Claims (14)
- 心臓再同期(resynchronization)療法を患者の心臓の少なくとも一方の心室に送達するための手段と、
前記心臓再同期療法の送達(delivery)中に前記心臓の心房頻脈性不整脈(tachyarrhythmia)を検知するための手段と、
前記検知された心房頻脈性不整脈への心室レート応答の範囲(extent)を検知するための手段と、
前記心臓再同期療法の送達中で前記検知された心房頻脈性不整脈中に前記心室レート応答に基づいて迷走神経刺激(vagal stimulation)を送達するための手段と、を備えているシステム。 - 前記心臓再同期療法を送達するための前記手段と、前記迷走神経刺激を送達するための前記手段は、前記心臓再同期療法及び前記迷走神経刺激を送達するように構成されている刺激生成器を備えており、
前記心臓再同期療法の送達中に前記心房頻脈性不整脈を検知するための前記手段と、前記検知された心房頻脈性不整脈への前記心室レート応答の範囲を検知するための前記手段は、
前記心臓再同期療法の送達中に、前記心臓の前記心房頻脈性不整脈を検知し、
前記検知された心房頻脈性不整脈への前記心室レート応答の範囲を検知し、
前記刺激生成器を制御して、前記心臓再同期療法の送達中で前記検知された心房頻脈性不整脈中に前記心室レート応答の範囲に基づいて前記迷走神経刺激を送達させる、ように構成されているプロセッサを備えている、請求項1に記載のシステム。 - 前記プロセッサは、少なくとも、前記心房頻脈性不整脈中に心室ペーシング(ventricular pacing)の範囲を検知することによって、前記心室レート応答の範囲を検知するように構成されている、請求項2に記載のシステム。
- 前記プロセッサは、ペーシングされた心室収縮の割合(percentage)又は固有の(intrinsic)心室収縮の割合のうちの少なくとも一方を閾値と比較し、前記刺激生成器を制御して、前記比較に基づいて前記迷走刺激を送達させる、請求項3に記載のシステム。
- 前記プロセッサは、少なくとも、前記心房頻脈性不整脈中に固有の心室レートを検知することによって、前記心室レート応答の範囲を検知するように構成されている、請求項2から4の何れかに記載のシステム。
- 前記迷走神経刺激は、複数のパルスの1つ又はそれ以上のパルス列を備えている、請求項2から5の何れかに記載のシステム。
- 前記迷走神経刺激は、房室結節迷走神経(atrioventricular nodal vagal)刺激を備えている、請求項2から6の何れかに記載のシステム。
- 前記システムは感知モジュールを更に備えており、前記刺激生成器と前記感知モジュールは、少なくとも1つの共通の電極を介して、前記刺激生成器は前記房室結節迷走神経刺激及び右心房ペーシングを送達するように、前記感知モジュールは心房脱分極を検知するように、構成されている、請求項9に記載のシステム。
- 前記プロセッサは、前記迷走神経刺激に続く心室レートを検知し、前記心室レートが閾値を上回っている場合は、前記迷走神経刺激の刺激パラメータを修正する、請求項2から8の何れかに記載のシステム。
- 前記閾値は、心室ペーシングレートに対応している、請求項9に記載のシステム。
- 前記プロセッサは、前記迷走神経刺激の送達に続く前記心室ペーシングの範囲を検知し、前記心室ペーシングの範囲に基づいて前記迷走神経刺激の刺激パラメータを修正する、請求項2から10の何れかに記載のシステム。
- 前記プロセッサは、前記心房不整脈が終結したかどうかを判定し、前記心房不整脈が終結していた場合は前記療法(therapy)モジュールを制御して前記刺激信号の送達を止めさせる、請求項2から11の何れかに記載のシステム。
- 前記プロセッサは、前記刺激生成器を制御して前記迷走神経刺激信号の送達を止めさせ、続けて前記迷走神経刺激のない状態での前記心室レート応答の範囲を検知し、前記刺激生成器を制御して、前記続けて検知された前記迷走神経刺激のない状態での前記心室レート応答の範囲に基づいて前記迷走神経刺激の送達を再開させる、請求項2から12の何れかに記載のシステム。
- 前記システムは植込型医療装置を更に備えており、前記植込型医療装置は前記刺激生成器と前記プロセッサを備えている、請求項2から12の何れかに記載のシステム。
