上気道の流れ制限の兆候に基づいて刺激プロトコルを自動的に選択する選択モジュールとを有し、
刺激プロトコルが、呼吸サイクルの呼気フェーズに略同調して刺激を付与するための第1のプロトコル、および、呼吸サイクルの吸気フェーズの一部分と呼気フェーズの一部分との両方において刺激を付与するための第2のプロトコルの少なくとも1つを含んでいる、刺激プロトコル決定システム。 - 上気道の流れ制限の兆候が、睡眠時無呼吸に関連する上気道の少なくとも部分的な閉塞に対応する、請求項1に記載の刺激プロトコル決定システム。
- 上気道の流れ制限の兆候が主に呼気フェーズと同時に起こる場合、第1のプロトコルが、自動的に選択される、請求項1に記載の刺激プロトコル決定システム。
- 上気道の流れ制限の兆候が主に吸気フェーズの第1の部分と呼気フェーズの第1の部分との両方に起こり、且つ、主に吸気フェーズの第2の部分と呼気フェーズの第2の部分とに起こらない場合、第2のプロトコルが自動的に選択される、請求項1に記載の刺激プロトコル決定システム。
- 吸気フェーズの第1の部分が少なくとも吸気フェーズの末期部を含み、
呼気フェーズの第1の部分が少なくとも呼気フェーズの初期部を含んでいる、請求項4に記載の刺激プロトコル決定システム。 - 吸気フェーズの第1の部分が吸気フェーズの初期部を含んでいない、請求項5に記載の刺激プロトコル決定システム。
- 呼気フェーズの第2の部分が呼気フェーズの末期部を含んでいない、請求項5に記載の刺激プロトコル決定システム。
- 第2のプロトコルが、少なくともいくつかの呼吸サイクルの少なくとも一部分において、略連続する刺激期間を付与することを含み、
略連続する刺激期間が、主に吸気フェーズの第1の部分と呼気フェーズの第1の期間とに起こり、
略連続する刺激期間が、吸気フェーズの始まりより後に位置する最初の開始点と、呼気フェーズの終わりより前に位置する最初の終了点とを含んでいる、請求項5に記載の刺激プロトコル決定システム。 - 第2のプロトコルが、第1の期間にわたる一連の連続する複数の呼吸サイクルにおいて略連続する刺激期間を付与することを含み、
入力モジュールが、
第1の期間内において(機能障害しきい値を超えた)少なくともいくつかの上気道の流れ制限の兆候を受け取ったか否かを判定し、
入力モジュールが第1の期間内において少なくともいくつかの兆候を受け取った場合には、略連続する刺激期間の継続時間を維持するまたは増加し、
入力モジュールが第1の期間内において兆候を受け取らなかった場合には、略連続する刺激期間の継続時間を減少する、ように構成されている、請求項8に記載の刺激プロトコル決定システム。 - 第1の期間が、無呼吸時の呼吸低下の兆候に基づく、刺激がないときに無呼吸が生じる可能性がある期間である、請求項9に記載の刺激プロトコル決定システム。
- 継続時間を減少させることには、
次の一連の連続する呼吸サイクルのために、最初の開始点を吸気フェーズの末期部に接近するように減少的に移動させること、および、
次の一連の連続する呼吸サイクルのために、最初の終了点を呼気フェーズの初期部に接近するように減少的に移動させることの少なくとも一方が含まれている、請求項9に記載の刺激プロトコル決定システム。 - 略連続する刺激期間の第1の部分が、吸気フェーズの第1の部分に一致するとともに、吸気フェーズ全体の少なくとも3分の1の継続時間、吸気フェーズ全体の少なくとも2分の1の継続時間、および吸気フェーズ全体の少なくとも3分の2の継続時間の少なくとも1つを有する、請求項8に記載の刺激プロトコル決定システム。
- 略連続する刺激期間の第2の部分が、呼気フェーズの第1の部分に一致するとともに、呼気フェーズ全体の少なくとも3分の1の継続時間、呼気フェーズ全体の少なくとも2分の1の継続時間、および呼気フェーズ全体の少なくとも3分の2の継続時間の少なくとも1つを有する、請求項8に記載の刺激プロトコル決定システム。
- 吸気フェーズの第1の部分が呼気フェーズの少なくとも大部分に相当し、

- 埋め込み可能なパルス発生器と、
第1および第2の刺激プロトコルそれぞれを付与するためにパルス発生器に電気的に接続可能な神経カフ電極とを有する、請求項1に記載の刺激プロトコル決定システム。 - 神経が舌下神経である、請求項1に記載の刺激プロトコル決定システム。
- 呼吸努力情報を検出する呼吸センサを有する、請求項16に記載の刺激プロトコル決定システム。
- 刺激プロトコル決定モジュールに提供される機械可読な指示を蓄えるメディアを読み出し可能な持続性コンピュータであって、
検出された呼吸努力情報を介して上気道の流れ制限の兆候を受け取る入力モジュールと、
上気道の流れ制限の兆候に基づいて刺激プロトコルを自動的に選択する選択モジュールとを有し、
刺激プロトコルが、呼吸サイクルの呼気フェーズに略同調して刺激を付与するための第1のプロトコル、および、呼吸サイクルの吸気フェーズの一部分と呼気フェーズの一部分との両方において刺激を付与するための第2のプロトコルの少なくとも1つを含んでいる、刺激プロトコル決定システム。 - 刺激プロトコル決定システムであって、
入力モジュールおよび選択モジュールに提供される機械可読な指示を蓄えるための不揮発性メモリと、
機械可読な指示を実行するプロセッサとを有し、
入力モジュールが検出された呼吸努力情報を介して上気道の流れ制限の兆候を受け取り、
選択モジュールが上気道の流れ制限の兆候に基づいて刺激プロトコルを自動的に選択し、
刺激プロトコルが、呼吸サイクルの呼気フェーズに略同調して刺激を付与するための第1のプロトコル、および、呼吸サイクルの吸気フェーズの一部分と呼気フェーズの一部分との両方において刺激を付与するための第2のプロトコルの少なくとも1つを含んでいる、刺激プロトコル決定システム。
