がんは、体の他の部分に侵入するか、または広がる可能性のある異常な細胞増殖をともなう疾患の群である。がんは、概して、(例えば、特定の体の部分または臓器に関係した)患者の体の特定のエリアに局在し得るか、または全体に広がり得る腫瘍の形態で組織の異常増殖として現れる。特に、がんが体の他の部分に広がっていない場合、手術が多くの場合に完全な除去および治癒のための最大の機会を提供するので、一般的に、良性と悪性との両方の腫瘍が、手術介入を介して処置および除去される。しかし、いくつかの場合において、局所的環境からすべてのがん性組織を適切に除去するには手術のみでは不十分である。
特に、がんが転移していない場合、手術が多くの場合に完全な除去および治癒のための最大の機会を提供するので、良性と悪性との両方の腫瘍が、一般的に、手術介入を介して処置および破壊される。しかし、腫瘍が破壊された後、中空の空洞が残り得、この空洞の周辺にあり、元の腫瘍部位の周辺にある組織が、依然として、執刀医が摘出に失敗した、または摘出することができない異常な細胞、または、潜在的がん性細胞を残し得る。この周辺組織は、「周縁組織」または「周縁部組織」と一般的に呼ばれ、腫瘍の再発が最も起こりやすくなり得る患者内の場所である。
概観すると、本開示は、概して、医療デバイスおよびこの医療デバイスを使用する処置方法に関する。特に、医療デバイスは、組織の表面において、または切除空洞内において標的組織に刺激を印加するように、および、さらには、刺激によりマーカーの発現を上方制御されるマーカーを介して標的組織の疾患細胞(例えばがん細胞)を標的とした治療薬を送達するように構成されている。処置方法は、ある現象であって、その現象によりある特定の刺激が細胞における鑑別的な発現を誘起する現象を使用し得る。例えば、RFアブレーションは、疾患細胞が同じ対象の健康な、または正常な細胞において観測されないレベルに、ある特定の細胞表面タンパク質の発現を上方制御することをもたらし得る。したがって、医療デバイスは、組織をアブレーションするように構成され得る(アブレーションが刺激である)ことにより、組織の疾患細胞(例えばがん細胞)における細胞表面マーカーの発現を上方制御し、さらに、細胞表面マーカーを介して組織の疾患細胞を標的とした治療薬を送達する。

Claims (20)
- 疾患組織を処置するための医療デバイスであって、
第1の流体と治療薬とのうちの少なくとも1つを受容するための第1の流体ラインと、第2の流体を受容するための第2の流体ラインとを含む非導電性ハンドル、
前記ハンドルから延びる非導電性遠位部であって、前記遠位部が、前記第1の流体ラインと流体連通し、前記第1の流体ラインから導電性流体と前記治療薬とのうちの少なくとも1つを受容するように構成された内側チャンバを規定する固定の直径をもつ硬質球形本体を備え、前記硬質球形本体が、前記内側チャンバから前記硬質球形本体の外面までの前記第1の流体と前記治療薬とのうちの少なくとも1つの通過を可能にするように構成された複数のポートをさらに規定する、非導電性遠位部、
前記内側チャンバ内に置かれ、前記第2の流体ラインと流体連通し、前記第2の流体ラインからの第2の流体の送達に応答して、潰れた構成から拡張された構成に遷移するように構成された、拡張可能内部バルーン部材、ならびに
前記内側チャンバ内に置かれ、前記内部バルーン部材を取り囲む親水性挿入体であって、前記親水性挿入体が、前記内部バルーン部材の外面の少なくとも一部と前記硬質球形本体の内面との間において前記内側チャンバ内の適所に保持され、前記親水性挿入体が、前記複数のポートに対して前記第1の流体と前記治療薬とのうちの少なくとも1つを受容するように、および一様に分配させるように構成された、親水性挿入体
を備えるプローブ組立体と、
複数の独立した導電性ワイヤを備える電極アレイであって、各々が、前記硬質球形本体の前記外面に沿って位置する長さをもつ電極アレイと
を備える医療デバイス。 - 前記複数のワイヤの各々、または、前記ワイヤの組合せの1つまたは複数の組が、電流を受容し、標的組織のアブレーションのための前記複数のポートのうちの1つまたは複数を通る前記第1の流体により搬送されるエネルギーを伝導するように構成されている、請求項1に記載の医療デバイス。
- 前記親水性挿入体内に分配された前記治療薬をさらに含む、請求項2に記載の医療デバイス。
- 前記治療薬が、前記アブレーションにより上方制御されるマーカーを有する細胞に対して活性である、請求項3に記載の医療デバイス。
- 細胞表面マーカーが細胞表面タンパク質である、請求項4に記載の医療デバイス。
- 前記治療薬が、前記細胞表面タンパク質に結合する抗体を含む、請求項5に記載の医療デバイス。
- 前記マーカーがPD−1であり、前記治療薬が抗PD−1抗体を含む、請求項6に記載の医療デバイス。
- 前記治療薬が、前記アブレーションにより上方制御される酵素を有するタンパク質を含む細胞に対して活性である、請求項3に記載の医療デバイス。
- 前記酵素が、チミジンホスホリラーゼである、請求項8に記載の医療デバイス。
- 前記潰れた構成および前記拡張された構成、ならびに、前記潰れた構成と前記拡張された構成との間の複数の構成の間における前記内部バルーン部材の遷移を制御する能動的手段
をさらに備える、請求項2に記載の医療デバイス。 - 前記能動的手段が、標的組織に対するアブレーション手技中に第1の直径まで、かつ、前記標的組織への前記治療薬の送達のために第2のより大きな直径まで前記内部バルーン部材を膨張させるように動作可能である、請求項10に記載の医療デバイス。
- 前記内部バルーン部材が前記第2のより大きな直径まで膨張させられたとき、前記内部バルーン部材が前記取り囲む親水性挿入体に、前記内部バルーン部材の前記外面と前記硬質球形本体の前記内面との間において前記親水性挿入体を圧縮し、さらに、前記親水性挿入体から前記複数のポートのうちの少なくとも1つを通して前記第1の流体と前記治療薬とのうちの少なくとも1つを吐出させるのに十分な力を印加する、請求項11に記載の医療デバイス。
- 同時の、前記標的組織のアブレーションと前記複数のポートのうちの1つまたは複数を通した前記治療薬の送達とのために動作可能である、請求項2に記載の医療デバイス。
- 前記複数のワイヤの各々が、前記電流の受容の際に、高周波(RF)エネルギーを伝導するように構成されている、請求項2に記載の医療デバイス。
- 前記複数のポートが、複数の近位ポートおよび遠位ポートを備え、前記複数のワイヤの各々が、前記近位ポートのうちの少なくとも1つを通り、かつ、前記遠位ポートの対応する1つを通り、一組の対応する近位ポートおよび遠位ポートを通る前記導電性ワイヤの一部が、前記対応する近位ポートおよび遠位ポートの間で前記遠位部の前記外面に沿って延びた長さをもつように、前記複数の近位ポートの各々が、前記複数の遠位ポートのうちの別個の1つに対応する、請求項1に記載の医療デバイス。
- 前記ワイヤが、前記遠位部の前記外面の長さの少なくとも20%に沿って延びる、請求項15に記載の医療デバイス。
- 前記複数の導電性ワイヤの各々が、異なる遠位ポートを通る、請求項15に記載の医療デバイス。
- 前記複数の導電性ワイヤの各々が、異なる近位ポートを通る、請求項15に記載の医療デバイス。
- 前記第1の流体が導電性流体である、請求項1に記載の医療デバイス。
- 前記複数のポートが、前記非導電性遠位部における1つまたは複数の中間ポートであって、前記内側チャンバから前記非導電性遠位部の外面に前記第1の流体と前記治療薬とのうちの少なくとも1つの通過を可能にするように構成された1つまたは複数の中間ポートを備える、請求項1に記載の医療デバイス。
