Abstract
【解決手段】本発明は、補体関連型眼疾患(本明細書において眼障害ともいう)の治療、予防および/または安定化のための材料および方法を提供する。本発明の一部のある実施形態において、抗補体アプタマーは、補体成分またはそのバリアントの機能をモジュレートする。特に好ましい実施形態では、抗補体アプタマーは、補体成分またはそのバリアントの機能を、好ましくは生体内で、好ましくは脊椎動物において、好ましくは哺乳動物において、より好ましくはヒトの生体内において阻害するか、または低下させる。

Description
本特許出願は、米国仮特許出願第60/780,905号(2006年3月8日出願)および米国仮特許出願第60/848,274号(2006年9月29日出願)(これらの各々は、その全体が本明細書中に参考として援用される)に基づく優先権を主張する。
本発明は、一般的に核酸の分野に関し、より詳しくは、補体関連型の眼科系、心臓系、炎症性、喘息性および自己免疫の障害、虚血性再灌流障害および/または特にC5媒介性補体活性化が関与している他の疾患もしくは障害における治療剤および診断剤として有用な補体系のタンパク質に対する結合能を有するアプタマーに関する。好ましい実施形態において、本発明は、より詳しくは、眼障害の治療および検出のための、例えばこれに限定されないが、C5媒介性障害(C5媒介性眼障害など)の治療および検出のための方法および材料に関する。本発明は、さらに、補体系のタンパク質(例えば、C5タンパク質)に対する結合能を有するアプタマーの投与のための材料および方法に関する。
アプタマーは、定義によると、高い特異性および親和性でタンパク質などの一部の標的に、ワトソン−クリック塩基対合以外の相互作用によって結合する単離された核酸分子である。アプタマーは、核酸系分子であるが、アプタマーと遺伝子やmRNAなどの他の核酸分子との間には基本的な違いが存在する。後者の場合、その核酸構造は、その線形の塩基配列によって情報をコードし、したがって、この配列は情報蓄積機能に重要である。全く対照的に、アプタマーの機能は、具体的な標的分子の結合に基づき、保存された線形の塩基配列ではなく、特定の二次/三次構造に依存性である。すなわち、アプタマーは非コード配列である。アプタマーが有し得るいかなるコード可能性も全く偶発的なものであり、アプタマーのそのコグネイト標的に対する結合において何の役割も果たさない。したがって、同じ標的、および該標的上の同じ部位にすら結合するアプタマーが類似した線形の塩基配列を共有することはあり得るが、ほとんどは共有していない。
1)速度および制御。アプタマーは、完全に試験管内プロセスによって作製され、最初のリード化合物(例えば、治療剤リード化合物)の速やかな作製が可能になる。試験管内選択によって、アプタマーの特異性および親和性が厳重に制御されることが可能となり、リード化合物、例えば、毒性および非免疫原性標的の両方に対するリード化合物の作製が可能になる。
補体系は、健康の維持において重要な役割を有するが、疾患を引き起こす可能性または疾患に寄与する可能性を有する。例えば、補体系は、冠動脈バイパス移植術(「CABG」)手術、数多くの腎臓系、リウマチ性、神経科系、皮膚科系、血液系、血管/肺、アレルギー、感染症ならびに生体適合性/ショック疾患および/または状態に関する副作用に関与している。補体系は、必ずしも疾患状態の唯一の原因ではないが、病因に寄与するいくつかの因子の1つであり得る。
本発明は、補体関連型眼疾患(本明細書において眼障害ともいう)の治療、予防および/または安定化のための材料および方法を提供する。
一実施形態において、本発明の方法は、補体媒介性眼障害の処置、安定化および/または予防に関し、該方法は、治療有効量の抗補体アプタマーを、それを必要とする被験体に投与する工程を含む。一実施形態において、処置される眼障害は黄斑変性である。一実施形態において、処置される眼障害は新生血管形成眼障害である。
本発明は例えば、以下の項目を提供する:

補体媒介性眼障害を処置、安定化および/または予防する方法であって、該方法が、治療有効量の抗補体アプタマーを、それを必要とする被験体に投与する工程を含む、方法。

最近のデータにより、加齢性黄斑変性(AMD)は炎症媒介性疾患でもあり、補体活性化がある役割を果たしていることが示されている。加齢性黄斑変性(「AMD」)は、慢性で進行性の目の疾患であり、米国、欧州および日本において、回復不可能な失明の主な原因となっている。AMDは、黄斑と呼ばれる網膜の中心部分の進行性の変質を特徴とする。AMDへの進行の最も明白な指標は、網膜下の黄白色の沈着物であって、網膜細胞の代謝性老廃物由来物質の斑であるドルーゼンの出現である。ドルーゼンの出現は、両方の形態のAMD:滲出性(「湿性」)および非滲出性(「乾性」)に重要な成分である。湿性AMDは、網膜直下での新たな血管成長が、ブルーフ膜として知られる膜を貫通して網膜層内に進入した場合に起こる。この異常な血管成長は、一般的に、新脈管形成または(脈絡膜)新生血管形成と呼ばれている。このような新たな血管は、脆弱である傾向にあり、多くの場合、出血して液体が黄斑内に漏出し、場合によっては突発性の、多くの場合で重症な視野の断裂がもたらされる。新たな治療(例えば、LucentisTM)によって、新脈管形成を停止させ、液体の蓄積を逆転させることができ、少数の患者においては視野を回復させることすらできるが、血管新生病変は、多くの場合、網膜細胞に対して瘢痕化および/または損傷をもたらし、永久的な失明が引き起こされる。湿性AMDは、一般的に、補体タンパク質(例えば、補体H因子(CFH))を含有するドルーゼンの発生と蓄積が先に起こる。多数の中型から大型のドルーゼンの存在は、地図状萎縮および/または新生血管形成を特徴とする後期疾患への進行の最大リスクと関連している。湿性AMDを有する患者の大部分は、該疾患の診断後、数ヶ月から2年以内に罹患した目が重症な失明に陥るが、失明は、数時間または数日以内に起こることもあり得る。乾性AMDは、より緩徐であり、黄斑内の光感受性細胞がゆっくりと萎縮している場合に起こり、罹患した目の中心視野が徐々にぼやけてくる。失明は、ドルーゼンの形成および蓄積によって悪化し、場合によっては、網膜が変質するが、異常な血管成長および出血はない。
補体媒介性眼障害の症状の治療、安定化、予防および/または低減における使用のためのC5特異的アプタマーは、SELEXTM法によって作製され得る。特別な実施形態では、本発明は、抗C5アプタマー剤を被験体に、眼障害の少なくとも1つの症状、特に、糖尿病性網膜症、滲出性および/または非滲出性AMDの症状を低減、安定化および/または予防する方法において投与することを含むものである。
アプタマーの作製に適した方法(一般的に図2に示す)は、「試験管内人工進化(Systematic Evolution of Ligands by Exponential Enrichment)」(「SELEXTM」)と称される方法を用いるものである。SELEXTMプロセスは、標的分子に対して高度に特異的な結合性を有する核酸分子の試験管内進化のための方法であり、例えば、米国特許出願第07/536,428号(1990年6月11日出願、現在は放棄されている)、米国特許第5,475,096号(発明の名称「Nucleic Acid Ligands」)、および米国特許第5,270,163号(また、WO91/19813も参照)(発明の名称「Nucleic Acid Ligands」)に記載されている。選択および増幅の反復サイクルを行なうことにより、SELEXTMを用いて任意の所望のレベルの目標の結合親和性を有するアプタマーが得られ得、これを、本明細書では「核酸リガンド」ともいう。
アプタマーが治療剤または診断剤としての使用に適するようにするため、生体内で安全で安定的に合成することが安価であることが好ましい。野生型RNAおよびDNAアプタマーは、ヌクレアーゼによって分解されやすいため、典型的には生体内で安定的ではない。ヌクレアーゼ分解に対する抵抗性は、2’位における修飾基の組込みによって大きく増大され得る。
HEPESバッファー濃度は0〜1Mの範囲であり得る。本発明ではまた、5〜10のpKaを有する他の緩衝剤、例えば、トリス(ヒドロキシメチル)アミノメタンなどの使用が想定される。
所望の標的に結合するアプタマーが特定されたら、任意選択で、特性されたアプタマー配列の結合特性および/または機能的特性を増大させるために、いくつかの手法が実施され得る。SELEXTMプロセス(2’修飾SELEXTMを含む)によって特定された所望の標的に結合するアプタマーは、所望の結合特性および/または機能的特性を有する最小アプタマー配列(本明細書において、「最小化構築物」ともいう)を得るために、任意選択で切断され得る。これを達成する方法の一例は、最小化構築物の設計の情報を得るためにフォールディングプログラムおよび配列解析(例えば、選択物から得られたクローン配列をアラインメントし、保存モチーフおよび/または共変動を調べる)を使用することによるものである。また、生化学的プロービング実験を行ない、アプタマー配列の5’および3’境を調べ、最小化構築物の設計の情報を得ることができる。次いで、最小化構築物を化学的に合成し、該構築物を誘導した非最小化配列と比較したときの結合特性および機能的特性について試験することができる。一連の5’、3’および/または内部欠失を含むアプタマー配列のバリアントもまた、直接化学的に合成され、該バリアントを誘導した非最小化アプタマー配列と比較したときの結合特性および/または機能的特性について試験され得る。
(1)既に体内に存在する置換基、例えば、2’−デオキシ、2’−リボ、2’−O−メチルプリンもしくはピリミジンまたは5−メチルシトシン
(2)既に承認された治療剤の一部である置換基、例えば、ホスホロチオエート結合オリゴヌクレオチド
(3)上記の2つのカテゴリーのうちの一方に加水分解または分解される置換基、例えば、メチルホスホネート結合オリゴヌクレオチド
の1つに含まれる置換を導入することが望ましいことがあり得る。本発明のアプタマーとしては、本明細書に記載のアプタマー医化学によって開発されたアプタマーが挙げられる。
アプタマーを含むすべてのオリゴヌクレオチド系治療剤の薬物動態特性が所望の医薬適用と適合するように調整することは重要である。細胞外標的に指向されるアプタマーは、細胞内送達に伴う問題点(アンチセンスおよびRNAi系治療剤の場合のような)はもたないが、かかるアプタマーは、やはり、標的の器官および組織に分布され、体内に(非修飾状態で)所望の投与レジメンに整合する期間滞留され得るものでなければならない。
上記のように、高分子量非免疫原性ポリマーでの核酸の誘導体化は、核酸の薬物動態および薬力学的特性を改変するより有効な治療用薬剤とする潜在性を有する。活性における好ましい変化は、ヌクレアーゼによる分解に対する抵抗性の増大、腎臓を介する濾過の低減、免疫系に対する曝露の低減、および身体中への治療剤の分布の改変を誘導し得るものである。
高分子量PEG(>10kDa)の作製は、困難で、非効率的および高価であり得る。高分子量PEG−核酸コンジュゲートの合成に対する経路として、これまでの研究は、より高分子量の活性化PEGの作製に集中されていた。かかる分子を作製するための方法は、2つ以上のPEGが、活性化された基を有する中心コアに結合された分枝鎖活性化PEGの形成を伴う。このような高分子量PEG分子の末端部分、すなわち、比較的非反応性のヒドロキシル(−OH)部分は、1つ以上のPEGを他の化合物に該化合物の反応性部位において結合させるために、活性化され得るか、または官能性部分に変換され得る。分枝鎖の活性化PEGは2つより多くの末端を有し、2つ以上の末端が活性化されている場合、かかる活性化された高分子量PEG分子を、本明細書において多重活性化PEGという。一部のある場合では、分枝鎖PEG分子内のすべての末端が活性化されているわけではない。分枝鎖PEG分子内の任意の2つの末端が活性化されている場合、かかるPEG分子を二重活性化PEGという。分枝鎖PEG分子の1つの末端だけが活性化されている一部のある場合、かかるPEG分子を単一活性化されているという。このアプローチの一例として、リシンコアへの2つのモノメトキシPEGの結合によって調製、続いて、反応のために活性化された活性化PEGが報告されている(Harrisら、Nature、vol.2:214−221、2003年)。
最も一般的には、高分子量PAG−核酸コンジュゲートの合成は、5’末端への遊離第1級アミンの付加によってなされている(固相合成の最後のカップリング工程で、修飾ホスホロアミダイトを用いて組み込まれる)。このアプローチを用い、反応性PEG(例えば、アミンと反応して結合を形成するように活性化されたもの)を、精製オリゴヌクレオチドと合わせ、カップリング反応を溶液中で行なう。
本発明はまた、2つ以上の核酸部分が少なくとも1つのポリアルキレングリコール部分に共有結合によりコンジュゲートされた高分子量アプタマー組成物を包含する。ポリアルキレングリコール部分は安定化部分としての機能を果たす。安定化部分は、本発明の高分子量アプタマー組成物の薬物動態および薬力学的特性を改善する分子または分子の一部分である。一部のある場合では、安定化部分は、2つ以上のアプタマーもしくはアプタマードメインを近接させるか、または本発明の高分子量アプタマー組成物の全体的な回転自由度の低減をもたらす分子または分子の一部分である。安定化部分は、ポリアルキレングリコール、例えば、ポリエチレングリコールであり得、線状または分枝鎖のホモポリマーまたはヘテロポリマーであり得る。他の安定化部分としては、ペプチド核酸(PNA)などのポリマーが挙げられる。オリゴヌクレオチドもまた安定化部分であり得、かかるオリゴヌクレオチドとしては、修飾ヌクレオチドおよび/または修飾された連結部、例えば、ホスホロチオエートなどが挙げられ得る。
一部のある実施形態において、本発明の材料には、生体内および/または細胞系アッセイにおいて、補体タンパク質C5に特異的に結合し、生体内および/または細胞系 アッセイにおいて補体タンパク質C5の活性を機能的にモジュレート(例えば、ブロック)する約15〜約60ヌクレオチド長の一連の核酸アプタマーが包含される。
本発明の抗C5剤には、補体タンパク質C5に指向されるアンタゴニスト抗体およびC5媒介性眼障害の治療におけるその使用が包含される。本発明のC5アンタゴニスト抗体は、C5に強固に結合し、その活性化および切断を抑制する。特別な実施形態において、本発明は、抗C5抗体剤を被験体に、眼障害の少なくとも1つの症状、特に、糖尿病性網膜症、滲出性および/または非滲出性AMDの症状を低減、安定化および/または予防する方法において投与することを含むものである。
本発明の抗C5剤には、C5に対して標的化され、メッセンジャーRNAからのタンパク質翻訳を阻害することにより、または対応するC5m RNAの分解を標的化することによりC5阻害効果を発揮するアンチセンスオリゴヌクレオチドおよびリボザイムが包含される。眼障害を処置する方法における抗C5アンチセンスおよびリボザイム剤の使用もまた提供する。特別な実施形態において、本発明は、抗C5アンチセンスまたはリボザイム剤を被験体に、眼障害の少なくとも1つの症状、特に、糖尿病性網膜症、滲出性および/または非滲出性AMDの症状を低減、安定化および/または予防する方法において投与することを含むものである。
本発明の一部のある実施形態は、RNA干渉(RNAi)によってC5の抑制を行なうための材料および方法を利用するものである。したがって、本発明の抗C5剤には、抗C5 RNAi剤が包含される。本発明は、本発明の眼障害を処置する方法における抗C5 RNAi剤の使用を包含する。特別な実施形態において、本発明は、抗C5 RNAi剤を被験体に、眼障害の少なくとも1つの症状、特に、糖尿病性網膜症、滲出性および/または非滲出性AMDの症状を低減、安定化および/または予防する方法において投与することを含むものである。
本発明の一部のある実施形態において、抗C5剤またはタンパク質またはポリペプチドである。本発明は、眼障害を処置する方法における抗C5タンパク質またはポリペプチド剤の使用を包含する。特別な実施形態において、本発明は、抗C5タンパク質またはポリペプチド剤を被験体に、眼障害の少なくとも1つの症状、特に、糖尿病性網膜症、滲出性および/または非滲出性AMDの症状を低減、安定化および/または予防する方法において投与することを含むものである。
本発明の一部のある実施形態において、抗C5剤は、小分子、特に有機小分子である。本発明は、眼障害を処置する方法における小分子抗C5剤の使用を包含する。特別な実施形態において、本発明は、小分子抗C5剤を被験体に、眼障害の少なくとも1つの症状、特に、糖尿病性網膜症、滲出性および/または非滲出性AMDの症状を低減、安定化および/または予防する方法において投与することを含むものである。
一部のある実施形態において、本発明の材料には、補体タンパク質C3に高い特異性で結合し、生体内および/または細胞系アッセイにおいて補体タンパク質C3活性を機能的にモジュレート(例えば、ブロック)する一連の核酸アプタマーが包含される。このようなアプタマーは、本発明の方法におけるさまざまな補体関連型眼疾患または障害、例えば、急性もしくは慢性炎症性および/または免疫媒介性眼障害、炎症性結膜炎(例えば、アレルギー性結膜炎および巨大乳頭性結膜炎)、黄斑浮腫、ブドウ膜炎、眼内炎、強膜炎、角膜の潰瘍、ドライアイ症候群、緑内障、虚血性網膜疾患、角膜移植片拒絶、眼内レンズ移植などの眼内手術に関連する合併症および白内障手術に伴う炎症、ベーチェット病、免疫複合体脈管炎、フックス病、フォークト−小柳−原田病、網膜下線維症、角膜炎、硝子体網膜炎症、眼の寄生虫外寄生/移動、色素性網膜炎、サイトメガロウイルス網膜炎および脈絡膜の炎症、黄斑変性、加齢性黄斑変性(「AMD」)、非滲出性(「乾性」)の型のAMD、または新生血管形成眼障害(例えば、糖尿病性網膜症もしくは滲出性(「湿性」)の型のAMDなどの処置、安定化および/または予防の低毒性で安全かつ有効なモダリティをもたらす。また、このようなアプタマーは眼科用診断剤においても使用され得る。
一部のある実施形態において、本発明の材料には、補体タンパク質C1qに高い特異性で結合し、生体内および/または細胞系アッセイにおいて補体タンパク質C1qの活性を機能的にモジュレート(例えば、ブロック)する一連の核酸アプタマーが包含される。
本発明はまた、抗C5剤を含有する医薬組成物、特に、補体タンパク質C5に結合するアプタマー分子を含有する医薬組成物、特に、補体タンパク質C5に結合してその切断を抑制するアプタマーを含有する医薬組成物を包含する。一部のある実施形態において、該組成物は内服に適したものであり、有効量の本発明の薬理学的に活性な化合物を、単独または1種類以上の薬学的に許容され得る担体との組合せで含む。該化合物は、毒性があったとしても非常に低いという点で特に有用である。
血管新生障害
血管新生障害、例えばAMD、特に滲出型AMDまたは糖尿病性網膜症の治療の有効性は、新脈管形成が遅滞または減弱されているか否かを調べる一般に認められた任意の測定方法によって評価される。これには、直接観察および間接評価(例えば、自覚症状または客観的な生理学的徴候を評価することによるものなど)が含まれる。例えば、治療の有効性は、新生血管形成、微小血管症、血管漏出もしくは血管浮腫またはその任意の組合せの予防、安定化および/または逆転に基づいて評価され得る。また、血管新生眼障害の抑制を評価するための治療の有効性は、視力の安定化または改善の観点から規定されるものであり得る。
炎症性結膜炎(例えば、アレルギー性結膜炎および巨大乳頭性結膜炎)、黄斑浮腫、ブドウ膜炎、眼内炎、強膜炎、角膜の潰瘍、ドライアイ症候群、緑内障、虚血性網膜疾患、糖尿病性網膜症、角膜移植片拒絶、眼内レンズ移植などの眼内手術に関連する合併症および白内障手術に伴う炎症、ベーチェット病、シュタルガルト病、免疫複合体脈管炎、フックス病、フォークト−小柳−原田病、網膜下線維症、角膜炎、硝子体網膜炎症、眼の寄生虫外寄生/移動、色素性網膜炎、サイトメガロウイルス網膜炎および脈絡膜の炎症の治療は、当該技術分野で一般に認められた方法によって評価される。眼障害の症状の安定化、低減および/または予防における抗補体アプタマー単独または別の薬剤との組み合わせの有効性の測定において、患者は、眼科医によって臨床的に評価されることもあり得る。臨床評価は、注射の数日後および次の注射の直前に行なわれ得る。臨床評価には、直接観察および間接評価(例えば、自覚症状または客観的な生理学的徴候を評価することによるものなど)が含まれ得る。例えば、治療の有効性は、血管漏出もしくは血管浮腫またはその任意の組合せの予防、安定化および/または逆転に基づいて評価され得る。緑内障の場合、治療の有効性は、眼底写真撮影または光コヒーレンストモグラフィーを用いてモニターされ得る網膜神経線維層または視神経の健常性の安定化に関して評価され得る。
古典的および第二補体経路における抗C5アプタマー活性
実施例1A:溶血アッセイ
CH50試験により、抗体コートヒツジ赤血球の標準化された懸濁液において、補体系が血清試験試料中で細胞の50%を溶解する能力が測定される。0.2%ヒト血清の溶液を抗体コートヒツジ赤血球(Diamedix EZ Complement CH50 Kit、Diamedix Corp.、Miami、FL)と、種々の抗C5アプタマーの存在下または非存在下で混合した。アッセイは、キットプロトコルに従って、カルシウム、マグネシウムおよび1%ゼラチンを含有するベロナール緩衝生理食塩水(GVB++補体バッファー)中で行ない、37℃で30分間インキュベートした。インキュベーション後、試料を遠心分離し、インタクトな赤血球をペレット化した。上清みの412nmにおける光学密度(OD412)を読み取り、可溶性ヘモグロビンの放出(これは、溶血の程度に比例する)を定量した(Greenら,(1995)Chem.Biol.2:683−95)。アプタマーがC5活性化をブロックしたことを確認するため、一部の溶血上清みをC5aおよびC5b−9の存在について、ELISA(C5b−9 ELISA kit,Quidel、San Diego、CA;C5a ELISA kit,BD Biosciences、San Diego、CA)によって、ELISAキットプロトコルに従って解析した。
補体系の第二経路に対する抗C5アプタマーの効果を、以下の全血アッセイを用いて解析した。抗凝固剤の非存在下、血液を正常ヒト志願者から採取した。血液のアリコート(抗凝固剤を含有しない)を、漸増濃度のARC186(配列番号4)とともに5時間、室温または37℃でインキュベートした。試料を遠心分離して血清を単離し、血清中のC5bの存在を、sC5b−9 ELISA(C5b−9 ELISAキット、Quidel、San Diego、CA)によって検出した。図15に示されるように、異なる温度でインキュベートされた試料間の抗補体活性(C5b−9の生成に反映される)は、3μMで分岐した。室温でのデータにより、定量的阻害に必要とされるアプタマーの濃度は、C5の既報の濃度はおよそ400nMであるが、3〜6μMの範囲内であるがことが示された。この結果は、10倍モル過剰より多くの抗C5アプタマー(ARC186;配列番号4)が、完全なC5の阻害活性に必要とされ得ることを示す。
ザイモサンは、酵母細胞壁の多糖成分であり、第二補体カスケードの強力な活性化剤である。血液、血漿または血清のエキソビボ試料へのザイモサンの添加により、補体活性化産物、例えば、C5aおよびC5b−9の可溶性型(sC5b−9)の蓄積がもたらされる。非希釈ヒト血清(Center for Blood Research、Boston、MA)、クエン酸塩添加ヒト全血(Center for Blood Research、Boston、MA)またはカニクイザル血清(Charles River Labs、Wilmington、MA)の試料に、漸増濃度のARC658(配列番号62)(ARC186(配列番号4)の30K PEG類縁体)を添加した。補体を活性化するため、ザイモサン(Sigma、St.Louis、MO)含有10×懸濁液を試料に最終濃度5mg/mLまで添加した。15分間37℃でのインキュベーション後、ザイモサン粒子を遠心分離によって除去し、補体活性化の程度をC5aおよび/またはsC5b−9 ELISA(C5b−9 ELISA kit、Quidel、San Diego、CA;C5a ELISAキット、BD Biosciences、San Diego、CA)によって測定した。
心肺バイパス回路に見られるような、異物への曝露によって誘発される補体活性化をブロックする抗C5アプタマーの能力を試験するため、本発明者らは、Nilssonおよびその共同研究者(Gongら、(1996)Journal of Clinical Immunology 16,222−9;Nilssonら、(1998)Blood 92、1661−7)によって記載されたチュービングループモデルを使用した。Tygon S−50−HL培地/外科用チュービング(1/4インチ内径)(United States Plastic Corp.((Lima、OH)、カタログ番号00542)を、およそ300mm(およそ9mL容)の長さに切断し、0.4単位/mLのヘパリン(Celsus)および種々の濃度のARC658(配列番号62)(0〜5μM)を含有する5mLのヒトドナー血を充填した。各長さのTygonチュービングを、短い切片(約50mm)の非外科用シリコーン製連結チュービング(3/8インチ内径)(United States Plastic Corp.(R−3603型、カタログ番号00271)により、Gongらに記載のようにして1つのループに封止した。チュービングループを1時間、およそ30rpmで37℃の水浴中で回転させた。次いで、ループ内容物を、EDTA(10mM最終濃度)を入れたポリプロピレン円錐チューブ内に注入し、補体活性化をクエンチした。血小板が不充分な血漿を遠心分離によって単離し、C5aおよびC3aについてELISA(C3a ELISA kit、Quidel、San Diego、CA;C5a ELISA kit、BD Biosciences、San Diego、CA)によって解析した。
デノボ選択および配列
dRmYプールによるC5選択
2つの選択法を実施し、dRmYアプタマーがヒト完全長C5タンパク質であると特定した。C5タンパク質(Quidel Corporation、San Diego、CAまたはAdvanced Research Technologies、San Diego、CA)は、完全長(「FL」)で、部分トリプシン処理(「TP」)された形態で使用し、選択法はともに、疎水性プレート上に固定化させたタンパク質標的に対する直接的選択法であった。両選択法により、天然非選択プールと比べて完全長C5結合が有意に富化されたプールを得た。この実施例に示した配列はすべて、5’から3’に示す。
TAATACGACTCACTATAGGGAGAGGAGAGAACGTTCTACN(30)GGTCGATCGATCGATCATCGATG(ARC520;配列番号70)の配列を有するDNA鋳型を、ABI EXPEDITETM DNA合成装置を用いて合成し、標準的な方法によって脱保護した。鋳型を、5’プライマーTAATACGACTCACTATAGGGAGAGGAGAGAACGTTCTAC(配列番号71)および3’プライマーCATCGATGATCGATCGATCGACC(配列番号72)を用いて増幅し、次いで、Y639F単一変異型T7 RNAポリメラーゼを用いる試験管内転写用の鋳型として使用した。転写は、200mM HEPES、40mM DTT、2mMスペルミジン、0.01% TritonX−100、10%PEG−8000、9.5mM MgCl2、2.9mM MnCl2、2mM NTP、2mM GMP、2mMスペルミン、0.01単位/μl 無機ピロホスファターゼ、およびY639F単一変異型T7ポリメラーゼを用いて行なった。
組成および配列最適化
実施例3A:ARC913の最小化:
ARC913(配列番号75)を基礎とする6種類の構築物を転写し、ゲル精製し、C5に対する結合についてドットブロットにて試験した。ARC954は、130nMのKdおよび20%の結合度を有する親クローンと類似していたが、ARC874(配列番号76)は、1uMのKdでC5に結合された唯一の他のクローンであった。
SELEX条件下で選択された選択されたアプタマーから誘導した。この選択によって誘導される(および配列表に反映された)本発明の実施形態では、プリン(AおよびG)はデオキシであり、ピリミジン(UおよびC)は2’−OMeである。表4に示した配列は、キャッピング(例えば、3’逆位dT)を含むもの、または含まないものであり得る。
C5結合親和性に対するクローンARC913(配列番号75)の最適化および重要な結合要素の決定の両方を行なうため、ドープ再選択を行なった。ドープ再選択法は、活性なクローンまたはミニマー内での配列要件を検討するために使用される。選択法は、単一の配列を基に設計された合成の縮重したプールを用いて行なわれる。縮重のレベルは、通常、70%〜85%の野生型ヌクレオチドで異なる。一般に、中立変異が観察されるが、場合によっては、配列変化により、親和性の改善がもたらされ得る。次いで、複合配列情報は、最小結合モチーフを同定するため、および最適化の取り組みを補助するために使用され得る。
オリゴヌクレオチド5’NH2−fCmGfCfCGfCmGmGfCfUfCfCmAmGmGfCGfCfUmGmAmGfUfCfUmGmAmGfUfUfUUAfCfCfUmGfCmG−3T−3’(ARC672、配列番号63)を、Expedite DNA合成装置(ABI、Foster City、CA)において、推奨された製造業者の手順に従い、標準的な市販の2’−OMe RNAおよび2’−F RNAおよびTBDMS保護RNAホスホロアミダイト(Glen Research、Sterling、VA)ならびに逆位デオキシチミジンCPG支持体を用いて合成した。末端アミン機能を、5’−アミノ修飾体である6−(トリフルオロアセチルアミノ)ヘキシル−(2−シアノエチル)−(N,N−ジイソプロピル)−ホスホロアミダイト、C6−TFA(Glen Research、Sterling、VA)により結合した。脱保護後、オリゴヌクレオチドを、Super Q 5PW(30)樹脂(Tosoh Biosciences)上でのイオン交換クロマトグラフィーによって精製し、エタノール沈殿させた。
単離された灌流された心臓モデル
実施例4A:ARC186を用いた原理の証明
ヒト血漿中の補体成分C5の平均濃度は、およそ500nMである。Krebs Heinseleitバッファーで灌流した単離されたマウス心臓を6%ヒト血漿に曝露すると、ヒト補体カスケードが活性化され、C5のC5aおよびC5bへの切断がもたらされる。続いて、C5b成分は補体成分C6、C7、C8およびC9と、「膜攻撃複合体」(「MAC」またはC5b−9)としても知られる複合体を形成し、これは、心臓血管および心筋細胞を損傷し、したがって、心筋機能不全(拡張終期圧の増大、不整脈)および不全収縮に至る(Evansら,Molecular Immunology、32、1183−1195(1995))。以前に、ヒトC5切断をブロックするモノクローナルの単鎖抗体(PexelizumabまたはPexelizumabの単鎖scFv型)がこのモデルにおいて試験され、心筋障害および機能不全を抑制することが示された(Evansら、1995年)。
この研究に用いた材料および方法は、上記の実施例4Aに記載のものと厳密に同じであった。実験計画および結果を図32に示す。実験の前半では、ヒトヘパリン化血漿(Center for Blood Research、Harvard Medical School,Boston、MA)を補体の供給源として使用し、後半では、ヘパリン化マカク属サル血漿(Charles River Laboratories、Wilmington、MA)を補体の供給源として使用した。PEG化されたアプタマー(ARC658;配列番号62)を系に漸増モル比で添加した。関連性のある心室の圧力追跡はすべて収集したが、表には、拡張終期圧(EDP)の増大の存在または非存在、不全収縮が起こったか否か、および心不全(EDPの上昇および不全収縮の存在として規定)までの時間を示す。
動物における抗C5アプタマーの薬物代謝および薬物動態
実施例5A〜5Gにおいて、質量基準の濃度データすべて、PEGコンジュゲーションによってもたらされる質量とは無関係に、アプタマーのオリゴヌクレオチド部分の分子量のみを示す。
アプタマーの非PEG化オリゴヌクレオチド前駆体(すなわち、ARC186;配列番号4)を、その安定性、反応速度論および分解経路を評価するために、ラットおよびマカク属サル血漿(Charles River Labs、Wilmington、MA)中で試験した。試験は、37℃で95%プール血漿(クエン酸塩添加)中50時間にわたてインキュベートした5’末端放射性標識(32P)アプタマーを用いて行なった。選択した時点で、アプタマー含有血漿のアリコートを採取し、直ちに液体窒素中でフラッシュ凍結し、−80℃で保存した。血漿中のアプタマーおよびその代謝産物の検出および解析を、液液(フェノール−クロロホルム)抽出の後、ゲル電気泳動(10%変性ポリアクリルアミド配列決定用ゲル上)および高分解能オートラジオグラフィーを用いて行なった。
ARC657(配列番号61)、ARC658(配列番号62)およびARC187(配列番号5)の薬物動態プロフィールを評価するため、ならびに霊長類およびヒトに必要とされる投与レベルおよび頻度を推定するため、薬物動態試験を、カテーテル挿入Sprague−Dawleyラット(Charles River Labs、Wilmington、MA)において行なった。アプタマーを、10mg/mL(オリゴ重量)での注射用に標準的な生理食塩水にて製剤化し、予め滅菌した投与バイアル内に無菌的条件下で滅菌濾過(0.2μm)した。ラットでの研究に使用した投与経路は、10mg/kgの用量での尾静脈を介する静脈内ボーラスであった。試験集団(arm)は1群あたり3匹の動物で構成し、該動物から、投与前および48時間にわたる経過において指定の時点で逐次的採血を行なった。研究計画の概略を図34に示す。血液試料を、外科的に埋入した頚静脈カテーテルから採取し、EDTAコートチューブに直接移し、反転させることによって混合し、血漿の加工処理まで氷上に置いた。
ARC187(配列番号5)と異なる40kDaの分枝鎖PEGにコンジュゲートさせたARC186(配列番号4)オリゴヌクレオチド主鎖の薬物動態プロフィールを評価するため、薬物動態試験を、雌CD−1マウス(Charles River Labs、Wilmington、MAから入手)において行なった。アプタマーを、2.5mg/mL(オリゴ重量)での注射用に標準的な生理食塩水にて製剤化し、予め滅菌した投与バイアル内に無菌的条件下で滅菌濾過(0.2μm)した。マウスでの研究に使用した投与経路は、10mg/kgの用量での尾静脈を介する静脈内ボーラスであった。試験集団は1群あたり3匹の動物で構成し、該動物から、投与前(すなわち、非投与対照群)および72時間にわたる経過において指定の時点で末端採血を行なった。研究計画の概略を図38Aに示す。
雌CD−1マウスをCharles River Labs(Wilmington、MA)から入手した。注射用のARC657(配列番号61)、ARC658(配列番号62)およびARC187(配列番号5)の製剤は、生理食塩水中5mg/mlとした。投与製剤を、無菌的条件下で予め滅菌した投与バイアル内に滅菌濾過(0.2μm)し、動物に、静脈内ボーラスで尾静脈を介して25mg/kgの用量で与えた。研究は、4つの時間点t=投与前、3、6、12時間の各々について3匹の動物の群で構成した。放血後、各動物の血管系を生理食塩水で充分に(V約30mL)フラッシュ洗浄した。血管系内に残留する血液(あれば)を除去した。組織(心臓、肝臓、腎臓)を回収し、重量測定し、次いで、標準的な生理食塩水中50%w/vでホモジナイズし、解析時点までまで−80℃で保存した。
注射用のARC657(配列番号61)、ARC658(配列番号62)およびARC187(配列番号5)の製剤は、標準的な生理食塩水中10mg/mLとし、投与製剤を、無菌的条件下で予め滅菌した投与バイアル内に滅菌濾過(0.2μm)した。マカク属サルでの研究に使用した投与経路は、30mg/kg(およそ50倍モル過剰)の用量での外科的に埋入した大腿静脈カテーテルを介した静脈内ボーラスであった。研究計画の概略を図40に示す。血液試料は、大腿静脈カテーテルから採取し、クエン酸ナトリウムコートチューブに直接移し、反転させることによって混合し、遠心分離して血漿(3000rpmで5分間)を分離するまで氷上に置いた。次いで、血漿を250μlのアリコートに分け、これを−80℃で保存し、各試料のアリコートの1つをアプタマー濃度について、上記のラットPKのセクションで前述した蛍光系OligreenTMアッセイを用いて評価した。
PEGアプタマーARC187(配列番号5)は血漿中に最長時間、残存したが、20kDa PEGアプタマーARC657(配列番号61)は、最短量の時間、残存した。図41に示すデータの詳しい検討により、このデータは、2コンパートメントモデルによって最良にフィットされたものであり得ることが示された。したがって、図42に報告した薬物動態パラメータ推定値は、該2コンパートメントモデルから業界標準の薬物動態モデル設計ソフトウエアWinNonLinTMバージョン4.0(Pharsight Corp.、Mountain View、CA)を用いて誘導した。
研究2は、以下のことを除き、上記の研究1と同様の計画であった。a)2種類の化合物だけ評価した(ARC658(配列番号62)およびARC187(配列番号5);b)動物の数を1群あたり4匹に増やした;およびc)1分間血漿試料を除き、代わりに144時間試料を用いて最終半減期計算がより多くのデータ点に基づくことを確実にした。製剤およびこれらの2種類のアプタマーの投与、血液試料採取ならびに血漿単離手法は、研究1で上記の方法と同一とした。研究2の計画を図44にまとめる。
C5インヒビターARC1905の薬力学を、静脈内投与後のマカク属サルにおいて評価した。注射用のARC1905の製剤は、標準的な生理食塩水中7.5mg/mLとし、投与製剤を、無菌的条件下で予め滅菌した投与バイアル内に滅菌濾過(0.2μm)した。カニクイザル(n=4)に、0(生理食塩水対照)または30mg/kgを静脈内ボーラス投与によって投薬した。血液試料は、末梢静脈または動脈アクセスポートから採取し、血液試料(0.5mL)を2カリウム(K2)EDTAチューブ内に移し、溶氷(wet ice)上に置き、およそ4℃で収集してから30分以内に遠心分離した。
また、ARC187(配列番号5)の薬物動態(PK)および薬力学的(PD)プロフィールを、静脈内負荷ボーラスの直後に静脈内注入を開始した後のマカク属サルにおいて評価した。この研究計画を図49に示す。
CABG術と関連する合併症の予防、改善または治療のためのヒト投与での必要量は、以下の仮定に基づく。第1に、CABG患者には、手術開始前に抗C5アプタマーの単回静脈内ボーラス投与で投与した後に連続注入を行ない、1.5μMの定常状態血漿濃度を、CABG術後24〜48時間確立および維持する。ボーラス投与および注入速度の推定値は、マカク属サルにおける前述のIVボーラス単独およびボーラス+注入研究から誘導された薬物動態パラメータを用いた計算値に基づく。ボーラス投与ARC187の推定値は1mg/kgであり、随伴する注入速度は0.0013mg/kg/分である。このボーラス+48時間注入レジメンでは、予測される薬物の全必要量は、ARC187で0.4gであり、ここで質量は、オリゴヌクレオチドの重量のみを示す(図53の表の第7欄を参照)。図53に示す表の第2欄は、ARC187のオリゴヌクレオチド部分にコンジュゲートされたPEG基の重量を示し、第3欄は、ARC187のオリゴヌクレオチド部分の分子量を示し(ARC186(配列番号4)をそのオリゴヌクレオチド配列として含む本明細書のすべてのアプタマーについても同じである)、第4欄は、上記の実施例3Cに記載のようなアミン反応性化学基を介してARC186(配列番号4)にコンジュゲートされた40kDA PEGの分子量を示し、第5欄は、2コンパートメントモデルにおけるARC187のα段階の半減期を示し、第6欄は、2コンパートメントモデルにおけるARC187のβ段階の半減期を示す。
抗C5アプタマーおよびヘパリン/プロタミン相互作用
抗C5アプタマーの予測される適用の一例は、冠動脈バイパス移植(CABG)術に伴う炎症性副作用予防または軽減のための予防薬である。CABG中、血栓症を予防するため、およびバイパスポンプの構成要素内の開通性を維持するために、典型的には、高濃度の抗凝固剤ヘパリン(3〜5単位/mLまたは1〜2μM)が投与される。治療後、ヘパリンの効果の逆転および通常の止血の回復が、同様に高濃度のプロタミン(約5μM)の投与によって達成される。患者に対し、これらの薬物のいずれかの有効性が妨害される潜在的な危険性を考慮すると、抗C5アプタマーは、(1)いずれかの薬物の活性を改変しない、または(2)患者の抗凝固治療を複雑にし得る止血に対する内因性効果を示さないことが必要であった。
血漿凝固能力に対するARC187(配列番号5)の内因性効果を、それぞれ、凝固カスケードの外因性および内因性集団、プロトロンビン時間(PT)ならびに活性化部分トロンボプラスチン時間(aPTT)の標準的な臨床試験を用いて検討した。図54に示されるように、充分過剰の推定用量(20μMまで)の濃度でのクエン酸塩添加ヒト血漿の滴定により、PTにおいて変化はもたらされず、aPTTが若干上昇したにすぎなかった。
抗C5アプタマーARC187(配列番号5)の同時投与におけるヘパリンをプロタミン間の機能の相互作用を、臨床用量のヘパリンおよび臨床用量/臨床用量以下/臨床用量超過のプロタミンにおいて検討し、臨床血漿濃度以下/臨床血漿濃度超過のARC187の存在が、プロタミンによるヘパリン抗凝固の逆転を妨害するか否かを調べた。この研究の結果を図56にまとめる。簡単には、試験したすべてのヘパリン用量において、ベースラインACT値は、10uM(すなわち、臨床用量の10倍モル過剰)のARC187によって影響されなかった。同様に、ヘパリンによる抗凝固の程度は、10uMのARC187によって影響されなかった。ARC187の非存在下では、プロタミンの最小有効用量は約30%(臨床用量=100%)であった。さらにまた、30%プロタミンによるヘパリン抗凝固の逆転は、臨床用量の10倍モル過剰(すなわち、10uM)のARC187によって影響されなかった。したがって、臨床状況(例えば、CABG)における補体阻害のためのARC187の使用は、典型的な用量でのヘパリンおよびプロタミンとの同時使用によって影響されないはずである。
ARC187(配列番号5)の抗補体活性に対するヘパリンおよびプロタミンの効果を、実施例1に記載のようにしてザイモサンにより活性化したクエン酸塩添加全血試料において検討した。ザイモサン活性化の直前、ARC187を4つ:1)処理なり(ヘパリンまたはプロタミンなし);2)4U/mLヘパリン;3)6μMプロタミン;4)4U/mLヘパリン+6μMプロタミンの条件下で処理したクエン酸塩添加血液試料中に滴定した。ザイモサンでの活性化後、C5活性化を血漿中のsC5b−9のELISA測定によって定量した(C5b−9 ELISA kit、Quidel、San Diego、CA)。各条件について、C5活性化の阻害割合対ARC187濃度として示した結果は、誤差内で識別不可能であった(図57参照)。すべての場合で、完全な阻害が1〜2μMのARC187により達成された。したがって、ヘパリンおよびプロタミンは別々または組合せで、そのCABG術における使用に関連性のある濃度において、ARC187の抗補体活性に影響を及ぼさないようである。
脈絡膜新生血管形成
レーザー誘導型脈絡膜新生血管形成は、多くの場合、加齢性黄斑変性のモデルとして使用される。また、これは、糖尿病性網膜症のモデルとしても使用され得る。脈絡膜新生血管形成の予防ならびに安定化および/または退行に対する抗C5剤(これは、本発明の好ましい実施形態では、本明細書に記載の抗補体アプタマーである)の投与の効果を、このモデルにおいて、後述およびKrzystolik,M.G.ら、Arch Opthalmol.、第120巻、pp338−346(2002)に記載のようにして評価する。
非滲出性AMDの網膜変性マウスモデル
単球化学誘引タンパク質1(MCP−1もしくはCcl−2)またはそのコグネイトC−Cケモカインレセプター2(Ccr−2)のいずれかに変異を有するマウスモデルは、ヒト加齢性黄斑変性の症状、例えば、ドルーゼンの発生、光レセプター萎縮および脈絡膜新生血管形成を模倣する。Ambatiら、Nature Medicine.2003年1月;9(11):1390−7を参照のこと。このマウスモデルは、網膜の色素上皮および脈絡膜内にC5の有意な蓄積を示し、これは、補体が疾患に随伴して発現されていることを示す。さらに、CD46(補体の膜結合型調節因子)、ビトロネクチン(MACの調節因子)およびC3c(C3bの分解生成物)が、網膜の色素上皮および/または脈絡膜に存在し、これは、補体活性化が起こっていることを示す。
