Abstract
Description
その方法は、患者にメチルチオニニウム(MT)含有化合物を経口投与する工程を含み、
投与は、1日あたり合計0.5〜20mgのMTを任意に2回以上の用量に分割してその対象に提供する。
「MT」は、活性成分であり、つまり、列挙される治療効果を提供するために存在する成分である。詳細には、それらの化合物は、上記のいずれかのMT部分を含み得る。上記のMT部分は、それ自体では安定でない。ゆえに、それらは、MT化合物として、例えば、LMT又はMT+塩として投与され得る。

微量栄養素及び抗酸化物質に関して上記に記載された又は相互参照される植物に加えて、他の植物抽出物及びハーブも、CNS障害において利点を有すると報告されている。Kumar, Vikas. "Potential medicinal plants for CNS disorders: an overview." Phytotherapy Research 20.12 (2006): 1023-1035を参照のこと。これらには、Ginkgo biloba、Hypericum perforatum(セイヨウオトギリソウ)、カバカバとも呼ばれるPiper methysticum Forst.(コショウ科)、Valeriana officinalis L.(セイヨウカノコソウ)、Bacopa monniera(インドの現地ではBrahmi又はJalanimbaとして知られている)、Convolvulus pluricaulis(Shankhpushpi又はshankapushpiとしても知られる)が含まれる。
例えば、ω−3ポリ不飽和脂肪酸(PUFA)が、加齢性の脳変性を予防するための有望なツールであり得ることが報告されている。PUFA、例えば(ドコサヘキサエン酸(DHA、22:6)及びエイコサペンタエン酸(EPA、20:5)の起源としては、魚油が挙げられる(Denis, I., et al. "Omega-3 fatty acids and brain resistance to ageing and stress: body of evidence and possible mechanisms." Ageing research reviews 12.2 (2013): 579-594)。

合格基準:各錠剤が、そのコーティングされた錠剤を0.1N HClに投入した後、30分以内にMTCの85%溶解を達成した。
コーティングされた錠剤を0.1N HClに投入した後45分以内にMTCの75%溶解、又は
コーティングされた錠剤を0.1N HClに投入した後30分以内にMTCの85%溶解、
コーティングされた錠剤を0.1N HClに投入した後15分以内にMTCの85%溶解
を提供する。
グリセリド(例えば、Gelucire44/14(登録商標)、ラウロイルマクロゴール−32グリセリドPhEur,USP)及びコロイド状二酸化ケイ素(例えば、2%Aerosil200(登録商標)、コロイド状二酸化ケイ素PhEur,USP)の一方又は両方であるか又はそれを含む。
対象/患者は、動物、哺乳動物、有胎盤類、げっ歯類(例えば、モルモット、ハムスター、ラット、マウス)、ネズミ科の動物(例えば、マウス)、ウサギ目の動物(例えば、ウサギ)、鳥類(例えば、鳥)、イヌ科の動物(例えば、イヌ)、ネコ科の動物(例えば、ネコ)、ウマ科の動物(例えば、ウマ)、豚のような動物(例えば、ブタ)、羊のような動物(例えば、ヒツジ)、ウシ亜科の動物(例えば、ウシ)、霊長類、猿のような動物(例えば、サル又は類人猿)、サル(例えば、マーモセット、ヒヒ)、単孔類(例えば、カモノハシ)、類人猿(例えば、ゴリラ、チンパンジー、オランウータン、テナガザル)又はヒトであり得る。
その方法は、対象にメチルチオニニウム(MT)含有化合物を経口投与する工程を含むが、
但し、処置は、アセチルコリンエステラーゼ阻害剤又はN−メチル−D−アスパラギン酸レセプターアンタゴニストのどちらか又はその両方の投与を含まない。
本明細書中に記載される単位投与量組成物(例えば、低用量MT含有化合物及び任意に他の成分又はADの処置のためのより一般的なMT組成物)は、ラベルされた包装容器の中にその使用についての指示書とともに提供され得る。
本発明の1つの態様は、タンパク質の凝集、例えば、神経変性疾患及び/又は臨床認知症に関連するタンパク質の凝集を制御する(例えば、後退させる及び/又は阻害する)ための、本明細書中に記載されるようなMT化合物又は組成物の使用である。その凝集は、下記で論じられるような疾患状態に関連し得る。

本発明の化合物及び組成物は、タンパク質凝集の疾患の処置又は予防において有用である。
本発明の好ましい実施形態は、タウタンパク質に基づく。用語「タウタンパク質」は、本明細書中で使用されるとき、タウタンパク質ファミリーの任意のタンパク質のことを広く指す。タウタンパク質は、集合と脱集合のサイクルの反復中に微小管とともに同時精製するたくさんのタンパク質ファミリーの中の1つとして特徴づけられ(例えば、Shelanski et al., 1973, Proc. Natl. Acad. Sci. USA, Vol. 70, pp. 765-768を参照のこと)、微小管結合タンパク質(MAP)として知られている。タウファミリーのメンバーは、特徴的なN末端セグメント、そのN末端セグメントに挿入された脳において発生的に制御されるおよそ50アミノ酸の配列、31〜32アミノ酸の3又は4つのタンデム反復からなる特徴的なタンデム反復領域、及びC末端テイルを有するという共通の特徴を共有する。
1つの実施形態において、本発明は、アルツハイマー病(AD)、例えば、軽度、中等度又は重度のADの処置のために使用される。
MTは、家族性ALSと孤発性ALSの両方の大部分の病理学的特徴でありFTLD−Pの特徴でもある細胞内のTDP−43タンパク質凝集を標的化し、それを減少させ得る作用様式を有する。
MTは、ハンチントン病の病理学的特徴である細胞内のポリグルタミンタンパク質凝集を減少させ得る。ハンチントン病は、ハンチンチンのN末端に位置する翻訳されるCAGリピートの拡大が原因である。野生型染色体は、6〜34個のリピートを含むのに対して、ハンチントン病では、染色体は36〜121個のリピートを含む。発症年齢は、タンパク質内のポリグルタミンリピートをコードするCAGトラクトの長さと逆相関する。
ミトコンドリア障害、特に呼吸鎖疾患(RCD)において最も高頻度で影響される器官は、骨格筋に加えて、中枢神経系(CNS)である。RCDのCNS所見は、脳卒中様エピソード、癲癇、片頭痛、運動失調、痙縮、運動障害、精神障害、認知機能低下又はさらには認知症(ミトコンドリア認知症)を含む。これまで、ミトコンドリア認知症は、MELAS、MERRF、LHON、CPEO、KSS、MNGIE、NARP、リー症候群及びAlpers-Huttenlocher病において報告されてきた(Finsterer, 2009)。ミトコンドリアの呼吸鎖には、一連の電子伝達に関わる複合体が4つある。これらの複合体のいずれかの異常な機能によって、異常な電子伝達鎖及びその後の異常なミトコンドリア呼吸に続発するミトコンドリア病がもたらされ得る。ミトコンドリア呼吸鎖の複合体IIIは、シトクロムcに電子を伝達するように作用する。
本発明において使用するためのMT化合物には、酸化型と還元型との混合物が含まれ得る。
本明細書中に記載されるMT含有化合物は、それ自体が塩であるが、混合塩の形態でも(すなわち、本発明の化合物を別の塩とともに)提供され得る。そのような混合塩は、用語「及びその薬学的に許容される塩」によって包含されると意図される。別段特定されない限り、特定の化合物に対する言及は、その塩も包含する。
本明細書中に記載されるMT含有化合物を化学合成するための方法は、当該分野で公知である。例えば、
化合物1〜7の合成は、国際公開第2012/107706号に記載されている方法又はそれらに類似の方法に従って行われ得る。
化合物8の合成は、国際公開第2007/110627号に記載されている方法又はそれらに類似の方法に従って行われ得る。
化合物9(MTC)の合成は、当該分野で周知である。高度に純粋なMTCの合成例は、国際公開第2006/032879号及び国際公開第2008/007074号に提供されている。
化合物10〜13の合成は、国際公開第2007/110630号に記載されている方法又はそれらに類似の方法に従って行われ得る。
本明細書中に記載されるMT含有化合物を化学合成するための方法は、当該分野で公知である。乾式圧縮を用いる方法例は、例えば、国際公開第2012/072977号に提供されている。
方法
アウトカム及び尺度
共主要有効性アウトカム(co-primary efficacy outcomes)は、11項目アルツハイマー病評価尺度−認知機能下位尺度(ADAS−cog)及び23項目アルツハイマー病共同研究−日常生活動作(ADCS−ADL)におけるベースラインからの変化であった。磁気共鳴画像(MRI)容積測定を、脳萎縮速度に対する潜在的影響を裏付ける重要な副次アウトカムとして選択した(Fox et al., 1997; Ridha et al., 2008)。脳室境界シフト積分(VBSI)(Salloway et al., 2014)は、測定のための高コントラスト境界を提供し、全脳又はそれより小さい構造よりもモーションアーチファクトによって影響されないので、VBSIによって測定される側脳室容積(LVV)の変化を重要な尺度として選択した。裏付けとなる重要なトポグラフィー尺度(側頭頭頂(TPV)及び海馬容積(HV))も報告する。
2013年1月から2014年8月の間に、16の国にわたる115の施設において患者を募集した。本試験を完了したおよそ500人の患者に対するデータを得るために、30〜40%の中断率を想定して、およそ833人の患者という登録者数を目標にし、実際には891人の患者が集まった。National Institute of Aging(NIA)及びAlzheimer’s Association(AA)の基準に従うあらゆる原因の認知症及びほぼ確実なADの診断を満たす90歳未満の患者が、1又は2という臨床的認知症尺度(CDR)の総スコア及び14〜26(両端を含む)というミニメンタルステート検査(MMSE)スコアを有した場合、それらの患者を含めた。成人の介護者の参加を求めた。AChEI及び/又はメマンチンの同時使用を許可したが、但し、それらの患者は、スクリーニング前の6週間以上にわたって投与量が変更されずに3ヶ月以上それらの薬を投与されていた。セロトニン作動性抗うつ薬の同時使用を許可したが、セロトニン症候群に対する理論上の可能性に起因して(Ramsay et al., 2007)、4つの発表されている診断基準から導かれた標的評点尺度(Alusik et al., 2014)を用いて患者を厳密にモニターした。患者が、ベースライン前の6週間以内に行われた脳磁気共鳴画像(MRI)においてAD以外の著しい中枢神経系障害又は著しい限局性若しくは血管性の頭蓋内病理を有した場合、それらの患者を本試験から除外した。高用量のMT+は、メトヘモグロビン血症を誘導し得るので、グルコース−6−リン酸デヒドロゲナーゼ欠損症を有するか又は別途血液学的リスクがある対象を除外した。他の選択規準/除外規準は、補足資料に提供する。
患者を、ベースライン時に、1日2回の75mg又は1日2回の125mgのLMTM(MT塩基当量として表現される)又はコントロールに3:3:4の比でランダム化した。このランダム化は、地理的領域(3つのレベル、つまり、北アメリカ、欧州、世界のその他の国々)、ADとラベルされた併用薬の使用(2つのレベル、つまり、使用する又は使用しない)及び重症度(2つのレベル、つまり、中等度のMMSE14〜19(両端を含む)、軽度のMMSE20〜26(両端を含む))に従って層別化された。対照群の患者には、盲検化を維持するために1日2回の4mgという用量を投与した。
すべての患者から、本試験に登録する前に書面によるインフォームドコンセントを得て、意志決定能力が低下している患者の代わりに、法律上許容され得る代理人が同意を示した。成人の介護者からも、介護者自身が関与することについての同意を得た。本試験は、ヘルシンキ宣言及び優良臨床試験基準に関する医薬品規制ハーモナイゼーション国際会議ガイドラインに従って実施され、試験プロトコル及び関連するすべての書類の承認は、すべての試験実施施設の適切な独立倫理委員会及び施設審査委員会から得られた。独立した効果安全性評価委員会が、生じる安全性情報を監視するために設立された。この試験は、www.clinicaltrials.gov(NCT01689246)及びEuropean Union Clinical Trials Registry(2012-002866-11)に登録された。
ADAS−cog及びADCS−ADLの評価を、ベースライン時及びその後13週間ごとに行い、65週間後(治療中の最後の来院時)の変化を主要有効性尺度とした。これらの評価を、第69週における治療終了後の追跡時の最後の来院時に繰り返した。副次的有効性尺度には、変化の臨床全般印象(ADCS−CGIC、独立した評価者によって行われる)、スクリーニング時並びに第26、52、65及び69週に行われたMMSEが含まれた。脳MRIスキャンは、ADNIの推奨であるFLAIR、T2*Gradient Echo及びT2重み付けシーケンスと一致する容積測定の3D T1重み付けデータを含む標準化プロトコルを用いてベースライン/スクリーニング時及び13週間ごとに行われた。ほぼ確実なAD以外の診断に至り得る病理が共存する患者を除外するために、拡散重み付けイメージングもスクリーニング時に利用可能だった。まず、標準化された取得プロトコル、患者の取り扱い及びデータ管理を保証する各施設を認定した。MRIデータは、Imaging Corelab(Bioclinica)によって一元的に収集された。データは、継続的に適格性及び安全性(アミロイド関連画像異常、すなわちARIAモニタリング)についてRadMDによって一元的に審査された。重要な副次アウトカム尺度としてLVVの変化並びに探索的評価項目として側頭頭頂容積(TPV)、全脳容積(WBV)及び海馬容積(HV、左及び右の平均値として推定される)(Wischik et al., 2016)を評価するために(境界シフト積分(BSI)法、Salloway et al., 2014を用いて)、容積測定の3DT1データも一元的に審査された。さらなる探索的評価項目には、18F−フルオロデオキシグルコースポジトロン放出断層撮影(FDG−PET)を用いて評価される、側頭葉におけるグルコース取り込みの変化が含まれ、これは、このイメージングを行うことができる施設の一部の患者においてスクリーニング中並びに第39及び65週に行われた。全タウ、ホスホ−タウ及びアミロイド−β1−42を含むアルツハイマー病の脳脊髄液バイオマーカーの変化を、ベースライン時及び第65週(又は早期の終了来院時)に回収された脳脊髄液サンプルを用いて調査した。対象自身が腰椎穿刺について明確に同意を示すことができた対象にだけこの手技を行った。
ADAS−cog及びADCS−ADLスコアのベースラインからの変化の主要有効性解析を、修正インテント・トゥ・トリート(modified intent-to-treat)(mITT)集団(少なくとも1用量の試験治療を受け、ベースライン後の治療中に少なくとも1つの有効性評価を受けた、ランダム化された全患者)において行った。主解析を、無構造共分散行列を用いるが欠測データに対する補完は行わない反復測定混合モデル(MMRM)解析として指定した。このモデルは、分類来院(categorical visit)(第13、26、39、52及び65週における評価に対応する5つのレベル)、処置(コントロール、1日2回の75mg及び1日2回の125mgに対応する3つのレベル)、治療−来院交互作用、及び層別変数を加法項として、並びにベースラインのADAS−cog又はADCS−ADLを共変量として含んだ。統計解析計画書(SAP)では、AChEI/Mem共変量を治療との交互作用項及び来院との交互作用項として用いる同様の探索的解析を指定した。アルファ0.05によるファミリーワイズエラーの強い制御を確保するために、多重比較補正のためのWestfallの方法を各工程において用いた(Westfall et al., 1997)。LVV、WBV、TPV、HV及びTPVの変化について同じ解析を実行した。
患者
患者の配置及び試験のデザインを図1に示す。ランダム化された891人の患者のうち、885人が、少なくとも1用量の試験薬を投与され、安全性集団を構成し、855人の患者が、図1(A)のmITT集団に含まれた。ランダム化された処置及びAChEI/Memあり又はなしでの処置別の患者の配置を図1(B)に示す。安全性集団のベースラインの人口統計及び臨床的特性を表1に示す。618人の患者が、65週間までの本試験を完了し(579人が治療に残った)、試験全体の中止率は31%だった。すべての予定来院からのMRIスキャンは、処置前の880人の患者及び65週間後の554人から入手可能であった。FDG−PETデータは、65週間後に101人の患者から入手可能であり、それらの患者のうち6人だけが、AChEI/Mem処置を受けていなかった。
表2は、ADAS−cog及びADCS−ADLスコアの変化、対照群における機能低下及びコントロールに対する差を治療群別に報告している。12ヶ月後のADAS−cog低下(3.85±0.47、平均値±se)は、12ヶ月後の最近の研究のメタアナリシス(4.58、95%CI:3.69〜5.47)(Salloway et al., 2014; Doody et al., 2014)と区別がつかなかった。同様に、12ヶ月後のADCS−ADL低下(−6.51±0.63)は、軽度/中等度のADにおける唯一入手可能な最近の研究(Doody et al., 2014)(−5.79、95%CI:−7.10〜−4.48)と類似していた。
主要臨床アウトカムに対する同じ最初の解析モデルを用いても、処置の効果は見られなかった(データ示さず)。AChEI/Mem因子を交互作用項として用いる事前に指定された解析モデルでは、臨床アウトカムについて示された結果と類似の結果がLVVに対しても見られ、1日2回の75mg用量又は1日2回の125mg用量に対するp値は、それぞれ4.0e−05及び3.8e−04であった。これらの結果は、TPV解析においても類似しており、1日2回の75mg用量及び1日2回の125mg用量に対するp値は、それぞれ2.4e−04及び0.00160であった。HV解析では、単独で投与された1日2回の125mg用量だけが、軽度/中等度の混合集団において名目上の有意性に近づいた(p=0.0558)。データを表5並びに図4、5及び6に示す。
疾患の重症度が、一次モデルにおいて高度に有意な共変量であったので、同じ解析を、軽度及び中等度の対象において別々に繰り返した(表7)。単独療法としてのLMTMによる処置に対する結果を、ランダム化された対照群と比較して、1日2回の75mg用量及び1日2回の125mg用量に対してのみ示す。1日2回の4mg用量の解析は、対照群にランダム化され、AChEI/Memあり又はなしで1日2回の4mgを投与された対象に限定された。
LMTMを単独療法として投与されたか又はされなかったかに応じた結果の違いを考えて、これらの群の間のベースライン時の違いを臨床重症度に従って解析した(表8)。齢分布又は性分布に差は見られなかった。ADとラベルされた併用薬を処方されなかった患者が、主にロシア、東欧(ポーランド及びクロアチア)及びマレーシアに位置する施設にいると見出された点で、軽度患者では有意な地域差があった(が中等度患者ではなかった)。ベースラインのADAS−cog又はMMSEに差は無かった。これらの薬剤を投与されなかった軽度の患者は、ADCS−ADL尺度がわずかに悪かったが、中等度の患者については、差が無かった。ベースライン時のMRIパラメータに関して、これらの薬剤を投与されていない軽度の患者は、わずかに大きいHV及びLVVを有した(が中等度患者は有さなかった)が、ベースライン時のWBV、TPV又はFazekasスコアによって示される血管病理負荷の程度(Murray et al., 2005)に差は無かった。同様に、LMTMの代謝又は排出の差を示唆し得るベースラインのビリルビン又はクレアチニンクリアランスについて差は見られなかった。
胃腸管及び尿路が、有害事象によって影響を受けることが最も多い体組織であった。これらは、高用量LMTMの中断に対する最も一般的な理由でもあった(それぞれ9%及び3%の患者)。比較として、これらの事象のために中断したコントロール患者はたった1〜2%であった。注目すべきことに、胃腸の有害事象の発生率は、アセチルコリンエステラーゼ阻害剤も投与された患者において約2倍高かった(データ示さず)。高用量LMTMにおいて対照群よりも高い5%以上で生じた処置誘発の有害事象を表9に示す。
本試験の目的は、150mg/日の総1日量を用いた先の第2相試験において報告された有効性を確認すること、及び新しく開発された安定化還元型のMTをLMTMとして使用したとき250mg/日が優れた利点をもたらし得るかを判定することであった。本試験は、排泄物の変色に関して盲検性を確保するために真のプラセボではなく低用量のLMTM(1日2回の4mg)を対照群において使用した。対照群において見られたADAS−cog及びADCS−ADL尺度の低下速度は、線形であり、最近の研究において報告されたものと区別がつかなかった。ADNIプログラムから入手可能なデータ(Frisoni et al., 2010; Nestor et al., 2008)と比べると、同じことが、LVVの変化によって計測された軽度のAD群における脳萎縮の進行速度について真であると見出された。これらの比較は、本試験の表面的妥当性を裏付ける。
目的
軽度のADにおける18ヶ月間の第3相試験における非ランダム化観察コホート解析において、単独療法としてのLMTMの潜在的有効性を修正主要アウトカムとして調べること。
軽度のAD患者(n=800)を、1日に2回の100mg又は1日に2回の4mgにランダムに割り当てた。
修正された解析は、両方の比較において、共主要臨床的有効性評価項目(ADAS−cog及びADCS−ADL)、MRI萎縮及びグルコース取り込みについて、p<0.025という要求された閾値において統計学的に有意であった。全脳萎縮は、追加治療群と単独療法群の両方において、軽度のADに対して予想されたとおり、最初は進行したが、処置の9ヶ月後に有意に分かれ、単独療法患者における最終的な萎縮速度は、正常な高齢者コントロールに特有のものであった。ベースライン時の単独療法患者と追加治療患者との差は、単独療法に有利なように有意差を説明しなかった。追加としてのLMTMに対する処置反応は、相対的な前脳基底部萎縮と逆相関した。
LMTMを用いた2つの第3相試験の全ランダム化(as-randomised)解析が本明細書中に記載される。第1は、軽度から中等度のADにおける解析(実施例3)であり、第2は、軽度のADにおける解析(この実施例)である。両方の試験が、最初は、150〜250mg/日の範囲内の高用量のLMTMと、盲検化を維持するための尿の変色剤として意図された低用量の1日に2回の4mgを比較するためにデザインされた。第2相試験においてMTCとしての69mgMT/日という用量が最小有効性を有すると見出されたので、この低用量は無効であり得ると想定された。

前記投与は、0.5〜20mgの総1日量のMTを任意に2回以上の用量に分割して前記対象に1日あたり提供する、方法。 - 前記総1日量が、およそ0.5、1、1.5、2、2.5、3、3.5、4mgのいずれかから、およそ5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20mgのいずれかまでである、請求項1に記載の方法。
- 前記総1日投与量が、1〜20mg、2〜15mg又は3〜10mgである、請求項1又は請求項2に記載の方法。
- 前記総1日投与量が、3.5〜7mgである、請求項3に記載の方法。
- 前記総1日投与量が、4〜6mgである、請求項4に記載の方法。

- 前記対象が、アセチルコリンエステラーゼ阻害剤及び/又はN−メチル−D−アスパラギン酸レセプターアンタゴニストによる処置を受けた履歴があるが、前記MT含有化合物による処置の少なくとも1、2、3、4、5、6、7日前又は2、3、4、5、6、7、8週間前などにその薬物療法を中止した、請求項1から40のいずれか1項に記載の方法。
- 前記対象が、アセチルコリンエステラーゼ阻害剤及び/又はN−メチル−D−アスパラギン酸レセプターアンタゴニストによる処置を受けている対象として選択され、
前記アセチルコリンエステラーゼ阻害剤及び/又はN−メチル−D−アスパラギン酸レセプターアンタゴニストによる前記処置が、前記MT含有化合物による処置の前に中断される、請求項1から40のいずれか1項に記載の方法。 - 前記障害が、タウオパチーである、請求項1から43のいずれか1項に記載の方法。
- 前記障害が、ピック病、進行性核上性麻痺、前頭側頭型認知症、17番染色体に連鎖しパーキンソニズムを伴うFTD、前頭側頭葉変性症症候群、脱抑制−認知症−パーキンソニズム−筋萎縮複合、淡蒼球橋黒質変性症、Guam−ALS症候群、淡蒼球黒質ルイ体変性症、大脳皮質基底核変性症、好銀性顆粒認知症、拳闘家認知症又は慢性外傷性脳障害、ダウン症候群、亜急性硬化性全脳炎、軽度認識障害、ニーマン・ピック病C型、サンフィリポ症候群B型又は筋緊張性ジストロフィDM1若しくはDM2からなるリストより選択される、請求項1から44のいずれか1項に記載の方法。
- 前記障害が、アルツハイマー病である、請求項1から44のいずれか1項に記載の方法。
- 前記障害が、ポリグルタミン障害である、請求項1から43のいずれか1項に記載の方法。
- 前記ポリグルタミン障害が、ハンチントン病、脊髄延髄性筋萎縮症、歯状核赤核淡蒼球ルイ体萎縮症又は脊髄小脳失調である、請求項47に記載の方法。
- 前記障害が、TDP−43プロテイノパチーである、請求項1から43のいずれか1項に記載の方法。
- 前記TDP−43プロテイノパチーが、FTLD−TDPである、請求項49に記載の方法。
- 前記障害が、シヌクレイン病である、請求項1から43のいずれか1項に記載の方法。
- 前記シヌクレイン病が、パーキンソン病、レビー小体型認知症及び多系統萎縮症から選択される、請求項51に記載の方法。
- 前記障害が、遺伝性脳血管障害である、請求項1から43のいずれか1項に記載の方法。

- 請求項1から35のいずれか1項において定義されるMT化合物及び薬学的に許容されるキャリア又は希釈剤を含む、任意に投与単位の形態の、薬学的組成物であって、
前記組成物中又は前記単位中のMTの量は、0.5〜10mgである、薬学的組成物。 - 請求項1から35のいずれか1項において定義されるMT化合物、
及び栄養化合物を含む、任意に投与単位の形態又は食料品の規定部分での、栄養補助組成物であって、
前記単位中のMTの量は、0.5〜10mgであり、
前記栄養素は、ビタミン、微量栄養素、抗酸化物質、ハーブ、植物抽出物、種子抽出物、植物油若しくは魚油、ミネラル、微量元素、酵素又はアミノ酸からなるリストより選択される、栄養補助組成物。 - 任意にビタミンB9、B6又はB12、ビタミンC、ビタミンEである、ビタミン、
任意にフラバノール、フラボノール、フラボン、イソフラボン、フラバノン、アントシアナジン、非フラボノイドポリフェノール、カロテノイド、クロシン、ジテルペンである、微量栄養素及び/又は抗酸化物質、
任意にGinkgo biloba、Hypericum perforatum、Piper methysticum、Valeriana officinalis、Bacopa monniera、Convolvulus pluricaulisである、ハーブ又は他の植物抽出物、
任意にω−3ポリ不飽和脂肪酸である、植物油又は魚油

- 1〜10mg、2〜9mg、3〜8mg、3.5〜7mg、4〜6mgのいずれかのMT化合物を含む、請求項56から61のいずれか1項に記載の組成物。
- 前記MT化合物が、少なくとも97%純粋である、請求項56から62のいずれか1項に記載の組成物。
- 約1.5、2、2.5、3、3.5、4、5、6、7、8、9、10、15、16、17又は18mgのいずれかのLMTMを含む、請求項56から63のいずれか1項に記載の組成物。
- 約1.5、2、2.5、3、3.5、4、5、6、7、8、9、10、15、18又は20mgのいずれかのMTC.5H2Oを含む、請求項56から63のいずれか1項に記載の組成物。
- 即時放出組成物である、請求項56から65のいずれか1項に記載の組成物。
- (i)その各々が請求項56から66のいずれか1項に記載の組成物である、複数の投与単位又は規定部分食料品、
(ii)請求項1から55のいずれか1項において定義される処置方法に従ってそれらを使用するためのラベル及び/又は指示書
を含む、容器。 - 前記容器が投与単位を含み、前記投与単位が、実質的に水分不透過性であるブリスター包装内に存在する、請求項67に記載の容器。
- 前記ラベル又は指示書が、前記組成物が対象とする障害に関する情報を提供する、請求項67又は請求項68に記載の容器。
- 前記ラベル又は指示書が、前記投与単位の許容最大1日投与量に関する情報を提供する、請求項67から69のいずれか1項に記載の容器。
- 前記ラベル又は指示書が、前記処置の持続時間の提案に関する情報を提供する、請求項67から70のいずれか1項に記載の容器。
- 前記ラベル又は指示書が、前記組成物が、アセチルコリンエステラーゼ阻害剤又はN−メチル−D−アスパラギン酸レセプターアンタゴニストのいずれかと併用されるべきでないことを指示する情報を提供する、請求項67から71のいずれか1項に記載の容器。
- 対象においてアルツハイマー病(AD)を治療的に処置する方法であって、
前記方法は、前記対象にメチルチオニニウム(MT)含有化合物を経口投与する工程を含み、
前記処置は、アセチルコリンエステラーゼ阻害剤又はN−メチル−D−アスパラギン酸レセプターアンタゴニストのいずれか又は両方の投与を含まない、方法。 - 対象においてADを予防的に処置する方法であって、
前記方法は、前記患者にMT含有化合物を経口投与する工程を含み、
前記処置は、アセチルコリンエステラーゼ阻害剤又はN−メチル−D−アスパラギン酸レセプターアンタゴニストのいずれか又は両方の投与を含まない、方法。 - 前記投与が、0.5〜300mgの総1日量のMTを任意に2回以上の用量に分割して前記対象に1日あたり提供する、請求項73又は請求項74に記載の方法。
- 前記対象が、アセチルコリンエステラーゼ阻害剤又はN−メチル−D−アスパラギン酸レセプターアンタゴニストによる処置を受けた履歴がない、請求項73から75のいずれか1項に記載の方法。
- 前記対象が、アセチルコリンエステラーゼ阻害剤及び/又はN−メチル−D−アスパラギン酸レセプターアンタゴニストによる処置を受けた履歴があるが、前記MT含有化合物による処置の少なくとも1、2、3、4、5、6、7日前又は2、3、4、5、6、7、8週間前などにその薬物療法を中止した、請求項73から75のいずれか1項に記載の方法。
- 前記対象が、アセチルコリンエステラーゼ阻害剤及び/又はN−メチル−D−アスパラギン酸レセプターアンタゴニストによる処置を受けている対象として選択され、
前記アセチルコリンエステラーゼ阻害剤及び/又はN−メチル−D−アスパラギン酸レセプターアンタゴニストによる前記処置が、前記MT含有化合物による処置の前に中断される、請求項73から75のいずれか1項に記載の方法。 - 前記処置が、特定の投与量の前記MT含有化合物である第1の作用物質と、アミロイド前駆体タンパク質からベータ−アミロイドへの阻害剤である第2の作用物質との併用処置である、請求項46又は請求項73から78のいずれか1項に記載の方法。
- 前記処置が:52週間にわたっての、11項目アルツハイマー病評価尺度−認知機能下位尺度(ADAS−cog)における3、4又は5点の改善;52週間にわたっての、23項目アルツハイマー病共同研究−日常生活動作(ADCS−ADL)における4、5又は6点の改善;52週間にわたっての、脳室境界シフト積分(VBSI)によって測定される側脳室容積(LVV)増加の1又は2cm3の減少;の基準の1つ以上において、対象における改善を達成するか又は達成するためのものである、請求項46又は請求項73から79のいずれか1項に記載の方法。
- (i)複数の投与単位であって、その各々は、請求項1から35のいずれか1項において定義されるMT化合物及び薬学的に許容されるキャリア又は希釈剤を含む組成物であり、前記単位中のMTの量は、0.5〜300mgである、複数の投与単位、
(ii)前記投与単位が、アルツハイマー病(AD)の処置に使用されることを意図しているという情報を提供する、ラベル又は指示書、
(iii)前記組成物が、アセチルコリンエステラーゼ阻害剤又はN−メチル−D−アスパラギン酸レセプターアンタゴニストのいずれかと併用されるべきでないことを指示する情報を提供する、ラベル又は指示書
を含む、容器。 - 請求項1から55又は73から80のいずれか1項において定義される処置方法に使用するための、請求項1から66のいずれか1項に記載のMT含有化合物又は組成物。
- 請求項1から55又は73から80のいずれか1項において定義される処置方法に使用するための医薬の製造における、請求項1から66のいずれか1項に記載のMT含有化合物又は組成物の使用。
- 対象においてタンパク質凝集の神経変性障害を治療的又は予防的に処置する方法であって、前記神経変性障害は、任意にアルツハイマー病であり、
前記方法は、前記対象に請求項56から66のいずれか1項において定義される組成物を経口投与する工程を含む、方法。
