Abstract
Description
急性骨髄性白血病(AML)は、予後不良の異質性障害であり、そして、成人において診断される急性白血病のうちで最も頻度の高い形態である。AMLは、造血前駆細胞における体細胞的に獲得された遺伝子変化の蓄積を特徴とするクローナルな悪性腫瘍である(Patel et al., 2012)。正常核型AML(患者のほぼ50%を占める)における再発突然変異は、約30個の遺伝子を数セット含み、最も再発の多いのは、FLT3、NPM1、KIT、CEBPA、及びMLLに作用するものである(Patel et al, 2012;Welch et al, 2012)。これら突然変異は、自己再生能、細胞の生存/増殖、及び骨髄分化等の様々な細胞機構に介入し、最終的には正常造血の障害と共に未分化芽球の蓄積につながる(Ferrara and Schiffer, 2013;Lowenberg et al., 1999;Mardis et al., 2009)。
第1の実施態様では、本発明は、薬剤耐性癌の治療において同時に又は逐次使用するための、a)DNAメチル化阻害剤とb)ビタミンD受容体アゴニストとの組み合わせに関する。
本発明は、DNAメチル化阻害剤が、特に、VDRプロモータの脱メチル化を介するエピジェネティック修飾を通して、ビタミンD受容体アゴニストと相乗的に作用し、VDR発現の妨害を解除するという本発明者らによる予想外の知見から得られた。この関連性は、インビトロのAML芽球において、そして、インビボのマウスにおけるAML異種移植腫瘍において有効であることが証明されている。本発明者らは、また、併用療法が、化学療法に対するAML患者の不応性の状態を改善し、そして、患者における静止期の白血病幹細胞(LIC)の細胞分化を誘導することも示した。
本発明は、癌に罹患している患者における薬剤耐性癌の治療において同時に又は逐次使用するための
i. DNAメチル化阻害剤と、
ii. ビタミンD受容体アゴニストと
の組み合わせを提供する。

ii. ビタミンD受容体アゴニストと
の組み合わせを提供する。
i. DNAメチル化阻害剤と、
ii. ビタミンD受容体アゴニストと
の組み合わせであって、静止期の白血病幹細胞(LIC)を根絶することができる組み合わせを提供する。
iii. DNAメチル化阻害剤と、
iv. ビタミンD受容体アゴニストと
の組み合わせであって、静止期の白血病幹細胞(LIC)を根絶することによって再燃を予防することができる組み合わせを提供する。
本発明者らは、VDRシグナル伝達のターゲティングが、AMLモデルにおいてLICを根絶することによって化学療法に対する感受性を回復させることを見出した。LICは、化学療法に対する耐性を有するバルク白血病集団における白血病細胞のサブセットであり、したがって、再燃の原因となる。

実施例1:実験手順
臨床サンプル及び細胞株
文書によるインフォームドコンセント及びNecker倫理委員会による承認を得た後にAML患者から末梢血液細胞を得た。任意の処理を投与する前の初期診断時に末梢血を回収した。単核細胞をフィコール・ハイパック(PAA laboratories)密度遠心分離によって精製し、そして、15% FCS(Biowest)、100ng/mL 幹細胞因子、10ng/mL IL−3、及び25ng/mL FLT3−L(Peprotech)を添加したDMEM(Invitrogen)に再懸濁させた。前骨髄芽球(HL−60)、骨髄球(OCI−AML3)、単芽球(THP1及びU937)。細胞を10% ウシ胎仔血清(FCS)及び抗生物質を添加したRPMI−1640培地(Invitrogen)で培養した。
標準的なプロトコールに従ってフローサイトメトリー分析を実施した。簡潔に述べると、単一細胞懸濁液をフローサイトメトリーバッファ(Mg2+もCa2+も含まないPBS(PBS−/−)、0.5% FCS、0.1% アジ化ナトリウム(pH7.4)に再懸濁した。トリパンブルーで染色した後、細胞計数によって生存細胞数を決定した。フローサイトメトリー分析については、24G2及びフルオロフォア結合抗体(BD Biosciences, eBioscience, Biolegend)で免疫染色した細胞を用いてFcブロッキングを実施した。系列(Lin)陽性細胞の免疫染色については、CD5、B220、CD11b、7−4、Gr−1、及びTer−119に対するビオチン標識一次抗体を含有するカクテル(BD Biosciences)を用い、続いて、ストレプトアビジン結合フルオロフォア標識二次抗体で染色した。
全てMSCV−IRESベクターにクローニングされたヒトMLL−ENL又はMLL−AF9又はFLT3−ITDをコードしているプラスミド(Dr Patrice Dubreuil, Cancerology Research Centre of Marseille (CRCM), Franceから供与頂いた)。レトロウイルスの生成は、Lipofectamine LTX plus(Invitrogen)を使用することによってレトロウイルスパッケージング細胞(Plat-E, Cell Biolabs)を一過的にトランスフェクションすることによって実施した。培養上清に放出されたレトロウイルスを、製造業者の手順に従ってレトロネクチン(Takara)でコーティングされたプラークを使用してマウス造血細胞の感染のために用いた。
記載の通り(Somervaille et al., 2009)、マウスMLL−ENL AMLを惹起した。簡潔に述べると、50mg/kg 5−FUを注射した5日間後の4〜8週齢のマウスの長骨からBM細胞を押し出した。細胞を、15% FCS、10ng/mL IL3、50ng/mL SCF、及び10ng/mL IL6(Peprotech)を添加したDMEM中37℃で48時間インキュベートして、細胞周期への移行を促進した。次いで、細胞を、37℃で48時間、製造業者の指示書に従って、レトロネクチン(Takara)上にレトロウイルス上清を遠心分離接種した。
Ki−67/PI染色については、細胞を、まず、製造業者の指示書(BD Biosciences)に従ってFix and Perm試薬で処理し、Ki−67抗体(16A8、Biolegend)と共にインキュベートし、次いで、5μg/mL RNaseA及びPIを含むPBSで洗浄し、そして、再懸濁した。染色された細胞を、FACS Fortessa(BD Biosciences)を用いて分析した。
製造業者の指示書(BD Biosciences)に従って細胞をAnnexin V及びヨウ化プロピジウムで染色し、次いで、フローサイトメトリーによって分析して、アポトーシスのレベルを評価した。
全RNAを抽出し、RNeasyキット(Qiagen)を用いて精製し、そして、RT(iScript Reverse Transcriptase supermix, Bio-Rad)に供した。CFX96 PCRシステム(Bio-Rad)を用いて定量リアルタイムPCRを実施し、そして、Ssofast Eva Green(Bio-Rad)を用いてPCR産物を定量した。デルタ−デルタCq法(Livak 2001)を用いてMIQE指針(Bustin SA Clin Chem 2009)に従ってリアルタイム定量PCRの結果を解析した。
対象となる細胞株からメチル化レベルを分析するために、製造業者の指示書に従ってQIAamp DNA Micro Kit(Qiagen)を用いて、各細胞株から全ゲノムDNA(gDNA)を単離した。精製後、各細胞株由来のgDNA 2μgを、製造業者の指示書に従ってEpiTect Bisulfite Kit(Qiagen)を介して重亜硫酸ナトリウム処理に供した。対象となる各領域を、PCR増幅した。プライマー対は、ウェブベースのソフトウェアMethPrimerを用いて設計し、そして、その最適なアニーリング温度は、グラジエントPCRを介して求めた。
腫瘍を確立するために、指定通りU937又はOCI−AML3細胞をMatrigel(BD Biosciences)と混合し(1:1、vol/vol)、そして、10週齢の雌の胸腺欠損ヌードマウス(Janvier laboratories, France)に皮下注射(5×106/側腹部)した。次いで、マウスに、5−アザシチジン(Celgene)(5、7、9、11、及び13日目に5mg/kg)、イネカルシトール(Hybrigenics)(20μg/日 1週間に3回)、及び5−アザシチジンとイネカルシトールとの組み合わせ、又はビヒクルコントロールとしてのPBSをi.p.注射した。既に記載されている通り(Lepelletier 2007)、腫瘍成長を測定した。
Methocult培地(MethoCult GF M3434, StemCell Technologies)を用いて形質転換された又はされていないマウス細胞によるコロニー形成について調べた。感染させたWT細胞及びVDR−/−細胞を、癌遺伝子を有するか又は有しないBMTから回収し、そして、Methocult培地に移した。コロニー数を求めるために、プレーティングの1週間後、コロニーをスコア化した。連続プレーティングについては、細胞をメチルセルロースから回収し、PBSで1回洗浄し、計数し、そして、Methocultに再プレーティングした。幾つかの実験では、10nM イネカルシトール(hybrigenics)を培養培地に添加した。
この試験で用いたAML及びHSPCサンプルは、既に報告されており(Metzeler et al Blood., 2008)、そして、GEOデータベースにおいてアクセッション番号GSE12417で入手可能である。R 2.14.0及びBioConductorを用いて解析を実施した。RMAパッケージを用いて生データを作製した。異なるアレイセットを比較するために、生の発現データを、コントロールGAPDHプローブセットの平均に対して正規化した。
データは、平均±SEMとして表す。統計解析は、Prism 5ソフトウェア(GraphPad Software, Inc.)を用いて実施した。2つの群を比較するためにスチューデントt検定又はMann-Whitney検定を用い、そして、複数の群の比較は、一元配置分散分析、続いて、事後Bonferroni検定を用いて行った。指示されている場合、ノンパラメトリック比較のために、Kruskal-Wallis検定、続いて事後Dunn検定を用いた。無腫瘍動物曲線を比較するために、ログランク検定を用いた。結果は、p値<0.05(*)、<0.01(**)、又は<0.001(***)において統計的に有意であるとみなした。
実施例2:VDR発現は、AMLにおける単球の分化及び生存に関連する。
VDRシグナル伝達の誘導は、AMLにおける処置標的として示されている(Elstner et al., 1994; Kim et al., 2012;Munker et al., 1986)。しかし、AMLの病理におけるVDRの役割は知られていない。AMLにおけるVDR発現/活性の結果を評価するために、フランス−アメリカ−イギリス(FAB)システム(Bennett et al., 1976)に従って分類された正常核型患者(n=163)を含む、公開されている遺伝子発現データセット(GSE12417シリーズ、Dr. K.H. Metzeler, University of Munich, Munich, Germanyによって寄付された)(Metzeler et al., 2008)の後向き解析を実施した。この患者コホートでは、未成熟/未分化AMLサブタイプ(AML0、AML1、及びAML2)に比べて、単球分化の特徴を表すAMLサブタイプ(AML4及びAML5)においてVDR発現が増加することが分かった(図1A)。CD14又はCSF1R(Friedman, 2002)等の他の単球分化マーカーの発現は、AML0〜2サブタイプとAML4〜5サブタイプとの間で差がなく、これは、AMLサブタイプによる発現の差が単球分化のマーカーに制限されないことを示唆する。SERPINB8(VDR標的遺伝子)の発現の評価によって、AML5患者におけるVDR発現の増加がVDRシグナル伝達の誘導に関連していることが確認された。したがって、未分化/未成熟AMLサブタイプは、単球経路に関与しているAMLサブタイプに比べてVDR発現及びVDR活性の減少を示した。
次に、単球分化が障害される原因となるAML細胞におけるVDR発現の減少の原因となる分子機構を評価しようとした。AMLは、CpG島におけるメチル化プロファイルの変化を特徴とし(Akalin et al., 2012)、そして、VDRプロモータは、メチル化によって調節されることが示されている(Marik et al., 2010)ので、メチル化がAMLにおけるVDR抑制を誘導するかどうかを評価した。VDRプロモータの解析によって、CpG島がVDRプロモータ、特に、ATG開始コドンの−75〜−1035bpの領域において頻出することが明らかになった(図4A)。次いで、脱メチル化剤5−AZAで処理した又は処理していない一次AMLサンプル由来の重亜硫酸塩変換ゲノムDNAにおいてVDRプロモータメチル化解析を実施した。5−AZAで処理した細胞では、メチル化が減少しており、これは、VDRプロモータにおけるCpG島がAML細胞においてメチル化されることを示唆する(図4B)。THP−1細胞における更なるメチル化感受性高解像度融解(MS−HRM)分析によって、VDRプロモータが、高レベルのメチル化の読み取り値の大部分を含む単峰性ピークを有し、そして、5−AZAが、VDRプロモータのメチル化を元に戻すことができることが確認された(図4C)。AML一次サンプルにおける試験によって、これら知見が確認された。
VDRアゴニストは、AML細胞における単球分化を促進することが示されている。しかし、臨床試験では、VD及びVDRアゴニストは、生命を脅かす高カルシウム血症の発症が原因で臨床応答を誘導することができなかった(Kim et al., 2012)。イネカルシトール(INEC)は、患者における高カルシウム血症を避けるために開発された非常に強力なVDRアゴニストである(Kim et al., 2012;Petrini et al., 1991)。用量応答試験は、AML細胞の分化を促進するのにINECがVDよりも1000倍有効であることを示した(図5A)。AMLサンプルにおいてVDR発現が阻害されることが示されたので、5−AZA及びVDRアゴニストによるプロモータのメチル化を低減する併用療法が、AML細胞におけるVDRシグナル伝達を強化することができると仮定した。5−AZA及びINECは骨髄分化を有意ではあるがわずかな増加を促進した(5−AZAについては1.3倍、そして、INECについては4.5倍)が、併用療法は、AML細胞株におけるCD11b及びCD14の発現の大きな増加(8倍)を促進した(図5B)。この相乗効果は、併用療法によって処理された細胞において発現がアップレギュレートされる単球分化マーカー(TERML2、SERPINB8)の発現でも観察されたが、顆粒球マーカーの発現(NE、PRTN3)は、コントロールと比べてダウンレギュレートされ、これは、併用療法が、VDについて既に示されている通り(Callens et al., 2010a)、顆粒球分化を損害して単球分化を支持することを示唆する(図5C)。更なる形態学的分析によって、これら知見が確認された。これらの結果は、AML一次サンプルにおいても確認された。予想通り、VDRアゴニストと脱メチル化剤との間の相乗効果は5−AZAに制限されなかったが、その理由は、デシタビンがAML細胞分化の促進において同様の結果を示したためである。
以前の研究で、VDR−/−マウスが正常数の循環単球を示すことが示されており、これは、VDRが骨髄の分化にとって必須ではないことを示唆する(O'Kelly et al., 2002)。定常状態条件におけるVDR−/−及び野性型のマウス由来の全血液細胞の分析によって、これら結果が確認され、そして、拡大解釈されたが、その理由は、野性型マウスとVDR−/−マウスとの間に血小板、顆粒球、赤血球、及び単球の数の差が見られなかったためである(図3A)。更に、骨髄細胞数及び脾臓細胞数の両方において、VDR−/−動物と野性型動物との間に差は存在しなかった(図3B)。しかし、前駆細胞の分析によって、系列限定前駆細胞(LRP)、共通の骨髄前駆細胞(CMP)が、野性型マウスに比べてVDR−/−マウスで増加することが明らかになった。しかし、単球マクロファージ前駆細胞(MMP)集団は、野性型マウスと比べてVDR−/−マウスにおいて減少し(図3C)、これは、VDR−/−マウスにおいて単球分化が障害されたことを示唆する。これに一致して、CFCアッセイによって、VDR−/−BMがマクロファージ(M)/顆粒球(G)コロニーの比の低下を示すことが明らかになった(図3D)。したがって、VDR−/−マウスは、単球前駆細胞の数の減少を示し、これは、VDR発現によって単球分化に関与している細胞が生じることを示す以前の結果を更に確認するものである。
VDR−/−細胞において単球分化が障害された結果についての更なる見識を得るために、得られたCD11b−F4/80−形質転換細胞の表現型を精査することを決定した。したがって、FLT3−ITD、MLL−ENL、及びMLL−AF9融合タンパク質のコンストラクトで形質転換した野性型細胞及びVDR欠損細胞における造血前駆細胞の表現型マーカーの発現について試験した。マルチパラメトリックなフローサイトメトリー分析によって、正常造血細胞において既に観察されている通り、LSK細胞の頻度が、野性型形質転換細胞と比べてVDR−/−形質転換細胞において増加することが示された(図2A)。これに一致して、LSKの表現型に関連し(EVI−1、HOXA−9、及びMEIS−1)、そして、骨髄前駆細胞の拘束によって容易にダウンレギュレートされた(Chen et al., 2008)遺伝子の発現は、VDR−/−形質転換細胞において増加した(図2B)。したがって、癌遺伝子形質転換細胞におけるVDRの不活化によって、未成熟な表現型の、そして、LIC潜在力の可能性を有する細胞が生じた。
LICは、化学療法に対する耐性を有するバルク白血病集団における白血病細胞のサブセットであり、したがって、再燃の原因となる。次に、LIC細胞の標的化におけるVDRアゴニスト療法の有効性について調べた。この仮説について試験するために、野性型マウス及びVDR−/−マウスに由来する骨髄前駆細胞をFLT3−ITDコンストラクトで形質導入した。INEC又はコントロールとしてのビヒクルを含有する半固体培地に細胞をプレーティングした(図6A)。INECを含有するメチルセルロース中におけるFLT3−ITD形質転換細胞の成長によって、ビヒクル処理細胞と比べてクローン形成能が5倍低下し(図6A)、そして、再プレーティングアッセイによって、INEC処理は、プレーティングの第4ラウンドにおいて成長しているコロニーが実質的に存在しないLICの能力の進行性消失を誘導するが、ビヒクル処理細胞は、メチルセルロースへの連続プレーティング中にクローン形成能を維持することが明らかになった(図6A)。INECの効果が可逆的であることを更に調べるために、2回目の再プレーティングでINEC処理された培地をINEC又はビヒクルを含有する2つのグループに分けた。INECの洗い流しによってCFCが再確立され、これは、VDRアゴニストによる連続処理がLICを根絶するための選択圧を維持することを示唆する(図6A)。したがって、インビトロコロニー再プレーティングアッセイは、VDRシグナル伝達が、LICの恒常性を制御する主な経路であり、そして、VDRアゴニストがLICを根絶することができることを示唆する。
メチル化は、哺乳類ゲノムにおいて頻繁に生じるプロセスであり、そして、ゲノムワイドな脱メチル化は、肺発生に必須である(Feng et al., 2010)。更に、エピジェネティック修飾は、近年、体細胞発生中に観察され、そして、5’−CpG−3’ジヌクレオチドの脱メチル化は、赤血球生成の進行に必須であることが示されている(Shearstone et al., 2011)。しかし、AMLの特徴である骨髄分化の妨害において影響を及ぼすエピジェネティック修飾によって抑制される遺伝子の同定については、詳細に言及されていなかった。本発明者らによるデータは、プロモータの過剰メチル化によるVDR発現の不活化が、分化/FAB分類だけではなく、AMLの進行にも関連する新規機構であることを示唆しているが、その理由は、VDR/VDR標的遺伝子の発現が、患者の生存の増大に関連していたためである。また、本発明者らによる研究によって、VDRが低メチル化剤の標的として同定されたが、その理由は、VDR−/−形質転換細胞が5−AZA処理に対して耐性であったためである。したがって、エピジェネティック修飾によるVDR発現の制御は、低メチル化剤療法における結果と共に、AMLにおけるこの核受容体の新規役割を規定する。

Claims (15)
- 薬剤耐性癌の治療において同時に若しくは逐次使用するため、及び/又は造血器悪性腫瘍に罹患している患者における腫瘍再燃の予防において同時に若しくは逐次使用するための
i. DNAメチル化阻害剤と、
ii. ビタミンD受容体アゴニストと
の組み合わせ。 - 前記造血器悪性腫瘍が、急性骨髄性白血病からなる群から選択される、請求項1記載の通り使用するための組み合わせ。
- 前記急性骨髄性白血病が、FLT3、NPM1、KIT、CEBPA、及びMLLからなる群から選択される遺伝子に変異を有する正常核型のAMLである、請求項2記載の通り使用するための組み合わせ。
- 前記急性骨髄性白血病が、変異型FLT3−ITDを発現している、請求項3記載の通り使用するための組み合わせ。
- 前記DNAメチル化阻害剤が、アザシチジンである、請求項1〜4のいずれか一項記載の通り使用するための組み合わせ。
- 前記ビタミンD受容体アゴニストが、ビタミンD3又はイネカルシトールである、請求項1〜5のいずれか一項記載の通り使用するための組み合わせ。
- 造血器悪性腫瘍に罹患している患者の化学療法剤に対する感受性の増大、回復、又は増強において同時に又は逐次使用するための
i. DNAメチル化阻害剤と、
ii. ビタミンD受容体アゴニストと
の組み合わせ。 - 前記造血器悪性腫瘍が、急性骨髄性白血病からなる群から選択される、請求項7記載の通り使用するための組み合わせ。
- 前記急性骨髄性白血病が、FLT3、NPM1、KIT、CEBPA、及びMLLからなる群から選択される遺伝子に変異を有する正常核型のAMLである、請求項8記載の通り使用するための組み合わせ。
- 前記急性骨髄性白血病が、変異型FLT3−ITDを発現している、請求項9記載の通り使用するための組み合わせ。
- 前記DNAメチル化阻害剤が、アザシチジンである、請求項7〜10のいずれか一項記載の通り使用するための組み合わせ。
- 前記ビタミンD受容体アゴニストが、ビタミンD3又はイネカルシトールである、請求項7〜11のいずれか一項記載の通り使用するための組み合わせ。
- 薬剤耐性癌に罹患している患者の治療及び/又は造血器悪性腫瘍に罹患している患者における腫瘍再燃の予防において使用するための
i. DNAメチル化阻害剤と、
ii. ビタミンD受容体アゴニストと、
iii. 薬学的に許容し得る担体と、
iv. 場合により、化学療法薬と
を含む医薬組成物。 - 前記造血器悪性腫瘍が、急性骨髄性白血病からなる群から選択される、請求項13記載の通り使用するための医薬組成物。
- 薬剤耐性癌を治療する及び/又は造血器悪性腫瘍に罹患している患者における腫瘍再燃を予防する方法であって、
i. DNAメチル化阻害剤と、
ii. ビタミンD受容体アゴニストと
の組み合わせを同時に又は逐次投与する工程を含む方法。
