Abstract

参加者
大学医療センターの治験審査委員会(IRB)は、この試験を承認した。体重≧15kgで5から17歳の13人の男性及び女性外来患者を、この試験に採用した。書かれたインフォームドコンセントは、各参加者の法的保護者(この報告における全ての子供では、親)から得られ、被験者は、可能であれば、同意した。FXSの診断は、少なくとも部分的な遺伝子メチル化を有するFMR1遺伝子における200個を超えるCGG反復配列伸長と合致する、サザンブロット及びPCRの結果によって確認された。被験者は、医療を要する他の目立った医学的状態を有していてはならなかった。グルタミン酸神経伝達に影響を及ぼさないと考えられる向精神剤の併用は、患者がベースラインの前に少なくとも4週間一定の投薬を経たのであれば、許容された。被験者は、Stanford-Binet 5th Editionによって決定して、18か月を超える精神年齢を有することが求められた。さらなる組入れ基準は、少なくとも4のCGI−S(Guy 1976)スコア(「軽度に悪い」)を含んでいた。Diagnostic and Statistical Manual of Mental Disorders, 4th Edition, Text Revision(DSM−IV−TR)による、精神病性障害、双極性障害、又は物質使用障害と診断される被験者は、試験から除外された。さらに、陽性の尿妊娠検査、<30のクレアチニンクリアランス、活動性発作障害、又は医療を要する他の目立った医学的状態を有する被験者は除外された。
10週の、前向き、非盲検試験デザインが、この集団での、あり得る将来の大規模、二重盲検、プラセボ対照試験のための、パイロットデータを収集するために、選択された。
全ての被験者は、スクリーニング及びベースライン診察を受け、その後、10週の非盲検試験期間の間、2週間ごとの追跡診察を受けた。1、3、及び5週の最後に、調査員は、薬物の忍容性を評価し、指示されたように用量調節を行うために、親を呼んだ。全ての被験者は、試験の第1週の間、333mg/日のアカンプロセートを与えられた。次いで、調査員は、最適な臨床反応(1「著明改善」に相当するCGI−I)が起こらず、耐えられない有害作用が生じていなかった場合、1,998mg/日(体重>50kg)又は1,332mg/日(体重<50kg)の最大値まで、試験の最初の6週間に渡って、投薬量を増やした。用量維持フェーズは、最適な投薬量で4週間続いた。
CGI−S評価は、上記の適格性基準の一部として、スクリーニング及びベースラインで行われた。この試験において、評価者は、これらに限らないが、注意欠陥/多動性、社会性障害、コミュニケーション障害、反復行動、過敏性、及び不安を含む、FXSにおいて広く認められる症状の重症度に関して、CGI−Sスコアを与えた。CGI−Sは、1から7(1=正常、全く悪くない;2=境界線上の悪さ;3=軽度に悪い;4=中程度に悪い;5=顕著に悪い;6=重度に悪い;7=最も極端に悪いFXS患者)の尺度で評価された。Autism Diagnostic Observation Schedule(ADOS)(Lord, Rutter et al. 1989)が、相伴うASD診断の可能性を評価するために、ベースラインで行われた。
BDNFのための血液試料は、スクリーニングと試験の10週で採取された。BDNFの分析は、患者の指定を伏せて行われた(処置の前又は後)。約4mlの血液を、EDTAを入れた管に採取した。採取後の30分以内に、血液を、2〜8℃、1000gで15分間遠心した。血漿を採取し、採取した血漿のさらなる遠心を、2〜8℃、1000gで10分間行って、試料から血小板を完全に除去した。全ての血漿試料を、−80℃で保存した。BDNFアッセイは、3連で、全ての試料で同時に行った。血漿BDNFを決定するために、ヒト血漿中に存在するBDNFの検出で正当性を立証された(Grassi-Oliveira et al., 2008)、R&D systems(ミネアポリス、ミネソタ州;米国)によるヒトBDNF ELISAキットを用いる、高感度ELISAに基づく方法を用いた。血漿試料中に存在するBDNFの量(pg/ml)は、被験者の試料と同時に実施された、既知量の純粋なヒトBDNFを用いる標準曲線で得られたpgの値から決定された。
全てのデータは、統計分析のために、IBM SPSS Statistics Professionalに記録された。用いられた全てのアウトカム評価尺度の治療前及び後の平均値のあり得る差は、対応のあるt−検定(paired t-test)を用い、計算された。データが正規性の仮定に反する場合には、ウィルコクソン(Wilcoxan)の符号順位検定を用いて、治療前及び後の平均値のあり得る変化を評価した。効果量(effect size)は、ベースラインからエンドポイントまでの平均変化を、ベースラインでの標準偏差で割ることによって計算した。
体重、脈拍、又は血圧における臨床的に有意な如何なる変化も認められなかった。QTc間隔における変化のないことを含めて、臨床的に有意な如何なる変化もECGでは認められなかった。試験室評価尺度に関しては、脂質、電解質、肝機能検査、又は血球数測定において、臨床的に目立った又はわずかな如何なる変化も認められなかった。
概観
ベータアミロイド前駆体タンパク質(APP)は、シナプスの形成に重要であると思われるタンパク質である。アミロイド生成APP切断経路は、アルツハイマー病に関連する斑点(plaque)の主成分であるアミロイドベータペプチド(Aβ)の産出に導く。非アミロイド生成経路は、神経栄養産物の分泌型APPα(sAPPα)を生じる。自閉症である若年者において、血清全sAPP及びsAPPαの増加によって特徴付けられる、非アミロイド生成経路の活性の増大の可能性が認められた。あたかも実際に、sAPPが、自閉症スペクトラム障害の患者を含む臨床試験における治療反応の、可能性があるマーカー及び予測因子として調べられたかのようである。
試験血漿試料は、採取後、すぐに測定される。必要である場合、血漿の試験試料は、凍結されたが、凍結/解凍サイクルの繰返しは行われなかった。試験試料は、使用直前に低温で解凍され、それらを完全に混合した。必要であれば、血漿試料は、EIA緩衝液により適当に希釈されてもよく、アッセイは、試験試料及び標準について、2連の測定で実施され得る。中性pH範囲の試験試料が用いられ、有機溶媒による汚染を避けるためのステップが取られた。sAPPαレベルの大きさを定めるための標準に関しては、0.78ng/mLから50ng/mLまでの、段階希釈によるEIA緩衝液中の一連のsAPPα標準が調製された。
sAPPレベルのためには、試験血漿試料は、採取の後、すぐに測定されなければならない。試験試料を保存するために、血漿試料を凍結保存し、凍結/解凍サイクルを繰り返さなかった。アッセイの直前に、低温で試験試料を解凍し、それらを完全に混合した。血漿試料は、必要であれば、EIA緩衝液により適当に希釈されるべきである。試験試料及び標準の2連での測定が推奨される。中性pH範囲の試験試料を用い、測定に影響を及ぼし得る有機溶媒の汚染を避けた。sAPPレベルの大きさを定めるための標準に関しては、0.39ng/mLから25ng/mLまでの、段階希釈によるEIA緩衝液中の一連のsAPP標準を調製した。
ASDである若年者におけるアカンプロセートの臨床試験を実施した。1つの研究は、特発性ASDである12人の若年者を登録した。さらに別の試験は、脆弱X症候群(FXS)関連ASDである12人の若年者を登録した。アカンプロセート前及び後の全sAPP及びsAPPαのレベルが、15人の参加者(FXSである9人、特発性ASDである6人)から入手できた。被験者の平均IQは56(範囲は36から96)であった。被験者の最終アカンプロセート投薬量は、1,054mg/日であった。結局、全sAPPは、アカンプロセートの使用により、治療前の平均32.6±38.3ng/mLから、治療後の21.4±32.3ng/mLまで低下した(p=0.01)。sAPPαは、アカンプロセートの使用により、治療前の平均8.4±7.9ng/mLから、治療後の5.5±7.2ng/mLまで低下した(p=0.003)。アカンプロセートによる治療によって誘発された、末梢全sAPP及びsAPPαの低下は、ASDの病理生理学を標的とするための、1つのメカニズムを指し示す。
5〜17歳の12人の若年者が、非盲検試験に登録された。12人の全ての被験者は、サザンブロット及び/又はPCR分析によって、完全なFXS変異であることが確認された。被験者は、さらに、IQ(SB5又はLeiter)、ADI−R、ADOS、Vinelandでスクリーニングされた。
Claims (21)
- ASD又はFXSと診断された患者からの第1の血漿試料を、BDNFに選択的に結合する第1の試薬、sAPPに選択的に結合する第2の試薬、及びsAPPαに結合する試薬に接触させるステップ;
血漿試料におけるBDNF、sAPP、及びsAPPαのレベルを決定するステップ;
少なくとも1種の化合物を患者に投与するステップ;
患者からの第2の血漿試料を、BDNFに選択的に結合する第1の試薬、sAPPに選択的に結合する第2の試薬、及びsAPPαに結合する前記試薬に結合させるステップ;
患者の血漿におけるBDNF、sAPP、及びsAPPαのレベルに変化があるかどうかを決定するステップ;及び
患者の血漿試料で決定したBDNF、sAPP、及びsAPPαのレベルの変化に応じて、患者に投与される化合物の量を調節するステップ
を含む、患者の治療方法。 - 第1の試薬がBDNFに選択的に結合する抗体である、請求項1に記載の方法。
- 第2の試薬がsAPPに選択的に結合する抗体である、請求項1に記載の方法。
- 前記試薬がsAPPαに選択的に結合する抗体である、請求項1に記載の方法。

- BDNFに選択的に結合する少なくとも1種の第1の抗体;
sAPPに選択的に結合する少なくとも1種の第2の抗体;及び
sAPPαに選択的に結合する少なくとも1種の抗体
を含む、患者をモニターするためのシステム。 - 緩衝液、キレート剤、殺菌剤、殺真菌剤、及びブロッキング剤からなる群から選択される少なくとも1種の試薬をさらに含む、請求項8に記載のシステム。
- 特発性又はFXS関連ASDの治療に有用な化合物のスクリーニング方法であって、
ASD又はFXSと診断された患者からの第1の血漿試料を、BDNFに選択的に結合する第1の試薬、sAPPに選択的に結合する第2の試薬、及びsAPPαに結合する試薬に接触させるステップ;
血漿試料におけるBDNF、sAPP、及びsAPPαのレベルを決定するステップ;
少なくとも1種の化合物を患者に投与するステップ;
患者からの第2の血漿試料を、BDNFに選択的に結合する第1の試薬、sAPPに選択的に結合する第2の試薬、及びsAPPαに結合する前記試薬に結合させるステップ;
患者の血漿におけるBDNF、sAPP、及びsAPPαのレベルに変化があるかどうかを決定するステップ;及び
投与した化合物が患者の血漿におけるBDNF、sAPP、及びsAPPαのレベルに変化を引き起こす場合、当該化合物を選択するステップ
