Abstract
Description

1)内因性BBB輸送体を用いる輸送によりASAがBBBを通過するのを可能にするASAの修飾;2)BBB輸送をもたらすのに必要な修飾後のASA活性の保持による全身投与した修飾ASAのCNSへの取り込み量と速度。本明細書に記載の方法および組成物の種々の局面は、介在配列有りまたは無しで、ヒトインスリンレセプターの細胞外ドメインに対する免疫グロブリン(重鎖または軽鎖)と融合したASA(すなわち、ASA活性を有するタンパク質)を含むヒトインスリンレセプター(HIR)抗体(Ab)-ASA融合タンパク質を得、(2)CNSおよび該比活性における取り込みに基づいて融合抗体の治療的有効全身用量を確立することによりこれら因子に対処する。
本明細書で用いている「治療(処置)」または「治療する(こと)」は、治療的利益および/または予防的利益を達成することを含む。治療的利益は、治療する基礎疾患または病状の根絶または改善を意味する。例えば、MLDを有する個体において、治療的利益は、該疾患の進行の部分的または完全な停止、または該疾患の部分的または完全な逆転を含む。また、治療的利益は、患者がまだ該病状に罹患しているかもしれないという事実にもかかわらず、患者に改善が認められるように基礎病状に関連する生理学的症状または精神的症状の1またはそれ以上の根絶または改善により達成される。治療の予防的利益は、病状の予防、病状の進行の遅延(例えばリソゾーム貯蔵障害の進行の遅延)、または病序が発生する可能性の減少を含む。本明細書で用いている「治療する」または「治療」には予防を含む。
血液脳関門
血中のある内因性低分子、例えばグルコースまたはアミノ酸は、水溶性であり、ある種のBBB担体系における担体介在輸送(CMT)によりBBBを透過することができる。例えば、グルコースはGLUT1グルコース輸送体によりCMTを介してBBBを透過する。治療的アミノ酸、例えばL-DOPAを含むアミノ酸は、LAT1大中性アミノ酸輸送体によりCMTを介してBBBを透過する。同様に、血中のある種の内因性高分子、例えば、インスリン、トランスフェリン、インスリン様成長因子、レプチン、または低密度リポタンパク質は、ある種のBBBレセプター系を介するレセプター介在トランスサイトーシス(RMT)によりBBBを透過することができる。例えば、インスリンは、インスリンレセプターによりRMTを介してBBBを透過する。トランスフェリンは、トランスフェリンレセプターによりRMTを介してBBBを透過する。インスリン様成長因子は、インスリン様成長因子レセプターによりRMTを介してBBBを透過する。レプチンは、レプチンレセプターによりRMTを介してBBBを透過する。低密度リポタンパク質は、低密度リポタンパク質レセプターによる輸送を介してBBBを透過することができる。
ASAをCNSに送達するための非侵襲性アプローチの一つは、ASAをインスリンレセプターのECDと選択的に結合する抗体と融合させることである。それにより、BBB上に発現したインスリンレセプターは、BBBを通過するASAの輸送のためのベクターとして働くことができる。ある種のECD特異抗体は、内因性リガンドを模倣し、特異的レセプター系による輸送を介して細胞膜障壁を通過する。そのようなインスリンレセプター抗体は、図1に模式的に記載した分子的「トロイの木馬」または「TH」として働く。ASAそれ自身は、通常、血液脳関門(BBB)を通過しない。しかしながら、ASAがTHと融合すると、該酵素は、脳のBBBおよび脳細胞膜に発現するIRなどの内因性BBBレセプターにより輸送されることにより、BBBおよび脳の細胞膜を通過することができる(図1)。
組換えASAの全身投与(例えば静脈内注射による)は、MLDに罹患した患者のCNS中のASA欠乏を救済することができない。BBBは、末梢投与後に組換えASAがCNSにおいて顕著な治療効果を示すのを妨げる。しかしながら、ASAが(例えば共有結合リンカーにより)HIR Abと融合すると、この酵素は、非侵襲性経路で投与、例えば、静脈内、動脈内、筋肉内、皮下、腹腔内、または経口投与後に血中からCNSに入ることができる。HIR Ab-ASA融合抗体の投与は、末梢血から脳内にASA活性を送達することができる。本明細書は、CNSにおけるASA欠乏を治療するのに治療的に有効なHIR Ab-ASA融合抗体の全身用量の決定について記載する。本明細書に記載するように、HIR Ab-ASA融合抗体の適切な全身用量は、HIR Ab-酵素融合抗体のCNS取り込み特性および酵素活性の定量に基づいて確立される。スルファチドは、中枢神経系の希突起神経膠細胞において主として合成される硫酸化ガラクトシルセラミドである。本明細書で用いている、ASA(例えば、GenBank受託番号NP_000478に記載のヒトASA配列;Swiss-Prot P15289)は、セレブロシド3-サルフェートのセレブロシドとサルフェートへの加水分解を触媒することができるあらゆる天然または人工酵素を表す。
本明細書に記載の二機能性HIR Ab-ASA融合抗体は、その別個の構成タンパク質の活性、すなわち、HIR AbのIR ECDとの結合およびASAの酵素活性、を高い割合で保持することが分かった。本明細書に記載のあらゆるタンパク質をコードするcDNAおよび発現ベクターの構築、およびその発現および精製も当業者のよく理解するところであり、本明細書、例えば実施例1〜3、およびBoado et al(2007)、Biotechnol Bioeng 96:381-391、米国特許出願No. 11/061,956、および米国特許出願No. 11/245,710に詳述されている。
本明細書に記載のHIR Ab-ASA融合抗体において、該抗体とASAの間の共有結合は、HIR Ab-ASA融合抗体がIRのECDと結合し、血液脳関門を通過することができ、ASAがその活性の治療的に有用な部分を保持しているかぎり、HIR抗体のカルボキシまたはアミノ末端とASAのアミノまたはカルボキシ末端でありうる。ある態様において、共有結合は、該抗体のHCとASAの間、または該抗体のLCとASAの間である。以下のあらゆる適切な結合を用いることができる:例えば、軽鎖のカルボキシ末端とASAのアミノ末端、重鎖のカルボキシ末端とASAのアミノ末端、軽鎖のアミノ末端とASAのアミノ末端、重鎖のアミノ末端とASAのアミノ末端、軽鎖のカルボキシ末端とASAのカルボキシ末端、重鎖のカルボキシ末端とASAのカルボキシ末端、軽鎖のアミノ末端とASAのカルボキシ末端、または重鎖のアミノ末端とASAのカルボキシ末端。ある態様において、該結合は、HCのカルボキシ末端とASAのアミノ末端である。
本発明は、本明細書に記載のごとく治療的有効量のHIR Ab-ASA融合抗体を全身投与することによりBBBを通過してCNSに有効量のASAを送達する方法を開示する。HIR Ab-ASA融合抗体を送達するための適切な全身用量は、本明細書に記載の、CNS取り込み特性およびASA比活性に基づく。ASA不足に罹患した対象に対するHIR Ab-ASA融合抗体の全身投与は、CNSにASAを非侵襲的に送達するための有効なアプローチである。
HIR-ASA融合抗体の量、すなわち、HIR Ab-ASA融合抗体の治療的有効全身用量は、一部、本明細書に記載の投与するHIR-ASA融合抗体のCNS取り込み特性、CNSに取り込まれる全身投与した用量の割合に依存する。
ある態様において、医薬的有効全身用量には、少なくとも50、100、200、300、400、500、600、700、800、900、1000、1500、2000、2500単位 ASA活性/脳、または約50〜2500単位ASA活性/脳のあらゆる他の全身用量が含まれる。
実施例
Sly症候群とも呼ばれるMPS-VII において突然変異したリソゾーム酵素はβ-グルクロニダーゼ(GUSB)である。MPS-VIIは、脳のグルコソアミノグリカンの蓄積をもたらす。GUSB酵素はBBBを通過しないので、MPS-VIIの酵素置換療法(ERT)は脳の治療に有効ではないようである。BBBを通過するヒトGUSBを再操作する努力において、HIR Ab-GUSB融合タンパク質プロジェクトを開始した。
HIR AbとGUSBの融合タンパク質を成功裡に生成するために、GUSBシグナルペプチドを含む成熟ヒトGUSBのカルボキシル末端をHIR AbのHCのアミノ末端と融合する新たなアプローチを実施した。この融合タンパク質をGUSB-HIR Abと呼んだ。第一工程は、この新融合タンパク質をコードする新たな発現プラスミドを操作することであり、このプラスミドをpCD-GUSB-HCと呼んだ。pCD-GUSB-HCプラスミドは、19アミノ酸シグナルペプチドを欠くHIRMAbの重鎖(HC)のアミノ末端を、22アミノ酸 GUSBシグナルペプチドを含み18アミノ酸 カルボキシル末端 GUSBプロペプチドを欠くヒトGUSBのカルボキシル末端と融合させた融合タンパク質を発現する。pCD-GUSBベクターを、22アミノ酸 GUSBシグナルペプチドを含みGUSB カルボキシル末端に18アミノ酸プロペプチドを欠くGUSBタンパク質を発現するGUSB cDNAのPCR増幅のテンプレートに用いた。pCD-GUSB中のGUSB 18アミノ酸カルボキシル末端プロペプチドを、部位指向性突然変異誘発(SDM)により除去した。後者は、GUSBのThr633残基の3’隣接領域のAfeI部位を生じ、pCD-GUSB-AfeIと呼んだ。次に、カルボキシル末端プロペプチドをAfeI およびHindIII(GUSBの3’-非コーディング領域に位置する)で除去した。19アミノ酸 IgGシグナルペプチドを欠き、HIRMAb HC終止コドンを含むHIRMAb HCオープンリーディングフレームを、HIRMAb HC cDNAをテンプレートに用いるPCRにより作製した。PCRで作製したHIRMAb HC cDNAをpCD-GUSB-AfeIのAfeI-HindIII部位に挿入しpCD-GUSB-HCを形成した。GUSBのカルボキシル末端とHIRMAb HCのアミノ末端の間のSer-Serリンカーを、HIRMAb HC cDNAのクローニングに用いるためのPCRプライマーによりAfeI部位内に導入した。pCD-GUSB-HC発現カセットのDNAシーケンシングは、22アミノ酸 GUSBシグナルペプチド、611アミノ酸 GUSB、2アミノ酸リンカー(Ser-Ser)、および443アミノ酸 HIRMAb HCからなる1,078アミノ酸タンパク質を発現するプラスミドを示した。GUSB配列は、ヒトGUSB(NP_000172)のMet1-Thr633と100%同一であった。
異染性白質ジストロフィー(MLD)において突然変異したリソゾーム酵素は、アリールスルファターゼA(ASA)である。MLDは、脳中、特にミエリン鞘中のスルファチドの蓄積をもたらす。ASA酵素はBBBを通過しないので、MLDの酵素置換療法は脳の治療に効果がないようである。ASAをHIR Abと融合し、BBBを通過し、酵素活性を示すことができる二機能性分子を開発した。ある態様において、成熟ASAのアミノ末端をHIR Abの各重鎖のカルボキシル末端と融合する(図2)。

チャイニーズハムスター卵巣(CHO)細胞を、1 x HT添加物(ヒポキサンチンおよびチミジン)を含む無血清HyQ SFM4CHOユーティリティ培地(HyClone)中で増殖させた。CHO細胞(5 x 106生細胞)を、5μg PvuI線状化TV-HIRMAb-ASAプラスミドDNAを用いてエレクトロポーレーションした。次に、細胞-DNAサスペンジョンを氷上で10分間インキュベートした。細胞を、CHO細胞用のBioRadプリセットプロトコール、すなわち、15msecおよび160ボルトのパルスの方形波でエレクトロポーレーションした。エレクトロポーレーション後、細胞を氷上で10分間インキュベートした。細胞サスペンジョンを4 x 96ウェルプレートに125 μl/ウェル(10,000細胞/ウェル)で播いた。1試験あたり合計10エレクトロポーレーションを4,000ウェルに実施した。
融合タンパク質のHIR細胞外ドメイン(ECD)に対する親和性はELISAで決定した。HIR ECDで不可逆的にトランスフェクションしたCHO細胞を無血清培地(SFM)中で増殖させ、次いでHIR ECDを、先にColoma et al.(2000) Pharm Res、17:266-274に記載のごとく、小麦胚芽凝集素アフィニティカラムで精製した。HIR ECDをNunc-Maxisorb 96ウェルディッシュに接種し、HIR AbまたはHIR Ab-ASA融合タンパク質のHIR ECDに対する結合を、ビオチン化ヤギ抗ヒトIgG(H+L)二次抗体、次いでアビジンおよびビオチン化パーオキシダーゼ(Vector Labs、Burlingame、CA)を用いて検出した。50%最大結合をもたらすHIR AbまたはHIR Ab-ASA融合タンパク質の濃度、ED50を非線形回帰分析により決定した。HIRに対する結合のED50は35±9ng/mLであり、HIR Ab-ASA融合タンパク質のHIRに対する結合のED50は106±33 ng/mLである(図12)。HIR AbのMWは150kDaであり、HIR Ab-ASA融合タンパク質のMWは288kDaである。したがって、MWの差を正規化後、キメラHIR AbまたはHIR Ab-ASA融合タンパク質のHIR ECDとの結合はそれぞれED50が0.23±0.06 nM および0.34±0.11 nMであり同等であった(図12)。これらの知見は、ASA分子のIgGの両重鎖のカルボキシル末端との融合にも関わらず、HIR Ab-ASA融合タンパク質のHIRとの結合親和性は保持されることを示す。
成熟ヒトASAは、3-アミノ酸リンカーのSer-Ser-SerによりHIR AbのHCのカルボキシル末端と融合する(図9中の下線)。リンカーの種々のバリエーションがSer-Ser-Serリンカーの代わりに用いられる。3-アミノ酸リンカーは保持されるかもしれないが、アミノ酸配列は代わりのアミノ酸に変化する(例えば、Gly-Gly-Gly、またはSer-Gly-Ser、またはAla-Ser-Gly、または20天然アミノ酸の種々の組み合わせ)。すなわち、該リンカーは2、1、または0アミノ酸に減少する。0アミノ酸リンカーの場合は、ASAのアミノ末端は、HIR AbのHCのカルボキシル末端と直接融合する。あるいはまた、該リンカーの長さは、3、4、5、6、7、8、9、10、11、12、13、14、15、または20アミノ酸に拡張される。そのようなリンカーは当該分野でよく知られており、融合タンパク質の操作において最適なアミノ酸リンカーを決定するための多くの公に利用可能なプログラムがある。しばしば用いられるリンカーには、反復配列におけるGlyおよびSerの種々の組み合わせ、例えば(Gly4Ser)3、または他のバリエーションが含まれる。
MLD繊維芽細胞をCoriell Institute for Medical Research(Camden、NJ)から得、Lab-Tekチャンバースライドプレート中で10%FBS含有DMEMを用いて>50%コンフルエントになるまで一夜増殖させた。培地を吸引し、ウェルをPBSで良く洗浄し、次いで、細胞を10ug/mLのHIRMAb-ASA融合タンパク質含有無血清新鮮DMEMで処理した。37℃で24時間インキュベーションした後、培地を吸引し、ウェルを冷PBSで充分に洗浄し、次いで細胞を-20℃で20分間100%冷メタノールで固定した。PBSで洗浄した後、プレートを10%ロバ血清でブロックし、次いで10 ug/mLのヤギ抗ASA抗体およびヒトリソゾーム結合膜タンパク質(LAMP)-1に対するマウスMAb 10 ug/mlで同時標識した。陰性コントロール抗体は、同濃度のヤギまたはマウスIgGであった。二次抗体は、それぞれ5ug/mLのAlexa Fluor-488 結合ロバ抗マウスIgG(緑色チャンネル)およびAlexa Fluor-594結合ロバ抗ヤギIgG(赤色チャンネル)であった。洗浄したスライドを4',6-ジアミジノ-2-フェニルインドール(DAPI)含有Vectashieldマウンティング媒質を用いてマウントした。共焦点顕微鏡検査をLeica TCS SP2 AOBS倒立蛍光顕微鏡を用いて行った。光学切片(1um、解像度300nm)を、各試料のz-平面を通して連続的に得た。細胞内のLAMP1免疫応答性を緑色チャンネルで検出し、ASA免疫応答性を赤色チャンネルで検出する。MLD繊維芽細胞中に豊富なASA免疫応答性がみられ、標的細胞がHIRMAb-ASA融合タンパク質を取り込むことを示す。ASAおよびLAMP1免疫応答性の重複が観察され、HIRMAb-ASA融合タンパク質が該細胞のリソゾームコンパートメントにトリアージ(triage)されることを意味する。アイソタイプコントロール抗体で標識した細胞中には免疫応答性がなく、このことは細胞内LAMP1およびASA免疫応答性が標的タンパク質に特異的であることを示す。
異染性白質ジストロフィーまたはMLDは、リソゾーム酵素、アリールスルファターゼA(ASA)をコードする遺伝子の異常によって生じるリソゾーム蓄積疾患である。ASA非存在下では、オリゴデンドロサイト、ニューロン、およびアストロサイト、および星状膠細胞を含む脳の細胞に、ある種のスルホ糖脂質が蓄積する(Eckhardt M.、The role and metabolism of sulfatide in the nervous system、Mol Neurobiol、37: 93-103、2008)。脳におけるスルファチド糖脂質の蓄積は、歩行困難および運動失調、痙性四肢麻痺、発作、および最終的には除脳状態による死を含むMLDの臨床症状をもたらす。ASA mRNAのヌクレオチド配列およびヒトASAタンパク質のアミノ酸配列は知られている(C. Stein et al、Cloning and expression of human arylsulphatase A、J. Biol. Chem. 264: 1252-1259、1989)。この配列は、MLDの酵素置換療法(ERT)のための組換えASAの製造を可能にする。チャイニーズハムスター卵巣で産生されるASAは、60単位/mg酵素の比活性を有する(U. Matzner et al、Enzyme replacement improves nervous sytem pathology and function in a mouse model for metachromatic leukodystrophy、Human Mol. Genet.、14: 1139-1152、2005)。ASA比活性は、p-ニトロカテコールサルフェート(NCS)分光光度アッセイにより検出される(H. Baum et al、The assay of arylsulphatase A and B in human urine、Clin. Chim. Acta 4: 453-455、1959)(ここで、1単位=1umol/min)(E. Shapira and H.L. Nadler、Purification and some properties of soluble human liver arylsulphatases、Arch. Biochem. Biophys.、170: 179-187、1975)。脳の重度のニューロパシーの治療における組換えASAを用いるMLDのERTの問題は、ASAは他の大分子医薬と同様に、BBBを通過しないことである。MLDマウスに対する大用量40mg/kgのIV投与は、マウス脳における免疫応答性ASAの増加をもたらさず、MLDマウスの脳におけるスルファチド含有量の減少をもたらさない(U. Matzner et al、Enzyme replacement improves nervous system pathology およびfunction in a mouse model for metachromatic leukodystrophy、HUman Mol. Genet.、14: 1139-1152、2005)。BBBを通過するASAの輸送がないために、大用量の該酵素を全身投与後に脳内のASAを増加させることができない。したがって、組換えASAを用いるMLD患者のERTは、脳に対する有益な効果がないことがわかった(C.i. Dali and A.M. Lund、Intravenous enzyme replacement therapy for metachromatic leukodystrophy (MLD)、Abstracts of American College Medical Genetics Annual Meeting、abstract No. 195、2009)。MLDマウスの脳内にASAを直接脳室内(ICV)注入することによりBBBを迂回するため、該酵素を4週間にわたり脳室内に注入した(S. Stroobants et al、Intracerebroventricular enzyme infusion corrects central nervous system pathology and dysfunction in a mouse model of metachromatic leukodystrophy、Human Molec. Genet.、20: 2760-2769、2011)。ICV注入後のASAの脳中組織半減期は<10分間であったが、IV投与後の末梢組織におけるASAの組織半減期は4日間であった(U. Matzner et al、Enzyme replacement improves nervous system pathology およびfunction in a mouse model for metachromatic leukodystrophy、Human Mol. Genet.、14: 1139-1152、2005)。ICV注射は緩徐な静脈内注射と同様であるため、薬剤が脳室コンパートメントから末梢静脈循環に急速に移動するため、ICV注入後の脳からのASAの急速な流出が予期される。それにも関わらず、脳内へのASAの注入は、MLDマウスのリソゾーム蓄積疾患を是正することがわかった。
MLD患者、特に幼児の組換えASAの慢性ICV注入による治療は、脳内カニューレを設置する脳外科的介入が必要であるため問題がある。しかしながら、ICV注入送達によるより大きな制約は、ICV注射後の該酵素の脳実質内への透過が制限されることである。該酵素が脳室腔から末梢血に急速に移動することにより、脳の上衣表面と接触するものを超える脳組織内への該酵素の拡散が制限される。MLD患者の脳へのASAの送達に対する好ましいアプローチは、レセプター介在輸送(RMT)を介してBBBを通過するよう再操作された形のASAの静脈内注入によるものである。HIRMAb-ASA融合タンパク質はヒトインスリンレセプターに対する高親和性結合を保持することにより、スルファターゼがBBBを透過し、内因性BBBインスリンレセプターのRMTを介して血液から脳に入るのを可能にする。HIRMAb-スルファターゼ融合タンパク質の脳への取り込みは、以下に記載するようにアカゲザルの脳100グラム当たり注射用量(ID)の1.1%である。該融合タンパク質の脳への取り込みがこのレベルであれば、50kgのヒトにおける2.5mg/kgのIgG-ASA融合タンパク質の静脈内注射は、1,375ug融合タンパク質/脳の脳濃度をもたらす。ASAは該融合タンパク質の約半分を構成するため、ASA酵素の脳濃度は687ug/脳であり、ヒトの脳は約1000グラムであるから687ng/グラムと等価であり、1グラムの脳は100mgのタンパク質を含むので、6.9ng/mg脳タンパク質と等価である。ヒト脳中の免疫応答性ASA濃度は100ng/mgタンパク質であるから、外来性ASAのこの脳への取り込みレベルは、ヒト脳中のASAの正常濃度の>6%を回復させる(C. Sevin et al、Intracerebral adeno-associated virus-mediated gene transfer in rapidly progressive forms of metachromatic leukodystrophy、Human Molec. Genet.、15: 53-64、2006)。リソゾーム蓄積疾患患者における、正常のわずか1〜2%の細胞酵素活性をもたらす酵素置換療法は、該疾患の兆候および症状を生じない(J. Muenzer and A. Fisher、Advances in the treatment of mucopolysaccharidosis type I、N. Engl J Med、350: 1932-1934、2004)。MLDに関して、人口の約7%がASA偽欠乏症の患者であり、臨床的に正常であるが、正常ASA酵素活性のわずか3%しかない(Penzien JM、et al.(1993) Compound heterozygosity for metachromatic leukodystrophy and arylsulphatase A pseudodeficiency alleles is not associated with progressive neurological disease. Am J Hum Genet 52:557-564)。これらのことは、全身用量2.5mg/kgのHIRMAb-ASA融合タンパク質を静脈内注入後に、ヒト脳の臨床的に重要なASA酵素置換法が可能であることを示す。
Claims (76)
- 医薬的有効量のアリールスルファターゼA活性を有する融合抗体を対象に全身投与することを含む、対象の中枢神経系のアリールスルファターゼA(ASA)欠乏症を治療するのに用いるための融合抗体であって、
(a)免疫グロブリン重鎖およびアリールスルファターゼAのアミノ酸配列を含む融合タンパク質、および(b)免疫グロブリン軽鎖を含み、血液脳関門(BBB)を通過する、融合抗体。 - アリールスルファターゼAのアミノ酸配列が免疫グロブリン重鎖のアミノ酸配列のカルボキシ末端と共有結合している請求項1に用いるための融合抗体。
- スルファターゼ修飾因子タイプ1(SUMF1)により翻訳後修飾される請求項1記載の融合抗体。
- ホルミルグリシンを含む請求項1に用いるための融合抗体。
- 該融合抗体がセレブロシドサルフェートエステルおよびスルファチドスフィンゴ脂質の加水分解を触媒する請求項1記載の方法。
