Abstract
Description
The present invention relates to an endoscope clip device.
2. Description of the Related Art There is provided a clip device that excises a living tissue in a body cavity under an endoscope and ligates the excised site to stop bleeding. With regard to this type of endoscope clip device and a medical clip used in the device (hereinafter abbreviated as a clip), Patent Document 1 discloses an arrowhead hook at the tip of an operation wire provided in a treatment instrument body. It is described that the clip is inserted and connected to a connecting member at the base end of the clip. The connecting member has a pair of opposed half-cylindrical through holes, and the arrowhead hooks are pushed into the connecting member and connected so as to expand the through holes.
The clip has a pair of opposing arm portions, and closes the arms so that the tips of the arm portions approach each other. The arm portion has a self-expanding force, and the arm portion is closed against the force and is inserted into the forceps hole of the endoscope in a state of being accommodated in the sheath. When the vicinity of the living tissue is reached, the arm portion is protruded from the sheath and opened by elastic restoring force, and then the arm portion is closed by pulling the operation wire and pulling the clip back into the sheath. Thereby, a biological tissue can be hold | gripped and ligated with an arm part.
Here, when ligating a living tissue, it is required to match the direction in which the arm portion of the clip is closed with respect to the living tissue as desired. For this reason, by rotating the operation wire by torque, the torque is transmitted to the clip through the arrowhead hook and the connecting member. In the device of Patent Document 1, when the operation unit main body is rotated by torque, the operation wire and the arrowhead hook are rotated in conjunction with this, and the connecting member is further rotated by the frictional force between the arrowhead hook and the connecting member. Thereby, the presser tube and the clip are rotated by torque, and the orientation of the arm portion can be adjusted.
However, the apparatus of Patent Document 1 has a problem that the arm portion of the clip cannot always be oriented in a desired direction even if the operation portion main body is rotated by torque for the following two reasons. The first reason is that the arrowhead hook rotates while rubbing inside the half-cylindrical through-hole, so that only a part of the torque is transmitted to the connecting member even if the operation unit body or the operation wire is rotated by torque. That is. For this reason, even if the user wants to rotate the arm portion of the clip by a desired angle, the user cannot know exactly how much torque angle should be given to the operation portion main body. The second reason is that since the arrowhead hook and the connecting member contact the inner surface of the sheath and receive frictional force, a part of the torque applied to the connecting member from the arrowhead hook is further lost and transmitted to the clip. is there. As a result, even if the operation unit main body and the operation wire are torque-rotated, the rotational force is not substantially transmitted to the clip arm unit, or until the clip arm unit rotates after the operation of the operation unit main unit is torqued. The time delay increases and it becomes difficult to point the arm portion of the clip in a desired direction.
The present invention has been made in view of the above-described problems, and provides an endoscope clip device capable of accurately pointing an arm portion of a clip in a desired direction.
An endoscope clip device according to the present invention includes a clip having a plurality of arms for grasping a living tissue and a locking portion provided on a proximal end side of the arms, a long sheath, and the sheath A treatment instrument body having an operation wire inserted into the inside of the device so as to be movable forward and backward, and provided with a tip connecting portion at a distal end thereof, and the locking portion and the tip connecting portion are arranged around an axis of the operation wire. A cylindrical sleeve that is locked and connected to each other and that accommodates the tip connecting portion is provided at a distal portion of the operation wire, and by rotating the operation wire by torque, the locking portion, The distal end connecting portion and the sleeve rotate around the axis inside the sheath.
According to the endoscope clip device of the present invention, since the clip engaging portion and the operation wire distal end connecting portion are engaged with each other around the axis of the operation wire, the torque applied to the operation wire is reduced. Good transmission to clip. In addition, since the cylindrical sleeve rotates axially inside the sheath in a state in which the clip locking portion and the operation wire distal end coupling portion are accommodated, the locking portion and the distal end coupling portion come into contact with the inner surface of the sheath to generate frictional force. It is prevented from receiving. Thereby, according to this invention, the arm part of a clip can be accurately oriented to a desired direction.
Hereinafter, embodiments of the present invention will be described with reference to the drawings.

The
The

Here, “outward” means a direction away from the axis of the
The arm
The clip 110 (except for the fastening member 150) of the present embodiment is made of a single material, that is, one
The arm
The
The arm
As shown in FIGS. 2 and 3, the locking
The
The protruding
The locking
The distal
As shown in FIGS. 3A and 3B, N (where N is an integer of 2 or more) projecting
On the other hand, the periphery of the
The projecting
When the
The
The
As a result, as will be described later with reference to FIGS. 5B and 9B, when the
The specific shape of the
As shown in FIG. 3B, a plurality of
The base portion on the distal side of the projecting
More specifically, the joint of the
The
The inner diameter dimension of the

One or both of the first

As shown in FIG. 4, the distal portion of the

The inner diameter of the sleeve
The reduced-diameter stepped
The diameter-reduced
7 to 10, the
As shown in FIG. 9, the tightening

FIG. 5A is an explanatory view showing a state in which the
The
Specifically, as shown in FIG. 5A, the
Here, the outwardly elastic deformation of the
Specifically, when the second
As shown in FIG. 6, the
As shown in FIG. 6, with the
At this time, at least the
When the treatment instrument main body 90 (see FIG. 1) is rotated by torque, the
The locking part 130 (projection piece part 142) and the tip connecting part 50 (large diameter body part 59) are connected to each other around the axis of the
According to the present embodiment, the locking
In addition, a cylindrical reduced diameter sleeve 70 (sleeve) that accommodates the
This prevents the
Further, the diameter-reduced
After determining the position and orientation of the
The
By pulling the
After adjusting the
When the
That is, by further pulling the
For this reason, by further pulling the
More specifically, the first
The
With the above operation, a series of procedures from connecting the
