以下の供給源からの化合物をそれ以上精製せずに使用した:D−グルコース(99.5%、Sigma−Aldrich、St.Louis、Missouri、米国)、2−デオキシ−D−グルコース(98%、Alfa Aesar、Ward Hill、Massachusetts、米国)、2−デオキシ−2−フルオロ−D−グルコース(99%、Synquest Laboratories、Alachua、Florida、米国)、4−アセトアミド−2,2,6,6−テトラメチルピペリジン1−オキシル(TEMPO;98%、Sigma−Aldrich)、臭化ナトリウム(分析グレード、Malinckrodt、Paris、Kentucky、米国)、水酸化ナトリウム(食品グレード、Mallinckrodt)、重炭酸ナトリウム(実験室グレード、Sigma Aldrich)、イソプロテレノール(ISO;Sigma−Aldrich)、および次亜塩素酸ナトリウム溶液(14%有効塩素、Alfa Aesar)。18F−FDGは、University of Oklahoma−Nuclear Pharmacyから購入した。
190nmに設定したRainin Dynamax UV−1吸光度検出器(Mettler Toledo、Columbus、OH、米国)およびBioscan B−FC−3300(Bioscan、Washington DC、米国)の放射能検出器とインターフェースで接続したBeckman System Gold 128 Solvent Module(Beckman Coulter、Brea、CA、米国)を使用してHPLCを実施した。反応生成物を70℃に温めたPhenomenex Rezex ROA−Organic Acid H+(8%)カラム(300×7.8mm)上で分離した。移動相は、毎分0.5mlの流速で0.025mMのH2SO4からなっていた。
該当する場合は、反応物を、60オングストローム孔径の200μmアルミニウム裏打ちのSi60シリカプレート(EMD Millipore、Darmstadt、ドイツ)上のTLCによってモニターした。展開溶媒は、7部のn−ブタノール、2部の氷酢酸、および3部の水からなっていた。分離した成分を、ヨウ素で染色することにより、または37%HCl1ml、アニリン2ml、85%H3PO310ml、ジフェニルアミン2gおよび酢酸エチル100mlの混合物を使用して可視化し、ホットプレート上で200℃で展開した。このような条件下で、4−アセトアミド−TEMPO、2−デオキシ−D−グルコース、および2−デオキシ−D−グルカル酸のRf値は、それぞれ0.62、0.4、および0.28であった。放射性反応物は、Bioscan mini−scan 1000(Bioscan、Washington DC、米国)で分析した。
1H NMRスペクトルおよび13C NMRスペクトルを、Mercury−VX 300およびVarian VNMRS−400 NMR分光計で300MHzおよび75MHzで記録し、Mnova(Santiago de Compostela、スペイン)を使用して処理した。スペクトルは残留プロトン化溶媒を基準とした。化学シフトおよびカップリング定数は、それぞれδ百万分率(ppm)およびヘルツ(Hz)で報告した。
本発明者らは以前に報告のTEMPOベースの方法を修正して、ナノスケールでの18F−FGAの合成を容易にした。4−アセトアミド−TEMPO(8mg、0.038mmol、0.2当量)およびNaBr(80mg、0.77mmol、5当量)を含有する5mlの丸底フラスコに、D−グルコース(0.166mmol)、2−デオキシ−D−グルコース(0.187mmol)、または2−デオキシ−2−フルオロ−D−グルコース(0.182mmol)をおよそ30mg添加した。1M NaHCO3緩衝液(pH11.6)およそ3mlを添加し、混合物を室温で5分間撹拌した。反応混合物を氷上でさらに3分間インキュベートすることにより0〜2℃に冷却した。NaOCl(14%溶液、0.75ml、1.69mmol、10当量)を2分間にわたって氷冷反応混合物に少しずつ添加した。残留酸化剤の存在についてKIストリップで反応をモニターした。酸化剤が完全に消費されたら、反応混合物を氷冷エタノール40mlと急速に混合し、続いて遠心分離(5,000rpmで5分間)して沈殿物を収集した。沈殿物を氷冷エタノールで洗浄し、100℃で一晩乾燥した。生成物をNMRおよびHPLC分析に供した。1H NMR(D2O、300MHz):δ4.38(br s、2H、H1、H4)、4.26(br s、1H、H3)、4.17(br s、1H、H2)。13C NMR(D2O、75MHz):δ178.65、178.60(C6、C1)、73.85、73.67、73.57、71.61(C2、C3、C4、C5)。1H−NMRおよび13C−NMRは、文献に引用されているものと一致した。
本発明者らは、酸化グルコースのマクロスケール合成について標準化された手順を適応させた。簡潔にいえば、4−アセトアミド−TEMPO(0.8mg)、NaHCO3緩衝液(pH11.6、1ml)、NaBr(8mg)、および18F−FDG(0.25〜0.5ml、約20mCi)の混合物を5mlの反応バイアル中で0〜2℃に冷却した。およそ20μlの14%NaOClを混合物に添加して反応を開始させた。反応の進行を、ラジオ−TLC向けに反応混合物5μlをサンプリングすることによってモニターした。反応が完了したら、混合物を氷冷エタノール10mlに移し、続いて遠心分離(5000rpm×5分)した。沈殿物を氷冷エタノールで1回洗浄し、中和のために2M HCl200μlを添加し、混合物を注射用水3mlに溶かした。透明な溶液を0.2μmシリンジフィルターを通して濾過滅菌した。最終生成物を、上記のラジオ−TLCにより分析した(18F−FDGのRf=0.65および18F−FGAのRf=0.2)。
本明細書に記載の全ての動物実験は、NIH Animal Use and Care Guidelinesに準拠して行われ、所内IACUCによって承認されたものである。9匹のオスおよび3匹のメスCD1マウス(20〜36g)をHarlan Laboratories(Indianapolis、IN)から入手し、12時間の昼夜サイクルの管理された環境に収容した。マウスを少なくとも1週間順応させた後に、研究に組み入れた。イメージングの日に、動物を酸素気流に含まれる2〜3%イソフルランで麻酔した。18F−FGAおよそ100μCiを尾静脈に静脈内注射した。動物を、生体内分布まで吸収パッドを備えたケージに入れた。簡潔にいえば、マウスを過剰用量のイソフルラン(4%)、それに続く頸椎脱臼によって、注射後1時間後または3時間後に安楽死させた。様々な臓器を摘出し、食塩水で洗浄し、秤量し、適切な組織試料を自動ガンマカウンター(Packard Cobra II Auto Gamma、Perkin Elmer、Boston、MA)でカウントした。総血液量、骨、および筋肉量はそれぞれ体重の5.7%、10%、40%と推定した。注入した18F−FGAの希釈試料を比較基準として供した。
5匹のオスのCD−1マウスに、18F−FGA100μCiを尾静脈を介して静脈内注射した。注射後0、30、60、90、120、および180分に、眼窩後洞から血液25〜50μlをサンプリングした。血液を秤量し、自動ガンマカウンター(Packard Cobra II Auto Gamma、Perkin Elmer、Boston、MA)でカウントした。剖検時の異常な腎臓の形態についての肉眼的観察に基づいて、1匹のマウスを研究から除外した。
オスのSprague Dawley系ラット(250〜300g)をHarlan(Indianapolis、IN、米国)から購入し、通常の明/暗サイクルで収容し、実験に先立ち少なくとも5日間順応させた。ISOの滅菌水溶液を用量割合150mg/kg体重で投与することによって心筋損傷を誘発した。薬物を2日連続して腹腔内注射した。
イソフルラン麻酔ラットの第1誘導ECGを、右前足に負電極、左前足に正電極、および左後足に接地リードを配置することによって記録した。電極をリード線(CB Sciences C−ISO−255)、生体アンプ(ETH−225)、およびアナログ−デジタル転換器(iWorx 118)に固定し、Labscripe 2.0ソフトウェア(iWorx、Dover、NH)を使用してシグナルを記録した。生データを解析する前に、シグナルをフィルタして60Hzの電源周波数を除去した。
損傷後にラットを3回のイメージングセッション:第3日目に18F−FGAイメージング、続いて第4日目に99mTc−セスタミビイメージングおよび18F−FDGイメージングに供した。損傷後のイメージングに加えて、ISO損傷を誘発する前にベースライン画像も取得した。18F−FGA、99mTc−セスタミビ、および18F−FDGの用量は、それぞれ、1mCi(0.5〜1ml)、2.5mCi(0.5〜1ml)、および0.1mCi(0.2〜0.4ml)とした。全ての注射は、麻酔した(2%イソフルラン−酸素混合物)ラットの尾静脈内に静脈内で投与し、イメージングについては注射後、示されてある時間に実施した。イメージング期間を通して、2%イソフルラン−酸素混合物で麻酔を維持した。PETについては、ラットを、PET−CTデュアルモダリティマシン(Gamma Medica Ideas、Northridge、CA)のガントリにおいて背臥で配置した。胸部領域のフライモードCTは、20分のリストモードPETデータ取得の前に取得した。SPECTイメージングは、それぞれ60秒の24フレームで、胸部領域のヘリカルSPECT取得により、NanoSPECTマシーン(Trifoil Imaging、Chatsworth、CA)で実施した。イメージングの後、ラットを覚醒し、安楽死の時まで彼らのケージで飼育した。
安楽死の前に血液試料を採取し、血漿を遠心分離(5,000rpmで5分間)によって分離した。本発明者らは、Life Diagnostics(West Chester、PA)から入手したラット特異的酵素結合免疫アッセイキットを使用することによって血漿中の心臓のTnI(cTnI)のレベルを決定した。推定前に、血漿試料をPBSで1:2希釈を行った。
本発明者らは、Cayman Chemicals(Ann Arbor、Michigan)製のELISAキットを使用することによって、未希釈血漿中のストレスホルモンであるコルチコステロンの濃度を測定した。コルチコステロンはラットに存在する唯一のグルココルチコイドである。
イメージングおよび血液試料採取の最終セッションの後、ラットを安楽死させ、その心臓を直ちに採取し、−20℃で2時間凍結した。心臓を2mmのスライスに切片とし、37℃で間欠的に振盪しながらPBS中1%TTC溶液に置いた。30分の染色後、TTCを除去し、一晩10%緩衝ホルマリンに置き換えた。
統計学的比較のために、本発明者らはGraphPad Prism 6ソフトウェア(GraphPad、La Jolla、CA)を用いた。スチューデントのT検定を使用して2群比較を実施し、一元配置ANOVAを使用して3群以上の間の比較を行った。薬物動態学的パラメータは、残差法を使用して時間−活性曲線の片対数プロットから計算した。
絶食マウスをO2:N2混合物の気流(30:70、1L/分)に含まれる1.5%イソフルランで麻酔し、37℃で恒温ブランケットで覆った。左後頭部および上甲状腺動脈、外頸動脈(ECA)の枝、翼状動脈、内頸動脈(ICA)の枝を電気凝固し、切断する。総頸動脈をマイクロクリップによって閉塞する。左のECAを結紮し、凝固させ、頭蓋甲状腺動脈の遠位側で切断する。15mmのモノフィラメントナイロン縫合糸(6−0、熱で丸くなった先端の直径=0.2〜0.3mm)をECAに挿入し、その先端がMCAの起点を閉塞するまでICAを通して静かに進める。縫合糸の正しい配置は、左MCA領域の局所皮質血流が基底流の10〜20%に急落することによって示される。CBFを、非侵襲的レーザードップラーフローメトリー(Moor Instruments)によってモニターする。>80%の持続的な低下によってMCAOの成功が示される。モノフィラメントを結紮糸で定位置に固定し、皮膚切開部を閉じる。モノフィラメント縫合糸は、永久MCAOについては留置するが、一過性MCAOについては、マウスを麻酔し創傷を再び開くことにより1時間後に閉塞縫合糸を除去する。
グルコースからのグルカル酸の合成
TEMPO/NaBr/NaOCl系を使用してグルコースをグルカル酸に酸化した(図1のスキーム1を参照されたい)。反応の進行は、KIストリップで読み取った酸化剤の消費によってモニターした。酸化反応は一般に、反応混合物にNaOClを添加して20分以内に完了した。酸化剤の濃度の増加によって、反応速度の上昇がもたらされたが、これによってまた、グルコースを望ましくない開裂生成物へと過剰酸化する傾向が高まるという結果となった。グルコースおよびグルカル酸は、それぞれ16分および12分の保持時間で、ROAカラム上のHPLCによって効果的に分離された(図2a〜c)。ブタノール:水:酢酸(7:3:2v/v)TLC系において、グルコースおよびグルカル酸は、それぞれ、0.62および0.28のRf値を示した(図2d)。最適条件下で実施された反応のエタノール沈殿生成物は、排他的にグルカル酸を示した1H NMRによって特徴付けられた。任意の副生成物、TEMPOまたは残留グルコースは上清中に残った。沈殿物の重量分析が示したところによると、等濃度のHClの添加により中和することができた過剰の重炭酸ナトリウムもグルカル酸とともに沈殿した。
本発明者らは、スキーム1の合成プロセスを適合させて市販の18F−FDGから18F−FGAを製造した(図3のスキーム2を参照されたい)。濃い重炭酸塩緩衝液の使用によって、pHをモニターし調整する必要なしに反応を進行させることが可能となった。酸化試薬は比例的に減少させ、反応は鉛遮蔽箱の内部で実施した。ラジオ−TLCにより完了について反応をモニターした(図4a〜b)。ラジオ−HPLCでモニターした反応の時間経過に示すとおり(図5a〜d)、反応は非常に急速に進行し、18F−FDGは、漂白剤(NaOCl)の添加後約3分以内に完全に消費された。反応を6分まで延長しても、望ましくない副生成物の生成はもたらされないと思われた。このシリーズの最終クロマトグラムによって、エタノール沈殿、遠心分離、洗浄、および注射用水へのペレットの再溶解後の18F−FGAのプロファイルが示されている(図5e)。本発明者らは通常、崩壊補正なしで50%超の収率を得ており、合成、精製、およびTLC品質管理についてはルーチンで1時間以内に完了した。
注射の1時間後および3時間後の正常マウスの様々な臓器における18F−FGAの全身分布分析の結果を表1に示す。正常な健康なマウスに18F−FGA(0.1mCi)を尾静脈を介して注射し、マウスを注射後の1時間目および3時間目に安楽死させた。注射された放射能の大部分は腎臓系を介して排泄されることがわかった。その他の全ての臓器には、注射用量/g組織の0.5%未満が蓄積した。循環中の18F−FGAの濃度もまたごく少量であった。これらの結果が示唆したところによると、18F−FGAは身体から急速にクリアされ、心筋イメージングを曖昧にする可能性を持つ組織である肝臓、肺、または骨に蓄積しない。
18F−FGAの循環動態について3時間にわたって研究した。図6に示すとおり(左パネル)、30分までに、注射された用量の99%超が循環から除去された。時間−活性関係の消失相から、本発明者らは半減期をおよそ35分、消失速度定数を0.83h−1であると計算した。薬物動態学的パラメータを30〜180分の時点から計算したが、表様式で提示する(図6、右パネル)。
図7aに示すとおり、2日間連続してISO注射した後、エバンスブルーで灌流した代表的な全心臓およびTTC染色した心臓スライスの切片は、広汎性かつ広範囲の壊死の区域(白色領域)を示した。ISO処理に応じた、これらの動物における第I誘導ECG記録を図7b〜cに示す。ECGの変化は全てのISO処理動物で起こったが、ISO処理は非特異的な壊死領域を生じるので、変化についてはラット全体にわたって重度または欠損が首尾一貫していなかった。しかし、ISO処理の同日に、R−R間隔は有意に減少し、心拍数の増加を示した(図8c)。ISO処理の経過にわたって、ST期間もまた増加した(図8d)。このことによって心室再分極が遅延したことが示されたが、これは心臓毒の投与の結果として起こることが知られている、プルキンエ線維中の活動電位の延長が原因とすることができる。
ISOで処理する前および2日間連続してISO処理した後、ラットを18F−FGAで画像化した。結果を図9に示す。本発明者らは、正常(ベースライン)心筋組織においてごくわずかな取込みを見いだしたが、ISO処理ラットの1時間の画像において、心臓組織は大量の注射した18F−FGAを蓄積していた。心筋組織と非標的組織とにおける18F−FGAの蓄積の間のコントラストは、注射後の4時間目にイメージングを繰り返した場合、非常に顕著になった。対照ラットおよびISO処理ラットにおける18F−FGAイメージングの日の翌日に、ラットを、臨床的に利用されている2種のイメージング剤、即ち99mTc−セスタミビおよび18F−FDGによっても画像化した。図10に18F−FDG注射の1時間後のPET画像を示し、図11に99mTc−セスタミビ投与の1時間後のSPECT画像を示す。99mTc−セスタミビ−SPECTでは、ISO誘発性の心筋病変を描出することができなかった(図11)。一方、18F−FDG−PETでは、ISO処理ラットにおいて18F−FDGの心筋蓄積の減少を示した。画像のROIベースの解析に基づいて、18F−FDG取込みの減少は、対照ラットとISO処理ラットとの間でおよそ48%であった。
本発明者らは、0.1mCi(50μl)の18F−FGAを正常な健康なマウス(n=8)に注射し、注射後1時間目にカウント向けに臓器を採取した。脳の蓄積は約0.1%未満であり、全ての主要臓器において蓄積は注射用量の0.5%を下回り、18F−FGAは腎臓系を介してクリアされた。臨床的に使用される脳血流製剤、99mTc−HMPAO(Ceretec)および18F−FDGが脳内に著明に蓄積することは留意すべきことであり、このことが、画像の読影に重大な問題を起こす。健康な脳組織における急速なクリアランスおよび蓄積の非存在によって、明瞭な梗塞イメージング向けに18F−FGAが有効であることが、示された。
本研究では、18F−FGAは、中大脳動脈閉塞(MCAO)によって作出された脳卒中のマウスモデルの脳に蓄積することが、見いだされた。脳内梗塞のイメージング向けの18F−FGAを試験するために、本発明者らは永久MCAOのマウスモデルを用いた。PETをMCAO手術の2時間後に実施した。およそ1mCiの18F−FGAを注射し(i.v.)、注射の1時間後に20分の画像を取得した。同側は中央区域の18F−FGA蓄積を示したが、このコア領域周辺の灌流は有意に減少した。対側は正反対のシナリオを示した(図12、上)。本発明者らはまた、MCAO後1日目に、18F−FGA注射の2時間後に摘出した脳のイメージングを実施した。摘出脳のこの画像は、同側において、対側におけるよりも高い蓄積を明瞭に示した(図12、下)。
II.グルカル酸は、両末端がカルボン酸に酸化されているグルコースの誘導体である。これはまた、グルコース代謝の自然経過で身体によって産生され、米国FDAによってGRAS(一般に安全と認められている)であるとみなされている。グルカル酸は急性壊死の区域に蓄積する傾向があるが、想定されているところでは、なぜなら、これは瀕死状態にある組織中の露出した正帯電のヒストンタンパク質に対して親和性を有するからである。このキットを使用すると、ユーザーが市販のF−18−FDG(18F−FDG)を滅菌用量のF−18−FGA(18F−FGA)に転換することが可能となる。F−18−FDGは、がん、脳、および心臓病のイメージング向けに陽電子放射断層撮影(PET)センターによって広く使用されている。

注射用F−18−FDG(およそ0.2〜2mL中1〜50mCi)
キットは使用まで4℃で保管する必要がある。
F−18−FDGを成分Aバイアルに注入し、続いて直ちに成分Bを成分Aバイアルに注入する。穏やかに混合し5分間待った後に、成分Cの内容物を成分Aバイアルに注入する。回転により混合し、成分Aバイアルの内容物を滅菌注射器に吸引する。任意選択で、内容物は供給された0.2μMシリンジフィルターを通して濾過することができる。
放射化学的純度:別々の2つのTLCストリップ上に1滴の前駆体F−18−FDGおよび生成物F−18−FGAをスポットする(F−18−FGA製造の品質管理のためのTLCプロトコルと題する図[図14]を参照されたい)。スポットを1〜2分間乾燥させる。供給されたTLCストリップを40mmまで展開する。溶媒先端を鉛筆でマークする。起点から20mmでTLCストリップを切断し、ウェルカウンターで上部と下部の断片の放射能カウントを測定して、転換率を計算する。転換率の値は95%を超えているべきである。あるいは、ラジオ−TLCリーダーを使用して切断せずにストリップを読み取ることができる。
転換率=100×下部断片のカウント/(下部断片のカウント+上部断片のカウント)。
注射液のpH(6.5〜7.5):pHを確認するためにpHペーパーストリップ上に調製物の小滴(25〜50μL)を置く。
急性心筋梗塞(MI)は、世界中で何百万人もの死亡の原因である最も深刻な形態の心機能不全である。2010年における米国での入院の110万件超が、MIが原因であるとされた。迅速な診断はこれらの患者の良好な予後にとって重要である。MIの一次診断は、古典的な心電図検査(ECG)の変化に基づくことができるが、ECGは多くの場合決定的ではない。筋細胞壊死は全ての虚血性事象の最終結果であるので、末梢血中の心筋トロポニンの上昇が、壊死に関して汎用されるバイオマーカーとして浮かび上がってきた。しかし、トロポニンレベルが心臓の状態を正確に反映しない可能性がある多くの事例が存在する。高感度心筋トロポニンは、非ST部分上昇型MIの臨床診断を有する患者において安定であると報告されてきた。第二に、心筋炎および腎不全などのMI以外の状態のためにトロポニンレベルが上昇する場合もある。さらに、心筋トロポニンアッセイの感度が高まると、特異性の低下が必然的に伴う。
或る量の2−デオキシ−2−[18F]フルオロ−D−グルコース(18F−FDG)を酸化剤およびアルカリ性緩衝剤と合せて、反応混合物を形成するステップと;反応混合物を約10分未満の期間反応させて、実質的に全部の18F−FDGの、2−デオキシ−2−[18F]フルオログルカル酸(18F−FGA)への転換を引き起こすステップとを含む方法。
Claims (31)
- 2−デオキシ−2−[18F]フルオログルカル酸、またはその薬学的に許容される塩を含むイメージング剤。
- 薬学的に許容される担体、希釈剤、ビヒクル、または賦形剤中に配された、請求項1に記載のイメージング剤を含む組成物。
- 対象の陽電子放射断層撮影(PET)イメージングの方法であって、
2−デオキシ−2−[18F]フルオログルカル酸(18F−FGA、)、またはその薬学的に許容される塩を含むイメージング剤を前記対象に投与するステップと;
壊死を含有すると疑われる、前記対象の組織へと前記イメージング剤を浸透させるステップと;
前記壊死を含有すると疑われる前記組織のPET画像を収集するステップと
を含む方法。 - 或る量の2−デオキシ−2−[18F]フルオロ−D−グルコース(18F−FDG)から、前記18F−FGAまたはその薬学的に許容される塩を調製するステップを含み、前記投与するステップは、前記18F−FGAを調製後約3時間以内に行われる、請求項3に記載の方法。
- 前記対象は、がん、脳卒中、外傷性脳損傷、または心筋梗塞に起因する組織障害の疑いがある、請求項3に記載の方法。

- 前記18F−FDGを含有する容器をさらに含む、請求項18に記載のキット。
