本発明は、マクロファージ炎症性タンパク質−1β(MIP−1β)の活性を抑制することによって、組織虚血と血管病理的変化を改善する使用方法に関する。特に本発明は、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤を用いた医薬組成物が糖尿病患者の血管生成を促進し、組織虚血及び糖尿病の血管病理的変化の改善に用いられるものに関する。
心臓血管病は、一型糖尿病患者および二型糖尿病患者(特に二型糖尿病患者)の主要な死因である。糖尿病患者は周辺の血管病理的変化の原因で手足の切断手術をする場合が多い。糖尿病血管病理的変化は、例えば血圧、コレステロール、血糖などの動脈硬化の一般的な危険因子を除き、明らかな全身的血管炎症反応(Vascular inflammation)が特徴である。しかし、現在、糖尿病血管病理的変化の治療効果は動脈硬化心臓血管病の治療効果より悪い。その中の一つの主要な原因は糖尿病血管病理的変化の炎症反応をコントロールする効果的な治療方法がないということである。
現在、血管病理的変化を予防と治療する薬物は主に動脈硬化症の一般危険因子例えば血圧、コレステロール、血糖などをコントロールする。しかしこれは糖尿病に関する血管病の発病を遅らせるしかできない。あるいは糖尿病に関する血管病が発病してからコントロールする。糖尿病の血管炎症反応をコントロールすることができない。それゆえ、直接糖尿病の血管炎症反応(Vascular inflammation)をコントロールし、血管を保護し、血管の新生を強め、組織虚血を改善し、糖尿病血管の病理的変化を予防する新たな治療法が必要とされる。
多数の研究によって、損傷された内皮細胞層が内皮前駆細胞(endothelial progenitor cell,EPC細胞)で修復されることが可能であり、これで血管内皮細胞層の機能性と完全性を維持することができる。内皮前駆細胞が低酸素組織の中で血管生成を促進する機能があると証明された。新生の血管は低酸素組織に必要な血液循環を提供し、低酸素による器官の損傷を減少する(非特許文献1、2)。それゆえ、どうやって内皮前駆細胞を増やすことで血管動脈硬化の改善と低酸素組織血管生成の増加を実現させるかが現在の重要な課題である。
EPC細胞が骨髄から移出するよう媒介するSDF−1受容体CXCR4の作用、循環性EPC細胞を阻害するCXCR4の作用は、EPCが再び断血部位まで補充することを阻止することはすでに知られている(非特許文献3)。糖尿病を有するマウスでは、損傷された組織の中のVEGF及びSDF−1の発現、周辺の血液中のEPC細胞の数量、及び、EPC細胞の上のCXCR4発現量が減少する現象が見られる(非特許文献4)。
マクロファージ炎症性タンパク質−1β(MIP−1β)はリポ多糖で活性化されたマクロファージの培養液で発見された。その分子量は7.8キロドルトンであり、タンパク質は92個アミノ酸前駆物で構成される。文献によると血液の中のマクロファージ炎症性タンパク質−1β(MIP−1β)の数値と糖尿病や心臓血管病が正の相関を持つ。生体外実験の中、マクロファージ炎症性タンパク質−1β(MIP−1β)はTHP−1細胞の活性酸素の生産を促進し、THP−1細胞がPI3k_Rac1を媒介とする酸化圧力を誘導し、人類の臍静脈内皮細胞に付着する(非特許文献5)。この他、高濃度糖で、マクロファージの分泌したタンパク質−1β(MIP−1β)が内皮細胞に付着分子を発現させる。例えば、内皮細胞セレクチン(E-selectin)がそれに該当する(非特許文献6)。
心筋梗塞を有する動物モデルにおいて、梗塞した心臓を誘導し細胞因子を作り、特徴のある白血球集団を虚血のところに補充させる。梗塞したマウスの心筋の中、MIP−1βおよび受容体CCR5は明らかに誘導を受け、発生する(非特許文献7)。しかし、今までMIP−1βは、血管保護および心筋損傷の修復に直接関わるのかということについて、関係する生体実験が不足する。
それゆえ、我々は、MIP−1βをコントロールすることで糖尿病患者の内皮前駆細胞の増殖を増加し、血管修復生成を促進し、組織虚血を改善し、さらに糖尿病併発の血管病理的変化と心臓血管病を予防することに努力し続ける。

本発明は、上述の目的で、生体内において、直接に体内のマクロファージ炎症性タンパク質(MIP)−1βを抑制する作用(例えば、マクロファージ炎症性タンパク質(MIP)−1βに反応する単クローン抗体(mAb)またはアンタゴニストを投与する)で、糖尿病動物の血管保護を有効に強化することを発現した。
よって、本発明の一態様は、糖尿病患者の血管生成を促進する医薬組成物を製造するのに用いられるマクロファージ炎症性タンパク質−1β抑制剤の使用方法に関する。

本明細書中の「マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤」の意味は、MIP−1βタンパク質含量を減少させる、及び/又は、MIP−1βタンパク質の少なくとも一つの活性を低下させる化合物をいう。本発明の実施形態では、MIP−1β−抑制剤化合物は、MIP−1βタンパク質の少なくとも一つの活性を10%、25%、50%、75%またはそれ以上低下させることができる。
本発明の実施形態によると、MIP−1β抑制剤化合物は、MIP−1βタンパク質の発現量を低減することで、膵臓を保護し血糖の上昇を防止する。例えば、MIP−1βを標的とするsiRNA、アンチセンス核酸、リボザイムを使用し、細胞内のMIP−1βの遺伝子発現を抑制することができる。また、MIP−1βタンパク質をコーディングする遺伝子転写を調節すること、または、対応するmRNAを不安定となるようにすることでMIP−1βタンパク質の発現量を減少させることができる。
本発明の実施形態によると、MIP−1β抑制剤化合物は、MIP−1βタンパク質と結合することにより、直接または間接にMIP−1βタンパク質の生物活性を抑制し、膵臓を保護し、血糖の上昇を防止する。例えば、本発明の実施例によると、MIP−1βに反応する抗体とMIP−1βタンパクを競争させ、細胞表面上の受容体に結合させることで、MIP−1βタンパク質の生物活性を抑制することができる。抗体は、全長の単クローン抗体、多クローン抗体、多特異性抗体(例えば、二特異性抗体)、及び、抗体フラグメントを含む。
本明細書中の「抗体」の意味は、特定の抗原と特異的に結合する能力を有する免疫グロブリン分子またはそのフラグメントを指す。抗体フラグメントは、抗体全長の一部を含み、好ましくは、抗体の抗原結合領域または可変領域である。

[実施例1、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤がCXCR4の発現を増加し、EPC細胞の増殖と機能を促進する。]
本実施例では、最初はMIP−1β蛋白質とMIP−1β抑制剤がEPC細胞に対する作用を評価する。二型糖尿病患者のEPC細胞(5×104個)をとり、単独のMIP−1β 1000ng/ml、または、MIP−1βと反応する30μg/mlの中和抗体と共に30分処理する。
図1Aの結果が示すように、単独のMIP−1βで処理するとCXCR4の発現量に影響を与えることがなく、MIP−1βと反応する抗体と共に培養するとEPC細胞のCXCR4の発現量が増加する。
本実施例では、最初はMIP−1β蛋白質とMIP−1β抑制剤がEPC細胞に対する作用を評価する。二型糖尿病患者のEPC細胞(5×104個)をとり、単独のMIP−1β 1000ng/ml、または、MIP−1βと反応する30μg/mlの中和抗体と共に30分処理する。
図1Aの結果が示すように、単独のMIP−1βで処理するとCXCR4の発現量に影響を与えることがなく、MIP−1βと反応する抗体と共に培養するとEPC細胞のCXCR4の発現量が増加する。
よって、さらにチャンバーアッセイ(chamber assay)を用いて、EPC細胞をMIP−1βに暴露する場合、それがSDF−1αに向いて移動する能力を抑制するかを測定する。
図1Bと1Cに示すように、MIP−1βで処理したEPC細胞(M1000セット)は、細胞移動と生体外血管形成能力が明らかに抑制された。しかしMIP−1β抑制?で処理されたEPC?胞(M1000+NEUとNEUセット)、DMセットおよびMIP−1β処理したセット(M1000セット)に比べ、回復した細胞移動と生体外血管形成能力を有する。
図1Bと1Cに示すように、MIP−1βで処理したEPC細胞(M1000セット)は、細胞移動と生体外血管形成能力が明らかに抑制された。しかしMIP−1β抑制?で処理されたEPC?胞(M1000+NEUとNEUセット)、DMセットおよびMIP−1β処理したセット(M1000セット)に比べ、回復した細胞移動と生体外血管形成能力を有する。
MTT及びBrdU細胞増殖分析で測定した結果が示すように、MIP−1βに反応する中和抗体(NEU)で処理したEPC細胞組は、細胞増殖も明らかに増加する(図1D、1E)。
MIP−1β−抑制剤(MIP−1βに反応する中和抗体、NEU)による処理は、VEGF及びSDF−1αの生成を増加する(図1F)。
MIP−1β−抑制剤(MIP−1βに反応する中和抗体、NEU)による処理は、VEGF及びSDF−1αの生成を増加する(図1F)。
[実施例2、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が一型糖尿病動物(ストレプトゾトシン(STZ)で誘導された糖尿病マウスモデル)の血管を保護する。]
糖尿病に患わない健康な動物にマクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤を注射すると、足部血管閉塞手術を行った後の血管修復と生成を抑制し、足部血流の回復を減速し、組織虚血を加重する。こういう効果はマクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤の使用量の増加(M0.1μg,1.0μg,10.0μg)につれて加重する(図2)。
糖尿病に患わない健康な動物にマクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤を注射すると、足部血管閉塞手術を行った後の血管修復と生成を抑制し、足部血流の回復を減速し、組織虚血を加重する。こういう効果はマクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤の使用量の増加(M0.1μg,1.0μg,10.0μg)につれて加重する(図2)。
本実施例は一型糖尿病動物(ストレプトゾトシン(STZ) で誘導された糖尿病マウスモデル)を対象とし生体実験を行う。マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤は糖尿病マウスの血管生成を促進でき、組織虚血を改善できることが証明された。まずマウスの骨髄細胞移植を行い、ストレプトゾトシン(STZ)を注射し、高血糖の糖尿病を誘導する。これから足部血管閉塞をする。
図3から明らかにするように、足部血管閉塞をしたマウスは足部血管閉塞をしないマウスと比べ、血流は同じ程度で減少する。二週間から四週間の後、健康なマウスはだんだん血流を回復するに対し、糖尿病マウスは自ら回復できない。しかし、マクロファージ炎症性タンパク質_1β(MIP−1β)抑制剤を注射した糖尿病マウスの虚血状況は明らかに改善され、健康なマウスのように血流を回復する。
健康な動物は足部血管閉塞を受け、足部虚血が発生する時、外周血の血管内皮細胞(Sca−1+/Flk−1+双陽性細胞)が大量増加し、足部の血管生成を促進する。しかし血糖値の高い一型糖尿病動物は外周血の血管内皮細胞は足部虚血する時上昇しない。MIP−1βに反応する単クローン抗体(mAb)を注射すると、体内のマクロファージ炎症性タンパク質(MIP)−1βに対して、効果的に一型糖尿病動物の手術を行なった後外周血の血管内皮細胞数量を増加し、それで血管修復と生成を促進し、組織虚血を改善する(図3C)。
[実施例3、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤がEPC細胞復位の増加とVEGFの増量調節とSDF−1の発生ということで糖尿病動物の血管生成を促進する。]
免疫組織化学的染色切片の結果が示すように(図4A)、糖尿病マウスの虚血の足部の細血管密度とEPC細胞(骨髄による派生)復位数量は健康なマウスより明らかに減少する。しかし、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤を注射したマウスは虚血筋肉の双陽性細胞数量が処理されていない糖尿病マウスより多い。これはマクロファージ炎症性タンパク質_1β(MIP−1β)抑制剤が効果的にEPC細胞(骨髄による派生)復位数量を増加し、糖尿病マウスの組織虚血による細血管減少の状況を改善する。
免疫組織化学的染色切片の結果が示すように(図4A)、糖尿病マウスの虚血の足部の細血管密度とEPC細胞(骨髄による派生)復位数量は健康なマウスより明らかに減少する。しかし、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤を注射したマウスは虚血筋肉の双陽性細胞数量が処理されていない糖尿病マウスより多い。これはマクロファージ炎症性タンパク質_1β(MIP−1β)抑制剤が効果的にEPC細胞(骨髄による派生)復位数量を増加し、糖尿病マウスの組織虚血による細血管減少の状況を改善する。
eGFP+(緑の蛍光)はEPC細胞(骨髄による派生)を代表する。CD31+(赤い蛍光)は細血管を代表する。DAPI染色(青い蛍光)は細胞核を代表する。緑の蛍光と赤い蛍光が重なり合う部分は当該の細胞が虚血の位置に血管修復をする能力があると表明する。標準化された定量の為に,緑の蛍光と赤い蛍光が重なり合う部分の数量と筋肉繊維の数量との比率を表す(図4B)。
ウェスタンブロット法で、血漿の中の血管生成サイトカインVEGF及びSDF−1αの含量を測定し、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤で処理すると糖尿病マウスの血液の血管細胞因子VEGFとSDF−1αの含有量を増加するという結果がわかる(図4C、4D)。同時に糖尿病による外周血液単球および骨髄単球表面のCXCR4発現量減少の状況を逆転する(図4E、4F)。
上述の研究結果を踏まえ、以下の結論が明らかにした。一型糖尿病マウスのモデルについて、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤はEPC−細胞類の復位により血管生成細胞因子VEGFとSDF−1αの血液の濃度を増加し、外周血液単球および骨髄単球表面のCXCR4発現量を回復する。それで、糖尿病組織の虚血の部分の血管生成を促進し、正常血流の回復を協力し、損傷した組織の壊死を受け止める。
[実施例4、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が二型糖尿病動物(Leprdb/dbマウスモデル)の血管を保護し、血管生成を促進する。]
本実施例は二型糖尿病動物(Leprdb/dbマウスモデル)生体研究を行い、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が二型糖尿病の血管生成と虚血した足部の血流の回復にも効果があると証明するつもりである。図5のように、血糖値の高い二型糖尿病動物にマクロファージ炎症性タンパク質_1β(MIP−1β)を対抗する単クローン抗体(mAb)を注射すると、効果的に足部血管閉塞をした後の血管修復生成と足部の血流回復を促進し、組織虚血を改善する(図5A、5B)。
本実施例は二型糖尿病動物(Leprdb/dbマウスモデル)生体研究を行い、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が二型糖尿病の血管生成と虚血した足部の血流の回復にも効果があると証明するつもりである。図5のように、血糖値の高い二型糖尿病動物にマクロファージ炎症性タンパク質_1β(MIP−1β)を対抗する単クローン抗体(mAb)を注射すると、効果的に足部血管閉塞をした後の血管修復生成と足部の血流回復を促進し、組織虚血を改善する(図5A、5B)。
マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が二型糖尿病動物モデルに対して、効果的に足部血管閉塞をした動物の外周血の血管内皮細胞数量(図5C)、細血管密度(図5D、5E)、循環性EPC細胞の復位数量(図5F)を増加し、骨髄単球細胞のCXCR4発現量を回復する(図5G)。
[実施例5、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が高脂肪飲食による糖尿病マウスモデルの血管を保護し、血管生成を促進する。]
本実施例は、高脂肪飲食による糖尿病マウス生体研究を行い、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が高脂肪飲食による糖尿病の血管生成と虚血した足部の血流の回復にも効果があることを証明するものである。図6のように血糖値の高い二型糖尿病動物にマクロファージ炎症性タンパク質_1β(MIP−1β)を対抗する単クローン抗体(mAb)を注射すると、効果的に足部血管閉塞をした後の血管修復生成と足部の血流回復とを促進し、組織虚血を改善する(図6A、6B)。
本実施例は、高脂肪飲食による糖尿病マウス生体研究を行い、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が高脂肪飲食による糖尿病の血管生成と虚血した足部の血流の回復にも効果があることを証明するものである。図6のように血糖値の高い二型糖尿病動物にマクロファージ炎症性タンパク質_1β(MIP−1β)を対抗する単クローン抗体(mAb)を注射すると、効果的に足部血管閉塞をした後の血管修復生成と足部の血流回復とを促進し、組織虚血を改善する(図6A、6B)。
肥満型糖尿病マウスモデルについて、マクロファージ炎症性タンパク質−1β(MIP−1β)抑制剤が効果的に足部血管閉塞をした動物の外周血の血管内皮細胞数量(Sca−1/Flk−1双陽性細胞)を増加し(図6C)、骨髄単球細胞のCXCR4発現量を回復する(図6D)。
生体内においてマクロファージ炎症性タンパク質−1β(MIP−1β)は血管修復生成を抑制し、足部虚血した後血流回復を減速し、組織虚血を加重する。単クローン抗体(mAb)で直接マクロファージ炎症性タンパク質−1β(MIP−1β)を対抗すると、効果的に糖尿病動物の血管を保護し、血管内皮細胞増殖及び復位を促進し、血管修復生成と組織虚血とを改善する。それゆえ、マクロファージ炎症性タンパク質−1β(MIP−1β)の活性および作用を直接抑制することは、糖尿病の血管病理的変化の予防と治療に直接的で重大な利益を有する。

- 糖尿病患者の血管生成を促進する医薬組成物を製造するのに用いられるマクロファージ炎症性タンパク質−1β抑制剤の使用方法。
- 前記医薬組成物は、糖尿病患者の組織虚血を改善するのに用いられることを特徴とする、請求項1に記載の使用方法。

- 前記マクロファージ炎症性タンパク質−1β抑制剤は、マクロファージ炎症性タンパク質−1βに対して特異的な結合性を有するリガンド化合物であることを特徴とする、請求項1または5に記載の使用方法。
- 前記マクロファージ炎症性タンパク質−1β抑制剤は、マクロファージ炎症性タンパク質−1βに反応する抗体であることを特徴とする、請求項6に記載の使用方法。
- 前記マクロファージ炎症性タンパク質−1β抑制剤は、マクロファージ炎症性タンパク質−1βまたはそのフラグメントに対して特異的に結合しマクロファージ炎症性タンパク質_1βに反応する単クローン抗体であることを特徴とする、請求項7に記載の使用方法。
- 前記マクロファージ炎症性タンパク質−1βに反応する単クローン抗体は、マクロファージ炎症性タンパク質−1βの構造のメイン機能性部位と結合することを特徴とする、請求項8に記載の使用方法。
- 前記マクロファージ炎症性タンパク質−1βに反応する単クローン抗体は、マクロファージ炎症性タンパク質−1βの46〜54:SFVMDYYET(SEQ ID NO:1)に位置するアミノ酸配列を含むペプチト類フラグメントと結合することを特徴とする請求項8または9に記載の使用方法。
- 前記マクロファージ炎症性タンパク質−1βに反応する単クローン抗体は、マクロファージ炎症性タンパク質−1βの62〜74:AVVFLTKRGRQIC(SEQ ID NO:2)に位置するアミノ酸配列を含むペプチト類フラグメントと結合することを特徴とする請求項8または9に記載の使用方法。
