Abstract
Description
本出願と併せて提出された出願データシートに記載の外国または国内の優先権が主張されるいかなる、またはあらゆる出願も、米国特許法施行規則第1.57条に従って本明細書に援用する。
膝関節全置換術(total knee arthroplasty、TKA)における大腿骨インプラントの
回転は、膝システム器具セットの標準構成要素である、4−in−1大腿骨切断ブロックを配置することによって設定される。この切断ブロックは、前方切除、後方切除、前方面取り切除、および後方面取り切除を案内するために使用される。切断ブロックは通常、2つの固定スパイク、または骨ピン用の2つの孔を含み、これらを用いて、遠位切除の完了後にその切断ブロックを大腿骨に固定する。こうした機構のために2つの孔を穿孔またはマーキングすることによって、切断ブロックが配向または配置されることになる。これらの孔の位置は、典型的には、外科医が、大腿骨上の解剖学的構造の標識と視覚的に位置合せしたドリルガイド装置によって画定されるが、これでは大腿骨の脛骨との力学的な位置合せを補償することはできない。脛骨を基準にしたドリルガイドがあれば、インプラント機能が向上しよう。
パドル12から孔14までの間隔は、インプラントシステムの4−in−1切断ブロックの寸法によって決まる。すなわち、この間隔は、後方切断スロットから切断ブロックスパイクまでの距離に、大腿骨インプラントの後方厚さを足したものに等しくなる。外科医は、AP位置決めガイド10を介して大腿骨20に穿孔する。
膝関節単顆置換術(unicompartmental knee arthroplasty、UKA)では、脛骨インプ
ラントを(通常は)脛骨の内側区画のみと置き換える。したがって、脛骨切除が2面で実施され、一方は横断面、もう一方は矢状面である。この矢状切除によって、インプラントの内側−外側位置が画定され、かつインプラントの脛骨に対する回転が設定される。この矢状切除の回転は、典型的には外科医の嗜好および経験に従って視覚的に位置合せされる。こうした視覚的な位置合せでは、大腿骨の脛骨との力学的な位置合せが補償されない。
大腿骨を基準にした切断ガイドがあれば、インプラントの機能が向上しよう。
手術用配向装置200には、特許文献1に記載の手術用配向装置(例えば14)と概ね同じ構成要素および基本測定機能が組み込まれている。特許文献1を参照によりその全体を本明細書に援用する。
TKAにおける脛骨インプラントの回転は、脛骨切除の完了後に設定される。脛骨インプラントは、切除された脛骨表面においていかなる方向にも回転させることができる。インプラントの最終的な回転は、典型的には外科医により以下の3つの方法のうちの1つまたは複数によって決定される。1)インプラントを骨の外側縁部にできる限り近付けて配置しようと切除表面を視覚的に最大に被覆する。2)インプラントの前方−後方(AP)軸を脛骨結節などの解剖学的指標と視覚的に位置合せする。3)インプラントを自由に回転させ、次いで脛骨インプラントを、膝が完全に伸展した状態で大腿骨と接触することによって指示される回転位置合せで固定する(以下では「従来法」と称する)。より精確かつ/または定量化可能な位置合せ法があると、インプラント性能、および患者の満足度が向上しよう。本発明は、ある実施形態において、インプラント性能、および患者の満足度を向上させる、このようなより精確かつ/または定量化可能な位置合せ法を提供する。
この動きの間、それぞれ互いに静止している2つの基準装置100は、2つの基準装置100間の相対的な位置ずれを計算するために「転送アライメント(transfer alignment)
」を実施し、それによって大腿骨装置の座標系において脛骨装置の配向を確立することが可能となる。

Claims (17)
- 患者の下肢の脛骨を単顆手技で切断するシステムであって、
大腿骨の遠位面に埋め込まれるように構成された第1の端部と、前記第1の端部がそのように配置されると、前記大腿骨から突き出るように構成された第2の端部とを有するガイドピンと、
前記ガイドピンの前記第2の端部と結合するように構成された第1の部分と、鋸合せ機構を備える第2の部分と、を有する矢状鋸ガイドと、
を備え、
前記矢状鋸ガイドの前記第1の部分が前記ガイドピンの前記第2の部分と結合されると、前記矢状鋸ガイドの前記第2の部分が前記ガイドピンから遠位に離れて突出して、前記鋸合せ機構が前記脛骨上に概ね矢状面に配置されることを特徴とするシステム。 - 慣性センサを備える配向装置をさらに備え、前記配向装置が、前記ガイドピンを前記大腿骨の軸を基準にして配置することができるように、前記ガイドピンを配置する間、前記ガイドピンと結合するように構成される、請求項1に記載のシステム。
- 前記矢状鋸ガイドの前記第2の部分が、前記鋸合せ機構の内側−外側位置を調節するように前記第1の部分に対して動くように構成される、請求項1に記載のシステム。
- 患者の下肢の脛骨を単顆手技で切断する方法であって、
大腿骨の力学的軸を、前記下肢と結合された少なくとも1つの慣性センサからの出力に基づいて位置特定するステップと、
ピンを、少なくとも1つの慣性センサからの出力に基づいて、前記大腿骨の前記力学的軸に対応する配向で前記大腿骨に配置するステップと、
矢状鋸ガイドを、鋸合せ機構が前記脛骨上に概ね矢状面に配設されるように、前記ピンと結合させるステップと、
前記脛骨を、前記鋸合せ機構に沿って切除するステップと、
を有し、
それによって矢状切除が、前記大腿骨の前記力学的軸の前記配向に基づいて行われることを特徴とする方法。 - 前記矢状鋸ガイドを結合させるステップが、前記矢状鋸ガイドの第1の部分の位置および配向を固定するステップを含み、前記矢状鋸ガイドの第2の部分を前記第1の部分に対して、前記矢状面を概ね横断する方向に調節するステップをさらに含み、前記矢状鋸ガイドの前記第2の部分が、前記鋸合せ機構を含む、請求項4に記載の方法。
- 大腿骨切断ブロックを受ける準備を大腿骨に対して行うシステムであって、
後方顆表面と接触するように構成された第1の部分と、前記第1の部分から離れて延び、切除された遠位大腿骨表面に隣接して配設されるように構成された第2の部分と、を有し、前記第2の部分が、大腿骨切断ブロックを受けるための取付け位置となるように、前記第1の部分からある間隔を置いて配置されたドリルガイド機構を有する、第1のガイドと、
スパイク部材を有する第1の部分と、前記第1の部分から離れて延びる第2の部分と、を有し、前記第2の部分がドリルガイド機構を備え、脛骨の平坦部と位置合せされるように構成された直線構造を有する、第2のガイドと、
を備え、
それによって前記システムが、大腿骨切断ブロックを大腿骨に取り付けるための複数の孔を形成することが可能となることを特徴とするシステム。 - 前記大腿骨および前記脛骨の一方または両方の回転を調節するように、前記大腿骨と前記脛骨との間に配置されるように構成された張力器をさらに備える、請求項6に記載のシステム。
- 大腿骨切断ブロックを受ける準備を大腿骨に対して行う方法であって、
大腿骨の遠位部分および脛骨の近位部分に切除平面を形成するステップと、
第1のガイドの第1の部分を、前記大腿骨の後方顆と接触させるステップと、
前記第1のガイドの第2の部分を、前記大腿骨の前記切除平面上に配置するステップと、
前記大腿骨の前記切除平面から上方に、前記第1のガイドの前記第2の部分を貫通して延びる第1の孔を形成するステップと、
第2のガイドの第1の部分を、前記第1の孔と結合させるステップと、
前記第2のガイドの第2の部分を、前記第2のガイドの機構が前記脛骨の前記切除平面と位置合せされるように配置するステップと、
前記大腿骨の前記切除平面から上方に、前記第2のガイドの前記第2の部分を貫通して延びる第2の孔を形成するステップと、
を含むことを特徴とする方法。 - 前記大腿骨と前記脛骨との間に張力器を配置するステップと、前記第2のガイドの前記機構が前記脛骨の前記切除平面と位置合せされるように、前記張力器を作動させて前記大腿骨を前記脛骨と回転により位置合せしてから、前記第2のガイドの前記第2の部分を配置するステップと、をさらに含む、請求項8に記載の方法。
- 慣性センサを用いて前記大腿骨、脛骨、または下肢の力学的軸を位置特定してから、前記力学的軸を基準にして前記大腿骨および脛骨の前記切除平面の少なくとも一方を形成するステップをさらに含む、請求項8に記載の方法。
- 脛骨インプラントの回転を設定するシステムであって、
大腿骨および脛骨の一方または両方と結合されるように構成された少なくとも1つの配向装置と、
前記脛骨と前記大腿骨との間に配置されるように構成された複数の試験用脛骨構成要素と、
(i)前記配向装置の1つまたは複数の慣性センサから測定値を収集する機能、
(ii)前記慣性センサからの前記測定値を、脛骨−大腿骨運動力学情報に変換するように計算を実施する機能、
(iii)前記脛骨−大腿骨運動力学情報を、脛骨−大腿骨運動力学目標値と比較する機能、および
(iv)前記脛骨−大腿骨運動力学情報および目標値の一方または両方に対応する出力をユーザに送信する機能、
の1つまたは複数を実施するように構成されたプロセッサと、
を備えることを特徴とするシステム。 - 脛骨インプラントの回転を設定する方法であって、
(a)少なくとも1つの慣性センサを、患者の下肢の脛骨および大腿骨の少なくとも一方と結合させるステップと、
(b)前記患者の前記脛骨の切除表面にインプラントを配置するステップと、
(c)前記脛骨が、前記大腿骨に対して屈曲、軸方向回転、および/または内反−外反が異なる複数の位置に配置されるように前記下肢を動かすステップと、
(d)屈曲、軸方向回転、および/または内反−外反の1つまたは複数について、脛骨−大腿骨運動力学を示すセンサの出力に基づいた値を、脛骨−大腿骨運動力学目標値と比較するステップと、
を含むことを特徴とする方法。 - 前記比較するステップが、前記センサの出力に基づいた値を、健康な膝で得られるモデル値と比較するステップをさらに含む、請求項12に記載の方法。
- 前記比較するステップが、前記センサの出力に基づいた値を、1つまたは複数の患者固有値と比較するステップをさらに含む、請求項12に記載の方法。
- 膝の脛骨−大腿骨運動力学の患者固有値を記録してから、前記脛骨を切除するステップをさらに含む、請求項14に記載の方法。
- 対側膝の脛骨−大腿骨運動力学の患者固有値を記録してから、前記脛骨を切除するステップをさらに含む、請求項14に記載の方法。
- 膝の前記回転を調節するステップをさらに含み、ステップ(c)〜(d)を繰り返す、請求項12に記載の方法。
