本明細書において、「Psapペプチドを用いた処置に応答性である」とは、限定されるものではないが、Psapペプチドを用いた処置に応答した、がんの発達の予防または軽減、がんの症状の軽減、がんの成長の抑制または阻害、既存のがんの転移および/または浸潤の予防、がんの退縮の促進または誘導、がん細胞の増殖の阻害または抑制、血管新生の低下および/またはアポトーシスがん細胞の量の増加が挙げられる。

本明細書で使用する場合、「Psapペプチドを用いた処置」とは、対象に対するPsapペプチドの投与を含むことを意味する。Psapペプチドは、本明細書に記載されている。Psapペプチドを用いた処置は、Psapペプチドのみを用いた処置を含んでよく、または複数の薬剤もしくは療法による処置、例えばPsapペプチドと他の化学療法剤、および/または外科手術、放射線療法、または化学療法などの他の形態による処置を含んでもよい。
本開示の他の側面は、がんを有する対象を処置するための方法に関する。いくつかの態様において、方法は、がんを有し、かつ対照レベルと比較して対象から得た試料中のCD36の上昇したレベルを特徴とする対象に対して、がんを処置するために本明細書に記載のPsapペプチドの有効量を投与することを含む。いくつかの態様において、方法は、
(a)がんを有する対象を、対象が対照レベルと比較して試料中のCD36の上昇したレベルを有することが知られていることに基づいて、選択すること;および
(b)対象が、対照レベルと比較して試料中のCD36の上昇したレベルを有しているので、対象に対してPsapペプチドの有効量を投与すること、

本開示の他の側面は、がんを有し、かつ対照レベルと比較して試料中のCD36の上昇したレベルを特徴とする対象を処置するための医薬の製造における組成物、および組成物の使用に関する。いくつかの態様において、組成物は本明細書に記載のPsapペプチドを含む。いくつかの態様において、試料は腫瘍試料である。
有効量とは、がんの処置などの、医学的に望ましい結果を提供するのに十分なPsapペプチドの投与量である。有効量は、処置される特定のがん、処置される対象の年齢および身体的条件、状態の重症度、処置の期間、任意の併用療法の性質、特定の投与経路、および医師の知識と経験の範囲内の同様の因子などによって変化する。ヒトなどの対象への投与のために、約0.001、0.01、0.1または1mg/kgから、50、100、150または500mg/kg以上までの投与量を、一般的に使用することができる。
種々の投与経路が利用可能である。選択された特定の様式は、処置するがんの種類および治療効果のために必要な投与量に依存する。本開示の方法は、一般的には、医学的に許容し得る任意の投与様式を用いて実施することができ、すなわち、臨床的に許容し得ない副作用を引き起こすことなく活性化合物の有効レベルを生じる任意の様式を意味する。かかる投与様式としては、経口、直腸、局所、鼻腔、皮内、または非経口経路を含む。用語「非経口」は、皮下、静脈内、筋肉内、または注入を含む。
経口投与に適した組成物は、カプセル剤、錠剤、トローチ剤などの個別の単位として提供することができ、それぞれが、抗炎症剤の所定量を含む。他の組成物としては、水性液体または非水性液体中の懸濁液、例えばシロップ剤、エリキシル剤またはエマルジョンなどが挙げられる。
いくつかの態様において、Psapペプチドの投与は、他の療法、例えば化学療法、放射線および/または外科手術などと組み合わせてもよい。
CD36(分化クラスター36)は、脊椎動物の多くの細胞型の表面上に見出される細胞膜内タンパク質であり、これはまた、FAT、GP4、GP3B、GPIV、CHDS7、PASIV、SCARB3、およびBDPLT10としても知られている。ヒトCD36のためのEntrez遺伝子IDは948である。例示的なヒトCD36転写物およびタンパク質は、以下の通りである:

細胞株および初代細胞
細胞株PC3は、以前に記載されている(Kang et al. PNAS. 2009; 106:12115-20)。PC3細胞を、10%FBSを含むRPMI中で培養した。ヒト乳がん細胞株MDA−MB−231およびMCF−7は、以前に記載されている(Ryu et al. 細胞を、プロテアーゼ阻害剤(Roche Applied Science)を含有する溶解緩衝液(BioRad)中でホモジナイズした。試料は、1×SDSサンプリング緩衝液中で煮沸し、4〜20%勾配のBis-Tris NuPAGEゲル(Invitrogen)にロードした。ウェスタンブロッティングは、CD36に特異的な抗体(AbCam、ab78054)またはβアクチン(Sigma-Aldrich)を用いて実施した。
細胞増殖は、MTT(3−{4,5−ジメチルチアゾール−2−イル}−2,5−ジフェニルテトラゾリウムブロミド、Sigma-Aldrich)アッセイを用いて測定した。細胞を96ウェル培養プレート中の50μLの増殖培地に播種し、一晩付着させた。次に50μLの増殖培地+2倍濃縮した処理試薬を添加した。各処置時点の後に、10μLの5%MTT溶液(PBS中に緩衝化)を各ウェルに加えた。プレートを37℃でさらに4時間インキュベートし、MTTを、細胞のミトコンドリアでのホルマザン結晶に代謝的に変換させた。ホルマザン結晶は、各マイクロプレートウェルに50%のN−N−ジメチルホルムアミド中の100μLの10%ドデシル硫酸ナトリウムを加えることにより、最終的に可溶化した。550および680nmでの吸光度(それぞれ、ホルマザン塩および参照波長に対応)を、測色マイクロプレートリーダーを用いて測定した。完全培地のみを含むウェルを、対照として使用した。各実験は、各薬物濃度について6回の反復を用いて、2回行った。
Psapペプチドによって上方制御されるTsp−1は、間接的に抗血管新生メカニズムを介するのみでなく、直接がん細胞に作用し得ると仮定した。これを試験するために、LLC細胞を、組換えTsp−1またはDWLPK(配列番号2)Psapペプチドのいずれかで処置し、細胞増殖をMTTアッセイを用いて測定した。Tsp−1は細胞増殖を減少させることができ、一方Psapペプチドは、細胞増殖に影響を与えなかったことがわかった(図1A)。これは、Tsp−1ががん細胞に直接作用することができるという仮説を支持し、なぜならばこのアッセイは、in vitroで任意の血管の非存在下で行われたからである。これらの結果はまた、Psapペプチド単独ではがん細胞の増殖に影響を与えないようであることを示し、PsapペプチドがTsp−1の上方制御を介して間接的にがんを処置できるという仮説を支持する。LLC細胞は、Tsp−1の受容体であるCD36を発現することが示されており、これは、Tsp−1がCD36を介してがん細胞に直接作用し得ることを示す(図1B)。
膵臓細胞株AsPc1も検討し、CD36を発現することが見出された。
CD36のレベルはまた、腹水を有する患者に由来する原発性卵巣がん細胞においても測定した。CD36タンパク質は、試験した全ての原発性卵巣がん細胞で検出可能であった(図3)。
方法
マウスおよび細胞株
すべての動物の作業は、Institutional Animal Care and Use Committeeによって承認されたプロトコルに従って実施する。野生型C57BL/6J、およびGFPトランスジェニックC57BL/6-Tg(ACTB-EGFP)1Osb/Jは、The Jackson Laboratory(Bar Harbor, Maine)から入手する。CB-17 SCIDマウスは、Charles River(Wilmington, MA)から入手する。
細胞株PC3およびPC3M−LN4は、以前に記載されている(14)。ヒト乳がん細胞株MDA−MB−231およびMDA−MB−LM2は、以前に記載されている(Ryu et al. アーカイブ標本(根治的前立腺切除標本、または転移の生検材料)は、Department of Pathology, The Gade Institute, Haukeland University Hospitalのファイルから取得する。ホルマリン固定前立腺切除標本をパラフィンに包埋し、5mm間隔での全体マウントステップ切片で検査した。組織マイクロアレイ(TMA)を、それぞれのケースにおける最高の腫瘍グレードの領域から3つの組織コア(直径0.6mm)を選択して構築した。
TMAパラフィンブロックからの薄いパラフィン切片(5μm)をキシレン/エタノールで脱脂し、その後20分間、クエン酸緩衝液(pH6.0)中で熱誘導性マイクロ波エピトープ回復し、CD36抗体と共に室温で60分間、インキュベートする。免疫染色は、検出システムとしてのEnVision鎖ポリマー法でDAKO Autostainer(Dako Cytomation, Copenhagen, Denmark)上で実施する。抗原の局在化は、DABジアミノベンジジンペルオキシダーゼ反応を用い、ヘマトキシリンで対比染色して達成する。
免疫染色は半定量的に推定され、染色強度(0〜3)と免疫陽性腫瘍細胞の割合(<10%=1、10〜50%=2、>50%=3)の積として得られる染色指数(SI)を算出する。染色指数(0〜9の範囲)は、各カテゴリー内でいくらかの変動が予想される、カテゴリースケール(categorical scale)である。
CD36レベルはがん細胞株において、CD36を標的とするmiRNAまたはshRNAをコードするレトロウイルスまたはレンチウイルスベクターを用いて低減される。ノックダウン効率は、qPCR分析を用いて試験する。全RNAを、製造業者のプロトコルに従ってPicoPureRNA抽出キット(Arcturus)を用いて抽出する。RNAを、qScript(登録商標)のcDNAスーパーミックス(Quanta biosciences)を用いてcDNAに変換する。qPCRは、プライマーおよびiQTM SYBER Greenマスターミックス(Biorad, Hercule, CA)を用いて実施する。標準プロトコルの、95℃で10分間の初期変性、次いで95℃で10秒間、60℃で30秒間および72℃で30秒間を40サイクル、次に72℃で5分間の最終伸長、および溶融曲線分析を、Bio-Rad-CFX Managerソフトウェアを組み合わせたBioRad CFX96 Real Time System(BioRad)で実施する。対照と比較して、各転写物の相対存在量は、デルタCt法を利用して計算する。

実験的転移のために、7週齢のC57BL/6マウスに、1×105個のルシフェラーゼ標識LLC細胞を、尾静脈を介して注入する。同所性乳がん細胞の注入のために、5×106個のMDA−MB−231またはその転移性変異型MDA−MB−LM2細胞を、0.1mlの容量でCB-17 SCIDマウスの脂肪パッドに注入する。腫瘍増殖および肺転移(原発腫瘍の切除後)は、生きている動物の生物発光イメージング(Xenogen)により、週に1回モニタリングした。同所性前立腺がん細胞の注入のために、2×106個の生きているLN4または細胞を、マウスの前立腺に注入した。
転移の負荷をin vivoで決定するために、マウスを麻酔し、75mg/kgのD−ルシフェリン(PBS中の30mg/mLを100μL)を腹腔内注射した。転移性増殖は生物発光イメージングを用いて経時的にモニタリングし、これは、D−ルシフェリン注入後5分間仰臥位にしたマウスを用いて、Living Image収集および分析ソフトウェア(Xenogen)を組み込んだXenogen IVISシステムで実施した。BLIプロットについては、各マウスについての光子束を、マウスの胸部を囲む関心のある同一の円形領域を用いて算出した。
8週齢のマウスを、PBSに希釈したPsapペプチド(例えばDWLPK(配列番号2)、DWLP(配列番号3)、またはその修飾物)を用いて、30mg/kg/日の用量で腹腔内注射により2週間まで処置する。
CD36レベルは、がんを有するヒト対象からの組織試料において測定する。
CD36レベルはがん細胞株でノックダウンされ、低減されたCD36を有するこれらの細胞株を、マウスに注入する。次いでマウスに、Psapペプチドを投与する。腫瘍増殖および転移の負荷をモニタリングする。がん細胞におけるCD36のノックダウンは、in vivoでのPsapペプチドの抗がん活性を低減させることが期待される。
方法
特に明記された以外は、例3で使用した方法は、例1および2で使用した方法と同一である。CD36の発現について試験した細胞株は、膵臓がん(AsPC1)、卵巣がん(DF−14およびID−8)、乳がん(MDA−MB231およびLM2)、前立腺がん(PC3、PC3−M−LN4、LN−CAP、およびLN−CAP−LN3)、黒色腫(B16−BL6)、および肺がん(LLC)の細胞株であった。これらの細胞株の全ては当分野で知られており、および/または市販されている。
CD36を発現する卵巣がん細胞を、対照またはトロンボスポンジン(Tsp−1、100ng、500ng、または1000ng)のいずれかで処置し、生存細胞のパーセントを、0時間または48時間に測定した。
膵臓がんのマウスモデルでは、1×106個のAsPc1ヒト膵臓細胞を、SCIDマウスの膵臓に注入した。マウスは、対照またはPsapペプチドdWlP(配列番号47、20mg/kg/日または40mg/kg/日)のいずれかで処置した。処置は25日目に開始し、21日間毎日続けた。次いで、マウスを安楽死させ、原発腫瘍塊を測定した。腹水の有無も測定した。
黒色腫マウスモデルについては、B16−BL6細胞をマウスに注入した。マウスは、PsapペプチドdWlP(配列番号47、10または40mg/kg)または対照のいずれかで処置した。腫瘍体積は、細胞注入後約20〜25日後まで、継時的に測定した。
複数のがん細胞株を、CD36の発現について試験した。CD36タンパク質は試験したすべての細胞株において検出され、特にAsPC1、DF−14、MDA−MB231、およびPC3細胞株において高レベルであったことが見出された(図5)。
CD36を発現する卵巣細胞は、用量依存的にTsp−1媒介の細胞殺傷に感受性であることが示された(図6)。
2つの「高」CD36細胞株(卵巣がん細胞およびAsPC膵臓がん細胞)と1つの「低」CD36細胞株(B16−B6がん細胞)をマウスに注入して、Psapペプチドの、腫瘍増殖および転移に対する効果を試験した。「高」CD36がんモデルは、Psapペプチドによる処置に応答して退縮したことが見出された(図5および7)。卵巣がんモデルでも、転移性疾患の退縮が示された(図5)。膵臓がんモデルもまた転移の阻害を示し、これは、Psapペプチドで処置した19匹のマウスのうち1匹のみが腹水を形成し、一方で対照で処置した10匹のマウスでは4匹が腹水を形成したことによる。「低」CD36黒色腫モデルにおいて、Psapペプチドでの処置は、原発腫瘍の成長を阻害したが、腫瘍退縮は引き起こさなかったことが見出された(図8)。これらの結果は、「高」CD36がんが、より強くPsapペプチド処置に応答する可能性があり(例えば、原発腫瘍および/または転移の退縮)、「低」CD36がんは、より弱く応答する可能性があること(例えば、原発腫瘍の退縮というより阻害)を示す。
