(v)β2−アドレナリン作動性受容体またはそのアゴニストの合成または細胞内貯蔵からの放出を増大させること;あるいは
(vi)β2−アドレナリン作動性受容体またはそのアゴニストの分解速度を低下させること。

本発明はまた、さらなる局面においては、上記局面による組成物を製造するためのプロセスであって、(状況に応じて)β2−AR立体配座、またはβ2−AR受容体活性、またはその活性化を陽性または陰性に調節する剤の治療効果的な量を医薬的に許容されるビヒクルおよび必要な場合には他の剤(例えば、UV吸収剤)と一緒にすることを含むプロセスを提供する。
Red Durocモデル
解剖学的および生理学的に、ブタの皮膚はヒトの皮膚と非常に類似している[19]。小型哺乳動物の皮膚における創傷は、皮膚が束縛されていない場合、主として創傷収縮によって治癒し[20]、これに対して、ブタおよびヒトの皮膚創傷は主に創傷の再上皮化によって閉じる[21]。Red Durocブタは、線維増殖性瘢痕および肥厚性瘢痕の形成の再現性のあるモデルとして十分に確立されている[22、23]。ヒトの肥厚性瘢痕と類似して、Red Durocブタは、創傷を過度な量のコラーゲン沈着および増大した創傷収縮を伴って治癒させる[23]。加えて、創傷が、創傷部の縁における色素沈着過剰を伴って治癒し[22]、これにより、色素沈着過剰の傷痕をもたらし[24]、かつ、傷痕は隆起し、これらはヒトの傷痕に類似している[25]。実際、Red Durocの傷痕モデルは多数のグループによって評価および使用されており[26、27、28、29]、傷痕化を軽減するための治療的介入を試験するために理想的である[22、26]。
ブタでの創傷傷痕研究をRed Durocブタにおいて行った。5つ(2×2cm)の全層創傷からなる4列を、10匹のRed Durocの剃毛した背中に、脊椎の両側で作製した(図1、図4)。5匹のコントロールブタおよび5匹の処置ブタを毎日、創傷部あたり500μlのGranugelを単独で用いて、または、5mMのサルブタモールを含有して処置した。動物あたり2つの創傷部を、創傷後7日、14日、21日、28日および42日で生検した(6mmの穿刺)。動物あたり残る10個の創傷傷痕は写真撮影し、創傷後56日目に集め、これにより、1つの位置あたり合計で5個の創傷を、すなわち、それぞれの群については合計で50個の創傷を得た。
いくつかの尺度が、ヒト傷痕の重篤度をスコア化するために開発されている。Vancouver傷痕尺度(VSS)[7]、Manchester傷痕尺度(MSS)[8]、および、患者・観察者傷痕評価尺度(POSAS)[9]。本発明者らは、VSS[7]およびMSS[8]に基づいて、Red Durocの傷痕の重篤度をスコア化するために本発明者自身の尺度を開発した。患者・観察者傷痕評価尺度(POSAS)は、患者の関与[9]を必要とするので使用することができなかった。
コントロール創傷およびsal処置創傷の両方について、それぞれの位置における平均傷痕面積に最も近い傷痕が示される(図5b)。傷痕面積が、sal処置したとき、創傷後56日で47.1%減少した(図6a)。実際、創傷後28日目および42日目までに、sal処置創傷の傷痕面積が既に、コントロール創傷よりも34.1%および37.7%小さかった(図6a、***P<0.001)。

Claims (16)
- 皮膚傷痕の色整合化を改善するための、例えば、傷痕の色素沈着過剰を軽減するための方法であって、β2−アドレナリン作動性受容体の立体配座、またはβ2−アドレナリン作動性受容体の活性、またはその活性化を陽性に調節する剤の治療効果的な量をその必要性のある対象に投与することを含む、方法。
- 皮膚傷痕の色整合化を改善すること、例えば、傷痕の色素沈着過剰を軽減することをその必要性のある対象においてもたらす際に使用される、β2−アドレナリン作動性受容体の立体配座、またはβ2−アドレナリン作動性受容体の活性、またはその活性化を陽性に調節する剤。
- 皮膚傷痕の色整合化を改善すること、例えば、傷痕の色素沈着過剰を軽減することをその必要性のある対象においてもたらすための医薬品の製造における、β2−アドレナリン作動性受容体の立体配座、またはβ2−アドレナリン作動性受容体の活性、またはその活性化を陽性に調節する剤の使用。
- 前記対象は、色素沈着過剰を有するか、またはその危険性があるために、皮膚傷痕の色整合化を改善すること、例えば、傷痕の色素沈着過剰を軽減することを必要としている、前記請求項のいずれか一項に記載の方法、剤または使用。
- 前記対象は、下記要因:
・前記対象は傷痕の色素沈着過剰をこれまでに引き起こしていること;
・前記対象は、日光または紫外(UV)線にさらされたとき、熱傷を生じさせるのではなく、むしろ、容易に日焼けすること;
・前記対象は非白色人種の人種起源を有すること;
・前記対象の皮膚の色(例えば、日焼けしていない領域における皮膚の色)が、生まれながら金髪の白色人種人に典型的な皮膚の色よりも濃いと見なされること
のうちの1つまたは複数に基づいて色素沈着過剰の危険性があるとして選択される、請求項4に記載の方法、剤または使用。 - 前記対象は、中国人、ブラックアフリカ、アジア人または南欧人の人種起源であることが少なくとも支配的であるために、ならびに/あるいは、皮膚のタイプが、Fitzpatrick尺度に基づいて、タイプIII、タイプIV、タイプVまたはタイプVIとして評価され得るならば、色素沈着過剰の危険性があるとして選択される、請求項4または5に記載の方法、剤または使用。
- β2−ARを陽性に調節する前記剤がβ2−アドレナリン作動性受容体アゴニストである、前記請求項のいずれか一項に記載の方法、剤または使用。
- 前記β2−アドレナリン作動性受容体アゴニストが、レボサルブタモール(Levosalbutamol)、イソプロテレノール(β1およびβ2)、メタプロテレノール、テルブタリン、イソエタリン(Isoetarine)、ピルブテロール、プロカテロール、リトドリン、エピネフリン、フェノテロール、ブトキサミン、サルブタモール、クレンブテロール、ホルモテロールまたはサルメテロールのうちの1つまたは複数である、請求項8に記載の方法、剤または使用。
- 前記剤がサルブタモールである、前記請求項のいずれか一項に記載の方法、剤または使用。
- 前記対象は、擦過傷、擦り傷、外科的切開および他の外科的手法、熱傷、炎症後色素沈着過剰(例えば、湿疹、乾癬またはざ瘡の後での色素沈着過剰)、ならびに、他の形態の外傷、医原性または遺伝的敏感性を含む物理的な傷害または損傷の結果として形成される穿通創または非穿通創のうちの1つまたは複数から生じる傷痕のために、皮膚傷痕の色整合化を改善すること、例えば、傷痕の色素沈着過剰を軽減することを必要としている、前記請求項のいずれか一項に記載の方法、剤または使用。
- 前記剤が、局所投与または全身投与により投与するためのものである、前記請求項のいずれか一項に記載の方法、剤または使用。
- 前記創傷がUV線から遮蔽される、前記請求項のいずれか一項に記載の方法、剤または使用。
- 前記剤が、標準的な創傷治療において使用されるギブス、包帯、創傷被覆材または他の閉鎖性保護製造物、あるいは、医薬的に許容される経皮パッチ、あるいは、親水コロイドのゲルまたは他の好適な組成物に配合される、前記請求項のいずれか一項に記載の方法、剤または使用。
- β2−ARの立体配座、またはβ2−ARの受容体活性、またはその活性化を陽性に調節する剤の治療効果的な量、および、必要な場合には医薬的に許容されるビヒクル、ならびに、必要な場合にはUV吸収剤を含む、傷痕色素沈着過剰症防止組成物。
- メラノサイト病変および/またはメラノーマを対象において処置または防止するための方法であって、β2−アドレナリン作動性受容体の立体配座、またはβ2−アドレナリン作動性受容体の活性、またはその活性化を陽性に調節する剤の治療効果的な量をその必要性のある対象に投与することを含む、方法。
- 皮膚傷痕の色整合化を改善すること、例えば、傷痕の色素沈着過剰を軽減することを、それを切望している対象においてもたらすための美容処置。
