乳癌発症の生涯リスクはほぼ8に1である。erb−B2癌遺伝子(HER−2/neu)は、分子作動体であり、それは乳癌、卵巣癌、胃食道癌、肺癌、膵臓癌、前立腺癌および他の固形腫瘍のかなりの数で過剰発現される。乳癌の約20〜25%を含むいくつかの腫瘍タイプにおける分子オンコドライバーである、HER2の過剰発現(HER2pos)は、より臨床的に悪性度の高い疾患、化学療法に対する抵抗性、高い割合での再発や転移、及びより悪い全体的な予後と関連する。初期乳癌において、HER2の過剰発現は、強化された浸潤性、腫瘍細胞の遊走、及び腫瘍形成性環境を促進するHER2の重要な役割を示唆する、血管新生促進因子の発現と関連する。非浸潤性乳管癌(DCIS)患者のレトロスペクティブ分析において、HER2を過剰発現するDCIS病変は、HER2を過剰発現しないDCIS病変よりも浸潤性の高い乳癌と6倍以上関連がある可能性が高かった。

好ましい実施形態は、HER2タンパク質に由来するか、またはそうでなければ基づいて単離されたペプチドの使用を含む。好ましい実施形態の結合ペプチドは、対応するHER2タンパク質のエピトープを提示する。現在好ましい実施形態は、6つのHER2 MHCクラスII結合ペプチド/エピトープを備えているが、患者において免疫学的に十分に活性である限り、HER2分子全体の任意の成分が他の結合ペプチドの供給源として使用することができ、他の可能なMHCクラスII HER2ペプチドが、本実施形態で使用されうる。

PBMCを被験体から得て、分離した後、本明細書に記載された任意の血液検査/アッセイを実行する前に、当業者に周知の方法を使用して凍結保存することができる。
好ましい実施形態は、以下の実験実施例を参照してさらに詳細に記載される。これらの実施例は、例示のみを目的として提供され、特記しない限り限定することを意図するものではない。したがって、好ましい実施形態は以下の実施例に決して限定されるべきでなく、むしろ、本明細書で提供される教示の結果として明らかになるありとあらゆる変更を包含すると解釈されるべきである。
方法
患者の選択と研究計画
上記で参照した6個のHER2クラスII結合ペプチドでパルスした患者の非培養の末梢血単核細胞(PBMC)から、酵素結合免疫吸着スポット(ELISPOT)アッセイを用いてIFN−γ産生を測定することによって循環抗HER2 CD4+Th1応答を生成した。ELISPOTはKoskiらにより記載された方法に従って行った。簡潔にいうと、PVDFメンブレンプレート(マブテック社、シンシナティ、OH)を、抗IFN−γ捕捉抗体(1−D1K(Mabtech社))により一晩コーティングした。密度勾配遠心分離を用いて単離した凍結保存PBMCを、5%ヒト血清を補充した予熱DMEM中で解凍した。プレートを洗浄し、ブロッキングした後、PBMCをトリプリケートで播種し(2×105細胞/ウェル)、プレートを24〜36時間37℃でHER2由来クラスII結合ペプチド(4μg)(ペプチド 42〜56(配列番号:l)、ペプチド 98〜114(配列番号:2)、ペプチド 328〜345(配列番号:3)、ペプチド 776〜790(配列番号:4)、ペプチド 927〜941(配列番号:5)、及びペプチド 1166〜1180(配列番号: 6)、培地単独(無刺激対照)で、又は陽性対照(抗ヒトCD3及び抗CD28抗体、(各0.5μg/mL)、共にBD Pharmingen社、San Diego,CA)と共にインキュベートした。洗浄後、検出抗体(7 B6−l−ビオチン(マブテック社);100μg/mL)を各ウェルに添加し、プレートを37℃で2時間インキュベートした。次に、PBS+0.5%FCS中で1:1000に希釈したストレプトアビジン−西洋ワサビペルオキシダーゼをインキュベーション前に37℃で追加の1時間添加した。その後、TMB基質溶液(Kirkegaard&Perry Laboratories、Gaithersburg、MD)を添加しスポット形成を明らかにした。発色後、ウェルを水道水で洗浄した。スポット形成細胞(SFC)は、自動ELISPOT リーダー(ImmunoSpot CTL,Cleveland,OH)を用いて計数した。
ELISPOTのアッセイ間の精度は前述のように、Maecker,H.T.,et al.BMC Immunology 9:9(2008)(Maecker,et al.)によって実行した。平均分散(CV)(3日間で3つの並列複製)を、HER2細胞外ドメイン(ECD)ペプチドミックス(ペプチド42〜56(配列番号:1);ペプチド98〜114(配列番号:2);及びペプチド328〜345(配列番号:3))で生体外で刺激した5人のドナーの累積Thl応答に対してプロットしたとき、特徴的な非直線関係が観測された。図3Aに示すように、累積応答がゼロに近づくと平均CVは劇的に増加した。CVと累積応答レベルとの間の非直線関係のため、別の3日間の3回のアッセイの標準偏差(SD)をアッセイ間の可変性の尺度として累積Th1応答に対してプロットした。同上。図3Bに示すように、SDは累積応答と直線関係であることが見出された(実線は、平行点線で示される回帰の95%信頼区間を伴う、生成されたSDの線形回帰を表す)。(R2=0.96.p<0.0001)
ELISAを、内因性のIgG1及びIgG4抗HER2抗体について患者の血清を試験するために実施した。EIA/RIAプレートを、重炭酸塩緩衝液中のHER2 ECDペプチド(5μg/ml、Speed Biosystems社、Rockville、MD)でコーティングし、室温(RT)で一晩インキュベートした。翌日、プレートをPBS中の1%カゼインでブロックし、血清(1:100希釈)をブロッキング緩衝液に四重(quadruplicate)に添加し、2時間インキュベートし、1:500希釈のIgG1またはIgG4(ライフテクノロジーズ、グランドアイランド、NY)のいずれかに特異的なHRP結合抗ヒト二次抗体を添加する前に、3回洗浄した。1時間のインキュベーション後、プレートを洗浄し、TMB基質溶液(カークガード&ペリー・ラボラトリーズ)を用いて現像した。
PBMC懸濁液を、FACS緩衝液(PBS+1%FCS+0.01%アジド)中で調製し、抗ヒト−CD3、−CD4、−CD8、−CD83、−HLA−DR、−CDlポンド、−CD33、−CD19、−CD56、−CD16(全てBD Biosciences社、San Jose, CA)、−CD4、及びCD25(ともにBiolegend、サンディエゴ、CA)を使用し、相対的なPBMCの免疫表現型を決定した。洗浄後、細胞を、抗体の混合物と室温で30分間インキュベートした。インキュベーション後、細胞をFACS緩衝液で3回洗浄し、2%パラホルムアルデヒドで固定した。染色した試料を、24時間以内に分析した。FoxP3固定/透過処理キット(Biolegend)を使用して抗FoxP3(eBioscience社、San Diego, CA)を有するPBMCの細胞内染色を、製造業者の指示に従って行った。フローサイトメトリー分析は、BD LSR−IIサイトメーターを用いて行い、データセットは、セルクエストプロTMソフトウェア(BD Biosciences)を用いて分析した。
HER2pos−DCISと−IBC腫瘍由来のホルマリン固定、パラフィン包埋組織ブロックの薄片を作り、ヘマトキシリン及びエオシン(H&E)で染色し、腫瘍周囲のリンパ球浸潤を評価した。HER2pos−DCISと−IBC腫瘍由来のサンプルケースにおいて、多重標識IF(パーキンエルマー、マサチューセッツ州ウォルサム)を使用してリンパ球亜集団を調べた(以下を参照,Wang,C,et al.,Journal for Immunotherapy of Cancer 118:1(Suppl.1)54(2013)(Wang,C,et al)。腫瘍は、チラミドシグナル増幅を用いる同種の蛍光標識で、CD4、CD8、CD20及び4’6’−ジアミノ−2−フェニルインドール(DAPI)について染色した。画像は、ベクトラマルチスペクトル顕微鏡を用いて、腫瘍、間質、及びT−/B−リンパ球を識別するパターン認識ソフトウェアで分析した。
HER2発現のスペクトラムを有するBC細胞株((Ithimakin,S.,et al,Can.Res.73:1635−46(2013)))−HER2highSK−BR−3、HER2intermediateMCF−7、HER2lowMDA−MB−231(American Type Culture Collection)−をRPMI−1640+10%FBS(CELLGRO/メディアテック、バージニア州マナッサス)で培養した。50x103BC細胞をトランスウェルシステム(BD Biosciences社)に播種し、そして、106CD4+T細胞及び105DC1(成熟DC)またはiDC(未成熟DC)と共培養した。DC1、iDC及びCD4+T細胞を、シャルマらにより記載されるように、ワクチン接種後の選択した患者から得た。DCl/iDCを、クラスIIHER2または無関係の対照BRAFペプチド(20μg/ml)で37℃24時間パルスした。具体的には、Figure10Aに示すように、50xl03SK−BR−3細胞を、培地のみ(完全培地)、106ヒトCD4+T細胞のみ(CD4+のみ)、106CD4+T細胞+105の各HER2クラスIIペプチド(iDCH)−または無関係なクラスII BRAFペプチド(iDCB)−パルスiDC、及び106CD4+T細胞+105の各HER2(DCl H)−またはBRAF(DCl B)−パルスDCIと共培養した。DC1/iDCを、クラスII HER2または無関係な対照BRAFペプチド(20μg/ml)で37℃24時間パルスした。対照ウェルは、培地またはCD4+T細胞のみを含有した。
IFN−γ及びTNF−α(BD Pharmingen社製)用の捕捉及びビオチン化検出抗体及び標準物質を製造業者のプロトコルに従って使用した。
記述統計を、患者の特性、免疫応答変数の分布を要約するために使用した。連続変数は平均、SEM、及び頻度と割合による範囲及びカテゴリの変数によって要約した。データ変換(自然対数または平方根)は、パラメトリック試験の仮定を満たすために必要なときに、適用した。事後Scheffeペアテスト(パラメトリック)又はKruskal−Wallisテスト(ノンパラメトリック)を有する分散分析は、3より大きい群の連続変数を比較するために使用した。スチューデントのt検定は、2群の比較のために使用した。フィッシャーの正確確率検定は、マルチレベルのテーブルにおけるカテゴリ変数を比較するために使用した。スチューデントのペアt検定とマクネマーの正確確率検定はTh1応答変数における患者内ペアの変化を評価するために使用された(例えば、ワクチン接種前対ワクチン接種後)。p値p<00.05は、統計的に有意とみなした。すべての試験を両側で行った。統計分析は、SPSS(IBM社製)またはStatXact(Cytel社カリフォルニア州サンディエゴ)のいずれかで行った。
患者の特性
ランダムで継続的な登録後、143人の被験者が試験の包含基準を満たした。参加者の平均年齢は53.1±1.4(範囲、21〜88)歳で大部分(79.0%)は白人種だった。採血された時点での患者/ドナーコホートは、図1及び上記の方法の節のセクションに示される。研究参加者のドナーの人口統計学及び腫瘍関連特性は、上記表1に詳述される。HER2pos−DCIS及び−IBCコホートの、それぞれの26人(83.9%)及び11人(50.0%)の患者は、それぞれ以前に、HER2pos−DCISのための術前補助1型極性化(DC1)ワクチン接種試験に登録していて、彼らの患者/腫瘍特性はSharmaらによって報告されている。
末梢血単核細胞(PBMC)を使用して、腫瘍形成の連続体に亘って全身抗HER2 CD4+Th1応答の変化を生体外でのHER2ペプチド刺激IFN−γELISPOTアッセイ法により、予め調べた。3個のTh1応答の測定基準をグループ間で比較した:(a)全体の抗HER2応答(1より多いペプチドに応答する患者の割合)、(b)反応性ペプチドの平均数(レパートリー)、及び(c)上述した6個のクラスIIペプチドに亘る累積応答。健康なドナー(HD)、または良性乳房疾患(BD)患者と比較したとき(図1、コホートA)、Th1応答における有意な段階的な減少がHER2pos乳癌患者において観察された。未治療HER2pos−DCIS(図1、コホートC)で開始し、治療ナイーブステージI/IIHER2pos−IBC(図1、コホートF)における低点に達し、Th1免疫のこの進行性の損失は、すべてのTh1応答の測定基準全体に均一に観察された。例えば、全体的な抗HER2応答は、HD/BDにおける100%からHER2pos−DCISにおける84%に、HER2pos−IBC患者における32%に減少した(p<0.0001)。同様の有意な段階的な減衰応答レパートリー(5.2±0.2対4.5±0.4対2.0±0.3対0.4±0.2、p<0.0001)及び累積応答(259.9±23.5対225.1±25.5対126.1±24.4対32.3±5.4、スポット形成細胞(SFC)/106細胞、p<0.0001)が、HD、BD、HER2pos−DCIS、及びステージI/IIHER2pos−IBC患者に亘る、それぞれに、図5Aに示すように全体で観察された。事後の比較では、応答レパートリー(p<0.001)及び累積応答(P=0.001)によって評価して全体的な応答性(P=0.07)で評価しないとき、HER2pos−DCIS患者におけるTh1応答は、HDにおける応答よりも有意に低かった。HER2pos−IBC患者でのTh1応答は、これらの患者が、HER2pos−DCIS患者と比較して有意に低い全体的な応答性(P=0.0003)、レパートリー(p=0.001)、及び累積応答(p<0.001)を示したことでさらに抑制された。106のPBMCあたりの反応性細胞の割合は、HDにおける0.03%からHER2pos−IBC患者における0.003%に及んだ。
HER2neg−IBC患者におけるTh1プロファイルを、抗HER2Th1免疫力の相対的低下を伴うサブグループを識別するために調べた。明確なHER2neg−IBC(IHC0/1+)患者(n=11)と比較して、曖昧なHER2発現(IHC2+/FISH陰性)IBC患者(n=7)は、有意に低い全体的な応答性(28.6%[IHC2+]対100%[IHC0/1+]、P=0.002)、レパートリー(0.3±0.2対3.9±0.3、p<0.0001)、及び累積応答(21.4±6.5対191.2±11.7 SFC/106細胞、p=0.002)を示した。曖昧なHER2発現IBC患者におけるTh1応答は、図5Aに示されるHER2pos−IBC患者に見られるものと同様だった。ELISPOTにより測定されるIL−10産生及びフローサイトメトリーによるTreg(CD4+CD25+FoxP3+)細胞の相対比率によって測定されるIL−10産生は、曖昧なHER2neg−IBC患者及び明確なHER2neg−IBC患者の間で有意差はなかった(データは示さず)。
評価可能なドナーのサブグループにおける免疫力をIFN−γELISPOTを介して抗CD3/抗CD28に対するTh1応答を測定することによって評価した;これらの応答はまた、すべてのELISPOTアッセイにおけるドナー特異的ポジティブコントロールとしての機能も果たした。抗CD3/CD28応答のメディアンは、それぞれ図5(b)に見られるように、HD/BD(n=31)、HER2neg−DCIS(n=11)、HER2neg−IBC(n=11)、HER2pos−DCIS(n=5)HER2pos−IBC(n=11)、及びT/C治療HER2pos−IBC(n=37)のコホートとの間に、差がなかった(1098対1104対1032対1099対1318対1032 SFC/2xl05細胞、p=0.22)。さらに、刺激をリコールするためのTh1応答は[破傷風トキソイド(105±17.0対96±15.6対101±11.3SFC/2xl05)、及びカンジダ・アルビカンス(185±10.2対199±15.3対181±14.6 SFC/2xl05)]、それぞれ図8Aに見られるように、評価したHD(n=10)、HER2pos−IBC(n=11)、及びT/C治療IBC(n=10)のコホート間で同様だった。まとめると、これらのデータは、HER2ドライブBCにおける進行性の抗HER2 Th1応答の損失が、ホストレベルのT細胞免疫反応不顕性またはIBC患者のPBMC中の損なわれた抗原提示能に起因するものではないことを示唆している。
全身IFN−γposCD4+応答の損失がIBC病変への不均衡なリンパ球輸送に関連しているかを決定するために、病理学的レビューのために利用可能な14個のHER2pos−DCIS及び8個のHER2pos−IBC病変の免疫組織化学(IHC)分析を実施した。結果を図9Aに示す。中程度(>15%の間質の関与)から高程度(>25%)のリンパ球のレベルが、評価可能な患者(上、矢印で示す)の大部分(12/14;85.7%)におけるDCIS含有管の外側の間質領域で集約することが観察された一方、リンパ球(矢印)の相対的な不足は、全8人のIBC患者において侵襲病巣の周りに見られた(98/8;100%)(下)。
試験管内でのHER2highSK−BR−3、HER2intermediateMCF−7、及びHER2lowMDA−MB−231 BC細胞株のTh1媒介効果もまた評価した。HER2パルスDC1で感作したHER2クラスIIペプチド特異的CD4+Th1細胞の増加割合を、上記のタイプのHER発現BC細胞と、トランスウェル培養系を用いて共培養したところ、図10Aで示されるウエスタンブロット分析でカスパーゼ3が増加していることから証明されているようにSK−BR−3は著しい用量依存性アポトーシスを起こし、図11Aで認められるようにMCF−7で著しい用量依存性アポトーシスを起こすがMDA−MB−231 BC細胞株で起こさなかった。対照的に、図10A及び11Aに見られるように、未成熟DC(iDC H及びiDC B)または対照クラスIIペプチド(BRAF)パルスDC1(DC1 B)で感作したCD4+ T細胞と共培養したBC細胞においては、アポトーシスは比較的わずかであった。ELISAによるこれらの共培養上清中でなされたTh1サイトカインの定量から、図10Aに示されるCD4+:BRAF制御DCl、共培養に比べて、CD4+T細胞:HER2パルスDClからのIFN−γ及びTNF−α産生が有意に増加することが示され、これは観察されたアポトーシスの程度に相当するものであった。
T/C治療及びHER2パルスDC1ワクチン接種後のHER2pos−IBC患者におけるTh1応答の差動効果を分析し、結果を図12A、上パネルに示した。未治療ステージI/II HER2pos−IBC患者(n=22;図1コホートF)及びT/C治療ステージI−III HER2pos−IBC患者(n=37;図1コホートG)は、抗HER2応答性(31.6%未処理対45.9%のT/C治療、P=0.39)、レパートリー(0.4±0.2対0.8±0.2、P=0.24)、または累積応答(32.3±5.4対54.5±12.0 SFC/106、P=0.97)で有意に異ならなかった。しかしながら、図12B、上パネルに示すように、11人のステージI HER2pos−IBC患者(図1コホートH)におけるHER2パルスDC1ワクチン接種後は、抗HER2応答性(18.2%ワクチン前対90.9%ワクチン後、p=0.0035)、レパートリー(0.3±0.2対3.7±0.5、p<0.0001)、及び累積応答(29.7±7.9対162.8±33.7 SFC/106、P<0.0001)において、著しい改善が認められた。T/C治療後でなく、DC1ワクチン接種後の著しいTh1回復効果は、図12Cに見られるように、ステージI未処理(n=11)、T/C治療(n=8)、及びワクチン接種(n=11)HER2pos−IBC患者間のステージ対応比較で持続した。
これらの知見のトランスレーショナルな(translational)関連性を評価するために、T/C治療HER2pos−IBC患者におけるTh1応答の変動がその後の乳房イベント(BE;任意の局所領域/遠隔再発と定義する)の発生に関連するか判断するために評価を行った。追跡期間の中央値は33.5(四分位範囲「IQR」25.5〜45.8)か月だった。以下の表2に示すように(トラスツズマブ及び化学療法後に、引き続き乳がんイベント(任意の局所領域または全身再発として定義される)を受けるHER2pos−IBC患者の人口統計学的及び臨床的特徴を示す)、8人の患者(21.6%)が29(IQR16.2〜36)カ月持続時間の中央値でT/C治療後のBEを負った。図13Eの左のパネル、BEのない患者と比較して、BEを受ける患者は抗HER2応答性(上)(12.5%+BE対55.2%BEなし;P=0.048)及び累積応答(下)(9.4±3.6対66.9±14.5 SFC/106;P=0.046)が大幅に抑制されたが、応答レパートリー(中央)(1.03±0.3対0.13±0.1;P=0.11)はそうでなかった、ことを示す。
チェックポイント阻害剤の出現(Topalian,S.L.,et al.,N.Eng.J.Med.366:2443−54(2012))、及び組織特異的エピトープに対するワクチン(Kantoff,P.W.,et al,N.Eng.J.Med.363:411−22(2010))、Toll様受容体アゴニスト又は養子T細胞療法(Kalos,M.,et al,Sci.Transl.Med.3(95):95ra73(2011)and Rosenberg,S.A.,Nature Reviews Clinical Oncology 8:577−85(2011))のような免疫調節戦略の使用はより効果的な癌免疫療法のためのお膳立てをする。これらの治療の多くは、広範な免疫調節を対象としている。これらの発見と同時に、ゲノムプロファイリングは、V−RAFマウス肉腫ウイルス癌遺伝子ホモログ−BL(BRAF)、上皮成長因子受容体(EGFR)、肝細胞増殖因子受容体(C−MET)、及びHER2を含む、腫瘍形成の特異的分子ドライバを特定してきた。治療法は、このような「オンコドライバー(oncodrivers)」を標的とした治療方法は応答速度を奨励することを達成する一方、その成功は相対的に短命である、なぜなら多くの腫瘍は最終的に再発するか、または治療抵抗性になるからである(Pohlmann,et al.and Flaherty,K.T.,et al,N.Eng.J.Med.363:809−19(2010))。腫瘍の発生時にオンコドライバー特異的な免疫欠陥を特定することは、特定の癌サブタイプに合わせた治療の機会を提供しうる。本明細書で記載されるのは、既定のBC表現型、すなわちHER2/neuの分子オンコドライバーに特異的な腫瘍形成においてCD4+Th1免疫欠陥を識別する第1の研究であると考えられる。
抗HER2 CD4+Th1応答は、HER2陽性乳癌における術前補助療法後の病理的応答に相関する新規免疫である
現代的な治療では、より大きな切除可能な腫瘍を有する患者は、多くの場合、トラスツズマブと化学療法(T/C)の術前補助投与から、ほぼ40%〜60%病理学的完全応答(pCR)を達成する恩恵を受ける。Gianni,L.,et al,Lancet 375:377−84(2010);Untch,M.,et al,J.Clin.Oncol.28:2024−31(2010);Untch,M.,et al,J.Clin.Oncol.29:3351−7(2011)を参照。外科手術での残留疾患の事実(<pCR)と比較することで、術前補助T/C後のpCRの達成は、減少した再発及び改善された長期生存のための確立された代理である。
参考例に記載のように、循環抗HER2 CD4+Th1応答を、生体外で6個のHER2由来クラスIIペプチド(ペプチド42〜56、ペプチド98〜114、ペプチド328〜345、776〜790ペプチド、ペプチド927〜941、及びペプチド1166〜1180)(配列番号:1−6)でパルスした非培養PBMCにおいて、酵素結合免疫吸着スポット(ELISPOT)アッセイを介してIFN−γ産生を測定することによって試験した。参考例に記載のようにELISPOTを行った。HLA−A2.1posドナー由来のPBMCを、PMA(50ng/ml)及びイオノマイシン(1μg/mlシグマアルドリッチ)を陽性対照として;二つのHER2由来クラスIペプチド:ペプチド369〜377(配列番号:7)及びペプチド689〜697(配列番号:8)で刺激した。
研究は、87人の患者を含んでいた。未治療HER2pos−IBC患者(n=22)における抑制された抗HER2Th1応答は、T/C治療(n=65)の後に、全体的に改善しなかった。術後補助T/Cと比較して、術前補助T/C(61.5%)はより高いTh1レパートリー(1.5対0.8、P=0.048)と関連していた。pCR(n=16)及び<pCR(n=24)患者は、人口統計学的/臨床的特性において差が認められなかった一方、pCR患者はERneg腫瘍をより持っているようだった。pCR患者は<pCR患者と比較して、劇的に高い抗HER2応答性(94%対33%、P=0.0002)、レパートリー(3.3対0.3、p<0.0001)、及び累積応答(148.2対22.4、p<0.0001)を示した。この相違は、CD4+T−bet+IFN−γ+表現型によって媒介され、<pCR患者の免疫不全、ホストレベルのT細胞免疫反応不顕性、または増加した免疫抑制集団に起因しなかった。4人の<pCR患者において、Th1レパートリー(3.7対0.5、P=0.014)及び累積応答(192.3対33.9、P=0.014)はHER2−パルスDC1ワクチン接種後に有意に改善した。
抗HER2 Th1応答は、術前補助T/C後の病理学的応答と相関する新規の免疫である。<pCR患者で術前補助療法を受けたHER−2発現患者において、抑制されたTh1応答は、HER2Th1免疫介入で復元することができ、pCRまたは再発率を改善させることができる。
抑制された抗HER2 CD4+ Tヘルパー1型応答は完全に治療したHER2−陽性乳癌患者の再発と関連する−免疫監視の新しい役割
試験計画
ペンシルベニア大学の施設内倫理委員会による承認の後、インフォームドコンセントを得た後95人のHER2pos−BC患者を、非バイアスされた方法で募集した。適格患者は、組織学的に確認された浸潤性乳癌(IBC)及びHER2/neuの過剰発現(IHC3+または2+/FISH陽性)を有し、免疫抑制薬を受けていなかった。未治療(すなわち、登録時に決定的な治療を受けていない)のステージI−III HER2pos−IBC患者(n=22)における抗HER2CD4+Th1応答をT+C治療を完了したステージI−IV HER2pos−IBC患者(n=73;すなわち、術前補助又は術後補助のT+C決定的な治療)のTh1応答と比較した。T+C治療を受けた患者では、分析は再発状態によって層別化した。
T+C−治療を受けた患者は、以下の療法のいずれかを術前または術後のいずれかで受けた:(1)AC/TH:アドリアマイシン/シクロフォスファミド(4サイクルで2週毎)その後タキソールとトラスツズマブの併用(12週間、毎週)。(2)TCH:トラスツズマブを併用したタキソテール/カルボプラチン(6サイクルで3週間毎)、または(3)TC−H:トラスツズマブを併用したタキソテール/シクロフォスファミド(4サイクルごとに3週間)。すべての患者は、通年の治療を完了するために追加のトラスツズマブ単独(3週間毎)治療を受けた。
参考例で上述したように、末梢血抗HER2CD4+Th1応答を、生体外で6個のHER2由来MHCクラスII−ペプチド(42〜56、98〜114、328〜345、776〜790、927〜941、1166〜1180)でパルスした非培養のPBMCで、ELISPOTを介してIFN−γ産生を測定することによって、試験した;Datta,J.,et al.,OncoImmunology;Datta,J.,et al.,Breast Cancer Res.、and Koski,et alも参照。簡潔にいうと、25〜30mLの全血を各研究参加者から、5本のヘパリン添加収集チューブ(BDバイオサイエンス)に回収した。静脈切開の直後(<4−6時間)、PBMCを製造業者の指示に従って密度勾配遠心分離(すなわち、フィコール−パック法、Fisher Scientific社)を用いて単離し、−80℃で、10%DMSOヒト血清中に10×106細胞/mLで凍結保存した。すべてのPBMCを凍結保存から4週間以内に利用した。解凍時の生存率は、60〜80%だった。PVDF膜プレート(Mabtech社社)を、抗IFN−γ捕捉抗体で一晩コーティングした。凍結保存したPBMCを、予熱したDMEM+5%ヒト血清中で解凍し、前記培地中で1×106のPBMCで調製した。プレートを洗浄してブロックした後、PBMCををトリプリケートで播種し(2×105細胞/ウェル)、いずれかのHER2ペプチド(複数可)(4μgの、GenScript社、ピスカタウェイ、NJ)、培地単独(非刺激対照)、または陽性対照(抗ヒトCD3/CD28抗体[0.5μg/mL;BD Pharmingen社製])と共に24〜48時間37℃でインキュベートした。
PBMCをFACSバッファー(PBS+1%FCS+0.01% アジド)内に懸濁した。PE/FITC/Cy5の共役マウス抗ヒトCD3、CD4、CD25、T−bet、GATA−3、IFN−γ、またはサブクラス適合対照(BDバイオサイエンス)を、Datta,J.,et al.,Breast Cancer Resに記載されたように相対PBMC免疫表現型を決定するために使用した。FoxP3の固定/透過化キット(Biolegend)を使用して、抗FoxP3の細胞内染色を、製造業者の指示に従って行った。分析は、BD LSR−IIサイトメーターを使用して実行し、データセットをCellQuest Proソフトウェアを用いて分析した。
PBMCを24ウェルプレートにDMEM+5%ヒト血清中1.2×106細胞/mLで再懸濁し、HER2クラスIIペプチド混合物(24μg/mL)でパルスした。刺激を受けていない及び各ドナーからの抗CD3/CD28抗体をパルスしたPBMCを、それぞれ、負および正の対照とした。37℃で6時間インキュベーションした後、タンパク質輸送阻害剤ブレフェルジンA(シグマアルドリッチ;を10μg/ml)を各試料に添加し、一晩インキュベートした。洗浄後、細胞を室温で30分間、抗ヒトCD4で染色した。細胞をその後、製造者の指示に従って二回洗浄し、Foxp3固定/透過化キット(Biolegend)を用いて固定し、透過処理し、そして、抗T−bet、抗GATA−3、および抗IFN−γ(Biolegend)で30分間染色した。インキュベーション後、細胞を洗浄し、BD LSR−IIサイトメーターで分析した。
記述統計により、患者の特性および免疫応答変数の分布を要約した。示されるように、非再発と再発HER2pos−IBCコホート間の比較を行った:(a)2群/単変量試験−独立スチューデントのt検定(パラメトリック連続)、ウィルコクソン順位和検定(ノンパラメトリック連続)、およびχ2検定(カテゴリカル)。(b)2群未満のテスト−事後ボンフェローニテストと一方向分散分析。単変量試験でp<0.1を持つ変数を、前方に段階的多変量ロジスティック回帰モデルに入力して再発に対する独立した相関を決定した(バイナリ結果 はい/いいえ)。
患者の特性
全体のコホート(n=95)の人口統計および腫瘍関連の特性を以下の表3に詳述する。T+C治療患者(n=73)では年齢の中央値は49歳(範囲、24〜85)で、大部分が白人だった(87.5%)。患者の大多数は、エストロゲン/プロゲステロン受容体陽性(ER/PRpos)腫瘍(58.9%)を有し、局所的な進行/リンパ節陽性疾患を提示する(臨床ステージI:9.6%、II:47.9%、III:42.5%)。術前補助T+Cを37人(50.7%)の患者に投与した。単純または非定型的乳房切除術を最も頻繁に(53.4%)行い、アドリアマイシン/シクロフォスファミド/タキソール/ハーセプチンは、一般に利用されるT+C措置(57.5%)だった。
図15Aにおいて、未治療(n=22)、T+C治療非再発(n=48)、及びT+C治療再発(n=25)患者の間で抗HER2Th1応答性、レパートリー、および累積応答を比較した。未治療患者からの抗HER2Th1応答を「ベースライン」として使用して、再発のない患者と比較して、再発を負ったHER2pos−IBC患者において、大幅に抑制された抗HER2Th1応答性(未治療:36.4%対再発:8.0%対非再発:83.3%、P<0.0001)(上)、レパートリー(0.6±0.2対0.1±0.1対1.5±0.2、P<O.0001)(中央)、および累積応答(32.8±4.7対14.8±2.0対80.2±11.0、P<0.0001)(下)をそれぞれ、観察した。
図15Cは、T−bet(Th1転写因子(Szabo,S.J.,et al,Science 295(5553):338−342(2002)を参照)またはGATA−3(Th2転写因子(Zheng,W.,et al,Cell 89(4):587−596(1997)を参照)及び細胞内IFN−γの共発現についてフローサイトメトリーにより評価したHER2刺激PBMCを示す(上)。非再発コホートと比較して再発コホートにおいて、それぞれ、HER2特異的CD4+GATA−3+のIFN−γ+またはCD4+ GATA−3+のIFN−γ−ではなくCD4+T−bet+のIFN−γ+(0.25±0.1%対0.02±0.01%、P=0.039)表現型の大幅の割合の減少が観察された(中央)。HER2−刺激IL−4+応答性(P=1.00)、レパートリー(P=0.84)、および累積応答(P=0.46)−Th2機能の基準−は非再発と再発コホートとの間で差がなかった(下)。
関連する人口統計学的/臨床病理学的特徴から交絡調整により、抗HER2 Th1応答と再発との間の関連の独立性を試験した。一変量テストで、再発及び非再発コホートにおいて年齢、閉経状態、人種、肥満度指数、併存疾患、臨床ステージ、ホルモン受容体状態、LVIの存在、核異型度、または乳房切除の必要によって差は認められなかった。しかし、術後補助T+Cを受けた患者(P=0.021)および術前補助療法後に残存病変を有するもの(P=0.006)の間で再発はより高頻度に見られた。多変量回帰を介してこれらの関連の変数を調節すると、以下の表5に見られるようにT+C療法の順序(P=0.304)ではなく抗HER2Th1応答(P<0.0001)が、依然として再発と独立して関連していた。
この分析では、循環抗HER2 CD4+ Th1応答が完全に治療されたHER2pos−IBC患者における乳癌再発と相関がある新規免疫となりうることが示されている。免疫不全またはホストレベルのT細胞アネルギーに起因しない一方、再発を被る患者における抑制された抗HER2 T細胞応答はTh2又はTregでなく、主にTh1表現型によって駆動される。関連する人口統計学的/臨床病理学的因子を制御すると、抗ITER2Th1応答は、独立して再発と関連していた、リスク調整後の分析で、Th1−非応答性の患者はTh1応答性患者と比較して有意に亢進したDFSを実証した。これは疾患の再発と乳房腫瘍経営における既知のオンコドライバーーすなわちHER2/neuと特異的に関連するホストレベルの免疫との間の相関の最初の証明であるようだ。
抗HER2Th1応答は、HER2陽性乳癌患者における術前補助化学療法に対する応答を評価する点で乳房MRIよりも優れている
ペンシルバニア大学で30人の患者を遡及的に組織学的に確認されたIBCと同定し、HER2/neuの過剰発現を確認した。いずれの患者においても、遠隔転移の証拠はなく、いずれも免疫抑制薬物療法を受けていなかった。患者に、偏りのない方法で抗HER2 TH1分析を行った。最初の治療後のMRIレポートを収集し、画像診断は、PCRおよび免疫応答を知らされていない乳房の放射線科医によって検討された。画像処理に基づく腫瘍応答を、標準RECIST基準に基づいて評価し、これは固体の病変と同様に非質量増大評価を含むように変更された。完全な病理学的応答は、切除乳房検体および試料採取されたリンパ節の病理学的検査での残留浸潤癌の不存在として定義した(すなわち、ypTO/TisのypNO)。参考例に記載されたように、循環抗HER2CD4+Th1応答を、累積応答を決定するために酵素結合免疫スポット(ELISPOT)アッセイを介してIFN−γ産生を測定することによって、生体外で6個のHER2由来MHCクラスIIペプチド(ペプチド42〜56、ペプチド98〜114、ペプチド328〜345、ペプチド776〜790、ペプチド927〜941、およびペプチド1166〜1180)(配列番号:1〜6)でパルスした非刺激PBMCにおいて検討した。参考例に記載のようにELISPOTを行った。MRIおよび抗HER2Thl応答は、病理学的応答と相関し、標準的な診断の指標が計算された。
完全な病理学的応答(PathCR)による被験者のHER2累積応答の分布は、13人の患者がPathCRを有しており(平均値±SD=167.0±115.6 SFC/106細胞、最小値=53.0 SFC/106、最大値=418.0 SFC/106細胞)、17人の患者がPathCRを有していなかった(平均値±SD=23.9±15.2 SFC/106細胞、最小値=0.4 SFC/106、最大値=53.3 SFC/106細胞)。患者は、50 SFC/106細胞のカットポイントに二分された(「低」<50、「高」>50)。
抗HER2Th1免疫応答は、治療後のMRIと比較して著しく正確な診断指標を実証した。「高い」抗HER2 Th1応答の存在は、HER2+ BCにおけるPCRの評価において治療後にMRIを使用することより優れている。当業者は、本明細書に提示される方法を用いて、HER2+ BC患者の術前補助化学療法に対する応答を評価する点で、患者の免疫応答を監視する利点を理解するであろう。

請求項1に記載の方法に従って、前記T/C治療後の前記患者における抗HER2 CD4+Th1応答の程度を測定して、前記応答が、術前補助療法の病理学的完全奏効(術後の病理で残存浸潤性乳癌がない)と関連する有意に高い抗HER2 CD4+Th1応答であるか、又は非病理学的完全奏効に関連するより低い応答であるかを判断するステップ;を含む、方法。 - 前記患者における非病理学的完全奏効の場合には、前記患者の抗HER2 CD4+Th1応答が、請求項11の方法に従って、DCワクチン接種によって回復される、請求項19に記載の方法。
- 癌を有するまたは発症するリスクがある哺乳動物被験体を診断または治療するための方法であって:
前記被験体から血液を得るステップ;
その後抗癌CD4+Th1応答の抑制を測定する血液検査を行うステップ、及び、抑制の場合には;
前記被験体に、DCワクチン、トラスツズマブのような標的癌治療、化学療法のような従来の癌治療、外科手術及び放射線照射からなる群から選択される癌治療薬を有効量で投与するステップ;を含む、方法。 - 標的乳がん治療に加え化学療法を完了した後に前記癌の再発リスクを評価するためにHERposIBC患者を監視する方法であって、請求項1の方法に従って前記治療後の患者における抗HER2 CD4+Thl応答度を測定して、前記応答が前記癌の再発と相関する有意に抑制された抗HER2 CD4+Thl応答か又は非再発と相関するより高い抗HER2 CD4+Thl応答かを決定するステップを含む、方法。
- 前記標的治療が、前記患者への薬剤トラスツズマブの投与を含む、請求項22に記載の方法。
