発明の分野
線維症性疾患、特に、肺、肝臓、膵臓、皮膚および腎臓における線維症性疾患は、世界中の死亡数の45%も占めている(Friedman,SLら、Science Translational Medicine,5(167):167sr1(2013))。肝疾患に関しては、ヒトにおいて使用するために利用可能な、肝線維症および肝硬変に対する抗線維化剤が存在しない。ウイルス性、肥満症関連およびアルコール関連の線維症および肝硬変の罹患率が上昇しており、ならびに移植のための肝臓の提供が不足しているので、肝線維症の治療は臨床上の緊急性が高い。1985年に、肝星細胞(HSC)は、細胞外マトリックス成分を過剰発現することによって肝線維症を発症する際の主犯として同定された(Friedman SLら、PNAS,82(24):8681−5(1985))。膵星細胞(PSC)は、膵線維症、膵炎および膵がんの発症において中心的役割を果たす筋線維芽細胞様細胞である(Omary,M.B.ら、J.Clin.Invest.117(1):50−59(2007))。膵臓の損傷または炎症に応答して、静止状態のPSCは、HSCと同様に、活性化されて筋線維芽細胞様細胞になり、a−平滑筋アクチンを発現する。
アゴニスティックTRAILレセプターのリガンドおよびアゴニストなどのアポトーシス促進物質が、線維症および基礎疾患(例えば、線維症、肝硬変またはそれらの合併症を特徴とする肝臓、膵臓、肺および皮膚の疾患)を引き起こす細胞のアポトーシスを誘導することができるかまたは増加させることができることが発見された。本明細書中に開示される組成物および方法は、肝線維症および肝硬変の起源物質である活性化された肝星細胞(HSC)、ならびに膵線維症および膵炎の起源物質である活性化された膵星細胞(PSC)を選択的に除去するために使用することができ、活性化された星細胞によって誘導される複数の線維症関連分子の制御を同時に減少させるのに有効であり得る。これは、全体的に、線維症を減少させるかもしくは逆転させるか、またはさらなる線維症関連合併症を予防する。上記組成物は、代表的には、生理学的条件において、活性化されたHSCおよびPSCにおいて選択的に発現されるアゴニスティックTRAILレセプター、例えば、TRAIL−R1/DR4およびTRAIL−R2/DR5を標的化するのに有効である。アゴニスティックTRAILレセプターを標的化するために使用することができるリガンドおよびアゴニストとしては、TRAIL−R1および/またはTRAIL−R2アゴニスト、例えば、組換えヒト(rh)TRAIL、操作されたTRAILアナログ、例えば、ポリマー、例えば、ポリ(エチレングリコール)、共重合体および分枝状アナログならびにヒアルロン酸などの生体高分子で改変された、長時間作用型TRAILタンパク質が挙げられるが、これらに限定されない。TRAILベースの長時間作用型製剤には、ポリマー系;TRAIL融合タンパク質;アゴニスティック抗TRAIL−R1抗体;アゴニスティック抗TRAIL−R2抗体;ならびにTRAIL−R1および/もしくはR2に結合するアゴニスティック小分子またはペプチド分子が含まれる。これらのアゴニストは、単独でまたは他の治療薬と組み合わせて、様々な器官における既存の線維症を減少させることができるかもしくはその発症を阻止することができるか、または様々な器官における既存の線維症を逆転することができる。線維症性疾患を処置するための例示的な方法は、有効量のアポトーシス促進物質を線維症性疾患の処置を必要とする被験体に投与して、過剰量の細胞外マトリックスを産生するかまたは誘導することにより肝臓、膵臓または他の器官の望まれない瘢痕をもたらす肝星細胞、膵星細胞、線維筋芽細胞(fibromyoblast)、線維筋芽細胞性細胞(fibromyoblastic cell)、活性化された内皮細胞または活性化された上皮細胞においてアポトーシスを誘導することを含む。そのアポトーシス促進物質は、TNF関連アポトーシス誘導リガンド(TRAIL)アゴニスト、例えば、PEG化されたTRAILまたはアゴニスティックTRAIL抗体であってよい。そのような抗体は、がん患者において臨床試験中であるが、十分に成功したと示されていない。TRAILは、例えば、天然のまたは遺伝子操作された形態(組換え型)のタンパク質であってよい。好ましい実施形態において、そのTRAILは、ヒトTRAILまたはその機能的フラグメントもしくはバリアントである。例えば、機能的フラグメントは、281アミノ酸のヒトTRAILのフラグメントであってよい。好ましい実施形態において、そのフラグメントは、完全長281アミノ酸のヒト型(1〜281)のうちの114〜281または95〜281のアミノ酸配列を有する。

ヒト初代星細胞
ヒト初代HSC、PSCおよび星細胞培地(SteCM)は、ScienCell Research Laboratories(Carlsbad,CA)から入手した。細胞を、ポリ−l−リジンがコーティングされたプレート内の、2%のFBS、1%の星細胞成長サプリメントおよび1%のペニシリン/ストレプトマイシン溶液が補充されたSteCM培地中で培養した。初代星細胞を活性化するために、次いで、細胞を6ウェルプラスチック培養プレートにおいて1、4、7および14日間培養し、回収した。培養された星細胞におけるDR−4、DR−5およびα−SMAの発現を、ウエスタンブロット解析およびリアルタイムPCRによって決定した。

- 前記TRAILアゴニストが、局所的に投与される、請求項1に記載の方法。
- 前記TRAILアゴニストが、全身的に投与される、請求項14に記載の方法。
- 有効量の前記TRAILアゴニストが、1もしくはそれ超の投与量で投与される、請求項1〜17のいずれか1項に記載の方法。
- 有効量の前記TRAILアゴニストが、1日間またはそれ超の日数の期間の間もしくは該期間に存在するかまたは該期間にわたって放出される、請求項1〜18のいずれか1項に記載の方法。
- 前記TRAILアゴニストが、さらなる活性な作用物質と併用して投与される、請求項1〜19のいずれか1項に記載の方法。
- 請求項1〜20のいずれか1項に記載の方法において使用するためのTRAILアゴニストを含む投薬単位。
- 肝臓または膵臓の疾患を処置するための有効量で製剤化される、請求項21に記載の投薬単位。
- 肺線維症を処置するための有効量および配合で製剤化される、請求項21に記載の投薬単位。
- 皮膚線維症を処置するための有効量および配合の製剤である、請求項21に記載の投薬単位。
