(图3A)。体外CsA暴露增加更原始的干细胞和多能祖细胞的百分比,而减少更定型的干细胞和多能祖细胞。值得注意的是,未观察到培养物中髓样分化随时间的变化。在TD后72小时通过FACS评估原始干细胞和祖细胞的百分比(图3B)。TD后3天,体外CsA暴露仍增加更原始的干细胞和多能祖细胞的百分比。
为了研究较慢的增殖速率是否可以解释在CsA存在下更原始细胞的保存,用红色荧光染料对人BM衍生的CD34+细胞染色,该红色荧光染料可用于监测单个细胞分裂。此染色后,在有或没有CsA的情况下转导细胞,并在TD后不同时间通过FACS分析以监测细胞增殖速率(图4A)。CsA显著降低体外细胞增殖(图4B)。转导后16和72小时通过FACS评估不同HSPC亚群内细胞增殖染料的水平(图4C-D)。CsA在范围为更定型的祖细胞至最原始的细胞的所有HSPC亚群中产生相等的细胞增殖减少。
原始HSC的特征在于更静止的细胞周期状态。我们调查了CsA介导的HSC的增加是否与细胞周期G0期中较高分数的细胞有关。我们使用了基于FACS的组合染色策略,该策略使我们能够区分处于细胞周期的G0、G1、S和G2-M期的细胞。CsA处理显著增加了在循环的静止G0期停滞的细胞比例(图4D)。由于氧化应激在HSPC生物学中起着至关重要的作用,并且已暗示CsA影响HSPC中的氧化还原平衡(Mantel,C.R.et al.(2015)Cell 161:1553-1565),因此我们还评估了CsA对人BM衍生的HSPC中ROS水平的影响。在我们的实验背景中,CsA不导致体外测量的ROS水平的任何变化(图4E)。总之,CsA介导的HSPC增殖减少和静止维持可以潜在有助于保留其干细胞特性并在体内产生更高的植入。
CypA不依赖性的环孢菌素揭示对HSPC中LV转导的早期阻断
为了直接解决人HSPC中的LV转导和CsA介导的效应期间CypA的作用,我们使用shRNA敲低(KD)CB衍生的CD34+细胞中的此宿主辅因子(图5A-B),并评估了其对转导的影响。与LV转导中CypA的积极作用相一致(Towers,G.J.et al.(2014)Cell Host Microbe16:10-18),CypA的KD导致更低的人HSPC转导(图5C)。这些结果暗示CsA增加HSPC中LV转导的能力可能是亚最佳的,因为它也会干扰此种与CypA的积极载体相互作用。
然后,我们测试了天然存在的CsA同种型,环孢菌素H(CsH),其不结合CypA并且不是免疫抑制的(Jeffery,J.R.(1991)Clin Biochem 24:15-21)。值得注意的是,CsH在人和鼠HSPC中(包括在临床上相关的mPB衍生的HSPC中)增加LV转导,比CsA显著更有效(图5D-F;图6M,N)。重要的是,在体外在不存在任何毒性迹象的情况下发生此种转导增加(图5G)。与CsA相反,并且与CsH的非免疫抑制特征一致,在此情况下未观察到增殖延迟(图5H)。令人感兴趣的是,CsH与其他两种早期作用的化合物雷帕霉素(Rapa)(Petrillo,C.et al.(2015Mol Ther 23:352-362;Wang,C.X.et al.(2014).Rapamycin relieves lentiviralvector transduction resistance in human and mouse hematopoietic stemcells.Blood)和前列腺素E2(PGE2;Zonari,E.et al.(2017).Efficient Ex VivoEngineering and Expansion of Highly Purified Human Hematopoietic Stem andProgenitor Cell Populations for Gene Therapy.Stem Cell Reports)而非与CsA是加性(additive)的(图5I-J),这提示了这两种环孢菌素作用于相同的慢病毒限制区,然而该区不同于分别由Rapa和PGE2靶向的早期事件。重要的是,CsH在所有CD34+亚群中都显著增加转导效率,包括在更原始的CD34+CD133+CD90+级份中(图5K),以及在用临床级LV转导的SCID再增殖mPB衍生的CD34+细胞中(图5L-M,10和11)而不改变亚群组成和细胞周期状态(图5N)。值得注意的是,CsH还有效地增强HSPC中的gamma逆转录病毒转导(图5O),提示它也可用于基于γRV的基因疗法策略(Ferrua,F.et al.(2017)HumGene Ther 28:972-981)。
我们还测试了CsH对其他细胞类型中LV转导的影响。CsH不改变原代人单核细胞衍生的巨噬细胞(MDM)中的转导(图6A),不依赖于SAMHD1介导的LV限制,该限制可通过将细胞预先暴露于SIV辅助蛋白Vpx来缓解(Berger,G.et al.(2011)Nat Protoc 6:806-816)。相反,在活化的CD4+T细胞的情况下,我们看到GFP+细胞的百分比略有增加(图6B)。这些数据与我们和其他人在这些细胞区室中对CsA观察到的数据形成鲜明对比,进一步强调了CsA的CypA相关效应有可能如何掩盖其对LV转导可能具有的其他潜在的整合前益处。实际上,CypA不依赖性的P90A LV壳体突变体以与CsH相比相似或甚至更高的程度受益于CsA(图6C)。在CsH存在的情况下,CPSF6不依赖性的N74D壳体突变体LV的转导水平增加(图6D),排除了来自CsH作用机制的此壳体-宿主相互作用。值得注意的是,猿免疫缺陷病毒(SIV)和γRV衍生的载体(它们不与CypA相互作用(Fujita,M.et al.(2001)J Virol 75:10527-10531;Sokolskaja,E.et al.(2006)Curr Opin Microbiol 9:404-408))不仅受益于CsH,而且受益于CsA(图6E),进一步证实了这两种环孢菌素对LV限制的CypA不依赖性缓解。一致地并且与CsA不同,CsH能够在未刺激的HSPC中(图6G)以及使用整合酶缺陷型LV(IDLV)增加LV转导(图6H),这可能是由于这些实验设置中载体与CypA的有益相互作用的维持。与较早的整合前效应一致,正如我们先前也报告(Petrillo,C.et al.(2015)Mol Ther 23:352-362),CsH导致不受CsA影响的晚期RT和2LTR循环复制中间体两者的显著增加(图6I)。
到目前为止,此项工作中使用的所有载体均已用VSV-g包膜糖蛋白假型化,这是LV基因疗法的背景中最常用的(Cronin,J.et al.(2005)Curr Gene Ther 5:387-398)。已显示雷帕霉素促进VSV-g介导的pH依赖性LV内吞(Wang,C.X.et al.(2014).Rapamycinrelieves lentiviral vector transduction resistance in human and mousehematopoietic stem cells.Blood)。为了评估VSV-g介导的LV颗粒的进入是否可参与CsH诱导的转导增加,我们生成了用经修饰的狒狒内源逆转录病毒包膜糖蛋白(BaEV-TR)假型化的LV,其通过受体介导的融合直接在质膜处进入并且先前已经描述了它有效地转导包括HSPC在内的几种原代人造血细胞(Girard-Gagnepain,A.et al.(2014)Blood 124:1221-1231)。令人感兴趣的是,对于在存在CsH的情况下在CB和mPB衍生的HSPC中BaEV-TR假型化的LV转导未能检测到明显的益处,而CsA在此情况下实际上削弱了转导(图6J-K)。这两种环孢菌素均不能改善腺相关载体(AAV)的转导(图6L),无论其内吞介导的靶细胞进入(Nonnenmacher,M.et al.(2012)Gene Ther 19:649-658)。
CsH增加SCID再增殖HSPC中的LV转导和基因编辑效率
为了评估在临床上更相关的环境中CsH增强的转导,我们用表达alpha-L-艾杜糖醛酸酶(IDUA)转基因的临床级LV(IDUA-LV)转导人mPB-CD34+细胞,其经设计以治疗受I型粘多糖贮积病(MPS-1)影响的患者(Visigalli,I.et al.(2016)Hum Gene Ther 27:813-829;Visigalli,I.et al.(2010)Blood 116:5130-5139)。我们将在存在/不存在CsH、PGE-2或它们的组合的情况下转导的细胞移植到人造血的异种移植NSG小鼠模型中,以追踪在体内的植入和转导效率(图10A)。为了进行比较,按照当前标准方案的指示,将细胞转导两次。在对照和经CsH/PGE-2处理的细胞之间,未能观察到集落形成能力的差异(图10B)。值得注意的是,在存在CsH的情况下一个单次LV剂量足以在体外(图11A,B)以及在长期再增殖HSC和体内衍生后代(图10C;图11C,D)中产生显著更高的基因标记。这超越了目前由两轮转导组成的标准,并且与单一转导对照组相比在体内长期基因标记方面实现了近10倍的增长。尽管在此情况下(图11A,B)以及在体内早期植入阶段(图11C)期间,CsH和PGE-2在体外是加性的,但令人惊讶的是在骨髓中的长期再增殖HSC和小鼠脾中的后代中丧失组合益处(图10C;图11D)。与2命中方案相比,CsH暴露不改变HSPC的短期植入能力,而与2命中方案相比,用单命中对照转导细胞移植的小鼠在移植物后8周早期显示出更高的人CD45+细胞植入(图10D),与较短的离体培养保留HSPC再增殖的观点一致(Zonari,E.et al.(2017)Stem CellReports 8:977-990)。在经CsH处理的细胞的背景中此类益处的缺乏可以与转导增强有关,因为我们最近显示了慢病毒基因转移由于载体介导的p53信号传导级联的触发而以剂量依赖的方式影响离体HSPC回收及其短期体内植入(Piras,F.et al.(2017)EMBO Mol Med 9:1198-1211)。一致的是,与对照细胞相比,仅将mPB-CD34+细胞暴露于CsH中并不影响HSPC的早期植入(图10E)。重要的是,对于在CsH存在下转导的细胞,长期在小鼠BM和脾脏中也回收到与较短的离体培养物相关的更高植入(图10F;图11E,F)。
供体DNA模板的亚最佳可用性可通过人HSPC中的同源重组促成差的基因编辑效率(Naldini,L.(2011)Nat Rev Genet 12:301-315)。IDLV和AAV目前是基因编辑方案中用于供体DNA递送的最常用的载体平台(Naldini,L.(2015)Nature 526:351-360)。由于CsH在人HSPC中增强IDLV转导,因此我们测试了其对IDLV介导的HSPC基因编辑效率的影响(图10G)。值得注意地,在存在CsH的情况下递送供体IDLV使人HSPC中的基因编辑效率提高了两倍(图10H),而在治疗后三天没有改变造血亚群的相对组成(图11G)。此种增加也发生在原始的CD34+CD133+CD90+级份中(图10H),这表明CsH有利于在HSPC的长期再增殖级份中的编辑。实际上,在存在CsH的情况下编辑的细胞有效地再增殖NSG小鼠,并且在小鼠的外周血以及骨髓中长期维持显著更高的靶向效率(图10I-K)。重要的是,在骨髓中所有HSPC衍生的后代中确认基因靶向效率的增加(图11H,I)。总之,这些结果将CsH鉴定为迄今为止描述的HSPC基因转移和编辑的最佳表现增强剂,并强烈支持其在治疗性HSPC基因工程中的用途。
环孢菌素抵消对VSV-g依赖性慢病毒载体进入的IFN诱导性阻断
为了获得有关CsH如何增强HSPC中基因转移和编辑的机制的机械论见解,我们利用了以下观察结果:已经报告CsA改善人IFNα刺激的单核THP-1细胞中的转导(Bulli,L.etal.(2016)J Virol 90:7469-7480)。我们首先证实,CsA部分挽救IFNα诱导的THP-1细胞中VSV-g假型化LV转导的抑制(图7A)。相比之下,CsH完全挽救IFNα的抑制(图7A),提示CsH在缓解I型IFN介导的抗病毒阻断中更有力。此外,感染的挽救不依赖于通过钙神经素抑制介导的CsA的免疫抑制效应(Petrillo,C.et al.(2015)MolTher 23:352-362)。CsH的最佳表征的宿主靶标是参与嗜中性粒细胞迁移和抗微生物防御的甲酰基肽受体1(FPR1)(dePaulis,A.et al.(1996)J Allergy ClinImmunol 98:152-164;Prevete,N.et al.(2015)Pharmacol Res 102:184-191)。然而,FPR1不参与被环孢菌素抑制的IFNα介导的限制,因为IFN继续抑制FPR1耗尽的THP-1细胞的转导(图12A)。令人感兴趣的是,此种行为是THP-1细胞系特异的,因为IFNα或环孢菌素处理均不影响慢性髓性白血病细胞K562中的LV转导(图7B),尽管这两种细胞类似地响应IFNα,这由I型IFN刺激的基因(ISG)上调证明(图12B,C)。这提示细胞类型特异性ISG和/或辅因子参与THP-1和HSPC中环孢菌素敏感性转导。这两种环孢菌素还挽救用IFNα预处理的人HSPC中的LV转导,与CsA相比,CsH再次具有更有力的效果(图7C;图12D)。
用MLV衍生的双向性包膜糖蛋白假型化的载体对I型IFN介导的抑制仍然不敏感(图7D),证明限制敏感度受病毒包膜的影响。与包膜依赖性作用机制相一致,用经修饰的狒狒内源逆转录病毒包膜糖蛋白(BaEV-TR)假型化的LV(它通过受体介导的融合直接在质膜处进入(Girard-Gagnepain,A.et al.(2014)Blood 124:1221-1231))对THP-1细胞中的I型IFN和环孢菌素保持不敏感(图7E)。重要的是,在HSPC中环孢霉素处理不增强BaEV-TR假型化LV的转导,并且对IFNα不太敏感(图7F)。总之,这些结果与THP-1细胞中对VSV-g介导的LV进入的I型IFN诱导性阻断一致,其在未处理的HSPC中对环孢菌素也有活性和敏感性。
CsH提高人和鼠HSPC以及原代人T细胞中的基因编辑效率
在人HSPC中有效的基因编辑的障碍之一可在于供体DNA模板的亚最佳可用性(Naldini,L.(2011)Nat Rev Genet 12:301-315)。IDLV和腺相关载体(AAV)目前是基因编辑方案中供体DNA递送媒介物的最常用的载体平台(Naldini,L.(2015)Nature 526:351-360)。考虑到CsH还提高人和鼠HSPC中的IDLV转导效率的能力,我们在HSPC基因编辑方法的背景下测试了其作用。值得注意的是,在鼠和人HSPC两者中,在CsH存在下递送供体IDLV使基因靶向效率提高两倍(图8A-B)。此种增加发生在所有亚群中,包括更原始的CD34+CD133+CD90+级份中,提示CsH可有助于在HSPC的长期再增殖级份中进行编辑,已经报告了所述级份是同源重组(HR)介导的基因靶向最难处理的(Genovese,P.et al.(2014)Nature 510:235-240)。处理后3天未观察到造血亚群相对组成的显著变化,提示在基因编辑程序过程中包含CsH不会不利影响更原始细胞的存活和生长(图8B,右图)。此外,在HSPC基因编辑过程中使用CsH可以用更小的IDLV实现与当前标准方案相当的基因编辑水平。值得注意的是,即使供体IDLV的量减少时,CsH可以改善人原代CD3+T细胞中的基因靶向效率。
讨论
改善HSPC对LV转导的允许性对于将基因治疗作为几种遗传性疾病的治疗选择的广泛实施仍然至关重要。我们为改善人HSPC中的LV转导效率所做的努力已导致鉴定环孢菌素A(CsA)为此种情况下有力的转导增强剂(Petrillo,C.et al.(2015)Mol Ther 23:352-362)。在这里,我们已经显示了其在使用骨髓(BM)衍生的HSPC和临床级LV的临床培养条件下的功效和安全性,并且基于这些化合物改善人HSPC中的LV转导的机制的分子表征,已将环孢菌素H(CsH)鉴定为甚至更有力的LV转导增强剂。
我们在这里显示了CsA不仅安全地改善LV转导,而且还实现在人造血重建的NSG异种移植物模型中人BM衍生的HSPC的更好植入。此种益处可能与其降低HSPC增殖并更好保留培养物中更原始细胞的能力有关,因为已显示了静止和较短的离体培养两者改善其他背景下的HSPC植入(Glimm,H.et al.(2000)Blood 96:4185-4193;Kallinikou,K.et al.(2012)Br J Haematol 158:778-787;Larochelle,A.et al.(2012)Blood 119:1848-1855)。还显示了CsA通过其抑制亲环蛋白D依赖性氧化应激的能力来改善鼠Lin和人CB衍生的CD34+细胞的植入(Mantel,C.R.et al.(2015)Cell 161:1553-1565)。在我们先前使用CB衍生的HSPC的研究中,我们没有观察到类似的益处(Petrillo,C.et al.(2015)Mol Ther 23:352-362),潜在地是由于我们没有在严格存在CsA的情况下分离和培养细胞的事实,如由Mantel等进行。较为静止的BM衍生的CD34+细胞对环境氧化应激可以不太敏感,反映了也仅在离体转导过程期间较短暴露后的CsA益处。然而,另外的机制也可以参与此种较短的暴露背景,因为我们无法检测到CsA对BM衍生的HSPC中ROS水平的任何明显影响。
由于担心与CsA的免疫抑制功能有关的毒性,已开发出多种非免疫抑制性环孢菌素衍生物,并针对多种应用进行了测试(Peel,M.et al.(2013)Bioorg Med Chem Lett 23:4485-4492)。然而,这些中的大多数仍与宿主因子CypA结合,我们在此证明了它们对于HSPC中的有效转导也是重要的。与非免疫抑制的和CypA不依赖的环孢菌素将是最佳的观点相一致,我们鉴定出CsH(一种天然存在的CsA亚型(Jeffery,J.R.(1991)Clin Biochem 24:15-21))与CsA相比是甚至更有力且毒性更小的LV转导增强剂。就我们所知,CsH是迄今为止描述的最有效的HSPC基因转移增强剂,由我们的下述观察结果证明:在存在CsH的情况下一个单轮转导就转录效率而言甚至优于标准的2命中方案,而不改变长期再增殖HSPC的植入。当使用CsH时减少载体剂量的进一步优化应当允许高基因标记以及改善的HSPC植入。这在需要高水平的基因标记但难以实现的基因疗法背景中是特别重要的,例如血红蛋白病,其中大型且复杂的人β-珠蛋白基因表达盒限制临床规模的LV产生。CsH介导的效应的表征使我们好得多地理解在触发抗病毒应答后人HSPC中但还潜在更广泛地在其他人造血细胞中发生的LV限制阻断。CsH与CsA共享多个功能,包括壳体不依赖性,但也呈现一些显著差异。特别地,整合酶缺陷型LV(IDLV)在CD34+HSPC中受益于CsH,并且CsH还能在静止的HSPC和原代人T细胞中增加LV转导,这与我们先前对CsA观察到的情况(Petrillo,C.et al.(2015)MolTher 23:352-362)形成鲜明对比。这些差异很可能与CsA对使用CsH时保留的载体-CypA相互作用的负面影响有关。CypA是在病毒脱壳、核输入和可能甚至通过与病毒壳体(CA)相互作用的整合的步骤期间HIV-1所利用的宿主因子(Towers,G.J.et al.(2014)Cell HostMicrobe 16:10-18)。在HIV-1研究的背景下,公知的是CsA对慢病毒感染产生负面影响,因为它破坏CypA与病毒壳体之间的相互作用(Sokolskaja,E.et al.(2006)Curr OpinMicrobiol 9:404-408;Towers,G.J.et al.(2014)Cell Host Microbe 16:10-18)。在这些前提下,CsA对人HSPC中的LV转导具有的积极效果可能被它对CypA-CA轴的负面干扰所抵消。与该假设一致的是,CypA不依赖性的LV确实也显示HSPC中存在CsA的情况下的早期益处(Petrillo,C.et al.(2015)Mol Ther 23:352-362)。
最近已经报告了CsA克服THP-1中IFN诱导的抗病毒状态的能力(Bulli,L.et al.(2016)J Virol 90:7469-7480),但分子机制仍然难以捉摸。我们在这里显示,CsA和更具体地CsH在人HSPC的情况下也克服了I型IFN诱导的LV限制。环孢菌素克服稳态时以及I型IFN刺激后LV限制的能力似乎取决于用于对载体进行假型化的包膜糖蛋白的类型。虽然VSV-g假型化的LV受益于环孢菌素,但用来自狒狒内源性逆转录病毒(BaEVTR)的经修饰的包膜糖蛋白对LV进行假型化使载体对CsA和CsH两者的有益效果不敏感。令人感兴趣的是,已经提示了用BaEVTR包膜糖蛋白对LV进行假型化与VSV-g假型化的载体相比改善人CD34+细胞中的LV转导效率,包括在未刺激的HSPC中(Girard-Gagnepain,A.et al.(2014)Blood 124:1221-1231)。基于本文呈现的结果并牢记CsH对未刺激的HSPC中的LV转导也有积极影响,此种差异也可以至少部分地由VSV-g假型化LV对人HSPC中环孢菌素敏感性限制阻断的易感性解释。
总之,我们的结果指示环孢菌素在LV生命周期的早期以及在载体整合的水平上均起作用,并且CsA的早期效果可能被其破坏当取而代之使用CsH时保留的有益的LV壳体-CypA相互作用的能力所掩盖。由于这两种环孢菌素与另两种早期作用化合物雷帕霉素和PGE2(已经提示了这两者在VSV-g介导的进入水平起作用((Wang,C.X.et al.(2014).Rapamycin relieves lentiviral vector transduction resistance in human andmouse hematopoietic stem cells.Blood;Zonari,E.et al.(2017).Efficient Ex VivoEngineering and Expansion of Highly Purified Human Hematopoietic Stem andProgenitor Cell Populations for Gene Therapy.Stem Cell Reports))是加性的,尚不清楚内吞途径的参与。此外,仅内吞进入并不允许载体受益于HSPC中的环孢菌素,因为这些化合物未改善AAV6载体转导。CsA和CsH彼此不是加性的,提示这两种同种型作用于限制HSPC中的LV转导的相同途径。此外,CsA对壳体-CypA相互作用的负面影响似乎占主导,因为组合两个环孢菌素获得与单独的CsA相似的转导水平,进一步强调了在HSPC中的LV转导期间此种宿主-载体相互作用的重要性。
CsH的最为表征的宿主靶标是甲酰基肽受体1(FPR1)(de Paulis,A.et al.(1996)J Allergy ClinImmunol 98:152-164)。FPR1属于调节先天免疫应答的模式识别受体(PRR)的N-甲酰基肽受体(FPR)家族(Prevete,N.et al.(2015)Pharmacol Res 102:184-191)。FPR表达最初在免疫细胞中描述,随后在非造血细胞和某些组织中描述,并且最近已显示它们参与神经干细胞分化(Zhang,L.et al.(2017)Sci Rep 7:206)。尽管到目前为止尚未有关于FPR1参与HSPC生物学的报告,但它可以触发的一些信号传导级联,例如PI3K-AKT信号传导在HSPC中确实起着至关重要的作用(Lechman,E.R.et al.(2012)Cell Stem Cell 11:799-811)。然而,FPR1不参与HSPC中环孢菌素介导的LV限制的缓解。
与其更宽的细胞范围和还增加IDLV转导效率的能力一致,CsH可能会允许改善的HSPC和原代人CD4+T细胞中的基因靶向以及IDLV剂量的按比例降低,而不损害效率。这些结果使基于IDLV的编辑平台成为基于AAV或寡核苷酸介导的供体DNA递送(Schiroli,G.etal.(2017)Sci Transl Med9:411)的其他方法的竞争性替代方案。特别地,关于AAV暴露的HSPC的免疫原性存在担心,这是由于患者经常含有针对此基因疗法载体的先存的适应性免疫。与基于IDLV的编辑平台组合的CsH可以为此部分的可鉴定的患者提供解决方案,并显著受益于一大批前沿治疗方法,包括免疫疗法和单基因造血疾病的基因校正(Naldini,L.(2015)Nature 526:351-360)。令人感兴趣的是,CsH似乎增加靶向效率,特别是在更原始的HSC中,以前显示更原始的HSC比定型祖细胞对基因靶向程序的细胞毒性更敏感,并且在进行HDR上不太熟练(proficient),这可能是由于它们的静止或慢循环(Genovese,P.et al.(2014)Nature 510:235-240)。对基因递送的增加的允许性允许使用低得多的供体载体剂量,从而也降低响应基因靶向程序的生长停滞和凋亡。我们改进的基于CsH的基因编辑方案将允许增加靶定HSPC的潜在限制数量,从而能够对人中特定基因缺陷进行有效和安全的矫正。此外,随着改进的用于离体HSC扩增的程序变得可用,也包括增强供体递送的组合方法可以增加基因靶向细胞的总产率。实际上,PGE2已经用于HSPC基因编辑背景中以离体保存HSPC(Genovese,P.et al.(2014)Nature 510:235-240)。CsH添加具有进一步改善基因编辑细胞的总产率的潜力。此外,在更规范的基因疗法方案的背景下将CsH与PGE2组合也具有在治疗载体特别低效或需要高拷贝数才能实现治疗功效的情况下显著改善临床结果。
总体上,我们已经鉴定了一种新型环孢菌素,其能够在原代人HSPC和T细胞中更有效,更安全地抵消基础以及I型IFN诱导的LV限制两者。揭示环孢菌素改善LV转导和供体递送的机制将对基因疗法和编辑应用产生重大影响。此外,我们发现环孢菌素似乎还在更宽的细胞类型范围中抵消I型IFN诱导的抗病毒效果,这可以有助于更好地理解能够减少HIV-1感染的先天免疫机制。

Claims (38)
1.环孢菌素(cyclosporin)H(CsH)用于增加通过病毒载体转导分离的细胞群体的效率和/或增加通过病毒载体转导时分离的细胞群体的基因编辑的效率的用途,其中对所述病毒载体进行假型化以通过内吞依赖性机制进入细胞,和/或其中所述病毒载体是VSV-g假型化载体。
2.权利要求1的用途,其中所述细胞是:
(a)造血干细胞和/或祖细胞;或
(b)T细胞,任选地CD4+和/或CD3+T细胞。
