Description
本願は、米国仮出願第62/036,377号(出願日2014年8月12日)の利益を主張するものであり、該仮出願は、参照により本明細書中に援用される。
尿管ステントは、腎臓、尿管、および膀胱を含む泌尿器系と関連付けられる、1つまたはそれを上回る合併症を被る患者集団内で使用される、医療デバイスである。合併症の宿主は、尿流、およびこれらの器官が本機能をどのようにして取り扱うかに影響を及ぼし得る。これらの合併症は、尿流量の減少から腎臓または膀胱の腫脹にまで及び、これらの症状の多くは、腎臓結石の形成による悪影響を受ける。泌尿器系合併症を緩和するために、1つまたは複数のデバイスが、膀胱、一方もしくは両方の腎臓、および/または一方もしくは両方の患者の尿管内のいずれかに留置される。これらの領域で使用されるデバイスは、腎瘻カテーテル(腎臓集合系内に経皮的に送達される)、腎尿管カテーテル(経皮的に送達され、膀胱の中へ遠位に延在する)、尿路カテーテル(尿道を通して送達される)、または尿管ステント(経皮的に、または尿道を通して送達される)として公知である。

Claims (22)
- 内側管腔と、近位端および遠位端とを有する、カテーテル本体と、
内側管腔と、前記カテーテル本体の前記遠位端と解放可能に連結される近位端とを有する、ステント部材と、
前記カテーテル本体の前記内側管腔の中に配置され、前記カテーテル本体および前記ステント部材を同心円状に整合させるように前記ステント部材の前記内側管腔の中へ延在する、内側部材アセンブリと、
前記内側部材アセンブリの少なくとも一部を覆ってループを形成するように、前記カテーテル本体を通って、またはそれに沿って、前記ステント部材の前記内側管腔の中へ延在し、それによって、前記ステント部材を前記カテーテル本体に固着する、テザーと、
を備え、
前記ステント部材の前記内側管腔からの前記内側部材の後退は、前記ステント部材がステント本体から解放されるように、前記ループから前記内側部材アセンブリを解放する、
ステント送達システム。 - 前記内側部材アセンブリは、係止引張ワイヤであって、前記テザーの前記ループを通して螺入可能である係止引張ワイヤを備える、請求項1に記載のステント送達システム。
- 前記内側部材アセンブリは、ハイポチューブを備える、請求項1に記載のステント送達システム。
- 前記内側部材アセンブリは、前記カテーテル本体の前記近位端に連結されたハブにおける1つまたはそれを上回るプルタブもしくは回転可能キャップを伴って作動させられるように構成される、請求項1に記載のステント送達システム。
- 前記テザーは、前記カテーテル本体の前記内側管腔を通って延在する、請求項1に記載のステント送達システム。
- 前記テザーは、前記カテーテル本体の前記遠位端付近の前記カテーテル本体の外側ポートから外へ延在する、請求項5に記載のステント送達システム。
- 前記テザーによって形成される前記ループは、前記ステント部材の前記内側管腔内で前記内側部材アセンブリの前記少なくとも一部によって螺入されるように、前記ステント部材の外側ポートを通してステント部材の中へ延在する、請求項5に記載のステント送達システム。
- 前記テザーは、前記カテーテル本体の前記遠位端付近の固定端と、自由端とを有し、前記テザーは、前記カテーテル本体の前記自由端および前記近位端に向かって近位に延在する、請求項1に記載のステント送達システム。
- 前記テザーは、第1の端部と、第2の端部とを有し、前記テザーは、前記カテーテル本体の前記第1および第2の端部の両方ならびに前記近位端に向かって近位に延在する、請求項1に記載のステント送達システム。
- 前記ステント部材は、近位ループと、遠位ループとを備える、請求項1に記載のステント送達システム。
- 前記ステント部材の前記近位ループもしくは前記遠位ループのうちの1つまたはそれを上回るものは、直線状構成と、ループ状構成とを有する、請求項10に記載のステント送達システム。
- 前記近位ループもしくは前記遠位ループのうちの前記1つまたはそれを上回るものは、前記ループ状構成を成すように付勢される、請求項11に記載のステント送達システム。
- 前記カテーテル本体の前記内側管腔を通って延在し、前記近位ループに連結される、ループ引張ワイヤをさらに備え、前記ループ引張ワイヤを後退させることは、前記ループ構成の中に前記近位ループを引き入れ、または前記近位ループの半径を低下させる、請求項11に記載のステント送達システム。
- 前記ループ引張ワイヤは、前記ステント部材の前記近位端付近の前記ステント部材の第1の外側ポートから外へ延在し、前記近位ループの遠位端付近の前記ステント部材の第2の外側ポートの中へ戻って延在する、請求項13に記載のステント送達システム。
- 前記ループ引張ワイヤは、前記カテーテル本体の前記近位端に連結されたハブにおけるプルタブまたは回転可能キャップから後退可能である、請求項13に記載のステント送達システム。
- 腎尿管ステントを送達する方法であって、
ステント送達システムのステント部材の遠位端が膀胱の中に位置付けられ、前記ステント部材の近位端が腎盂の中に位置付けられるように、経皮アクセス部位を通して前記ステント送達システムを前進させるステップであって、前記ステント部材の前記遠位端が前記膀胱の中で遠位ループを形成する、ステップと、
前記腎盂の中で近位ループを形成するように、ステント部材の前記近位端を作動させるステップと、
前記ステント送達システムのカテーテル本体から前記ステント部材を分断するステップと、
定位置に前記ステント部材を残して、前記経皮アクセス部位から前記ステント送達システムの前記カテーテル本体を後退させるステップと、
を含む、方法。 - 前記腎盂の中で近位ループを形成するように、前記ステント部材の前記近位端を作動させるステップは、前記ステント部材の前記近位端の半径を縮小するように、前記カテーテル本体を通って延在するループ引張ワイヤを後退させるステップを含む、請求項16に記載の方法。
- 前記カテーテル本体から前記ステント部材を分断するステップは、前記カテーテル本体から前記ステント部材の中へ延在するテザーループを解放するように、前記ステント部材から係止引張ワイヤを後退させるステップを含む、請求項16に記載の方法。
- 前記カテーテル本体から前記ステント部材を分断するステップは、前記ステント部材から内側部材を後退させるステップを含み、前記内側部材は、それを通して前進させられるときに、前記カテーテル本体を前記ステント部材と同心円状に整合させるように構成される、請求項16に記載の方法。
- 前記カテーテル本体から前記ステント部材を分断し、前記経皮アクセス部位から前記カテーテル本体を後退させる前に少なくとも3日間、定位置に前記ステント送達システムの前記ステント部材および前記カテーテル本体を残すステップをさらに含む、請求項16に記載の方法。
- 前記少なくとも3日間定位置に残された前記ステントの前記カテーテル本体を通して尿を排出するステップをさらに含む、請求項20に記載の方法。
