本発明の態様は、特に、1)適切に個人化されたオピオイドの処方及び処方薬の管理(例えば個人化された分配)を確実にすることによる、薬物の安全性プロフィールの改善、2)薬物が有効であることを確実に行うこと、3)診断用の補助/ツールとして、4)薬物の滴定、5)薬物を介した有害事象の排除、6)過量摂取の排除、7)過少投与の排除、8)誤用の機会の低減、9)濫用の機会の低減、10)過量摂取の機会の低減、11)薬物処方との遵守の向上、12)不注意による重複用量の摂取からの患者の保護、13)嗜癖の機会の低減、14)依存の機会の低減、15)オピオイド耐性のより良い管理、16)薬物禁断症状の管理、17)必要に応じた(PRN)投与の許容、例えば管理「患者制御痛覚脱失TM」(PCOATM)、18)薬物流用の排除、19)小児による偶発的なオピオイドの服用の防止、20)薬物−薬物相互作用の回避、21)薬物を介した副作用のより良い管理、22)処方薬の固執の促進、23)製薬業者又はオピオイド処方者2から患者6への任意の潜在的な法的責任の移動、24)薬物の分配の制御及び/又は臨床試験規制当局の報告要件への適合に必要な、患者の自己評価、自己試験、自己報告、及びデジタル的に捕捉された情報の捕捉、25)患者のより良い管理を支援するオピオイド関連副作用の捕捉、26)オピオイドリスク評価・リスク緩和戦略(REMS)報告の合理化、27)処方薬物モニタリングプログラム(PDMP)記録保持及び報告の合理化等の使用に利用され得る。要するに、オピオイドの薬物/安全性プロフィールを改善することによりオピオイド療法を個人化することに対する。
オピオイドは、オピオイド及び他の薬物の管理及び患者の結果を改善するために、本発明の態様をどのように利用し得るかの素晴らしい典型例として役立つ。
以下は、次に続く本態様のオピオイドの実施例の背景として、オピオイドの重要な副作用の全てではないがいくつかを記述する:
処方
オピオイド特異的アプリケーション12は、それぞれの患者6にカスタマイズできるように設計されている。処方者2が処方箋4に入力すると、彼/彼女は、臨床試験及び/又は薬物認証のための規制当局、例えばFDA又はEMAが必要とする全ての可能性のあるオピオイドの副作用及び/又は任意の情報が含まれたリストから選択し得る。処方者2は、情報の各部分について、例えば、オピオイドを分配する前に毎回特定の時間間隔で別の値が生じた場合、特定の値を上回るか又は下回る別の値が生じた場合、又は特定の範囲内等のどのくらいの頻度で患者が促されるかについても選択し得る。これらの値には、デジタル的に捕捉された、又は患者が入力した生理学上の値、心理学上の値、生活スタイル、直近のオピオイド投与以来の摂取した同時薬物、及び/又は環境情報が含まれ得る。捕捉された情報は、用量を分配するか又はしないことを決定するために、オピオイド特異的分配アルゴリズム13により利用されてもされされなくてもよい。
処方が一旦電子的に入力され、提出されると150、データサーバー10上のアプリケーション作成プログラム200により、個人化された患者6のオピオイド特異的アプリケーション12が作成され、データサーバー10上に存在する、関連する患者6の記録/データベースが作成され、結合される。その後、患者6へダウンロードリンク(download link)が送られ、患者6のための適切な職員、例えば介護者、処方者2及び薬局8等へはコピーが送られ(with copies)、患者6によりインターフェースデバイス14(一つ又は複数)上に、又は独立型使い捨て薬物ディスペンサー16上に、オピオイド特異的アプリケーション12がダウンロードされる。患者6がオピオイド特異的アプリケーション12をダウンロードすることに何らかの問題がある場合、介護者、処方者2又は彼/彼女のスタッフ、統合サポートセンター22及び/又は薬局8のスタッフが、患者6がこの問題を解消することを手助けし得る。
個人化されたオピオイド特異的アプリケーション12をダウンロードした時、患者6はオピオイド特異的アプリケーション12を開き、生体測定によるログイン280を可能にし、患者6を処方箋4に及びオピオイド特異的アプリケーション12にリンクさせるのに必要な患者6の情報を入力する。
再利用可能な薬物ディスペンサー16において、薬物ディスペンサー16を開くためには、e-処方箋4が提出150された時に患者データベース152アプリケーション作成プログラム200により自動的に発行される固有のディスペンサー識別番号158が必要である。再利用可能な薬物ディスペンサー16を開いて制限された薬物を装填するためには承認コードが必要である。ディスペンサーがオピオイドのために用いられている場合、補充オピオイド薬物カセット18を装填してオピオイドの流用を減少させるために、承認コードにより再利用可能な薬物ディスペンサー16が制限され、認定された健康管理の専門家のみにより開かれてオピオイドの処方が扱われる。再利用可能な薬物ディスペンサー16を開くためにはUSBケーブルが必要である。USBケーブルを電源に差し込むと、再利用可能な薬物ディスペンサー16のマイクロアクチュエータ1028がロック又はアンロックされ、再利用可能な薬物ディスペンサー16を開くためには承認コードの入力をしなければならない。
使い捨て薬物ディスペンサー16の場合、分配は、薬局8が任意の他の封入された薬物とほとんど同じオピオイドを分配することのみを要求する。オピオイド特異的アプリケーション12は、使い捨て薬物ディスペンサー16とのハンドシェークをもたらし得、そして二以上の薬物ディスペンサー16から同時に、処方されたオピオイドの同じ用量を二回分配することを自動的に排除し得る。オピオイド特異的アプリケーション12は、全ての分配が処方箋4の指示に従うように制限されていることを保証する。
統合システムにより、処方者2、処方箋4、患者6、薬局8、データサーバー10、オピオイド特異的アプリケーション12、インターフェースデバイス14、薬物ディスペンサー16、薬物カセット18、及び適用可能な場合はRFID丸薬20、及び又は患者6が丸薬を服用しているところを捕捉する自分撮り21を含むオピオイドの閉じた回路追跡が可能となる。オピオイドが薬物カセット18に充填された時点から、それが分配されるか又は摂取されるまで、オピオイドは追跡され得る。
オキシコンチン(登録商標)等のオピオイドの適切な利用を保証することは、どのようにして本態様が、薬物の有効性、適切なオピオイドの利用、並びに薬物を介した有害事象、過量摂取、濫用、誤用、過少投与、過量摂取、嗜癖、依存症、及び薬物間の有害な相互作用の回避を保証するために使われるのかの一例としての役割を果たす。薬物の滴定、禁断症状、副作用(例えばオピオイド誘導性便秘)の予防、及び特定の薬物関連副作用を捕捉することによるより良い患者の管理のために、それらは使用され得る。情報を利用して、規制当局への提出のための臨床試験の間に要求される患者報告結果(PRO)情報を捕捉することもできる。

情報の捕捉は、それが使用可能でなければほとんど価値がない。薬物特異的分配アルゴリズム13は、たとえ、処方により違ったように分配が認められていたとしても、特定の薬物特異的な値を取り込んで分配する又は分配しないことの決定を行うように設計される。このことにより、個別の患者6への個々の薬物の処方箋4の個人化が可能となり、薬物の有効性/安全性プロフィールが大きく向上する。
薬剤リコールの場合、又は薬物が臨床試験の状態にある場合、統合サポートセンター22は、手続及び規制の範囲内で、特定のオピオイドを含有するすべての薬物ディスペンサー16を遠隔操作でロックし得、同時に彼/彼女がオピオイドを分配し得ないこと及び彼らがすべきことについて、患者6に警報を出す。
本発明の態様を利用して、オピオイド滴定を支援し得る。滴定は体を薬物に適合させることに役立ち、オピオイド治療が開始されたときに生じ得る一般的な副作用をしばしば低減する。医師/処方者6は最初は低い用量のオピオイドで開始し、適切なレベルにまで用量を注意深く高める調整を行う傾向にある。滴定は、最適な用量を見つけて日々の機能を改善することに役立つ。例えば、オピオイド用量は徐々に耐え得る最も高い用量まで増加する。一旦用量の増加に伴って症状のこれ以上の改善がなくなると、処方者6は以前の用量にまで用量を低くする。より高い用量があまりにも多くの副作用を生じさせる場合、用量は低められる。オピオイドの管理は一人の人間の有する個人的な必要性及び応答に基づく。最適なオピオイドの用量とは、日々の機能が大きく改善され、副作用が最小化される用量である。
(i)夜間の良好な睡眠、
(ii)日中の安静時の疼痛のコントロール、及び
(iii)患者6が活発に歩行している時の疼痛のコントロール。
オピオイド摂取の以前の経歴がない場合、開始用量は、疼痛の重症度、患者の年齢、体重、性別及び全身的な健康状態を評価して計算される。
鎮痛薬投与後の疼痛の評価及び再評価又は疼痛の管理は、連合委員会(Joint Commission)のような認証評価組織による健康管理の施設において制御される。薬物の治療効果を得るための種々の経路には、種々の時間が必要になるので、連合委員会は、鎮痛薬の投与経路が疼痛の評価の回数に影響することを記載した疼痛の評価の基準を2001年に設定し始めた。経口での即時放出(IR)薬物は、45〜69分間を必要とする。
処方箋4は、低用量のオピオイドを処方し、それにより患者6がその用量をより頻繁に分配されるように処方することにより患者6の用量が徐々に増えるように、又は規定された丸薬の数が分配されるように、記載され得る。一例として、患者6は用量非常に多くの時間ごとに(every so many hours)及び/又はそれを分割して投与され得、又は患者6は一日を通してより頻繁に投与され得る。例えば、b.i.d.〜t.i.d.、t.i.d.からq.i.d.、q.i.d.から二時間ごと、及びに時間ごと〜q.h。就寝前に二回投与することを許容する指示もあり得、傾向付けられた患者の自己評価データ862及び関連する自己報告されたオピオイド誘導性副作用の減少に基づいて、疼痛がコントロール下にある場合、必要に応じて投与される。
PRN投与には認められていないオキシコンチン(登録商標)等の薬物を考慮すると、本態様の属性は患者制御痛覚脱失(PCOATM)を可能にする。例えば、処方パラメータが設定されたPRN等の認められている通りに自己治療できる患者は、より少量の薬物を用いる傾向にあり、さらなる潜在的な副作用を軽減しているという研究が示された。
本発明の態様を利用して、診断を支援し得る。一例として、多数の異なるタイプの疼痛や異なるタイプの頭痛が存在する。患者はアスピリン等の市販の(OTC)鎮痛薬を用いて自己治療を始めるのが一般的である。疼痛又は不快感が強くなると、患者は摂取する錠剤の数(即ち用量)や自己治療の頻度を増やす。ある時点で、かれらは十分な緩和を求めて医師のところを訪れる。
1) 緊張性頭痛、最も一般的なタイプの頭痛であり、一般的には、市販の治療法、例えばアスピリン、イブプロフェン又はアセトアミノフェン(Tylenol)にて適切に治療し得る。専門家は、(ストレスへの反応を含む)首及び頭皮の筋肉の収縮により生じると信じており、脳内化学物質の変化の可能性もある。
2) 群発性頭痛、女性よりも男性に多く、群発的に又は周期的に起こる再発性の頭痛である。頭部の片側に突然起こり、程度が酷く消耗性の疼痛という特徴があり、顔の同じ側で、涙目及び鼻づまり又は鼻水を伴うことが多い。症状があらわれている間、人々は眠れないことが多く、くつろぐことができない;偏頭痛になったかもしれない人のように彼らは横になれそうにない。群発性頭痛の原因は不明であるが、遺伝要素があるのかもしれない。不治ではあるが、薬物により頻度と期間を抑え得る。
3) 多くは感染が原因で副鼻腔に炎症が生じると、副鼻腔炎による頭痛が起こる。これらは症状又は光ファイバースコープにより可視化された膿の存在により、通常診断され得る。副鼻腔への感染による頭痛は抗生物質、並びに抗ヒスタミン剤又は充血除去剤により治療され得る。
4) 反跳性頭痛は、皮肉なことに、頭痛用の鎮痛剤の過量使用により生じ得る。疑わしい物としては、アスピリン、アセトアミノフェン(タイレノール)又はイブプロフェン(モトリン、アドビル)のような市販の薬物、並びに処方薬である。
5) 偏頭痛は家族内で生じ得、特定の基準を用いて診断される:(i)少なくとも五回の以前の頭痛の発作、(ii)4〜72時間の継続、(iii)四回の頭痛のうちの少なくとも二回は一方の側の疼痛、拍動性の疼痛、日常活動に干渉するか、悪化させるか又は妨げる中程度〜重度の疼痛、及び(iv)以下のもののうちの少なくとも一つが疼痛に付随すること:吐き気及び/又は嘔吐又はこれらがない場合は、光及び音への感受性。偏頭痛には、前兆、例えば視覚障害又は手のしびれが前兆となり得る(偏頭痛を伴う人々の約15パーセント〜20パーセントがこれらを経験する。)
6) 混合性頭痛症候群(mixed headache syndrome)は、変形型(transformed)偏頭痛とも呼ばれ、偏頭痛及び緊張性頭痛が組み合わさったものである。
7) 急性頭痛は突然起こり、比較的短時間で症状が弱まる頭痛である。
8) ホルモン性頭痛(hormone headache)は、月経、妊娠及び閉経の間の女性のホルモンレベルの変化に付随することが多い。いくらかの女性においては、化学的に誘導されたホルモンの変化、例えば経口避妊薬も頭痛の引き金である。
9) 慢性の進行性頭痛はけん引性頭痛又は炎症性頭痛とも言われる慢性の進行性頭痛であり、長い時間をかけて悪化し、起きる頻度が高くなる。最後の一般的なタイプの頭痛は、大人の全ての頭痛の5パーセント未満の割合を占め、子供の全ての頭痛の2パーセント未満の割合を占める。慢性の進行性頭痛は脳又は頭蓋骨の疾患又は障害結果かもしれない。
本態様は、地固め療法(consolidation therapy)アプリケーション1000と連動する多剤薬物ディスペンサー1002、1004、1006、1008の使用を包含し、該地固め療法が2以上の薬物の分配とコントロールを可能にすることにより、薬剤併用療法がより良く管理される。一例として、特定の自己評価862、自己試験864、自己報告866又はデジタル的に捕捉された情報が症候性薬物療法(symptomatic medication)を指示する場合、長時間作用型オピオイド、即時放出オピオイド、及びオピオイド誘導性副作用に取り組むための単数又は複数の薬物を分配することは、副作用、例えば下痢及び嘔吐の治療のためには当然である。
オピオイド(例えばコデイン、フェンタニル及び類似物、ヒドロコドン、ヒドロモルフォン、メタドン、オキシコドン、オキシモルホン等が挙げられる。)は疼痛をコントロールすることに有効である。しかし、それらの過量摂取、濫用、嗜癖及び流用の可能性並びに関連するREMSプログラムが理由で、医師はそれらを処方することに積極的ではない。それらの嗜癖可能性が理由で、いくらかの患者もそれらを摂取することに積極的ではない。本態様は、コントロールとリアルタイムモニタリング及びそれによるオピオイドの欠陥への取り組みを提供する。
嗜癖は全世界的な危機である。一例として、合衆国においてオピオイドに依存する人々は2400万人と推定され、欧州では1300万人、残りの全世界では2000万人である。オピオイドの過量摂取は、合衆国における事故死の第二の主因である。CDCによれば、2014年の合衆国だけで、処方が必要なオピオイドを超えた過量摂取により、18,000を超える命が犠牲になった。
オピオイド特異的アプリケーション12が薬物を分配すべきかすべきでないかを決定できるために、薬剤特異的な患者の自己評価862、自己試験864、自己報告866及びデジタル的に捕捉された850、852、854、856、857、858、859、860、880、882、884、886、生理学、心理学、生活スタイル、現在摂取中の他の薬剤、並びに環境情報と共に薬物の処方箋4並びに薬物分配及び薬物ディスペンサー16の貯蔵履歴に基づいて捕捉し、保存し、分析し、そして実行するように、本システムは設計される。定義された臨床試験ガイドライン並びに処方箋4に基づいたオピオイド特異的アプリケーション12により、分配が実行できないようにし得る。このような方法で、患者のモニタリング情報を利用して、潜在的な有害事象が特定された場合、処方用量の分配の実行を停止し得る。
Claims (29)
該オピオイド特異的アプリケーションは、(i)生体認証モジュール、(ii)処方モジュール、(iii)処方者副作用追跡選択モジュール (iv)患者合図モジュール、(v)インターフェースデバイス上のオピオイド特異的アプリケーションと薬物ディスペンサーモジュールの間のインターフェースAPI、(vi)インターフェースデバイス(または独立型薬物ディスペンサー)とデジタルデータ捕捉デバイス(1つまたは複数)および/またはデータ捕捉ソフトウェアおよび/またはデータ集合デバイスの間のアプリケーションプログラムインターフェース(1つまたは複数)API、(vii)患者自己評価スクリーン(1つまたは複数)およびデータ集合のモジュール、(viii)患者自己試験スクリーン(1つまたは複数)およびデータ集合のモジュール、(ix)患者自己報告スクリーン(1つまたは複数)およびデータ集合のモジュール、(x)インターフェースデバイスデータベースモジュール (xi)オピオイド特異的分配アルゴリズムモジュール、(xii)分配通信および報告モジュール、(xiii)インターフェースデバイスと統合サポートセンターのサービスモジュールの間のインターフェースAPI、(xiv)患者報告モジュール、(xv)安全確保が制御された薬物カセット交換モジュール、(xvi)GPSモジュール、(xvii)添付文書モジュール、(xviii)ディスペンサー使用説明書モジュール、(xix)アプリケーション使用説明書モジュール、(xx)ヘルプおよび問題解決モジュール、(xxi)言語選択モジュール、(xxii)声制御モジュール、(xxiii)アプリケーション識別子モジュールならびに(xxiv)アプリケーションおよび分配ユニット操作訓練モジュールで構成され、
