Claims (17)
- カニューレ型ねじ込み体(2)及び受支体(4)を備える骨固定システムであって、
前記ねじ込み体(2)は、その基端(10)の領域に位置しているおねじ頭(8)、及び、前記ねじ込み体(2)の先端(14)に向かって長軸(A)に沿って前記おねじ頭(8)から延在している細長いおねじ杆(12)を備え、
前記おねじ頭(8)は、前記基端(10)に面する表側に表面(20)、及び前記先端(14)に面する背面側に裏面(22)を備え、少なくともこの裏面(22)の一部は、接触面(24)を形成し、
前記受支体(4)は、前記おねじ杆(12)の最大径(ds)よりも大きく、かつ前記おねじ頭(8)の最大径(dh)よりも小さい径(d1)を有する中心孔(26)を備え、さらにこの受支体(4)は、この骨固定システムが完全に係合した状態にある場合、前記接触面(24)に当接するようになっている当接面(30)及び前記骨に当接するようになっている底面(32)を備え、
さらに、前記骨固定システムは、前記ねじ込み体(2)に対して、前記受支体(4)の軸方向の回転を同時に可能にしながら、前記ねじ込み体(2)と前記受支体(4)とが、離脱不能な態様で接続されるように、前記受支体(4)と接続可能に、又は一体的に形成され、かつ、前記おねじ頭(8)を収容するための収容部(40)を有する保持体(6)を備える骨固定システム。 - 前記保持体(6)は、長手方向に見た場合、前記おねじ頭(8)の表面(20)と少なくとも部分的に重なるようになっている保持面(7)を備え、この保持面(7)は、前記骨固定システムが完全に係合した状態にある場合、前記表面(20)に少なくとも部分的に当接するようになっている請求項1に記載の骨固定システム。
- 前記保持体(6)は、前記おねじ頭(8)の最大径(dh)よりも小さい径(d2)を有する中心孔(28)を備えているか、又は定めている、請求項の1または2に記載の骨固定システム。
- 前記受支体(4)の前記中心孔(26)の直径(d1)及び/又は前記保持体(6)の中心孔(28)の直径(d2)を、それぞれ一時的に変更しうるようにするために、前記受支体(4)は、少なくとも前記中心孔(26)のまわりの領域において弾性変形可能であり、及び/又は、前記保持体(6)は、少なくとも前記中心孔(28)のまわりの領域において弾性変形可能である、請求項3に記載の骨固定システム。
- 前記保持体(6)は、前記受支体(4)と一体的に形成されている、請求項1〜4のいずれかに記載の骨固定システム。
- 前記保持体(6)は、前記受支体(4)に取り外し可能に接続される端部キャップ(34)の形態をしている、請求項1〜4のいずれか1項に記載の骨固定システム。
- 前記端部キャップ(34)は、前記受支体(4)の対応するめねじ部(38)と係合しうるおねじ部(36)を備えている、請求項6に記載の骨固定システム。
- 前記骨固定システムは、完全係合状態にされている場合、前記受支体(4)が相対的な長手方向変位を防止するようにして、前記ねじ込み体(2)に固定されるようになっている請求項1〜7のいずれかに記載の骨固定システム。
- 前記おねじ頭(8)と、前記ねじ込み体(2)の先端(14)に配置されている第一の窓(50)を経て外に開く前記おねじ杆(12)とを通じて前記長軸(A)に沿って延在する軸方向チャネル(48)を備える、請求項1〜8のいずれかに記載の骨固定システム。
- 前記軸方向チャネル(48)は、さらに、前記おねじ杆(12)の所定の高さのところで、前記先端(14)からオフセットして配置されている少なくとも一つの第二の窓(52)を備えている、請求項9に記載の骨固定システム。
- 前記おねじ杆(12)は、直径が7.0mm〜8.0mmの範囲、好ましくは7.2mm〜7.5mmの範囲である、請求項1〜10のいずれかに記載の骨固定システム。
- 前記ねじ込み体(2)の長さは、20mm〜200mmの範囲、好ましくは68mm〜180mmの範囲である、請求項1〜11のいずれかに記載の骨固定システム。
- 請求項1〜12のいずれか1項に記載の少なくとも一つの骨固定システム、前記骨固定システムの前記ねじ込み体(2)を固定するためのガイドワイヤポジショナ(60)、ガイドワイヤ(62)、及び、挿入ツール(64)を備える外科用キット。
- 前記ガイドワイヤポジショナ(60)は、斜めにカットした先端(66)を備え、及び/又は、前記ガイドワイヤ(62)は、ねじ部(68)を備えている、請求項13に記載の外科用キット。
- 骨盤病態の治療のための請求項1〜12のいずれか1項に記載の骨固定システムの使用。
- 安定化を必要とする腸骨仙骨関節の変性疾患、及び/又は、骨盤リングの骨折の取り扱い、特に、前記骨盤リングの高エネルギー骨折、腸骨仙骨関節破壊、及び、関連する骨盤骨折、仙骨の骨粗しょう症骨折、及び/又は、寛骨臼骨折の前記治療のための、請求項15に記載の使用。
- a)蛍光透視法を用いて、骨固定システムのためのエントリポイントをマークするステップと、
b)前記エントリポイントで皮膚切開を行うステップと、
c)ガイドワイヤポジショナ(60)を前記切開部位に導入し、かつ、ガイドワイヤ(62)を腸骨の外側皮質を通じて前記骨に挿入するステップと、
d)筋膜及び筋肉組織を拡張し、かつ、ワーキングチャネルチューブを前記切開部位に挿入するステップと、
e)蛍光透視誘導下で、前記骨固定システムを前記ガイドワイヤ(62)上で挿入し、それを前記骨表面に配置し、かつ、前記ねじ込み体(2)を前記骨の中に進めるステップとを備える骨盤リングの中に、請求項1〜12のいずれか1項に記載の骨固定システムの挿入方法。
