本発明のフォームは、好ましくは、生理学的機能(呼吸または心拍出量など)に干渉をしないように軟質性であり、容易に圧縮される。例えば、好ましい実施形態では、該フォームは、インプラントを形成するために腹部に配置したとき、下大静脈を通って戻る静脈血に干渉しないように十分に軟質性である。好ましい実施形態では、該フォームは、25kPa未満のCFD値を特徴とし、65%圧縮されるのに必要とされるのは60mJ未満である。25kPAより大きいが60kPA未満のCFD値を有するフォームも同様に本発明において有用であり得る。
本発明のフォームは、出血部位と接触すると止血を促進させる。好ましい実施形態では、本発明のフォームは、血液が該フォームに進入することは許容するが血流に対して抵抗をもたらす特性(例えば、大きさ、形態構造、および屈曲度)を有する気泡および細孔構造を有するものである。一般に、小さな創傷は凝血する傾向にあり、ホメオスタシスが迅速かつ確実になされるが、大きな創傷はそうでない。大きな創傷からの大量の流量によって、生成中の血塊が破壊され、活性化凝固因子が有効濃度未満に希釈されることにより凝血が阻害されると考えられ、大きな創傷では、血塊がより大きなサイズに達しなければならない。理論に拘束されることを望まないが、本発明のフォームは、「大きな創傷のダイナミクス」を、通常どおりに凝血し、したがって止血が誘導され得る多くの小さな創傷のものに変化させることにより、止血を誘導し得る。理論に拘束されることを望まないが、止血は、本発明のフォームによりいくつかの機構によって誘導されると考えられる。第1に、血流を低減させることにより、該フォームによって凝血が補助され、安定な血塊が形成されることが可能となり得る。流動抵抗に加え、該フォームによって、表面ファウリング(fouling)、血小板および細胞の結合および活性化、ならびに凝血カスケードの開始のための大きなポリマー表面積がもたらされ得る。止血を助長するための該フォームの好ましい特性としては、0.01〜1mmの孔径を有する連続気泡構造、高度膨張(10倍より大きい、または6.2pcf未満のフォーム密度)、および大きな表面積対体積比が挙げられる。最後に、その構造内にある程度の血流が許容されるフォームが得られることのさらなる利点は、圧力が低減されること、およびフォームと損傷部位との間に作り出され得る任意のシール部に加えられる力が、ある程度の血流が許容されないフォームと比べて小さいことである。

(項目6) 上記ポリオールが、ポリプロピレングリコール、ポリエチレングリコール、ポリカーボネート、ポリブタジエン、ポリエステルならびにそのコポリマーおよびブレンドからなる群より選択される、項目1に記載のシステム。
(項目7) 上記イソシアネートが、ヘキサメチレンジイソシアネート(HDI)、トルエンジイソシアネート(TDI)、メチレンジフェニルジイソシアネート(MDI)、ポリマーMDI、およびMDI異性体の混合物のうちの1つである、項目1に記載のシステム。

多官能性イソシアネートを含む第2の組成物を内包する第2のチャンバ;ならびに
該第1の組成物を該第2の組成物と接触させて、ポリウレタンフォームを形成する機構
を備えており;
該フォームは、身体組織と接触させられたときに、出血または体液の流れを抑制、制限またはコントロールするように構成される、

Claims (21)
- 体液移動を制限するための医療用インプラントを形成するためのシステムであって、
ポリオールを含む第1の組成物を内包する第1のチャンバ;
多官能性イソシアネートを含む第2の組成物を内包する第2のチャンバ;および
該第1の組成物を該第2の組成物と接触させて、ポリウレタンフォームを形成する機構
を備える、システム。 - 前記第1の組成物が50重量パーセントまでのポリエチレンオキシドを含む、請求項1に記載のシステム。
- 前記第1の組成物が25重量パーセントまでのポリエチレンオキシドを含む、請求項2に記載のシステム。
- 前記第1の組成物が20pphpまでの水を含む、請求項1に記載のシステム。
- 前記第1の組成物が10pphpまでの量のアミン触媒を含む、請求項1に記載のシステム。
- 前記ポリオールが、ポリプロピレングリコール、ポリエチレングリコール、ポリカーボネート、ポリブタジエン、ポリエステルならびにそのコポリマーおよびブレンドからなる群より選択される、請求項1に記載のシステム。
