Abstract
Description
本出願は、参照によりその全体が本明細書に組み込まれる2016年12月29日に出願された米国特許仮出願第62/440,305号の利益を主張する。
本発明は、遅発型小児性神経セロイドリポフスチン沈着症などの神経変性疾患を治療するための方法に関する。
別途定義しない限り、本明細書で用いられる全ての技術的および科学的用語は、当業者によって一般的に理解されるものと同じ意味を有する。矛盾する場合、定義を含む本明細書が制御するものとする。好ましい方法および材料を以下に記載するが、本明細書に記載のものと類似するか、または等価な方法および材料を、本発明の実施または試験において用いることができる。本明細書に記載の全ての刊行物、特許出願、特許および他の参考文献は、その全体が参照により組み込まれる。本明細書に開示される材料、方法、および実施例は、例示に過ぎず、限定を意図するものではない。
本明細書では、神経変性疾患を治療する方法が提供される。神経変性疾患は、神経セロイドリポフスチン沈着症、アルツハイマー病、ハンチントン病、筋萎縮性側索硬化症(ALS)、多系統萎縮症(MSA)などのパーキンソン・プラス疾患を含むパーキンソン病、進行性核上性麻痺(PSP)、大脳皮質基底核変性症(CBD)またはレビー小体型認知症(DLB)であってよい。バッテン病は、神経セロイドリポフスチン沈着症(NCL)と呼ばれる障害群の最も一般的な形態である。神経セロイドリポフスチン沈着症(NCL)は、小児性NCL(INCL)、遅発型小児性神経セロイドリポフスチン沈着症(LINCL)、若年性NCL(JNCL)、または成人性NCL(ANCL)であってよい。神経変性疾患は、リソソーム蓄積障害によって引き起こされ得る。リソソーム蓄積障害は、例えば、テイサックス病、ファブリ病、ナイマン・ピック病、ゴーシェ病、ハンター症候群、アルファ-マンノシドーシス、アスパルチルグルコサミン尿症、コレステリルエステル蓄積症、慢性ヘキソサミニダーゼA欠損症、シスチン蓄積症、ダノン病、ファーバー病、フコシドーシス、またはガラクトシアリドーシスであってよい。
薬剤は、フィブラートなどの脂質低下薬であってもよい。フィブラートの非限定例としては、ゲムフィブロジル、フェノフィブラート、クロフィブラート、ベザフィブラート、シプロフィブラートおよびクリノフィブラートが挙げられる。ゲムフィブロジル(5-(2,5-ジメチルフェノキシ)-2,2-ジメチルペンタン酸)は、Pfizerによる商標Lopid(登録商標)の下で商業的に入手可能である。フェノフィブラート(2-(4-(4-クロロベンゾイル)フェノキシ)-2-メチル-プロパン酸1-メチルエチルエステル)は、AbbvieによるTricor(登録商標)として商業的に入手可能である。さらなるフィブラートとしては、クロフィブラート(2-(4-クロロフェノキシ)-2-メチル-プロパン酸エチルエステル)、ベザフィブラート(2-(4-(2-クロロ-ベンゾイルアミノ)-エチル)フェノキシ)-2-メチル-プロパン酸)、シプロフィブラート(2-(4-(2,2-ジクロロシクロプロピル)フェノキシ)-2-メチルプロパン酸)およびクリノフィブラート(2-[4-[1-[4-(2-カルボキシブタン-2-イルオキシ)フェニル]シクロヘキシル]フェノキシ]-2-メチルブタン酸)が挙げられる。
薬剤を、対象(ヒトまたは非ヒトであってもよい、患者など)への投与にとって好適な医薬組成物中に組み込むことができる。
遅発型小児性神経セロイドリポフスチン沈着症などの神経疾患を治療する方法は、薬剤または薬剤の医薬組成物を投与する任意数の様式を含んでもよい。投与様式は、錠剤、ピル、糖衣錠、硬質および軟質ゲルカプセル、顆粒、ペレット、水性、液体、油性または他の溶液、水中油エマルジョンなどのエマルジョン、リポソーム、水性または油性懸濁液、シロップ、エリキシル剤、固体エマルジョン、固体分散剤または分散性粉末を含んでもよい。経口投与のための医薬組成物の調製のために、薬剤を、例えば、アラビアゴム、タルカム、スターチ、糖(例えば、マンニトース、メチルセルロース、ラクトースなど)、ゼラチン、界面活性剤、ステアリン酸マグネシウム、水性もしくは非水性溶媒、パラフィン誘導体、架橋剤、分散剤、乳化剤、潤滑剤、保存剤、香料(例えば、エーテル油)、溶解促進剤(例えば、安息香酸ベンジルもしくはベンジルアルコール)または生体利用率増強剤(例えば、Gelucire.TM.)などの一般に公知であり、使用されるアジュバントおよび賦形剤と混合することができる。医薬組成物中では、薬剤を、微粒子、例えば、ナノ粒子状組成物中に分散させることもできる。
材料および方法:
試薬および抗体:凍結脳切片上でのTUNELアッセイのための試薬を、EMD Millipore (Billerica、MA)から購入し、製造業者のプロトコールに従って実験を行った。免疫ブロット(IB)のためのブロッキングバッファーおよび二次抗体(IRDye 700またはIRDye 800標識)を、Licor (Lincoln、NE)から購入した。免疫組織化学(IHC)のための二次抗体(FITCまたはCy5標識)を、Jackson ImmunoResearch (West Grove、PA)から購入した。本研究において用いた一次抗体の供給源をその適用および希釈率と共に表1に列挙する。
gem処置はCln2(-/-)マウスにおける寿命を延長させる
Cln2(-/-)マウスは、LINCLに対する新しい治療手法を試験するための重要な動物モデルとして役立つ(Cabrera-Salazarら、2007、Sleatら、2008、Changら、2008、Sleatら、2004)。通常、LINCLは急速に進行し、8〜10歳の間に死に至る(Soharら、1999、Sleatら、1997)。同様に、Cln2(-/-)マウスもまた、140日以内に死亡する(Sleatら、2004)。したがって、第1に、本発明者らは、経口gem処置がCln2(-/-)マウスの寿命を増加させることができるかどうかを検査した。以前に、本発明者らは、経口投与後、gemがCNSに進入することを証明した(Dasguptaら、2007)。マウスを、4週齢から強制摂取によりgem(7.5 mg/kg体重/日)で毎日処置した。gemを0.1%メチルセルロース中に溶解したため、1群のCln2(-/-)マウスにも、ビヒクルとして0.1%のメチルセルロースを投与した。未処置のCln2(-/-)オスおよびメスマウスは、95日目から死に始め、137日以内に、全てのCln2(-/-)マウスが死亡した(図1A)。生存に関しては、本発明者らは、オスとメスのCln2(-/-)マウスの間でいかなる差異も見出さなかった。しかしながら、gem処置したCln2(-/-)マウスは、204日目まで生存し、これは、gemが2ヶ月を超えてCln2(-/-)マウスの寿命を増加させることができることを示唆している(図1A)。他方、全てのビヒクル処置マウスが、150日以内に死亡した(図1A)が、これは、ビヒクルのみによる非常に軽度の保護を示唆している。これらの結果はまた、各マウス群の平均生存日数によっても支持される(図1B)。
gem処置はCln2(-/-)マウスにおける運動行動を改善する
寿命の増加と共に、LINCL患者のための神経保護の別の治療目標は、機能障害を減少させることである。したがって、gemがCln2(-/-)マウスにおける寿命を増加させるだけでなく、運動行動も改善するかどうかを検査するために、本発明者らは、自発運動をモニタリングした。自発運動を、gem処置後8週間モニタリングした。Cln2(-/-)マウスは、WTマウスと比較して水平活動(図2A)、移動時間(図2B)、移動回数(図2C)、総移動距離(図2D)、および常同行動数(図2E)の顕著な減少を示した。他方、休止時間は、WTマウスよりもCln2(-/-)マウスにおいてより多かった(図2F)。しかしながら、gemの経口投与は、Cln2(-/-)マウスにおける自発運動を有意に改善した(図2A〜F)。
gem処置はCln2(-/-)マウスの脳における貯蔵物質量を低下させる
LINCLを含むリソソーム蓄積障害の一般的な特徴の1つは、脳を含む全ての組織における自己蛍光封入体の蓄積である(Boustany 2013、Hachiyaら、2006)。最近、本発明者らは、ゲムフィブロジルがTFEBのPPARα媒介性転写上方調節を介してリソソーム生合成を刺激することができることを詳述した(Ghoshら、2015)。gem処置はCln2(-/-)マウスの寿命を延長し、運動行動を改善するため、本発明者らは、gem処置が運動皮質中、in vivoで貯蔵物質量を減少させることができるかどうかを検査した。予想通り、本発明者らは、WTマウスと比較して、Cln2(-/-)マウスの運動皮質においてミトコンドリアATPシンターゼ(SCMAS)蓄積のサブユニットcの顕著な増加を観察した(図3A〜B)。しかしながら、Cln2(-/-)マウスのgem処置は、SCMASの有意な減少をもたらした(図3A〜B)。ビヒクル処置は貯蔵物質のそのような減少をもたらさなかったため、これらの結果は特異的であった(図3A〜B)。
gem処置はCln2(-/-)マウスの脳における神経細胞アポトーシスを防止する
他の神経変性障害において見られるように(Cotman & Anderson 1995、Saha & Pahan 2006)、アポトーシスはまた、LINCLにおける神経変性も担っている(Dharら、2002、Puranamら、1997、Laneら、1996)。したがって、本発明者らは、gem処置がCln2(-/-)マウスのCNSにおいて神経細胞アポトーシスを抑制することができるかどうかを検査した。NeuN陽性ニューロンがCln2(-/-)マウスの運動皮質においてアポトーシスを受けたことが図4A〜Bから明らかである。
gemはCln2(-/-)マウスの脳における抗炎症因子のレベルを増加させる
本発明者らは、gemが抗炎症性であり(Jana & Pahan 2012、Janaら、2007、Pahanら、2002)、gemが様々な脳細胞において、抗炎症因子であるSOCS3およびIL-1Raのレベルを増加させる(Corbettら、2012、Ghosh & Pahan 2012b)ことを証明した。したがって、ここで、本発明者らは、gem処置が、Cln2(-/-)マウスの脳においてin vivoでこれらの抗炎症分子を上方調節することができるかどうかを精査した。12週齢で、本発明者らは、週齢を一致させたWTマウスと比較して、Cln2(-/-)マウスの運動皮質(図6A〜C)および線条体(図6D〜F)においてSOCS3およびIL-1Raの減少を観察した。しかしながら、ビヒクルではなく、gemによる経口処置の8週後に、Cln2(-/-)マウスの運動皮質と線条体との両方において、SOCS3(図6A、B、DおよびE)およびIL-1Ra(図6A、D、CおよびF)の増加が見られた。これらの知見をさらに確認するために、次に、本発明者らは、線条体切片において二重標識免疫蛍光を実施した。本発明者らは、WTマウスと比較してCln2(-/-)マウスの線条体切片においてより多くのアストログリア(図7A)およびミクログリア(図7B)を観察したが、後者と比較して前者において、SOCS3の喪失があった(図7A〜C)。しかしながら、ウェスタンブロットの結果と同様、gem処置の後にCln2(-/-)マウスの線条体においてSOCS3の顕著な増加が見られた(図7A〜C)。SOCS3のこの増加は、アストロサイト(図7AおよびD)、ミクログリア(図7BおよびE)ならびに他の脳細胞(図7)において可視的であった。SOCS3と同様、本発明者らは、WTマウスと比較してCln2(-/-)マウスの線条体におけるIL-1Raの喪失にも気付いた(図8A〜E)。しかしながら、ビヒクルではなく、gemによるCln2(-/-)マウスの処置は、線条体におけるIL-1Raの回復および/または上方調節をもたらした(図8A〜E)。また、Cln2(-/-)マウスの線条体におけるin vivoでのIL-1Raのgemにより誘導される増加は、アストロサイト(図8AおよびD)とミクログリア(図8BおよびE)との両方において見られた。興味深いことに、Cln2(-/-)マウスの運動皮質と線条体との両方において、本発明者らは、gem処置後にアストログリアまたはミクログリアのいかなる減少も観察しなかった(図7および8)が、これは、一度、アストログリア生成またはミクログリア生成がCln2(-/-)マウスにおいて見られたような慢性神経変性状態の脳においてin vivoで生じたら、ゲムフィブロジル処置は、その数を減少させることなく、抗炎症モードに向かってその機能をモジュレートし得るに過ぎないことを示唆している。
Cln2の突然変異は、TPP1酵素の機能の欠損および/または喪失をもたらし、最終的には、LINCLを引き起こす(Bellettato & Scarpa 2010、Walusら、2010、Soharら、1999)。酵素補充療法および遺伝子療法の臨床試験が進行中であるが、確立された薬物媒介療法は、LINCLについては現在のところ利用可能ではない。したがって、LINCL患者の疾患進行を遅延させる、運動機能を改善する、およびその生存を増加させるための神経保護的治療手法の開発は、最重要である。Cln2(-/-)マウスは、LINCLのための新しい治療戦略の決定および新規薬物の効能の試験において有用である。ここで、本発明者らは、ヒトにおける高脂血症のためのFDAに認可された薬物であるゲムフィブロジル(gem)が、Cln2(-/-)マウスの運動機能を改善し、その寿命を延長することを初めて証明する。最近、本発明者らは、ゲムフィブロジルが、培養されたマウスおよびヒトの脳細胞ならびにマウスの脳においてin vivoで、PPARα/RXRα経路を介して、TPP1を上方調節することができることを詳述した(Ghoshら、2012)。しかしながら、Cln2(-/-)マウスにはTPP1は存在しない。したがって、この場合、gemは、Cln2(-/-)マウスにおいて疾患進行を遅延させるために、TPP1に依存しない機構を用いている。
Claims (22)
- 対象に、治療有効量の、神経細胞アポトーシス細胞死を減少させる薬剤であって、フィブラートを含む前記薬剤を含む組成物を投与することを含む、神経変性疾患を有する対象における神経細胞アポトーシス細胞死を減少させる方法。
- フィブラートが、ゲムフィブロジル、フェノフィブラート、クロフィブラート、ベザフィブラート、シプロフィブラート及びクリノフィブラートから成る群より選択される、請求項1記載の方法。
- フィブラートがゲムフィブロジルである、請求項1記載の方法。
- 神経変性疾患が、神経セロイドリポフスチン沈着症、アルツハイマー病、ハンチントン病、筋萎縮性側索硬化症(ALS)、多系統萎縮症(MSA)などのパーキンソン・プラス疾患(Parkinson's plus disease)を含むパーキンソン病、進行性核上性麻痺(PSP)、大脳皮質基底核変性症(CBD)及びレビー小体型認知症(DLB)から成る群より選択される、請求項1〜3のいずれか1項記載の方法。
