Abstract
Description
グラチラマーアセテート(GA)は、RRMS患者における頻繁な再発を減少させる市販製品であるCopaxone(登録商標)の活性成分である。そのRR-MSにおける再発頻度および障害の累積を減少させる効果は、他の利用可能な免疫調節治療の効果に匹敵する(8、9、10)。グラチラマーアセテートは、4つの天然アミノ酸:L−グルタミン酸、L−アラニン、L−チロシンおよびL−リジンを含む合成ポリペプチドの酢酸塩からなる。グラチラマーアセテートの平均分子量は、5,000と9,000ダルトンの間である。1日の標準的用量は20mgであり、GAは一般的によい薬物耐性を示すが、該医薬に対する反応は様々である。様々な臨床試験において、GAはRR-MSに伴う患者の再発率を低下させ、障害の進行を減少させた。GAの治療効果は、様々な臨床センターにおける磁気共鳴画像法(MRI)の所見の結果によってサポートされている(11)。しかし、GA治療に対する反応を有効に予測することができるバイオマーカーは存在しない。
MSには、5つの異なる疾患の段階および/またはタイプが存在する。:
1)良性型多発性硬化症;
2)再発寛解型多発性硬化症(RRMS);
3)二次進行型多発性硬化症(SPMS);
4)進行性再発型多発性硬化症(PRMS);および
5)一次進行型多発性硬化症(PPMS)
良性型多発性硬化症は、後方視的診断であり、完全な回復を伴う1〜2の再燃、持続性の障害がないこと、および最初の発症後10〜15年間疾患の進行がないことによって特徴付けられる。しかし、良性型多発性硬化症は、他の型の多発性硬化症に移行しうる。
再発型MSという語には以下のものが含まれる。: 1)RRMSの患者; 2)SPMSであって重なる再発を示す患者;および3)CISであって、その後のマクドナルド診断基準によるMRI走査において病変の散在を示す患者
本明細書の記載では、多発性硬化症の再発型には以下のものを含む。:
再発寛解型多発性硬化症(RRMS)は、予測できない神経学的機能障害(再発)の急性のエピソードによって特徴付けられ、その後、多様な回復および臨床的に安定な期間を示す;
二次進行型MS(SPMS)は、RRMSを有する患者が、重なる再発の有無にかかわらず持続的な悪化に進行する;および
一次進行型の多発性硬化症(PPRMS)または進行性再発型多発性硬化症(PRMS)は、まれな型だが、患者は当初から進行型の劣化へと移行し、後に再発にも進行しうる。
クルツケ(Kurtzke)拡大身体障害状態スケール(EDSS)は、多発性硬化症における障害を定量化する方法である。EDSSは、MSの症状を示す患者をより低位の区分に括るために用いられていた以前の身体障害状態スケールに取って代わった。EDSSは8つの機能系(FS)において、障害を定量し、そのそれぞれを神経科医が機能系スコア(FSS)へ割り当てることを可能とする。機能系とは、:錐体、小脳、脳幹、感覚、腸および膀胱、視覚ならびに大脳である(www.mult-sclerosis.org/expandeddisabilitystatusscaleによる。)。
臨床的再発は、本明細書では「再発」、「確認された再発」または「臨床的に定められた再発」としても用いられるが、一以上の新たな神経学的異常の発生、または一以上の以前に観察された神経学的異常の再現として定義される。
再発寛解型多発性硬化症患者(n=12)は、TevaによるFORTEと称される臨床試験において1日に20mgのGAまたは40mgのGAのいずれかで治療された(www.medicalnewstoday.com/articles/48863.php)。患者から、全血がベースラインを含む3回(ベースライン、2ヶ月および6ヶ月)採取され、末梢血単核細胞(PBMCs)はベースライン、2ヶ月および6ヶ月の時点で凍結保存された。
ベースライン、2ヶ月および6ヶ月の時点で、血液が患者から採取された。末梢血単核細胞(PBMCs)は、血液からフィコール−ハイパック濃度勾配を用いて精製され凍結保存された。凍結保存された各時点のPBMCsは解凍され、5%血清が補充されたAIM V培地中で一晩置かれ、PMA(1mg/ml;SIGMA)およびイオノマイシン(5mg/ml;SIGMA)で6時間の間刺激された(最終量200マイクロリットル中40,000のPBMCs)。上澄みは、刺激されたおよび刺激されなかった細胞から除かれ、ヒト27-plexキット(Bio-Rad Laboratories、 Hercules、CA)によるアッセイまで、−20℃で保存された。データは、Bio-Plexアレイリーダーを用いることにより得られ、Bio-Plex Manager 4 ソフトウエア(Bio-Rad)を用いて分析された。グラフは、プリズム(Prism)ソフトウエア(GraphPad Software、Inc.)を用いて描かれた。われわれは、2-plex(IL−17およびIFNγ)および27マルチプレックス(27のヒトサイトカインを含む)の双方を、この包括的なサイトカイン分析において用いた。これら二つのアッセイで得られたマルチプレックスデータは、各患者を基準に個別化され図2に示されている。
臨床的な再発は、一またはそれ以上の新たな神経学的異常の出現または以前に観察された一またはそれ以上の神経学的な異常の再現として定義される。
完全な神経学的評価は、1ヶ月前(選別)、0(ベースライン)、3、6、9、12(二重盲験フェーズの終了)、18および24ヶ月(終了/早期打ち切り)の時点で行われた。
神経学的変化が確認された再発と考えられるか否かについての決定は、神経科の検査医により評価されたEDSS/FSの実際の(換算されたものではない)スコアに基づき、治療医によりなされた。
患者は、再発の発生を示唆するいかなる症状が出た場合でも、48時間以内に研究場所に電話をするように指示された。

多発性硬化症または多発性硬化症に合致する単独の臨床的発症(single clinical attack)に苦しむヒト患者を、グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物により治療する方法であって、以下の段階を含む方法:
a)ヒト患者がグラチラマーアセテートへの反応者であるかを、ヒト患者の血中におけるIL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度からなる群またはそれらの組み合わせから選択されるバイオマーカーを評価することにより決定し、;および
b)グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物を、ヒト患者がグラチラマーアセテートへの反応者として同定された場合にのみ投与する。

グラチラマーアセテートによる治療法に対する、多発性硬化症または多発性硬化症に合致する単独の臨床的発症(single clinical attack)に苦しむヒト患者の臨床的反応を予測する方法であって、該方法は、該ヒト患者の血中におけるIL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度からなる群またはそれらの組み合わせから選択されるバイオマーカーを評価し、それによりグラチラマーアセテートへの臨床的反応を予測する方法。

c)該ヒト患者がグラチラマーアセテートへの反応者として同定された場合には該医薬組成物の投与を続け、該ヒト患者がグラチラマーアセテートへの反応者として同定されなかった場合には該医薬組成物の投与を変更する。

[7]または[8]記載の方法において、前記医薬組成物は、20mgのグラチラマーアセテートを含む0.5ml水性溶液の単位用量である、方法。

[1]−[9]のいずれかに記載の方法であって、前記IL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度、またはそれらの組み合わせが、PBMCの上澄み濃度である、方法。

[1]−[9]のいずれかに記載の方法であって、前記IL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度、またはそれらの組み合わせが、治療前の段階で観察される、方法。

[1]−[11]のいずれかに記載の方法であって、前記IL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度、またはそれらの組み合わせが、グラチラマーアセテートの最初の投与から2ヵ月後に観察される、方法。

[1]−[12]のいずれかに記載の方法であって、前記ヒト患者がグラチラマーアセテートへの反応者として同定された場合、該ヒト患者は、その後、グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物を単剤療法として投与される、方法。

Claims (25)
- 多発性硬化症または多発性硬化症に合致する単独の臨床的発症(single clinical attack)に苦しむヒト患者を、グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物により治療する方法であって、以下の段階を含む方法:
a)ヒト患者がグラチラマーアセテートへの反応者であるかを、ヒト患者の血中におけるIL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度からなる群またはそれらの組み合わせから選択されるバイオマーカーを評価することにより決定し、;および
b)グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物を、ヒト患者がグラチラマーアセテートへの反応者として同定された場合にのみ投与する。 - 請求項1の方法において、前記グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物の投与は、7日間にわたり各皮下注射の間を少なくとも1日空けて、ヒト患者に該医薬組成物を3回皮下注射する、方法。
- 請求項1または2の方法において、前記医薬組成物は、20mgのグラチラマーアセテートを含む0.5ml水性溶液の単位用量である、方法。
- グラチラマーアセテートによる治療法に対する、多発性硬化症または多発性硬化症に合致する単独の臨床的発症(single clinical attack)に苦しむヒト患者の臨床的反応を予測する方法であって、該方法は、該ヒト患者の血中におけるIL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度からなる群またはそれらの組み合わせから選択されるバイオマーカーを評価し、それによりグラチラマーアセテートへの臨床的反応を予測する方法。
- 請求項4の方法において、グラチラマーアセテートによる治療法が、グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物の、7日間にわたり各皮下注射の間を少なくとも1日空けて、ヒト患者に該医薬組成物を3回皮下注射することによる投与を含む、方法。
- 請求項5の方法において、前記医薬組成物は、20mgのグラチラマーアセテートを含む0.5ml水性溶液の単位用量である、方法。
- 多発性硬化症または多発性硬化症に合致する単独の臨床的発症(single clinical attack)に苦しむヒト患者を、グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物により治療する方法であって、以下の段階を含む方法:
a)該ヒト患者に、治療量のグラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物を投与する;
b)該ヒト患者がグラチラマーアセテートへの反応者であるかを、該ヒト患者の血中におけるIL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度からなる群またはそれらの組み合わせから選択されるバイオマーカーを評価することにより決定し、;および
c)該ヒト患者がグラチラマーアセテートへの反応者として同定された場合には該医薬組成物の投与を続け、該ヒト患者がグラチラマーアセテートへの反応者として同定されなかった場合には該医薬組成物の投与を変更する。 - 請求項7の方法において、前記ヒト患者への投与が、治療量のグラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物を、7日間にわたり各皮下注射の間を少なくとも1日空けて、該ヒト患者に該医薬組成物を3回皮下注射する、方法。
- 請求項7または8記載の方法において、前記医薬組成物は、20mgのグラチラマーアセテートを含む0.5ml水性溶液の単位用量である、方法。
- 請求項1−9のいずれかに記載の方法であって、前記IL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度、またはそれらの組み合わせが、PBMCの上澄み濃度である、方法。
- 請求項1−9のいずれかに記載の方法であって、前記IL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度、またはそれらの組み合わせが、治療前の段階で観察される、方法。
- 請求項1−11のいずれかに記載の方法であって、前記IL−17濃度、TNF−α濃度、IL−2濃度およびIFN−γ濃度、またはそれらの組み合わせが、グラチラマーアセテートの最初の投与から2ヵ月後に観察される、方法。
- 請求項1−12のいずれかに記載の方法であって、前記ヒト患者がグラチラマーアセテートへの反応者として同定された場合、該ヒト患者は、その後、グラチラマーアセテートおよび医薬的に許容される担体を含む医薬組成物を単剤療法として投与される、方法。
- 請求項1−12のいずれかに記載の方法であって、前記ヒト患者がグラチラマーアセテートへの反応者として同定されなかった場合には、該ヒト患者はその後、グラチラマーアセテートではない多発性硬化症治療薬を投与される、方法。
- 請求項1−14のいずれかに記載の方法であって、前記バイオマーカーがIL−17濃度である、方法。
- 請求項1−14のいずれかに記載の方法であって、前記バイオマーカーがIL−17(A)濃度である、方法。
- 請求項1−14のいずれかに記載の方法であって、前記バイオマーカーがTNF−α濃度である、方法。
- 請求項1−14のいずれかに記載の方法であって、前記バイオマーカーがIFN−γ濃度である、方法。
- 請求項1−14のいずれかに記載の方法であって、前記バイオマーカーがIL−2濃度である、方法。
- 請求項15または16記載の方法であって、120pg/ml以上である前記IL−17濃度またはIL−17(A)濃度を、グラチラマーアセテートへの反応者として同定される患者と関連付ける、方法。
- 請求項17記載の方法であって、20000pg/ml以上である前記TNF−α濃度を、グラチラマーアセテートへの反応者として同定される患者と関連付ける、方法。
- 請求項18記載の方法であって、6000pg/ml以上である前記IFN−γ濃度を、グラチラマーアセテートへの反応者として同定される患者と関連付ける、方法。
- 請求項19記載の方法であって、30000pg/ml以上である前記IL−2濃度を、グラチラマーアセテートへの反応者として同定される患者と関連付ける、方法。
- 請求項1−23のいずれかに記載の方法であって、前記ヒト患者が未処置の患者である、方法。
- 請求項1−23のいずれかに記載の方法であって、前記ヒト患者が以前にグラチラマーアセテート以外の多発性硬化症薬を投与されていた患者である、方法。
