Claims (14)
- 以下のステップを含む、プライマリーケアセッティング内において神経学的疾患をスクリーニングするためのデータを収集する方法。
プライマリーケアセッティングにおいて対象から得られた血液被検試料における、
IL7、TNFα、IL5、IL6、CRP、IL10、TNC、ICAM1、FVII、I309、TNFR1、A2M、TARC、エオタキシン3、VCAM1、TPO、FABP、IL18、B2M、SAA、PPY、DJ1及びα−シヌクレインから選択される2、3、4、5、6、7、8、9、10、11、12、又は13のバイオマーカーを測定するステップ;
前記バイオマーカーの1つ又は組み合わせのレベルを、正常血液試料における相当するバイオマーカーの1つ又は組み合わせのレベルと比較するステップ;
前記対象が神経学的疾患を有する可能性が高いことを示す、前記正常血液試料のレベルに関連した前記血液被検試料における前記2又は3つ以上のバイオマーカーのレベルの増加を測定するステップ;
測定された前記2つのバイオマーカーに基づき前記神経学的疾患を同定するためのデータを収集するステップ;及び
予測された前記神経学的疾患に基づき前記対象のための治療コースを選択するためのデータを収集するステップ; - 個体又はプライマリーケア従事者に、計算されたリスクの変化を通知するためのデータを収集するステップをさらに含む、請求項1に記載の方法。
- 試料が血清又は血漿試料であり、プライマリーケアクリニック内における神経学的疾患の仮判定後に、このプライマリーケア提供者に、この特別な血液スクリーニングの知見に適切な特定の種類の専門医の紹介に関する情報を提供し、個体を前記神経学的疾患及びそれに従った治療の専門医に導く、請求項1又は2に記載の方法。
- 神経学的疾患が、アルツハイマー病、パーキンソン病、ダウン症候群、前頭側頭型認知症、レビー小体型認知症及び神経変性疾患から選択される、請求項1〜3のいずれかに記載の方法。
- 患者年齢及び認知神経科学的スクリーニング検査のパラメータを含むことにより、解析を緻密化するステップであって、本発明の血清に基づくマーカー、年齢及び認知神経科学的スクリーニング検査のうちの2又は3以上の組み合わせが、神経学的疾患又は障害を持たない対照対象と比較して、アルツハイマー病の判定のためのプライマリーケアセッティングにおいて少なくとも90%正確であるステップ;及び
神経変性疾患の正確な検出及び識別のために、睡眠障害(はい/いいえ)、幻視(はい/いいえ)、精神医学的/人格変化(はい/いいえ)、年齢、認知神経科学的スクリーニング及び2又は3以上の本発明らの血清に基づくマーカーのパラメータのうちの1又は2以上を判定するためのデータを収集するステップ
のうち少なくとも1つをさらに含む、請求項1〜4のいずれかに記載の方法。 - 様々なタンパク質の発現レベルが、蛍光検出、化学発光検出、電気化学発光検出及びパターン形成アレイ、逆転写ポリメラーゼ連鎖反応、抗体結合、蛍光励起分取、検出可能ビーズ分取、抗体アレイ、マイクロアレイ、酵素アレイ、受容体結合アレイ、アレル特異的プライマー伸長、標的特異的プライマー伸長、固相結合アレイ、液相結合アレイ、蛍光共鳴移動又は放射性標識化のうちの少なくとも1種によって測定される、請求項1〜5のいずれかに記載の方法。
- 全体的精度94、95、96、97、98、99若しくは100%(感度(SN)、特異性(SP)(SN=0.94、SP=0.83))による軽度AD(CDRグローバルスコア1.0以下)、又は全体的精度91、92、93、94、95、96、97、98、99若しくは100%(SN=0.97、SP=0.72)による非常に初期のAD(CDRグローバルスコア=0.5)のうちの少なくとも1つのスクリーニングに用いられる、請求項1〜6のいずれかに記載の方法。
- 感度よりも高い特異性を用いるプライマリーセッティングにおいてスクリーニングに用いられ、前記特異性が、0.97〜1.0の範囲内であり、前記感度が、0.80〜1.0の範囲内である、請求項1〜7のいずれかに記載の方法。
- 以下のステップを含むものとしてさらに定義される、請求項1〜8のいずれかに記載の方法。
神経学的疾患を有すると疑われる個体から単離された試料から、バイオマーカーIL−7及びTNFαを含むバイオマーカーの測定値を得るステップ;
対象の年齢及び前記対象から得られた1又は2以上の認知神経科学的スクリーニング検査(時計描画、言語流暢性、睡眠障害、幻視、行動障害、運動障害)の結果を加えるステップ;
モデルの出力から前記神経学的疾患を発症する前記個体のリスクを計算するステップであって、前記モデルへの入力が、前記2つのバイオマーカーの前記測定値、前記対象の年齢及び1又は2以上の認知検査の前記結果を含み、さらに、前記モデルが、選択された個体集団の縦断的研究のデータを適合させることにより開発され、前記適合されたデータが、前記選択された個体集団における前記バイオマーカーのレベル、前記対象の年齢及び1又は2以上の認知検査の前記結果並びに神経学的疾患を含むステップ;及び
前記個体の前記計算されたリスクを、前記個体由来の少なくとも1つのより初期の試料から得られる以前に計算されたリスクと比較するステップ; - 以下のステップを含む、神経学的疾患の治療において有用であると考えられる候補薬物を評価するために用いられる、請求項1〜9のいずれかに記載の方法。
(a)神経学的疾患を有すると疑われる1又は2名以上の患者から得られる1又は2以上の血液試料からの、IL7、TNFα、IL5、IL6、CRP、IL10、TNC、ICAM1、FVII、I309、TNFR1、A2M、TARC、エオタキシン3、VCAM1、TPO、FABP、IL18、B2M、SAA、PPY、DJ1及びα−シヌクレインから選択される2又は3つ以上のバイオマーカー、前記患者の年齢並びに前記患者の1又は2以上の認知神経科学的スクリーニング検査の結果を測定するステップであって、前記患者が、神経学的疾患、アルツハイマー病、パーキンソン病又は認知症であると疑われるステップ;
(b)候補薬物が投与された前記患者の第1のサブセットから得られる1又は2以上の血液試料からの前記バイオマーカー、前記患者の年齢、及び前記患者の1又は2以上の認知神経科学的スクリーニング検査の結果と、プラセボが投与された前記患者の第2のサブセットから得られる1又は2以上の血液試料からの前記バイオマーカー、前記患者の年齢、及び前記患者の1又は2以上の認知神経科学的スクリーニング検査の結果とを測定するステップ;
(c)前記候補薬物が、前記第2のサブセットの患者において生じるいかなる低下と比較して統計的に有意に、前記1又は2以上のバイオマーカーの発現を低下させるか否かを判定するステップであって、統計的に有意な低下が、前記候補薬物が前記神経学的疾患の治療において有用であることを示し、参照レベルと比較してより低いCRP及びIL10のレベル、又は参照レベルと比較して上昇した血液中のIL−7及びTNFアルファのレベルが、前記個体が前記神経学的疾患、アルツハイマー病、パーキンソン病、又は認知症を患っている可能性がより高いことを示す、ステップ;及び
(d)治療後に前記1又は2以上の認知ステップを反復してもよいステップ; - 患者プロファイルに最もマッチする1又は2以上の神経学的疾患プロファイルを判定するステップをさらに含む、請求項1〜10のいずれかに記載の方法であって、前記判定が以下のステップを含む、前記方法。
適切にプログラムされたコンピュータにおいて、1又は2以上の神経学的疾患を有すると疑われる患者由来の血液又は血漿試料におけるIL7、TNFα、IL5、IL6、CRP、IL10、TNC、ICAM1、FVII、I309、TNFR1、A2M、TARC、エオタキシン3、VCAM1、TPO、FABP、IL18、B2M、SAA、PPY、DJ1バイオマーカーの発現レベルを、参照データベースにおける参照プロファイルと比較して、前記患者プロファイルと前記参照プロファイルのそれぞれとの間の類似性の尺度を判定するステップ;
適切にプログラムされたコンピュータにおいて、ステップ(a)において判定された類似性の前記尺度間の最大類似性に基づき、前記患者プロファイルに最もマッチする参照データベースにおける参照プロファイルを同定するステップ;
ユーザー・インターフェース・デバイス、コンピュータ可読記憶媒体又はローカル若しくは遠隔コンピュータシステムに出力するステップ、或いは前記最大類似性又は前記患者プロファイルに最もマッチする前記参照データベースにおける前記参照プロファイルの前記疾患細胞試料の前記疾患を表示するステップ;及び
IL7、TNFα、IL5、IL6、CRP、IL10、TNC、ICAM1、FVII、I309、TNFR1、A2M、TARC、エオタキシン3、VCAM1、TPO、FABP、IL18、B2M、SAA、PPY、DJ1及びα−シヌクレインから選択される、血液試料由来の2、3、4、5、6、7、8、9、10、11、12又は13のバイオマーカーの発現レベルを判定するステップであって、前記方法が、感度よりも高い特異性を用いる一次設定におけるスクリーニングに用いられ、前記特異性が0.97〜1.0の範囲内であり、前記感度が0.80〜1.0の範囲内である、ステップ; - ヒト個体から得られる血液試料におけるIL−7及びTNFαの発現レベルを検出するための1又は2以上の試薬と、1又は2以上の神経学的スクリーニング検査シートと、IL7、TNFα、IL5、IL6、CRP、IL10、TNC、ICAM1、FVII、I309、TNFR1、A2M、TARC、エオタキシン3、VCAM1、TPO、FABP、IL18、B2M、SAA、PPY、DJ1及びα−シヌクレインから選択される発現マーカーのレベルを検出するための、存在していてもよい1又は2以上の試薬と、
前記個体の血液試料由来の前記IL−7及びTNFαバイオマーカーの正規化された測定レベルを参照レベルと比較するための、或いは、前記患者の年齢及び前記患者の前記神経学的スクリーニング検査の結果を参照と比較するための説明書であって、各神経学的疾患診断バイオマーカーの前記参照レベルが、神経学的疾患ではないヒト個体の1又は2以上の血液試料由来の前記神経学的疾患診断バイオマーカーの正規化された測定レベルを含み、各神経学的疾患診断バイオマーカーの前記参照レベルに満たない神経学的疾患診断バイオマーカーのレベルが、前記個体が神経学的疾患を患っている可能性がより高いことを示し、前記検査が少なくとも90%正確であり、前記参照レベルと比較して上昇した血液中のIL−7及びTNFアルファの発現レベル、又は前記参照レベルと比較してより低いCRP及びIL10のレベルのうち少なくとも1つが、前記個体が前記神経学的疾患を患っている可能性がより高いことを示す、前記説明書と
を含む、プライマリーケアセッティングにおける神経学的疾患の診断を補助するための迅速スクリーニングキット。 - IL7、TNFα、IL5、IL6、CRP、IL10、TNC、ICAM1、FVII、I309、TNFR1、A2M、TARC、エオタキシン3、VCAM1、TPO、FABP、IL18、B2M、SAA、PPY、DJ1及びα−シヌクレインから選択される発現マーカーのレベルを検出するための1又は2以上の試薬をさらに含む、請求項12に記載のキット。
- 試料が血清試料であり、神経学的疾患の仮判定後に、個体を前記神経学的疾患の専門医に導く、請求項12に記載のキット。
